ABSTRACT

OBJECTIVE@#To explore the high risk factors of adult complex appendicitis, and to provide a reference for the development of a reasonable treatment strategy for acute appendicitis.@*METHODS@#A retrospective case-control study was conducted to collect clinical data of 312 adult patients with acute appendicitis confirmed by pathology undergoing appendectomy, including open and laparoscopic surgery, from May 2011 to August 2016 at Affiliated Hospital of Qingdao University. Age <14 years old, pregnant women, complicating abscess around the appendix, AIDS, blood system diseases, autoimmune diseases, inflammatory bowel disease or progressive cancer patients were excluded. According to the intra-operative findings and pathological types, patients were divided into complex appendicitis(112 cases, including gangrene and perforation) and non-complex appendicitis (200 cases, including simple and non-perforated appendicitis, ie suppurative appendicitis). After comparing the clinical data of these two groups, statistically significant variables were induded for multivariate logistic regression analysis to identify risk factors of complex appendicitis, and to establish a regression model. Enter method was applied to establish the regression equation: P=ExpiΣBiXi/1+ExpΣBiXi, and to calculate the relative risk of each variable. Meanwhile, retrospective and prospective verification was performed on this predictive model (cases of acute appendicitis from September 2016 to December 2017 were further collected). The sensitivity, specificity, accuracy, positive predictive value, and negative predictive value of complex appendicitis were calculated with the regression model.@*RESULTS@#Comparison of the clinical data between the complex appendicitis group and the non-complex appendicitis group showed that differences of 10 preoperative indexes were statistically significant, including period from abdominal pain to admission [(59.1±42.6) hours vs. (47.5±34.4) hours, t=3.051, P=0.002], white blood cell count [(12.9±3.7)×10/L vs. (9.2±4.0)×10/L, t=9.755, P<0.001], neutrophil count [(9.8±4.0)× 10/L vs.(7.1±3.9)×10/L, t=6.020, P<0.001], neutrophil percentage[(84.5±8.7)% vs.(68.2±16.0)%, t=12.754, P<0.001], C-reactive protein levels [(86.0±45.4) μg/L vs. (55.9±35.8) μg/L, t=7.614, P<0.001], serum albumin levels [(334.0±4.8) g/L vs. ( 41.0±4.3) g/L, t=16.055, P<0.001], vomiting ratio [44.6%(50/112) vs. 23.5%(47/200), χ²=14.980, P<0.001], high fever(≥39°C) ratio [16.1%(18/112) vs. 7.5%(15/200), χ²=5.577, P=0.022], the proportion of patients ≥60 years old [22.3%(25/112) vs. 13.0%(26/200), χ²=4.562, P=0.038] and previous history of appendicitis [16.1%(18/112) vs. 7.5%(15/200), χ²=5.577, P=0.022]. The above 10 variables were included in the logistic regression model for multivariate analysis. The results showed that six variables were associated with complex appendicitis. According to their strength, they were old age (≥60 years old) X1(OR=5.094), high fever (≥39°C) X2(OR=4.464), neutrophil count X6 (OR=1.269), neutrophil percentage X4 (OR=1.077), C-reactive protein level X5 (OR=1.027), and serum albumin level X3 (OR=0.763). A predictive regression model was established: P=1/[1+e], whose sensitivity and specificity of judging complex appendicitis were 76.8%(86/112) and 90.0%(180/200),respectively. Sensitivity and specificity for predictive value of complex appendicitis in further prospective validation of the model were 76.2%(48/63) and 81.1% (30/37), respectively.@*CONCLUSIONS@#Age ≥ 60 years old, body temperature ≥39°C, increased neutrophil count, neutrophil percentage and C-reactive protein levels, and hypoalbuminemia are risk factors for complex appendicitis. The establishment of predictive model may help determine complex appendicitis.

ABSTRACT

Objective To investigate the correlation and significance between miRNA-802 and inflammato-ry factors in the patients with inflammatory bowel disease (IBD) ,and to understand pathologic mechanisms of IBD.Methods 80 patients with active IBD ,who were hospitalized in Department of Gastroenterology ,Taizhou People's Hospital ,from August 2015 to February 2017 ,were selected as active IBD group.Another 40 inactive IBD patients and 40 healthy persons who underwent the healthy assessment during the same period were se-lected as inactive IBD group and healthy control group.The expression level of miRNA-802 in intestinal muco-sa and peripheral blood mononuclear cells of each group was detected by fluorescence quantitative PCR (RT-qPCR).In addition ,the relative expression of miRNA-802 in the intestinal mucosa and peripheral blood mono-nuclear cells of 43 patients with Crohn's disease (CD) treated by IFX (IFX) was detected and the correlation with the serum TNF-α was analyzed.Results The expression level of miRNA-802 in the intestinal mucosa and peripheral blood mononuclear cells of active IBD patients was significantly higher than that of inactive IBD pa-tients and healthy control group ,and the difference was statistically significant (P<0.01).After 12 weeks of IFX treatment ,the expression level of miRNA-802 in the intestinal mucosa and peripheral blood mononuclear cells of CD patients was significantly lower than that before treatment ,and the difference was statistically sig-nificant (P<0.01).The results of correlation analysis have shown that the expression level of serum TNF-α was positively correlated with miRNA-802 expression in peripheral blood monocytes ,and the expression of miRNA-802 mRNA was significantly increased after the stimulation of TNF-α on peripheral blood monocytes cultured in vitro.Conclusion The increased expression level of miRNA-802 in IBD patients is closely correla- ted with the expression level of TNF-α ,and miRNA-802 could be a new treatment target for IBD.

ABSTRACT

OBJECTIVE:To explore the effects of olmesartan medoxomil on blood pressure variability(BPV)and vascular en-dothelial function of patients with mild and moderate essential hypertension. METHODS:Sixty patients with mild and moderate es-sential hypertension were included in study group,and other 60 healthy volunteers were included in control group. Study group was given olmesartan medoxomil 20 mg orally,qd,for 4 weeks,followed by same dose if their SBP≤140 mmHg and DBP140 mmHg or DBP≥90 mmHg received double dose till 12th weekend. Aver-age blood pressure,BPV,FMD and NMD of study group were measured before and after treatment as well as the changes in se-rum concentrations of NO and ET,and then compared with control group. RESULTS:Compared with before treatment,average blood pressure,BPV and ET level of study groups were decreased significantly after treatment,serum level of NO was increased significantly,while FMD and NMD were improved significantly,with statistical significance (P0.05). There were 3 cases of mild diarrhea and 2 cases of mild dry cough,and they relieved after medication withont speical treatment. CONCLUSIONS:Olmesartan medoxomil can play a good effect on de-creasing blood pressure and is tolerable. It can also improve the vascular endothelial function.

ABSTRACT

Aim To observe the protective effects of probiotics on alcoholic liver injury in rats .Methods Male Wistar rats were randomly divided into the follow-ing three groups: control group , normal diet with nor-mal (5 ×108 CFU· kg -1· d -1) treatment group.Ex-cluding the rats in the normal control group , the other animals were initially received intragastric administra-tion with 56%( V/V) ethanol 5.5~11.0 mL· kg -1 · day -1 for 8 weeks.Then the rats’ faeces were collect-ed, and the liver and the small intestine were obtained for pathologic and ultrastructural observation .Serum ALT, AST and ALP was measured by method of bio-chemistry .Serum DAO and D-LA was measured by en-zyme linked immunosorbent assay .The expression of FOXO4 in small intestine was detected by immunohis-tochemistry .The intestinal flora genome DNA was ex-tracted from faeces and the sequence of 16 S rDNA was analyzed by high-throughput sequencing technologies . Results Hepatic steatosis was obviously improved in probiotics treatment groups compared with ethanol-trea-ted group , and the ultrastructural such as mitochondri-al and rough endoplasmic reticulum pathological chan-ges was significantly alleviated . The ultrastructural changes in intestinal were better in probiotics treatment group than in the ethanol-treated group .And ethanol-induced rats ’ serum ALT, AST, ALP, D-LA and DAO levels showed a significant reduction in the probi-otics treatment groups compared with the ethanol-trea-ted group ( P<0.05 ) .The FOXO4 expression was in-creased obviously in the probiotics treatment groups compared with the ethanol-treated group ( P <0.05 ) . And the intestinal flora diversity was impacted after feeding alcohol , and probiotics had a certain regulative action in helping the intestinal flora back to normal state; At phylum level , the Firmicutes quantity was lower and the Bacteroidetes quantity was higher in eth-anol-treated group than those in the control group ( P<0.05 ) , and the conditions were improved after supple-menting probiotics .At genus level , the percent of ge-nus abundance was similar to normal control group in the probiotics treatment groups compared with the etha-nol-treated group .Conclusion Probiotics can relieve liver injury induced by alcohol in rats , and the mecha-nism may be related to the modulation of probiotics on the intestinal flora distribution and intestinal barrier .

ABSTRACT

AIM:To explore an ideal method to induce the differen-tiation of human umbilical cord mesenchy-mal stem cells (hUCMSCs) into neuron-like cells and to provide some evidence for the transplantation of hUCMSCs for spi-nal cord injury .METHODS:The hUCMSCs were isolated from human umbilical cord digested with collagenase Ⅱ.The hUCMSCs was verified by flow cytometry analysis .The passage 5 cells were randomly divided into 4 groups.The differentiation of hUCMSCs was induced by bFGF in group A , bFGF and BDNF in group B, or BHA, bFGF and BDNF in group C, while the cells in group D served as a control group cultured with DMEM-F12 and 10%FBS.Two weeks later , the expression of nestin , neurofilament protein H ( NEFH) and glial fibrillary acidic protein ( GFAP) was detected by real-time PCR and immunocytochemistry .The morphological changes of cells were observed under an atomic force microscope . RESULTS:Mesenchymal stem cells were isolated and cultured from human umbilical cord by enzyme digestion .hUCMSCs expressed CD29, CD44 and CD105, but no CD34, CD45 or HLA-DR.After cultured with inducing medium for 2 weeks, the cells were successfully induced into neuron-like cells.The appearance of the cells had great change .The induced hUC-MSCs developed round cell bodies with multiple neurite-like extensions observed under an atomic force microscope .The re-sult of real-time PCR showed that nestin was positive in A , B and C groups , and NEFH was positive in A and B groups , but GFAP was negative in 4 groups.The difference of nestin and NEFH expression among the induced groups was signifi -cant (P<0.05).CONCLUSION:Mesenchymal stem cells were isolated and cultured from human umbilical cord by en-zyme digestion in vitro, and all the hUCMACs presented stable biological properties .Moreover, hUCMSCs were induced to differentiate into neuron-like cells in vitro via bFGF combined with BDNF .

ABSTRACT

<p><b>OBJECTIVE</b>To explore the influence of visceral fat area(VFA) on laparoscopic radical gastrectomy and short-term outcome in patients with gastric carcinoma.</p><p><b>METHODS</b>Clinicopathological and imaging data of 202 patients with gastric cancer who were treated by laparoscopic radical gastrectomy in the Affiliated Hospital of Qingdao University from May 2011 to July 2014 were retrospectively analyzed. VFA of all the patients were measured by CT, of which more than 10 000 m(2) were found in 104 patients(VFA-L group), less than 10 000 m2 in other 98 patients (VFA-S group). Intraoperative and postoperative outcomes were compared between the two groups.</p><p><b>RESULTS</b>There were no differences in operation time, intraoperative blood loss and number of harvested lymph node between the two groups(all P>0.05). As compared to VFA-S group, VFA-L group had a higher incidence of conversion to open surgery[9.6%(10/104) vs. 2.0%(2/98), P=0.023], higher complication rate [22.1%(23/104) vs. 10.2%(10/98), P=0.003], longer postoperative fever time [(3.0±1.3) d vs. (2.4±1.1) d, P=0.000], total length of hospital stay [(18.6±11.8) d vs. (15.8±6.0) d, P=0.039]. There were no significant differences in first flatus passage time and total hospitalization costs between the two groups.</p><p><b>CONCLUSION</b>Elevated VFA can raise the difficulty of laparoscopic radical gastrectomy, increase the risk of postoperative complication and prolong the postoperative recovery.</p>

ABSTRACT

Objective To investigate the effect of health literacy on stroke education in patients with ischemic cerebrovascular disease and their family members.Methods Health literacy questionnaire was used to conduct the health literacy questionnaire survey for patients with ischemic cerebrovascular disease and their family members.The respondents received stroke education randomly through neurologists' explanation or self-reading.The corresponding questionnaire was used to evaluate the mastering of the content of stroke education for the respondents.Results The complete data of 42 patients with ischemic cerebrovascular disease (34 ischemic stroke and 8 transient ischemic attack) and 57 family members of the patients with ischemic cerebrovascular disease were collected and enrolled in the analysis.The good health literacy score group (< 10) and the poor health literacy score group (≥ 10) were 61 and 38 subjects respectively.The proportions of younger age (x2 =16.410,P <0.001) and high school or above (68.85% vs.23.68% ;x2 =19.25,P < 0.001) of the former were higher than those of the latter,and the proportion of patients (32.79%vs.57.89%;x2 =6.043,P=0.014) was significantly lower than that of the latter,but there was no significant difference in the proportion of the males between the two groups (55.74% vs.44.74% ;x2 =1.134,P =0.287).Multivariate logistic regression analysis showed that health literacy was independently associated with the age of respondents (≤40 years:odds ratio [OR] 7.753,95% confidence interval [CI]1.208-49.740; P=0.031) and the level of education (high school and above:OR,3.993,95% CI 1.305-12.218; P=0.015),and it was not correlated with gender and role.Those with good stroke education score (≥7)and poor stroke education score (<7) were 57 and 42,respectively.The proportions of younger age (x2 =17.095,P <0.001),high school and above (70.00% vs.23.08% ;x2 =20.836,P < 0.001),and good health literacy score (88.33% vs.20.51%;x2 =45.968,P<0.001) were significantly higher than those of the latter,while there were no significant differences in the proportions of gender,role,and stroke education.Multivariatelogistic regression analysis showed that only the good health literacy was independently associated with the stroke education score (OR 20.147,95% CI6.089-66.655; P<0.001).Conclusions Health literacy can significantly affected the effect of the stroke education in patients with ischemic cerebrovascular disease and their family members.In the development of individualized stroke education program,it should be considered.

ABSTRACT

ObjectiveTo discuss the clinical characteristics and therapeutic strategy of acute epidural hematoma(EDH) with mixed density on CT.MethodsThe clinical data of 45 patients with acute EDH with mixed density on the first CT after trauma were analyzed retrospectively.Acute EDH form trauma with mixed density on CT images were compared with those which had homogenesis density on CT images at the same time.ResultsThe opportunity of increasing size of hematoma and the mortality was significantly higher in mixed density hematoma( 82.1%,16.2% )than that of homogenesis density( 17.7%,2.3% ) ( all P < 0.01 ).ConclusionEDH with mixed density was a hyperacute EDH.The operation for acute EDH from trauma with mixed density on CT image should be prompt.

ABSTRACT

Objective To investigate the implementation of the surgical safety checklist in the hospital.Methods The investigation covered the participants of 560 surgical operations of a tertiary hospital,including the surgeons,surgical assistants,scrub nurses and anesthetists,to learn their compliance and awareness of the content of the surgical safety checklist.Results Poor compliance and unawareness of some items of surgical safety checklist in surgical team members were found,plus insufficient understanding of some the items on the checklist.This checklist can improve the quality and safety awareness of the team.Conclusion The implementation of the surgical safety checklist is feasible and effective for avoidance of risks in selective operations,and conducive to promoting communication among the surgical team and preventing surgical errors.

ABSTRACT

Objective To explore the associations between vitamin D receptor ( VDR) Fok- Ⅰ and Apa- Ⅰ polymorphisms and myasthenia gravis (MG) in Chinese Han population.Methods Polymorphisms of VDR Fok- Ⅰ and Apa-Ⅰ were determined by polymerase chain reaction-restriction fragment length polymorphism (PCR-RFLP) method.The frequencies of genotypes and hyplotypes were compared among 286 normal controls and 267 MG patients in different subgroups classified by gender,age of onset,presence of thymoma,and Osserman classification sat maximal severity in the follow-up.The association between the genotypes and maximal severity of MG and short-term glucocorticoid treatment were also investigated.Results There were no significant differences in frequencies of genotypes and hyplotypes of both Fok-Ⅰ and Apa-Ⅰ between MG group and control group,and among subgroups of MG.The Fok- Ⅰ showed no statistical difference between the patients with better and less improvement after short-term treatment of glucocorticoid.The frequency of Apa-Ⅰ alleles in the patients with better improvement (55/186,29.6% ) significantly differed from the less improved group ( 7/48,14.6%,OR = 2.46,95% CI 1.04-10.43,x2 = 4.400,P = 0.036).The patients with the genotype A A/Aa were more likely to improve better after the treatment(48/93,51.6%) than in the worse group(7/24,29.2%,OR =2.59,95% CI 0.98-14.60,x2 =3.858,P= 0.049).Conclusions Alleles and genotypes of VDR-Fok- Ⅰ and Apa-Ⅰ were not found to be related with MG onset and severity.MG patients with VDR-Apa-A allele may have better improvement short-term treatment of glucocorticoid.

BACKGROUND: Bronchial asthma is one of those diseases that are significantly affected by meteorological factors. There is close relationship between asthma attack and meteorological factor. There have been some studies on the relationship between asthma and meteorological factors at home and abroad, but the results are various due to different area conditions.OBJECTIVE: To analyze the relationship between asthmatic attack and meteorological factors in children in Qingdao city.DESIGN: Observational experiment.SETTING: The Affiliated Hospital of Qingdao University Medical College.PARTICIPANTS: Altogether 445 inpatient children, 307 males, 138 females, aged < 14 years, who were diagnosed of asthma attack definitely, of the Affiliated Hospital of Qingdao University Medical University, Qingdao Haici Hospital,Qingdao Children Hospital between January 1st 2000 and December 31st 2004 were retrieved.METHODS: The asthmatic attack time of 445 children patients with asthmatic attack of Qingdao city and concurrent month mean temperature, air pressure, relative humidity, wind speed and other meteorologic al data were collected.Seasonal rhythm of asthmatic attack in children and the relationship between asthmatic attack and meteorological factors were analyzed with circular distribution analysis and linear regression analysis, respectively.MAIN OUTCOME MEASURES: ①Month attack information of 445 asthma patients. ② Circular distribution analysis results of asthmatic attack time. ③ Linear regression analysis results about the relationship between asthma attack and meteorological factors.RESULTS: ① Data revealed that asthmatic attack in Qingdao city focused on November, December, January and February, which took up 47.9% of total amount in that year. There were 307 male patients (incidence 3.12%)in a year,female patients only 138 cases (incidence 1.51%), which revealed that boys have a higher incidence of asthma attack than girls. ②The peak season of incidence of asthmatic attack was found in winter and peak season for male and female patients was in January and December, respectively (P < 0.05). ③ Higher incidence of asthmatic attack of children was related with higher air pressure, lower temperature, lower relative humidity(r =0.435, -0.320, -0.296, all P < 0.05).CONCLUSION: The incidence of asthma attack presents seasonal rhythm in Qingdao city and it is influenced by climate conditions. Therefore, enough consideration of meteorological factors should be paid on preventive works for asthma.

OBJECTIVE To study the characteristics of articles,citations and authors in Chinese Journal of Nosocomiology(Chin J Nosocomiol).METHODS The data about articles,citations and authors of Chin J Nosocomiol from 2005 to 2006 were collected and analyzed in terms of bibliometric parameters.RESULTS There were 1365 papers published in Chin J Nosocomiol during 2 years and papers with funded projects accounted for 12.3%.The average number of articles per issue was 57,the average publishing cycle of the articles was 8.4 months;87.1% of which shared with co-authors;the cooperation ratio was 3.57.The authors comed from 31 provinces.The papers with citation accounted for 68.9%and 92.3% of all the citations came from journals.The Price's indicator was 82.0%.CONCLUSIONS Chinese Journal of Nosocomiology,an academic journal of high quality,with strong influence,pushes the subject of Nosocomiology in China forward.

BACKGROUND:Behavioral problems among preschool children affect the quality of life in childhood, and it may predict psychiatric disorders in adulthood.OBJECTIVE:To investigate the correlation of behavioral problem occurrence with gender and breastfeeding in children aged 4-5 years old.DESIGN: A stratified random cluster sampling investigation.SETTING:Department of Child Health Care,Affiliated Hospital of Medical College of Qingdao University,and Department of Preventive Medicine, Medical College of Qingdao University.PARTICIPANTS:Totally 600 children (301 boys and 299 girls) aged4-5 years old from 10 kindergartens in 4 districts in Qingdao city were chosen by stratified random cluster sampling during May in 2004.They. all used to be full-term infants of natural labor.METHODS:Achenbach Child Behavior Checklist (CBCL) for parents and self-made inventory scale influencing children's behavior were adopted. Behavioral problems were dependent variables whereas feeding category and breastfeeding duration were independent variables. The effects of feeding category in the first 4 months of life and breastfeeding duration on children's behavior were examined by single factor and multiple factors logistic regression (backward selection).MAIN OUTCOME MEASURES:Breastfeeding condition, breastfeeding duration and behavioral development.RESULTS:According to intention-processing analysis, 600 children 14.2% (85/600) among the 600 children investigated. It was 17.94%(54/301) in males and 10.4% (31/299) in females,with a significant indicated that boys who had been fed on "more infant formula than problem occurrence (OR=2.72,4.29). Breastfeeding duration ≥9months was a protective factor for males (OR=0.43) Girls who feeding duration < 1 month had a risk of behavioral problem ocfeeding duration were set as independent variables and with or without behavioral problems was set as dependent variable to perform multiand breastfeeding duration ≥ 9 months were the risk factors and protective factors of behavioral problem occurrence for males (OR=3.76,ioral problem occurrence for females (OR=2.55,7.79,7.79), which were tective factors of behavioral problem occurrence for males (OR=3.48,behavioral problem occurrence for females ( OR=2.49,9.81,12.89).CONCLUSION:Male is a risk factor of behavioral problems;less breastfeeding and shorter breastfeeding duration are the risk factors of behavioral problem occurrence in children aged 4 to 5 years.

Results There were 648 papers published in Chin J Neurol during 3 years and papers with funded projects accounted for 25.0%. The average number of articles per isseu was 36; the average number of article per page was 0.43; the average publishing cycle of the articles was 10 months. The authors distributed in 27 mainland provinces and Hongkong, 91.5% of whom shared with co-author; the coporation rate was 4.19. The papers with citation accounted for 65.3% and 95.0% of all the citations came from journals. The Price’s indicator was 56.6%. Conclusions Chinese Journal of Neurology, an academic journal of high quality, with strong influence, pushes the subject of neurology in China forward.

ABSTRACT

Objective To study the relationship between cerebral vascular disease (CVD) and meteorological factors. Methods From 1998 to 2002, 6186 patients with CVD in Qingdao urban were studied. There were 1179 cases of intracerebral hemorrhage (CH), 4821cases of cerebral infarction (CI) and 186 cases of subarachnoid hemorrage (SAH).
