ABSTRACT
@#Objective To retrospectively analyze the pregnancy outcome of pregnant women exposed to botulinum toxin A(BTX-A)during pregnancy,so as to provide experience for clinicians to guide safe drug use in pregnant women.Methods To investigate the pregnancy outcome of pregnant women exposed to BTX-A during pregnancy or the last three months before pregnancy who visited the medication consultation clinic of Hangzhou Women's Hospital from January 2017 to March 2023.Results A total of 17 pregnant women exposed to BTX-A during pregnancy or the last three months before pregnancy were included in this study.Specifically,8 cases had known pregnancy outcomes,among which 1 case had birth defect with the ratio accounting for 12.5%,while the remaining 9 cases required early induced abortion.Conclusion Exposure to BTX-A during pregnancy is probably safe,but further studies are needed.

Triple-negative breast cancer(TNBC)is characterized by strong aggressiveness,high metastatic potential,easy recurrence and poor prognosis.Due to lack of expression for estrogen receptor(ER),progesterone receptor(PR)and human epidermal growth factor(HER2)in TNBC patients,there is a lack of specific therapeutic targets and effective treatment plans for TNBC.At present,immunotherapy has been widely used in solid tumors.Immune checkpoint inhibitors(ICIs)have shown some preliminary clinical effects in the treatment of TNBC,but there were still some patients who do not respond to ICIs therapy or develop resistance,and multiple clinical studies cannot obtain consistent results on the efficacy of ICIs.Therefore,,we reviewed the potential biological factors that influence the efficacy of ICIs in TNBC,aiming to explore the mechanisms affecting the efficacy of ICIs and analyze its potential clinical value.
ABSTRACT
Objective:To apply the best evidence for the prevention of radial artery occlusion after transradial coronary angiography or intervention to clinical practice and evaluate its effect.Methods:This was a quasi-experimental study. Based on the evidence continuous quality improvement model, evidence-based practice method was used to obtain the best evidence, formulated review indicators, analyzed the obstacles in the practice process and took action strategies. The 88 patients who underwent transradial coronary angiography or intervention in the Cardiology Department of Qilu Hospital of Shandong University (Qingdao) from June 1 to 30, 2020 were selected as the baseline review group by convenience sampling. The 94 patients who underwent this treatment from September 1 to 30, 2020 were selected as the evidence application group. The baseline review group used the original perioperative management plan, and the evidence application group used the perioperative management plan based on the best evidence. The implementation rate of each review indicator, the incidence of radial artery occlusion, the degree of compression pain, and the comfort level of patients were compared between the two groups.Results:The implementation rates of review indicators 1, 2, 3, 5, 6, 7, 8 in the evidence application group were 100.0% (94/94), 100.0% (94/94), 11.7(11/94), 88.3% (83/94), 100.0% (94/94), 100.0%(94/94), 85.1%(80/94), respectively, which were higher than those in the baseline review group(all 0), except for the review indicator 4, the differences were statistically significant ( χ2 values were 9.00-178.02, all P<0.05). The incidence of radial artery occlusion and the incidence of pain Numerical Rating Scale>3 points in the evidence application group were 2.1% (2/94) and 3.2% (3/94), respectively, which were lower than 14.8% (13/88) and 23.9% (21/88) in the baseline review group; the comfort level of patients in the evidence application group was 96.8% (91/94), which was higher than 63.6% (56/88) in the baseline review group. The differences were statistically significant ( χ2 = 8.01, 15.21, 30.10, all P<0.05). Conclusions:The best evidence for the prevention of radial artery occlusion after transradial coronary angiography or intervention can be applied to clinical practice, which can standardize the behavior of medical staff, reduce the incidence of postoperative radial artery occlusion, reduce the degree of compression pain, and improve the comfort of patients.

Objective:To analyze the clinical and radiological features of patients with hypoplasia of the internal carotid artery.Methods:Four patients with hypoplasia of the internal carotid artery (including 3 patients with congenital absence of the internal carotid artery), admitted to our hospital from January 2010 to December 2021, were chosen in our study. The clinical data and imaging features of these patients were retrospectively analyzed.Results:The age of these patients ranged from 31 to 73 years, and two patients were female. Clinically,2 patients presented with dizziness, 1 patient presented with limb weakness, and 1 patient was asymptomatic. In 3 patients with congenital absence of the internal carotid artery, DSA results showed that the internal carotid artery was not developed, and CT bone window showed that the internal carotid artery canal was absent; 1 patient with hypoplasia of the internal carotid artery showed thin imaging in DSA results. One patient was combined with anterior communicating artery aneurysm and left internal carotid artery aneurysm. One patient was with vertebral arterial dolichoectasia. The collateral circulation compensation: type A was noted in 3 patients and type C was noted in 1 patient.Conclusion:The patients with hypoplasia of the internal carotid artery are usually asymptomatic, and patients with congenital absence of the internal carotid artery can have ischemic symptom; these patients can be complicated with vascular abnormalities such as aneurysm and vascular thickening.

Objective:To investigate the clinical value of pelvic floor biofeedback combined with acupuncture and moxibustion for the prevention of postoperative recurrence of endometriosis, and to evaluate its safety.Methods:From January 2017 to December 2017, 80 patients with endometriosis treated in Shenzhen Hospital of Guangzhou University of Traditional Chinese Medicine were selected and divided into observation group and control group according to the random digital table method, with 40 cases in each group.The control group received GnRH-A injection (zoladex) for 3 months after surgery.The observation group was treated with pelvic floor biofeedback combined with acupuncture on the basis of the control group.The cumulative recurrence of the two groups at 3, 6 and 12 months after operation was compared, and the safety and quality of life(QOL) of the patients were evaluated.Results:The cumulative recurrence rate of the control group was 12.5%(5/40) at 12 months after surgery, which was slightly higher than that of the observation group[10.0%(4/40)], but the difference was not statistically significant (χ 2=0.125, P>0.05). The serum alanine aminotransferase level of the two groups at 3 months after operation was within the normal reference range, and the difference was not statistically significant ( P>0.05). There was statistically significant difference in QOL score between the two groups at 3 months after operation ( t=4.923, P<0.01). Conclusion:The postoperative recurrence rate of pelvic floor biofeedback combined with acupuncture for the prevention and treatment of endometriosis is not significantly different from that of the control group, and it is safe and reliable.Besides, it can make the body immune system well regulated, improve microcirculation, and promote the quality of life of patients.

Objective:To explore the changes of serum zinc-2 glycoprotein (ZAG) levels in patients with polycystic ovary syndrome (PCOS) with metabolic indicators, and to seek the correlation between metabolic disorders and ZAG in patients with PCOS, so as to provide a basis for the diagnosis and treatment of metabolic disorders in patients with PCOS.Methods:A total of 200 Han PCOS patients admitted to Shenzhen Hospital of Guangzhou University of Traditional Chinese Medicine (Futian) from January 2017 to June 2018 were selected as the observation group, and divided into the group A (BMI ≥ 25kg/m 2) and the group B (BMI <25kg/m 2) according to body mass index(BMI). Meanwhile, 100 healthy Han women who came to the hospital for physical examination during this period were selected as the control group.ZAG, fasting blood glucose (FPG), fasting insulin (FINS), triglyceride (TG), total cholesterol (TC), low-density lipoprotein (LDL), and high-density lipoprotein (HDL) were measured, and the differences among the three groups were compared.After weight control, group A reviewed the above indicators and compared the differences of these indicators before and after weight control. Results:In group A, the FPG, FINS, HOMA-IR, TC, TG, HDL, LDL and ZAG were (5.92±0.51)mmol/L, (19.94±3.15)mIU/L, (5.29±1.19), (5.59±0.49)mmol/L, (2.03±0.35)mmol/L, (0.72±0.21)mmol/L, (3.05±0.43)mmol/L, (35.80±8.12)ng/mL, respectively, which in group B were (5.38±0.62)mmol/L, (15.67±4.59)mIU/L, (3.84±1.45), (4.61±0.65)mmol/L, (1.49±0.35)mmol/L, (1.24±0.40)mmol/L, (2.61±0.40)mmol/L, (45.22±10.27)ng/mL, respectively, which in the control group were (4.77±0.53)mmol/L, (11.66±3.23)mIU/L, (2.52±0.95), (4.59±0.53)mmol/L, (1.39±0.26)mmol/L, (1.40±0.31)mmol/L, (2.50±0.35)mmol/L, (59.15±8.90)ng/mL, respectively, and the differences were statistically significant between group A and group B, between group A and control group, and between group B and control group( F=36.81, 41.04, 44.05, 33.65, 38.61, 39.25, 18.45, 56.88, all P<0.01). After weight control, the FPG, FINS, HOMA-IR, TC, TG, HDL, LDL and ZAG in group A were (5.27±0.54)mmol/L, (15.67±3.16)mIU/L, (3.72±1.06), (4.98±0.45)mmol/L, (1.62±0.30)mmol/L, (1.03±0.25)mmol/L, (2.82±0.43)mmol/L, (43.59±8.20)ng/mL, respectively, and the differences were statistically significant in group A before and after weight control ( t=15.26, 15.67, 17.70, 11.04, 17.61, -9.61, 22.83, -13.79, all P<0.01). The serum ZAG was positively correlated with HDL ( r=0.628, P<0.01), and negatively correlated with BMI ( r=-0.685, P<0.01), HOMA-IR ( r=-0.729, P<0.01), and TC ( r=-0.579, P<0.01). Conclusion:ZAG value is reducedin PCOS patients, with lower level in obesity group, and is positively correlated with HDL, negatively correlated with BMI, HOMA-IR, TC.The metabolism in patients with PCOS can be reflected by ZAG, which has a certain value in diagnosis and treatment of PCOS patients with metabolic disorder, and provides theory basis for further study of the metabolic disorder of PCOS.
ABSTRACT
Objective:To investigate the therapeutic mechanism of high-frequency repetitive transcranial magnetic stimulation (rTMS) in treating mild amnestic cognitive impairment (aMCI).Methods:Twenty-five patients with aMCI were randomly divided into an observation group of 13 and a control group of 12. The observation group was given 10-Hz rTMS over the left dorsolateral prefrontal cortex at 80% of the motor threshold-400 pulses a day, 5 times a week for 4 consecutive weeks. The control group received sham stimulation on the same schedule. Before and after the experiment, both groups were evaluated using the Montreal cognitive assessment (MoCA) and received fMRI scans.Results:After the intervention, the average MoCA score of the observation group had improved significantly more compared with that of the control group and compared with before the intervention. According to the fMRI results, regional homogeneity in the right middle frontal gyrus of the observation group had increased significantly, while that of the control group both there and in the left precuneus had decreased significantly.Conclusions:High-frequency rTMS can effectively improve the cognitive function of patients with aMCI and synchronize neuron activity in cognition-related brain regions.

Objective@#To investigate the current situation of clinical nurses′ caring behavior and patients′ perception of caring, to understand the level of nursing care behavior and analyze the influencing factors, and to discuss the relationship between patients′ perception of caring and clinical nurses′ caring behavior.@*Methods@#The convenience sampling method was used to investigate clinical nurses and inpatients from October to December 2018. The Caring Behavior Checklist was used to observe nurses' nursing care behavior, and the Client Perception of Caring Scale was used to investigate patients' perception of Caring.@*Results@#A total of 132 clinical nurses were observed and 132 patients were investigated. The score of verbal caring behavior was (3.36±1.44) points (out of a possible 7 points), and the score of non-verbal caring behavior was (2.33±1.07) points (out of a possible 5 points).The overall score of Caring Behavior Checklist was (5.70±2.03) points (out of a possible 12 points). Nurses aged 26-35 (6.42±2.07) points had a higher level of nursing care behavior than nurses of other age groups, and the difference was statistically significant (F=3.902, P<0.05).N1 nurses had a higher level of nursing care behavior than nurses at other levels, it was (7.72±2.30) points, and the difference was significant (F=14.290, P<0.05). There was no significant difference in caring behavior level among nurses with different gender, departments, education backgrounds, academic titles and nursing ages (P>0.05). The overall score of Client Perception of Caring Scale was (43.49 ±3.84) (out of a possible 60 points), and there was a high positive correlation between client perception of caring and nursing care behavior (r=0.720, P<0.05).@*Conclusions@#The level of clinical nurses′ caring behavior is low, and the training of caring knowledge, belief and behavior should be carried out to promote nurses′ caring behavior, so as to further improve client perception of caring.

Objective To evaluate the diagnostic value of cerebral vascular DSA in vertebral artery variation,understand the types of vertebral artery variation and the imaging characteristics of vertebral artery variation.Methods Nine hundred and forty-six patients received cerebral vascular DSA in our hospital from October 2010 to May 2015 were chosen.DSA characteristics of patients with vertebral artery variations were analyzed and the vertebral artery variation types and incidence were conformed.Results There were 140 patients with vertebral artery variations.Sixteen patients (1.7%) was diagnosed with anomalous origin of vertebral artery,including 14 of left vertebral artery anomalous origin,2 of right vertebral artery anomalous origin.One patient (0.1%) had persistent trigeminal artery originated from the right internal carotid artery and one patient (0.1%) had proatlantal intersegrnental artery originated from the left external carotid artery.The vertebral artery of 10 patients (1%) was found with fenestration.The posterior inferior cerebellar artery of 25 patients (2.6%) was originated from the lower portion of extracranial location.Ninety-two patients (9.7%) were found with vertebral artery hypoplasia,including 54 with right vertebral artery hypoplasia and 38 with left vertebral artery hypoplasia.There were 12 patients with anastomosis abnormalities in the vertebral artery hypoplasia,including 4 with right vertebral artery anastomosis abnormalities,and 8 with left vertebral artery anastomosis abnormalities.Conclusion Cerebral vascular DSA can clearly show the location and shape of vertebral artery variation,and it is important in the clinical diagnosis and treatment of cerebrovascular disease,interventional treatment and posterior fossa surgery.

OBJECTIVE:To systematically review the current situation and facing problems of hierarchical diagnosis and treat-ment in China,and provide basis for nation health-related decisions. METHODS:Using"hierarchical diagnosis and treatment"as keyword,relevant literatures in CNKI,VIP database,and Wanfang database from building to Jun. 2016 were retrieved. Study for current situation of hierarchical diagnosis and treatment in China was comprehensively collected,and systematical review was con-ducted from aspects of outcome evaluation indicators and facing problems. RESULTS:Totally 23 studies were included,in which, 18 were investigative studies,3 were retrospective/investigative studies,1 was retrospective study,and 1 was randomized con-trolled study. In the outcome evaluation indicators,9 took awareness of hierarchical diagnosis and treatment as review indicators, showing that the public had poor awareness;7 took the current situation as review indicator,showing that the overall current situa-tion was unsatisfactory;3 took human resources as review indicator,showing that human resources in primary medical institutions were not ideal;3 took satisfactory degree as review indicator,showing that the public were generally satisfied with hierarchical di-agnosis;2 took the implementation model as review indicator,showing that the current model had been able to promote hierarchi-cal diagnosis and treatment;2 took medical cost as review indicator,showing that individuals paid higher. In the facing problems, 19 studies suggested the imperfect systems for hierarchical diagnosis and treatment,16 suggested that primary medical treatment was limited in capability,7 suggested that patients still preferred big hospitals to primary medical institutions,and 7 suggested that it lacked publicity. CONCLUSIONS:The current situation of hierarchical diagnosis and treatment is certainly not ideal but still fac-ing many problems in China. Improving the levels in primary institutions,hierarchical diagnosis and treatment system and patients' willingness to primary medical institutions,and increasing its publicity can deeply promote the implementation of hierarchical diag-nosis and treatment and provide objective and valuable reference for health decision-making.
ABSTRACT
Objective To observe the effects of mind-regulating brain-developing acupoint magnetic therapy in patients with mild cogni-tive impairment (MCI). Methods From May to October, 2015, 60 patients with MCI were randomly divided into control group (n=30) and observation group (n=30). The control group received magnetic therapy on Yangming meridians, and the observation group received magnet-ic therapy on mind-regulating brain-developing acupoints, for two months. They were assessed with Montreal Cognitive Assessment (Mo-CA), digit-span and digit symbol of Wechsler Adult Intelligence Scale (WAIS). Results After treatment, the scores of MoCA, digit-span and digit symbol of WAIS significantly increased in the observation group (t>4.858, P2.188, P5.527, P2.324, P2.262, P0.05). Conclusion Magnetic therapy on acupoints related with cognition could improve the cognitive function in patients with MCI.

Objective PRDX-2 of the basic level in patients with PCOS,and its correlation with cardiovascular risk factors.Methods Collected 78 cases of PCOS patients for POCS group,at the same time,choose healthy check-up 40 cases for control group.Determination of plasma Peroxiredoxin-2,blood sugar,glycosylated hemoglobin,cholesterol,triglycerides,and other biochemical indicators.Results Peroxiredoxin-2 level in PCOS group are higher than the control group(F=26.03;P<0.05).Peroxiredoxin-2 and cardiovascular risk factors exist correlation,including systolic blood pressure (r=0.213),diastolic blood pressure (r=0.380),fasting blood glucose (r=0.308),glycosylated hemoglobin (r=0.303),cholesterol (r=0.190),triglyceride (r=0.276),LDL-C (r=0.484) were positively correlated,and CRP (r=0.227),HDL-C negative correlation (r=0.355).Conclusion Determination of Peroxiredoxin-2 level can be used as evaluation of patients with PCOS risk factors for cardiovascular disease.

Objective To investigate the efficacy and safety of long guidewire exchange technique in performing cerebral angiography via radial artery approach.Methods The clinical data of 257 patients,who received cerebral angiography during the period from June 2014 to May 2016 at Daxing Hospital of Capital Medical University,Beijing,China,were retrospectively analyzed.Among the 257 patients,186 had type Ⅱ or type Ⅲ aortic arch.According to the different looping technologies for Simmons-2 catheter used in the performance of cerebral angiography,these 186 patients were divided into the study group (n=90) and the routine group (n=96).The success rate of radial artery puncturing,the success rate of Simmons-2 catheter looping formation,the success rate of supra-arch arterial catheterization,the time spent for angiography,the radiation exposure time,complications,etc.were recorded.Results The success rate of radial artery puncturing was 100% in both groups.The success rate of Simmons-2 catheter looping formation in the study group was 97.8%,which was significantly higher than 72.9% in the routine group (P<0.05).The success rates of catheterization for the left subclavian artery,left common carotid artery and right carotid artery in the study group were 97.8%,97.8% and 97.8% respectively,which were remarkably higher than 72.9%,72.9% and 72.9% respectively in the routine group (P<0.05).No statistically significant difference in the success rates of catheterization for right subclavian artery and for right vertebral artery existed between the two groups (P> 0.05).The time spent for angiography in the study group and the routine group was (22.5±9.4) min and (28.3±8.3) min respectively.The radiation exposure time in the study group was (8.9±4.5) min,which was obviously lower than (12.6±6.5) min in the routine group (P=0.017 and P=0.035,respectively).The incidences of complications in the study group and the routine group were 4.4% and 5.2% respectively,the difference was not statistically significant (P>0.05).No severe complications,such as intracranial arterial embolie events,puncture bleeding,arterial occlusion,etc.,occurred in all patients of both groups.Conclusion The use of long guidewire exchange technology can improve the success rate of Simmon-2 catheter looping formation and the success rate of supra-arch arterial catheterization,it can also shorten the time for angiography procedure and reduce the radiation exposure time.This technique is very effective with higher safety.Therefore,this long guidewire exchange technique should be used as a common method for cerebral angiopraphy via radial artery approach in patients with type Ⅱ or type Ⅲ aortic arch.

Objective To explore the influence of aerobic exercise on plasma lipoprotein in overweight and obese PCOS women.Methods 12 obese and overweight patients with PCOS were selected.Before and after 12 weeks of aerobic exercise,the height,weight,BMI,WHR,body fat percentage and fasting blood samples of high-density lipoprotein(HDL),low density lipoprotein(VLDL),triglyceride and cholesterol levels were measured and detected.Results After 12 weeks of aerobic exercise,the BMI[(25.43±2.17)kg/cm2 vs.(22.27±1.57)kg/cm2,t=8.52],WHR[(0.864±0.021) vs.(0.831±0.019),t=2.99],fat rate[(33.76±5.36)% vs.(30.53±5.66)%,t=5.580],and triglyceride[(1.90±0.58)mmol/L vs.(0.95±0.51)mmol/L,t=7.400]were obviously decreased,the high-density lipoprotein cholesterol[(2.07±0.39)mmol/L vs.(3.31±0.48)mmol/L,t=3.352]was obviously increased,the differences were statistically significant (all P<0.05).Conclusion Obese and overweight PCOS women can lose weight through aerobic exercise,so as to adjust lipoprotein spectrum and improve health.
ABSTRACT
Objective To explore the application of Duff in treatment for patients with early stage threat ened abortion.Method 1000 patients with early stage threatened abortion admitted from Jan.2015 to Jun.2016 were selected,and were divided into two groups.All patients were treated by conventional therapy and Duff therapy.Treatment effects of the two groups were observed and compared.Results The differences of clinical characteristics between the two groups had no statistical significance (all P>0.05).No serious adverse reactions oc curred to either group.There was no significant difference in remission and disappearance time of clinical symptoms between the two groups (t=0.334,0.367,all P>0.05).The total treatment efficiency was higher in the observation group than in the control group,but the differences had no statistical significance (x2=0.058,/P>0.05).Conclusion There is a certain application value of Duff treatment for early stage threatened abortion,which is safe,efficient,and convenient,worthy of promotion.
ABSTRACT
Human 5-lipoxygenase (5-LOX) is a well-validated drug target and its inhibitors are potential drugs for treating leukotriene-related disorders. Our previous work on structural optimization of the hit compound 2 from our in-house collection identified two lead compounds, 3a and 3b, exhibiting a potent inhibitory profile against 5-LOX with IC50 values less than 1 µmol/L in cell-based assays. Here, we further optimized these compounds to prepare a class of novel pyrazole derivatives by opening the fused-ring system. Several new compounds exhibited more potent inhibitory activity than the lead compounds against 5-LOX. In particular, compound 4e not only suppressed lipopolysaccharide-induced inflammation in brain inflammatory cells and protected neurons from oxidative toxicity, but also significantly decreased infarct damage in a mouse model of cerebral ischemia. Molecular docking analysis further confirmed the consistency of our theoretical results and experimental data. In conclusion, the excellent in vitro and in vivo inhibitory activities of these compounds against 5-LOX suggested that these novel chemical structures have a promising therapeutic potential to treat leukotriene-related disorders.
ABSTRACT
Objective To compare the effect of diversification rehabilitation team mode on function in patients with cerebral infarction. Methods Sixty-six patients with cerebral infarction were divided into diversification rehabilitation team mode group (diversification group) and routine rehabilitation mode (routine group) according to the rehabililation method with 33 cases each. All patients of 2 groups were treated for 2 weeks. Evaluations were made before and after treatment. The simplified Fugl-Meyer motor function rating scale was used to evaluate motor function, modified Barthel index was used to evaluate activities of daily living, and MOS 36-item short form health survey was used to evaluate quality of life;and 0-100 digital simulation assessment was used to evaluate patient satisfaction after treatment. Results The simplified Fugl-Meyer motor function rating scale score, modified Barthel index score, MOS 36-item short form health survey score and patient satisfaction rate after treatment in diversification group were significantly better than those in routine group: (76 ± 4) scores vs. (63 ± 3) scores, (65 ± 3) scores vs. (52 ± 4 ) scores, (57 ± 7) scores vs. (44 ± 6) scores, (92 ± 5) scores vs. (77 ± 3) scores, and there were statistical differences (P<0.05). Conclusions Both kinds of rehabilitation model can promote functional recovery in patients with cerebral infarction, but diversification rehabilitation team model is better than conventional model.
ABSTRACT
Objective To investigate the clinical effect of caffeine citrate combined with continuous positive airway pressure( CPAP) in the treatment of primary apnea of prematurity.Methods 80 premature infants with primary apnea who received therapy from January 2014 to August 2015 were selected as research subjects.According to ran-dom number table,the infants were divided into two groups equally.Besides conventional treatments,the control group received treatment of caffeine citrate,while the observation group received CPAP and caffeine citrate.Then,the cura-tive efficacy,situation of apnea and adverse reactions were compared.Results The total effective rate between the observation group and the control group had statically significant difference(95.0%vs.77.5%,P0 .0 5 ) . Conclusion Caffeine citrate in combination with CPAP is effective for primary apnea of prematurity,which can sig-nificantly increase curative efficacy,lessen seizure frequency of apnea,shorten duration of seizure and disappearance time of symptoms with less adverse reactions.
