ABSTRACT
Objective:To analyse the pathogenic bacteria distribution and clinical characteristics of late-onset sepsis (LOS) among premature infants with gestational age less than 34 weeks in Henan Province.Methods:The clinical data of 6 590 premature infants admitted to 17 medical institutions in Henan Province from January 2019 to December 2020 were retrospectively analyzed. The gestational age of infants was less than 34 weeks and was admitted to the neonatal ward within 7 days after birth. SPSS 19.0 statistical software was used for data analysis.Results:Among 6 590 premature infants LOS developed in 751 cases (11.40%), of whom the diagnosis was confirmed in 276 cases (36.75%) and 475 cases (63.25%) were diagnosed clinically. The fatality rate related to LOS was 13.58%. There were significant differences in the incidence of LOS and infection-related mortality among infants with different gestational ages and body weights ( χ2=388.894 and 13.572, χ2=472.282 and 9.257, P<0.05 or <0.01). Among 276 children with confirmed LOS, 286 strains of pathogenic bacteria were isolated. Gram-negative bacteria were most prevalent (178 strains), accounting for 62.24% of all infections, followed by fungi (58 strains, 20.28%). Klebsiella pneumoniae was most frequently detected Gram-negative bacteria (117 strains, 40.91%), among which 32.48% (38/117) was carbapenem-resistant Klebsiella pneumoniae. The proportion of diagnosed sepsis, the proportion of catheterization, and the infection-related mortality of infants with LOS in tertiary hospitals were all higher than those in secondary hospitals ( χ2=6.212, 5.313 and 4.435, all P<0.05). The proportion of exclusive breastfeeding in secondary hospitals was lower than that in tertiary hospitals ( χ2=19.216, P<0.05). The time of antibacterial drug use before infection in specialized hospitals was longer than that in general hospitals ( χ2=3.276, P<0.05). Conclusion:The incidence of LOS among preterm infants in Henan Province is high, which was mainly caused by Gram-negative bacteria. The clinical characteristics of LOS caused by different pathogens and in different health institutions are different, the prevention and control strategy should be developed accordingly to reduce the incidence LOS of preterm premature infants.

Objective:To explore the effect of "Internet +" continuous nursing on medication compliance of patients with severe osteoporosis.Methods:A retrospective comparative study was conducted to analyze the clinical data of patients with severe osteoporosis hospitalized in Pinghu Hospital of Traditional Chinese Medicine from October 2017 to October 2019. A total of 120 patients with osteoporosis were included and divided into the continuous care group and routine care group using double-blind randomization method. The continuous care group was given "Internet +" continuous care, and the routine care group was given routine health education upon discharge. The compliance and Bone mineral density were compared at 1, 3, 6 and 12 months after discharge.Results:Based on "Internet +" continue nursing group of patients discharged from hospital after 1, 3, 6, 12 months medication adherence scores respectively (6.92 ± 0.93), (6.72 ± 1.04), (6.28 ± 1.26) and (6.27 ± 1.21) points, routine nursing group, respectively (6.27±1.27), (5.55±1.24), (5.28±1.59) and (4.60±1.71) points, continuity of care compliance significantly better in routine nursing group, contrast at each time point in the 2 groups were statistically significant ( t values were 3.197-6.172, P<0.01). After 1, 3, 6 and 12 months of treatment, the bone mineral density of patients in the continuous care group was -2.97±0.10, -2.92±0.11, -2.88±0.11, -2.87±0.13, and that in the conventional care group was 2.99±1.08, 3.01±0.11, 3.02±0.11, 3.02±0.11. Bone mineral density in the continuous care group was significantly better than that in the conventional care group. There was no statistically significant difference between the two groups 1 month after treatment ( t value was 1.289, P>0.05), and there was a statistically significant difference between the two groups 3, 6 and 12 months after treatment ( t values were 4.215, 5.992, 6.726, P<0.01). Conclusion:Continuous nursing based on "Internet +" can greatly improve the medication compliance of osteoporosis patients, effectively improve the bone mineral density of patients, reduce the incidence of brittle fracture, reduce the readmission rate of patients, and improve the quality of life of patients.
ABSTRACT
This study proposed a method to calibrate tube focus spot and the center plane of rotation in computed tomography system. In the method, the tube was rotated to 0° and 180° respectively, and then one metal jig with symmetric windows A and B was scanned at each position under the tube cool and static condition. According to the geometry of tube focus spot, aperture center of the collimator and jig, the distance between tube focus spot and the center plane of rotation were calculated with the X ray transmittance data after denoising, mean value and normalization. To verify the practicability and validity of the method, the tube focus spot in a 16 slices CT system (Brivo CT385, GE, China) was calibrated, and the result after calibration was validated by scanning a polaroid film. The validation result showed that the deviation between tube focal spot and center plane of rotation was 0.02 mm and was in the error range within ± 0.1 mm. The results of this study showed that, as a simple and low-cost design, the method could be used for fast calibration between tube focus spot and the center plane of rotation.

One algorithm is designed to implement longitudinal auto-tracking of the the detector on X-ray in the digital radiography system (DR) with manual collimator. In this study, when the longitudinal length of field of view (LFOV) on the detector is coincided with the longitudinal effective imaging size of the detector, the collimator half open angle ( ), the maximum centric distance ( ) between the center of X-ray field of view and the projection center of the focal spot, and the detector moving distance for auto-traking can be calculated automatically. When LFOV is smaller than the longitudinal effective imaging size of the detector by reducing , the can still be used to calculate the detector moving distance. Using this auto-tracking algorithm in DR with manual collimator, the tested results show that the X-ray projection is totally covered by the effective imaging area of the detector, although the center of the field of view is not aligned with the center of the effective imaging area of the detector. As a simple and low-cost design, the algorithm can be used for longitudinal auto-tracking of the detector on X-ray in the manual collimator DR.
ABSTRACT
Objective To investigate the incidence and risk factors of acute kidney injury (AKI). Methods The clinical data of 438 patients with severe pneumonia who were admitted in the intensive care unit in 2016 were retrospectively analyzed for analysis of AKI and assessment of risk factors. Results Among the 438 patients with severe pneumonia,the incidence of AKI was 53.42%. Within 72 hours after admission into the ICU, 86.75% of them developed AKI mainly from hypovolemic or septic shock(accounting for 80.34%). The detection rate of AKI by the standards of RIFLE and AKIN was lower than by the standards of KDIGO (45.89%,48.17%vs. 53.42%). Anemia ,shock /hypotension ,drugs ,RRT ,APPACHE II scores were the predictors of AKI. Conclusions The incidence of severe pneumonia complicated with AKI is high. Most of the patients develop it in 72 hours after admission into the ICU. Anemia,shock/hypotension,drugs,RRT,APPACHE II scores are its risk factors.

Objective To observe integrin linked kinase(ILK)expression in obstructive nephropathy with renal tubular acidosis, and investigate the function of renal tubular acidosis on renal interstitial fibrosis.Methods 75 rate were divided into control group and unilateral ureteral obstruction(unilateral ureteral obstruction(UUO,n=25)group,and other 50 rats in UUO group.UUO group was divided into renal tubular acidosis (UUO1 group,n=31)group and non-renal tubular acidosis(UUO2,n=19)group,re-nal function and histology test was carried at the same time.7,14,21,28 days later after operation,the rats were killed in batches, then measured the expression of ILK in renal tissue by immunohistochemistry and western blot.Results Inflammatory cell infiltra-tion and interstitial fibrosis of UUO1 group were more serious under the light microscope than that of UUO2 group;there was only a small amount of ILK protein expression in control group,almost no FN protein deposition,expression of ILK and FN protein pre-cipitation of UUO2 group increased,and UUO1 group was more than UUO2 group.Conclusion In obstructive nephropathy,renal tubular acidosis can up-regulate expression of ILK and FN sedimentation,then lead to interstitial fibrosis promotion.
ABSTRACT
The diagnosis of sleep apnea syndrome (SAS) has a significant importance in clinic for preventing diseases of hypertention, coronary heart disease, arrhythmia and cerebrovascular disorder, etc. This study presents a novel method for SAS detection based on single-channel electrocardiogram (ECG) signal. The method preprocessed ECG and detected QRS waves to get RR signal and ECG-derived respiratory (EDR) signal. Then 40 time- and spectral-domain features were extracted to normalize the signals. After that support vector machine (SVM) was used to classify the signals as "apnea" or "normal". Finally, the performance of the method was evaluated by the MIT-BIH Apnea-ECG database, and an accuracy of 95% in train sets and an accuracy of 88% in test sets were achieved.

OBJECTIVE Through nosocomial infection targeted surveillance in intensive care unit(ICU),to evaluate the characteristics of nosocomical infection and their its correlation with the results of environmental monitoring at the same period.METHODS Nosocomial infection monitoring was explored on patients who were in ICU more than 48 h from Jan 1997 to Jun 2008.Correlation analysis was conducted between the rate of nosocomial infection in ICU and the same period environmental monitoring results.Data were analyzed with the SPSS10.34 software.RESULTS There was significant difference(P0.05)in the rate of infection cases before and after target monitoring was practiced.The site of infection was mainly in respiratory tract,followed by the gastrointestinal tract and urinary tract;The full-time personnel in duty for nosocomial infection surveillance monitored the indoor air,object surface,medical personnel's hands in ICU,their qualified rate was 78.28%,90.94% and 95.83%,respectively.CONCLUSIONS There are many risk factors in patients in ICU,ICU should be looked as one of the important nosocomial infection monitoring department.The target monitoring of nosocomial infection is an effective method;the nosocomial infection rate in ICU and the environmental result at the same period don't show relevance,the environmental hygiene monitoring should be scientific and practical.

Objective:To explore,on the basis of principles of bibliometrics,the distribution of the writers,institutions,regions and journals of the publications on HIFU and the literature amount,and to learn the situation of cross discipline study on HIFU and of the different regions where the articles on HIFU came in order to make a clear view of the characteristics and the status of co-operations in HIFU work and to determine,among them,the key authors,key institutes and key regions so that the progress of HIFU study was turned out.Method:Based on the information drawn from VIP-database,bibliometric method was used to analyze all the data on HIFU before the year of 2007.Result:833 articles were published before the year of 2007,in which 686(72.87%) were written by more than one authors,with a degree of collaboration as 2.78.The number of articles by mono-author was 312(37.45%).186 articles(22.33%) were from Chongqing,taking the first place in number.134 of the key authors published 410 papers(49.22%).10 of the institutes with more than 9 articles turned out 249 papers(29.89%).2000 saw an obvious change in the situation of publications on HIFU and the collaboration among the authors and from that year to 2006,792 papers were published with a degree of collaboration as 3.72 and the average rate of co-authership as 81.12%.Conclusion:The authors on HIFU,with a group as their key authors who were advanced in academy,productive in papers and influential on various journals in medical field,were widely distributed and had a high cooperative rate.But it was also revealed that the researchers in the field of HIFU were unevenly distributed,with the institutions in Chonqing taking the lead.

Objective:To investigate the changes of pulmonary IMD and its receptor system - calcitonin receptor-like receptor (CL) and receptor activity modifying proteins (RAMPs) mRNA in acute lung injury(ALI) induced by oleic acid of rats. Methods: Contents of IMD in plasma and lung homogenates were measured by radioimmunoassay(RIA). The lung mRNA of IMD, CL and RAMPs was determined by semi-quantitative RT-PCR. Results: Compared with control group, in ALI group, the contents of IMD_ 1-53 in plasma and lung homogenates were decreased by 20.8% and 74.5% (all P
