ABSTRACT
BackgroundSuicide is one of the serious public health problems worldwide. The relationship between suicide and neutrophil-to-lymphocyte ratio (NLR) may vary in different regions and different age. It is necessary to further investigate the relationship between NLR and suicidal ideation in Chinese children and adolescents with depression. ObjectiveTo explore the correlation between NLR and suicidal ideation in children and adolescents with depression, so as to provide clues for exploring the biomarkers of suicide. MethodsA retrospective analysis of 536 children and adolescents with depression who were hospitalized in the Third People's Hospital of Fuyang from January 2020 to December 2022 and met the diagnostic criteria of the International Classification of Diseases, tenth edition (ICD-10) was performed. Patients were divided into two groups according to whether they reported suicidal ideation. Demographic data, discharge diagnosis, Hamilton Depression Scale-17 item (HAMD-17) score and hematological test data (neutrophil counts, lymphocyte counts) on the second day were collected from medical records. Receiver operating characteristic (ROC) curve was used to determine the optimal cut-off point of NLR for predicting suicidal ideation in children and adolescents with depression, and binary Logistic regression was used to analyze the risk factors for suicidal ideation. ResultsAmong the 536 patients, 429 cases (80.04%) had no suicidal ideation, and 107 cases (19.96%) had suicidal ideation. Compared with patients without suicidal ideation, the HAMD-17 score [(25.28±8.86) vs. (21.21±8.46), F=19.400, P<0.01], neutrophil level [(3.85±1.68)×109/L vs. (3.15±1.14)×109/L, Z=4.073, P<0.01], and NLR level [(1.96±1.50) vs. (1.52±0.71), Z=3.532, P<0.01] in the suicidal ideation patients were significantly higher. The optimal critical NLR value determined by the ROC curve was 1.52 (59.80% sensitivity, 58.50% specificity), with an area under the curve of 0.610. Logistic regression analysis showed that the risk of suicidal ideation was 1.94 times higher in those with high NLR than in the low NLR after controlling for age, sex, age at onset, duration of illness, and HAMD-17 score (OR=1.940, 95% CI: 1.251~3.009, P=0.003). ConclusionNLR may be a risk factor and potential biomarker influencing suicidal ideation in the children and adolescents with first-episode depression. ABSTRACT
In order to implement the teaching philosophy of "early clinical, multiple clinical, and repeated clinical", this study takes the "New Medical Professional Experience Social Practice" as a research course, analyzes the reform of pediatric clinical teaching, and evaluates the teaching effectiveness through classroom interviews and post-class questionnaire surveys. The results showed that through the combination of online and offline learning and multiple evaluation models, it was found that students with more hands-on teaching content in offline teaching were more interested and evaluated. Eight-year students had higher participation in offline teaching than five-year students, and 8-year students also rated online teaching higher than five-year students. A total of 151 [84.36%(151/179)] students have learned about the clinical work of pediatricians through this course. The reform of pediatric clinical teaching based on the concept of early clinical teaching can achieve the teaching goal of early clinical practice and early contact for medical students.
ABSTRACT
@#The umbrella trial has received increasing attention in the design of clinical trials for oncology drugs in recent years. This trial design categorizes a single disease into multiple sub-types based on predictive biomarkers or other predictive factors, and simultaneously evaluates the efficacy of multiple targeted therapies. When compared with the traditional drug development model of phase Ⅰ, phaseⅡ, and phase Ⅲ randomized controlled trials, umbrella trials are a more scientifically rigorous trial design that can speed up drug evaluation to address the conflict between numerous untested drugs and diseases with a lack of effective treatment options. This article will focus on the concept, main characteristics, eligibility criteria, design and statistical considerations, ethical considerations, and future directions of umbrella trials, with the aim of providing methodological guidance for the design of clinical trials for oncology drugs.
ABSTRACT
【Objective】 To explore the effect of recovery autologous blood transfusion combined with bilateral internal iliac artery presetting in high-risk patients with hemorrhage during cesarean section. 【Methods】 A total of 162 high-risk patients with hemorrhage who underwent cesarean section from January 2021 to May 2023 in our hospital were prospectively selected and divided into in Groups A, B, and C with 54 cases in each group according to the indications for the method of transfusion. Group A received allogeneic blood transfusion, Group B received autologous blood transfusion, Group C received autologous blood transfusion combined with bilateral internal iliac artery balloon presetting. 【Results】 Intraoperative blood loss (mL) (1 600 vs 1 500 vs 800), postoperative hospital stay(d) (7 vs 7 vs 6) and operative time(min) (107 vs 104.50 vs 77) in group C were all lower than those in group A and B (P0.05); The autologous blood transfusion volume(mL) in group C was lower than that in group B (525.5 vs 261, P0.016 7). 【Conclusion】 Recovery autologous blood transfusion combined with bilateral internal iliac artery balloon presetting in cesarean section for high-risk patients with hemorrhage achieved ideal effects, which can significantly reduce intraoperative blood loss, intraoperative autologous blood transfusion, allogeneic red blood cells and plasma transfusion, as well as the operation time and postoperative hospital stay. In addition, it can improve the coagulation function and hysterectomy, which is conducive to ensuring the safety of maternal and promoting early rehabilitation, and preserving the fertility of patients to a certain extent, which is worthy of further clinical promotion.
ABSTRACT
【Objective】 To investigate whether there is a correlation between the differences in ABO blood group distribution in patients with pancreatic cancer, and to evaluate the relative risk. 【Methods】 Patients with pathological diagnosis or discharge diagnosis of pancreatic cancer who underwent ABO blood group typing in our hospital from January 2017 to October 2021 were selected, and the blood group distribution of patients and the correlation were analyzed. 【Results】 There was a statistically significant difference between the pancreatic cancer group and the control group (P<0.05). The study showed that type A may be a relative risk factor for pancreatic cancer patients (χ2=42.44, P<0.001), and type B may play a protective role (χ2=16.28, P<0.01). Significant differences were found in distribution between different gender groups (χ2=64.35, P<0.05). The test results showed that type A may be a risk factor for pancreatic cancer in men (χ2=35.2, OR=1.7, 95%CI=0.59-1.02, P<0.001), and type O may play a protective role in pancreatic cancer(χ2=18.22, OR=0.6, 95%CI=0.25-0.32, P<0.01); type A may be a relative risk factor for female pancreatic cancer patients (χ2=7.06, OR=1.4, 95%CI=0.59-1.02, P<0.001), while type B may play a protective role (χ2=20.32, OR=0.5, 95%CI=0.32-0.43, P<0.01). In pancreatic cancer group, the risk factors of blood type A were higher than those of non-A group, and the protective effect of type B was significantly higher than that of non-B group. 【Conclusion】 The distribution of blood group and relative risk factors in pancreatic cancer patients suggest that A type is predominant; in the population with A blood group, more attention should be paid to early prevention and early treatment, so as to reduce the risk of disease.
ABSTRACT
Objective:To evaluate the significance of the white blood cells count (WBC), squamous epithelial cells count (SEC), bacteria count (BACT), the ratio of squamous epithelial cells count and white blood cells count (S/W), and the examination of leukocyte esterase by dry chemical method in urine contamination and infection. Methods:A total of 60 cases of urine infection and 60 cases of urine contamination in female outpatients and inpatients in Taizhou Hospital of Traditional Chinese Medicine from January to December 2020 were retrospectively analyzed. The five indexes of WBC, SEC, S/W, BACT and leucocyte esterase were compared between the two groups, WBC, SEC, S/W and BACT were analyzed by receiver operating characteristic (ROC).Results:By comparing the detection results of WBC, SEC, S/W, BACT and leukocyte esterase between the contaminated group and the infected group, using the nonparametric test of two independent samples, the results showed that the median level of WBC in the contaminating group specimens was lower than in the infected group: 58.00 (28.00, 102.00)/μl vs. 329.00 (145.00, 775.50)/μl, the difference was statistically significant ( Z = -7.03, P<0.01). The median level of SEC, S/W and BACT in the contaminated group were higher than those in the infected group: 43.00 (16.25, 73.75)/μl vs. 5.00 (3.00, 12.00)/μl, 0.620 (0.285, 1.209) vs. 0.018 (0.007, 0.037), 21.00 (6.25, 78.50)/μl vs. 204.50 (22.50, 816.75)/μl, and the differences were all statistically significant ( P<0.01). There was no significant difference in the results of leukocyte esterase between the two groups ( χ2 = 1.37, P = 0.24). The ROC curves of WBC, SEC, S/W, BACT were drawn, and the results showed that the four indexes had statistical significance in distinguishing pollution from infection ( P<0.01). Conclusions:Vaginal secretion will affect the detection of leukocyte esterase, WBC, squamous epithelial cells and bacteria in urine. WBC, SEC, S/W and BACT can be used as the differential diagnosis indexes of urine infection and urine contamination, and S/W can be selected as the differential diagnosis index of urine contamination.
ABSTRACT
China is stepping up its standardized management of investigator initiated trials(IIT)carried out by medical and health institutions, spotlighting the establishment and improvement of the quality control system of IIT projects than ever before. The authors retrieved official websites of clinical research related units of medical institutions and research institutes at home and abroad, and by means of literature review analyzed the current quality management of IIT projects at home and abroad. They found such setbacks as lack of quality management standards and norms, imperfect quality control mechanism, poor awareness of quality risk, insufficient quality supervision and poor quality control ability of clinical researchers. Based on the above, the paper made the following recommendations for building an IIT project quality control system in China: developing quality management standards and norms, setting up a systematic quality control mechanism(i.e., exploring a three-level quality control mode composed of the project team/department-hospital-national supervision institution/peer review expert team, and implementing the whole process quality control mechanism), strengthening policy guidance and system construction, and strengthening the standardized training of clinical researchers.
ABSTRACT
@#Objective Zero-events studies frequently occur in systematic reviews of adverse events, which consist of an important source of evidence. We aimed to examine how evidence of zero-events studies was utilized in the meta-analyses of systematic reviews of adverse events. Methods We conducted a survey of systematic reviews published in two periods: January 1, 2015 to January 1, 2020 and January 1, 2008, to April 25, 2011. Databases were searched for systematic reviews that conducted at least one meta-analysis of any healthcare intervention and used adverse events as the exclusive outcome. An adverse event was defined as any untoward medical occurrence in a patient or subject in healthcare practice. We summarized the frequency of occurrence of zero-events studies in eligible systematic reviews and how these studies were dealt with in the meta-analyses of these systematic reviews. Results We included 640 eligible systematic reviews. There were 406 (63.45%) systematic reviews involving zero-events studies in their meta-analyses, among which 389 (95.11%) involved single-arm-zero-events studies and 223 (54.93%) involved double-arm-zero-events studies. The majority (98.71%) of these systematic reviews incorporated single-arm-zero-events studies into the meta-analyses. On the other hand, the majority (76.23%) of them excluded double-arm-zero-events studies from the meta-analyses, of which the majority (87.06%) did not discuss the potential impact of excluding such studies. Systematic reviews published at present (2015-2020) tended to incorporate zero-events studies in meta-analyses than those published in the past (2008-2011), but the difference was not significant [proportion difference=–0.09, 95%CI (–0.21, 0.03), P=0.12]. Conclusion Systematic review authors routinely treated studies with zero-events in both arms as "non-informative" carriers and excluded them from their reviews. Whether studies with no events are "informative" or not, largely depends on the methods and assumptions applied, thus sensitivity analyses using different methods should be considered in future meta-analyses.
ABSTRACT
Objective To explore the role of impulsivity in manic episodes of bipolar I disorder and cognitive impairment. Methods Sixty-one patients with bipolar I manic-episode ( study group) and 43 healthy volunteers (control group) were included in the study, and the cognitive function and impulsivity of the subjects were assessed using the MATRICS consensus cognitive battery (MCCB) and BIS-11 impulsivity scale (BIS-11). The differences in impulsivity and cognitive function between the study group and control group were compared. Partial correlation analysis was used to analyze the correlation between impulsivity and cognitive function in patients with bipolar I manic episode. Results ( 1) The scores of several cognitive function in study group, were significantly lower than those in healthy control group including information pro-cessing speed, attention alertness, word learning, visual learning, working memory, reasoning and problem sol-ving (all P<0. 01). (2)The total score, motor factor score and cognitive factor score of BIS-11 impulse scale in study group were significantly higher than those in control group (58. 39±15. 77 vs 48. 02±11. 16,62. 09± 19. 01 vs 44. 24±21. 09,56. 97±16. 57 vs 50. 06±13. 87,all P<0. 05). Increased overall scores on the bis-11 impulse inventory may be a risk factor for bipolar I episodes( OR=1. 204,95% CI=1. 032-1. 404). (3) In study group, the total score of BIS-11 was negatively correlated with the speed of information processing, working memory, word learning, reasoning and problem solving, and the total score of MCCB(r=-0. 417,-0. 360,-0. 294,-0. 348,-0. 348,P<0. 05). The score of unplanned factor was negatively correlated with the speed of information processing, word learning, the total score of MCCB(r=-0. 397,-0. 302,-0. 358,P<0. 05). The score of cognitive factor was negatively correlated with the speed of information processing,work-ing memory, word learning, reasoning and problem solving, and the total score of MCCB(r=-0. 327,-0. 351,-0. 374,-0. 391,-0. 463,P<0. 05). The score of motor factor was negatively correlated with working memo-ry and the total score of MCCB(r=0. 370,r=0. 389,P<0. 05). Conclusion High impulsivity is a risk factor for manic episodes of bipolar I disorder and may be associated with cognitive impairment in patients with ma-nic episodes of bipolar I disorder.
ABSTRACT
Objective To investigate the comprehensive effect and impact path of family environment, rearing style and personality on adolescents with mental disorder. Methods A sample of 200 adolescents with mental disorders and 228 normal adolescents were tested with Family Environment Scale (FESCV),Family Upbringing Style Questionnaire (FUSO),Big Five Personality Questionnaire (NEO-FFI-R) and Psychological Symptom Inventory-100(PSI-I00).Regression analysis and path analysis were used to explore influence and path of rearing style, family environment and personality on mental health. Results The total score PSI-100 in case group (204.0±68.6) was higher than that in control group (146.9±38.4)(t=10.43,P<0.01).Regression analysis revealed that mental health was affected by rearing style (R2 =0.208) such as mother supervision-control (β =-0.249),father understands-blame (β =-0.195),mother warmth-wildness (β=-0.228) and mother acceptance-rejection (β=0.201),family environment (R2 =0.244) such as conflict (β=0.309),organization (β=-0.196) and expressiveness (β=-0.141),and personality traits (R2 =0.452)such as neuroticism (β=0.412),agreeableness (β=-0.237) and conscientiousness (β=-0.203).Path Analysis showed that personality traits directly affected mental health (β=-0.90),parenting style indirectly influenced mental health (β=0.51) by personality, the family environment had direct (β=0.20) and indirect (β=0.72) effects on mental health, and the model fits well (x2/df =2.160,RMSEA =0.052,GFI =0.88,AGFI =0.84,PGFI =0.67,NFI =0.91,RFI =0.89,IFI =0.95,TLI =0.94,CFI =0.95),and three factors explained 61% of variation on symptom total score. Conclusion The family environment, family upbringing style and personality traits have significant effect on the adolescent's mental disorder. The personality traits are the most important variables affecting mental health family upbringing style affect mental health indirectly through personality traits, and family environment directly and indirectly affected on the mental health.
ABSTRACT
Objective To explore the memory function and the effects of sleep on memory consolidation in patients with stable paranoid schizophrenia.Method Sixty patients with stable paranoid schizophrenia were randomly assigned into research group (n =30) and control group (n =30).30 healthy people were raised as a healthy control group. The polysomnography (PSG) was used for 10 hours of sleep monitoring in the research group and the healthy control group and the muhiple memory assessment scale(MMAS) was used to test the subjects memory before and after the sleep monitoring.Control group performed memory tests in equal length daytime.Pre-test and post-memory measurements were compared. Results The prememory test of three groups had significant statistical differences (F(2.87) =31.40,39.89;P<0.01).Both the auditory verbal/picture memory conservation rate and recognition accuracy rate between research group and healthy controls had no statistically significance differences (P>0.05).But compared with the control group, auditory verbal memory conservation rate ((62.64±9.06) % vs (43.00±9.73) %,P<0.01),recognition accuracy rate ((69.11±11.04)% vs (61.78±11.67)%,P<0.05),picture memory conservation rate((71.20±14.95) % vs (58.72 ± 16.19) %,P< 0.01),recognition accuracy rate ((86.89 ± 8.02) % vs (78.89 ±12.63) %,P<0.01) of the research group showed statistical significance differences (P<0.05).Both N2 (r=0.377,P<0.05) and REM (r=0.436,P<0.05) had positive correlation with recognition accuracy rate of auditory verbal memory in research group. Conclusion Stable paranoid schizophrenic patients have poorer memory function(auditory verbal words memory recall, picture memory recall) than healthy people.Sleep promotes the consolidation of memory in patients with schizophrenia. Both N2 and REM of patients with stable paranoid schizophrenic have positive correlation with recognition accuracy rate of auditory verbal memory.

Objective To explore the cognition ,attitude and intention of seeking doctor on hierarchical diagnosis and treat-ment among the patients suffering from chronic diseases in Chongqing City .Methods A multistage stratified sampling method was adopted to investigate 1125 patients with chronic diseases extracted from 9 public hospitals as well as basic medical and health in-stitutions in 9 districts and counties of Chongqing City .Meanwhile ,the differences of intentions of seeking doctor and cognition situ-ation of hierarchical diagnosis and treatment were analyzed .Results The patients with chronic diseases in basic medical and health institutions were more likely to choose the basic medical and health institutions for seeking doctor than the patients treated in public hospitals ,the difference was statistically significant (χ2 = 190 .051 ,P<0 .01);31 .2% of the patients with chronic diseases never contacted the hierarchical diagnosis and treatment .The technical capabilities of basic medical and health institutions meeting treat-ment needs ,gradual improvement of medical condition and service level and trending to seeking doctor in basic medical and health institutions had statistical difference between the patients in public hospitals and patients in basic medical and health institutions (P<0 .01) .The aspects of understanding the current referral system between medical institutions and downward referral conducing the disease to obtain continuous long tern treatment had statistical difference between the patients in public hospitals and patients in basic medical and health institutions (P< 0 .01) .Conclusion In order to better carry out hierarchical diagnosis and treatment in Chongqing City ,it is necessary to perfect the supporting polices of hierarchical diagnosis and treatment for guiding the primary diag-nosis in the patients with chronic disease ,meanwhile establish an efficient dual referral mechanism and enlarge the propaganda of hi-erarchical diagnosis and treatment policy .
ABSTRACT
Objective To describe the cardiovascular risk profile of inpatients with schizophrenia and estimate the 10-year CVD risk in schizophrenia patients. Methods Four hundred sixty-six randomly selected cases of schizophrenia patients and 507 health controls were included in the study. Body mass index (BMI), Fasting blood glucose (FBG), triglyceride (TG),,total cholesterol (TC), smoking rate (smoking people/total people), Framing-ham Risk Score (FRS), high density lipoprotein-cholesterol (HDL-C) and age of the vessel (VA). The 10-year cardiovascular risk(FRS) and age of the vessel (VA) were assessed using the Framing-ham Risk Score. Results 10-year CVD risk was significantly higher in patients with schizophrenia than in control group [(6.71 ±6.95)% vs. (4.76 ±3.07)%], (P<0.05). Comparing with the control group, smoking prevalence[(41%) vs. (19%)], MS[(25%) vs. (17%)], BMI[(23.67 ±3.61)kg/m2 vs. (21.73±4.11)kg/m2] and VA[(46.7±15.3) vs. (43.7±9.8)] were higher in patients group. SBP[(119.86±14.90)mmHg vs. (128.10±15.41)mmHg] and HDL-C [(1.08±0.27)mmol/L vs. (1.38±0.22)mmol/L] were lower in patients group than in the healthy controls. The FRS score of the patient group was positively correlated with BMI, FBG level and SBP, waist circumference and smoking index (P<0.05) and was negatively correlated with the levels of HDL-C. Multivariate Linear Regression analysis demonstrated that FRS was correlated with FBG (β=0.181,P<0.01)、SBP (β=0.149,P<0.01)、HDL-C (β=-0.107,P<0.01) and smoking index(β=0.554,P<0.01). Conclusion The risk of cardiovascular disease in patients with schizophrenia over the next 10 years is 6.7%, which is significantly higher compared with the control group. Fasting blood glucose, systolic blood pressure, high density lipoprotein, smoking may be associated with 10-year CVD risk of schizophrenia patients.

Objective To explore the status of college students’ attachment and locus of control, and explore their relationship. Methods A total of 198 college students were surveyed adopting Experiences in Close Relationships Inventory and Internality, Powerful Others and Chance Questionnaire. Results The distribution of college student′s attachment type was not balanced, the proportion of dismissing type, security type, fearful type, preoccupied type accounting for 43. 9%,31. 3%,21. 7%,3. 1% respectively. The scores of internality, chance and powerful others were (27. 59 ± 5. 44),(17. 92 ± 6. 66),(17. 70 ± 7. 32) respectively. Attachment avoidance correlated negatively with internality(r= -0. 259,P<0. 01), and positively with chance and powerful others(r=0. 206,0. 221,P<0. 01);besides there was a positive correlation between attachment anxiety and chance, powerful others(r=0. 378, 0. 410,P<0. 01). Attachment avoidance had prediction on internality(R2 =0. 067,P<0. 01). Attachment anxiety had prediction on chance and powerful others(R2 =0. 143,0. 168,P<0. 01). Conclusion College students’ at-tachment has significant effects on locus of control.
ABSTRACT
Objective To develop a Quick Cognitive Screening Scale for Elder(QCSS-E) and examine its reliability and validity. Methods In the light of rating model of MMSE and MoCA,a screening scale was developed to assess cognitive function, which contained 47 items across twelve domains.Five hundred community subjects were interviewed with the QCSS-E,then 150 subjects were retested after two weeks, and 75 subjects were assessed with MMSE and CCAS.The test-retest reliability, internal consistency reliability, empirical validity and constmct validity of QCSS-E were calculated. Results The test-retest reliability and Cronbach's α coefficient were 0.972 and 0.814 for QCSS-E total score,0.656-0.911 and 0.679-0.746 for domain scores. There were low to moderate correlation of 0.133-0.612 among domain scores, correlation domain scores with total score ranged from 0.303-0.664.QCSS-E scores decreased with increasing agc, increased with the level of education increasing, and decreased with the aggravation of brain damage. The QCSS-E was significantly correlated with MMSE(r=0.886) and CCAS (r=0.899).Factor analysis got two factors accounting for 49.4% of variance, which were named vulnerability factor and resistance to damage factor. Conclusion The stability, internal consistency, and validity of the QCSS-E are good and meet with psychometric standard.
