ABSTRACT
BACKGROUND:With the variation of disease treatment modes and the in-depth research on senile osteoporosis in recent years,increasing studies have confirmed that traditional Chinese medicine has a significant effect on the prevention and treatment of senile osteoporosis. OBJECTIVE:To investigate the effect of Yishen Gushu Formula on bone metabolic markers in patients with osteoporosis of kidney deficiency and blood stasis type. METHODS:102 patients with senile osteoporosis of kidney deficiency and blood stasis type who were treated at Ruikang Hospital Affiliated to Guangxi University of Traditional Chinese Medicine from July 2020 to March 2022 were enrolled,including 32 males and 70 females,aged 71-93 years.All patients were randomly divided into two groups,with 51 patients in each group.The control group was treated with calcium carbonate D3 granules and sodium alendronate tablets,while the treatment group was treated with Yishen Gushu Formula beyond the control group.Treatments in each group lasted 3 months.Bone mineral density of the L1-4 lumbar vertebrae and left femoral neck,visual analog scale score,and serum levels of osteocalcin,osteopontin,type Ⅰ collagen cross-linked C-terminal peptide and tartrate resistant acid phosphatase were measured before and 3 months after treatment.Traditional Chinese medicine syndrome score and therapeutic efficiency were also assessed. RESULTS AND CONCLUSION:After 3 months of treatment,the bone mineral density of the lumbar vertebrae(L1-4)and left femoral neck was significantly increased in both two groups(P<0.05),and the bone mineral density of the lumbar vertebrae(L1-4)and left femoral neck was significantly higher in the treatment group than the control group(P<0.05).The visual analog scale scores of both groups after 3 months of treatment were lower than those before treatment(P<0.05),and the visual analog scores of the treatment group after 3 months of treatment were lower than those of the control group(P<0.05).After 3 months of treatment,the serum levels of osteocalcin,osteopontin,type Ⅰ collagen cross-linked C-terminal peptide and tartrate resistant acid phosphatase were significantly improved in both two groups,while compared with the control group,the serum levels of osteocalcin and osteopontin were significantly higher(P<0.05)and the serum levels of type Ⅰ collagen cross-linked C-terminal peptide and tartrate resistant acid phosphatase were significantly lower in the treatment group(P<0.05).After 3 months of treatment,the Traditional Chinese medicine syndrome scores were decreased in both two groups,while the Traditional Chinese medicine syndrome scores in the treatment group were lower than those in the control group.After 3 months of treatment,no significant adverse reactions occurred in both groups.The total effective rate was 88.2%and 70.6%in the treatment and control groups respectively,and there was a significant difference between the two groups(P<0.05).To conclude,Yishen Gushu Formula combined with anti-osteoporosis drugs can significantly improve the clinical symptoms of patients with senile osteoporosis of kidney deficiency and blood stasis type and prevent disease progression by regulating bone metabolism,increasing bone mineral density,and relieving pain.

BACKGROUND:Knee osteoarthritis is a degenerative disease caused by multiple factors.Its pathogenesis is complex and still unclear.Chinese medicine in the treatment of knee osteoarthritis is fruitful,and in-depth study of Chinese medicine in the treatment of knee osteoarthritis is of great significance. OBJECTIVE:To review the progress of Chinese medicine monomers and compounds in the treatment of knee osteoarthritis and to provide ideas and reference for the effective prevention and treatment of knee osteoarthritis. METHODS:CNKI,WanFang,VIP,PubMed,MEDLINE,Nature,and Cochrane databases were retrieved for relevant literature published from database inception to 2022.The keywords were"knee osteoarthritis,cartilage damage,traditional Chinese medicine,Chinese herbal compound,treatment"in Chinese and English.Duplicates and obsolete non-referenced literature were excluded,and a total of 62 standard papers were included for further review. RESULTS AND CONCLUSION:Some of the pathogeneses of knee osteoarthritis include immune inflammatory response,chondrocyte autophagy and apoptosis,vascular endothelial growth factor level and biomechanical imbalance.The mechanisms by which traditional Chinese medicine treats knee osteoarthritis mainly focus on regulating inflammatory factor levels,chondrocyte autophagy and apoptosis,and vascular endothelial growth factor level and improving cartilage performance,so as to delay the occurrence and development of knee osteoarthritis.

BACKGROUND:Studies have found that chondrocyte ferroptosis can be used as an important pathogenesis to induce the occurrence and development of osteoarthritis.With the continuous development of Chinese medicine,some scholars have found that some traditional Chinese medicine monomers and traditional Chinese medicine compounds can inhibit chondrocyte ferroptosis through various mechanisms of action,and finally play a role in the treatment of osteoarthritis. OBJECTIVE:To explore the relationship between ferroptosis and osteoarthritis and the mechanism by which traditional Chinese medicine deals with ferroptosis in the treatment of osteoarthritis,in order to provide new ideas for the treatment and prevention of osteoarthritis. METHODS:With"osteoarthritis,ferroptosis,chondrocytes,lipid metabolism,traditional Chinese medicine,ROS,GPX 4"as the English and Chinese search terms,CNKI,PubMed,WanFang,and VIP databases were searched for relevant articles regarding the relationship between ferroptosis and osteoarthritis and its traditional Chinese medicine intervention until 2022.Then,a systematic analysis was conducted. RESULTS AND CONCLUSION:Intracellular iron overload will cause lipid peroxidation,resulting in the accumulation of reactive oxygen species and eventually the occurrence of ferroptosis,while excessive iron will accelerate the apoptosis of chondrocytes and affect chondrocyte homeostasis,thereby inducing the development of osteoarthritis.When amino acid metabolism is abnormal,an imbalance in glutamate and cystine exchange causes the accumulation of glutamate outside the cells,and high extracellular concentration of glutamate will cause the depletion of glutathione,indirectly inhibiting the function of ferroptosis-related factors,and contributing to ferroptosis.When cellular lipid metabolism is abnormal,unsaturated fatty acids,an important component of the cell membrane,lose their diallyl hydrogen atoms,leading to the destruction of cells to induce ferroptosis,while lipid metabolism is closely involved in the progression of osteoarthritis.Traditional Chinese medicine can play a role in the prevention and treatment of osteoarthritis by inducing the occurrence of ferroptosis.The research on iron death is still immature,and further research on its action mechanism is expected to open up new ways for the clinical treatment of osteoarthritis.

Objective:To investigate the efficacy and safety of acute stent implantation during endovascular treatment for patients with emergent large vessel occlusion due to intracranial atherosclerotic stenosis.Methods:A retrospective analysis was carried out on 46 patients with emergent large vessel occlusion due to intracranial atherosclerotic stenosis who received endovascular treatment at the Strategic Support Force Medical Center from January 2015 to August 2022. Twenty-seven patients underwent balloon angioplasty alone and 19 patients underwent acute stent implantation. The baseline characteristics, modified thrombolysis in cerebral infarction (mTICI) score of the responsible vessels, modified Rankin scale (mRS) score 90 days after operation, incidence of symptomatic intracranial hemorrhage and mortality of the two groups were evaluated.Results:The proportion of effective recanalization of the offending vessels (mTICI≥2b) in the acute stenting group was slightly higher than that in the balloon angioplasty group (16/19 vs. 81.5%), but the difference was not statistically significant ( P>0.05). Besides, there was no significant difference in the median of mRS between the acute stenting group [3.0(0, 4.0)] and the balloon angioplasty group [4.0(1.0, 5.0)] 90 days after operation ( P>0.05). In terms of safety, the incidence of symptomatic intracranial hemorrhage and mortality were comparable between the two groups ( P>0.05). Conclusions:The effect of acute stent implantation during endovascular treatment for patients with emergent large vessel occlusion due to intracranial atherosclerotic stenosis is not inferior to that of balloon angioplasty, and it does not increase the risk of intracranial bleeding complications.

Objective:To analyze identification of copper death gene related subtypes,construction of prognosis model and influence of immune infiltration in osteosarcoma(OS)on basis of copper death gene.Methods:Survival and prognosis of OS associated copper death gene were analyzed combining by TARGET and GEO database.OS was divided into different subtypes of copper death by consistent clustering method.SSGSEA was used to analyze difference of immune cells in classification of copper death.Setting P value= 0.05 and q value=0.05,GO and KEGG enrichment analysis were performed on differential genes of copper death typing.Prognosis model was constructed according to results of Lasso regression analysis and cross validation,risk assessment analysis and ROC curve were used to evaluate accuracy of model prediction.Combined with clinical characteristics,nomograms were constructed to predict survival time of patients,and risk differences were analyzed.Immune cell infiltration and tumor microenvironment analysis were performed on OS samples."pRRophetic"package in R software was used to analyze drug sensitivity of OS samples.Results:FDX1,GLS,DLAT and PDHB as high-risk genes for OS prognosis were identified.According to copper death classification of OS samples,OS could be divided into two types:CRGclusterA and CRGclusterB.CRGclusterA was associated with Th2 cells,and CRGclusterB was associated with Th1 cells.Most OS copper death genes were highly expressed in CRGclusterA.Immune cell infiltration analysis results showed that γδ T cells,resting mast cells and resting dendritic cells were positively correlated with risk score,while CD8 T cells were negatively correlated with risk score.Drug sensitivity analysis showed that OS showed higher sensitivity to Elesclomol and GW.441756.Conclusion:Two subtypes of CRGclusterA and CRGclusterB are identified in this study.Four high-risk prognostic genes FDX1,GLS,DLAT and PDHB are identified,providing new insights into prognostic evaluation and immunotherapy target candidates for OS.

Objective:To study the differential gene expression and immune cell infiltration of gout patients,to find the key genes and immune cells of gout pathogenesis,and to explore the relationship between immune cells and gout.Methods:The gout chip GSE160170 was downloaded from the GEO database,and the differential gene expression analysis was carried out with the help of R language.Then,the STRING database was used to analyze the differential gene,and the Cytoscape software was used to screen the key genes,and then carry out enrichment analysis.At the same time,the infiltration of immune cells were analyzed.Results:The study found that IL-6,IL-1β,TNF,CCL3,CXCL8 and CXCL1 were key genes in the pathogenesis of gout,which were mainly exerted by IL-17,Toll-like receptor,NOD-like receptor,NF-κB and other signaling pathways.Processes such as cellular responses to lipo-polysaccharides,bacteria-derived molecules,and biological stimuli lead to disease;immune infiltration results indicate that memory B cells,activated NK cells,activated dendritic cells,activated mast cells and eosinophils were involved in the disease.It was signifi-cantly expressed in gout patients;the correlation analysis between immune cells showed that the expression of follicular helper T cells were positively correlated with the expression of activated mast cells,and the expression of unactivated NK cells and monocyte were negatively correlated.Conclusion:Key genes and differentially expressed immune cells are closely related to the pathogenesis of gout,providing new ideas for the study of the molecular mechanism of gout.

Objective:To explore the molecular mechanism of Danggui Niantong Decoction in the treatment of gouty arthritis (GA) based on network pharmacology and molecular docking.Methods:By selecting for the active components and targets of Danggui Niantong Decoction with TCMSP, and retrieving the GeneCards, OMIM, PharmGKB and DrugBank databases to obtain GA related targets. The potential targets of Danggui Niantong Decoction in the treatment of GA were obtained by the intersection of mappings. The regulation network of Chinese medicine compound and protein-protein interaction network of Danggui Niantong Decoction were constructed by Cytoscape software, and the targets of Danggui Niantong Decoction in the treatment of GA were analyzed by GO and KEGG enrichment by David Database. Finally, molecular docking was performed by using Autodock software.Results:There are 198 active components that could treat GA in Danggui Niantong Decoction. The key active components are Quercetin and Kaempferol. There are 46 key targets, the core targets are NFE2L2, HMOX1, PPARA, PTGS2, IL1β, CXCL8. GO enrichment suggests that the key genes are primarily involved in many biological processes such as Inflammatory response regulation, response to oxidative stress, Fatty acid metabolism process, steroid metabolism, lipopolysaccharide response and reactive oxygen species metabolism. KEGG pathway indicates that Danggui Niantong Decoction mainly acted on IL-17 signal pathway, HIF-1 signal pathway, TNF signal pathway and AGE-RAGE signal pathway. Molecular docking shows that the active components of Danggui Niantong Decoction and action target of GA can combine toghether with high efficiency, and the structure is stable.Conclusion:Danggui Niantong Decoction has multi-component, multi pathway and multi-protein characteristics. Danggui Niantong Decoction can treat GA by regulating immune inflammatory reaction and oxidative stress reaction.

Osteoporosis (OP) is a systemic metabolic bone disease caused by various factors, with a high incidence, and its pathogenesis is not yet clear. There is no specific drug for prevention and treatment, making it a significant global public health issue. In recent years, research has found that autophagy plays a role in the development of OP, and intervention in autophagy has become a hot topic in OP treatment. With further research, there has been a gradual increase in studies related to autophagy regulation by traditional Chinese medicine (TCM) to treat OP, and the treatment efficacy has been recognized. However, there is still a lack of systematic reviews on the mechanisms of autophagy in OP and the specific targets of TCM intervention in autophagy for OP treatment. Therefore, this article systematically reviewed the impact of autophagy on bone marrow mesenchymal stem cells (BMSCs), osteoblasts, osteoclasts, and bone cells in the development of OP, as well as studies on TCM intervention in cell autophagy for OP treatment, aiming to provide a theoretical reference for the treatment of OP and further research in this field.

With the development of modern sequencing techniques and bioinformatics, genomes that were once thought to be noncoding have been found to encode abundant functional micropeptides (miPs), a kind of small polypeptides. Although miPs are difficult to analyze and identify, a number of studies have begun to focus on them. More and more miPs have been revealed as essential for energy metabolism homeostasis, immune regulation, and tumor growth and development. Many reports have shown that miPs are especially essential for regulating glucose and lipid metabolism and regulating mitochondrial function. MiPs are also involved in the progression of related diseases. This paper reviews the sources and identification of miPs, as well as the functional significance of miPs for metabolism-related diseases, with the aim of revealing their potential clinical applications.

Objective To investigate the clinical diagnostic value of anti-cyclic citrullinated peptide(CCP)antibody,anti-mutated citrullinated vimentin(MCV)antibody and 25-hydroxyvitamin D[25-(OH)D]in rheumatoid arthritis(RA)patients.Methods A total of 466 patients with RA who were admitted the Affiliated Huai'an No.1 People's Hospital of Nanjing Medical University from August 2016 to November 2021 were included in RA group,100 patients with other autoimmune diseases and metabolic diseases without RA were included in non-RA group,100 healthy subjects were included in control group.Serum anti-CCP antibody,anti-MCV antibody and 25(OH)D were detected in three groups.Receiver operating characteristic curve was drawn to analyze the diagnostic efficacy of anti-CCP antibody,anti-MCV antibody and 25(OH)D in RA.The correlation between anti-MCV antibody and clinical indexes in RA patients was analyzed.Results Serum anti-MCV antibody and anti-CCP antibody in RA group were significantly higher than those in non-RA group and control group(P<0.05).The serum 25(OH)D level in RA group was significantly lower than that in control group(P<0.05).The area under the curve(AUC)of anti-CCP antibody,anti-MCV antibody and 25(OH)D for the diagnosis of RA were 0.74,0.81 and 0.75,respectively.The AUC for the combined diagnosis of RA was 0.90,the sensitivity was 83.70%,and the specificity was 82.40%.Anti-MCV antibody was positively correlated with erythrocyte sedimentation rate and C-reactive proten(r=0.66,0.64,P<0.05),and negatively correlated with complement C3 and C4(r=-0.69,-0.62,P<0.05).Conclusion Anti-CCP antibody,anti-MCV antibody and 25(OH)D have good sensitivity and specificity for the diagnosis of RA,and anti-MCV antibody was helpful for the diagnosis of the disease activity.

Objective:To investigate the clinical application value of magnetically controlled capsule endoscopy (MCCE) for risk assessment of upper gastrointestinal bleeding in elderly patients taking enteric-coated aspirin.Methods:Clinical data of elderly patients taking enteric-coated aspirin and undergoing MCCE from January 2018 to December 2020 in Beijing Chaoyang and Beijing Anzhen Hospital, Capital Medical University were analyzed. Patients were divided into low-risk group (scores ≤ 3) and moderate/high risk group (scores >3) to study the risk assessment of upper gastrointestinal bleeding in elderly patients taking enteric-coated aspirin.Results:A total of 66 patients (aged 60-81 years, 45 males and 21 females) were enrolled and 17 patients developed bleeding. The indicators of low-risk ( n=51) and moderate/high risk groups ( n=15) were as follows: the incidences of upper gastrointestinal bleeding were 17.6% and 53.3%( P<0.001), gastric ulcer 5.9% and 26.7% ( P<0.001), median gastric Lanza score 2.0 and 2.0( P=0.621), duodenal ulcer 2.0% and 18.8% ( P<0.001), median duodenal mucosal injury score 1.0 and 1.0( P=0.936), respectively. Receiver operator characteristic curve showed that the area under the curve of risk assessment of upper gastrointestinal bleeding in elderly patients taking enteric-coated aspirin by MCCE was 0.855. Conclusion:Risk assessment of upper gastrointestinal bleeding in elderly patients taking enteric-coated aspirin can be used to predict the risk of upper gastrointestinal bleeding, but the scoring rules need to be further improved. Moderate and high-risk patients should undergo MCCE to monitor aspirin related upper gastrointestinal mucosal injury.

In China, hepatocellular carcinoma (HCC) accounts for about 50% of cases in the world, which is still challenging. The implementation of multi-disciplinary treatment (MDT) and whole course management of HCC are conducive to improve the prognosis of patients, so as to achieve the goal of increasing by 15% of overall survival of HCC patients as proposed by the "Healthy China 2030" . The standardized management of HCC includes several key points, such as exact diagnosis of the HCC, selection of proper treatment methods, individual postoperative adjuvant therapy and regular follow-up. MDT is the key measure to implement the standardized and whole course management of HCC. Herein, the author tends to summary the experience on standardized management of HCC.

BACKGROUND: Percutanoues pedicle screw internal fixation is widely used for treating lumbar disease. Because the surgery is guided by the C-arm, it is hard to control the position between screws and facet joint to cause the facet joint violation completely. Body mass index > 29.9 kg/m2, facet joint angle > 35°and age < 65 years old are considered as the high risk factors for percutaneous pedicle screw injury of articular processes. Due to the different positional relationship between the pedicle axis of the lumbar spine and the articular process joint, there is no related report on whether the destruction of the articular process joint by screws is different. OBJECTIVE: To investigate the significance of lumbar CT multi-planar reconstruction for preestimating facet joint violation in lumbar percutaneous pedicle screw placement. METHODS: One hundred patients with lumbar fracture and degenerative disease who underwent percutaneous pedicle screw placement were selected. All patients signed the informed consents and the study was approved by the ethics committee of the hospital. The position line of the oblique axial view, oblique sagittal view and oblique coronal view was adjusted to reconstruct the section of pedicle. A circle with the 6.5 mm of position line of the oblique coronal view as its center to check the relationship between the circle and facet joint served as preoperation group. CT examination was performed after surgery for evaluating the degree of facet joint violation by screws and served as postoperation group. The amount of screws which damage the facet joint of different vertebrae between two groups and the damage rate among vertebrae in each group were compared. RESULTS AND CONCLUSION: (1) Totally 478 screws were placed, 109 (22.8%) screws violated facet joints in the preoperation group, and the amount of circles which violated facet joint was 6 (6.8%), 9 (10.5%), 19 (18.3%), 30 (30.0%), and 45 (45.0%) from L1-L 5. (2) 115 (24.1%) screws violated facet joints in the postoperation group, the amount of circles which violated facet joint was 10 (11.4%), 7 (8.1%), 15 (14.4%), 26 (26.0%) and 41 (41.0%) from L1-L 5. (3) The P values of McNamara test of different vertebrae were 0.08 (L1), 0.22 (L2), 0.20 (L3), 0.05 (L4) and 0.08 (L5). There was no significant difference between two groups. (4) The results of Kappa test were 0.67 (L1), 0.80 (L2), 0.80 (L3), 0.87 (L4) and 0.92 (L5). The damage rate among vertebrae in each group had significant differences (P=0.000). (5) To conclude, the relationship between percutanoues pedicle screw and facet joint could be recognized by lumbar CT multiple level reconstruction, which provides a reliable method for preestimating the possibility of facet joint damage caused by the percutanoues pedicle screw.

BACKGROUND: Silk fibroin and chitosan are commonly used as scaffolds In tissue engineering, but there are some shortcomings In their separate application. When they are mixed, they can be modified each other. They give full play to each other’s advantages and become Ideal composite scaffolds. OBJECTIVE: To prepare Silk fibroin/chitosan composite scaffold and determineits properties. METHODS: The silk fibroin/chitosan composite scaffolds were prepared by freeze-drying method. The morphology and structure of the composite scaffolds were examined by scanning electron microscopy. The properties of the composite scaffolds were tested by thermogravlmetric analysis, mechanical properties test, and cytotoxicity test. The quaternion chitosan was prepared. The nuclear magnetic resonance spectrum was detected by nuclear magnetic resonance instrument. The potential and particle size distribution were detected by Zeta potentiometer. The protection of DNA was detected by gel electrophoresis. The binding with DNA was observed by transmission electron microscope. RESULTS AND CONCLUSION: (1 ) The results of scanning electron microscopy showed that the silk fibroin/chitosan composite scaffolds had a good three-dimensional pore structure, with a pore size of 50-100 urn. (2) The results of thermogravimetric analysis showed that when the temperature was less than 200 °C, the mass loss rate of silk fibroin/chitosan composite scaffold was lower. When the temperature Increased to 200-500 °C, the mass loss rate of the scaffold began to accelerate, and the loss amount increased. At 800 °C, the residual mass of the composite scaffold was 38%. (3) The maximum strain of silk fibroin/chitosan composite scaffold reached 94.94%, and the maximum stress was 7.01 MPa. (4) The results of CCK-8 experiment showed that silk fibroin/chitosan composite scaffolds had no cytotoxicity to rabbit bone marrow mesenchymal stem cells and had good cell compatibility. (5) The results of nuclear magnetic resonance spectra showed that the quaternion degree of quaternary ammonium chitosan was about 20%. (6) The particle size distribution of quaternized chitosan was (588.56±52.39) nm, and the surface of quaternized chitosan was positively charged with a potential of (16.3±3.92) mV, which was beneficial to the combination with DNA. (7) The results of gel electrophoresis experiments showed that the higher the proportion of quaternion chitosan, the better the encapsulation of DNA. When the ratio of chitosan/DNA was 1 : 3, the encapsulation effect was achieved. (8) The results of transmission electron microscopy showed that most of the particles of quaternized chitosan/DNA were solid and round; the particle size difference was small; and the average particle size was about 200 nm. (9) The results showed that silk fibroin/chitosan composite scaffolds had a good biocompatibility and cell permeability, which was conducive to the growth of cells between scaffolds.

Objective@#To investigate the clinicopathologic features and possible causes of granulomatous lobular mastitis(GLM).@*Methods@#Three hundred cases of GLM were collected from surgical specimens diagnosed at Longhua Hospital, Shanghai University of Traditional Chinese Medicine from January 2015 to November 2017. Morphologic features were reviewed using HE staining. A total of 116 cases were investigated by Gram staining. The expression of CD3, CD20, CD68, IgG, IgG4, CD38 and CD138 was detected by immunohistochemical staining.@*Results@#The age of the patients was 23 to 47 years and the median age was 32 years. All patients were female, 96.7% (290/300) had a history of lactation.There were 143 cases of left breasts, 138 cases of right breast and 19 cases of bilateral breasts. Serum prolactin increased in 39.7%(119/300) patients. Within 15.7%(47/300) of patients were associated with nodular erythema or joint swelling and pain of the lower extremities. Pathological observation showed that lobular-centric suppurative granulomatous inflammation, accompanied by dilatation of intralobular and interlobular ducts. There were 16 cases accompanied with duct ectasia. Immunohistochemistry showed CD3-positive lymphocytes were more than CD20-positive lymphocytes in the peripheral aggregation zone of neutrophils within granulomatous lesions. Gram positive bacteria were found in the lipid vacuoles of the 51.7%(60/116) patients.@*Conclusions@#GLM has distinctive histologic features. It may be related to corynebacterium infection, or accompanied by the increase of serum prolactin and erythrocyte sedimentation rate. The age, location and history of the disease are importance in the diagnosis and differential diagnosis.

Objective@#To explore the nursing experience of a child with multiple organ dysfunction syndrome caused by physical abuse and severe soft tissue injury.@*Methods@#In this case, multiple soft tissue injuries and multiple organ dysfunction syndrome were caused by domestic violence abuse. Nursing included strengthening artificial airway nursing, continuous blood purification nursing, skin injury nursing, sedation and analgesia management, nutritional support nursing, psychological nursing and family education.@*Results@#After careful treatment and nursing, the child′s condition improved and was transferred to the general ward for further treatment and discharged.@*Conclusions@#The nursing of multiple soft tissue injuries and multiple organ dysfunction syndrome includes strengthening artificial airway nursing, continuous blood purification nursing, skin injury nursing, sedation and analgesia management, nutritional support nursing, psychological nursing and family education.
ABSTRACT
Objective To analyze the distribution of traditional Chinese medicine (TCM) syndromes and characteristics of patients with acute respiratory distress syndrome (ARDS), and to provide reference for further clinical study of ARDS. Methods The clinical data of ARDS patients admitted to the Department of Critical Care Medicine of Affiliated Hospital of Chengdu University of TCM from November 2017 to February 2019 were retrospectively analyzed. The general data (gender and age) and inducing cause of disease, clinical manifestations, intensive care unit (ICU) stay time, total hospitalization time, outcome in ICU, outcome in 28-day follow-up and TCM syndromes were recorded. The different degrees of disease severity, the clinical manifestations and distribution characteristics of TCM syndromes were analyzed. Results A total of 110 patients with ARDS were enrolled, including 2 patients in mild ARDS group, 33 patients in moderate ARDS group and 75 patients in severe ARDS group. In the etiology analysis of ARDS, infection was the commonest cause, including 46 cases of pulmonary infection (41.8%), 27 cases of sepsis (24.5%), 4 cases of abdominal cavity infection (3.6%), 2 case of urinary tract infection (1.9%), and 13 cases had surgical histories (11.8%). The stay in ICU was 9.00 (3.00, 18.00) days and the total hospital stay was 18.00 (10.00, 30.00) days. The mortality in ICU was 32.7% (36/110), and that in 28-day follow-up was 60.0% (66/110). The clinical symptoms of the patients in moderate and severe groups ARDS were mainly dyspnea, cough, fever, sputum, chest tightness, and palpitations, accounting for 75.0% (81/108), 43.5% (47/108), 28.1% (31/108), 26.9% (29/108), 22.2% (24/108) and 25.9% (28/108), respectively. Compared with the ARDS moderate group, the proportions of patients with dyspnea, cough and palpitation in severe ARDS group were significantly higher [80.0% (60/75) vs. 63.6% (21/33), 50.7% (38/75) vs. 27.3% (9/33), 33.3% (25/75) vs. 9.1% (3/33), respectively, all P < 0.05]. The main TCM syndromes were yang deficiency, exuberant heat-toxin, and wind-heat invading lung, accounting for 53.7% (58/108), 28.7% (31/108), and 25.0% (27/108) respectively. The proportion of patients with exuberant heat-toxin syndrome in severe ARDS group was obviously higher than that in the moderate ARDS group [34.7% (26/75) vs. 15.2% (5/33), P < 0.05], while the proportion of patients with wind-heat invading lung syndrome in moderate ARDS group was more than that in the severe ARDS group [42.4% (14/33) vs. 17.3% (13/75), P < 0.05]. Conclusion ARDS is a critical illness with high mortality and various complicated clinical symptoms, the TCM syndromes of ARDS are mainly yang deficiency, exuberant heat-toxin, wind-heat invading lung, and intermingling of deficiency and excess easily leading to collapse syndrome.
ABSTRACT
Objective To summarize the characteristics of IgG4 related disease (IgG4-RD),and to avoid unnecessary surgical procedures due to misdiagnosis.Methods Retrospective analysis of the clinical data of one case of IgG4 related disease involving the renal pelvis in our hospital,and the clinical features of IgG4 related disease involving the renal pelvis were reviewed and discussed.A 56-year-old man presented with microscopic hematuria,CT showed right renal pelvis and ureteral wall thickening,local soft tissue density,post contrast CT showed low-density renal pelvic mass and the wall of the ureter-pelvic was irregularly thicken,indicating renal pelvic cancer and lymph node metastasis.PET-CT (18F-FDG) findings indicated that the renal pelvic mass was a malignant tumor,because the glucose metabolism was very high.The preoperative diagnosis was retroperitoneal lesion,suspecting renal pelvic carcinoma or lymphoma.The patient underwent 3D laparoscopic nephroureterectomy with out preoperative biopsy.During operation,we found extensive enlargement of the right renal portal and retroperitoneal lymph nodes,including that the renal portal and the renal pelvis mass had a serious adhesion to the vena cava and renal arterial vein.Results Postoperative pathology results revealed lymphatic tissue hyperplasia and germinal center formation.Immunohistochemical staining showed that most plasma cells positive expression of IgG4 (IgG4 > 40%,IgG4 > 100/HPF),reactive hyperplasia of lymph node,considering the IgG4 related diseases.Serum IgG4 level was 15.2 g/L (0.03-2.01 g/L)after operation.Combined with serological examination,IgG4 related disease was confirmed.There was no evidence of IgG4-related disease at any other site during the follow up period,thereby obviating the need for any additional therapy.Conclusions IgG4 related disease is a novel clinical disease entity characterized by elevated serum IgG4 concentration and tumefaction or tissue infiltration by IgG4-positive plasma cells,which leads to the enlargement of the organ or nodular/proliferative lesions.The disease can be revealed as a soft tissue tumor of the renal pelvis and be misdiagnosed as malignant tumor.Surgical intervention could be avoided according to the correct diagnosis.

BACKGROUND: With the increasing number of patients undergoing knee arthroplasty in China, the complications caused by the mismatch of prosthesis with anatomical parameters arouse extensive attentions.OBJECTIVE: To measure the anatomical parameters of the knee joints in healthy Zhuang ethnic group from Guangxi Zhuang Autonomous Region, China, using three-dimensional (3D) reconstruction technology, thus providing reference for the design and placement of the hip prosthesis.METHODS: Sixty-eight individuals, including 30 females and 38 males, were selected from 217 healthy people undergoing CT examination. All data were imported into Mimics 15.0 software used for 3D reconstruction, and then the knee parameters were then measured and analyzed.RESULTS AND CONCLUSION: There was no significant difference in the parameters of the bilateral femur and tibia (P > 0.05). The femoral parameters had significant differences between genders (P 0.05), while there were significant differences in the width of medial/lateral tibial platform, and sagittal length of medial tibial platform between genders (P < 0.05). The sagittal length of medial femoral condyle was negatively correlated with age, and all femoral parameters were positively correlated with height; while only the width of medial and lateral tibial platform were positively correlated with height. To conclude, 3D reconstruction technology is available for research on the morphology of the knee joint in the Zhuang ethnic group from Guangxi Zhuang Autonomous Region, which provides references for prosthesis design and research appropriate for the Zhuang ethnic group.
