ABSTRACT

ObjectiveTo analyze the functions, formulae, dosage forms, and methods of administration of the menstruation-regulating Chinese patent medicines included in the 2020 edition of the Chinese Pharmacopoeia, so as to provide reference for rational clinical use. MethodThe relevant Chinese patent medicines were recorded one by one, and the efficacy, dosage forms, methods of administration, and contraindications were counted, classified, and summarized. Further, we analyzed the Chinese medicines used in these Chinese patent medicines, identified the high-frequency Chinese medicines for menstrual regulation, and analyzed their natures, tastes, meridian tropism, and functions, aiming to guide the clinical use. ResultA total of 142 Chinese patient medicines for menstrual disorders were included in this study. They were classified into 12 categories according to their efficacy, mainly for regulating menstruation and blood, tonifying, activating blood, and eliminating mass. The representative Chinese patent medicines were Bazhen Yimu pills, Shaofu Zhuyu pills, Lyujiao Buxue granules, and Guizhi Fuling pills, which are in line with the principles of moving Qi and blood and regulating liver and spleen. Menstruation-regulating Chinese patents medicines are mostly in pills and capsules and are mainly taken with yellow wine or ginger decoction. Pregnancy was the contraindication with the highest frequency, followed by menstruation and dietary precautions. The high-frequency Chinese medicines mainly had the functions of tonifying, activating blood, resolving stasis, and clearing heat, with the top three being Angelicae Sinensis Radix, Paeoniae Radix Alba, and Chuanxiong Rhizoma. These medicines mainly had warm nature, sweet, bitter, and pungent tastes, and tropism to liver and spleen meridians. ConclusionThe treatment of menstrual disorders should focus on nourishing and activating blood, regulating Qi, tonifying kidney, supporting spleen, nourishing liver, and harmonizing stomach. The appropriate dosage form should be selected according to the patient's specific conditions. The medicinal guide and the method of administration should be selected on the basis of syndrome differentiation with attention to the contraindications. In summary, the Chinese patient medicines for menstrual regulation should be chosen based on the patient’s syndrome under guidance of the theory of traditional Chinese medicine.

ABSTRACT

Ovarian cancer is a heterogeneous malignant tumor, one of the most common causes of cancer death in women worldwide, and there is currently no effective treatment available for its treatment, which seriously affects women's life and health. In order to deeply explore the pathogenesis of ovarian cancer and study the specific therapeutic drugs and methods, this paper summarizes, summarizes, assigns and evaluates the clinical characteristics of traditional Chinese and Western medicine for ovarian cancer, and screens out a more complete animal model.Through CNKI,Wanfang,Pubmed and other databases, the relevant content of ovarian cancer animal models was collected and sorted, and the model diagnostic indicators were assigned and the consistency was evaluated. In the evaluation of the model,it is found that the animal model of spontaneous ovarian cancer can simulate the pathogenesis process of human natural ovarian cancer, and the model has high modulus rate, high similarity, and high consistency with the clinical characteristics of traditional Chinese and Western medicine. The subcutaneous tumor graft type model and the blood stasis type model were in good clinical agreement with traditional Chinese medicine,a high degree of similarity, and a longer survival time.At present, animal models of ovarian cancer are mainly based on Western medicine models, and research on animal models consistent with traditional Chinese medicine syndromes is rare. Therefore, the improvement direction of ovarian cancer animal models and the necessity of improving the evaluation system are proposed,so as to make the animal models of ovarian cancer more clinical, and provide a theoretical basis for the determination of the efficacy of traditional Chinese medicine and the discussion of pharmacological effects of ovarian cancer, and provide a theoretical basis for the subsequent research on the pathogenesis and treatment measures of ovarian cancer,in order to improve the model of combining ovarian cancer symptoms in line with the clinical characteristics of traditional Chinese medicine and Western medicine.

ABSTRACT

Objective:To explore the efficacy and safety of intermittent infusion of ilaprazole sodium and high-dose continuous infusion of esomeprazole sodium in preventing rebleeding in patients with peptic ulcer bleeding after successful endoscopic hemostasis. Methods:This is a multi-center, interval randomized, double-blind, double-dummy, parallel controlled study. From March 3rd to June 15th, 2021, 151 patients with high risk of peptic ulcer bleeding and successfully underwent endoscopic hemostasis from 33 hospitals including the First Affiliated Hospital of Zhejiang University School of Medicine were enrolled. Patients were interval randomly divided into the trial group (74 cases) and the control group (77 cases). Patients in the trial group received intermittent intravenous infusion of ilaprazole sodium once daily (20 mg administered as a 60 min intravenous infusion on day 1, and 10 mg administered as a 30 min intravenous infusion on day 2 and 3); patients in the control group received continuous intravenous infusion of esomeprazole sodium for 72 h (esomeprazole sodium 80 mg at first dose in half an hour, and 8 mg per hour continuous intravenous infusion for 71.5 h). After intravenous infusion treatment, patients of both groups were given oral ilaprazole enteric-coated tablets, 10 mg each time, once a day for 4 d. The rebleeding rate after 72 h and within 7 d after treatment and the proportion of patients who received endoscopic retreatment or surgery due to rebleeding within 72 h after treatment were analysised based on the full analysis set (72 cases in the trial group and 75 cases in the control group); and the incidence rate of adverse reactions was observed in the two groups based on the safety analysis set (74 cases in the trial group and 76 cases in the control group). Chi-square test or Fisher exact probability test was used for statistical analysis. Results:There was no rebleeding case in the trial group within 72 h and 1 case of rebleeding within 7 d (1.39%, 1/72). In the control group, there was 1 case of rebleeding (1.33%, 1/75) within 72 h and 4 cases of rebleeding (5.33%, 4/75) within 7 d. There was no significant difference in rebleeding rate either after 72 h or within 7 d after treatment between the two groups (both P>0.05). Within 72 h of treatment, no patients in both groups needed endoscopic or surgical retreatment due to rebleeding. Adverse reactions occurred in 5 cases (6.8%, 5/74) and 6 cases (7.9%, 6/76) in the trial group and control group, respectively, which recovered spontaneously without treatment. No serious adverse reactions occurred in both groups. Conclusion:In patients with high-risk peptic ulcer bleeding with successful endoscopic hemostasis, intermittent intravenous infusion of ilaprazole sodium has similar efficacy and safety as continuous high-dose intravenous infusion of esomeprazole sodium, but the dosage of intermitten regimen is less, the administration is more convenient, and it is worthy of clinical promotion.

ABSTRACT

To investigate the effect and mechanism of metformin on intestinal epithelial barrier injury in ulcerative colitis. A cell model of colitis was established by co-culture of human colon cancer cell line Caco-2 and human monocyte cell line THP-1. The colitis model cells were treated with metformin at concentration of for Flow cytometry was used to detect Caco-2 cell apoptosis, and Western blotting was used to detect the protein expression of tight junction proteins and endoplasmic reticulum stress-related proteins. After metformin treatment, the apoptosis rate of Caco-2 cells was decreased from (14.22±2.34)% to 0.61)% (=3.119, <0.05), and the expression levels of tight junction protein-1 and claudin-1 increased (=5.172 and 3.546, both <0.05). In addition, the expression levels of endoplasmic reticulum-related proteins glucose regulated protein (GRP) 78, C/EBP homologous protein (CHOP) and caspase-12, as well as the phosphorylation level of PRKR-like endoplasmic reticulum kinase (PERK) and eukaryotic translation initiation factor 2α (eIF2α) decreased (all <0.05). Metformin may alleviate the intestinal epithelial barrier damage in colitis by reducing intestinal epithelial cell apoptosis and increasing the expression of tight junction proteins, which may be associated with the inhibition of endoplasmic reticulum stress-induced apoptotic pathway.

ABSTRACT

Objective To explore the mechanisms of synergic action of commonly-used herb pair of Radix Sophorae Flavescentis (Kushen, KS) and Fructus Cnidii (Shechuangzi, SCZ) for the treatment of pruritus. Methods We predicted and analyzed the potential targets of KS and SCZ based on network pharmacology method, and then established Chinese herbs-compound skeletons-targets networks to reveal anti-pruritus targets, predicting targets, and the interaction of KS and SCZ, as well as the components of the herb pair. The RAW264.7 inflammatory cell model was established to observe the synergistic anti-inflammatory activity of KS and SCZ. Results KS and SCZ had possible synergic action on the pruritus-related targets such as histamine receptors, cannabinoid receptors and proteinase-activated receptor 2. Additionally, KS and SCZ probably had synergic regulation of inflammation-related pathway (Toll -like receptor signaling pathway , chemokine signaling and Fc epsilon RI signaling pathway) and nerve-related pathway(neurotrophin signaling pathway) during the treatment of pruritus. Flavones from KS and coumarin of SCZ had various synergistic anti-inflammatory activities(P<0.05), indicating that they played an important role in inhibiting pruritus induced by inflammation. Conclusion The method may reveal the molecular mechanism of KS and SCZ in inhibiting pruritus, which is important for the modernization of Chinese medicine and new drug development.

ABSTRACT

Objective To explore the pharmacodynamic action of the extract from raw materials of Rhizoma Anemarrhenae and Cortex Phellode ndri, a classic herb pair in traditional Chinese medicine, on mice with diabetic peripheral neuropathy(DNP). Methods The mice were administrated with intraperitoneal injection of streptozotocin (STZ, 30 mg/kg) and fed with high-fat diet to establish the DNP mouse model. One hundred and sixty male C57BL/6j mice were divided into normal group, model group, metfomin group(130 mg/kg), Xuezhikang group(200 mg/kg), high- and low-dose Rhizoma Anemarrhenae extract group(360, 90 mg/kg) , high-and low-dose Cortex Phellodendri extract group(135, 45 mg/kg), 20 mice in each group. The medication lasted for 24 weeks. On medication week 8, 16 and 24, the plasma levels of glucose (GLU), total cholesterol (TC), triglyceride(TG), low density lipoprotein-cholesterol(LDL-C) and insulin(INS) were determined respectively. On medication week 10, oral glucose tolerance test(OGTT)was carried out. On medication week 24, the plasma nuclear factor kappa B(NF-κB) activity and the plasma levels of transforming growth factor-beta 1(TGF-β1), superoxide dismutase(SOD) and reduced glutathione(GSH) were detected. Results Five weeks after injection of STZ, the body mass of the model group was firstly increased and then decreased, and FBG was increased(P<0.05 compared with the normal group). Compared with the normal group, FBG, TC, TG, LDL-C and INS levels were increased(P<0.01 or P<0.001), plasma NF-κB activity, TGF-β1 and SOD levels were enhanced, and GSH level was decreased in the model group, the difference being statistically significant (P<0.01 or P<0.001). Contrasted with the model group, FBG, TC, TG, LDL-C and INS levels in high-and low-dose Rhizoma Anemarrhenae extract group and in high-and low-dose Cortex Phellodendri extract group were decreased to various degrees. After continuous medication, plasma NF-κB activity and TGF-β1 and SOD levels in high-dose Rhizoma Anemarrhenae extract group and in high-dose Cortex Phellodendri extract group were reduced(P<0.05 or P<0.001), and plasma GSH level was increased(P<0.01). Conclusion Extract from raw materials of Rhizoma Anemarrhenae and Cortex Phellodendri has obvious hypoglycemic effect and protective effect on experimental mice with DPN.

ABSTRACT

The adsorption and activation of dimethyl carbonate on the surface of solid base were investigated by in situ FTIR, and the solid bases included magnesia, magnesium fluoride, Mg-Al mixed oxide and fluorine-modified Mg-Al mixed oxide. The FTIR results showed that dimethyl carbonate adsorbed on the surface of solid based by two modes of bidentate and unidentate complex. The bidentate was more active than the unidentate. Methoxyl group was formed from the adsorbed dimethyl carbonate on the surface of magnesia and Mg-Al mixed oxide. And fluomethyl group was formed from the adsorbed dimethyl carbonate on the surface of sodium fluoride. However, dimethyl carbonate on the surface of fluorine-modified Mg-Al mixed oxide showed preference for generating fluomethyl group. With the increasing of the treating temperature of samples, the methoxyl group was gradually formed on the surface. Accordingly, the fluorine-modified Mg-Al mixed oxide was found to be an excellent catalyst for methylation.

ABSTRACT

[Objective] Molecular distillation technique was applied for the separation and purification of Atractylodes Lancea oil (ALO) to increase the yield of atractylodin. [Methods] Atractylodes Lancea oil prepared with supercritical extraction was refined by molecular distillation technique. The optimum refining conditions were selected by uniform design containing two factors of temperature and vacuum and five levels. The content of atractylodin from refined Atractylodes Lancea oil was measured and the remainder after isolation was detected by high performance liquid chromatography (HPLC). [Results] A yield rate of atraotylodin from Atractylodes Lancea oil was 52.17% at the temperature of 105℃ and under the vacuum of 100 Pa. [Conclusion] The application of molecular distillation technique makes it easy to collect the volatile atractylodin from Atractylodes Lancea oil at lower temperature and under higher vacuum; this method ensures the yield rate of atractylodin over 50% and has no problem of environmental pollution.
