ABSTRACT

Objective To establish the intelligent identification method for the big data of liver injury-related adverse drug reaction (ADR) based on the construction of text database. Methods With the keywords including "drug-induced liver injury" and "abnormal liver function" and a search time of January 1, 2012 to December 31, 2016, 5% (4152 cases) of the case reports of liver injury-related ADR were retrieved and extracted from the China Adverse Drug Reaction Monitoring System, and then based on clinical reevaluation by physicians, these cases were classified into "negative cases", "suspected cases", and "confirmed cases". On this basis, key elements (including ADR name, biochemical parameter, and clinical symptoms) were identified. An intelligent identification method for liver injury-related ADR was established based on the correlation analysis between key elements and clinical reevaluation and the receiver operating characteristic (ROC) curve for determining cut-off values, and the method of cross validation was used to evaluate the performance of this intelligent identification method. Results The formula for the evaluation and identification of liver injury-related ADR was as follows: total score (M)=symptom score+index score+ADR name score. This formula showed the best discriminatory ability to distinguish "negative case" from "suspected case" or "confirmed case" at M=5 (area under the ROC curve [AUC]=0.97), with a sensitivity of 99.57% and a specificity of 84.61%, and it showed the best discriminatory ability to distinguish "confirmed case" from "suspected case" or "negative case" at M=12 (AUC=0.938), with a sensitivity of 87.93% and a specificity of 85.98%. Conclusion This method provides reference and basis for intelligent identification and evaluation of big data on liver injury-related ADR and is expected to effectively reduce the burden of manual processing of ADR big data and provide effective tools and methodological demonstration for early risk signal identification and warning of liver injury-related ADR.

ABSTRACT

Objective:To apply the best evidence of evidence-based volume management in patients with chronic heart failure to clinical practice and evaluate its effect.Methods:From February 2018 to May 2019, according to the clinical evidence practice application mode of Joanna Briggs Institute (JBI), and the best evidence obtained in the previous study, we constructed audit indicators. And then perfomed clinical audits for the patients with chronic heart failure in the department of cardiology of Shandong Provincial Hospital affiliated to Shandong First Medical University. The obstacle factors were analyzed, and the action strategies were formulated. The cognition of the medical staff on the volume management, and the fasting body mass monitoring rate, the volume management target compliance rate, the time to dry body mass, average hospitalization day of the patients′, number of negative events related to volume management were compared before and after the application of the best evidence.Results:After the implementation of the reform, Indicator 8-14 and Indicator 20 of the 20 audit indicators implementation rate had been improved, and the difference was statistically significant ( χ 2 values were 6.668-97.000, P<0.05); the volume management target compliance rate increased from (34.24±30.33)% to (61.28±16.10)%, the difference was statistically significant ( t value was 30.21, P<0.001); the time to dry body mass decreased from (10.89±3.46) days to (5.48±1.58) days, the difference was statistically significant ( t value was 13.97, P<0.001); compared with the negative events related to the access control, the number of acute left heart failure cases decreased from 7 to 2 ( χ2 value was 4.194, P=0.041). Conclusion:Evidence-based volume management of patients with chronic heart failure can improve clinical nursing practice, shorten the time for patients to reach dry body mass, and reduce the average hospitalization day. Quality review still needs to continue to improve the quality of nursing.

ABSTRACT

Objective:To observe short-term and long-term effect of modified Shenfuwan on lumbar disc herniation due to kidney deficiency and cold coagulation. Method:100 cases were randomly divided into the control group and observation group,50 cases each. The control group were taken meloxicam tablets,7.5 mg each time,once a day and the observation group were given modified Shenfuwan,300 mL each time,twice a day. The treatment course was 3 months. The short form McGill pain questionnaire(SF-MPQ),oswestry disability index(ODI),and lumbar intervertebral disc herniation due to kidney deficiency and cold coagulation by traditional Chinese medicine (TCM ) syndrome were observed before and after treatment. Whole blood viscosity (high,medium,low),serum inflammatory factors [tumor necrosis factor-alpha(TNF-<italic>α</italic>),interleukin-1<italic>β</italic>(IL-1<italic>β</italic>),interleukin-6(IL-6),pain mediators[endothelin-1(ET-1),prostaglandin E<sub>2</sub>(PGE<sub>2</sub>),cyclooxygenase-2(COX-2)] were detected. The clinical efficacyand,recurrence rate followed up for 12 and 24 months was was observed. The type and time of adverse reactions were recorded during the study. Result:After treatment,the total effective rate in the observation group was 98.0%(49/50) higher than that 82.0%(41/50) in the control group(<italic>P</italic><0.05). After 12 months of follow-up,the recurrence rate in the observation group was 23.9%(11/46) lower than that 52.5%(21/40) in the control group (<italic>χ</italic><sup>2</sup>=5.193,<italic>P</italic><0.05). After 24 months of follow-up,the recurrence rate in the observation group was 43.9%(18/41) lower than that that 80.6%(29/36) in the control group(<italic>P</italic><0.05). Compared with the control group after treatment,SF-MPQ,ODI,TCM symptoms,whole blood viscosity,TNF-<italic>α</italic>,IL-1<italic>β</italic>,IL-6,ET-1,PGE<sub>2</sub> and COX-2 were significantly reduced in the observation group(<italic>P</italic><0.05). The incidence of adverse reactions in the observation group was 2.0%(1/50) lower than that 52.0%(26/50) in the control group(<italic>χ</italic><sup>2</sup>=6.264,<italic>P</italic><0.05). Conclusion:Modified Shenfuwan can significantly improve the short-term and long-term clinical efficacy of patients with lumbar disc herniation due to kidney deficiency and cold coagulation syndrome,and the incidence of adverse reactions is low.

ABSTRACT

Objective:This study aims to investigate the clinical efficacy of modified Anshentang on the treatment of ankylosing spondylitis in early and middle stages with kidney deficiency and cold-governing syndrome and its effect on serum inflammatory factors, immune function, and bone metabolism indexes of patients. Method:In this study, 120 patients were randomly divided into control group and observation group, 60 cases in each group. On the basis of ethotrexate treatment, patients in control group took Bushen Shuji granule orally, while patients in observation group took modified Anshentang orally for 8 weeks. Before and after treatment, patients in two groups were observed for clinical symptoms [ bath ankylosing spondylitis patient global score (BAS-G), bath ankylosing spondylitis disease activity index (BASDAI), spondyloarthritis research consortium of Canada (SPARCC), traditional Chinese medicine symptoms (TCM symptoms)<italic> </italic>], serum inflammatory factors [ tumor necrosis factor-<italic>α </italic>(TNF-<italic>α</italic>), macrophage migration inhibitory factor (MIF), interleukin-23 (IL-23)], immune function [ immunoglobulin A(IgA), immunoglobulin G(IgG), immunoglobulin M(IgM)], bone metabolic indicators [osteocalcin (BGP), bone morphogenetic protein-2 (BMP-2), bonespecific alkaline phosphatase (BALP)]. The clinical efficacy, adverse reactions and recurrence rates of 12 months in two groups were observed. Result:During the study, 4 cases dropped out from control group and 2 cases from observation group. The total effective rate of 96.55% (56/58) in observation group was higher than 80.36% (45/56) in control group (<italic>χ</italic><sup>2</sup>=4.827,<italic>P</italic><0.05). The recurrence rate of 5.17% (3/58) in observation group was lower than 19.64% (11/56) in control group (<italic>χ</italic><sup>2</sup>=5.187, <italic>P</italic><0.05). Compare with control group after treatment, BAS-G,BASDAI, SPARCC, TCM symptoms, TNF-<italic>α</italic>, MIF and IL-23 in observation group were significantly decreased (<italic>P</italic><0.05), while BGP, BMP-2, BALP, IgA, IgG and IgM were significantly increased (<italic>P</italic><0.05). The incidence of adverse reactions was 12.07%(7/58) in observation group, which was lower than 32.14%(18/56) in control group (<italic>χ</italic><sup>2</sup>=4.826,<italic>P</italic><0.05). Conclusion:Modified Anshentang is effective in the treatment of ankylosing spondylitis in early and middle stages with kidney deficiency and cold-governing syndrome, and the incidence of adverse reactions is low.

ABSTRACT

The quality control of Epimedii Folium, composed of diverse constituents, is single at present. In view of this, an eva-luation method of 13 chemical constituents based on quantitative analysis of multi-components by single marker(QAMS) was established to further explore the composition differences of raw products and alcohol extracts in different batches and the influence of alcohol extraction on the composition, so as to provide a reference for improving the quality evaluation and control of Epimedii Folium. The fingerprints of different batches of Epimedii Folium were constructed by ultra-high performance liquid chromatography(UPLC) to evaluate the inter-batch consistency. The changes of the flavonoids in Epimedii Folium during alcohol extraction were analyzed based on determined levels and heat map, and the reasons for the changes were preliminarily discussed. With icariin, the quality control component recorded in the Chinese Pharmacopoeia, as the internal reference, the stability of the relative correction factors of chemical components under different conditions was investigated to obtain the relative correction factors. Then the determination results of QAMS and the external standard method were compared to verify the accuracy of QAMS. The results revealed that all batches of Epimedii Folium met the requirements specified in the Chinese Pharmacopoeia, and the fingerprints of Epimedii Folium from the same place of origin exhibited a high similarity. Raw products and alcohol extracts of Epimedii Folium could be clearly distinguished by prenylated flavonoids, which are potential biomarkers for quality control. Additionally, the glycoside hydrolysis in the alcohol extraction was preliminarily explored. The QAMS method has good accuracy, durability, and repeatability in determining 13 chemical components in Epimedii Folium under different experimental conditions. No significant difference in the results obtained by the two methods was observed. This study can provide a reference for comprehensive, rapid and reasonable quality evaluation of Epimedii Folium.

ABSTRACT

Objective:To compare liver adhesive suspension method and V-shaped liver suspension of anterior abdominal wall in laparoscopic radical gastrectomy.Methods:This study retrospectively evaluated 115 patients who underwent laparoscopic radical gastrectomy from Jan 2018 to Dec 2019 at Shanxi Provincial People′s Hospital, 59 cases were under liver adhesive suspension (study group) and 56 cases under V-shaped liver suspension of anterior abdominal wall (control group). Intraoperative and postoperative conditions were observed and compared between the two groups.Results:All the 115 cases successfully completed surgery. The liver suspension time in study group was (2.4±0.5)min, which was significantly shorter than that of the control group((12.5±2.6) min)( t=-28.100, P<0.01). The total operation time in was (284±69) min and (307±78) min, respectively, ( t=-1.656, P=0.10). There were no statistically significant differences between the two groups in preoperative ALT, AST, total bilirubin, intraoperative incidence of hepatic lacerations and subcapsular hematoma, postoperative total bilirubin level, AST on the day 7 postoperatively, tumor location, TNM staging, surgical method, postoperative duration of hospital stay, and VAS score ( P>0.05). Conclusions:Both methods of liver suspension are safe and feasible in laparoscopic surgery for gastric cancer.Compared with V-shaped liver suspension of anterior abdominal wall, liver adhesive suspension has the advantages of less invasive, safe, and time-saving.

Objective@#The expression patterns of ribosomal large subunit protein 23a (RPL23a) in mouse testes and GC-1 cells were analyzed to investigate the potential relationship between RPL23a expression and spermatogonia apoptosis upon exposure to X-ray.@*Methods@#Male mice and GC-1 cells were irradiated with X-ray, terminal dUTP nick end-labelling (TUNEL) was performed to detect apoptotic spermatogonia @*Results@#Ionizing radiation (IR) increased spermatogonia apoptosis, the expression of RPL11, MDM2 and p53, and decreased RPL23a expression in mice spermatogonia @*Conclusion@#These results suggested that IR reduced RPL23a expression, leading to weakened the RPL23a-RPL11 interactions, which may have activated p53, resulting in spermatogonia apoptosis. These results provide insights into environmental and clinical risks of radiotherapy following exposure to IR in male fertility. ABSTRACT

Objective:To observe the clinical efficacy of modified Bixie Shengshitang on acute gouty arthritis due to hot and humid syndrome. Method:According to the random number table method, 130 cases were randomly divided into control group and observation group, with 65 cases in each group. All of the cases were given the basic non-drug therapy. The control group was given colchicine, while observation group was given modified Bixie Shengshitang + colchicine for 14 d. Before treatment and at 3, 7 and 14 d after treatment, total symptom score (TSS) and traditional Chinese medicine (TCM) syndrome were observed between the two groups, respectively. Blood uric acid (BUA), urinary uric acid (UUA), erythrocyte sedimentation rate (ESR), proinflammatory factors [tumor necrosis factor-α (TNF-α), interleukin-1 (IL-1), interleukin-8 (IL-8)] and anti-inflammatory factors [interforon gamma receptor (IFN-γ), interleukin-4 (IL-4), interleukin-18(IL-18)] in serum and joint fluid were detected before and after treatment. The clinical efficacy and safety of the two groups were compared. Result:The total effective rate was 96.9% (62/64) in observation group, which was higher than 80.6% (50/62) in control group (χ2=5.713, P<0.05). Compared with control group at 3, 7 and 14 d after treatment, TSS and TCM syndrome scores in observation group were significantly reduced (P<0.05). Compared with control group after treatment, BUA, ESR, TNF-α, IL-1β, IL-8 and UUA, IFN-γ, IL-4 and IL-18 were significantly decreased in observation group (P<0.05). There was no serious adverse event during the study period. The incidence of adverse reactions was 54.7% (35/64) in observation group, which was lower than 82.3% (51/62) in control group (χ2=9.326, P<0.05). Conclusion:Modified Bixie Shengshitang can significantly alleviate the clinical symptoms of patients with acute gouty arthritis due to hot and humid syndrome, and adjust levels of uric acid and inflammatory cytokines, with a low recurrence rate.

ABSTRACT

Objective:To investigate the current situation of kinesiophobia in patients with knee osteoarthritis and its influencing factors, in order to provide basis for clinical intervention.Methods:Totally 230 patients with knee osteoarthritis were selected from 2 tertiary hospital in Ji′nan by convenience sampling method, the self-designed general data questionnaire, Tampa Scale for Kinesiophobia, Numerical Rating Scale, Self-efficacy for Managing Chronic Disease, Hospital Anxiety and Depression Scale were used as research tools. Logistic regression analysis was used to analyze the influencing factors.Results:A total of 219 valid questionnaires were collected. Kinesiophobia occurred in 166 patients, with an incidence of 75.80% (166/219). Logistic regression analysis showed that gender, education level, chronic complications, pain and self-efficacy were the main influencing factors of kinesiophobia in patients with knee osteoarthritis ( P<0.05). Conclusion:The incidence of kinesiophobia in patients with knee osteoarthritis is relatively high, and medical staff should pay attention to the evaluation of kinesiophobia symptoms in this population, early identify the high-risk factors, provide timely health education, implement the appropriate intervention measures to reduce the occurrence of kinesiophobia.

ABSTRACT

Ricin is a highly toxic type 2 ribosome-inactivating protein (RIP) which is extracted from the seeds of castor beans. Ricin is considered a potential bioterror agent and no effective antidote for ricin exists so far. In this study, by structural modification of a retrograde transport blocker Retro-2, a series of novel compounds were obtained. The primary screen revealed that compound has an improved anti-ricin activity compare to positive control. pre-exposure evaluation in Madin-Darby Canine Kidney (MDCK) cells demonstrated that is a powerful anti-ricin compound with an EC of 41.05 nmol/L against one LC (lethal concentration, 5.56 ng/mL) of ricin. Further studies surprisingly indicated that confers post-exposure activity against ricin intoxication. An study showed that 1 h post-exposure administration of can improve the survival rate as well as delay the death of ricin-intoxicated mice. A drug combination of with monoclonal antibody mAb4C13 rescued mice from one LD (lethal dose) ricin challenge and the survival rate of tested animals is 100%. These results represent, for the first time, indication that small molecule retrograde transport blocker confers both and post-exposure protection against ricin and therefore provides a promising candidate for the development of anti-ricin medicines.

ABSTRACT

Objective To explore the impact of maternal hepatitis B (HBV) virus infection status on the vaccination effect of the offspring. Methods Eighty-six pairs of HBV-infected mothers and newborns who were treated in Dalian Sixth People's Hospital from 2017 to 2019 were selected as the research subjects. According to the strength of the hepatitis B vaccine response, all patients were divided into two groups, the observation group (none or weak response) and the control group (strong response). Hepatitis B virus infection was compared between the mothers of the two groups, and multivariate logistic regression was used to analyze the influencing factors of vaccination effect of the offspring. Results The positive rate of HBV DNA in mothers of the observation group was higher than that in the control group. HBV DNA was mainly at high load and HBeAg was at low level. The HBV DNA of the control group was mainly at low load, while the distribution of HBeAg was close. The difference was statistically significant (P "HBsAg(+), HBeAg(+), anti-HBc(+)" in 14 cases (51.85%), "HBsAg(+), anti-HBe(+), anti- HBc(+)" in 11 cases (40.74%) , while there were 27 cases (45.76%), 25 cases (42.37%), and 7 cases (11.86%), respectively, in the control group. The difference was statistically significant (P<0.05). Multivariate logisitic regression analysis showed that HBV DNA positivity, HBV DNA high load, HBeAg positivity, and low HBeAg levels were risk factors for weak response of the offspring vaccination. Conclusion HBV DNA positivity, high HBV DNA load, HBeAg positivity, and low HBeAg levels in HBV-infected mothers were risk factors for weak response of the offspring to vaccines, which should be paid attention to in clinic.

OBJECTIVES@#To develop a new Chinese medicine (CM)-based drug and to evaluate its safety and effect for suppressing acute respiratory distress syndrome (ARDS) in COVID-19 patients.@*METHODS@#A putative ARDS-suppressing drug Keguan-1 was first developed and then evaluated by a randomized, controlled two-arm trial. The two arms of the trial consist of a control therapy (alpha interferon inhalation, 50 µg twice daily; and lopinavir/ritonavir, 400 and 100 mg twice daily, respectively) and a testing therapy (control therapy plus Keguan-1 19.4 g twice daily) by random number table at 1:1 ratio with 24 cases each group. After 2-week treatment, adverse events, time to fever resolution, ARDS development, and lung injury on newly diagnosed COVID-19 patients were assessed.@*RESULTS@#An analysis of the data from the first 30 participants showed that the control arm and the testing arm did not exhibit any significant differences in terms of adverse events. Based on this result, the study was expanded to include a total of 48 participants (24 cases each arm). The results show that compared with the control arm, the testing arm exhibited a significant improvement in time to fever resolution (P=0.035), and a significant reduction in the development of ARDS (P=0.048).@*CONCLUSIONS@#Keguan-1-based integrative therapy was safe and superior to the standard therapy in suppressing the development of ARDS in COVID-19 patients. ABSTRACT

Objective@#To survey the nutritional status of maintenance hemodialysis (MHD) patients and explore the role of oral nutritional supplement (ONS) in improving nutritional status among MHD patients.@*Methods@#From January 2016 to December 2017, one hundred and sixty-two MHD patients were selected randomly from hemodialysis centre of No. 2 peoples’ Hospital and assessed malnutrition incidence by objective score of nutrition on dialysis (OSND). Patients with malnutrition were divided into control group and ONS group randomly, patients in control group were conducted with traditional nutritional education, while patients in ONS group were conducted with ONS as a substitution of diet on the basis of control group. Finally, nutrition-related indexes were observed after 6 months.@*Results@#The malnutrition incidence of MHD patients was 66.7% (108/162) screening by OSND, the malnutrition incidence of patients over 60 years old was 82.8% (72/87), significantly higher than below 60 years old, 49.3%(37/75)(P<0.05). After 6-month intervention, the differences of nutrition-related physical and laboratory indexes between control group and ONS group was observed [BW:(53.2±6.5)kg vs. (50.7±6.0)kg, t=2.08, P=0.04; TSF:(10.3±3.1)mm vs. (9.0±3.0)mm, t=2.21, P=0.03; ALB:(39.6±5.6)g/L vs. (35.6±5.5)g/L, t=3.74, P<0.01; HB:(104.5±10.8)g/L vs. (99.7±12.6)g/L, t=2.13, P=0.04; TF:(2.0±0.5)g/L vs. (1.5±0.4)g/L, t=5.74, P<0.01], while opposite before intervention (P>0.05). There was no significant difference of OSND, GNRI and MIS scores between control group and ONS group before intervention [OSND:(24.1±1.9) vs. (25.3±1.7), t=3.46, P<0.01; GRNI:(90.4±5.4) vs. (92.4±4.5), t=2.09, P=0.04; MIS:(9.2±3.2) vs. (10.9±3.3), t=2.72, P=0.01], while opposite after intervention (P<0.05). Subjective feeling score of patients in ONS group was significantly higher than control group after intervention [(10.7±3.5)vs(8.9±4.0), t=2.49, P=0.01]. The extra cost in control group and ONS group was (688.8±254.6) RMB and (749.2±44.9) RMB respectively with no significance (t=1.72, P=0.09).@*Conclusion@#The malnutrition incidence is high among MHD patients, traditional nutrition education takes poor patients compliance, while ONS could improve patients′ compliance and nutritional status because of scientific formula and good taste, which provide another diet selection for MHD patients.

Objective:To explore the difference of effect of raw and wine-processed products of Rhei Radix et Rhizoma extract on brain protection of rats with intracerebral hemorrhage(ICH) and the connotation of the theory of "wine processing could promote efficacy" of Rhei Radix et Rhizoma. Method:Wistar male rats were used to establish the ICH model,extracts of raw and wine-processed products of Rhei Radix et Rhizoma were used for intervention,and these rats were randomly divided into 6 groups(blank group,sham-operated group,model group,raw Rhei Radix et Rhizoma group,wine-processed Rhei Radix et Rhizoma group and Angong Niuhuangwan group).Then the apoptosis of hippocampal neurons was detected by terminal deoxynucleotidyl transferase-mediated dUTP nick end labeling(TUNEL) assay.The expression of zonula occludens-1(ZO-1) and vascular endothelial cadherin-2(VE-cadherin-2) in the hemorrhagic brain tissue was detected by immunohistochemical assay.The expression of reduced form of nicotinamide-adenine dinucleotide phosphate(NADPH) oxidase 2(NOX2) and tumor necrosis factor-α stimulated gene-6(TSG-6) in the hemorrhagic brain tissue was detected by Western blot.The content of glutathione(GSH) in the serum of ICH rats was detected by automatic enzyme-linked immunosorbent assay systems.Through these above methods,we investigated the differences between raw products and wine-processed products on brain protection of ICH rats. Result:Compared with the sham-operated group,the apoptosis index of the hippocampal neurons in the model group were increased significantly(PPPPPPPPPPPPConclusion:The raw and wine-processed products of Rhei Radix et Rhizoma all have brain protective effect on ICH rat model,but the protective effect of wine-processed products is slightly better than that of raw products.The result of this study provides experimental basis for exploring the theory of "wine processing could promote efficacy" of this herb.

ABSTRACT

Objective: To explore the clinical efficacy and safety of modified Huqianwan in treatment of rheumatoid arthritis (RA) liver-kidney Yin deficiency syndrome, and investigate its possible mechanism. Method: A total of 184 patients with RA liver-kidney Yin deficiency syndrome were randomly divided into Chinese medicine group (62 cases), western medicine group (57 cases) and integrated Chinese and western medicine group (65 cases) according to the digital table method. The patients in Chinese medicine group were treated with Huqianwan; the patients in western medicine group were treated with methotrexate tablets and leflunomide tablets; and the patients in integrated Chinese and western medicine group received Huqianwan+methotrexate tablets and leflunomide tablets,with a treatment course of 12 weeks in all groups. The pain visual analog scale (VAS), swelling and tenderness scores of 28 joints (DAS28), average hands grip strength, morning stiffness time and liver-kidney Yin deficiency syndrome differentiation of traditional Chinese medicine (TCM) syndrome score were compared between groups before and after treatment. The changes of erythrocyte sedimentation rate (ESR), C reactive protein (CRP), immunoglobulin (Ig) G, tumor necrosis factor-alpha (TNF-α) and rheumatoid factor (RF) were detected in all groups after treatment. Clinical efficacy, and incidence of adverse reactions such as gastrointestinal response, liver injury, leukopenia, serum glutamate oxaloacetic aminotransferase (GOT) and platelet (PLT) level changes were compared between the groups, so as to investigate the efficiency and safety of the different medicines. Result: After 12 weeks of treatment, the total clinical effective rate was 79.0%, 80.7%, and 92.3% respectively in Chinese medicine group, western medicine group, and integrated Chinese and western medicine group; the integrated Chinese and western medicine group was significantly better than the Chinese medicine group and western medicine group (PPPPConclusion: The efficacy in treating RA liver and kidney Yin deficiency syndrome shows no significant difference between modified Huqianwan and methotrexate tablets+leflunomide tablets. In the treatment of RA liver and kidney Yin deficiency syndrome, Huqianwan has fewer adverse reactions. Huqianwan combined with methotrexate tablets+leflunomide tablets is superior to that in methotrexate tablets+leflunomide tablets in treatment of RA liver-kidney Yin deficiency syndrome.

Objective@#We predicted the molecular mechanism of Panax notoginseng in the treatment and improvement of scar hyperplasia, by using the methods of network pharmacology and bioinformatics.@*Methods@#We collected of related active constituents and targets of Panax notoginseng by retrieving TCM systems pharmacology database and analysis platform (TCMSP), and collected of related active constituents and targets of scar by Genecards database and OMIM database. Cytoscape 3.6.1 software was used to construct "drugs-chemical components-targets-diseases" interaction network diagram. The protein in teraction network map (PPI) was constructed by STRING database. The key targets were used to analyze gene ontology (GO) enrichment and kyoto encyclopedia of genes and genome (KEGG) pathway enrichment.@*Results@#Totally 7 chemical compponents, including beta-sitosterol, quercetin, Stigmasterol and etc. and 108 targets, including AKT1, JUN, MAPK1, IL6 and ect. Panax notoginseng exerts its effects on scar mainly by acting on signal pathways, including PI3K-AKt signal pathway, MAPK signal pathway, TNF signal pathway and ect.@*Conclusions@#Based on the methodology of network pharmacology, this study preliminarily predicted the major targets and pathways Panax notoginseng in the treatment of scar, providing a direction for further studies.

ABSTRACT

Objective To survey the nutritional status of maintenance hemodialysis (MHD) patients and explore the role of oral nutritional supplement (ONS) in improving nutritional status among MHD patients. Methods From January 2016 to December 2017, one hundred and sixty-two MHD patients were selected randomly from hemodialysis centre of No. 2 peoples’Hospital and assessed malnutrition incidence by objective score of nutrition on dialysis (OSND). Patients with malnutrition were divided into control group and ONS group randomly, patients in control group were conducted with traditional nutritional education, while patients in ONS group were conducted with ONS as a substitution of diet on the basis of control group. Finally, nutrition-related indexes were observed after 6 months. Results The malnutrition incidence of MHD patients was 66.7% (108/162) screening by OSND, the malnutrition incidence of patients over 60 years old was 82.8% (72/87), significantly higher than below 60 years old, 49.3%(37/75)(P<0.05). After 6-month intervention, the differences of nutrition-related physical and laboratory indexes between control group and ONS group was observed [BW:(53.2±6.5)kg vs. (50.7± 6.0)kg, t=2.08, P=0.04; TSF:(10.3±3.1)mm vs. (9.0±3.0)mm, t=2.21, P=0.03; ALB:(39.6±5.6)g/L vs. (35.6± 5.5)g/L, t=3.74, P<0.01; HB:(104.5±10.8)g/L vs. (99.7±12.6)g/L, t=2.13, P=0.04; TF:(2.0±0.5)g/L vs. (1.5± 0.4)g/L, t=5.74, P<0.01], while opposite before intervention (P>0.05). There was no significant difference of OSND, GNRI and MIS scores between control group and ONS group before intervention [OSND:(24.1± 1.9) vs. (25.3 ± 1.7), t=3.46, P<0.01; GRNI:(90.4 ± 5.4) vs. (92.4 ± 4.5), t=2.09, P=0.04; MIS:(9.2 ± 3.2) vs. (10.9±3.3), t=2.72, P=0.01], while opposite after intervention (P<0.05). @#Objective     To report our experience with enhanced recovery after surgery (ERAS) protocol in patients undergoing transapical transcatheter aortic valve implantation (TAVI) and to determine perioperative predictors for ERAS failure. Methods     Between May 2018 and January 2019, 80 patients undergoing TAVI in our hospital were recruited. Among them, 40 patients (24 males, 16 females, aged 73.0±5.0 years) successfully received ERAS, defined as successful extubation in operating room (an ERAS group) and the other 40 patients (28 males, 12 females, aged 73.0±7.0 years, a non-ERAS group) failed to perform ERAS.  Results     Compared with the non-ERAS group, patients in the ERAS group were associated with a significantly lower incidence of postoperative complications (30.0% vs. 52.5%, P=0.04), shorter postoperative ICU stay (2.2±0.4 d vs. 4.0±4.8 d, P=0.00) and hospital stay (7.0±2.3 d vs. 9.5±4.8 d, P=0.00), and less medical cost (311±20 thousand yuan vs. 347±80 thousand yuan, P=0.00). Independent predictors of ERAS failure were poor preoperative heart function and elevated B-type natriuretic peptides.  Conclusion     ERAS protocol is feasible and effective in patients undergoing TAVI. Poor preoperative heart function is an independent predictor of failure in early extubation which, in turn, is associated with prolonged ICU and hospital stay and dramatic worsening of patient outcomes.

Complex physical activity is a prerequisite for the completion of advanced sports skills, while trunk control is a prerequisite for complex physical activity. Cerebral palsy is often characterized by poor trunk control and poor balance response. An accurate measurement for trunk control function of patients is of great guiding significance for clinical treatment. Trunk Control Measurement Scale (TCMS) is applied to children with neuromotor dysfunction aged five years and above. TCMS has high reliability and validity, and high test-retest reliability and inter-rater reliability in evaluating spastic cerebral palsy, and it is a reliable, effective and appropriate measurement tool in evaluating the trunk control ability. It can combine with other functional scales to provide more valuable information for the function of patients with cerebral palsy because of its high correlation with other types of scales.
