ABSTRACT

Patients with abdominal surgery history always present different degrees of abdominal adhesion. In the past, it was regarded as a relative contraindication of laparoscopic surgery. With the development of minimally invasive concept, reoperative minimally invasive surgery is proposed in clinic to avoid huge trauma caused by multiple open surgeries. In June 2022, a laparoscopic assisted ileostomy reduction was performed for a patient with history of multiple abdominal injuries in Shanghai Changzheng Hospital. Minimally invasive achieved after huge trauma, and it maximized the benefit to the patient.

ABSTRACT

Objective:To investigate the clinical effect of arthroscopic minimally invasive Kirschner wire tension band internal fixation in the treatment of transverse patellar fracture.Methods:From March 2016 to March 2018, 108 patients with transverse patellar fracture admitted to the Department of orthopedics, Linxi hospital, Kailuan general hospital, Hebei Province were analyzed retrospectively.According to the different operation methods, the patients were divided into arthroscopic group (68 cases) and control group (40 cases). The patients in the arthroscopic group were treated with arthroscopic minimally invasive Kirschner wire tension band internal fixation, and the patients in control group were treated with open reduction and Kirschner wire tension band internal fixation.The operation condition, clinical efficacy, Lysholm score and visual analogue scale(VAS) score were compared between the two groups.Results:The incision length((3.5±0.8) cm) of arthroscopic group was shorter than that of control group((13.7±2.2) cm), the amount of intraoperative bleeding ((47.4±12.5) ml) was significantly less than that of control group ((86.9±14.7) ml), and the length of hospitalization((11.8±2.6) d) and fracture healing time ((11.5±2.4) weeks)were significantly less than those in control group ((15.8±3.0) d, (15.7±2.8) weeks), the difference was statistically significant( t value were 34.63, 14.85, 7.29, 8.25; all P<0.05). The Lysholm score of two groups increased significantly with time( Fintra-group=87.53, P<0.05). The increasing of Lysholm score in arthroscopic group was significantly higher than that in control group( Finteraction=7.64, P<0.05). The overall level of Lysholm in arthroscopic group was significantly higher than that of control group( Finter-group=11.42, P<0.05). The VAS score of two groups decreased significantly with time( Fintra-group=76.47, P<0.05). The decreased of VAS score in arthroscopic group was significantly higher than that in control group( Finteraction=9.96, P<0.05). The overall level of VASin arthroscopic group was significantly lower than that of control group( Finter-group=10.38, P<0.05). Conclusion:Arthroscopic minimally invasive Kirschner wire tension band internal fixation for the treatment of transverse patellar fracture has better efficacy and safety, less injury, less pain after operation.

ABSTRACT

Objective:To compare the clinical effect of hollow screw tension band internal fixation and Kirschner wire tension band internal fixation for patellar fracture, so as to provide reference for clinical operation.Methods:A retrospective case-control study was conducted to analyze the clinical data of 82 patients with transverse patellar fracture admitted to the Department of Orthopedics of Linxi Hospital of Kailuan general hospital and the Department of orthopedics of Tangshan Central Hospital from May 2016 to October 2018.According to different operation methods, they were divided into Kirschner wire group (44 cases) and cannulated screw group (38 cases). The Kirschner wire group was treated with Kirschner wire tension band internal fixation, and the hollow screw group was treated with cannulated screw tension band internal fixation. The operation conditions, postoperative complications and postoperative knee function recovery of the two groups were compared.Results:The operation time of hollow screw group was significantly longer than that of Kirschner wire group((68.25±10.81) min vs.(59.72±12.32) min, t=3.31, P<0.05). Fracture healing time of hollow screw group was significantly shorter than that of Kirschner wire group((3.12±0.40) month vs.(3.68±0.45) month, t=5.91, P<0.05). B?stmanscores of the two groups increased significantly with time( Fintra-group=74.69, P<0.05). The B?stmanscore of cannulated screw group was significantly higher than that of Kirschner wire group ( Finteraction=4.87, P<0.05). The B?stmanscore of hollow screw group was significantly higher than that of Kirschner wire group( Finter-group=7.52, P<0.05). VAS scores in both decreased significantly with time( Finter-group=86.24, P<0.05). The decreasing of visual analogue scale (VAS) score in hollow screw group was significantly higher than that in Kirschner wire group( Finteraction=6.62, P<0.05). The VAS score in hollow screw group was significantly lower than that in Kirschner wire group ( Finter-group=7.74, P<0.05). The postoperative complications were 20.5% (9/44) in Kirschner wire group and 5.3% (2/38) in hollow screw group.The incidence of adverse reactions in hollow screw group was lower than that in Kirschner wire group (χ 2=4.05, P<0.05). The excellent and good rates of Kirschner wire group and cannulated screw group were 86.4% (38/44) and 94.7% (36/38), respectively, with no significant difference (χ 2=0.51, P>0.05). Conclusion:The results show that the two methods have the same effect on patella fracture, but the hollow screw tension band internal fixation has less trauma and less complications, which is conducive to the recovery of knee joint function.

ABSTRACT

Objective To investigate the clinical effect of minimally invasive Kirschner wire tension band internal fixation under arthroscopy in the treatment of patellar fracture.Methods From March 2016 to April 2018,ninety patients with patellar fracture admitted to the Department of Orthopedics,Lin Xi Hospital of Kailuan General Hospital were divided into minimally invasive group (58 cases) and control group (32 cases) according to different operative methods.The control group received traditional open reduction and Kirschner wire tension band internal fixation.Minimally invasive group received arthroscopic minimally invasive Kirschner wire tension band internal fixation.The operation status,joint function recovery and VAS scores were compared between the two groups.Results The amount of blood loss,hospitalization time and fracture healing time in minimally invasive group were significantly less than those in control group ((48.7 ±13.6) mlvs.(85.5±12.3) ml,t=12.7,P<0.05;(12.5±2.8) d vs.(14.8±1.4) d,t=4.35,P<0.05;(11.2± 1.3) weeks vs.(15.4± 2.2) weeks,t =11.40,P<0.05).After 6 months of follow-up,the VAS score of the two groups decreased with the time (the VAS score of the minimally invasive group decreased from (7.2±1.1) points preoperatively to (0.9±0.2) points at 6 months postoperatively,and that of the control group decreased from (7.3±1.2) points preoperatively to (1.1±0.3) at 6 months postoperatively).There was a significant difference between the two groups (F group =77.87,P<0.05).The VAS score of both groups decreased with time (Ft ime=263.47,P<0.05).The decrease of VAS score in the minimally invasive group was significantly larger than that in the control group (F interaction =28.63,P<0.05).Conclusion Arthroscopic minimally invasive Kirschner wire tension band internal fixation for patellar fracture has better clinical effect and less trauma.

ABSTRACT

Objective To investigate the clinical effect of arthroscopy minimally invasive Kirschner wire tension band internal fixation combined with lower limb feedback training for patellar fracture.Methods Fifty-eight cases of patellar fracture who received arthroscopy minimally invasive Kirschner tension band internal fixation from March 2016 to April 2018 in Linxi Hospital,Kailuan General Hospital were included in this study.All cases were divided into control group (32 cases) and observation group (26 cases) according to the different rehabilitation methods.Control group received routine rehabilitation,observation group received intelligent feedback system of the lower limbs based on the control group.The recovery of joint function was compared between two groups.Results The clinical efficacy of the observation group was excellent (18 cases),good (6 Cases),and medium (2 cases),the excellent and good rate was 92.30% (24/ 26).In the control group,the clinical effect was excellent (19 cases),good (9 cases),and medium (4 cases).The excellent and good rate was 87.50% (28/32).There was no significant difference in clinical efficacy (z =0.80,P> 0.05) and excellent and good rate (x2 =0.36,P> 0.05) between two groups after 6 months follow-ups.The scores and total scores of B(o)stman patellar fracture function before treatment in observation group were pain (3.3 ± 0.8),Knee range of motion (3.2 ± 0.7),work (1.8 ± 0.3),muscular atrophy (1.8±0.2),auxiliaries (2.9 ± 0.6),effusion (1.2 ± 0.4),giving way (0.3 ± 0.2),climb stairs (0.3 ±0.2) andtotal score (15.0 ±2.2) respectively.The scores and total scores of B(o)stman patellar fracture function after treatment in observation group were pain (5.7 ± 0.4),Knee range of motion (5.8 ± 0.2),work (3.6 ±0.5),muscular atrophy (3.5 ± 0.6),auxiliaries (3.7 ± 0.4),effusion (1.8 ± 0.3),giving way (1.7 ±0.4),climb stairs (1.7 ± 0.5) and total score (28.3 ± 1.6) respectively.The B(o)stman patellar fracture functional score in observation group increased significantly (t =14.62,19.15,16.47,14.78,6.96,7.18,17.26,14.16,26.30,P < 0.05).The scores and total scores of B(o)stman patellar fracture function before treatment in control group werepain (3.4 ± 0.6),Knee range of motion (3.1 ± 0.6),work (1.9 ± 0.4),muscular atrophy(1.7 ±0.3),auxiliaries (2.8 ±0.5),effusion(1.1 ±0.3),giving way(0.4±0.2),climb stairs (0.4 ±0.2) andtotal score (14.8 ±2.3),respectively.The scores and total scores of B(o)stman patellar fracture function after treatment in control group were pain(5.2±0.6),Knee range of motion(4.9 ±0.7),work(3.1 ±0.6),muscular atrophy (2.5 ± 0.5),auxiliaries (3.2 ± 0.4),effusion (1.3 ± 0.4),giving way (1.3±0.3),climb stairs (1.2 ± 0.4) and total score (22.7 ± 2.5),respectively.The functional scores of B(o)stman patellar fracture in control group were significantly increased (t =13.26,12.44,10.16,8.17,3.38,3.22,15.14,11.31,14.13,all P < 0.05).The scores and total scores of B(o)stman patellar fracture in observation group after treatment were significantly higher than those in control group (t =3.64,6.34,3.39,6.93,4.73,5.28,4.35,4.23,9.89,P < 0.05).In observation group,the range of knee joint activity at different time were before operation (30.2 ± 6.1) °,2 weeks after operation (85.6 ± 6.8) °,4 weeks after operation (100.6± 7.5) °,6 weeks after operation (118.5 ± 8.3) °,8 weeks after operation (138.9 ± 8.2) °,respectively.In control group,the range of knee joint activity at different time werebefore operation (29.3 ±7.2) °,2 weeks after operation (74.8 ± 6.9) °,4 weeks after operation (92.8 ± 7.8) °,6 weeks after operation(102.8 ± 9.4) °,8 weeks after operation (121.1 ± 7.3) °,respectively.The range of knee joint activity of two groups were significantly increased with the duration of treatment,Fgroup =124.58,P <0.05.The increases of range of knee joint activity in observation group was significantly higher than that in control group,Fintersecrion =11.78,P<0.05.The overall levels of range of knee joint activity in observation group were significantly higher than that of control group,and Fbetween =36.27,P< 0.05.The KSS scores of in observation group werebefore operation (40.5 ± 8.8),2 weeks after operation (66.4 ± 9.0),4 weeks after operation(76.8±9.1),6 weeks after operation (83.4 ±9.5) and 8 weeks after operation (89.4 ± 8.1),respectively.The KSS scores in control group at different time were before operation(38.9 ±9.2),2 weeks after operation (60.1 ± 8.3),4 weeks after operation (70.4± 8.2),6 weeks after operation (77.6± 7.3) and 8 weeks after operation(82.5±8.6) respectively.The KSS score of two groups were significantly increased with the duration of treatment,Fgroup =84.32,P<0.05.The increases of KSS score in observation group were significantly higher than that in control group,Fintersecrion =8.94,P<0.05.The overall levels of range of KSS score in observation group were significantly higher than that of control group,and Fbetween =28.52,P <0.05.Conclusion The application of lower limb feedback training system after arthroscopic minimally invasive Kirschner wire tension band fixation can promote the recovery of knee joint function in patients with patellar fracture.

ABSTRACT

Objective To study the therapeutic effect of rheum(Chinese herbal medicine) preparation made by using ultrasonic technique on pro-inflammatory cytokines and sepsis in rats.In order to offer novel measure for the treatment of critically ill patients.Methods Firstly, rheum sterile solution was prepared through ultrasonic technique.Secondly, fifty healthy male SD rats were randomly(random number) divided to CLP group and rheum group.Moderate degree of sepsis model was established by using cecal ligation and puncture(CLP).Rats in group rheum received the liquid rheumpreparation via intragastric administration, while rats in group CLP received saline instead.The 7-day survival rate was recorded and was compared between two groups.In addition, another fifty-four rats were randomly(random number) divided to sham group, CLP group and rheum group(n=18 in each group).CLP was performed to induce sepsis in CLP group and rheum group.Then rats in rheum group received rheum sterile solution via intragastric administration, while rats in CLP group received saline instead.At 12 hours, 24 hours and 48 hours after modeling, six rats in each group were randomly sacrificed.Serum TNF-α and HMGB1 levels were detected by ELISA method.Levels of RAGE, HMGB1 and NF-κB P65 in small intestine were detected by Western Blot.Results Level of anthraquinones extracted from rheum by ultrasonic technique was higher than that by conwentional decoction method.The 7-day survival rate of rats in rheum group(76%) was higher than that in CLP group(48%)(P0.05).At 24 hours and 48 hours after modeling, serum HMGB1 levels were significantly lower in rheum group than those in CLP group(P<0.05).Compared with sham group, protein levels of HMGB1, RAGE and NF-κB in small intestine were elevated in CLP group and rheum group at 48 hours after modeling(P<0.01), while protein levels of above biomarker were higher in CLP group than those in rheum group(P<0.05).Conclusions Rheum sterile solution could down-regulate the level of pro-inflammatory cytokines, modulate the inflammatory response, and improve the survival rate in rats with sepsis.

ABSTRACT

Objective: :To establish a quality standard for glycyrrhizic acid in compound loquat and pentoryverine granule.Methods: TLC was applied in the qualitative detection of glycyrrhizic acid.An HPLC method was used for the quantitative determination of glycyrrhizic acid.A C 18 (250 mm× 4.6 mm ,5 μm)column was used.The mobile phase was a mixture of methanol-0.2 mol·L-1 ammonium acetate-acetic acid(60∶39∶1).The flow rate was at 1.0 ml·min-1 , and the detection wavelength was at 250 nm.Results: The TLC spots were clear and well separated without interference from the negative sample.The calibration curves were linear within the range of 0.01-1.01 g·L-1 (r=1.000 0).The average recovery was 103.2% (RSD %=1.8% , n =9).Conclusion: The method is simple and accurate, which can be used for the quality control of compound loquat and pentoryverine granule.

ABSTRACT

Objective To explore the efficacy and safety of sequential telbivudine ( LDT) therapy following interferon-α( IFN-α) in hepatitis B e antigen (HBeAg) positive chronic hepatitis B patients.Methods 94 chronic hepatitis B patients in our hospital hepatitis clinic archives from August 2008 to September 2013 were divided into two groups, 45 patients treated with LDT following IFN-αdue to unsatisfactory response to peginterferon-α-2a (peg-IFN-α-2a)treatment in the sequential group and 49 patients treated with LDT initially in the controlled group.In the sequential group, 22 patients failed to respond to IFN-αtreatment, 4 patients obtained IFN-resistance, 6 patients developed severe side effects, 1 patients had pre-C mutation and 12 patients switched to LDT due to economic factors or fertility requirement.The biochemical response and virological response, virological breakthrough and serological response were observed.Chi-square test was adapted for data analysis.Results After 32.35 months of LDT therapy, 88.9%(40/45) patients had with HBV-DNA <100 IU/mL in the experiment group compared with 44.9%(22/49) patients in the control group (P<0.05).The rates of HBeAg loss and HBeAg seroconversion in the experiment group were 53.3%(24/45) and 51.1%(23/45) respectively, which were significantly higher than that of the control group, 24.5%(12/49) and 24.5% (12/49) (P<0.05).6.7% (3/45) patients developed virological breakthrough in the sequential group compared with 34.7%(17/49) in the control group (P<0.05).Above results suggested the synergistic antiviral activity between peg-IFN-α-2a and LDT.Conclusion Sequential LDT therapy following peg-IFN-α-2a is better efficacy and safety compared with treating patients with LDT alone initially.

ABSTRACT

Objective:To establish a method to determine benzoic in ibuprofen suspension. Methods:The determination of ben-zoic was performed on a Kromasil C18 (250 mm × 4. 6 mm, 5 μm) column with the mobile phase consisting of 0. 05 mol·L-1 monopo-tassium phosphate and acetonitrile (72 ∶28). The detection wavelength was 235 nm and the column temperature was 30℃. The injec-tion volume was 10μl. Results:The limit of detection was 3. 57 ng and benzoic had a good linear relationship within the range of 12. 5-200. 0 μg·ml-1(r=0. 999 9). The solution was stable in 24 hours. The average recovery was 98. 42% and RSD was 0. 98% (n=9) . Conclusion:The method is simple, repeatable and accurate, and can be used for the determination of benzoic in ibuprofen sus-pension.

ABSTRACT

Objective:To study the effects of Radix Angelicae Sinensis ( RASI) on expression of airway MUC5AC and related inflammatory factors in asthmatic mice with Yin deficiency syndrome.Methods:Injecting ovalbumin ( OVA) to sensitize,inhaling OVA to stimulate,using Thyroxin during late stimulation,the asthmatic mouse with Yin deficiency syndrome was established and evaluated through asthmatic behaviors, lung histopathology, active factors ( IL-13, TNF-αand MUC5AC ) in broncho-alveolar lavage fluid (BALF), and MUC5AC expression in lung tissue.Results: 2,4,8 g/kg RASI could reduce asthmatic behaviors score, relieve pathological changes of lung tissue,reduce the contents of IL-13,TNF-αand MUC5AC in BALF,and depress MUC5AC expression in lung tissue ( P<0.05,0.01).In addition,there was a certain synergy between RASI and dexamethasone ( DXM) on depressing the ex-pression of IL-13 and MUC5AC (P<0.05).Conclusion:RASI has certain anti-asthma effect and one of mechanisms is to regulate the MUC5AC expression through inhibit IL-13 and TNF-α.On the expression of IL-13 and MUC5AC,the compatibility of RASI with glu-cocorticoid has some synergy effect.

ABSTRACT

Objective To investigate the expression of Chemerin in cervical intraepithelial neoplasia(CIN) and cervical cancer and its relation with clinicopathological characteristics.Methods The expression of Chemerin was determined with enzyme-linked immunosorbent assay (ELISA) in 35 cervical cancer cases,20 cervical intraepithelial neoplasia cases and 20 normal persons.The association of serum level of Chemerin with clinicopathological charactersistics was analyzed.Results The level of Chemerin was the highest in cervical cancer cases [(2113.35 ± 198.64) ng/L],higher in CIN [(1562.78 ± 158.65) ng/L],and low in normal persons [(946.36 ± 113.57) ng/L],showing significant difference among three groups.The serum level of Chemerin in cervical cancer were positively correlated with Federation International of Gynecology and Obstetrics (FIGO) stage and lymph node metastasis (P < 0.05).Serum Chemerin levels were increased with clinical FIGO stage and showed significantly positive correlation with FIGO stage (rs =0.92,P =O.000).Conclusions The expression of Chemerin in patients with cervical carcinoma is increased.Chemerin may be used as reference indicator to predict its progression in cervical carcinoma.

ABSTRACT

Objective To investigate the protection of tea polyphenols (TP) against pulmonary ischemia-reperfusion injury in rats. Methods A total of 100 rats were randomly devided into five groups:the sham operation group, the model control group, the high-, medium-and low-dose TP groups. Two hours after reperfusion, the PO2 and the lung tissue wet/dry (W/D) ratio were examined; the apoptosis index (AI) was analysised;the activity of SOD, CAT and the content of MDA in lung tissue were determined. Results Compared with the model control group, the W/D (5.7 ± 0.4, 5.5 ± 0.4 vs. 6.5 ± 0.5) and the AI (19.3%± 1.8%, 14.2%± 1.4%vs. 31.2%± 2.4%) of the high-, medium-dose groups were significantly decreased (P<0.05 or P<0.01);the activity of SOD (115.2 ± 18.1 U/mg, 128.7 ± 20.9 U/mg vs. 94.7 ± 12.1 U/mg), CAT (1.3 ± 0.2 U/mg, 1.7 ± 0.4 U/mg vs. 0.9 ± 0.2 U/mg) in lung tissue of the the high-, medium-dose groups groups were significantly increased (P<0.05 or P<0.01), and the content of MDA (1.1 ± 0.3 nmol/L, 1.0 ± 0.3 nmol/L vs. 1.6 ± 0.5 nmol/L) were significantly decreased (P<0.05 or P<0.01). The PO2 (93.4%± 4.0%vs. 85.9%± 3.4%) of the high-dose group were significantly increased compared to the model group (P<0.01). Conclusion TPs could effectively lower the W/D, increase the PO2, inhibit the hepatocyte apoptosis, lower the AI, suggesting that TPs had protective effects against pulmonary ischemia-reperfusion injury in rats, which perhaps related to its effects of improving antioxidant ability and inhibiting the oxidative stress.

ABSTRACT

Objective To establish and evaluate a BALB/c mouse model of anaphylactic asthma with yin-deficiency syndrome. Methods Ovalbumin (OVA) was injected to sensitize and was inhaled to stimulate to replicate the BALB/c mouse model of anaphylactic asthma. Thyroxin was used for gavage during late stimulation to replicate the BALB/c mouse model of asthma with yin-deficiency syndrome. On the basis of monitoring food and water intake and body weight, asthma related indexes, such as asthmatic behaviors, respiratory function, lung histopathology, were detected, and yin-deficiency syndrome related indexes, such as cAMP and cGMP, and pulmonary fluid clearance were examined. Results The BALB/c mouse model of asthma with yin-deficiency syndrome showed obvious asthma symptoms; comprehensive scores of asthmatic behaviors increased significantly;inhaling peak flow, exsufflation peak flow, and tidal volume decreased significantly; lung tissue histopathology showed obvious inflammatory response. Meanwhile, food and water intake of BALB/c mouse model of asthma with yin-deficiency syndrome increased significantly;body weight increased slowly;wet/dry ratio of lung tissue decreased;cAMP and cAMP/cGMP increased, while cGMP decreased, which showed that the mice were in the condition of yin-deficiency and yang-excess. Conclusion Combining thyroxin at the basis of sensitivity induced by OVA is a good pattern to successfully replicate the BALB/c mouse model of anaphylactic asthma with yin-deficiency syndrome.

ABSTRACT

Objective To explore the biological function of rhoptry protein 38(ROP38)of Toxoplasma gondii,and to iden?tify the reactogenicity of the recombinant protein(rROP38). Methods The ROP38 was amplified by RT?PCR from T. gondii RH strain,and was cloned into prokaryotic expression vector pET?28a(+). The recombinant plasmid was transformed into E. co?li BL21(DE3)competent cells. Then the rROP38 was analyzed by SDS?PAGE and identified by Western blot. Results SDS?PAGE showed that rROP38 was efficient expression with a molecular weight of about 43 kD. Western blot showed that rROP38 reacted with antibody of His tag or human positive antibody,which indicated that ROP38 had good reactogenicity and could be a serological diagnostic antigen. Conclusion The study successfully obtains the rROP38 of T. gondii with good reactogenicity.

ABSTRACT

Objective To compare analgesic and anti-inflammatory effects of Gentianopsis Paludosa and Sophora Alopecuroide of different kinds of compatibility on the mice.Methods Mice were randomly divided into normal control group, positive medicine group, and different kinds of compatibility groups, 10 mice in each group. Mice received gavage for successive 7 days, once a day. Glacial acetic acid twist body method and hot board to pain method were used to observe and compare analgesic effects of different kinds of compatibility. Xylene to ear swelling method and agar to granulation swollen method were used to observe and compare inflammatory effects of different kinds of compatibility.Results Compared with normal group, compound Gentianopsis Paludosa had obvious analgesic and anti-inflammatory effects (P<0.01). When the ratio of Gentianopsis Paludosa and Sophora Alopecuroide was 20:0.6 and 20:0.9, the analgesic and anti-inflammatory effects were the best.Conclusion Gentianopsis Paludosa and Sophora Alopecuroide have good analgesic and anti-inflammatory effects, among which, when the ratio of Gentianopsis Paludosa and Sophora Alopecuroide is 20:0.6, the best effects show.

ABSTRACT

Objective To investigate the prevalence of tuberculosis among silicosis patients and silica exposure patients,and to analysis the risk factors of tuberculosis among these population.Methods A total of 1 227 silica exposure patients from Wenling,Zhejiang were enrolled in this field study.Basic demographic information was collected and chest X-ray was taken for each patient.Sputum was collected for Mycobacterium tuberculosis culture and strain identification. In univariate analysis,t test was performed for continuous variables andχ2 test for categorical variables.In multivariate analysis,the odds ratio (OR )was calculated along with a 95 % confidence interval (CI )by binary Logistic regression. Results A total of 1 204 silica exposure patients had full basic information and 99.8% were male patients with mean age of (59.4 ± 6.8 )years.The patients in phase 0 + to phase Ⅲ were 172 (14.3%),255 (21 .2%),160 (13.3%)and 617 (51 .2%),respectively.The tuberculosis prevalence rate was about 7.3% among these population.The risk factors for tuberculosis including phase Ⅱ silicosis (OR =2.96, 95 %CI :1 .05 -8.32,P =0.04)and phase Ⅲ silicosis (OR=3.88,95 %CI :1 .58-9.56,P <0.01),and contacting with tuberculosis patients (OR=4.14,95 %CI :1 .91 -8.98,P <0.01).Patients complicated with tuberculosis lacked specific symptoms,but fever and weight loss were more frequent.Conclusion Tuberculosis is highly prevalent in silicotic patients,especially in patients with phase Ⅱ/Ⅲ silicosis and in patients with tuberculosis contact history.

ABSTRACT

Objective To investigate the recent effect of full arthroscopic dual-beam reconstruction of the posterior cruciate ligament(PCL)using tibial Inlay technique.Methods From March 2007 to September 2009,17 PCL injured patients underwent full arthroscopic dual-beam PCL reconstruction using Inlay technique,including 16 males and 1 female,with an average age of 25 years(range,19-54).Of all cases,Lysholm score was(53.4±2.1)points,International Knee Documentation Committee(IKDC)rated C in 7,D in 10,and posterior drawer test(+)in 17.We used self-designed tibia tunnel drill system to produce the deep-limited bone tunnel.Follow-up began at 12 months after operation.Evaluate Lysholm knee score,IKDC rating,and posterior drawer test to compare the knee stability with that of preoperative.Observe the location of the bone block and healing by checking knee X-ray and spiral CT scan.Results Seventeen patients were followed up between 12 to 28 months,with an average of 17.8 months.In the last follow-up study,Lysholm score(93.5±1.7)points compared with that of preoperative was statistically significant different(P=0.016).IKDC rating of A grade in 15 cases,B in 2,compared with that of preoperative was statistically significant different(P=0.021).Posterior drawer test were negative in 15 cases,slight positive in 2.The X-ray and spiral CT scan showed the location of the bone block were perfect and healed well.Conclusion We can accurately produce the deep-limited bone tunnel by the tibia tunnel drill system with minor trauma,and the recent clinical effects of PCL reconstruction were pretty good.

ABSTRACT

Objective To investigate the effect of individualized therapeutic programs with combination of interferon and ribavirin (RBV) in chronic hepatitis C (CHC) and study the influential factors of virological response rates.Methods A total of 139 patients with CHC were enrolled and given the intensive treatment doses of interferon and RBV according to their basic clinical condition.At the treatment of 0,4,12,24 weeks,the end of treatment and 24 weeks after treatment stop,the serum HCV RNA was determined.Timely adjustment to dosage and time periods was made according to the virological response to treatment,and the predictive value of rapid virological response (RVR) and complete early virological response (cEVR) for sustained virological response (SVR) were analyzed.Results At the 4th week of treatment,the level of serum HCV RNA was monitored in 120 patients,and 84.2% (101/120) of patients obtained RVR; among them,90.7% (88/97) obtained SVR.The virus load of patients obtained RVR at pretherapy was lower than that of patients didn't obtained RVR [(5.883±1.246) lg copies/ml vs (6.502±0.693)lg copies/ml,P =0.034].The RVR rate of initial treatment patients with PEG-IFNα-2a [87.8%(79/90)]was significantly higher than that of retreatment patients with PEG-IFNα-2a [65.0% (13/20)](P =0.031).At the 12th week of treatment,the level of serum HCV RNA was monitored in 132 patients,and 92.4% (122/132) of patients obtained cEVR; among them,90.8% (108/119) obtained SVR.The SVR rate of patients obtained cEVR was significantly higher than that of patients didn't obtained cEVR (5/9) (P =0.007).There was no significant difference between the cEVR rate of initial treatment patients [94.7% (90/95)]and retreatment patients [85% (17/20)]with PEG-IFNα-2a (P =0.158).Conclusions cEVR was predictor of SVR.Individualized therapy can increase the obtaining probability of RVR,cEVR and SVR.Adjusting drug dose timely and extending treatment period of HCV RNA-negative based on virological response to treatment are important in CHC individualized therapy.

ABSTRACT

The human Gadd45 protein family plays critical roles in DNA repair, negative growth control, genomic stability, cell cycle checkpoints and apoptosis. Here we report the crystal structure of human Gadd45γ [corrected], revealing a unique dimer formed via a bundle of four parallel helices, involving the most conserved residues among the Gadd45 isoforms. Mutational analysis of human Gadd45γ [corrected] identified a conserved, highly acidic patch in the central region of the dimer for interaction with the proliferating cell nuclear antigen (PCNA), p21 and cdc2, suggesting that the parallel dimer is the active form for the interaction. Cellular assays indicate that: (1) dimerization of Gadd45γ [corrected] is necessary for apoptosis as well as growth inhibition, and that cell growth inhibition is caused by both cell cycle arrest and apoptosis; (2) a conserved and highly acidic patch on the dimer surface, including the important residues Glu87 and Asp89, is a putative interface for binding proteins related to the cell cycle, DNA repair and apoptosis. These results reveal the mechanism of self-association by Gadd45 proteins and the importance of this self-association for their biological function.

ABSTRACT

Objective To discuss the expressions of NF-κB and Vimentin in bladder urothelium carcinoma and their significance. Methods 60 bladder urothelium carcinomas and 20 controls were evaluated and the presence of NF-κB and Vimentin were examined by inmunohistochemical SP method.Results Both NF-κB and Vimentin in bladder urothelium carcinoma and nomal bladder tissue had significant differences (x2 =21.8,P < 0.01; x2 =10.45,P < 0.01).The positive rates of NF-κB expression in different grade of bladder urothelium carcinoma:grade Ⅰ 32 % (5/16),grade Ⅱ 68 % (13/19),grade Ⅲ 96 %(24/25) (x2 =4.8,P < 0.05,x2 =4.24,P < 0.05).The positive rates of NF-κB expression in different stage:Ta-T1 stage 57 % (16/28),T2-T4 stage 81% (26/32) (x2 =4.13,P < 0.05).The positive rates of NF-κB expression in lymph node metastasis or not:87 % (21/24) and 58 % (21/36) (x2 =5.83,P < 0.05).The positive rates of Vimentin expression in different grade of bladder urothelium carcinoma:grade Ⅰ 19 %(3/13),grade Ⅱ 53 % (10/19),grade Ⅲ 84 % (21/25) (x2 =4.25,P < 0.05,x2 =4.94,P < 0.05).The positive rates of Vimentin expression in different stage:Ta-T1 stage 36 % (10/28),T2-T4 stage 75 %(24/32)(x2 =9.38,P < 0.05).The positive rates of NF-κB expression in lymph node metastasis or not:50 % (12/24)and 61% (22/36) (x2 =0.72,P > 0.05).The expression of NF-κB increased was positively correlated with expression of Vimentin (x2 =12.4,P < 0.005,r =0.42).Conclusion Both NF-κB and Vimentin in bladder urothelium carcinoma and normal bladder tissue have significant difference.The expression of NF-κB is positively correlated with tumor grade,clinical stage and lymph node metastasis,but not with age and sex.The expression of Vimentin is positively correlated with tumor grade,and clinical stage,but not with age,sex and lymph node metastasis.The expression of NF-κB is positively correlated with the expression of Vimentin.
