ABSTRACT

Tropheryma whipplei (TW) is an opportunistic pathogen that can cause Whipple′s disease (WD). Recently, the biomedical field is concerned about whether the bacteria cause pneumonia. When BALF was detected by PCR, the TW positive rate was about 6.1%. Etiology status is generally described as “associated” or “probable”. Immunocompromised status is the susceptible factor; Different species of the same genus have been reported. The current method may have missed detection. All relevant information needs further study. This article reviews the acute pneumonia caused by TW and the detection technology.

ABSTRACT

Objective:To explore the potential molecular mechanism of Nelumbinis Plumula alkaloids (NAPs) in the prevention and treatment of non-small cell lung cancer (NSCLC) based on network pharmacology and cell experiment. Method:The main active components of NAPs were obtained by searching Traditional Chinese Medicine System Pharmacology Database and Analysis Platform (TCMSP) and Bioinformatics Analysis Tool for Molecular Mechanism of Traditional Chinese Medicine (BATMAN-TCM), and their main targets were predicted and analyzed by employing Swiss Target Prediction. The main target genes of NSCLC were retrieved from GeneCards, Online Mendelian Inheritance in Man (OMIM) and DrugBank databases. The resulting common targets were imported into STRING platform for constructing the protein-protein interaction (PPI) network, followed by gene ontology (GO) and Kyoto Encyclopedia of Genes and Genomes (KEGG) enrichment analysis based on Database for Annotation, Visualization, and Integrated Discovery (DAVID). The NAPs-common target -pathway network was constructed by Cytoscape 3.7.1. After NSCLC cell line A549 was treated with isoliensinine, the cell morphology was observed under an inverted fluorescence microscope. The effect of isoliensinine on A549 vitality was detected by cell counting kit-8 (CCK-8) assay and the target protein changes were verified by Western blot. Result:The main active components for NAPs against NSCLC were lysicamine, liensinine, and isoliensinine. The phosphatidylinositol-3-kinase-protein kinase B (PI3K-AKT), RAS-related protein 1 (Rap1), epidermal growth factor family of receptor tyrosine kinases (ErbBs), and hypoxia inducible factor-1 (HIF-1) pathways were mainly involved for binding adenosine triphosphate (ATP) and regulating protein kinase activity. The main targets included protein kinase B-1 (AKT1), alpha catalytic subunit of phosphoinositol-3-kinase (PIK3CA), cyclin-dependent kinase 2 (CDK2), mitogen-activated protein kinase-1 (MAPK1), epidermal growth factor receptor (EGFR), adenosine triphosphate-binding cassette B1 (ABCB1), mammalian target of rapamycin (mTOR), tyrosine kinase (Src), Janus kinase 1 (JAK1), and G1-phase-specific gene cyclin-D<sub>1</sub> (CCND1). The <italic>in vitro</italic> cell experiment also revealed that isoliensinine down-regulated the expression of phosphorylated AKT (p-AKT) and phosphorylated mTOR (p-mTOR) in a concentration- and time-dependent manner and inhibited the growth of A549 cells. Conclusion:NAPs exert the preventive and therapeutic effects against NSCLC through multiple components, multiple targets, and multiple pathways, especially the PI3K-AKT pathway.

ABSTRACT

According to human carboxylesterase 2(hCE2) inhibitors reported in the literature, the pharmacophore model of hCE2 inhibitors was developed using HipHop module in Discovery Studio 2016. The optimized pharmacophore model, which was validated by test set, contained two hydrophobic, one hydrogen bond acceptor, and one aromatic ring features. Using the pharmacophore model established, 5 potential hCE2 inhibitors(CS-1,CS-2,CS-3,CS-6 and CS-8) were screened from 20 compounds isolated from the roots of Paeonia lactiflora, which were further confirmed in vitro, with the IC_(50) values of 5.04, 5.21, 5.95, 6.64 and 7.94 μmol·L~(-1), respectively. The results demonstrated that the pharmacophore model exerted excellent forecasting ability with high precision, which could be applied to screen novel hCE2 inhibitors from Chinese medicinal materials.

ABSTRACT

@#AIM: To explore the mechanism of rejection of bioengineered keratoplasty by the laser confocal microscope and immunopathological studies.<p>METHODS: Retrospective case study. Five patients(5 eyes)who underwent bioengineered keratoplasty for infectious keratitis from Sep.2018 to Dec.2020 and were immunologically rejected included. All observers were monocular. Slit-lamp was used to examine corneal transparency and corneal neovascularization. The cornea was dynamically observed under laser confocal microscopy, the density of subepithelial langerhans cells and stromal inflammatory cells was recorded after graft rejection, and the contralateral eye was used as the control group for statistical analysis. The infiltration and distribution of CD4<sup>+</sup> cells, CD8<sup>+</sup> cells and inflammatory cells were observed after immunohistochemical staining of bioengineered corneas during corneal allograft.<p>RESULTS: Confocal laser microscopy showed that a large number of langerhans cells were activated under the corneal epithelium of the intraoperative eye, and the activation ratio was significantly different from that of the contralateral eye(χ<sup>2</sup>=38.29, <i>P</i><0.001). The stromal layer was rich in inflammatory cells, which was significantly different from that of the contralateral eye(<i>t</i>=32.5, <i>P</i><0.05). Immunohistochemical staining and HE staining were performed on the bioengineering corneal tissue. CD4<sup>+</sup> and CD8<sup>+</sup> cells were observed in all corneal stromal layers, and only one case had a large number of eosinophils.<p>CONCLUSION: Immunopathology of rejection after bioengineered keratoplasty confirmed that T-cell-mediated cellular immunity plays an important role in the pathogenesis, and hypersensitivity to xenoantigen may be involved in it.

ABSTRACT

Objective:To observe the influence of SBAR communication mode on the condition of patients undergoing thoracolaparoscopic combined with esophageal cancer resection.Methods:A total of 200 patients who were admitted to the Department of Gastroenterology and Oncology from October 2017 to October 2019 in Affiliated Hospital of Southwest Medical University were selected for elective thoracolaparoscopic combined with esophageal cancer resection. According to the random number method, they were divided into 100 cases in the control group and the SBAR group. The control group used conventional preoperative communication visit mode, and the SBAR group used SBAR communication mode and preoperative visit based on the control group. The following items such as the preoperative negative mentality scores, quality of life score, postoperative complications and nursing satisfaction rate of the two groups before and after nursing interventionwere compared.Results:Before the intervention, the difference of the Self-rating Anxiety Scale (SAS) and Self-rating Depression Scale (SDS) scores of the two groups was not statistically significant ( P>0.05). After the intervention, the SAS and SDS scores of the two groups were 49.32±4.96, 55.95±5.32, 50.93±4.74, and 56.33±5.89, respectively, which were significantly reduced. The SDS and SAS scores of the SBAR group were lower than those of the control group ( t values were -9.115 and -7.142, P<0.001). The difference of the World Health Organization Brief Table on Quality of Life (WHOQOL-BREF) scores of the two groups before the intervention was not statistically significant ( P>0.05). After the intervention, the WHOQOL-BREF scores of the two groups were 12.27±1.39 vs. 9.18±1.24, 16.13±1.51 vs. 12.48±1.22, 16.13±1.95 vs. 12.71±1.28, 15.09±1.81 vs. 13.08±1.48, 18.11±1.82 vs. 13.51±1.47, 14.09±1.61 vs. 12.98±1.28. The scores of the SBAR group were higher than those of the control group ( t values were 5.397-19.662, P<0.001). The postoperative complications rate of the SBAR group was 3.00% (3/100), which was less than 18.00% (18/100) of the control group ( χ 2 value was 11.971, P<0.001). The overall satisfaction rate of patients in the SBAR group with care was 100.00% (100/100), which was higher than the 84.00% (100/100) in the control group ( χ 2 value was 17.391, P<0.001). Conclusions:In patients with esophageal cancer undergoing thoracic laparoscopy combined with esophageal cancer resection, the use of SBAR communication mode for the patient′s preoperative visit can significantly reduce the patient′s preoperative anxiety and depression negative scores and improve the patient′s quality of life, reduce the occurrence of postoperative complications, improve patient satisfaction with nursing with ideal effect.

ABSTRACT

Due to the complicated anatomical structures in the furcation area of multirooted mandibular first molars, dental hygiene is greatly compromised once the furcation is involved in the periodontitis, leading to the unfavorable prognosis of teeth with furcation involvement. A patient came to a dental office with the chief complaint of "mobile mandibular posterior tooth" 27 years ago. The periapical film showed alveolar bone resorption at the root furcation of the right mandibular first molar. Flap surgery and fine supportive therapy were conducted. The patient was diagnosed with "furcation involvement Class Ⅲ" during a revisit three years ago. Satisfactory and healthy periodontal statuses were observed 2, 9, 24, and 33 months after the periodontal flap surgery plus tunneling procedures. A follow-up of 27 years in the present case demonstrated that a favorable prognosis of furcation involvement can be achieved after adequate periodontal treatment.

OBJECTIVE@#To detect and analyze coagulation related indexes and genotypes of a patient with congenital fibrinogen deficiency and his family members, and to investigate the possible molecular pathogenesis.@*METHODS@#Four peripheral blood samples (proband and 3 family members) were collected and the prothrombin time (PT), activated partial thromboplastin time (APTT), thrombin time (TT), fibrinogen (Fg), D-Dimer and eight coagulation factor indicators were detected. All exons and flanking sequences of the FGA, FGB, and FGG genes encoding the three peptide chains of fibrinogen were sequenced and analyzed by bioinformatics.@*RESULTS@#Among the eight coagulation factors of the proband and the elder sister, F Ⅴ and F Ⅷ were slightly higher, TT was significantly prolonged, and Fg was significantly reduced. Sequencing results showed that c.901C>T heterozygous mutation existed in the FGG gene. Bioinformatics analysis showed that the mutation changed the original protein structure and reduced the number of hydrogen bonds.@*CONCLUSION@#The fibrinogen gamma chain c.901C>T heterozygous mutation is the main cause of congenital fibrinogen deficiency in this family. This mutation is reported for the first time at home and abroad.

ABSTRACT

Collagen α3 (IV) chains are one of the major constituent components of the basement membrane in the mammalian testis. Studies have shown that biologically active fragments, such as noncollagenase domain (NC1)-peptide, can be released from the C-terminal region of collagen α3 (IV) chains, possibly through the proteolytic action of metalloproteinase 9 (MMP9). NC1-peptide was shown to promote blood-testis barrier (BTB) remodeling and fully developed spermatid (e.g., sperm) release from the seminiferous epithelium because this bioactive peptide was capable of perturbing the organization of both actin- and microtubule (MT)-based cytoskeletons at the Sertoli cell-cell and also Sertoli-spermatid interface, the ultrastructure known as the basal ectoplasmic specialization (ES) and apical ES, respectively. More importantly, recent studies have shown that this NC1-peptide-induced effects on cytoskeletal organization in the testis are mediated through an activation of mammalian target of rapamycin complex 1/ribosomal protein S6/transforming retrovirus Akt1/2 protein (mTORC1/rpS6/Akt1/2) signaling cascade, involving an activation of cell division control protein 42 homolog (Cdc42) GTPase, but not Ras homolog family member A GTPase (RhoA), and the participation of end-binding protein 1 (EB1), a microtubule plus (+) end tracking protein (+TIP), downstream. Herein, we critically evaluate these findings, providing a critical discussion by which the basement membrane modulates spermatogenesis through one of its locally generated regulatory peptides in the testis.

ABSTRACT

Placenta, the direct and vital connection between mother and fetus, can be considered as an inflammatory organ that plays an essential role in maternal-fetal immune tolerance in maintaining pregnancy and mediating inflammation in some gestational complications. When gestational diabetes mellitus (GDM) occurs, the mother and placenta are in a state of mild inflammation. We hypothesize that the placental immune cells may participate in the pathogenesis of GDM. This review discusses the relation between placental immune cells and GDM from three aspects: the regulatory function of the placental immune cells, the association between abnormal placental immune cells and GDM, and the function of placental immune cells during clinical management of GDM.

ABSTRACT

Objective:To investigate the relationship between the methylation of apelin receptor early endogenous ligand ( APELA) gene promoter and preeclampsia. Methods:A systematic review was conducted to evaluate the cg02779075 locus methylation in APELA gene associated with preeclampsia in six previous studies on placenta genome-wide methylation based on the GEO gene expression database from 2007 to 2017. After testing the heterogeneity, the random-effects model was applied for meta-analysis. Placenta samples of 17 preeclamptic patients and 24 healthy gravidas were retrospectively collected in Obstetrics and Gynecology Hospital of Fudan University from 2008 to 2010. MassARRAY was used to quantify the methylation level of CpGs, and the expression of APELA gene was determined by qRT-PCR. All data were analyzed by t test or Mann-Whitney U test. Results:(1) Analysis of the six genome-wide methylation studies showed significant heterogeneity ( I2=0.64, P=0.016) and the meta-analysis using random-effects model showed that the methylation of cg02779075 locus in the placenta samples of preeclamptic women was down-regulated significantly ( Pmeta=6.7×10 -6). (2) Placenta tissue analysis revealed that the methylation of CpG1 [0.12 (0.00-0.25) vs 0.21 (0.09-0.33), U=-2.569] and CpG2 [0.07 (0.01-0.14) vs 0.17 (0.09-0.34), U=-4.160] in patients with preeclampsia was significantly reduced compared with those in healthy control (both P<0.05). The expression of APELA gene in the placenta was up-regulated in patients with preeclampsia, but no significant difference was observed between the two groups ( U=0.891, P=0.384). Conclusions:Aberrant methylation of APELA gene promoter was detected in the placentas of patients with preeclampsia.

BACKGROUND: Blood oxygenation level dependent functional MRI (BOLD-fMRI) has proven to be a powerful tool for studying the functional change of the brain. In task-state fMRI study, the functional reorganization of sensory and motor cortex has been observed in patients with cervical spondylotic myelopathy. OBJECTIVE: To discuss the correlations between task-state fMRI measurements with clinical symptoms and surgical outcomes of cervical spondylotic myelopathy. METHODS: Eighty-two patients with cervical spondylotic myelopathy undergoing posterior cervical decompression (cervical spondylotic myelopathy group) and forty-five healthy volunteers (normal group) were recruited from January 2018 to January 2019. All subjects underwent fMRI and performed a finger-tapping paradigm with the right hand. The Japanese Orthopaedic Association score was used to evaluate the function of the spinal cord. Japanese Orthopaedic Association score recovery rate less than 50% was defined as a poor result. RESULTS AND CONCLUSION: (1) Japanese Orthopaedic Association score was significantly improved after surgery in the cervical spondylotic myelopathy group compared with that before surgery (P 0.05). Before surgery, VOA ratio (left precentral gyrus/left postcentral gyrus) was significantly higher in the cervical spondylotic myelopathy group than in normal group (P 0.05). VOA ratio was significantly decreased compared with that preoperatively (P < 0.05). (4) Correlation analysis revealed that the VOA in the left precentral gyrus and left postcentral gyrus and VOA ratio were significantly correlated with preoperative Japanese Orthopaedic Association score and postoperative Japanese Orthopaedic Association score recovery rate (P < 0.05). The absolute value of correlation coefficient of VOA ratio with preoperative Japanese Orthopaedic Association score and postoperative recovery rate was largest. (5) Receiver operating characteristic curve analysis showed that the area under the curve value for the VOA ratio was 0.803, indicating strong predictive discrimination, and the cut-off value was 3.621. The area under the curve value for Japanese Orthopaedic Association score was 0.751, and the cut-off value was 8. The predictive effect of VOA ratio was higher than Japanese Orthopaedic Association score. (6) The results indicate that the VOA in the left precentral gyrus and left postcentral gyrus was negatively correlated with severity of clinical symptoms (Japanese Orthopaedic Association score). Preoperative VOA ratio can effectively predict the recovery of spinal cord function after operation in patients with cervical spondylotic myelopathy.

ABSTRACT

Objective: To synthesize 8-substituted quercetin derivatives and test their cytotoxicity on human cancer cell lines, so as to find out hit or lead compounds via structure activity relationship(SAR)analysis. Methods: Using rutin as the raw material, quercetin was obtained by the acid hydrolysis of rutin, and the 8-substituted quercetin derivatives, 3a-3h, were synthesized via the whole hydroxyl protection of quercetin and then halogenation, followed by the Suzuki coupling or heck coupling reaction. The in vitro inhibitory activity of these derivatives was assayed by the MTT method using human cancer HepG2, HT-29 and K562 cell lines. Results and Conclusion: Eight new target compounds, 3a-3h, were synthesized, and their structures were confirmed by the 1H NMR, 13C NMR and MS data. The inhibitory effect of 3b-3h on HepG2 cell line, 3b and 3d on HT-29 cell line, and 3b-3d and 3f-3h on K562 cell line has remarkably and significantly enhanced than that of quercetin, and the present research results provide a suggestive preliminary SAR.

ABSTRACT

Objective:To explore the effect of match between childhood stress and adulthood stress on mental health status.Methods:Adult healthy volunteers( n=239) and adult mental disorder patients were examined by questionnaires or telephone interviews.Childhood trauma questionnaire (CTQ) and lifetime stress event list were used for measure of CS and AS, DSM5-level-1-cross-cutting-symptom-measure (DSM5-L1CCSM) and self-rating depression scale (SDS) for mental health outcomes.Subjects were grouped according to positive (+ ) / negative (-) stress events into five groups: CS+ AS+ Matched ( n=108), CS+ AS+ mismatched( n=240), CS-AS+ ( n=100), CS+ AS-( n=79), and CS-AS-( n=99). The data of stress and mental health status were compared and analyzed among stress groups. Results:The distributions of positive stress events among 626 volunteers were 68.2% with CS+ , 71.6% with AS+ , 17.3% with C+ A+ M+ .There were differences among groups in all parameters (all P<0.05) except for gender.Age, years of education, CS and emotional abuse were influencing factors of onset of AS( P<0.01). MANCOVA analysis showed that factors about the interaction of AS and CS had attribution on DSM5-L1CCSM and SDS ( P<0.01). Conclusion:CS has a facilitatory effect on AS.The match of CS-AS is an important risk factor of adulthood mental health outcomes.Re-experiencing the same type of CS in adulthood would worsen adulthood mental health status.

To investigate a new teaching model so as to improve the teaching and learning efficiency of pharmaceutical analysis, a total of 61 undergraduates who majored in chemistry (pharmacy) were chosen as the trainees. The teaching reform based on Aalborg PBL model was conducted from such three aspects as teaching contents, teaching manners and assessment methods. Under the new teaching model, students' learning interest could be improved significantly, and their knowledge understanding, mastery and application were enhanced. Besides, the students' abilities of team work and communication were also cultivated.

Objective::To explore the regulatory effect of Longmu Guizhi decoction and its decomposed formula on the hypothalamic-pituitary-adrenal axis of spontsneously hyoertensive rats (SHR) with attention deficit hyperactivity disorder (ADHD), so as to explain the scientific connotation of compound compatibility. Method::Young male SHR rats were randomly divided into saline control group, methylphenidate hydrochloride group(2 mg·kg-1), Longmu Guizhi decoction group(30 g·kg-1), Guizhi+ Longgu Muli decoction group(21 g·kg-1), Ganmai Dazaotang group(9.24 g·kg-1) and Polygalae Radix-Acori Tatarinowii Rhizome group(4.62 g·kg-1). The corresponding drugs were intragastrically administered for 14 days. Serum adrenocorticotropic hormone (CRH), adrenocorticotropic hormone (ACTH) and corticosterone (CORT) were detected by enzyme-linked immunosorbent assay(ELISA), and mRNA expressions of glucocorticoid receptor (GR), mineralocorticoid receptor (MR), CRH in hypothalamus and GR, MR in hippocampus were detected by real-time fluorescent quantitative polymerase chain reaction (Real-time PCR) method. Result::Serum CORT, ACTH and CRH, GR and CRH mRNA expressions of hypothalamus, GR and MR mRNA expressions of hippocampus in methylphenidate hydrochloride group and Longmu Guizhi decoction group were significantly higher than those of saline control group (P<0.05, P<0.01). MR mRNA expression of hypothalamus in Longmu Guizhi decoction group was significantly higher than that in the saline control group (P<0.05, P<0.01), and GR expression in hippocampus was significantly higher than that in the methylphenidate hydrochloride group (P<0.01). The dissection study showed that the levels of serum ACTH and CRH, MR mRNA, GR and CRH mRNA of hypothalamus, GR and MR mRNA of hippocampus in Guizhi+ Longgu Muli decoction group and Ganmai Dazaotang group were significantly higher than those in saline control group (P<0.05, P<0.01). The serum CORT level of Guizhi+ Longgu Muli decoction group was higher than that of normal saline group (P<0.01). The serum ACTH level of Polygalae Radix-Acori Tatarinowii Rhizome pair was significantly higher than that in saline control group(P<0.05). Conclusion::The mechanism of Longmu Guizhi decoction in treating ADHD is related to increase of the levels of CORT, ACTH and CRH in serum and the expressions of MR, GR, CRH in hypothalamus and GR and MR in hippocampus, so as to regulate the function of HPA axis. The order of the effect degree of each decomposition group on HPA axis function from high to low was Guizhi+ Longgu Muli decoction>Ganmai Dazao decoction>Polygalae Radix-Acori Tatarinowii Rhizome pair drug.

Objective::To observe the effect of Jianpi Qingchang decoction on hormone withdrawal in patients with hormone-dependent spleen deficiency damp-heat ulcerative colitis. Method::The 60 patients with hormone-dependent ulcerative colitis with spleen deficiency and damp-heat were selected and collected from the outpatient department and the inpatient department of Longhua Hospital, Shanghai University of Traditional Chinese Medicine(TCM) between April 1, 2012 and January 31, 2014.All of patients in two groups were treated with standard hormone reduction method. The control group was given orally Bupi Yichang pills, 6 g/time, 3 times/day, and the experimental group was given orally Jianpi Qingchang recipe, 300 mL water decoction, 1 dose/d, 2 times/d. All of the patients received continuous treatment for 3 months. After treatment, disease activity index, mucosal healing evaluation, curative effect changes of TCM syndromes and changes of inflammatory factors in the two groups were observed. Result::Compared with before treatment, the two groups of Mayo scores after treatment were significantly reduced (P<0.01), and the experimental group was reduced more significantly than the control group (P<0.01). After treatment, 66.67%of patients in experimental group were in remission, and 13.33%of patients in control group were in remission, with statistically significant differences between two groups (P<0.01). After treatment, the healing rate of the control group was 46.67%, while that of the experimental group was 70.0%, with statistically significant differences (P<0.01). After treatment, the effective rate of TCM syndromes in control group was 80.0%, while that in experimental group was 96.67%, with statistically significant differences (P<0.01). Compared with control group before treatment, the levels of IL-1 in both groups were decreased (P<0.05), while the levels of IL-6 and IL-10 were increased (P<0.05). Compared with control group after treatment, the changes in experimental group were significantly better than those in control group (P<0.05). Conclusion::Jianpi Qingchang decoction can reduce the index of disease activity in patients with hormone-dependent ulcerative colitis due to spleen deficiency and dampness-heat, promote mucosal healing and improve the curative effect of TCM symptoms, and thus is worthy of clinical promotion.

ABSTRACT

@#AIM: To evaluate the clinical effects and safety of surgical techniques in Descemet stripping automatic endothelial keratoplasty(DSAEK)in bullous keratopathy.<p>METHODS: A retrospective analysis of 10 patients with bullous keratopathy treated in our hospital from December 2018 to December 2019 in our hospital, including 4 males(4 eyes), 6 females(6 eyes). Descemet stripping automatic endothelial keratoplasty(DSAEK)was performed with every patient. In addition to the conventional surgical procedures, the surgical technique such as the setting of the anterior chamber perfusion tube, the design of the incision, and the peripheral corneal puncture during the operation were performed. Follow-up for 6mo, the recovery of corneal grafts and postoperative dislocations, double anterior chambers, and other complications were observed, including best corrected visual acuity(BCVA), anterior segment optical coherence tomography, corneal endothelial cell count and incidence of postoperative complications.<p>RESULTS: All patients had smooth surgery, no intraoperative complications occurred, and no postoperative dislocations or interlaminar effusions occurred; postoperative intraocular pressure was normal, and the BCVA was improved to different degrees than before surgery. The symptoms such as tearing and photophobia gradually reduced from 1d after surgery, and completely relieved after 2wk. Corneal stroma edema decreased within 1mo after operation, and the central corneal thickness(596.8±19.11μm)was significantly thinner than that before operation(874.0±58.64μm). During the follow-up period, all patients were stable and the corneal grafts remained transparent, but the corneal endothelial counts were reduced to varying degrees.<p>CONCLUSION: The application of surgical techniques in DSAEK can significantly reduce intraoperative and postoperative complications, improve the safety of surgery, and has clinical value in bullous keratopathy.

ABSTRACT

OBJECTIVE: To study the mechanism of analgesic effect of 8-O-acetyl-safalinoside (8-OaS) on chronic inflammatory pain model rats. METHODS :Totally 30 male SD rats were divided into sham operation group (normal saline ), model group (normal saline ),8-OaS low-dose ,medium-dose and high-dose groups (3,10,30 μg/kg),with 6 rats in each group. Except for sham operation group ,other groups were given planter injection of Freund ’s complete adjuvant to induce chronic inflammatory pain model. After successful modeling ,the rats in each group were given corresponding drugs intrathecally ,once a day,for 7 consecutive days. Then Von-Frey filaments were used to detect the planter pain threshold of the rats in each group ;the area under the planter pain threshold curve of each group and the half effective dose (ED50)of 8-OaS were calculated. Another 36 male SD rats were divided into sham operation group (normal saline ),model group (normal saline )and 8-OaS group (dose of ED50),and the modeling method and administration route were the same as above. Immunofluorescence histochemical staining was used to observe the positive expression of ionized calcium binding adapter molecule 1(Iba-1)and signal molecule phosphorylated p38 mitogen-activated protein kinase (p-p38 MAPK);Western blotting assay was used to determine the expression of Iba- 1,p-p38 MAPK,IL-1β,IL-6 and TNF-α in spinal dorsal horn of rats. RESULTS:Compared with sham operation group ,plantar pain threshold and area under the curve in model group were reduced significantly (P<0.01). Compared with model group ,plantar pain threshold increased significantly after 5,6,7 days of administration in 8-OaS low-dose group (P<0.05),plantar pain threshold and area under the curve in 8-OaS medium-dose and high-dose groups were increased significantly (P<0.05 or P<0.01). Most of above indexes in each dose group of 8-OaS were signifficantly different ,and ED 50 of 8-OaS was 18.87 μ g/kg. Results of immunohistochemistry staining and Western blotting showed that p-p 38 MAPK was mainly expressed in Iba- 1 positive cells. Compared with sham operation group ,the fluorescence density of Iba- 1 and p-p 38 MAPK in spinal dorsal horn ,the expression of Iba-1,p-p38 MAPK,IL-6,IL-1β and TNF-α were significantly increased in model group(P<0.05 or P<0.01). Compared with model group ,the fluorescence density of Iba- 1 and p-p 38 MAPK in spinal dorsal horn ,the expression of Iba- 1,p-p38 MAPK, IL-6,IL-1β and TNF-α were decreased significantly in 8-OaS group (P<0.05). CONCLUSIONS :Intrathecal administration of 8-OaS can effectively alleviate chronic inflammatory pain in rats. The mechanism may be related to the inhibition of the phosphorylation of p 38 MAPK and the expression of IL- 6,IL-1β and TNF-α.

BACKGROUND@#Human epididymis secretory protein 4 (HE4) is a new ovarian cancer biomarker. The factors influencing HE4 levels are not clear, and the reference data in China are limited. Here, we aim to evaluate the effects of menopause and age on HE4 levels and to provide a possible reference value for HE4 in healthy Chinese people.@*METHODS@#A total of 2493 healthy females aged 40 years or older were recruited from March 2013 to March 2017 with the cooperation of four medical institutions across Beijing, China. The serum levels of HE4 and cancer antigen 125 (CA125) were measured by enzyme-linked immunosorbent assay. The Wilcoxon rank-sum test of variance and a stratified analysis were used to analyze the relationships among age, menopausal status, and levels of HE4 or CA125. Confidence intervals (5%-95%) were determined for reference ranges in different populations.@*RESULTS@#There was a statistically significant difference in median HE4 levels between the post-menopausal (n = 2168) and pre-menopausal groups (n = 325) (36.46 vs. 24.04 pmol/L, Z = -14.41, P < 0.001). HE4 increased significantly with age in the post-menopausal groups (H = 408.18, P < 0.001) but not in the pre-menopausal subjects (Z = -0.43, P = 0.67). The upper 95th percentile of HE4 levels were 44.63 pmol/L for pre-menopausal women, 78.17 pmol/L for post-menopausal women, and 73.3 pmol/L for all women. In the post-menopausal population, the HE4 reference ranges were 13.15 to 47.31, 14.31 to 58.04, 17.06 to 73.51, 24.50 to 115.25, and 35.71 to 212.37 pmol/L for different age groups from forty divided by decade. The CA125 level was affected mainly by menopausal status and not age.@*CONCLUSIONS@#Menopausal status and age were both important factors influencing the level of HE4, and age affected HE4 levels mainly in post-menopausal women. The HE4 level was higher in the post-menopausal population than in the pre-menopausal population and increased with age.
