ABSTRACT

OBJECTIVE@#To investigate the molecular mechanism underlying the anti-hepatic fibrosis activity of ethyl acetate fraction Dicliptera chinensis (L.) Juss. (EDC) in human hepatic stellate cells (HSCs) in vitro and in a carbon tetrachloride (CCl4)-induced hepatic fibrosis mouse model in vivo.@*METHODS@#For in vitro study, HSCs were pre-treated with platelet-derived growth factor (10 ng/mL) for 2 h to ensure activation and treated with EDC for 24 h and 48 h, respectively. The effect of EDC on HSCs was assessed using cell counting kit-8 assay, EdU staining, transmission electron microscopy, immunofluorescence staining, and Western blot, respectively. For in vivo experiments, mice were intraperitoneally injected with CCl4 (2 ° L/g, adjusted to a 25% concentration in olive oil), 3 times per week for 6 weeks, to develop a hepatic fibrosis model. Forty 8-week-old male C57BL/6 mice were divided into 4 groups using a random number table (n=10), including control, model, positive control and EDC treatment groups. Mice in the EDC and colchicine groups were intragastrically administered EDC (0.5 g/kg) or colchicine (0.2 mg/kg) once per day for 6 weeks. Mice in the control and model groups received an equal volume of saline. Biochemical assays and histological examinations were used to assess liver damage. Protein expression levels of α -smooth muscle actin (α -SMA) and microtubule-associated protein light chain 3B (LC3B) were measured by Western blot.@*RESULTS@#EDC reduced pathological damage associated with liver fibrosis, downregulated the expression of α -SMA and upregulated the expression of LC3B (P<0.05), both in HSCs and the CCl4-induced liver fibrosis mouse model. The intervention of bafilomycin A1 and rapamycin in HSCs strongly supported the notion that inhibition of autophagy enhanced α -SMA protein expression levels (P<0.01). The results also found that the levels of phosphoinositide (PI3K), p-PI3K, AKT, p-AKT, mammalian target of rapamycin (mTOR), p-mTOR, and p-p70S6K all decreased after EDC treatment (P<0.05).@*CONCLUSIONS@#EDC has anti-hepatic fibrosis activity by inducing autophagy and might be a potential drug to be further developed for human liver fibrosis therapy.

ABSTRACT

OBJECTIVES@#To investigate the incidence of extrauterine growth retardation (EUGR) and its risk factors in very preterm infants (VPIs) during hospitalization in China.@*METHODS@#A prospective multicenter study was performed on the medical data of 2 514 VPIs who were hospitalized in the department of neonatology in 28 hospitals from 7 areas of China between September 2019 and December 2020. According to the presence or absence of EUGR based on the evaluation of body weight at the corrected gestational age of 36 weeks or at discharge, the VPIs were classified to two groups: EUGR group (n=1 189) and non-EUGR (n=1 325). The clinical features were compared between the two groups, and the incidence of EUGR and risk factors for EUGR were examined.@*RESULTS@#The incidence of EUGR was 47.30% (1 189/2 514) evaluated by weight. The multivariate logistic regression analysis showed that higher weight growth velocity after regaining birth weight and higher cumulative calorie intake during the first week of hospitalization were protective factors against EUGR (P<0.05), while small-for-gestational-age birth, prolonged time to the initiation of total enteral feeding, prolonged cumulative fasting time, lower breast milk intake before starting human milk fortifiers, prolonged time to the initiation of full fortified feeding, and moderate-to-severe bronchopulmonary dysplasia were risk factors for EUGR (P<0.05).@*CONCLUSIONS@#It is crucial to reduce the incidence of EUGR by achieving total enteral feeding as early as possible, strengthening breastfeeding, increasing calorie intake in the first week after birth, improving the velocity of weight gain, and preventing moderate-severe bronchopulmonary dysplasia in VPIs.

ABSTRACT

OBJECTIVE@#To observe the clinical effect of penetrating moxibustion on migraine without aura (MO) patients.@*METHODS@#Totally 60 MO patients from the Acupuncture Clinic of the Third Affiliated Hospital of Henan University of Chinese Medicine were collected from November 2015 to February 2017. All patients were assigned to a treatment group and a control group using a random number table, 30 cases in each group. The treatment group was treated with penetrating moxibustion, and the control group was treated with mild moxibustion, thrice a week for 4 consecutive weeks. The total effective rate, Visual Analogue Scale (VAS) scores, headache intensity, and Migraine Specific Quality of Life Questionnaire (MSQ) scores of patients after treatment were compared between the two groups. The moxibustion sensation and reaction after moxibustion were observed, and the adverse reactions were evaluated. All patients were followed up at 4 and 16 weeks after treatment.@*RESULTS@#The total effective rate of the treatment group was significantly higher than that of the control group (93.33% vs. 80.00%, P0.05).@*CONCLUSIONS@#Penetrating moxibustion can significantly relieve pain and improve quality of life of MO patients. After penetrating moxibustion, flushing and sweating of patients were obvious, and the curative effect was superior to the mild moxibustion.

ABSTRACT

Objective:To establish a high performance liquid chromatography (HPLC) fingerprint of the substance benchmark of Xiao Chengqitang and evaluate its quality with chemical pattern recognition method. Method:Diamonsil C<sub>18</sub> column (4.6 mm×150 mm, 5 μm) was used, mobile phase was consisted of methanol (A)-0.1% phosphoric acid solution (B) for gradient elution (0-60 min, 20%-90%A; 60-70 min, 90%-100%A), the flow rate was 1 mL·min<sup>-1</sup>, the column temperature was 25 ℃, and the detection wavelength was 254 nm. The similarity evaluation system of chromatographic fingerprint of traditional Chinese medicine (2012 edition) was used to evaluate the similarity of HPLC fingerprint of 15 batches of substance benchmark of Xiao Chengqitang, and the chromatographic data were analyzed by cluster analysis, principal component analysis and orthogonal partial least squares-discriminant analysis, in order to evaluate the quality difference between different batches of substance benchmarks of Xiao Chengqitang and find out the main chemical components that caused the quality difference. Result:The HPLC fingerprint of Xiao Chengqitang substance benchmarks was established, 31 common peaks were identified, and 18 components were identified by comparing with the reference substances. The similarities of 15 batches of HPLC fingerprint of Xiao Chengqitang substance benchmarks were >0.92. The samples could be divided into two categories by three chemical pattern recognition methods. Nine main components leading to the quality discrepancy of samples between batches were screened out, including rhein, chrysophanol-8-<italic>O</italic>-<italic>β</italic>-<italic>D</italic>-glucoside, aloe-emodin-8-<italic>O</italic>-<italic>β</italic>-<italic>D</italic>-glucoside, sennoside A, chrysophanol-1-<italic>O</italic>-<italic>β</italic>-<italic>D</italic>-glucoside, rhein-8-<italic>O</italic>-glucoside and others. Conclusion:The established fingerprint analysis method is accurate, stable and reproducible, which basically reflects the overall chemical composition characteristics of Xiao Chengqitang, and can be used for the quality control of Xiao Chengqitang preparations.

OBJECTIVES@#To study the clinical features and prognosis of children with acute leukemias of ambiguous lineage (ALAL) under different diagnostic criteria.@*METHODS@#A retrospective analysis was performed on the medical data of 39 children with ALAL who were diagnosed and treated from December 2015 to December 2019. Among the 39 children, 34 received treatment. According to the diagnostic criteria for ALAL by World Health Organization and European Group for the Immunological Characterization of Leukemias, the 39 children were divided into two groups: ALAL group (@*RESULTS@#The 34 children receiving treatment had a 3-year event-free survival (EFS) rate of 75%±9% and an overall survival rate of 88%±6%. The children treated with acute myeloid leukemia (AML) protocol had a 3-year EFS rate of 33%±27%, those treated with acute lymphoblastic leukemia (ALL) protocol had a 3-year EFS rate of 78%±10%, and those who had no remission after induction with AML protocol and then received ALL protocol had a 3-year EFS rate of 100%±0% (@*CONCLUSIONS@#ALL protocol has a better clinical effect than AML protocol in children with ALAL, and positive MRD after induction therapy suggests poor prognosis. Hyperleukocytosis and adverse genetic changes are not observed in children with myeloid expression, and such children tend to have a good prognosis, suggesting that we should be cautious to take it as ALAL in diagnosis and treatment.

OBJECTIVE@#To investigate the prevalence of respiratory viral infections in patients with primary immunodeficiency disease (PID) during hematopoietic stem cell transplantation.@*METHODS@#108 specimens of nasopharyngeal aspirate were collected from 22 PID patients before and after hematopoietic stem cell transplantation from July 2016 to July 2018 in the Department of Hematology. The TR-PCR was used to detect for respiratory viruses including respiratory syncytial virus(RSV),human metapneumoviros(hMPV),coronavirus(CoV) and parainfluenza 1-3 (PIV1-3). And the clinical characteristics and co-infection were analyzed.@*RESULTS@#Among the total 108 specimens, viral pathogens were identified in 41 (37.96%) specimens. Among which the pathogens of highest detection rate was RSV (25.9%). Different types of PID showed different virus infection rates, among which the highest infection rate was severe combined immunodeficiency disease (SCID) patients, with the virus detection rate was 57.9%. The incidence of co-infection with two or more than two viruses was 19.5%.@*CONCLUSION@#Patients with PID who undergo hematopoietic stem cell transplantation are more susceptible to respiratory viruses. RSV is an important respiratory tract virus pathogen after hematopoietic stem cell transplantation.

ABSTRACT

Xiao Chengqitang, derived from Treatise on Febrile Diseases, is composed of Rhei Radix et Rhizoma, Magnoliae Officinalis Cortex and Aurantii Fructus Immaturus. It is often used to treat Yangming Fushi light syndrome. This formula was included in the Catalogue of Ancient Classical Prescription (The First Batch), which has a high clinical application value. Current researches on the chemical composition, pharmacological effect and clinical application of Xiao Chengqitang have been carried out, but the history of Xiao Chengqitang is unclear and the dose conversation method is vague. It has great difficulties in clinical rational applications of Xiao Chengqitang and the research and development of its compound preparations. In this article, the literature reports on Xiao Chengqitang were summarized from the aspects of historical evolution, literature researching of Chinese herbs, dosage conversation method, modern research in case to clarify the historical context of Xiao Chengqitang and to provide basis for the clinical application and modern scientific research of this formula.

ABSTRACT

OBJECTIVE:To screen the components with better anti-inflammatory effect from Codonopsis Radix polysaccharide and explore the anti-inflammatory mechanism. METHODS :Using Codonopsis Radix as sample ,the crude polysaccharide (CP) was obtained by water extraction and alcohol precipitation. After CP was separated and purified by Fast Flow DEAE Sepharose , SephadexG-150 gel column and so on ,the components CP 1-2-1 and CP 3-1-1 were obtained and then characterized. The survival rate of RAW 264.7 cell was determined by MTT method after cultured with CP ,CP1-2-1 and CP 3-1-1(0.01,0.1,1,10,100 μg/mL)for 24 and 48 h respectively. The cells were divided into blank group (blank culture medium ),model group (1 μg/mL LPS)and low-,medium- and high-concentration groups of 3 kinds of Codonopsis Radix polysaccharide (1 μg/mL LPS+25,50, 100 μg/mL CP,CP1-2-1,CP3-1-1 solution). After cultured for 24 h,NO content in cell culture solution ,mRNA expressions of TLR4,IL-6,NF-κB and TNF-α in cells were determined. RESULTS:The sugar content of CP reached (92.20±0.73)%. CP 1-2-1 was a single neutral polysaccharide composed of fructose with a relative molecular weight of 25.8 kDa. CP 3-1-1 was an acidic heteropolysaccharide composed of arabinose ,rhamnose,galactose and galacturonic acid and so on ,with a relative molecular weight of 49.5 kDa. Cell survival rates were higher than 99%,after cultured with 3 kinds of polysaccharide for 24 and 48 h. Compared with blank group ,NO content in cell culture solution and mRNA expressions of TLR 4,IL-6,NF-κB and TNF-α in cells were increased significantly in model group (P<0.05 or P< . Compared with model group , NO content in cell 2016CFB412) culture solution of each administration group was significantly reduced,mRNA expressions of TLR 4,NF-κB,TNF-α,IL-6 in cells in 50,100 μg/mL CP group were significantly decreased ong1991@163.com (P<0.05 or P<0.01);mRNA expressi ons of TLR 4,NF-κB in cells in 25 µg/mL CP 1-2-1 group and mRNA expressions of TLR 4,NF-κB,TNF-α,IL-6 in cells in 50,100 µg/mL CP 1-2-1 group were significantly decreased (P<0.05 or P<0.01);mRNA expressionsof IL- 6 in cells in 25 µg/mL CP 3-1-1 group,mRNA expressions of NF-κB,TNF-α,IL-6 in cells in 50 µg/mL CP 3-1-1 group and mRNA expressions of TLR 4,NF-κB,TNF-α,IL-6 in cells in 100 µg/mL CP 3-1-1 group were significantly decreased (P<0.05 or P<0.01). CONCLUSIONS :CP,CP 1-2-1 and CP 3-1-1 all have certain anti-inflammatory activities. The mechanism may be related to inhibiting the generation and releasing of inflammatory cytokines such as TNF-α,IL-6 by inhibiting the activation of TLR 4/NF-κB pathway.

ABSTRACT

During a short period of time, the outbreak of pneumonia caused by a novel coronavirus, named Novel Coronavirus Pneumonia (NCP), was first reported in China, spreading to 24 countries and regions rapidly. The number of confirmed cases and deaths continued to rise. World Health Organization (WHO) announced that the outbreaks of the novel coronavirus have constituted a Public Health Emergency of International Concern. Efficient infection control can prevent the virus from further spreading, which makes the epidemic situation under control. Due to the specialty of oral healthcare settings, the risk of cross infection is severe among patients and oral healthcare practitioners. It's more urgent to implement strict and efficient infection control protocols. This paper, based on existing guidelines and published researches pertinent to dental infection-control principles and practices, mainly discusses epidemiological characteristics of NCP and the features of nosocomial infection in oral healthcare settings, and furthermore provides recommendations on patient's evaluation, and infection control protocols in department of stomatology under current circumstance..

Objective@#To explore the recovery time and risk factors of coagulopathy caused by rodenticide poisoning through analyzing and following up the confirmed cases, and to provide more useful guidance information for the clinic practice.@*Methods@#A total of 96 cases with coagulation dysfunction caused by anticoagulant rodenticide poiso-ning in Children′s Hospital, Chongqing Medical University from January 2014 to December 2016, were analyzed retrospectively.The recovery time of coagulation function and the relationship between recovery time and drug involved way, dysfunction organs and poison concentration were studied respectively.@*Results@#(1) A total of 96 patients were hospitalized because of severe coagulopathy caused by the poisoning of second generation anticoagulant rodenticide.Brodifacoum was detected from 33 blood samples and the median concentration was 364 μg/L (55-4 654 μg/L). Bromadiolone was detected from 7 blood samples and the median concentration was 130 μg/L (18-652 μg/L). Brodifacoum and Bromadiolone were both detected from 8 cases and the median concentration was 741 μg/L (63-6 000 μg/L) and 11 μg/L (3-3 694 μg/L), respectively.(2) A total of 57 cases of the patients were successfully followed up.A total of 18 cases were confirmed with oral poisoning, 16 cases with dermal poisoning, while 23 cases denied any involved ways of poisoning, and 7 cases had organs dysfunction.The follow-up time was 12-54 months.All the hospitalized patients were given specific antidote Vitamin K treatment and recovered successfully without any sequelae.(3) The median recovery time of coagulopathy caused by rodenticide poisoning was 2.5 months.(4) The recovery time of coagulation function was positively correlated with the plasma concentration of Brodifacoum(r=0.619, P<0.01). (5) There was no significant correlation between recovery time and organ dysfunction or drug involved ways of poisoning involvement (all P>0.05).@*Conclusions@#The recovery time of coagulation dysfunction caused by anticoagulant rodenticide in children is much longer.The higher the concentration of Brodifacoum poison is, the longer the recovery time.Enough supplementation course of Vitamin K should be given according to the follow-up of coagulation function.

OBJECTIVE@#To observe the therapeutic effect of pre-acupuncture at Neiguan (PC 6) and Zusanli (ST 36) on exercise-induced fatigue.@*METHODS@#A total of 50 subjects were divided into an observation group and a control group by random number table method, 25 cases in each one. In the observation group, acupuncture was applied at Neiguan (PC 6) and Zusanli (ST 36) before exercise, once a day, consecutive 5-day treatment was required. In the control group, there was no intervention. The subjective fatigue perception score, serum levels of lactate and lactate dehydrogenase, heart rate and oxygen saturation before and after exercise were observed in the two groups.@*RESULTS@#Compared before exercise, the subjective fatigue perception scores, serum levels of lactate and lactate dehydrogenase and heart rates after exercise were increased in the two groups (0.05).@*CONCLUSION@#Pre-acupuncture at Neiguan (PC 6) and Zusanli (ST 36) can effectively reduce the subjective fatigue perception score,serum levels of lactate and lactate dehydrogenase and heart rate to improve exercise-induced fatigue.

BACKGROUND@# Many controversies still exist regarding ventilator parameters during cardiopulmonary resuscitation (CPR). This study aimed to investigate the CPR ventilation strategies currently being used among physicians in Chinese tertiary hospitals.@*METHODS@# A survey was conducted among the cardiac arrest team physicians of 500 tertiary hospitals in China in August, 2018. Surveyed data included physician and hospital information, and preferred ventilation strategy during CPR.@*RESULTS@# A total of 438 (88%) hospitals completed the survey, including hospitals from all 31 Chinese mainland provinces. About 41.1% of respondents chose delayed or no ventilation during CPR, with delayed ventilations all starting within 12 minutes. Of all the respondents who provided ventilation, 83.0% chose to strictly follow the 30:2 strategy, while 17.0% chose ventilations concurrently with uninterrupted compressions. Only 38.3% respondents chose to intubate after initiating CPR, while 61.7% chose to intubate immediately when resuscitation began. During bag- valve-mask ventilation, only 51.4% of respondents delivered a frequency of 10 breaths per minute. In terms of ventilator settings, the majority of respondents chose volume control (VC) mode (75.2%), tidal volume of 6–7 mL/kg (72.1%), PEEP of 0–5 cmH2O (69.9%), and an FiO2 of 100% (66.9%). However, 62.0% of respondents had mistriggers after setting the ventilator, and 51.8% had high pressure alarms.@*CONCLUSION@#There is a great amount of variability in CPR ventilation strategies among cardiac arrest team physicians in Chinese tertiary hospitals. Guidelines are needed with specific recommendations on ventilation during CPR.

Objective Young and middle-aged people have become the fastest growing group of diabetes, but the compliance of regular exercise is poor. It is urgent to formulate effective intervention strategies or measures to promote the change of their behavior. The article is Objective ly based on the attitude-social influence-self efficacy model, to investigate the stages distribution and influencing factors of regular exercise behavior in young and middle-aged patients with type 2 diabetes.Methods A convenience sampling was used to select 430 young and middle-aged type 2 diabetic patients from the department of endocrinology in two Nanjing 3A grade hospitals. A cross-sectional survey was conducted with a general questionnaire, a regular exercise behavioral stage questionnaire, a diabetes behavioral attitude and social impact scale, and a diabetes self-efficacy scale.Results The proportion of regular exercise behavior in young and middle-aged patients with type 2 diabetes is as follows. There were 14.9% patients at the precontemplation, 25.8% at the contemplation,15.8% at the preparation, 12.3% at the action, 31.2% at the maintenance. Logistic regression analysis showed that on-the-job (OR=1.77), accepting health education (OR=0.49), HbA1c level (OR=0.78), attitude (OR=5.32), subjective norm (OR=2.43), social support (OR=1.87) and self-efficacy (OR=1.28) are main factors to affect the stages of physical activity (P<0.05) .Conclusion Most of the young and middle-aged patients with type 2 diabetes are in the pre-action stages of regular exercise behavior, which needs to be improved, and many factors can affect the stages of regular exercise behavior. Clinical medical staff should provide individualized interventions for these patients with reference to the characteristics of different behavioral stages of patients.

ABSTRACT

Objectives: To explore the association between HbA1c level and the degree of coronary artery disease in patients with stable coronary artery disease (CAD) and type 2 diabetes mellitus (T2DM). Methods: A total of 3 068 patients with stable coronary artery disease and T2DM treated with percutaneous coronary intervention (PCI) from 2014-07 to 2015-06 in Fuwai hospital were included in this retrospective study. Patients were divided into three groups according to SYNTAX score: mild lesions group (SYNTAX score: 0 to 22, n=2 661),moderate lesion group (SYNTAX score: 23 to 32, n=342), and severe lesion group (SYNTAX score > 32, n=65). The relationship between the level of HbA1c and the severity of coronary lesions in patients with T2DM was analyzed using ordered Logistic regression. Results: There were 65 (2.1%) patients with SYNTAX score> 32 and 407 (13.3%) patients with SYNTAX score >22. In SYNTAX score > 32 group, mostly patients had advanced age,longer duration of hyperlipidemia and coronary disease, higher percent of old myocardial infarction and family history of coronary artery disease. Blood glucose levels and HbA1c levels increased and left ventricular ejection fraction decreasedin proportion with increasing SYNTAX score. Patients with SYNTAX score between 23 and 32 had the highest percent of the left anterior descending coronary artery and right coronary artery lesion, followed by left circumflex artery and left main trunk lesion. However, in patients with SYNTAX score> 32, the right coronary artery was the most common culprit artery. The incidence of coronary chronic total occlusions, mean lesion number and the average number of stents increased in proportion with increasing SYNTAX score (P<0.001). Multiple regression analysis showed that HbA1c level (OR:3.51,95%CI:2.90-4.25,P<0.001), duration of CAD (OR:1.06,95%CI:1.01-1.10,P<0.01) and current smoking (OR:1.49, 95%CI: 1.27-1.89,P<0.05)were positively associated with the severity of coronary artery lesions. Conclusions: Diffuse and severe multiple coronary artery lesions are the characteristic changes of patients with stable CAD complicating T2DM.HbA1c level is an important risk factor affecting the complexity and severity of the coronary artery lesions in these patients.

ABSTRACT

Objective To investigate the characteristic changes of biochemical markers of mineral metabolism, vascular calcification, and renal osteodystrophy in an adenine-induced rat model of chronic kidney disease (CKD). Methods A total of 20 male Sprague Dawley rats (SD rats) were randomly divided into two groups: the normal group fed with a diet without adenine, and the CKD group fed with an adenine-containing diet (7. 5 g/kg) for the first 4 weeks and then a diet without adenine for the following 2 weeks. At the end of the 2nd week, serum biochemical markers were detected. At the end of the 6th week, the SD rats were sacrificed and serum biochemical markers were detected once again. The aortas were collected for pathological examination and detection of vascular calcium and phosphorus contents. Femurs and the fifth lumbar vertebrae were taken for bone mineral density (BMD) measurement and bone histomorphometric analysis. Results At the end of the 2nd and 6th weeks, compared with the normal control group, the levels of serum creatinine, urea nitrogen, phosphorus and parathyroid hormone (PTH) in the CKD group were significantly increased (P<0. 05 or P< 0. 01), and the level of serum calcium was significantly decreased (P< 0. 05 or P< 0. 01). Medial layer vascular calcification of the aorta occurred in 50% of the rats in the CKD group, but was not observed in the normal control group. Vascular calcium and phosphorus contents were significantly higher in the CKD group compared with the normal control group (P< 0. 05). The BMD of total femur, cortical and trabecular bone tissues of the femur, and the fifth lumbar vertebra was significantly decreased in the CKD group (P< 0. 05 or P< 0. 01). The histomorphometric analysis showed that both bone resorption and bone formation of the trabecular bone in the CKD group were increased, indicating a high bone turnover status. The volumes of both trabecular and cortical bones of rats in the CKD group were significantly lower than that of the normal control group (P < 0. 05 or P < 0. 01). However, the trabecular bone mineralization was not significantly different between the two groups. Conclusions The adenine-induced rat model of chronic kidney disease (CKD) established in this study shows reduced serum calcium and increased serum phosphorus and PTH, and medial layer vascular calcification of the aorta. With respect to renal osteodystrophy, this model shows a high trabecular bone turnover, normal trabecular bone mineralization, and low bone volume of cortical and trabecular bone, which meets the characteristics of osteitis fibrosa. This model may become a useful tool for future study of chronic kidney disease-mineral and bone disorder (CKD-MBD).

ABSTRACT

Objective To explore the effect of traditional incision and drainage combined with vacuum sealing drainage (VSD)in treating patients with purulent exudation-staged genual septic arthritis. Methods The patients with purulent exuda-tion-staged genual septic arthritis were selected in the Zhengzhou Orthopedics Hospital from January 2010 to December 2015, and they were divided into control group and observation group. Fifty-four patients in the control group were treated with open operative drainage,while fifty patients in the observation group were treated with open operative drainage combined with VSD. The operation time,intraoperative bleeding volume,tourniquet application time,washing time,antibiotic use time,hospitalization time,therapeutic effect and relapse rate were compared between the two groups. Results There was no statistical difference in operation time and tourniquet application time and intraoperative bleeding volume between the two groups(t = - 0. 216,2. 518, 11. 320;P > 0. 05). Compared with the control group,the antibiotic use time and hospitalization time were shorter,the washing time was longer in the observation group(t = 2. 270,- 46. 310,2. 518;P < 0. 05). The occurrence rate of postoperative compli-cations in the control group and the observation group was 9. 3%(5 / 54)and 6. 0%(3 / 50),there was no statistical difference in the occurrence rate of postoperative complications between the two groups(χ2 = 0. 331,P > 0. 05). The fineness rate in the control group and the observation group was 25. 9%(14 / 54)and 22. 0%(11 / 50),there was no statistical difference in the fineness rate between the two groups(χ2 = 0. 219,P > 0. 05). The relapse rate in the control group and the observation group was 33. 3%(18 / 54)and 8. 0%(4 / 50),the relapse rate in the observation group was lower than that in the control group (χ2 =0. 331,P <0. 05). Conclusion Traitional incision and drainage combined with VSD has advantages of shortening hospital-ization and intravenous antibiotic time and decreasing relapse rate in treating purulent exudation-staged genual septic arthritis.

ABSTRACT

<p><b>Background</b>Dandelion is commonly used in traditional Chinese medicine with several active compounds found in extracts. It has a variety of pharmacological effects, such as a reduction in swelling and inflammation, and detoxification. The mechanism by which dandelion extract inhibits the inflammatory response in skeletal muscle cells remains unknown; therefore, the aim of this study was to investigate the effects of dandelion extract root on the proliferation of skeletal muscle cells and the alleviation of lipopolysaccharide (LPS)-induced inflammatory response in vitro.</p><p><b>Methods</b>Rat skeletal muscle cells were isolated from Sprague-Dawley rat and cultured in vitro which were cultured in basal medium, or medium containing LPS or dandelion extract. Cell counting kit-8 (CCK-8) was employed to measure cell proliferation; meanwhile, the optimal concentration of dandelion extract and treatment time were selected. Crystal violet staining was used to detect the proliferation of muscle cells. Western blotting analysis was used to detect the levels of inflammatory factors, myogenic factor, and p-AKT protein expression.</p><p><b>Results</b>The optimal concentration and treatment time of dandelion extract for the following study were 5 mg/ml and 4 days, respectively. Dandelion extract was found to increase proliferation of rat skeletal muscle cells (t = 3.145, P < 0.05), with the highest effect observed at 5 mg/ml. LPS was found to decrease proliferation of skeletal muscle cells (t = -131.959, P < 0.001), and dandelion extract could against this affection (t = 19.466, P < 0.01). LPS could induce expression of inflammatory factors, including interleukin (IL)-1β, IL-6 and tumor necrosis factor (TNF)-α (IL-1β: t = 9.118, P < 0.01; IL-6: t = 4.346, P < 0.05; TNF-α: t = 15.806, P < 0.05), and dandelion extract was shown to reduce LPS-induced expression of IL-1β, IL-6 and TNF-α (IL-1β: t = -2.823, P < 0.05; IL-6: t = -3.348, P < 0.01; and TNF-α: t = -3.710, P < 0.01). Furthermore, LPS was also shown to decrease expression of myogenic factor, including myod1 and myogenin (MyoD1: t = 4.039, P < 0.05 and myogenin: t = 3.300, P < 0.01), but dandelion extract was shown to against this effect of LPS (MyoD1: t = -3.160, P < 0.05 and myogenin: t = -3.207, P < 0.01). And then, LPS was found to increase expression of p-AKT protein (p-AKT/AKT: t = 4.432, P < 0.05). Moreover, expression of p-AKT protein was found to decrease, with 5 mg/ml of dandelion extract (p-AKT/AKT: t = -3.618, P < 0.05).</p><p><b>Conclusions</b>The findings indicate that dandelion extract plays an important role in skeletal muscle cells viability regulation, promote cells proliferation by increasing level of p-AKT protein expression, and reduce LPS-induced expression of inflammatory factors, inhibiting the inflammatory response of rat skeletal muscle cells.</p>

ABSTRACT

Objective To explore the effect of free anterolateral thigh (ALT) perforator flap in the repairment of complex maxillary defects. Methods The clinical data of 51 patients undergoing the reconstruction of maxillary defects in ALT perforator flap between January 2013 and October 2016 in the Peking University Hospital of Stomatology were retrospectively analyzed. All the patients received ALT reconstruction during the maxillary tumor resection. Results Four patients had blood vessel crisis after surgery in 51 cases. Two cases were removed due to flap necrosis, and 49 cases survived with a satisfactory appearance and function, and the overall flap survival rate of 49 cases was 96.08 % (49/51). Conclusion ALT perforator flap provides an ideal reconstruction for complex maxillary defects with a well-restored function in the surgical ares which could avoid the facial asymmetry and contribute to the following treatment.

ABSTRACT

Objective To investigate the relationship of ultrasonographic features and their pathologic basics in women of child-bearing period with abnormal uterine bleeding, and to explore the diagnostic ideas in detecting endometrial lesions. Methods A total of 149 women in childbearing age who had endometrial lesions with pathologically confirmed endometrial lesions, including 88 cases of endometrial hyperplasia, 41 cases of endometrial polyp, 20 cases of endometrial carcinoma, at Fuxing Hospital, Capital Medical University from October 2001 to September 2016 were selected. Ultrasound exams were performed on patients in early proliferative phase of menstrual cycle or within three days after vaginal bleeding stopped. We recorded patients′ age, measured sum diameters of length, width and thickness of uterine, mean thickness of endometrium and maximum diameter of polypoid lesions. We grouped the patients by pathologic results and compared ultrasound parameters between groups. The thickness of endometrium over 1.0 cm was considered as ″increased″. The pathologic exams also included ovarian structures. Results (1) There was no significant difference in the average age of patients with endometrial hyperplasia, endometrial polyps and endometrial cancer (P>0.05). (2) Ultrasonography: there was no statistically significant difference in the sum of the diameters of the uterus in the three groups (P>0.05); the thicker diameter of the uterine cavity in the intimal hyperplasia group was compared with the endometrial polyp group and the endometrial cancer group [(11.70±5.42)mm vs(10.59±3.83)mm vs(17.14±8.70)mm],the difference was statistically significant (F=9.779, P < 0.001), endometrial hyperplasia and endometrial cancer, endometrial polyps compared with the endometrial cancer group, the difference in uterine cavity echo thickness was statistically significant (both P<0.001); there was no significant difference in the uterine cavity echo thick diameter between the intimal hyperplasia group and the endometrial polyp group (P > 0.05). Of the 88 cases of hyperplasia of intimal hyperplasia, 53 were polypoid, and the mean diameter of the polypoid uplift was compared with the endometrial polyp group[(17.13±10.45)mm vs(12.14±4.67)mm],the difference was statistically significant (t=3.112, P=0.002). The proportions of point and strip blood flow signals in 41 cases of endometrial polyps and 53 cases of polypoid hyperplasia were 75.61% (31/41), 41.51% (22/ 53). Of the 163 patients in the three groups, 63 patients had a uterine cavity echo of <10 mm (42.3%). (3) Pathological examination: 20 cases of endometrial cancer were endometrioid carcinomas, 11 cases of hysterectomies, 7 cases of well-differentiated endometrioid carcinoma, 3 cases of moderate differentiation, 1 case of poorly differentiated; at the same time, 8 cases of ovariectomy. In 7 cases, there were multiple atresia follicles, and one case was pure cyst. Of the 88 patients with endometrial hyperplasia, 77 were hyperplasia and hyperplasia, 1 had dysplasia, 11 had complicated hyperplasia, and 3 had dysplasia. Of the 41 cases of endometrial polyps, 33 were non-functional polyps, 3 were functional polyps, and in 4 cases of adenoid fibroid polyps, there was 1 case of canceration. Conclusion In the early stage of endometrial proliferation or within 3 days after vaginal bleeding cases, the uterine cavity echo thickness diameter ≥ 10 mm suggests abnormal endometrium; uterine cavity echo thickness diameter <10 mm, endometrial abnormalities cannot be excluded; dynamic monitoring of ovulation by ultrasound may indicate the presence or absence of risk factors for abnormal growth of the endometrium.

ABSTRACT

Aim To study the effect of puerarin( Pue) against myocardial ischemia/reperfusion injury and in-volved mitochondrial mechanism. Methods Anoxia/reoxygenation( A/R) injury model was established in H9c2 cell. Recombinant plasmid pFLAG-VDAC1 was constructed. Cells were randomly divided into 4 groups, normal control group ( Control) , A/R group, puerarin group ( Pue + A/R ) , and pFLAG-VDAC1-Pue group. Real-time PCR was used to investigate the expression of VDAC1 at mRNA level, and the expres-sion of protein level was detected by Western blot. LDH and CK activities were measured by automatic bi-ochemical analyzer. Mitochondrial membrane potential ( Δψm) and cell apoptosis were observed by flow cy-tometry method. Mitochondrial swelling test was used to detect the opening of mitochondria permeability tran- sition pore ( mPTP) . Results Compared with control group, the expression of VDAC1 and mRNA was up-regulated in A/R group, LDH and CK activity were el-evated, and then mPTP opened, Δψm collapsed, cell apoptosis was significantly increased. Puerarin pre-treatment can lower the expression of VDAC1, main-tain Δψm, prevent the opening of mPTP, and reduce apoptosis. However, the protective effect of Puerarin could be cancelled by transfection of pFLAG-VDAC1. Conclusions The cardioprotection of Puerarin against A/R injury is closely related to down-regulation of VDAC1 and prevention of mPTP opening.
