ABSTRACT

Objective:To study the risk factors of necrotizing enterocolitis (NEC) after surgery for intestinal atresia.Method:From August 2013 to June 2020, children with intestinal atresia receiving surgery in our hospital were retrospectively reviewed. The patients were assigned into NEC group and non-NEC group according to the occurrence of postoperative NEC. Demographic data and clinical characteristics were summarized and the risk factors for postoperative NEC were analyzed using Logistic regression analysis method.Result:A total of 96 infants were enrolled and NEC occurred in 13 patients (13.5%) after surgery for intestinal atresia. Compared with the non-NEC group, the NEC group were diagnosed of intestinal atresia [4.0(1.5,6.0)d vs. 1.4(0,2.0)d, P<0.001] and received surgery [4.8(2.0,7.0)d vs. 3.1(1.0,4.0)d, P=0.034] at later ages. The incidences of complex intestinal atresia [76.9%(10/13) vs. 44.6%(37/83), P=0.030] and blood transfusion [46.2%(6/13) vs. 13.3%(11/83), P=0.007] in the NEC group were higher than the non-NEC group. Logistic regression analysis showed that the age of initial diagnosis of intestinal atresia ( OR=3.346, 95% CI 1.493~7.500, P=0.003), complex intestinal atresia ( OR=9.052, 95% CI 1.119~73.209, P=0.039) and blood transfusion ( OR=6.835, 95% CI 1.399~33.380, P=0.018) were independent risk factors for postoperative NEC. Conclusion:Patients with delayed diagnosis of intestinal atresia, complex intestinal atresia and blood transfusion within 48 hours after surgery should be monitored for the occurrence of postoperative NEC.

ABSTRACT

In order to study the alkaloids from branches and leaves of Ervatamia hainanensis, silica gel, ODS, Sephadex LH-20 and HPLC chromatography were used to obtain six alkaloids from the branches and leaves of E. hainanensis with use of. Based on the physicochemical properties and spectral data, their structures were identified as 10-hydroxydemethylhirsuteine(1), 3R-hydroxycoronaridine(2), 3-(2-oxopropyl)coronaridine(3), pandine(4), 16-epi-vobasine(5), and 16-epi-vobasinic acid(6). Among them, compound 1 was a new monoterpenoid indole alkaloid, and compounds 5 and 6 were obtained from this plant for the first time.

ABSTRACT

Objective:To reveal the expression and regulation network of microRNA (miRNA) and mRNA in ovarian endometriosis using high-throughput sequencing, and then explore the potential pathogenesis.Methods:From January 2017 to January 2018, twenty patients with ovarian endometriosis in Xiangya Hospital of Central South University were enrolled. Ectopic endometrium and paired eutopic endometrium were collected when surgery was conducted. After total RNA was extracted, miRNA sequencing and mRNA sequencing were performed respectively. Differential miRNA and mRNA expression profiles were analyzed. Predicted mRNA were obtained by Targetscan and miRDB databases, and then intersected with differential mRNA to obtain candidate mRNA. miRNA-mRNA regulatory network was analyzed using Cytoscape software, whereas gene ontology (GO) and pathway function enrichment were performed by DAVID database.Results:Compared with eutopic endometrium, there were 369 miRNAs (197 up-regulated, 172 down-regulated) and 3 765 mRNAs (1 975 up-regulated and 1 790 down-regulated) differentially expressed in ectopic endometrium. Real time quantitative polymerase chain reaction (qRT-PCR) confirmed that the expression of mir-202-5p and mir-514a-5p in ectopic endometrium was higher than that in eutopic endometrium ( P<0.05), while the expression of mir-375-3p, mir-449b-5p in ectopic endometrium was lower than that in eutopic endometrium ( P<0.05). Bioinformatics found that the candidate mRNA related to these four miRNAs are mainly enriched in biological processes of nuclear steroid hormone receptor binding, transmembrane receptor and protein kinase activity, and participate in transforming growth factor β (TGF β) signal pathway, adhesion junction, Wnt signal pathway and Rap1 signal pathway. Conclusions:miRNA and mRNA are differentially expressed in endometriosis, and exact important regulatory network in the development of endometriosis by involving multiple biological processes.

ABSTRACT

Objective:To explore the effects of horticultural therapy on upper limb function and cognitive impairment of hemiplegic patients after stroke. Methods:From June, 2017 to March, 2019, 60 hemiplegic stroke patients were randomly divided into control group(n = 30) and experimental group (n = 30). All the patients accetped routine rehabilitation, while the experimental group received group-based horticulture therapy in addition, for eight weeks. They were assessed with Fugl-Meyer Assessment-Upper Extremity (FMA-UE), modified Barthel Index (MBI) and Montreal Cognition Assessment (MoCA) before and after intervention. Results:The scores of all the assessment improved in both groups after intervention (|t| > 8.077, P < 0.001), and improved more in the experimental group than in the control group (|t| > 2.293, P < 0.05). Conclusion:Horticultural therapy can promote the recovery of upper-limb and cognitive function after stroke.

OBJECTIVE@#To observe the expression of HELQ and RAD51C in normal endometrial and endometrial stromal sarcoma (ESS) and analyze their correlation with the clinical features of the patients.@*METHODS@#The expressions of HELQ and RAD51C proteins were detected by immunohistochemical staining in normal endometrial tissues (14 cases) and tumor tissues from patients with ESS (37 cases) treated in Hunan Provincial Cancer Hospital from January, 2013 to December, 2016. The correlations of the expressions of the two proteins with the patients'age, FIGO staging, tissue type, tumor size, and lymph node metastasis were analyzed.@*RESULTS@#Immunohistochemical staining showed that the expressions of HELQ and RAD51C were both decreased in ESS patients compared with the normal group, and there was a positive correlation between HELQ and RAD51C expression ( < 0.05). HELQ expression in ESS was correlated with the tumor size and type. The expressions of HELQ and RAD51C were not correlated with the patients' age, FIGO stage and status of lymph node metastasis ( > 0.05).@*CONCLUSIONS@#Homologous recombination- directed DNA repair involving HELQ and RAD51C may participate in the occurrence and progression of ESS.

Based on the research fields of applications and funded projects and the funding cost, we analyzed the sub-disciplines of the funded applications, key support areas, research frontiers and trends in the subjects of physiology and integrative biology, which gives us an insight into the future applications to optimize the layout of research areas in Division of Physiology and Integrative Biology.

ABSTRACT

To investigate the value of contrast-enhanced ultrasound(CEUS)quantitative parameters in the diagnosis of thyroid benign and malignant nodules. The CEUS features of 85 histopathologically confirmed thyroid nodules were quantitatively analyzed using five parameters including rising time(RT),time to peak(TTP),area under the curve(AUC),maximum intensity(Imax),and mean transit time(mTT).The dynamic vascular pattern(DVP)curves were also drawn. The Imax(=-7.08,=0.01)and AUC(=-2.03,=0.04)of thyroid malignant nodules were significantly smaller than those of thyroid tissue,and the Imax(=-1.35,=0.02)and AUC(=-0.21,=0.02)of thyroid benign nodules were significantly larger than those of thyroid tissue.There were significant differences between thyroid benign and malignant nodules in Imax(=-4.16,=0.00),AUC(=-3.01,=0.01),and DVP curve types(=0.00).RT(=-0.28,=0.62),TTP(=-0.10,=0.89),and mTT(=-0.79,=0.05)were not significantly different between thyroid benign and malignant nodules. The quantitative parameters of CEUS,especially Imax and AUC parameters,are valuable in the diagnosis of benign and malignant thyroid nodules.

Objective:To evaluate the effects of different output pressures of extracorporeal shock wave therapy (ESWT) on triceps surae spasticity after stroke, and to explore the electrophysiologic mechanism involved. Methods:From January, 2015 to December, 2017, 90 hemiplegic stroke patients with triceps surae spasticity were randomly assigned into control group (n = 30), treatment group 1 (n = 30) and treatment group 2 (n = 30). All the patients accepted routine medicine and rehabilitation, while the treatment groups 1 and 2 received ESWT of 1.5 bar and 2.0 bar in addition, respectively, for four weeks. They were assessed with Composite Spasticity Scale (CSS), passive range of motion (PROM), 10-meter walk test (10MWT), and H reflex latency and the Hmax/Mmax ratio before and after treatment, and the two treatment groups were assessed with Visual Analogue Scale (VAS) immediately after ESWT. Results:All the groups improved in scores of CSS, PROM and 10MWT after treatment (t > 7.261, P < 0.001), and improved more in scores of CSS and 10MWT in the two treatment groups than in the control group (P < 0.05), and the 10MWT was less in the treatment group 2 than in the treatment group 1 (P < 0.05). H reflex latency lengthened (t > 15.025, P < 0.001) and Hmax/Mmax ratio decreased (t > 14.850, P < 0.001) after treatment in the two treatment groups, and the H reflex latency was longer in the two treatment groups than in the control group (P < 0.01). There was no significant difference in VAS between the two treatment groups (t = 0.735, P > 0.05). Conclusion:ESWT of both 2.0 bar and 1.5 bar can improve triceps surae spasticity after stroke, and ESWT of 2.0 bar may more benefit the ability of walking. Further study is needed to explore the involved electrophysiologic mechanism.

ABSTRACT

Objective To explore the efficacy and safety of two antibody induction therapies during donor after cardiac death (DCD) kidney transplantation .Methods Retrospective analysis was performed for the clinical data of DCD kidney patients in 2016 .Patients using basiliximab monoclonal or thymoglobulin (ATG ) polyclonal antibody were divided into two groups . Early postoperative biopsy proven ,acute rejection rate ,creatinine level and patient/graft survival rate were compared between two groups at 1 ,3 or 6 month post-operation .Results Basiliximab (n=44) and ATG (n=60) was used as induction .No significant inter-group difference existed in donor age ,primary disease , creatinine pre-donation , recipient age or cause of renal failure . And recipient male ratio and body weight were greater in ATG group than those in basiliximab group [87% vs .55% ;(70 ± 13) vs .(64 ± 12) kg] .Outcomes of basiliximab group showed acute rejection rate was 9% ,average creatinine 112 .4 at 1 month ,127 .0 at 3 months and 107 .8 at 6 months and total infection rate 16% .Graft/patient 6-month survival rates were 95% (42/44)and 98% (43/44) .Outcomes of ATG group showed that acute rejection rate was 3% ,average creatinine 135 .6 at 1 month ,119 .0 at 3 months and 118 .0 at 6 months and total infection rate 22% .Graft/patient 6-month survival rates were both 100% (60/60) .Conclusions During DCD kidney transplantation ,both induction therapies may prevent acute rejection immediately post-operation .No difference exists in acute rejection rate ,infection rate ,graft/recipient 6-month survival rate or graft function . These two inductions have an excellent early prognosis .

ABSTRACT

Jatrogricaine A (1), a new diterpenoid possessing a 5/6/6/4 carbon ring system, together with eight known diterpenoids (2-9) were isolated from the stems of Jatropha podagrica. Their structures were elucidated by extensive spectroscopic methods and the absolute configuration of 1 was determined by single crystal X-ray diffraction analysis. All compounds were evaluated for their anti-inflammatory activities in vitro, and compound 3 showed significant inhibitory effects against nitric oxide (NO) production in lipopolysaccharide (LPS)-induced RAW264.7 macrophage cells with an IC of 13.44 ± 0.28 μmol·L, being comparable to the positive control, quercetin (IC 17.00 ± 2.10 μmol·L).

ABSTRACT

OBJECTIVE: To evaluate the effectiveness and economics of Danshen ligustrazine injection and Shenxiong glucose injection in the treatment of cerebral infarction, and to provide reference for clinical treatment decisions. METHODS: Retrieved from CNKI, VIP, WANFANG DATA and CBM, RCTs about Danshen ligustrazine injection or Shenxiong glucose injection vs. Compound danshen injection in the treatment of cerebral infarction were collected. Clinical efficacies of two drugs were compared indirectly by using Meta-analysis, and the costs were obtained through domestic routine treatment plan and cost level. Based on it, pharmacoeconomic cost-effectiveness analysis was conducted. RESULTS: A total of 14 literatures were included, involving 1 305 patients. Average total response rates of Danshen ligustrazine group and Shenxiong glucose injection group were 89. 63% and94. 11%, and the costs were 3 855. 29 and 4 198. 68 yuan, respectively; cost-effectiveness ratio were 43. 01 and 44. 61. The incremental cost-effectiveness ratio was 76. 65, i. e. Shenqiong glucose injection group needed extra cost of 76. 65 yuan for each additional 1% of the average total response rate, which was higher than the daily disposable income of 60. 18 yuan in China. Meanwhile, the results of cost-effectiveness analysis were supported by sensitivity analysis. CONCLUSIONS: Compared to Shenxiong glucose injection, Danshen ligustrazine injection is probably more cost-effective for cerebral infarction. Furthermore, the conclusion need to be confirmed by pharmacoeconomics evaluation based on RCTs.

ABSTRACT

OBJECTIVE:To evaluate the effects of the implementation of National Essential Medicine System on the utilization rate of hormone drugs in primary medical institutions in China systematically.METHODS:Retrieved from CJFD,Wanfang database and VIP,the literatures about the use of hormone drugs were collected before and after the implementation of National Essential Medicine System.Meta-analysis was performed by using Stata 13.0 software after utilization rate dam extraction and quality evaluation with Cochrane system evaluator manual 5.1.0.RESULTS:A total of 20 literatures were included.Results of Meta-analysis showed that utilization rate of hormone drugs after the implementation of National Essential Medicine System was significantly lower than before implementation,with statistical significance [RD =-0.03,95 % CI (-0.05,-0.02),P< 0.001].Results of subgroup Meta-analysis of utilization rate of hormone drugs in different areas before and after the implementation of National Essential Medicine System showed that the utilization rate of hormone drugs in eastern areas was significantly lower after the implementation of National Essential Medicine System than before [RD=-0.06,95% CI(0.09,-0.03),P=0.001] with statistical significance.There was no statistical significance in the utilization rate of hormone drugs in middle area [RD=-0.02,95 % CI(0.06,0.02),P=0.235] or western area [RD=-0.01,95% CI (-0.02,0),P=0.122] before and after the implementation of National Essential Medicine System.CONCLUSIONS:The implementation of National Essential Medicine System reduces the utilization of hormone drugs in primary medical institutions.The effect of the eastern area is more obvious than in middle and western area.

ABSTRACT

Objective To study the best formulation and technology of quercetin-loaded polylactic-co-glycolic acid-D-α-tocopheryl polyethylene glycol 1000 succinate(PLGA-TPGS) nanoparticles(QPTN) with QT as model drug and PLGA-TPGS as polymer materials by orthogonal tests,and to investigate the in vitro stability of QPTN. Methods To ensure the best formula-tion and technology for preparing QPTN,single-factor test was established to determine the influence of the ratio of quercetin to PLGA-TPGS,the concentration of TPGS as emulsifiers,the ultrasonic power and ultrasonic time to the particle size,drug loading (DL) and entrapment efficiency(EE).According to single-factor test,the factor levels of nanoparticles were set to select the best prescription and technology of QPTN by orthogonal test.The stability of QPTN was examined using the effecting factor test,accel-eration test and long-term test. Results The best formulation and technology of QPTN was that the ratio of quercetin to PLGA-TPGS was 3:10 (W:W) with 0.05% TPGS as emulsifier,the mixed solution was sonicated for 6 min at 200 W.The average particle size,DL and EE of QPTN prepared under the conditions described above were (155.4 ± 2.7) nm,(21.6 ± 1.5)% and (93.7±2.9)% (n=6),respectively.In the in vitro stability test,QPTN showed a good stability at high temperature,high humidity and strong light condition. Conclusion The best formulation and preparation technology of QPTN was selected.QPTN got small particle size,high DL and EE,and good in vitro stability.

ABSTRACT

Objective To observe changes of left ventricular function and the level of autophagy after treatment with trimetazine in rats with heart failure after myocardial infarction(MI). Methods Twenty healthy male Wistar rats with the ligation of the proximal part of the left descending branch were randomly and equally allocated into two groups,model group (M group) and trimetazine group (Q group). A sham group (S group) was made up by 10 sham-operated rats. Rats of trimetazine group were given trimetazine(15 mg/kg)once a day for 4 weeks.Then left ventricular function was measured by echocardiography,and hemodynamics was evaluated by Millar pressure-volume system.Serum levels of NT-proBNP and hs-TnT were tested by ELISA.Pathological changes and fibrosis of myocardium were observed by HE and Masson staining.The myocardial apoptosis level was observed by TUNEL, and expressions of autophagy related protein and gene in myocardial tissue were detected by Western blot assay and RT-PCR. Results Trimetazine treatment significantly improved left ventricular dilatation and dysfunction in rats with myocardial failure. Trimetazine treatment also significantly improved pressure overload and the compliance decrease of left ventricular in rats with heart failure after myocardial infarction. Trimetazidine reduced the edema,necrosis and myocardial fibrosis of cardiac myocytes in rats with heart failure.The results from ELISA showed that serum levels of NT-proBNP and hs-TnT were significantly lower in the trimetazine group than those of model group.Compared with model group,the cardiomyocyte apoptosis decreased significantly in the trimetazine group.The results from Western blot assay and RT-PCR showed that autophagic flow of myocardium was increased remarkably in the trimetazine group than that of model group. Conclusion Autophagy has a protective effect on myocardial cells. Trimetazine can improve cardiac function through up-regulation of autophagy in cardiomyocytes in MI rats with heart failure.

ABSTRACT

General anesthesia is an unconscious state induced by anesthetics for surgery. The molecular targets and cellular mechanisms of general anesthetics in the mammalian nervous system have been investigated during past decades. In recent years, K channels have been identified as important targets of both volatile and intravenous anesthetics. This review covers achievements that have been made both on the regulatory effect of general anesthetics on the activity of K channels and their underlying mechanisms. Advances in research on the modulation of K channels by general anesthetics are summarized and categorized according to four large K channel families based on their amino-acid sequence homology. In addition, research achievements on the roles of K channels in general anesthesia in vivo, especially with regard to studies using mice with K channel knockout, are particularly emphasized.

Clavicle fracture, a very rare delayed complication following radical neck dissection of oral carcinoma, is normally ignored by oral and maxillofacial surgeons. We report and analyze a male patient with clavicle fracture after primary extended excision and bilateral radical neck dissection. This case was misdiagnosed as cervical metastasis.

Methods A total of 104 patients with hepatocellular carcinoma clinical staging of early metaphase were collected from the First Affiliated Hospital of Xi'an Jiaotong University in Jan.2010 to Dec.2016, all treated with liver resection and follow-ups. Adopting the method of retrospective analysis, univariate and multivariate analyses were performed to investigate the risk factors for recurrence-free survival (RFS) and overall survival (OS). Multiple logistic regression was used to identify the clinicopathological characteristics associated with MVI. The rates of RFS and OS were compared among patients with or without MVI treated with liver resection alone or followed by PA-TACE. Results In HCC patients, the gender HBs Ag, total bilirubin, and prothrombin time, AFP, HBV-DNA, the size of largest tumor, capsule, operation type, hepatic door block, Child-Pugh grade, classification comparison differences had no statistical significance between the two groups of with or without TACE (P>0.05); Age, nodules, tumor cell resection, and BCLC were statistically significant in the non-MIV infiltration group (P<0.05).Hypersplenism and recurrence rate were statistically significant in MIV group (P<0.05).Univariate and multivariate analyses demonstrated that AFP (> 400 ng/m L), tumor diameter, membrane integrity, MVI, and most liver resection were significantly associated with overall survival rates (P<0.05), the AFP (>400 ng/m L), Child-Pugh, grade B, tumor diameter, hepatocellular carcinoma (HCC) and other abnormal degree, membrane integrity, MVI have significant correlation with disease-free survival rate (P<0.05). Multiple logistic regression identified serum AFP level> 400 ng/L, tumor size> 5 cm, and incomplete tumor envelope was independent correlation factor (P>0.05). In the HCC patients with MIV, both OS and DFS were significantly improved in patients who received PA-TACE as compared to those who underwent liver resection alone (P> 0.05), but no significant effect in HCC patients wieh no MIV invasion (P> 0.05).Conclusions There is a certain correlation between microvascular infiltration and the recurrence rate and survival rate of hepatocellular carcinoma, and the method of using adjuvant hepatic arterial chemoembolization is helpful to reduce the recurrence rate and prolong the survival period.

ABSTRACT

Objective To evaluate the current situation of rational drug use in medical facilities at all levels in Sichuan province by means of empirical research via WHO /INRUD selected drug use indicators SDUIs .Methods Six sample cities in Sichuan Province was extracted ,in which 1 to 3 tertiary hospitals ,1 to 3 secondary hospitals ,5 community health service cen-ters and 5 township hospitals were selected .Prescription indicators were indicated by collecting outpatient prescriptions from sample medical facilities which contained 30 prescriptions per month of every January ,April ,July and October from January 2012 to April 2015 .Results The average number of drugs was 3 .02 per prescription .The percentage of drug generic name used in prescription drugs was 90 .33% .The percentage of antimicrobial use was 40 .09% .The percentage of injections used was 14 .46% .The percentage of essential drugs used among prescription drugs was 83 .27% .The percentage of essential drugs used was 93 .05% .Average fee amount all prescriptions was 66 .04 yuan .Conclusion Some of the indicators was of good ra-tionality ,but there were still some unreasonable phenomena .
