Objective:To observe the effect of combining biofeedback therapy (BFT) based on virtual reality technology with repeated transcranial magnetic stimulation (rTMS) on dysphagia among stroke survivors. Methods:Eighty patients were randomly divided into a control group, an rTMS group, a BFT group and a combined treatment group, each of 20. In addition to routine dysphagia rehabilitation, the rTMS and BFT groups were given those treatments, while the combined treatment group was given both for 4 weeks. Swallowing function was evaluated before and after the treatment using the standardized swallowing assessment (SSA) and the functional oral intake scale (FOIS). Videofluoroscopy was used to quantify the subjects′ oral and pharyngeal phases and their aspiration status. Results:Significant improvement was observed in the average FOIS and SSA scores, as well as in the average oral and pharyngeal phases and in aspiration. The combined treatment group′s results were significantly better in all those aspects than those of the other 3 groups. Conclusion:The combined application of biofeedback therapy based on virtual reality technology and repeated transcranial magnetic stimulation can improve the swallowing function of stroke survivors with dysphagia. It is worthy of clinical promotion.
ABSTRACT
BACKGROUND: Phospholipase D (PLD) is used as the biocatalyst for phosphatidylserine (PS) production. In general, PLD was expressed in insoluble form in Escherichia coli. High-level soluble expression of PLD with high activity in E. coli is very important for industrial production of PLD. RESULTS: Streptomyces chromofuscus PLD coding gene was codon-optimized, cloned without signal peptide, and expressed in E. coli. The optimal recombinant E. coli pET-28a+PLD/BL21(DE3) was constructed with pET-28a without His-tag. The highest PLD activity reached 104.28 ± 2.67 U/mL in a 250-mL shake flask after systematical optimization. The highest PLD activity elevated to 122.94 ± 1.49 U/mL by feeding lactose and inducing at 20 C after scaling up to a 5.0-L fermenter. Substituting the mixed carbon source with 1.0 % (w/v) of cheap dextrin and adding a feeding medium could still attain a PLD activity of 105. 81 ± 2.72 U/mL in a 5.0-L fermenter. Fish peptone from the waste of fish processing and dextrin from the starch are both very cheap, which were found to benefit the soluble PLD expression. CONCLUSIONS: After combinatorial optimization, the high-level soluble expression of PLD was fulfilled in E. coli. The high PLD activity along with cheap medium obtained at the fermenter level can completely meet the requirements of industrial production of PLD.

Objective: To synthesize the zinc gluconate carbon dots (Zn-CDs) by one-step hydrothermal method, and to investigate their effects on the cell imaging and inducing osteoblastic differentiation of preosteoblasts in the mice. Methods: The Zn-CDs were synthesized by one-step hydrothermal method, and the characteristics were observed and detected by transmission electron microscope (TEM), Fourier transform-infrared spectrum (FT-IR) and fluorescence spectrometer. The MC3T3-E1 cells were divided into blank control group and experimental groups; different concentrations (0.01, 0.10, 1.00, 10.00, 100.00, 1 000 mg middot; L-1) of Zn-CDs were added into the cells in experimental groups, and nothing was added into the cells in blank control group. MTT assay was used to determin the relative growth rate (RGR) of MC3T3-E1 cells in various groups; the imaging characteristics of MC3T3-E1 cells were observed under confocal microscope; the relative expression levels of Runt-relateed transcription factor-2 (Runx2), alkaline phosphatase (ALP) and osteocalcin (OC) mRNA in the MC3T3-E1 cells in various groups were detected by qRT-PCR; the number of calcified nodules was detected by alizarin red staining. Results: The TEM results showed that the particle size of Zn-CDs was about 5. 25 nm. The fluorescence spectrometer results showed that the Zn-CDs had the fluorescence properties of 360 nm ultraviolet excited light and 450 nm blue emission light, and the Zn-CDs showed the characteristics of excitation wavelength dependence. The FT-IR results showed that the surface of Zn-CDs was mainly composed of carboxyl groups and hydroxyl groups. Compared with blank control group, the RGR of MC3T3-E1 cells in 1 000. 00 mg middot; L-1 Zn-CDs group was decreased significantly at 24 h after co-culture (P<0. 01). The results of fluorescence imaging showed that the blue, green and red fluorescence in the MC3T3-E1 cells, the outline was clear, and the fluorescence intensity of the cytoplasm was stronger than that of the nucleus after co-cultured with Zn-CDs. The qRT-PCR results showed that the relative expression levels of Runx2, ALP and OC mRNA were significantly increased with the increasing of Zn-CDs concentration. The alizarin red staining results showed that the number of calcium deposits in the MC3T3-E1 cells in different concentrations of Zn-CDs groups were more than that in blank control group after induced for 21 d. Conclusion: Zn-CDs can effectively perform the fluorescence imaging in the MC3T3-E1 cells, and Zn-CDs have a certain ability to promote the osteoblastic differentiation of the MC3T3-E1 cells.

ABSTRACT
Neuropathic pain is a typical chronic pain that has a great impact on the psychology and physiology of patients. Recent studies have shown that spinal microglial activation and the up-regulated expression of P2X4 receptor on its surface were occured after peripheral nerve injury. P2X4 receptor is an ionic subtype of ATP receptor, and was regulated by various factors under pathological conditions of nerve injury, such as chemokines, extracellular matrix fibronectin and interferon regulatory factor 5. Inhibition of microglial activation and P2X4 receptor expression can reduce neuropathic pain. Therefore, this article mainly describes the research progress of P2X4 receptor in the pathogenesis of neuropathic pain.

Objective To cxplore the variation of the range of motion (ROM) of operative level after different heights of artificial cervical disc replacement, and to provide guidance for clinical work in selecting appropriate height of artificial cervical disc prosthesis. Methods The preoperative cervical anteroposterior and lateral Ⅹ-rays of 9 fresh male cadaveric cervical spine specimens were obtained to measure the intervertebral height of C5-6,and 3 screened specimens with the height of about 5 mm were included in the experiment. The experiment was designed to test self-control,and other four groups of cervical specimens including intact group, appropriate height (5 mm) of C5.6 artificial cervical disc replacement group,1mm increased (6 mm) group and 2 mm increased (7 mm) group were made biomechanical test sequentially. The specimens were fixed to the cervical three-dimensional movement machine, with a 75 N follower load and pure moments of 2 Nm for flexion/extension 、left/right bending and left/right axial rotation,to measure the ROM of operative level under the condition of changes in 0.2 Nm/s.Results There were no significant differences in the ROM of flexion/extension,lateral bending and axial rotation between 5 mm group and intact group;the ROM of flexion/extension、lateral bending and axial rotation in 6 mm group increased compared with 5 mm group, but the difference was not statistically significant;the ROM of flexion/extension in 7 mm group was significantly less than that of intact,5mm and 6 mm group (9.5°± 1.0° vs 12.5°±0.9°、11.3°±0.8°、11.6°±0.9°),but significantly greater in axial rotation than 6 mm group (10.4°±1.4°vs 8.6°±0.3°),and there was no significant difference in lateral bending compared with other 3 groups. Conclusion 2 adjacent heights of cervical disc prostheses are implantedsuitably when testing the mold of disc prosthesis, the choice of cervical disc prosthesis with 1 mm increased can improve the ROM of operative level to some extent;while the height with 2 mm increased can lead to the ROM of flexion/extension at the operative level reduced, but the ROM of rotation shows an increasing trend.
ABSTRACT
OBJECTIVE:To evaluate the role of PDCA cycle management on standardizing clinical use of atomization inhala-tion drug. METHODS:Using a retrospective method,300 medical records of atomization inhalation drug use were collected from our hospital during Jan.-Dec. in 2013(non-intervention group),200 medical records collected during Apr.-Jun. in 2014(one cycle intervention group),and 180 medical records collected during Oct.-Dec. in 2014(two rounds of cycle intervention group). The use of atomization inhalation drugs was compared before and after the application of PDCA cycle management. Before and after two cy-cle intervention,4 types of medical staff were investigated on cognition of the related knowledge of atomization inhalation drugs, as physicians, pharmacists, nurses and mechanic. RESULTS:After two rounds of PDCA cycle management and intervention, the proportion of unsuitable route of administration of antibiotics, long acting glucocorticoid,non-atomization inhalation dosage form of resolving phlegm drugs, protease decreased from 54.0%,62.0%,59.7%,44.7% before intervention to 0.6%,1.1%,15.0%, 1.1% after intervention;those phenomena had not been found, such as unsuitable route of administration of TCM injection and non-atomization dosage form of theophyllines, unsuitable atomization frequence, unsuitable indication, drug mixture for preventing and controlling symptom, etc. The proportion of medical staffs being familiar with medical device, drug classification, drug selec-tion for prerention and treatment and the proportion of hospital drug stock to 93.4%,86.7%,92.4%,96.2% after intervention from 38.0%,79.0%,49.0%,39.5%before intervention. CONCLUSIONS:The application of PDCA cycle management can effec-tively regulate the use of atomization inhalation drugs in our hospital;the method can be promoted and applied.
ABSTRACT
OBJECTIVE@#To investigate the correlation of laryngeal squamous cell carcinoma (LSCC) and precancerous lesion with HPV infection subtypes and possible clinical relationship.@*METHOD@#Eighty-three cases in paraffin embedded tissues were detected with thirty seven HPV subtypes by flow-through hybridization and gene chip (HybriMax), including 31 cases of laryngeal squamous cell carcinoma, 52 cases of precancerous lesions (29 cases of vocal cord leukoplakia and 23 cases of laryngeal papilloma), and 36 cases of vocal cord polyp as normal vocal mucosa were used as control.@*RESULT@#The total positive rate of HPV was 19.4% in the group of laryngeal squamous cell carcinoma (6/31), 0 in vocal cord leukoplakia, 65.2% in laryngeal papilloma (15/23), and the control group were all negative, HPV virus subtype of HPV-positive laryngeal squamous cell carcinoma were all high-risk HPV16; and there were 6 HPV virus subtypes in laryngeal papilloma (8: HPV6,4: HPV52, 1: HPV11, 1: HPV18, 2: HPV45, 3: HPV16), individual mixing two or more subtypes infection. HPV infection of laryngeal squamous cell carcinoma and precancerous lesions has no statistically significant difference according to gender, high low-risk subtypes.@*CONCLUSION@#HPV infection related to laryngeal squamous cell carcinoma and precancerous lesions, but no significant correlation with the subtype distribution of high and low risk; HPV detection is making positive sense to clinical diagnosis of laryngeal carcinoma and precancerous lesions as well as the development of specific HPV subtype vaccine.
