ABSTRACT
Objective To establish a clinical prediction model for preeclampsia by monitoring risk rating of MP gestation and levels of placental growth factor(PLGF)combined with uterine artery pulsatility index(PI)measured during examination of fetal nuchal translucency(NT).Methods Twenty-four patients with preeclampsia who met the inclusion criteria were selected as the case group,and 95 healthy pregnant women during the same period were randomly selected as the control group.Serum concentrations of PLGF,uterine artery PI values measured by quantitative immunofluorescence assay at 11-14 weeks of gestation,risk ratings for MP hypertension monitoring at 11-20 weeks of gestation,and other relevant data,BMI,age,gestation,mode of delivery,neonatal birth weight and Apgar score were collected in the two groups.Results Results of univariate regression analysis showed that BMI,age,high risk of PI,MP and PLGF<12 were influencing factors for adverse outcomes.Results of multivariate regression analysis showed that high PI,medium high risk in MP and PLGF<12 were independent risk factors for adverse outcomes.The prediction model of PE established was logit(P)=-15.767 + 0.020×PI + 0.072×MP risk(medium-high risk = 1,low risk = 0)+ 0.181×PLGF classification(<12 = 1,≥12 = 0),with an AUC area of 0.883,specificity of 0.816 and sensitivity of 0.846.Conclusion The combination of PI,MP risk and PLGF to establish a clinical predictive model for preeclampsia has certain value,and its combined predictive value is higher than that of single application.

BACKGROUND:In recent years,the demand for in vitro maturation of immature oocytes has increased.Oocyte maturation is affected by many factors,among which the selection of medium is particularly important,and there is currently no unified plan. OBJECTIVE:To compare the in vitro maturation of germinal vesicle stage oocytes with different maturation media and to investigate its effects on oocyte quality and developmental potential. METHODS:Germinal vesicle oocytes were matured in G-1TM PLUS medium,CZB medium and M16 medium,and mature oocytes in vivo were used as control group to compare in vitro fertilization and early embryo development among various groups.The immunofluorescence method was used to evaluate mitochondrial function in mature oocytes of each group.Calcium oscillation was detected by confocal microscopy real-time imaging system. RESULTS AND CONCLUSION:(1)There was no significant difference in the first polar body ejection rate among the three groups(P>0.05).(2)The rate of in vitro fertilization was higher in the G-1TM PLUS group(52.86±11.24)%than that in the M16 group(37.76±6.70)%and the CZB group(30.62±5.51)%.The blastocyst rate was lower in the CZB group(36.23±6.63)%than that in the control group(78.16±4.17)%,G-1TM PLUS group(55.75±7.63)%and M16 group(53.36±6.33)%.(3)Compared with the control group,the length-to-width ratio of the spindle in the CZB group increased(P<0.005).(4)The mitochondrial function of the CZB group was worse than that of the control group,G-1TM PLUS group and M16 group,and abnormal mitochondrial agglutination occurred in the CZB group.(5)The frequency of calcium oscillations in the CZB and M16 groups was significantly higher than that in the G1 and control groups.In conclusion,during in vitro maturation of mouse oocytes,in vitro maturation rate was not significantly different among G-1TM PLUS,CZB and M16 media,but the G-1TM PLUS medium had a higher rate of fertilization and blastocyst formation.

Placental and maternal-fetal circulation Doppler ultrasound are the main noninvasive means for maternal-fetal monitoring. However, Doppler studies on placental abnormalities are not well studied yet. Doppler monitoring of the maternal-fetal circulation, involving uterine arteries, umbilical arteries, and fetal vessels, is still used to screen diseases related to placental dysfunction (such as preeclampsia and fetal growth restriction) and to guide clinical management. This article reviews the advances in the clinical application of placental and maternal-fetal circulation Doppler in obstetrics to optimize the clinical management of disorders associated with abnormal placental structure and function.

Fetal alloimmune thrombocytopenia (FAIT) is a severe perinatal complication, which can seriously affect fetal development, and may even lead to intrauterine hemorrhage and intrauterine death. There are controversies in the clinical diagnosis and treatment of the disease due to its low incidence and limited treatment experience. This paper reviews the progress to date in understanding the condition, incidence, screening of high-risk factors, prenatal and delivery management of FAIT based on domestic and foreign guidelines, in order to help obstetricians in the clinical management of FAIT.

Objective:To study the diagnostic value of multimodal ultrasound in thyroid microcarcinoma(TMC).Methods:The clinical data of 203 TMC patients in the Sixth Hospital of Shanxi Medical University from March 2015 to January 2019 were retrospectively analyzed.The preoperative multimodal ultrasound results [high frequency ultrasonography(HFU), color Doppler (DU), ultrasonic elastography (UE)] were collected, the postoperative pathological results were seen as the gold standard, and the consistency of multimodal ultrasound diagnosis and the pathological results was analyzed, the diagnosis effect index of multimodal ultrasound was calculated, ROC curve was used to analyze the diagnostic value of multimodal ultrasound for TMC.Results:The diagnostic results of HFU, UE were highly consistent with the postoperative pathological results (0.8<Kappa≤1.0). There was a high consistency between DU and gold standard (0.6<Kappa≤0.8). The sensitivity, specificity and diagnostic coincidence rate of multimode ultrasonic in the diagnosis of TMC were 97.52%, 91.94% and 95.96%, respectively.HFU and UE had high diagnostic value for TMC (AUC>0.9). DU had medium diagnostic value for TMC (0.7≤AUC≤0.9).Conclusion:Multimodal ultrasonography has high diagnostic value for TMC, and it can be used in many fields such as clinical diagnosis, efficacy evaluation and reexamination due to its good repeatability and non-invasive.

Objective To investigate the expression and clinical significance of DJ-1 protein in the endometrial carcinoma and delineate its functional roles in endometrial carcinoma progression.Methods Expression of DJ-1 was examined with immunohistochemical staining of 69 paraffin-embedded endometrial carcinoma samples who underwent surgery at Hunan province Tumor Hospital.Kaplan-Meier analysis was performed to describe the survival curve.Univariate and multivariate analyses were performed with COX proportional hazard model.Results (1) The expression of DJ-1 protein in endometrial carcinoma was significantly higher than that in normal endometrial tissues (P < 0.05).(2) There was positive correlation between the high expression of DJ-1 protein in endometrial carcinoma samples and the pathological grade,the International Federation of Gynecology and Obstetrics (FIGO) stage,depth of myometrial invasion,or lymph node metastasis (P < 0.05).(3) Kaplan-Meier analysis indicated that the cumulative 5-year survival rate in the low-DJ-1 expression group was 80.8%,whereas it was only 51.2% in the high-DJ-1 expression group (P < O.05).COX analysis indicated that the expression of DJ-1 was not the endometrial carcinoma independent prognostic factors for overall survival time (P > 0.05).Conclusions The high expression of DJ-1 protein in endometrial carcinoma samples demonstrated a significantly positive correlation with the pathological grade,FIGO stage,depth of myometrial invasion,and lymph node metastasis.However,DJ-1 is not enough to be independent prognostic factor for overall survival time.

Objective To investigate the efficacy and tolerability of estradiol (E2) gel with postoperative patients of moderately severe ovarian endometriosis cyst,who suffer from the side effect of GnRH agonist.Methods Sixty samples were selected with stage Ⅲ ~ Ⅳ ovarian endometriosis after conservative operation by the same operator at the Department of Gynecology,Xiangya Hospital,Central South University,between February 2013 and August 2014.All of the patients after surgeries were randomly divided into three groups:control group (n =20) with only goserelin injection after conservative surgery,treatment group A (n =20) who received estradiol gel,and treatment group B (n =20) who received tibolone as an add-back therapy.The serum E2,follicle stimulating hormone (FSH),luteinizing hormone (LH),hypoestrogenic symptoms,the incidence of uterine bleeding,and the recurrence after conservative operation were compared among three groups.Results Three months later aster treatment with goserelin,the serum level of E2 of three groups showed statistically significant difference (P < 0.05).Serum E2 level of treatment group A was not significantly higher than treatment group B (P > 0.05).The kupperman sore,the incidence of hot flashes sweating,tiring,and agrypnia of treatment groups A and B were significantly lower than the control group (P < 0.05).The incidence of uterine bleeding and recurrence dont differ sharply among three groups (P > 0.05).Conclusions GnRHa combined with estradiol gel can improve symptoms of postoperative patients,reduce side effects effectively,and avoid increase of the risk of vagina bleeding and relapse.

Objective:To observe dynamic change of phosphorylated tau protein and non phosphorylated tau protein in axon and explore whether GSK3 in serum were related with phosphorylated tau protein in brain of EAE mice.Methods: Mice were randomly divided into six groups:EAE group of acute stage,EAE group of paralytic stage,EAE group of remission stage,control group of acute stage,control group of paralytic stage,control group of remission stage,each group had twelve mice.EAE model were constructed by MOG35-55 peptides in EAE group, the saline was used in control group.The nerve function scores and incidence were observed and compared.We observed degree of brain inflammation by HE staining and measured axons which contained two kinds of tau protein by immunohistochemistry.GSK3 in serum was tested to find correlation with phosphorylated tau protein in brain by ELISA method.Results:Nerve function scores in EAE group were higher than control group.The degree of inflammation damage was more serious in EAE group than control group,gradually enhancing with time.Phosphorylated tau protein in brain gradually increased before mice paralyzing(P<0.01),but it decreased when symptom relieved(P<0.01).GSK3 in serum were correlated with phosphorylated tau protein in brain(r=0.9326,P<0.01),linear regression equation:Y=2.950+0.418X.Conclusion:Phosphorylated tau protein in brain are correlated with axon damage and GSK3 in serum could indirectly reflect axon damage in brain.

Objective To investigate the association between high-risk types of human papillomavirus (HR-HPV) load and the expression of nestin in different cervical lesions.Methods The hybridization capture Ⅱ (HC-Ⅱ) assay was used to test the HR-HPV load from 60 patients who were the first time to do the cervical cancer screening in Xiangya Hospital,and immunohistochemistry was used to detect the expression of nestin in those biopsy tissue samples.Results (1) The lgHPV level (logarithm of HR-HPV load) in the high level lesion group was higher than the low level one (P <0.05),and HR-HPV load was positively associated with the degree of cervical lesions (rs =0.269,P =0.037).(2) The expression of nestin in A group was weaker than groups B and C (H =7.271,22.843,P <0.01),and the expression of nestin in C group was stronger than B group (H =7.270,P <0.01),and the expression of nestin was positively associated with the degree of cervical lesion (rs =0.646,P =0.000).(3) The HR-HPV load was positively associated with the expression of nestin (P < 0.05).Conclusions The HR-HPV load and the expression of nestin are closely related to the cervical lesions,and the joint detection has a referential value for early prevention of cervical lesions and prediction of progress of cervical precancerous lesion.It might guide the early prevention and treatment of cervical cancers.

OBJECTIVE:To evaluate the cost and effectiveness of Shenmai injection and Salvianolate for injection in adjuvant treatment of coronary heart disease with heart failure. METHODS:In prospective randomized controlled clinical study,103 pa-tients with coronary heart disease with heart failure were randomized into 2 groups:55 patients in group A received Shenmai injec-tion 50 ml added into 5% Glucose solution 250 ml intravenously on the basis of routine treatment,once a day;48 patients in group B received Salvianolate for injection 200 mg added into 5% Glucose solution 250 ml intravenously on the basis of routine treatment,once a day;treatment course lasted for 2 weeks. The improvement of clinical indicators and ADR were observed before and after treatment;2 drug treatment programs were evaluated and analyzed with pharmacoeconomics. RESULTS:Total effective rates of group A and B were 89.09% and 72.92%,with statistical significance(P0.05). The cost-effectiveness ratio of group A was significantly lower than that of group B,with statistical significance(P<0.05);the results of sensitivity analysis was consistent with it,indicat-ing the cost of Shenmai injection was in low level. CONCLUSIONS:Shenmai injection is more economic in adjuvant treatment of coronary heart disease with heart failure.

<p><b>OBJECTIVE</b>To investigate the effects of sevoflurane on cytosolic phospholipase A₂ (C-PLA₂) and clara cell secretory protein (CCSP) in lung tissues of rabbits with one-lung ventilation (OLV)-induced lung injuries.</p><p><b>METHODS</b>Thirty-six healthy Japanese white rabbits were randomized into sham-operated group, OLV group, and OLV plus sevoflurane group subdivided into 4 subgroups with sevoflurane concentrations of 1%, 2%, 3% and 4%. CCSP and C-PLA₂ mRNA and protein expressions in rabbit lung tissues were detected by Western blotting and real-time PCR, and the content of arachidonic acid (AA) was measured using ELISA. The severities of the lung injury were evaluated according to lung wet/dry weight (W/D) ratio and histological scores.</p><p><b>RESULTS</b>In the OLV group and OLV+ sevoflurane groups, pulmonary CCSP expressions were significantly lower, while C-PLA₂ expression, lung W/D ratios and lung histological scores were significantly higher than those in the sham-operated group (P<0.05). Compared with OLV group, the OLV+sevoflurane groups showed significantly increased expressions of CCSP and reduced C-PLA₂ expression, lung W/D ratios and histological scores (P<0.05). In the 4 OLV+sevoflurane groups, CCSP expressions underwent no significant changes as sevoflurane concentration increased, but C-PLA₂ expressions, lung W/D ratios and histological scores all decreased gradually as the concentrations of sevoflurane increased (P<0.05).</p><p><b>CONCLUSION</b>OLV can result in down-regulated CCSP expressions and up-regulated C-PLA₂ expressions in rabbit lung tissues. Sevoflurane can protect against OLV-induced acute lung injury possibly by inhibiting C-PLA₂ expression via up-regulation of CCSP expressions or through other mechanisms resulting in down-regulated expression of C-PLA₂.</p>

Long non-coding RNAs (lncRNAs) are non-coding RNAs that have transcript lengths exceeding 200 bp and do not have the capacity for protein coding because of having no open reading frame. In the human genome, lncRNAs play important regulato-ry roles in the process of epigenetic, transcription and post-transcription, so they have become the focus of research followed by mi-croRNAs. The abnormal expression of lncRNAs in cancer usually represents different functions, such as the function of oncogenes or tumor suppressor genes, which promote or inhibit tumor growth. Lung cancer is a common malignant tumor with a five-year survival rate of 17%. Previous literature shows that MALAT1, H19, lincRNA p21, UCA1, and BC200 are closely related to the development of lung cancer. They could promote cancer growth, invasion and metastasis, apoptosis and induce drug resistance, and so on. This review aims to provide assistance in the diagnosis, treatment, and prognosis of lung cancer by clarifying the functions and mechanisms of ln-cRNAs.
ABSTRACT
Lipid droplets, which are conserved across almost all species, are cytoplasmic organelles used to store neutral lipids. Identification of lipid droplet regulators will be conducive to resolving obesity and other fat-associated diseases. In this paper, we selected 11 candidates that might be associated with lipid metabolism in Caenorhabditis elegans. Using a BODIPY 493/503-based flow cytometry screen, 6 negative and 3 positive regulators of fat content were identified. We selected one negative regulator of lipid content, C13C4.5, for future study. C13C4.5 was mainly expressed in the worm intestine. We found that this gene was important for maintaining the metabolism of lipid droplets. Biochemical results revealed that 50% of triacylglycerol (TAG) was lost in C13C4.5 knockout worms. Stimulated Raman scattering (SRS) signals in C13C4.5 mutants showed only 49.6% of the fat content in the proximal intestinal region and 86.3% in the distal intestinal region compared with wild type animals. The mean values of lipid droplet size and intensity in C13C4.5 knockout animals were found to be significantly decreased compared with those in wild type worms. The LMP-1-labeled membrane structures in worm intestines were also enlarged in C13C4.5 mutant animals. Finally, fertility defects were found in C13C4.5(ok2087) mutants. Taken together, these results indicate that C13C4.5 may regulate the fertility of C. elegans by changing the size and fat content of lipid droplets by interfering with lysosomal morphology and function.

Objective To study the influence of hierarchical model on the professional attitude and quality improvement of nurses and supply references for the apphcation of hierarchical model.Methods 310 nurses were divided into the control group(158 cases)and the practice group(152 cgses).The practice group adopted hierarchical model while the control group did not.Investigation was carried out in 15 aspects such as training requirement,choosing of nursing posts and departure intention,etc.Results The nursing teaching pest and nursing management post in the aspect of post requirement between the two groups had no statistical difference(P>0.05),but statistical difference existed in other items(P<0.05).In the aspect of training requirement evident difference existed in two groups(P<0.05).Conclusion Application of hierarchical model proved to be a pivotal method to improve the professional passion and loyalty of nurses.

Objective To discuss the variance of working expectation values and actual satisfaction degree in nursing college graduates from various aspects and evaluate their actual career mentality.Methods We investigated the working expectation values and actual satisfaction degree in nursing college graduates who graduated from Chengde medical college in 2004 and 2005.Results The working expectation values and actual satisfaction degree had statistical difference in 5 aspects(P<0.05).We subsequently compared the expectation values between each two groups and drew out the conclusion that these nursing graduates had great requirement of being understood and respected.Conclusion The nursing graduates had higher working expectation values and lower satisfaction degree.They paid more attention to be understood and respected and were in great need of emotional support and mental satisfaction.Hospital administrators should help them accurately orient theirselves,establish career development plan,attach importanee to their mental requirement and adopt effective measures to improve their satisfaction degree and promote the healthy growth of nursing college graduates.

Aim To investigate the protective effects of high concentration fentanyl on the brain slice injury induced by oxygen glucose deprivation(OGD).Methods Rat brain slices were made and randomly assigned to four groups:control(n=10),OGD(n=10),fentanyl 50 ?g?L~(-1)(F_(50),n=10) and fentanyl 500 ?g?L~(-1)(F_(500),n=10).Changes of the neuron injury and apoptosis were observed with TTC staining,LDH releases,TUNEL staining,immunohistochemistry and electromicroscope.In addition,changes of intracellular calcium were measured with confocal laser-scanning microscopy.Results F_(50) and F_(500) attenuated the decrease of TTC staining and the increase of LDH release induced by OGD in brain slices.Neuronal apoptosis and changes of neuronal ultrastructures were attenuated by F_(50) and F_(500).Bcl-2 and Bax protein expressions were increased after OGD.Bax protein expression was decreased by F_(50) and F_(500),while Bcl-2 protein expression was increased by F_(50)and F_(500).Intracellular calcium concentration was increased by OGD and then it was lowered by F_(50) and F_(500).The protective effects of F_(50) were more obvious than that of F_(500).Conclusions High concentrations of fentanyl have neuron protective effects against OGD injury in rat brain slices,and fentanyl 50 ?g?L~(-1) has more obvious protective effects than fentanyl 500 ?g?L~(-1).

Aim To compare the different effect of intracarotid and intravenous injection of propofol on the neuronal apoptosis of hippocampus in rats undergoing cardiopulmonary bypass.Methods Fifty rats were randomly assigned to five groups. Shame group (Group Non-CPB,n=10); group cardiopulmonary bypass(group CPB,n=10);group intravenous injection of propofol 2 mg?kg -1?h -1 (group P_ 2V,n=10);group intravenous injection of propofol 10mg.kg -1?h -1 (group P_ 10V,n=10);group intracarotid injection of propofol 2 mg?kg -1?h -1 (group P_ 2a,n=10).Mean arterial pressure(MAP)and heart rate(HR) were monitored. Changes of the neuronal apoptosis of hippocampus were observed with TUNEL staining, Bax and bcl-2 mRNA and protein expressions were measured by RT-PCR and Western blot. In addition,neuronal ultrastructures were observed. Results Neuronal apoptosis and changes of neuronal ultrastructures of hippocampus were attenuated in group P_ 2a and P_ 10V,but the apoptosis of neurons was not attenuated in group P_ 2v. Bcl-2 and Bax mRNA expression increased undergoing CPB. Bax mRNA expression decreased after CPB in group P_ 2a and P_ 10V. Bcl-2 mRNA expression increased in group P_ 2a and P_ 10V. But Bcl-2 and Bax mRNA expression in group P_ 2V was not significantly different compared with that of group CPB. Bcl-2 and Bax protein expression measured by Western blot showed the same trend with mRNA expressions. Compared with baseline values, MAP and HR in group P_ 2V and P_ 2a remained unchanged. But the MAP and HR in group P_ 10V changed compared with group CPB and baseline value.Conclusions intracarotid injection of propofol attenuated the injury of brain at a fraction of the intravenous dose and with less adverse systemic and cerebrovascular side effects.
