ABSTRACT
Objective:To analyze the clinical characteristics of Chlamydia psittaci pneumonia and to investigate the correlation between serum inflammatory biomarkers and severity of the disease. Methods:Sixteen patients with Chlamydia psittaci pneumonia admitted to the Huizhou Municipal Central Hospital from January 2020 to July 2021 were selected as the study subjects, including 10 severe cases and 6 mild cases. Clinical data were collected and analyzed, such as baseline characteristics, clinical symptoms, laboratory inspection and chest imaging manifestations. Results:① Thirteen Chlamydia psittaci pneumonia were associated with a definite bird or poultry contact history. Common symptoms included high fever, chill, cough, fatigue, and anorexia (16 cases), dyspnoea (12 cases), and other systemic symptoms. ② Laboratory test results showed normal white blood cell count (WBC, 10 cases), decreased lymphocyte count (LYM, 13 cases), increased high sensitive C-reactive protein (hs-CRP, 16 cases), D-dimer (15 cases), lactate dehydrogenase (LDH, 13 cases), aspartate aminotransferase (AST, 16 cases) and alanine aminotransferase (ALT, 12 cases) levels, however, the albumin (Alb, 15 cases) lever was decreased. The numbers of CD3 + T cells and CD4 + T cells decreased in 10 patients. ③ The levels of D-dimer, interleukins (IL-2, IL-6, IL-10) in severe Chlamydia psittaci pneumonia were significantly higher than those in mild Chlamydia psittaci pneumonia [D-dimer (μg/L): 10 257±4 203 vs. 1 085±642, IL-2 (ng/L): 1.1 (0.8, 1.7) vs. 0.3 (0.1, 0.7), IL-6 (ng/L): 315 (182, 505) vs. 75 (18, 131), IL-10 (ng/L): 7.0±4.1 vs. 2.3±0.7], but the LYM was lower (×10 9/L: 0.4±0.1 vs. 1.1±0.4), with significant differences (all P < 0.05). ④ Chest imaging manifestations were exudative lesions and large consolidation of lungs. Large consolidation of both lungs can occur in some critically ill patients. Conclusions:Chlamydia psittaci pneumonia is mainly associated with a bird or poultry contact history. The clinical manifestations usually present high fever, dyspnea, normal or slightly increased leucocytes, and lung consolidation. The levels of LYM, D-dimer, IL-2, IL-6 and IL-10 in serum are expected to predict the severity of the Chlamydia psittaci pneumonia.
ABSTRACT
OBJECTIVE: To analyze the characteristics and regularity of new/severe pediatric ADR in Shandong province, and to promote the safe use of drugs in children. METHODS: A retrospective analysis of new/severe pediatric ADR in the Shandong Provincial ADR database 2016-2017 was conducted in respects of children’s gender and age, primary disease, ADR history, family ADR history, dosage form, route of administration, drug type, systems/organs involved in ADR, clinical manifestations, off-label drug use, drug combination, occurrence time, effects of ADR on primary disease, outcome, etc. RESULTS: A total of 44 742 pediatric ADR cases were collected from Shandong province ADR database from 2016 to 2017, including 27 060 male, 17 664 female and 18 gender unknown. 530 cases were diagnosed as new/severe pediatric ADR, including 334 male and 196 female with ratio of male to female 1.70 ∶ 1. New/severe ADR reports of children aged 1-3 took up the highest proportion (158 cases, 29.81%). Primary diseases were mainly respiratory disease (190 cases, 25.85%); there were 10 children with ADR history (1.89%), 2 with family ADR history (0.38%). Dosage forms were mainly injection (358 cases, 67.55%). Route of administration were mainly intravenous drip (431 cases, 81.32%). The drugs that caused ADR included 20 categories and 162 species, mainly including drug for regulating hydroelectric acid-base balance (148 cases caused by 8 kinds of drugs, 27.92%), antibiotics (89 cases caused by 33 kinds of drugs, 16.79%), traditional Chinese medicine and its extract (80 cases caused by 24 kinds of drugs, 15.09%). The systems/organs involved in ADR were mainly systemic injury (201 cases, 37.92%, main clinical manifestations included chills and fever, etc.), followed by skin and its appendants (99 cases, 18.68%, mainly clinical manifestations included rash and itching, etc.), respiratory system (76 cases, 14.43%, main clinical manifestations included dyspnea and cough, etc.). Off-label drug use were found in 41 cases (7.74%), including the safety of drug use was not clear in drug instructions (20 cases, 3.77%); no drug testing was carried out and no reliable references were available (13 cases, 2.45%), that medicine was prohibited was stated in drug instructions (2 cases, 0.38%). 106 cases (20.00%) had drug combination, including combined use of two drugs, three drugs and four drugs (62, 36, 8 cases). ADR occurred mainly within 0-5 min (140 cases, 26.42%). Among 530 children, ADR had no obvious effect on the outcome of the disease in 457 cases (86.23%); ADR caused longer course of disease in 57 cases (10.75%). 278 cases (52.45%) were cured and 243 cases (45.85%) were recovered. CONCLUSIONS: It is necessary to strengthen the monitoring of drug use in children, formulate national standards and relevant laws and regulations for children’s rational drug use, improve the awareness of medical staff to children’s ADR, strengthen the education and publicity of the knowledge about safe drug use in children, and to promote rational drug use.
ABSTRACT
Objective To explore the effective method for preserving laryngeal function in the treatment of laryngeal cancer of T3 lesions. Methods Utilizing unilateral pedicled muscle-periosteal flap, 32 cases with laryngeal carcinoma of T3 lesions underwent extended vertical partial laryngectomy for laryngeal cavity structure after one-stage reconstruction. Results With 1-3 years postoperative follow-up, the patients had good pronunciation, unobstructed respiration and no tumor recurrence, with extubation rate of 100%. Conclusion It showed that treating patients with laryngeal carcinoma of T3 lesions with extended vertical partial laryngectomy using unilateral pedicled muscle-periosteal flap reconstruction for laryngeal cavity structure can remove tumor effectively and obtain the satisfactory respiratory pronunciation function. The method has a good clinical application prospect.

Objective To evaluate the accuracy of contrast enhanced ultrasonography (CEUS) in determining the depth of myometrial invasion in endometrial carcinoma in stage Ⅰ.Methods Seventy-six patients previously diagnosed of endometrial carcinoma by curettage of uterine underwent transabdominal sonography (TAS) and CEUS to assess myometrial invasion,among which 48 patients proved to endometrial carcinoma in stage Ⅰ after total abdominal hysterectomy and bilateral salpingo-oophorectomy were studied.The findings of TAS and CEUS to determine endometrial carcinoma IA (no myometrial involvement or invasion of the inner half of the myometrium) and IB( invasion of the outer half of the myometrium) were compared with pathology after abdominal hysterectomy.Results Twenty one tumours (43.75%,21/48) were enhanced earlier than or simultaneously as myometrium and cervix,among which 12 cases were IA stages,while 9 cases were IB stages (P0.05).The sensitivity,specificity,positive predictive value,negative predictive value and accuracy of TAS and CEUS in diagnosing endometrial carcinoma in IB were 61.25% vs 69.23%,77.14% vs 85.71%,50.00% vs 64.28%,72.92% vs 88.23%,72.92% vs 81.25% respectively.Conclusions CEUS is not superior to TAS in detecting deep invasion of endometrial carcinoma in stage Ⅰ.
ABSTRACT
The manufacturing process, quality Control and Stability test of "901" anti-influenza complex granule were presented. The granule tallied with requirements of Chinese Pharmacopiea (1990th edition). It s main components (Scutellaria baicallensis Georgi. and Bupleurum chinese DC.) were identified with TLC. First derivative spectrometry was used to determine the content of baicalin in this preparation. As a result of stability test, the shelf life at 250℃ is estimated to be 1.97 years.
