ABSTRACT

Objective:To evaluate the effect of health education project on prevention and control of endemic diseases in Henan Province.Methods:According to the distribution and condition of endemic diseases in Henan Province, from 2008 to 2015, five project counties (cities, districts, referred to as counties) were selected in each provincial city. Three townships (towns) were selected from each project county as project townships (towns), and one central primary school and the village where the school was located were selected from each township (town) as health education survey sites. The prospective study method was adopted. Firstly, the students in the primary school of the project township (town) and the housewives in the project village were investigated with questionnaire. The intervention activities of health education were carried out in the county, township (town), village and school of the project. The intervention effect of health education was evaluated by questionnaire survey two months later.Results:There were 111 870 people investigated in Henan Province, including 74 580 students and 37 290 housewives. A total of 99 671 people were surveyed after the intervention, including 66 249 students and 33 422 housewives. Elementary student's awareness rate of endemic diseases prevention and control knowledge increased from 59.74% before intervention to 92.89% after intervention, the housewives' awareness rate increased from 65.62% before intervention to 91.82% after intervention. The awareness rate of the target population increased from 61.70% before intervention to 92.53% after intervention ( P < 0.01); among them, the awareness rates of iodine deficiency disorders, drinking water type fluorosis, coal-burning type fluorosis, drinking water type arsenic poisoning, Kashin-Beck disease and Keshan disease were significantly increased ( P < 0.01). Conclusion:With the implementation of health education project, the awareness rate of knowledge on endemic diseases has increased distinctly.

ABSTRACT

Objective:To summarize the clinical characteristics and experiences in the diagnosis and treatment of congenital tuberculosis.Methods:This study involved four infants with congenital tuberculosis diagnosed in Guangdong Women and Children Hospital from January 2010 to January 2020. Moreover, publications of 52 congenital tuberculosis cases from Chinese hospitals between January 1994 and January 2019 were retrieved from Wanfang Database, China National Knowledge Infrastructure and PubMed. Clinical data of all patients were retrospectively analyzed.Results:All four cases we reported here were preterm infants with the onset of the disease at 14-30 d after birth and received anti-tuberculosis treatment. The interval between disease onset and diagnosis was 1-34 d. One baby was cured, one was improved, while the other two died. The median age at the onset of the disease in these 56 cases (including the four we reported and 52 from literature) was 14.5 d (7.0-20.7 d) and the median interval between the onset and diagnosis was 10.5 d (7.5~22.0 d). Forty-eight (88.9%) out of the 54 mothers were suffered from active tuberculosis during the perinatal period, including 16 diagnosed before delivery. The main clinical symptoms in these neonates were fever (82.1%, 46/56), respiratory distress (69.6%, 39/56), drowsiness and/or irritation (57.1%, 32/56), cyanosis (53.6%, 30/56), feeding difficulties (53.6%, 30/56), hepatomegaly (48.2%, 27/56) and splenomegaly (41.1%, 23/56). The positive rates of acid-fast staining, culture and polymerase chain reaction detection of Mycobacterium tuberculosis in sputum smears or gastric juice were all less than 33.3%, and the positive rate of spot test for Mycobacterium tuberculosis infection was 7/12. Chest X-ray showed miliary nodules and diffused nodules-patchy in 37.0% (20/54) and 25.9% (14/54) of the patients, respectively. Abdominal ultrasonography found 52.4% (11/21) with hepatomegaly and/or splenomegaly and 47.6% (10/21) with multiple low-density nodules in liver and spleen. The misdiagnosis rate was 28.6% (16/56). The overall mortality rate was 51.8% (29/56). However, the figure was 28.9% (11/38) in infants with anti-tuberculosis treatment and 18/18 in those without. Conclusions:Nonspecific clinical manifestations of congenital tuberculosis, together with low detection rate through laboratory tests, may lead to a higher rate of misdiagnosis and mortality. Therefore, a comprehensive assessment of the mothers of infants with suspected congenital tuberculosis is necessitated. Diagnosis and effective anti-tuberculosis treatment as early as possible are essential to improve the prognosis.

ABSTRACT

Objective:To evaluate the effectiveness of multi-disciplinary treatment (MDT) for reducing carbapenem-resistant Enterobacteriaceae (CRE) infections in intensive care unit(ICU).Methods:Patients admitted in the emergency ICU (EICU) and neurosurgical ICU (NICU) of the First People’s Hospital of Lianyungang during January 2018 to December 2019 were enrolled in the study, which was analyzed by historical control study. For patients admitted in 2018, the conventional method was adopted for prevention and control of drug-resistant bacteria (control group, n=1, 076), and for patients admitted in 2019, the MDT was adopted for the prevention and control of CRE (intervention group, n=1, 237). Chi-square test was used to compare the incidence rate of CRE infection, CRE detection rate, rate of rational antibiotic use and compliance with implementation of prevention and control measures between two groups. Results:Compared to control group, the incidence rate of CRE infection in EICU and NICU decreased from 3.45% (14/406) and 3.58% (24/670) to 1.65% (9/547) and 2.32% (16/690) in intervention group, respectively; while the detection rate of CRE decreased from 66.21% (96/145) and 57.72% (86/149) to 41.11% (51/124) and 33.06% (40/121), the pathogens were mainly carbapenem-resistant Klebsiella pneumoniae (CRKP). The rational medication rate of carbapenem antibiotics was significantly increased from 65.00%(78/120) in 2018 to 92.73%(319/344) in 2019 ( χ2=55.382, P<0.05). In addition, the single room isolation rate, the rate of specialized nursing care, the cleaning and disinfection quality of bench surface and the special use rate of articles were also significantly improved( χ2=21.646, 18.116, 39.869 and 19.713, P<0.01). Conclusion:The establishment of multi-department collaborative management based on MDT can effectively improve the prevention and control effect of CRE in ICU and significantly reduce the prevalence of CRE infection.

ABSTRACT

Objective:To explore the value of 18F-fluorodexyglucose (FDG) PET/CT combined with integrated contrast-enhanced CT on the diagnosis of hepatic epithelioid hemangioendothelioma(HEH). Methods:Six patients (2 males, 4 females, age: (41.0±5.6) years) histopathologically confirmed to be HEH in Qingdao Central Hospital between November 2013 and November 2018 were retrospectively analyzed. All patients underwent 18F-FDG PET/CT dual-phase imaging and three-phase dynamic enhanced scanning with integrated CT. Characteristics of 18F-FDG PET/CT and contrast-enhanced CT images were classified and analyzed. Results:All 6 patients had multi-lesions (30 lesions in total). The capsule retraction sign was found in 16.7% (5/30) lesions, target sign was found in 33.3% (10/30) lesions, and " lollipop sign" was found in 13.3% (4/30) lesions. There were three ways of enhancement showed by CT: mild progressive enhancement, delayed enhancement, and black target sign/white target sign in the portal phase. Among the 30 lesions, 66.7% (20/30) had higher 18F-FDG uptake than liver parenchyma, with maximum standardized uptake value (SUV max) of 4.18±0.64 during routine imaging and 4.23±0.70 during delayed imaging, and the retention index was 0.65(-1.88, 4.60). The rest 33.3% (10/30) showed similar 18F-FDG uptake to liver parenchyma, with SUV max of 2.75±0.52 during routine imaging, and 2.78±0.55 during delayed imaging. The uptake of 18F-FDG increased with time in 22 lesions and decreased in 8 lesions. In the metabolically heterogeneous lesions, the relatively high-metabolization site was also the site with higher peak enhancement; in the lesions with uniform metabolism, the CT enhancement was also uniform. Bilateral pulmonary metastases were found in 2/6 patients. Conclusion:18F-FDG PET/CT dual-phase imaging combined with CT three-phase dynamic enhanced scanning is helpful in accurate diagnosis of HEH and could show extrahepatic metastases.

ABSTRACT

Objective:To explore the main influencing factors of the impact of new nurses′ career transition shock in three years, and to provide theoretical basis for the clinical development of new nurses' vocational training and management strategy.Methods:By using the method of objective sampling, 386 new nurses in 5 first-class tertiary hospitals in Tianjin were selected as the research objects within three years. The Jefferson Scale of Empathy (JSE), Organizational Support Scale (OSS), the Transition Shock of Newly Graduated Nurses Scale (TSNGNS) and the self-designed general information questionnaire were used to analyse the main impact of new nurses' career transition shock in three years.Results:The total score was (85.76±25.14) points for 386 new nurses in three years; the scores of empathy ability and organizational support were (81.24±05.21) points and (45.74±12.51) points. The regression analysis showed that whether a new nurse loves her major, how long she has been engaged in nursing work, empathy, total score of empathy ability and emotional support are the main influencing factors for the impact of the new nurse ′s career transition shock in three years ( t values were 2.068, 2.324, 4.155, 2.137, 3.938, P<0.05). Conclusions:To strengthen the training and emotional support intervention for new nurses who have just been engaged in nursing work for 7-9 months, who have conflicts in nursing work and lack of empathy ability, so as to reduce the impact of their career transition shock.

ABSTRACT

OBJECTIVE@#To explore the clinical phenotype and pathogenic variants in a Chinese pedigree affected with Smith-Lemli-Opitz syndrome.@*METHODS@#Peripheral blood samples were collected from five members, including two affected ones, from the pedigree for the extraction of genomic DNA. Whole exome sequencing was carried out, and candidate variants were verified by Sanger sequencing as well as reverse transcription sequencing at the RNA level.@*RESULTS@#The proband and another affected child from the pedigree showed mental retardation, dyskinesia, microcephaly, micrognathia, anteverted nares, and 2/3 toe syndactyly. The proband also had hypospadia, single upper incisor, and lower serum cholesterol level. Both children were found to harbor a paternally derived c.278C>T (p.T93M) variant and a maternally derived c.907G>A (p.G303R) variant of the DHCR7 gene. Both were known pathogenic mutations.@*CONCLUSION@#The compound heterozygous mutations of c.278C>T (p.T93M) and c.907G>A (p.G303R) of the DHCR7 gene probably underlay the disease in this pedigree. Above finding has enabled early diagnosis and treatment of Smith-Lemli-Opitz syndrome.

ABSTRACT

Objective: To explore the effects of atorvastatin on the natural antisense transcription of apolipoprotein A1 (apoAl-NAT) in the HepG cells and its influence in the expressions of lipid metabolism related genes. Methods: The HepG cells were intervened with different concentrations (0, 1, 10 and 100 nmol • L-1 ) of atorvastatin, and the 0 nmol • L-1 atorvastatin group was used as control group. The total RNA of HepG cells in various groups were extracted at different time points (6, 12, 24, and 48 h). The mRNA expression levels of lipid metabolism-related genes and apoAl-NAT expression levels were detected by Real-Time PCR method. Results: The morphology of HepG cells in 1 and 10 nmol • L-1 atorvastain groups was normal. ABSTRACT

Objective@#To investigate the clinicopathological features of extensively myxoid and hyalinized sinonasal capillary hemangioma (EMH-SNCH).@*Methods@#Three cases of EMH-SNCH were collected from the First Affiliated Hospital of Zhejiang University School of Medicine and Zhejiang Provincial People′s Hospital from January 2014 to December 2017. The clinical features, histomorphologic changes and immunohistochemical findings were analyzed and the relevant literature was reviewed.@*Results@#All three patients were elderly men aged 65, 51 and 80 years, respectively. Clinically, all cases presented with nasal congestion with repeated epistasis. The maximum diameters of the tumors were 2.0, 3.5 and 1.8 cm respectively. Microscopically, the lesions consisted of randomly distributed, diffusely proliferating capillary-like blood vessels within a loose interstitium with extensive myxoid and hyalinized degeneration. Myxoid and hyalinized regions occupied the vast majority of tumors, ranging from 80% to 95%. Scattered staghorn-shaped branching blood vessels were seen in the tumor, some of which were distended by thrombi. In the deep part of the lesion, capillary clusters rich in endothelial cells with focal lobular arrangement were observed. Immunohistochemically, the vascular endothelial cells expressed CD31, CD34 and ERG diffusely and β-catenin at the membrane, and the pericytes surrounding the capillaries expressed SMA. All three patients were followed up for 16 to 46 months, and no tumor recurrence was observed.@*Conclusions@#EMH-SNCH is a rare subtype of capillary hemangioma of the sinonasal cavity. It is biologically benign and its histological differential diagnosis spectrum is broad. Careful morphologic observation combined with immunohistochemical staining can help its diagnosis and differential diagnosis.

Objective@#To analyze the predictive value of lactate/albumin(L/A) ratio in neonatal infants with septic shock.@*Methods@#The data of 185 neonatal infants with sepsis from January to December 2017 were retrospective analyzed in Guangdong Women and Children Hospital.The data were divided into low level of L/A group(L/A<0.1), medium level of L/A group(0.1≤L/A<0.2) and high level of L/A group(L/A≥0.2)according to L/A ratio level within 24 hours of sepsis.The clinical features, related laboratory indications and prognosis among the 3 groups were compared.Multivariate Logistic regression was used to analyze the effects of various factors on the occurrence of neonatal septic shock.Receiver operating characteristic (ROC) curve analysis was performed to evaluate the early predictive va-lue of related indicators for neonatal septic shock.@*Results@#A total of 185 neonatal infants with sepsis were collected.There were 88 cases in low level of L/A group, 64 cases in medium level of L/A group and 33 cases in high level of L/A group.There were no significant difference in gender, gestational age, birth weight, 1 and 5-minute Apgar scores, small for gestational age, use of pulmonary surfactant, maternal preeclampsia, gestational diabetes mellitus, maternal fever and severe meconium stained (all P>0.05). There were no statistical differences among three groups in C-reactive protein, white blood cell count, neutrophil count and proportion, leucine count and proportion and platelet count(all P>0.05). Compared with low and medium level of L/A groups, the high level of L/A group had higher proportion of cesarean section, higher procalcitonin and lactate, severe intracranial hemorrhage, periventricular leucomalacia, multiple organ dysfunction syndrome, disseminated intravascular coagulation, septic shock and the dead; low level of L/A group had higher proportion of maternal premature rupture of membranes>24 hours, albumin, pH or base excess(BE), and there were statistically significant differences in all of those(all P<0.05). Multivariate Logistic regression analysis showed that hypoalbuminemia, hyperlacticemia, decreased pH and L/A≥0.1 were independent predictors for neonatal septic shock.L/A more than 0.114 within 24 hours was associated with neonatal septic shock, with a sensitivity of 76.90% and specificity of 81.30%.@*Conclusions@#The increased of L/A within 24 hours of sepsis can be used as an independent risk factor for predicting septic shock in neonatal infants.

Objective@#To investigate the value of mechanical power (MP) to predict the potential of the lung recruitment maneuver and assess prognoses in patients with acute respiratory distress syndrome (ARDS).@*Methods@#Patients with moderate-to-severe ARDS who required mechanical ventilation therapy longer than 24 hours were randomly selected April 2017 to April 2019. The lung recruitment maneuver (RM) protocol was performed according to the patient's condition, their MP, lung ultrasound score (LUS), oxygenation index (P/F), and dead volume ratio (Vd/Vt), which were monitored before the RM and one hour after the RM. The patients were divided into the lung recruitment maneuver potential positive group (RMP-P group) and lung recruitment maneuver potential negative group (RMP-N group) according to whether the variation in the patient's RM aeration score (ΔLUS) was≥5. Differences in MP between the two groups were compared and correlations between MP variation (ΔMP), aeration score variation (ΔLUS), oxygenation index variation (ΔP/F), and dead volume ratio variation (ΔVd/Vt) were analyzed. The receiver operating characteristic curve (ROC) was used to calculate the predictive value of MP for the potential of RM and the prognosis (28-day mortality) of patients with moderate or severe ARDS.@*Results@#Eighty-three patients were enrolled in the study, 45 in the RMP-P group and 38 in the RMP-N group. There was no statistical difference in the baseline levels between the two groups (P>0.05). After RM treatment, MP, LUS scores, and Vd/Vt for each patient in the two groups decreased significantly compared to the pre-RM values, whereas, the P/F increased significantly (P<0.05). The MP, LUS scores, and Vd/Vt in the RMP-P group were significantly lower than those in the RMP-N group (P<0.05), whereas there was no difference in the P/F between the two groups after RM treatment (P>0.05). The ΔMP, ΔLUS, and ΔVd/Vt in the RMP-P group were significantly higher than those in the RMP-N group (P<0.05). There was no difference in the ΔP/F between the two groups (P>0.05). There was a significant positive correlation between ΔMP and ΔLUS and ΔVd/Vt in all patients (r=0.4746, 0.3995, P<0.01) and no significant correlation withΔP/F (r=0.0314, P>0.05). The area under the ROC curves of ΔMP for predicting the potential of RM was 0.856, with a sensitivity of 91.1% and a specificity of 63.2%. The AUC of the 28-day survival status was 0.788, with a sensitivity of 84.3% and a specificity of 62.5% (P<0.05).@*Conclusions@#Mechanical power monitoring before and after RM can be used to predict the potential of the lung recruitment maneuver and assess the prognosis in patients with moderate-to-severe ARDS.

ABSTRACT

Objective To describe the characteristics of radiation induced mucositis in patients withhead and neck cancer ( HNC) during radiotherapy, and analyze the effect of radiation induced mucositis on diet patterns and weight change and the influencing factors for radiation induced mucositis. Methods Patients with HNC treated with radiotherapy in one cancer hospital were recruited. Data were collected before, during and at the end of the radiotherapy, which included radiation induced oral and pharyngeal mucositis, pain during eat-ing, diet patterns and weight. Results Two hundred and two patients were completely investigated and 43.5%and 34. 2% of the patients suffered from moderate to severe (≥grade 2) oral mucositis and pharyngeal mucosi-tis, respectively during the radiotherapy. At the end of radiotherapy, 53. 5% and 51. 5% of the patients suffered from moderate to severe oral mucositis and pharyngeal mucositis (≥grade 2 ) , respectively. Oral and pharyngeal mucositis were significantly correlated with pain during eating, diet patterns and weight ( P<0. 05) . Tumor site was the main reason that affected the severity of mucositis ( Wald χ2 =26. 033, 14. 216;P<0.001). Conclusion Radiation induced mucositis was gradually aggravated with radiotherapy progress, which is closely related to pain during eating, change of diet patterns and weight loss. The severity of mucositis is re-lated to the tumor site. Measures should be taken to strengthen the management of adverse reactions and nutri-tional status of patients.

ABSTRACT

OBJECTIVE: To establish the elimination method of outliers based on Grubbs rule and MATLAB language, and to evaluate the effects of it on drug bitterness evaluation. METHODS: Referring to Grubbs rule, the automatic cyclic outliers elimination method based on MATLAB language was established. Totally 20 volunteers were included in single oral taste test (Tetrapanax papyrifer) and multiple oral taste test (10 kinds of medicinal material as T. papyrifer, Changium smyrnioides, Poria cocos, etc.). Seven sensors were selected for electronic tongue test (Clematis armandii). The data of bitterness evaluation in above tests (oral taste test as bitterness value, electronic tongue test as response value of sensors) were used as the data source. Five researchers were selected and adopted table-by-table elimination method based on Grubbs rule (method one), Excel software elimination method based on Grubbs rule (method two) and automatic cyclic outliers elimination method based on Grubbs rule and MATLAB language (method three) to judge and eliminate the outliers. The effects of above three methods were evaluated with the removal time and error rate of outliers as indexes. RESULTS: There were two outliers in the data of bitterness evaluation in single oral taste test; the elimination time of the three methods were(745.400 0±25.904 4),(288.333 3±31.253 1)and(0.000 3±0.000 0)s, respectively; error rates were 20.0%, 0 and 0, respectively. There were six outliers in the data of bitterness evaluation in multiple oral taste test; the elimination time of three methods were (3 693.107 7±75.023 3), (1 494.761 4±53.826 9), (0.005 2±0.000 0)s, respectively; error rates were 10.0%, 4.0%, 0, respectively. There were three outliers in the data of bitterness evaluation in electronic tongue test; the elimination time of three methods were (2 992.673 3±84.117 6), (1 276.367 1±55.024 5), (0.002 3±0.000 0)s, respectively; error rates were 5.7%, 2.9%, 0, respectively. The elimination results of the three methods were consistent. The elimination time of method two was significantly shorter than that of method one (P<0.01); the elimination time of method three was significantly shorter than those of method one and method two (P<0.01). There was no significant difference in error rate of 3 methods (P>0.05). CONCLUSIONS: The automatic cyclic elimination method of outliers based on Grubbs rule and MATLAB language can significantly shorten the elimination time of outliers in data of drug bitterness evaluation, improve the efficiency of data processing, and is suitable for drug bitterness evaluation.

Objective To describe the characteristics of dysphagia in patients with head and neck cancer during radiotherapy,and analyze the cause of dysphagia and the relationship between dysphagia and weight change.Methods Patients with head and neck cancer treated with radiotherapy in Beijing Cancer Hospital from November 2017 to June 2018 were recruited.The M.D.Anderson Dysphagia Inventory (MDADI) was used to assess the dysphagia of patients before (T1),during (T2) and at the end (T3) of the radiotherapy.Meanwhile,the weight,dietary intake and radiation adverse effects were investigated.Generalized estimating equations were used to analyze the impact factors of dysphagia and the relationship among dysphagia,dietary intake and weight change.Results Ninety-six patients were completely investigated and the incidence of dysphagia were 6.3% (6/ 96),80.2% (77/96) and 85.4% (82/96) in T1,T2 and T3.With the progress of radiotherapy,the total score and the scores of 4 dimensions of MDADI dropped and the dietary intake declined.The average weight loss was (5.09±3.31) kg,and 76.1% of the patients had weight loss over 5% at the end of radiotherapy.The occurrence of dysphagia was associated with the radiotherapy-related oral and laryngeal mucositis and the weight loss was closely associated with the site of radiation,dysphagia,and dietary intake decline.Conclusions The dysphagia of patients with head and neck cancer is affected by radiation adverse effects,and is most severe at the end of radiotherapy.Dysphagia might result in dietary intake decline and weight loss.Clinicians should pay close attention to the swallowing function of the patients and take measures earlier accordingly.

ABSTRACT

Objective To investigate the iodine nutritional status among children aged 8-10 years old in Henan Province after five years of implementation of new standard iodized salt,and to provide evidence for scientific adjustment of control strategy.Methods In 2017,according to "Henan Surveillance Program on Iodine Deficiency Disorders",children aged 8-10 years old (half males and half females) were chosen as research targets.In nonhigh iodine areas of 156 non-high iodine counties (cities,districts) and high iodine counties (cities,districts) in Henan Province,household salt samples were collected;in 113 counties (cities,districts),urine samples were collected;in 67 counties (cities,districts),thyroid volume was measured.Salt iodine was tested by direction titration,the salt samples from Sichuan and other enhanced salt samples were tested by the arbitration method;urinary iodine was tested by arsenic cerium catalytic spectrophometry;thyroid volume was measured by ultrasound method.Results In 2017,totally 31 174 household salt samples of 8-10 years old children were collected and tested,the median of salt iodine was 25.8 mg/kg;the coverage rate of iodized salt was 95.0% (29 613/31 174) and the consumption rate of qualified iodized salt was 85.6% (26 673/31 174).Totally 22 442 urine samples were collected and the median of urinary iodine content was 208.0 μg/L.Totally 13 439 school children were examined thyroid volume and the goiter rate.was 1.5% (196/13 439).Conclusions After the promotion of new standard iodized salt,the iodine nutritional status of children aged 8-10 years old in Henan Province is appropriate.However,the consumption rate of qualified iodized salt is lower,so the universal salt iodization prevention and control strategies should be performed for a long time.At the same time,health education of iodine deficiency knowledge should be strengthened.

ABSTRACT

Objective:To explore the effects of atorvastatin on the natural antisense transcription of apolipoprotein A1 (apoA1-NAT) in the HepG2cells and its influence in the expressions of lipid metabolism related genes.Methods:The HepG2cells were intervened with different concentrations (0, 1, 10and 100nmol·L-1) of atorvastatin, and the 0nmol·L-1 atorvastatin group was used as control group.The total RNA of HepG2cells in various groups were extracted at different time points (6, 12, 24, and 48h) .The mRNA expression levels of lipid metabolism-related genes and apoA1-NAT expression levels were detected by Real-Time PCR method.Results:The morphology of HepG2 cells in 1and 10nmol·L-1 atorvastain groups was normal.Compared with 6h, the expression levels of apoA1-NAT in the HepG2cells in 10nmol·L-1 atorvastatin group at 12, 24and 48hwere significantly decreased (P<0.01) in a time-dependent manner.Under the same condition, the expression level of apoA1mRNA in the HepG2cells at 48hwas increased significantly compared with 6h (P<0.01) .Compared with control group, the expression levels of low density lipoprotein receptor (LDLR) in the HepG2 cells in100nmol·L-1 atorvastatin group, scavenger receptor-class B type 1 (SRB1) in 10nmol·L-1 atorvastatin group and ATP binding cassette transporter A1 (ABCA1) in 1and 10nmol·L-1 atorvastatin groups were increased (P<0.01) in different degrees.Conclusion:Atorvastatin can promote the expressions of lipid metabolism-related genes by inhibiting the expression of apoA1-NAT, and promote the process of reverse cholesterol transport.

ABSTRACT

Iodine nutrition surveillance of vulnerable population was conducted in 18 cities of Henan Province in 2015. The medians and quartiles of urinary iodine concentration in women of reproductive-age (n=3 318), pregnant women (n=36 366), lactating women (n=4 153), infants <2 years of age (n=1 017), and children aged 8-10 years (n=4 806) were 202.1(124.0, 310.0), 192.4(133.6, 262.4), 168.3(119.1, 248.3), 214.2(156.1, 272.3) and 90.0 (121.8, 285.6) μg/L, respectively. Iodine status of the vulnerable populations was generally regarded as adequate in Henan Province. But the median of urinary iodine concentration of reproductive-age women were slightly above the adequate level. The proportions above iodine adequate level were 26.7% in reproductive-age women, 29.4% in pregnant women and 22.5% in children aged 8-10 years.

ABSTRACT

Objective To investigate the differential diagnostic value of 18F-fluorodeoxyglucose (FDG) PET/CT for benign and malignant vertebral compression fractures in patients with malignant tumor.Methods From August 2007 to July 2016,79 patients with vertebral compression fractures were enrolled.Patients were divided into 3 groups based on clinical follow-up or pathological results:osteoporosis group (n =40;13 males,27 females,median age:77 years),metastasis group (n=27;18 males,9 females,median age:64 years) and myeloma group (n=12;7 males,5 females,median age:67.5 years).Characteristics of 18 F-FDG PET/CT imaging and maximum standardized uptake value (SUVmax) of involved vertebrae were compared using x2 test,one-way analysis of variance and the least significant difference t test.SUVmax of normal L2 vertebral body was regarded as the control.Results (1) There were 53,41 and 16 compression fractures identified in osteoporosis group,metastasis group and myeloma group,respectively.No paravertebral soft tissue mass or spinal accessory involvement was found in osteoporosis group.In metastasis group,14(34.15%,14/41) paravertebral soft tissue masses and 28 spinal accessory involvements (68.29%,28/41) were detected,and the numbers were 2 (2/16) and 16 (16/16) for myeloma group respectively.There were significant differences for paravertebral soft tissue masses and spinal accessory involvements among 3 groups (x2 values:21.75,73.10,both P<0.01).(2)In osteoporosis group,all lesions displayed strip-like (100%,53/53) 18F-FDG accumulation.In metastasis group,there were nodular accumulation (12.20%,5/41),bulk accumulation (43.90%,18/41),and irregular accumulation (43.90%,18/41).In myeloma group,the 18 F-FDG accumulation were strip-like (14/16) and irregular (2/16).The accumulation patterns among 3 groups were significantly different (x2=103.67,P<0.01).(3)SUVmax of osteoporosis group,metastasis group,myeloma group and control group was 4.00±0.14,7.33±4.05,4.17±0.39 and 2.33±0.06,respectively (F=46.45,P<0.01).The SUVmax between each 2 groups were significant (t values:12.38-29.51,all P<0.05) except for that between osteoporosis group and myeloma group (t=0.26,P>0.05).Conclusions Strip-like 18F-FDG accumulation,SUVmax of vertebral body,paravertebral soft tissue masses and spinal accessory involvements in 18F-FDG PET/CT imaging are important for differential diagnosis of benign and malignant compression fractures.

ABSTRACT

Objective To verify the influence of oncology nurses'empathy on the psychological dis-tress and benefit finding in breast cancer patients. Methods 30 oncology nurses were selected by random sampling and their empathy scores were measured with the interpersonal reactivity index (IRI-C).And they were divided into three groups based on empathy scores. 12 breast cancer patients from each primary nurse were selected by random sampling.Their psychological distress and benefit finding were measured by distress thermometer (DT) and benefit finding scale (BFS).Variance analysis was used to compare psychological distress and benefit finding among patients in different groups. The effect of nurse empathy on patients psy-chological distress and benefit finding was measured by Spearman correlation analysis and logistic regression. Results The empathy score of nurses was (44.87±17.23).The difference of empathy score in the three groups of nurses was statistically significant (F=55.941,P<0.01).There was no significant difference in psy-chological distress (low-score group:5.0(4.0,6.0),middle-score group:5.0(4.0,6.0),high-score group:5.0 (4.0,6.0)) and benefit finding (low-score group:47.5(42.0,52.8),middle-score group:48.0(43.0,54.0), high-score group:46.0(42.0,52.0)) in the three groups of patients at admission(P>0.05),while at dis-charge,the psychological distress of the patients in the high-score nurse group was lower than that of the low-score nurse group (P<0.01),and the benefit was higher than that of the low-score group (P<0.01).There was a negative correlation between the nurses'empathy abilities and patients'psychological distress ( r=-0.470, P<0.01) ,and a positive correlation between the nurses'empathy abilities with the patients'benefit finding ( r=0.521,P<0.01).When controlling for other variables,patients in nurse group with higher empathy scores would result in lower psychological distress(95%CI=0.042-0.397) and higher benefit finding (95%CI=0. 095-0.554) than patients in nurse group with lower empathy scores.Conclusion Oncology nurses'empathy effect breast cancer patients'psychological distress and benefit finding.Nurses should enhance their empathy further.

ABSTRACT

Objective To study the effects of reduced glutathione(GSH) in the treatment of acetamino-phen poisoning. Methods A total of 60 patients with acetaminophen poisoning treated in our hospital from June 2014 to May 2016 were selected.The patients were randomly divided into the control diagnosis and treatment group (n=30) and GSH diagnosis and treatment group(n=30).The patients of two groups were treated with anti-infec-tion,correction of water electrolyte disturbance and acid-base imbalance,and nutrition support treatment accord-ing to the poisoning routine.The control diagnosis and treatment group was treated with GSH,and the GSH diagno-sis and treatment group were treated with GSH according to the use assessment table.The liver function and the lev-el of cholinesterase(CHE) of the two groups before and after treatment were observed,and the adverse reactions and economic benefits of the two groups were statistically analyzed. Results The incidence of adverse drug reac-tions in the GSH diagnosis and treatment group was 10.00%,while that in the control diagnosis and treatment group was 23.33%,and there was a significant difference in the incidence of adverse drug reactions between the 2 groups(P<0.05).There was no significant difference in the levels of AST,ALB and CHE between the 2 groups before treatment(P>0.05).After treatment,the levels of AST and ALB of the GSH diagnosis and treatment group were significantly lower than those of the control diagnosis and treatment group(P < 0.05). The CHE level of the GSH diagnosis and treatment group was significantly higher than that of the control diagnosis and treatment group (P<0.05).The GSH dose,cost of treatment and days of hospitalization of the GSH diagnosis and treatment group were significantly lower than those of the control diagnosis and treatment group(P < 0.05). Conclusions The GSH for treatment of acetaminophen poisoning is effective,in terms of improving the level of liver function and re-ducing the level of inflammatory factors.Therefore,it is worthy of popularization and application.
