ABSTRACT

@#BACKGROUND: Computed tomography (CT) is a noninvasive imaging approach to assist the early diagnosis of pneumonia. However, coronavirus disease 2019 (COVID-19) shares similar imaging features with other types of pneumonia, which makes differential diagnosis problematic. Artificial intelligence (AI) has been proven successful in the medical imaging field, which has helped disease identification. However, whether AI can be used to identify the severity of COVID-19 is still underdetermined. METHODS: Data were extracted from 140 patients with confirmed COVID-19. The severity of COVID-19 patients (severe vs. non-severe) was defined at admission, according to American Thoracic Society (ATS) guidelines for community-acquired pneumonia (CAP). The AI-CT rating system constructed by Hangzhou YITU Healthcare Technology Co., Ltd. was used as the analysis tool to analyze chest CT images. RESULTS: A total of 117 diagnosed cases were enrolled, with 40 severe cases and 77 non-severe cases. Severe patients had more dyspnea symptoms on admission (12 vs. 3), higher acute physiology and chronic health evaluation (APACHE) II (9 vs. 4) and sequential organ failure assessment (SOFA) (3 vs. 1) scores, as well as higher CT semiquantitative rating scores (4 vs. 1) and AI-CT rating scores than non-severe patients (P<0.001). The AI-CT score was more predictive of the severity of COVID-19 (AUC=0.929), and ground-glass opacity (GGO) was more predictive of further intubation and mechanical ventilation (AUC=0.836). Furthermore, the CT semiquantitative score was linearly associated with the AICT rating system (Adj R2=75.5%, P<0.001). CONCLUSIONS: AI technology could be used to evaluate disease severity in COVID-19 patients. Although it could not be considered an independent factor, there was no doubt that GGOs displayed more predictive value for further mechanical ventilation.

ABSTRACT

OBJECTIVE To systema tically evaluate the effectiveness and safety of gen eric and original drugs of atorvastatin , and to provide the latest evidence-based reference for drug selection in clinic. METHODS Retrieved from PubMed ,Cochrane Library,Embase,CNKI,VIP and Wanfang database ,intervention trials and observational studies about generic and original drugs of atorvastatin were collected during the inception to Apr. 2021. After data extraction of literatures met inclusion criteria ,the Cochrane risk bias evaluation tool 5.1.0 was used to evaluate the quality of intervention trials ;Newcastle-Ottawa Scale (NOS)was used to evaluate the quality of observational studies. RevMan 5.4 software was used to conduct meta-analysis ,and descrptive analysis was performed at the same time. RESULTS A total of 24 studies were included ,involving 21 randomized controlled trials (RCTs)and 3 retrospective cohort studies (RCSs),with 20 001 patients involved. Meta-analysis results of RCT showed there was no statistically significant difference between the two groups in reducing low-density lipoprotein cholesterol (LDL-C)levels [MD = - 0.05,95% CI(- 0.12,0.02),P=0.16] and increasing Δ 基金项目:国家重点研发计划项目(No.2017YFC0910004);山 东省重点研发计划项目(No.2020RKB14165) high-density lipoprotein cholesterol (HDL-C)levels [MD = *硕士研究生 。研究方向:临床药学。E-mail:1677032023@qq. - 0.00,95% CI(- 0.02,0.01),P=0.52];the degree of com reducing total cholesterol (TC)level [MD =-0.11,95%CI # 通信作者:主任药师,硕士生导师。研究方向:临床药学、药事 ( - 0.17,- 0.06),P<0.000 1] and triglyceride (TG) 管理。电话:0351-89268349。E-mail:13791120711@126.com level [MD =-0.05,95%CI(-0.09, -0.01),P=0.02] in ·358· China Pharmacy 2022Vol. 33 No. 3 中国药房 2022年第33卷第3期 generic drug group was lower than orig inal drug group ,with statistical significance difference. There was no statistical significance difference in total incidence of adverse drug reaction (ADR)[OR=1.08,95% CI(0.85,1.37),P=0.55] and the incidence of other ADR(P>0.05). The results of subgroup analysis showed that the reductions of TC and TG of generic drugs produced by Beijing Jialin Pharmaceutical Enterprise (hereinafter refer to Jialin generic drugs )were less than those of the original drug ,and the difference was statistically significant ;compared with original drugs ,there was no significant difference in other indexes or all indexes of the generic drugs from other manufacturers. Compared with original drugs ,the reductions of TC and TG in 20 mg/d group of Jialin generic drugs were less than original drug group ;the degree of TC reduction at 12 and 24 weeks of follow-up and TG reduction at 24 weeks of follow-up were less than those of the original drugs ,the difference was statistically significant ;there was no significant difference in other indexes. The qualitative description of RCS showed that for elderly patients with death/acute coronary syndrome ,there was no statistical difference between the two groups in terms of cardiovascular events or serious side effects. For the adult patients who switched from original drugs to generic drugs ,the effect of generic drugs instead of original drugs would not be reduced ,but the increase of HDL-C was less than that of original drug. CONCLUSIONS In terms of effectiveness, generic drugs of atorvastatin can replace original drugs and caution should be taken on the levels of HDL-C ,TC and TG for long time use ;in terms of safety ,generic drugs are similar to the original drugs.

ABSTRACT

Activation of the heart normally begins in the sinoatrial node (SAN). Electrical impulses spontaneously released by SAN pacemaker cells (SANPCs) trigger the contraction of the heart. However, the cellular nature of SANPCs remains controversial. Here, we report that SANPCs exhibit glutamatergic neuron-like properties. By comparing the single-cell transcriptome of SANPCs with that of cells from primary visual cortex in mouse, we found that SANPCs co-clustered with cortical neurons. Tissue and cellular imaging confirmed that SANPCs contained key elements of glutamatergic neurotransmitter system, expressing genes encoding glutamate synthesis pathway (Gls), ionotropic and metabotropic glutamate receptors (Grina, Gria3, Grm1 and Grm5), and glutamate transporters (Slc17a7). SANPCs highly expressed cell markers of glutamatergic neurons (Snap25 and Slc17a7), whereas Gad1, a marker of GABAergic neurons, was negative. Functional studies revealed that inhibition of glutamate receptors or transporters reduced spontaneous pacing frequency of isolated SAN tissues and spontaneous Ca

ABSTRACT

Drug combination can effectively enhance the anti-tumor effect, reduce the drug dose, and improve medication safety. The use of nano-carrier for drug co-delivery can effectively avoid the differences in drug delivery behavior in vivo. Triptolide and celastrol are the main anti-tumor active components of Tripterygium wilfordii Hook f. Modern studies have shown that the combination of triptolide and celastrol can significantly enhance the antitumor effect, but they are limited by poor water solubility and low tumor tissue delivery rate. In this study, a biomimetic erythrocyte membrane liposome co-loaded with triptolide and celastrol was prepared to characterize the morphology, particle size, potential, drug release, serum stability, and other properties. The immunogenicity, uptake behavior, and anti-cell proliferation ability of the biomimetic liposome was compared. All the animal experiments were carried out in accordance with protocol evaluated and approved by the Ethics Committee of Chengdu University of Traditional Chinese Medicine (Chengdu, China). The results showed that the biomimetic erythrocyte membrane liposome co-loaded with triptolide and celastrol (C+T/RBCm@Lip) in this study had an average particle size of 119.12 ± 2.78 nm and a spherical "core-shell" structure. The zeta potential value was -16.9 ± 1.2 mV, and the drug release behavior in vitro was slow. In addition, the process of coating the cell membrane maintained the characteristics of erythrocyte membrane protein, had good stability in serum, and could effectively avoid the recognition and clearance of macrophages, without causing immunogenicity in vivo. The uptake effect of co-loaded biomimetic liposomes on HepG2 hepatocellular carcinoma cells was enhanced compared with that of uncoated cell membrane liposomes, and the inhibitory effect on proliferation of HepG2 cells was enhanced. In conclusion, the biomimetic liposomes coated with erythrocyte membrane prepared in this study is beneficial to the anti-tumor delivery of triptolide and celastrol, and could enhance the inhibitory effect on the growth of HepG2 liver cancer cells, providing a new idea for the anti-tumor application of Tripterygium wilfordii Hook f.

ABSTRACT

Objective:Considering the efficacy of Gegen Qinliantang (GQT) in releasing exterior and clearing interior to alleviate dampness-heat dysentery, we analyzed the mechanism of the chloroform extract of GQT in alleviating enterotoxicity caused by irinotecan to provide an experimental basis for the development of GQT. Method:Kunming mice (n=60) were randomly divided into a blank group, a model group, a loperamide group (positive drug of loperamide hydrochloride capsule, 0.4 mg·kg-1), and high- (2.3 g·kg-1) and low-dose (1.16 g·kg-1) GQT chloroform extract groups. The mouse model of delayed diarrhea was established by intraperitoneal injection of irinotecan hydrochloride (CPT-11, 55 mg·kg-1) for four consecutive days, meanwhile, the mice in the blank group only received the same volume of normal saline. Corresponding drugs were administered by gavage on the fifth day, respectively, while the ones in the blank group and model group were given distilled water for five consecutive days. The general condition of mice in each group was observed, and diarrhea indexes of mice were recorded. Pathological changes in colon tissues of mice were observed by hematoxylin-eosin (HE) staining. The tumor necrosis factor (TNF)-α, interleukin (IL)-1β, cyclooxygenase (COX)-2, intercellular adhesion molecule (ICAM)-1, glutathione peroxidase (GSH-Px), superoxide dismutase (SOD), malondialdehyde (MDA) and nitric oxide (NO) levels in colon tissues were detected with the assay kits. Furthermore, the expression levels of Kelch sample epoxy chloropropane associated protein 1 (Keap1), nuclear factor E2 related factor 2 (Nrf2), tight junction protein-1 (ZO-1), heme oxygenase-1 (HO-1) and tight junction protein (Occludin) were detected by Western blot. Result:Compared with the blank group, the model group showed declined body weight and reduced contents of GSH-Px and SOD (P<0.01), whereas increased diarrhea indexes and TNF-α, IL-1β, COX-2, ICAM-1, MDA and NO levels (P<0.01). Abundant inflammatory cells and colonic mucosa with defects, swelling, bleeding, and inflammatory exudation were revealed by HE staining in the mice of the model group. The expression levels of Keap1, Nrf2, ZO-1, HO-1 and Occludin in colon tissues significantly declined (P<0.01). Compared with the model group, the loperamide group and the high- and low-dose GQT chloroform extract groups exhibited improved weight loss, reduced diarrhea indexes, diminished TNF-α, IL-1β, COX-2, ICAM-1, MDA and NO, and elevated GSH-Px and SOD. HE staining indicated that the cells were compactly arranged with clear nuclei in the high- and low-dose GQT chloroform extract groups, and the expression levels of Keap1, Nrf2, HO-1, Occludin, and ZO-1 were up-regulated. Conclusion:GQT chloroform extract may alleviate CPT-11-induced delayed diarrhea by regulating inflammation and oxidative stress for enhancing the intestinal barrier function. These findings are expected to provide a reference for exploring the toxicity-attenuating effect of Chinese medicinals on chemotherapy drugs and for developing famous classical formulas.

ABSTRACT

Objective:To evaluate the accessibility construction status of road traffic sidewalks in Shanghai. Methods:From July to September, 2019, 46 road traffic sidewalks in Shanghai were selected using a convenience sampling, and evaluation tools were designed based on the Codes for Accessibility Design (GB 50763-2012) and Standards for Design on Accessibility of Buildings and Facilities (DGJ08-103-2003). After field check and experience for the sidewalks, the compliance rates of evaluation dimensions and evaluation contents of sidewalks were analyzed using the descriptive analysis. The weights of each dimension were calculated using the principal component analysis to construct the sidewalk comprehensive barrier score model, and to compare the sidewalk comprehensive barrier scores in different areas of Shanghai. From September to November 2019, in-depth interviews were conducted with eleven persons with disabilities to understand the sidewalk barriers for them. Results:The overall compliance rate of sidewalk accessibility was 4.35%. Among the evaluated contents, 93.5% sidewalks had a curb ramp, 91.3% had a height difference between the ramp and the carriageway, 26.1% were not paved with tactile ground surface indicators; 71.7% tactile ground surface indicators were blocked by poles, etc., and 84.8% were occupied by obstacles. The warning indicator was the greatest weight among the evaluated dimensions. The barrier of sidewalks was high (36.7%) and medium (20.0%) in the central area, which was higher than that in the sub-central area (both 6.3%) (P < 0.05). There were many obstacles during the experience of sidewalks using, resulting in less using of the barrier-free facilities. Conclusion:Most of the sidewalks in Shanghai do not meet the standards of the Codes for Accessibility Design. It is urgent to construct and maintain the barrier-free facilities.

ABSTRACT

Purpose@#In this pilot study, using next-generation sequencing and integrated messenger RNA (mRNA) sequencing, we investigated circulating microRNA (miRNA) expression profiling from bile-derived exosomes to identify dysregulated miRNA signatures and oncogenic pathways and determine their effects on targeted mRNAs in cholangiocarcinoma (CCA).Moreover, we explored the possibility that genetic analysis using bile-derived exosomes may replace gene analysis using tissue. @*Methods@#Bile was collected from a patient with perihilar CCA before curative resection. As a control, bile was collected from a patient with a common bile duct stone. Exosomes were isolated from the bile, and we performed next-generation miRNA sequencing using isolated exosomes. To evaluate miRNA-mRNA interactions, mRNA sequencing was performed using bile fluid in both patients. @*Results@#We identified 22 differentially expressed miRNAs. More than 65% of the predicted mRNA targets of those miRNAs were actually differentially expressed between control and CCA bile samples. In functional pathway analysis, targets of 22 miRNAs were primarily enriched in mitogen-activated protein kinase, platelet derived growth factor, vascular endothelial growth factor, epidermal growth factor receptor, and p53 signaling. In particular, in the functional assessment of miRNAmRNA interactions, RAS pathways, including downstream pathways (PI3K-AKT-mTOR and RAS-RAF-MEK-ERK), were determined to be enriched. @*Conclusion@#Circulating miRNAs in bile-derived exosomes provide new information for the development of miRNA analysis in CCA. These miRNAs may represent the oncogenic characteristics of CCA tissue, enabling them to be used instead of tissue samples for the diagnosis of CCA. Further research investigating circulating miRNAs in bile exosomes may lead to more rational, targeted approaches to treatment.

ABSTRACT

Radiotherapy is still a major part of the program about treating various head and neck cancers. While improving the survival rate of head and neck cancer patients, radiotherapy can cause the decline of learning, memory, orientation and other functions, seriously, even lead to dementia, namely radiation induced cognitive dysfunction (RICD), which has become a critical factor affecting the quality of life for patients. Because of occult onset and unclear pathogenesis, so far there is few effective preventive and therapeutic tools for RICD. In this paper, the latest research progress on the pathogenesis and treatments of RICD is summarized.

ABSTRACT

@#BACKGROUND: Acute respiratory distress syndrome (ARDS) causes substantial mortalities. Alveolar epithelium is one of the main sites of cell injuries in ARDS. As an important kind of microRNAs (miRNAs), microRNA-145 (miR-145) has been studied in various diseases, while its role in ARDS has not been investigated. METHODS: Lipopolysaccharide (LPS) was intratracheally instilled to establish a rat ARDS model. Cytokines from bronchoalveolar lavage fluid (BALF) were measured using rat tumor necrosis factor-α and interleukin-6 enzyme-linked immunosorbent assay kits (R&D Systems), and the pathological structures were evaluated using hematoxylin and eosin (H&E) staining and transmission electron microscope; the lung miR-145 messenger RNA (mRNA) was detected using quantitative polymerase chain reaction. Bioinformatics focused on the target genes and possible pathways of gene regulation. RESULTS: A rat model of LPS-induced ARDS was successfully established. The miR-145 was down-regulated in the LPS-induced ARDS lung, and mitochondrial dysfunction was observed in alveolar epithelial cells, most obviously at 72 hours after LPS. TargetScan and miRDB databases were used to predict the target genes of miR-145. A total of 428 overlapping genes were identified, seven genes were associated with mitochondrial function, and Ogt, Camk2d, Slc8a3, and Slc25a25 were verified. Kyoto Encyclopedia of Genes and Genomes (KEGG) pathways were enriched in the mitogen-activated protein kinase (MAPK) signaling pathway, and Gene Ontology (GO) biological process was mainly enriched in signal transduction and transcription regulation. CONCLUSIONS: The miR-145 is down-regulated in LPS-induced ARDS, and affects its downstream genes targeting mitochondrial functions.

ABSTRACT

Overtaking lung cancer, breast cancer is now the most commonly diagnosed cancer seriously threatening people's health and life. As the main effective component of Tripterygium wilfordii, triptolide( TP) has attracted increasing attention due to its multitarget and multi-pathway anti-tumor activity. Recent studies have revealed that breast cancer-sensitive TP enables the inactivation of breast cancer cells by inducing tumor cell apoptosis and autophagy, interfering in tumor cell metastasis, resisting drug resistance, arresting tumor cell cycle, and influencing tumor microenvironment. It has been recognized as a promising clinical antitumor agent by virtue of its widely accepted therapeutic efficacy. This paper reviewed the anti-breast cancer action and its molecular mechanisms of TP on the basis of the relevant literature in the past ten years, and proposed application strategies in view of the inadequacy of TP to provide a reference for further research on the application of TP in the treatment of breast cancer.

ABSTRACT

Objective:To develop an early warning index system to identify the outbreak of respiratory infectious diseases and to assess the epidemic risk of these diseases in Shanghai. Methods:A two-round Delphi survey with a panel of 31 experts was used to select the modifying indicators. All indicators were evaluated for necessity, feasibility, stability, and sensitivity through online questionnaires. Results:The consultation recovery rates of both rounds were 100%. The authority coefficient was 0.88, and the Kendall's W of the second-round consultation was 0.138 (P<0.001), suggesting a good coordination among experts’ opinion. Our early warning index system was developed after the two-round Delphi survey and included two sets of indicators, one for rapid risk assessment and another for monitoring and tracing risk. Both sets of the indicators involved three aspects: transmissibility, clinical severity and potential influence of emerging diseases. The rapid risk assessment system part covered 10 indicators while the risk monitor system part covered 23 indicators. In the rapid risk assessment system part, the weight value of the top three indicators were greater than 0.10, with the highest weight value of 0.171 for titled visiting rate of influenza-like illness patients in the Emergency Department or other outpatient visits. In the risk monitor system part, the weight value of the top eight indicators were greater than 0.05, and the greatest was 0.087 for R0 (basic reproductive number). Conclusion:The developed scientific and reliable evaluation indicator system can be used to forecast the outbreak and epidemic risk of respiratory infectious diseases. However, sensitivity and stability of the indicators need further validation and evaluation.
