ABSTRACT

Objective:To investigate the therapeutic effect and possible mechanism of low intensity pulsed ultrasound(LIPUS)on painful gait in mice with Achilles tendon injury.Methods:Male C57BL/6 mice were selected to establish the Achilles tendon injury model by surgically full-thinkness tear of the right Achilles tendon. The mice were divided into Achilles tendon injury group and Achilles tendon injury+LIPUS group according to the random number table method,with 7 mice per group. The Catwalk gait analysis system was used to evaluate the gait function of the mice by measuring the following five parameters 14 days after operation,including print area,standing time,step cycle,max intensity and stride length. Morphological changes of the Achilles tendon were observed by HE staining. Immunofluorescence staining was used to detect the expression level of nitric oxide synthase(iNOS)in the Achilles tendon tissues. At the same time,Achilles tendon cells were isolated and cultured in vitro. The cells were induced by 100 ng/ml lipopolysaccharide(LPS)for 12 hours to establish in vitro model(LPS group),and treated with LIPUS(LPS+LIPUS group). The control group was set as well(without any treatment). The nuclear translocation of nuclear transcription factor-κB(NF-κB)P65 was detected by immunofluorescence staining,and the expression of iNOS and phosphorylation(p)-NF-κB p65 protein was detected by Western blotting. Results:Compared with Achilles tendon injury group[(0.14±0.10)cm 2],the print area of the affected limb in Achilles tendon injury+LIPUS group[(0.28±0.13)cm 2]was increased( P<0.05). Compared with Achilles tendon injury group[(0.11±0.04)seconds],the standing time of the affected limb in Achilles tendon injury+LIPUS group[(0.21±0.03)seconds]was increased( P<0.05). Compared with Achilles tendon injury group[(0.25±0.05)seconds],the step cycle of the affected limb in Achilles tendon injury+LIPUS group[(0.40±0.05)seconds]was increased( P<0.05). There was no significant difference in the max intensity and stride length between Achilles tendon injury group and Achilles tendon injury+LIPUS group( P>0.05). HE staining showed obvious hyperplasia of Achilles tendon tissues in Achilles tendon injury group,with loosely and randomly arranged fibers,accompanied by neovascularization and inflammatory cell infiltration. Achilles tendon injury+LIPUS group showed more orderly arranged fibers in Achilles tendon tissues,and the degree of neovascularization and inflammatory cell infiltration were reduced. Compared with Achilles tendon injury group[(5.70±0.81)%],the expression level of iNOS in Achilles tendon injury+LIPUS group[(2.84±0.94)%]was decreased 14 days after operation( P<0.05). Immunofluorescence results of Achilles tendon cells showed that NF-κB p65 protein entered the nucleus in LPS group compared with control group,and that LIPUS treatment inhibited nuclear translocation. In control group,Western blotting showed that iNOS was not expressed,and that the expression of phosphorylated NF-κB p65 was 0.63±0.16. Compared with control group,the expression levels of iNOS(0.99±0.22)and p-NF-κB P65(1.02±0.19)in LPS group were significantly increased( P<0.05). Compared with LPS group,the expression levels of iNOS(0.62±0.10)and p-NF-κB P65(0.65±0.21)in LPS+LIPUS group were decreased( P<0.05). Conclusion:LIPUS treatment can alleviate pain gait in mice with Achilles tendon injury and inhibit iNOS expression in vivo and in vitro,which may be related to inhibition of NF-κB signaling pathway.

ABSTRACT

Objective:To investigate the efficacy of osettinib in the treatment of first-line tyrosine kinase inhibitor-resistant advanced lung adenocarcinoma and its effects on carcinoembryonic antigen (CEA) and vascular endothelial growth factor (VEGF) levels.Methods:Seventy-two patients with first-line tyrosine kinase inhibitor-resistant advanced lung adenocarcinoma (T790M test negative or rejected) who received treatment in Jinhua Guangfu Cancer Hospital from June 2017 to June 2019 were included in this study. They were randomly assigned to undergo either conventional pemetrexed plus cisplatin (control group, n = 36) or osimertinib mesylate (observation group, n = 36) for 4 successive weeks. Before and after 4 weeks of treatment, serum CEA and VEGF levels were measured. Curative effects were evaluated. Adverse reactions and 6-month, 1-year and 2-year survival rate were recorded. Results:Effective rate and disease-control rate in the observation group were 80.6% (29/36) and 94.4% (34/36) respectively, which were significantly higher than 58.3% (21/36) and 75.0% (27/ 36) in the control group ( χ2 = 4.193, 5.261, both P < 0.05). Before treatment, there were no significant differences in serum CEA and VEGF levels between the two groups (both P > 0.05). Compared with before treatment, serum CEA and VEGF levels were significantly increased after treatment (both P < 0.05). After treatment, serum CEA and VEGF levels in the observation group were (5.36 ± 0.33) U/mL and (121.56 ± 11.57) ng/L respectively, which were significantly lower than those in the control group [(8.25 ± 0.54) U/mL, (163.68 ± 14.59) ng/L, t = 27.399, 13.572, both P < 0.001]. The incidence of adverse reactions in the observation group was significantly lower than that in the control group [19.4% (7/36) vs. 44.4% (16/36), χ2 = 5.173, P = 0.011]. 6-month, 1-year and 2-year survival rate were 94.29%, 77.14% and 60.00% respectively, in the observation group and 91.43%, 54.29% and 34.29% respectively in the control group. There was no significant difference in 6-month overall survival rate between the two groups ( χ2 = 0.352, P = 0.251). 1-year and 2-year survival rate in the observation group were significantly higher than those in the control group ( χ2 = 4.058, P = 0.044; χ2 = 4.644, P = 0.031). Conclusion:Ositinib is effective in the targeted treatment of first-line tyrosine kinase inhibitor-resistant advanced lung adenocarcinoma. It can effectively decrease serum CEA and VEGF levels and prolong the survival of patients, thereby exhibiting a clinical application value.

ABSTRACT

Objective:To investigate the effect of Guizhi Fulingwan on ovulation dysfunction in rats with polycystic ovary syndrome with insulin resistance (PCOS-IR) induced by letrazole combined with high fat emulsion. Method:A total of 72 female SD rats were randomly divided into control group, model group, metformin group and Guizhi Fulingwan low, medium and high dose groups, with 12 rats in each group. Except for control group, rats were given letrozole 0.001g·kg<sup>-1</sup> combined with high-fat emulsion 15 mL·kg<sup>-1</sup> for 21 consecutive days to establish model of PCOS-IR. Guizhi Fulingwan low, medium and high-dose groups were administrated with Guizhi Fulingwan 0.31, 0.62, 1.24 g·kg<sup>-1</sup> respectively, metformin group was administrated with metformin 0.27 g·kg<sup>-1</sup>, control group and model group were administrated with 12 mL·kg<sup>-1</sup> of normal saline daily for 30 days. Hematoxylin-eosin(HE) staining was used to observe ovarian tissue pathology morphology, and enzyme-linked immunoassay method (ELISA) was used to detect serum follicle stimulating hormone (FSH), luteinizing hormone (LH), testosterone (T), fasting insulin (FINS) level,and LH/FSH and insulin resistance index (HOMA-IR) were calculated. Western blot was used to detect the expression levels of autophagy key molecular Atg6 yeast homologue (Beclin-1), autophagy related gene 5(Atg5), microtubule associated protein light chain 3 (LC3) Ⅱ proteins in the phosphatidylinositol 3-kinase/protein kinase B/rapamycin target protein (PI3K/Akt/mTOR) signaling pathway and autophagy related indicators in rat ovarian tissue. Beclin-1 and LC3Ⅱ protein expressions were detected by immunohistochemistry (IHC). Result:Compared with control group, the thickness of follicles and follicular granulosa cells in the ovary of the model group also decreased, and the number of corpus luteum significantly decreased, while the white membrane thickness of the ovary increased, and the number of atresia follicles and cystic dilatation follicles increased significantly. Serum T, LH, LH/FSH, FINS, FINS, HOMA-IR were significantly increased (<italic>P</italic><0.01). Phosphorylated (p) -PI3K, p-Akt, and p-mTOR proteins in ovarian tissue were all decreased (<italic>P</italic><0.05,<italic>P</italic><0.01). The relative expression levels of autophagy-related protein LC3Ⅱ and Beclin-1 were significantly increased (<italic>P</italic><0.05,<italic>P</italic><0.01). Compared with model group, the number of follicles in the low, medium and high dose Guizhi Fulingwan group and the metformin group decreased, the number of follicles in atresia and atresia increased, and the follicular granulosa cell layer thickness increased. Serum T, LH, LH/FSH, FINS and HOMA-IR of Guizhi Fulingwan group were significantly decreased (<italic>P</italic><0.05,<italic>P</italic><0.01), and serum FINS and HOMA-IR of metformin group were significantly decreased (<italic>P</italic><0.01). The expressions of p-PI3K, p-Akt, and p-mTOR proteins were increased (<italic>P</italic><0.05,<italic>P</italic><0.01). The expression levels of LC3Ⅱ, Atg5 and Beclin-1 in the medium and high dose groups were significantly decreased (<italic>P</italic><0.01). Conclusion:Guizhi Fulingwan can activate the PI3K/Akt/mTOR signaling pathway of granular cells, inhibit excessive autophagy of granular cells, improve ovarian function and insulin resistance, and restore ovulation, and the effect is better with high dose.

ABSTRACT

Gout is the second largest metabolic disease in China, which can cause joint tissue damage and a variety of chronic diseases, and seriously affect human life and health. The increase in uric acid caused by disorder of purine metabolism or abnormal uric acid excretion is the biochemical basis of its pathogenesis. Western medical treatment mainly uses anti-inflammatory drugs such as colchicine, non-steroidal anti-inflammatory drugs, and uric acid lowering drugs such as febuxostat and benzbromarone, which have obvious effects, but there are problems such as easy to recurrence after drug withdrawal and more adverse reactions. Traditional Chinese medicine (TCM) has a long history in the treatment of gout, and has the advantages of multi-channel, multi-target, and multi-level symptomatic treatment. It exerts therapeutic effects through lowering uric acid, anti-inflammatory, anti-oxidation, and protecting the kidneys. Its curative effect is obvious and the adverse reaction rate is low. In recent years, there have been many studies on the mechanism of TCM for gout animal models. Based on the review of relevant literature in recent years, this article has systematically sorted out the pathogenesis of gout, the mechanism of TCM for gout and related experimental design. The paper summarized and analyzed the mechanism of TCM in the treatment of gout from the aspects of regulating the level of inflammatory factors, inhibiting oxidation reaction, reducing uric acid and regulating signaling pathway, so as to provide reference for the research and development of drugs for gout.

ABSTRACT

Objective:To investigate the effect of Guizhi Fulingwan on autophagy of ovarian granulosa cells in mice with polycystic ovary syndrome (PCOS). Method:Twenty SD mice were randomized into a normal group (<italic>n</italic>=10) and a PCOS model group (<italic>n</italic>=10), followed by PCOS modeling and <italic>in vitro</italic> culture of extracted ovarian granulosa cells. The ovarian granulosa cells of normal mice were classified into the control group and treated with 10% blank serum while those of PCOS mice into the experimental groups and with 10% Guizhi Fulingwan-containing serum at different concentrations (17.6, 35.1, 70.2 mg·kg<sup>-1</sup>) and 10% metformin-containing serum (25 mg·kg<sup>-1</sup>), respectively, for 72 h. During the modeling, the changes in mouse body weight were measured. After modeling, the ovarian morphology was observed by microscopy, and the fasting blood glucose (FBG) was measured by Roche glucometer. Following the detection of fasting insulin (FI) and testosterone (T) levels by radioimmunoassay, the proliferation of ovarian granulosa cells was determined using cell counting kit-8 (CCK-8) to figure out the maximal dose of drug-containing serum that did not obviously affect the cell viability for subsequent assay. The autophagy of ovarian granulosa cells was examined by flow cytometry, and the protein expression levels of intracellular microtubule-associated protein 1 light chain 3Ⅰ (LC3Ⅰ), LC3Ⅱ, Beclin1, and p62 were assayed by Western blott. Result:Compared with the blank group, the model group showed increased body weight and elevated FI, FBG, and T levels (<italic>P</italic><0.05,<italic>P</italic><0.01), indicating the successful modeling of PCOS mice. Flow cytometric assay proved that the incubation with 10% Guizhi Fulingwan serum-containing medium resulted in a decline of autophagy (<italic>P</italic><0.05). As demonstrated by Western blot assay results, the protein expression levels of Beclin1 and LC3 Ⅱ/Ⅰ in the model group increased significantly as compared with those of the blank group, whereas the expression level of p62 decreased significantly (<italic>P</italic><0.05,<italic>P</italic><0.01). Compared with the model group, the medium- and high-dose Guizhi Fulingwan groups exhibited significantly down-regulated Beclin1 and LC3 Ⅱ/Ⅰ levels but remarkably up-regulated p62 (<italic>P</italic><0.05,<italic>P</italic><0.01). Conclusion:Guizhi Fulingwan inhibits the autophagy of ovarian granulosa cells by down-regulating the protein expression levels of Beclin1 and LC3 Ⅱ/Ⅰ.

ABSTRACT

Objective:To observe the effect of tetrahydroxy stilbene glycoside (TSG) on the expression of glycogen synthase kinase 3<italic>β </italic>(GSK3<italic>β</italic>), cyclic adenosine monophosphate-dependent protein kinase (PKA) and Serine/threonine phosphatase 2A(PP2A) in the brain of amyloid precursor protein/presenilin-1/Tau (APP/PS1/Tau) triple-transgenic mice dementia model. Method:A total of forty-five 8-month-old APP/PS1/Tau transgenic mice were randomly divided into model group, positive control group (Huperzine-A, 0.15 mg·kg<sup>-1</sup>), low, medium and high dose TSG groups (TSG, 0.033,0.1,0.3 g·kg<sup>-1</sup>), with 9 mice in each group, and another nine C5B7L/6J mice of the same age were selected as normal control group. After 60 days of intragastric administration, the general structure of hippocampal neurons was observed by hematoxylin-eosin (HE) staining, immunohistochemical (IHC) was used to detect the expression of PKA protein in the brain of mice in each group, the mRNA expression levels of GSK3<italic>β</italic>, PKA and PP2A were detected by real time quantitative reverse transcription polymerase chain reaction (Real-time PCR), and protein expression levels of GSK3<italic>β</italic> and PP2A were detected by Western blot. Result:Compared with the normal control group, the apoptosis level of neurons in the model group was significantly increased, the protein and mRNA expression levels of GSK3<italic>β</italic> and PKA were significantly increased (<italic>P</italic><0.05, <italic>P</italic><0.01), and the protein and mRNA expression levels of PP2A were significantly decreased (<italic>P</italic><0.05, <italic>P</italic><0.01). Compared with the model group, the apoptosis level of neurons in each treatment group was significantly down-regulated, the protein and mRNA expression levels of GSK3<italic>β</italic> and PKA were significantly down-regulated (<italic>P</italic><0.05, <italic>P</italic><0.01), and the protein and mRNA expression levels of PP2A were significantly increased (<italic>P</italic><0.05, <italic>P</italic><0.01). Conclusion:The mechanism of TSG in the treatment of Alzheimer's disease (AD) may be related to lowering the transcription and expression of GSK3<italic>β</italic> and PKA, increasing the transcription and expression of PP2A.

@#Objective    To investige the effects of multidisciplinary perioperative nutrition management on nutrition and postoperative complications of patients with esophageal cancer. Methods    A total of 239 patients with esophageal cancer who received elective surgical treatment were included in the study. They were divided into a trial group (120 patients) and a control group (119 patients) according to the random number table method. There were 97 males and 23 females in the trial group with an average age of 63.78±9.13 years, and 94 males and 25 females in the control group with an average age of 64.12±7.91 years. The control group received routine diet management, and the trial group received multidisciplinary perioperative nutrition management. The differences of nutrition and postoperative complications between the two groups were compared. Results    The total protein and albumin levels on postoperative days 3 and 7 in the trial group were higher than those in the control group (P<0.05), patients' postoperative anal exhaust time was shorter than that in the control group (P<0.05), the incidence of postoperative gastrointestinal adverse reactions, lung infection, postoperative anastomotic fistula, hypoproteinemia on postoperative days 3 and 7 was lower than that in the control group (P<0.05), and hospitalization cost was lower than that in the control group (P<0.05). Conclusion    Multidisciplinary nutrition management can effectively improve the nutrition of patients, promote the rapid recovery of postoperative gastrointestinal function, reduce postoperative complications, and reduce hospitalization costs. It has high clinical reference and promotion value.

ABSTRACT

Objective Focused cardiac ultrasound (FCU) and lung ultrasound (LU) are increasingly being used in critically ill patients. This study aimed to investigate the effect of FCU in combination with LU on these patients and to determine if the timing of ultrasound examination was associated with treatment change. Methods This is a multicenter cross-sectional observational study. Consecutive patients admitted to the intensive care unit (ICU) were screened for enrollment. FCU and LU were performed within the first 24 h, and treatment change was proposed by the performer based on the ultrasound results and other clinical conditions. Results Among the 992 patients included, 502 were examined within 6 h of ICU admission (early phase group), and 490 were examined after 6 h of admission (later phase group). The early phase group and the later phase group had similar proportions of treatment change (48.8%

ABSTRACT

Objective To evaluate the effects of antiretroviral therapy(ART)for the prevention of mother-to-child transmission(PMTCT)of acquired immune deficiency syndrome(AIDS)on the growth and development of 18-month-old children born by human immunodeficiency virus(HIV)-positive pregnant women in Lingshan County,Guangxi Zhuang Autonomous Region,and provide scientific evidence for improving the ART medication plan for PMTCT.Methods Lingshan County,ranking the first in the HIV-epidemic counties of Guangxi,was selected as the research site.According to the design of retrospective case-control study,we assigned all the subjects into the case group and the control group:(1)The case group included the HIV-positive pregnant women who had received ART for PMTCT and their HIV-negative infants in Lingshan County from 2010 to 2017.The historical cards and PMTCT data of them were collected from the national PMTCT database.(2)The control group included the healthy pregnant women and their healthy babies born in the Lingshan Maternity and Infant Hospital in 2017,and the children's growth and development data were collected.The stunted growth in children was defined as at least one of the three main indicators of body height,body weight,and head circumference below the normal range.Results The number of HIV-positive mothers and their infants in the case group was 391 and 368,respectively,and 87.21%(341/391)and 95.38%(351/368)of mothers and infants respectively received ART medication.The HIV positive rate,mortality rate,and mother-to-child transmission rate of 18-month-old children were 1.36%(5/368),4.35%(16/368),and 2.01%(5/249),respectively.The incidence of stunted growth of 18-month-old children in the case group and the control group was 42.12%(155/368)and 23.06%(101/438),respectively,with significant difference(

ABSTRACT

Objective To evaluate the effectiveness of the integrated schistosomiasis control measures in Changzhou City from 2015 to 2020, so as to provide insights into the formulation of the strategy used to consolidate the schistosomiasis elimination achievements. Methods The annual schistosomiasis control working report and integrated schistosomiasis control data were collected in Changzhou City from 2015 to 2020, and the prevalence of Schistosoma japonicum infections in humans and livestock and snail status were analyzed to evaluate the effectiveness of the integrated schistosomiasis control measures. Results During the period from 2015 to 2020, a total of 112 061 person-time individuals received serological tests for S. japonicum infections in Changzhou City, and the sero-prevalence was 0.15% to 1.09% during the 6-year period, with a significant difference seen among years (χ2 = 288.11, P < 0.05). From 2015 to 2020, a total of 13 435 person-time individuals received stool examinations, with no egg-positives identified; among 5 840 herd-time livestock receiving schistosomiasis examinations, no positives were detected, while a 100% coverage of fencing livestock was seen each year. During the 6-year period, a total of 38.40 hm2 snail habitats were found, including 8.97 hm2 emerging snail habitats, and among the 2 344 snails dissected, no S. japonicum infection was found. Chemical treatment covered an area of 385.71 hm2, and environmental improvements covered an area of 200.39 hm2. The mean density of living snails was less than 0.1 snails/0.1 m2 in snail habitats found in Changzhou City each year from 2015 to 2020, and the coverage of harmless toilets was 100% in 2020. During the 6-year period, a total of 3.740 6 million person-time individuals were given schistosomiasis health education in Changzhou City. Conclusions Changzhou City is now at the post-elimination surveillance stage; however, there are still factors affecting schistosomiasis transmission. The schistosomiasis surveillance system remains to be improved to consolidate the schistosomiasis elimination achievements in Changzhou City.

Objective:To compare the effects of different fractions of inspired O 2 (FiO 2) during general anesthesia induction on perioperative pulmonary function in patients undergoing gynecological laparoscopic surgery. Methods:A total of 100 patients, aged 30-64 yr, with body mass index<30 kg/m 2, of American Society of Anesthesiologists physical status Ⅰ or Ⅱ, scheduled for elective total laparoscopic hysterectomy, were divided into 4 groups ( n=25 each) using a random number table method: group A, group B, group C and group D. In A, B, C and D groups, FiO 2 was 40%, 60%, 80% and 100%, respectively.Immediately after the establishment of electrocardiogram monitoring after entering the operation room (T 0), immediately after tracheal intubation (T 1), immediately after creation of pneumoperitoneum (T 2), at 30 min of pneumoperitoneum (T 3), immediately before tracheal intubation (T 4) and at 15 min after tracheal intubation (T 5), ventral (ROIⅠ), middle ventral (ROIⅡ), middle dorsal (ROIⅢ) and dorsal (ROI Ⅳ) lung ventilation were recorded using electrical impedance tomography.The impedance ratio (IR) was calculated and the safety time without ventilation was recorded.Blood samples were collected from the radial artery at T 0-2 and T 5 for blood gas analysis, arterial oxygen partial pressure (PaO 2) and arterial carbon dioxide partial pressure (PaCO 2) were recorded, and oxygenation index (OI) was calculated.The occurrence of pulmonary complications within 3 days after operation was recorded. Results:Compared with group D, the ventilation in ROIⅠ and ROIⅡwas significantly increased, the ventilation in ROIⅢ and ROIⅣ was decreased, and IR was increased at T 1 and T 2 in A, B and C groups, PaO 2 and OI were decreased at T 1, and the safety time without ventilation was prolonged in A and B groups, and PaO 2 was decreased, OI was increased at T 1 ( P<0.05), and no significant was change in the safety time without ventilation in group C ( P>0.05). There was no significant difference in the incidence of pneumoperitoneum among the 4 groups ( P>0.05). Conclusion:During general anesthesia induction, 80% FiO 2 has the least effect on lung function in the patients undergoing gynecological laparoscopic surgery.

ABSTRACT

Screening suitable reference genes is the premise of quantitative Real-time PCR(qRT-PCR)for gene expression analysis. To provide stable reference genes for expression analysis of genes in Aconitum vilmorinianum, this study selected 19 candidate re-ference genes(ACT1, ACT2, ACT3, aTUB1, aTUB2, bTUB, 18S rRNA, UBQ, eIF2, eIF3, eIF4, eIF5, CYP, GAPDH1, GAPDH2, PP2A1, PP2A2, ACP, and EF1α) based on the transcriptome data of A. vilmorinianum. qRT-PCR was conducted to profile the expression of these genes in the root, stem, leaf, and flower of A. vilmorinianum. The Ct values showed that 18S rRNA with high expression level and GAPDH2 with large expression difference among organs were not suitable as the reference genes. NormFinder and geNorm showed similar results of the expression stability of the other candidate reference genes and demonstrated PP2A1, EF1α, and CYP as the highly stable ones. However, BestKeeper suggested EF1α, ACT3, and PP2A1 as the top stable genes. In view of the different results from different softwares, the geometric mean method was employed to analyze the expression stability of the candidate re-ference genes, the results of which indicated that PP2A1, EF1α, and ACT3 were the most stable. Based on the comprehensive analysis results of geNorm, NormFinder, BestKeeper, and geometric mean method, PP2A1 and EF1α presented the most stable expression in different organs of A. vilmorinianum. PP2A1 and EF1α were the superior reference genes for gene expression profiling in different organs of A. vilmorinianum.

OBJECTIVE@#To observe the effect of acupoint thread-embedding on tight junction of intestinal mucosal epithelial barrier in rats with ulcerative colitis (UC) under the state of "deficiency and stasis", and to explore its mechanism.@*METHODS@#Sixty male SD rats were randomly divided into a control group (@*RESULTS@#Compared with the control group, in the model group the body weight was decreased (@*CONCLUSION@#The thread-embedding could repair the tight junction of intestinal mucosa epithelium and reduce the permeability of intestinal mucosa epithelium, which may be related to the decrease of the expression of CaMKⅡ, MLCK and other protein kinases.

ABSTRACT

ObjectiveTo investigate the mechanism of action of Xiaoyao powder combined with Sijunzi decoction, Artemisia capillaris Thunb. decoction, Longdan Xiegan decoction combined with Xiayuxue decoction, Wupi decoction combined with Sijunzi decoction, and Yiguan decoction in the treatment of hepatocellular carcinoma (HCC) based on network pharmacology and molecular docking. MethodsDatabases including TCMSP, TCMID, BATMAN-TCM, and TCM-MESH were used to screen out effective components and predict their targets, and databases including TTD, Drugbank, Disgenet, Liverome, OncoDB.HCC, and GEO were used to investigate HCC-related targets. The drug and disease targets were mapped to obtain the intersecting targets, and the visualization software Cytoscape 3.7.1 was used to construct the core component-intersecting target network and the protein-protein interaction (PPI) network. The core components and key genes were screened out and a survival analysis was performed in the GEPIA database. The active components and key genes screened out were imported into the DockThor online website for molecular docking. In addition, David database was used to perform gene ontology (GO) enrichment analysis and Kyoto Encyclopedia of Genes and Genomes (KEGG) pathway analysis of the intersecting targets. ResultsThe analysis showed that 110, 19, 154, 121, and 51 active components, respectively, were obtained for the above five classic prescriptions, and the numbers of drug targets were 7426, 1435, 9544, 6619, and 2427, respectively. Finally 4001 HCC disease targets were screened out. There were 260, 169, 276, 242, and 192 intersecting targets, respectively, between the five prescriptions and the HCC disease targets, and the survival analysis on the GEPIA online website obtained the common hub genes of PIK3CA, SRC, MAPK1, MAPK3 (all P<0.05) and AKT1 (P>0.05). Quercetin was the common active component of the five prescriptions, and isobavachin and Kanzonol W were the common active components of Xiaoyao powder combined with Sijunzi decoction, Longdan Xiegan decoction combined with Xiayuxue decoction, and Wupi decoction combined with Sijunzi decoction; the results of molecular docking showed that the above three components had a strong ability of binding to PIK3CA and SRC. GO enrichment analysis showed that these targets were involved in various biological processes including drug response, protein phosphorylation, inflammatory response, and angiogenesis, and KEGG enrichment analysis showed that the common pathways involved were cancer pathway, PI3K-Akt signaling pathway, MAPK signaling pathway, Ras signaling pathway, HIF-1 signaling pathway, hepatitis B pathway, and hepatitis C pathway. ConclusionQuercetin, isoflavone, and Kanzonol W have the potential mechanism of action involving multiple targets and pathways in the treatment of HCC.

ABSTRACT

Literatures on pain intervention with transcutaneous electrical acupoint stimulation (TEAS) were collected by searching the databases both in Chinese and English, and summarized to understand the research progress of TEAS effects on pain mediators in recent years. This will provide a more objective and scientific theoretical basis for clinical practice of TEAS to treat pain syndrome, thus promoting the clinical application of TEAS. Our literature analysis indicated that TEAS effectively regulated the release levels of various pain factors such as prostaglandin, 5-hydroxytryptamine, interleukins, substance P and tumor necrosis factor-α to achieve the analgesic effects by affecting the conduction pathways. TEAS is a safe, non-invasive and effective treatment for pain syndrome. However, further research is necessary due to the lack of rigor of the current clinical trial design.

ABSTRACT

Objective:To explore the effects of interpregnancy interval (IPI) on pregnancy outcomes of subsequent pregnancy.Methods:A multicenter retrospective study was conducted in 21 hospitals in China. Information of age, height, pre-pregnancy weight, IPI, history of diseases, complications of pregnancy, gestational age of delivery, delivery mode, and pregnancy outcomes of the participants were collected by consulting medical records of pregnant women who had two consecutive deliveries in the same hospital during 2011 to 2018. The participants were divided into 4 groups according to IPI:<18 months, 18-23 months, 24-59 months and ≥60 months. According to the WHO′s recommendation, with the IPI of 24-59 months group as a reference, to the effects of IPI on pregnancy outcomes of subsequent pregnancy were analyzed. Stratified analysis was further carried out based on age, history of gestational diabetes mellitus (GDM), macrosomia, and premature delivery, to explore the differences in the effects of IPI on pregnancy outcomes among women with different characteristics.Results:A total of 8 026 women were included in this study. There were 423, 623, 5 512 and 1 468 participants in <18 months group, 18-23 months group, 24-59 months group and ≥60 months group, respectively. (1) The age, pre-pregnancy body mass index (BMI), history of cesarean section, GDM, gestational hypertension and cesarean section delivery rate of <18 months group, 18-23 months group, 24-59 months group and ≥60 months group were gradually increased, and the differences were statistically significant ( P<0.05). (2) After adjusting for potential confounding factors, compared with women in the IPI of 24-59 months group, the risk of premature delivery, premature rupture of membranes, and oligohydramnios were increased by 42% ( OR=1.42, 95% CI: 1.07-1.88, P=0.015), 46% ( OR=1.46, 95% CI: 1.13-1.88, P=0.004), and 64% ( OR=1.64, 95% CI: 1.13-2.38, P=0.009) respectively for women in the IPI≥60 months group. No effects of IPI on other pregnancy outcomes were found in this study ( P>0.05). (3) After stratified by age and adjusted for confounding factors, compared with women in the IPI of 24-59 months group, IPI≥60 months would significantly increase the risk of oligohydramnios for women with advanced age ( OR=2.87, 95% CI: 1.41-5.83, P=0.004); and <18 months could increase the risk of premature rupture of membranes for women under the age of 35 ( OR=1.59, 95% CI: 1.04-2.43, P=0.032). Both the risk of premature rupture of membranes ( OR=1.58, 95% CI: 1.18-2.13, P=0.002) and premature delivery ( OR=1.52, 95% CI: 1.07-2.17, P=0.020) were significantly increased in the IPI≥60 months group. After stratified by history of GDM and adjusted for confounding factors, compared with women in the IPI of 24-59 months group, IPI≥60 months would lead to an increased risk of postpartum hemorrhage for women with a history of GDM ( OR=5.34, 95% CI: 1.45-19.70, P=0.012) and an increased risk of premature rupture of membranes for women without a history of GDM ( OR=1.44, 95% CI: 1.10-1.90, P=0.009). After stratified by history of macrosomia and adjusted for confounding factors, compared with women in the IPI of 24-59 months group, IPI≥60 months could increase the proportion of cesarean section for women with a history of macrosomia ( OR=4.11, 95% CI: 1.18-14.27, P=0.026) and the risk of premature rupture of membranes for women without a history of macrosomia ( OR=1.46, 95% CI: 1.12-1.89, P=0.005). After stratified by history of premature delivery and adjusted for confounding factors, compared with women in the IPI of 24-59 months group, IPI≥60 months would significantly increase the risk of premature rupture of membranes for women without a history of premature delivery ( OR=1.47, 95% CI: 1.13-1.92, P=0.004). Conclusions:Both IPI≥60 months and <18 months would increase the risk of adverse pregnancy outcomes in the subsequent pregnancy. Healthcare education and consultation should be conducted for women of reproductive age to maintain an appropriate IPI when they plan to pregnant again, to reduce the risk of adverse pregnancy outcomes in the subsequent pregnancy.

ABSTRACT

@#AIM: To study the changes of the red blood cell volume distribution width(RDW)level and the correlation between them in patients with primary angle-closure glaucoma(PACG).<p>METHODS: Retrospective case-control study was conducted. A total of 306 PACG patients in Eye & ENT Hospital of Fudan University from February 2019 to October 2020 were selected as the case group and 126 healthy patients who received physical examination at the hospital during the same period were selected as the control group. The PACG patients were divided into three subgroups according to the mean defect of visual field(MD): mild(MD≤6dB), moderate(6<MD≤12dB), and severe(MD>12dB). The detailed eye and physical examinations information of the two groups were collected from the medical data platform of Eye & ENT Hospital of Fudan University. The blood sample parameters were detected by automated hematology and biochemistry analyzers. Independent sample <i>t</i>-test, Chi-square test, one-way ANOVA test, Pearson correlation analyses and multiple linear regression analyses were performed in this research. <p>RESULTS: There was no significant difference between the PACG group and the control group in age, gender, body mass index and blood pressure(<i>P></i>0.05). Based on the laboratory results, the mean RDW level was significantly higher(<i>t</i>=2.132, <i>P</i>=0.034)in the PACG group(12.64%±0.96%)than that in the control group(12.45%±0.49%). Moreover, the mean RDW level and the mean hemoglobin(HG)level was significantly lower(<i>P</i><0.05)in the mild PACG subgroup than that in the moderate and severe PACG subgroups. The Pearson correlation analyses showed a positive correlation between the MD and the RDW level(<i>r</i>=0.141, <i>P</i>=0.013), a significantly positive correlation between the IOP and the RDW level(<i>r</i>=0.148, <i>P</i>=0.002), and a negative correlation between the MS and the RDW level(<i>r</i>=-0.154, <i>P</i>=0.007)in patients with PACG. After adjusting for the confounding factors, multiple linear regression analyses showed a positive association between between the MD and the RDW level in patients with PACG(<i>β</i>=1.405, <i>P</i>=0.018, 95%<i> CI</i>=0.240-2.570). <p>CONCLUSION: The RDW level of PACG patients is higer than normal. The RDW level may help to predict the PACG patients' conditions for better treatments.

ABSTRACT

OBJECTIVE: To observe the expression of acidic mammalian chitinase(AMCase) in lung tissue of silicosis model rats, and bronchoalveolar lavage fluid(BALF) and serum of patients with occupational pneumoconiosis, and to evaluate the value of AMCase in the early diagnosis of pneumoconiosis. METHODS: i) The specific pathogen free adult male Wistar rats were randomly divided into control group and model group, with 15 rats in each group. The rats in the silicosis model group was exposed to free silica dust with a concentration of 2 000.0 mg/m~3 by dynamic inhalation for three hours a day, while the rats in control group were not exposed to dust. Five rats in the two groups were sacrificed at 4, 12 and 24 weeks after dust exposure. ii) By random number table method, a total of 191 patients with occupational pneumoconiosis who received large capacity lung lavage were selected as the pneumoconiosis group, 12 dust-exposed workers who received large capacity lung lavage were selected as the dust control group, and 200 healthy coal miners exposed to dust were selected as healthy control group. iii) Western blotting was used to detect the relative protein expression of AMCase, type Ⅰ collagen(COLⅠ), α-smooth muscle actin(α-SMA) in lung tissues of the rats and the relative protein expression of AMCase in human BALF. Enzyme linked immunosorbent assay was used to detect the level of AMCase protein in human serum. Receiver operating characteristic(ROC) curve was used to evaluate the value of AMCase protein level in human serum for early diagnosis of pneumoconiosis. RESULTS: The relative expression of AMCase, COLⅠand α-SMA protein in lung tissue of rats in the silicosis model group were higher than that of control group(all P<0.01). The relative expression of AMCase protein in BALF of pneumoconiosis group and pneumoconiosis stage Ⅰ, Ⅱ and Ⅲ subgroups were higher than that of dust control group(all P<0.05). The level of AMCase protein in serum of pneumoconiosis group and pneumoconiosis stage Ⅰ, Ⅱ and Ⅲ subgroups were higher than that of healthy control group(all P<0.05). The results of ROC curve analysis showed that the area under ROC curve was 0.78(95% confidence interval: 0.74-0.82).When the cut-off value of serum AMCase protein level was 466.0 ng/L, the sensitivity was 73.8%, and the specificity was 72.6%. CONCLUSION: AMCase protein in human serum has value for early diagnosis of pneumoconiosis and it could be a potential biomarker for early diagnosis of pneumoconiosis.

ABSTRACT

Objective:To investigate the prevalence of metabolic syndromes (MS) and MS scores as well as related health behavior factors in Zhongshan Community, Songjiang, Shanghai. Methods:A total of 6 802 residents aged 20-74 years old in Zhongshan Community were selected. Face-to-face survey and body measurement were used to collect information such as MS-related behavioral factors (including smoking, alcohol intake, exercise and diet) and to determine the MS scores. MS scores were divided into 6 levels. Ordered logit model was used to analyze the factors related to MS score, and logit model was used to analyze the factors related to MS. Results:The prevalence of 6 metabolic syndrome scores in the sample population were 13.5%, 24.3%, 25.1%, 19.7%, 12.3%, 5.1%, respectively. The prevalence of MS was 37.0%. The related factors of high MS score in male were advanced age, alcohol intake and tea drinking, while the related factors of high MS score in female were advanced age and previous smoking. The related factors of MS was alcohol intake in male while female with advanced age had higher risk in developing MS. Conclusion:The prevalence of MS in Zhongshan Community is relatively high, which has become one of the important public health problems in this community. Attention should be paid to the elderly men who drink alcohol and tea, and aged women who have ever smoked.

OBJECTIVE@#To investigate the effect of complement C3 on the prognosis of patients with multiple myeloma (MM), and to establish a predictive model to evaluate the overall survival.@*METHODS@#Eighty newly diagnosed MM patients were enrolled, and clinical characteristics, such as sex, age, platelet count, white blood cell count, ISS stage, FISH, levels of kappa and lammda chain, complement C3 and C4 were retrospectively analyzed. Cox regression model was used for univariate and multivariate analysis about risk factors that affecting the prognosis of the MM patients. A nomogram based on C3 level was established for predicting the prognosis of MM patients.@*RESULTS@#The average age of the MM patients was 63.15±10.41, including 36 males and 44 females. The median overall survival (OS) was 36.3 months, and the median progression-free survival (PFS) was 35.2 months, the 3-year OS rate and PFS rate of the MM patients were 67.5% and 52.5%, respectively. The variants selected by univariate analysis were put into multivariate regression model, the result showed that C3 level ≥0.7 U/L and PLT count <100×10@*CONCLUSION@#Patients with C3 level≥0.7 U/L or PLT count <100×10
