ABSTRACT

Objective:To study the expression of miRNA-7 in B lymphocytes of primary Sj?gren's syndrome (pSS) and its relationship with phosphatase andtensin homolog deleted (PETN) and disease activity. Methods:Twenty newly diagnosed outpatient and inpatient pSS patients were used as case group collected from January 2017 to December 2019 of Qinghai Provincial People's Hospital. Twentyhealthy persons were used as the control group. Disease-related indicators of the case group were collected. Quantitative polymerase chain reaction (RT-qPCR)was used to detect miRNA-7 and PETN mRNA expression in B lymphocytes of the two groups and the consistency between miRNA-7 expression in the plasma and B lymphocytes of the case group was analyzed. Western Blotting method was used to detect the PETN protein in B lymphocytes of the two groups. Correlation analysis was used to analyze the relationship between miRNA-7 expression in B lymphocytes and disease activity in the case group. Linear regression analysis was performed between miRNA-7 and PETN mRNA.Results:The expression of miRNA-7 (0.53±0.17) in the B cells increased and the expression of PTEN mRNA (0.88± 0.24) and protein (0.51±0.12) in the case group were reduced compared with that of miRNA-7(0.39±0.11), PTEN mRNA(2.32±0.30) and protein(1.03±0.21) of the control group. The above differences were statistically significant ( t=2.990, P<0.05; t=16.98, P<0.05; t=8.41, P<0.05). Linear regression showedthat PTEN miRNAwas negatively correlated with miRNA-7 ( b=-0.78, P<0.01), the expression of miRNA-7 in the case group was positively related with EULAR Sj?gren′s syndrome Disease Activity Index (ESSDAI), IgG, IgA, anti-SSB and was negatively correlated with C4 and WBC. Conclusion:There is a certain relationship between miRNA-7 and disease activity. MiRNA-7 may participate in the pathogenesis of pSS byregulating PETN in B cells of pSS. MiRNA-7 has certain clinical value for disease activity evaluation.

ABSTRACT

A splicing mutation in VPS4B can cause dentin dysplasia type I (DD-I), a hereditary autosomal-dominant disorder characterized by rootless teeth, the etiology of which is genetically heterogeneous. In our study, dental follicle cells (DFCs) were isolated and cultured from a patient with DD-I and compared with those from an age-matched, healthy control. In a previous study, this DD-I patient was confirmed to have a loss-of-function splicing mutation in VPS4B (IVS7 + 46C > G). The results from this study showed that the isolated DFCs were vimentin-positive and CK14-negative, indicating that the isolated cells were derived from the mesenchyme. DFCs harboring the VPS4B mutation had a significantly higher proliferation rate from day 3 to day 8 than control DFCs, indicating that VPS4B is involved in cell proliferation. The cells were then replenished with osteogenic medium to investigate how the VPS4B mutation affected osteogenic differentiation. Induction of osteogenesis, detected by alizarin red and alkaline phosphatase staining in vitro, was decreased in the DFCs from the DD-I patient compared to the control DFCs. Furthermore, we also found that the VPS4B mutation in the DD-I patient downregulated the expression of osteoblast-related genes, such as ALP, BSP, OCN, RUNX2, and their encoded proteins. These outcomes confirmed that the DD-I-associated VPS4B mutation could decrease the capacity of DFCs to differentiate during the mineralization process and may also impair physiological root formation and bone remodeling. This might provide valuable insights and implications for exploring the pathological mechanisms underlying DD-I root development.

ABSTRACT

Objective To evaluate the effect of isokinetic muscle training combined with semiconductor laser on acute knee osteoarthritis (KOA). Methods Ninety-eight KOA patients were randomly divided into 4 groups. All patients were treated with conventional rehabilitation treatment and nursing. Based on that treatment, the group 1 received semiconductor laser irradiation, the group 2 received isokinetic muscle training, and the group 3 received laser irradiation combined with isokinetic muscle training. All patients were assessed with WOMAC osteoarthritis rating scale and affected knee extensor and flexor muscles strength measurement including peak torque, peak work, average power, average work and flex/extend before and 4 weeks after the treatment. Results All of the four treatments can significantly alleviate the condition of acute KOA patients and improve the muscle condition around the ipsilateral knee joint. The isokinetic muscle training improves the knee function of KOA patients better than the laser irradiation treatment. Isokinetic strength training combined with laser irradiation can get the most significant improvement of knee joint pain, stiffness, dysfunction, muscle strength of flexors and extensors in KOA patients. Conclusions The combination of isokinetic muscle training and semiconductor laser irradiation has a significant effect on relieving pain, reducing the stiffness, improving the function of knee of the patients with KOA in the acute phase and improving the muscle strength of the affected lower extremities. That methad is superior to drug therapy, physical therapy, or exercise alone, and is better to solve the problem of relieving symptoms and enhancing function simultaneously.

ABSTRACT

Acute kidney injury (AKI) has a high incidence in neonates. A variety of perinatal factors may contribute to neonatal AKI, resulting in short- and long-term adverse outcomes, including chronic kidney disease. Early diagnosis and timely treatment can significantly improve the outcomes of these babies. Diagnostic criteria for AKI in adults and children are inapplicable to neonates due to the specific pathophysiological characteristics in newborns. In addition, current diagnosis criteria for neonatal AKI are still inconsistent. These all make it more difficult to diagnose neonatal AKI accurately. New biomarkers for kidney injury, such as neutrophil gelatinase-associated lipocalin, kidney injury molecule-1 and interleukin-18, have been found valuable for early and accurate diagnosis and prognosis of neonatal AKI. However, more studies are required to clarify the influencing factors, diagnostic values and clinical significance of those biomarkers.

ABSTRACT

Objective To discuss clinical characteristics of a family with Andersen-Tawil syndrome, test and analyze the mutation of their pathogenic gene, and to discover the dependency of genotype and phenotype by searching every reported Andersen-Tawil syndrome patient in all accessible literature worldwide. Methods In December 2nd,2016,two patients in the Department of Neurology, the First Hospital of Shanxi Medical University, received medical history data collection and relevant examinations.PCR and DNA sequencing were applied to detect ion channel diseases related genes such as SCN4A, CACNA1S, KCNJ2, KCNE3, KCNE4, KCNJ18, KCNJ5.Random selection of 200 healthy volunteers from Shanxi Medical University served as normal controls.Andersen-Tawil syndrome cases, which are accorded with statistical criteria in published literature, were collected.Their genotype and phenotype were analyzed and summarized. Results Clinical manifestation of the pre-confirmed patient and his father was accorded with diagnostic criteria of Andersen-Tawil syndrome.Prominent characteristics included ventricular arrhythmia, periodic paralysis, and dysmorphic features. The two patients had renal tubular acidosis. One of these two patients also had increased level of renin-angiotensin-aldosterone.New mutation Q164R in the KCNJ2 gene was found in these two patients. There was no report in any literature or any database that we could find about this gene mutation at present. The mutation was not found among other healthy family members and 200 healthy controls. In this study,we referenced 55 samples of Andersen-Tawil syndrome in 12 articles,in which 54 samples are KCNJ2 gene mutation, one is KCNJ5 gene mutation. We concluded a negative correlation between the onset age of periodic paralysis and the onset age of cardiac symptoms after a statistical analysis of these 54 patients with KCNJ2 gene mutation(rs=-0.698 1,P=0.005 5).The incidence of cardiac symptoms in patients of Andersen-Tawil syndrome with periodic paralysis was reduced(33.33%(14/42)vs 9/11, χ2=6.485,P=0.011).Men(96.00%(24/25))were found more likely to have periodic paralysis than women(65.52%(19/29); χ2=7.691,P=0.006).Women (64.29%(18/28))were found more likely to have cardiac symptoms than men(20.00%(5/25); χ2=10.545,P=0.001).Conclusions New mutation Q164R in the KCNJ2 gene is the cause of Andersen-Tawil syndrome, which could cause renal tubular acidosis. We speculate that the gene may play a role in the way of potassium regulating aldosterone. For women with the KCNJ2 gene mutation and the late-onset of periodic paralysis,it is important to take drug or manual intervention as early as possible to prevent the occurrence of cardiogenic adverse events.

<p><b>OBJECTIVE</b>To observe the effect difference between plum-blossom needle combined with rehabilitation training and conventional rehabilitation training for hand spasm after stroke.</p><p><b>METHODS</b>A total of 61 patients were randomly divided into a comprehensive treatment group (30 cases) and a rehabilitation training group (31 cases). In the rehabilitation training group, Bobath occupational therapy, OT training, and hand function training were adopted, once every day; on the basis of treatment in the rehabilitation group, plum-blossom needle was applied at the lung meridian of hand-, heart meridian of hand-, pericardium meridian of hand- in the comprehensive treatment group. The treatment was given once every two days, three weeks as one course in the two gnoups. After 3 courses of treatment, clinical efficacy evaluation was performed, and the modified Ashworth scale and Fugl-Meyer (FMA) motor function scores were assessed before and after treatment.</p><p><b>RESULTS</b>After treatment, the grade for Ashworth scale and FMA scores in the comprehensive treatment group and the rehabilitation trainning group were better than those before treatment (all <0.05), and the improvements in hand spasm and hand fuction in the comprehensive treatment group were superior apparently to those in the rehabilitation trainning group (both <0.05) The total effective rate of hand function was 93.3% (28/30) in the comprehensive treatment group, which was better than 74.2% (23/31) in the rehabilitation training group (<0.05).</p><p><b>CONCLUSION</b>Plum-blossom needle combined with rehabilitation training are more effective than simple rehabilitation training for hand spasm after stroke.</p>
