ABSTRACT
Objective:To evaluate the effect of gender on dose-effect relationship of remimazolam combined with alfentanil in painless gastroscopy. Methods:Subjects who planned to undergo elective painless gastroscopy, aged 18-60 years old, body mass index 19-24 kg/m2,American Society of Anesthesiologists physical status Ⅰ or Ⅱ,were enrolled. They were divided into male group and female group. The first subject in both groups received afentanil 5 μg/kg and remimazolam 0.2 mg/kg,and was implanted into a gastroscope 2 minutes later.Positive reactions were defined as body movement, coughing, swallowing and frowning during gastroscopy placement and examination.Remimazolam 0.05 mg/kg was used as a dose gradient by using modified Dixon's up-and-down method. The dose of the next subject was adjusted according to whether the subject had a positive reaction. If there was a positive reaction, the dose of the next subject was increased by one level of gradient, otherwise, the dose was decreased by one level of gradient, and so on.The process was terminated at the seventh intersection point of positive-negative reaction. And 50% effective dose(ED50),95% effective dose(ED95)and 95% confidence interval(CI)of remimazolam for inhibiting gastroscopic implantation reaction was calculated by Probit method. Results:A total of 46 subjects were included, with 23 subjects in each group. There was no significant difference in general data between the two groups. The ED50 of afentanil combined with remimazolam was 0.193 mg/kg(95% CI:0.145-0.286),and the ED95 was 0.293 mg/kg(95% CI:0.237-0.903)in male group. The ED50 of afentanil combined with remimazolam was 0.215 mg/kg(95% CI:0.155-0.293),and the ED95 was 0.316 mg/kg(95% CI:0.261-0.968)in female group. The ED50 and ED95(P<0.05).Conclusion:When combined with 5 μg/kg of afentanil, remimazolam is more effective in inhibiting responses to gastroscopy inserting in male subjects than in female subjects.
ABSTRACT
OBJECTIVE@#To investigate the risk factors of elbow stiffness after open reduction and internal fixation of intercondylar fracture of humerus.@*METHODS@#From March 2015 to February 2019, 120 patients with humeral intercondylar fractures were treated with open fixation including 59 males and 61 females, aged from 25 to 77 years with an average of(53.5±3.2) years. According to the occurrence of elbow stiffness after operation, 120 patients were divided into stiffness group(37 cases) and control group(83 cases). The related factors of elbow stiffness were analyzed by single factor analysis, and the risk of elbow stiffness after internal fixation of humeral intercondylar fracture was analyzed by logistic regression factor.@*RESULTS@#There were 37 cases of elbow stiffness(stiff group), and 83 cases had no elbow stiffness(control group). The incidence of joint stiffness was 30.83%. There were significant differences between the stiffness group and the control group in age, injury energy, fracture to operation time, AO classification of fracture, open injury and postoperative premature or hyperactivity. Multivariate logistic regression analysis showed that age>50 years old, high energy injury, AO classification of fracture, open fracture and postoperative premature or hyperactivity were risk factors for elbow stiffness after internal fixation of humeral intercondylar fracture. The postoperative mobility and Mayo elbow performance score(MEPS) scores of the postoperative stiffness group were lower than those of the non-stiffness group with statistical significance(P<0.05). There were no significant differences in postoperative mobility and MEPS scores between flexion stiffness and rotation stiffness after humeral intercondylar fracture(P>0.05).@*CONCLUSION@#In view of the risk factors of elbow stiffness after internal fixation of humeral intercondylar fracture, reasonable operation plan and rehabilitation strategy should be formulated before operation to minimize the incidence of elbow stiffness.
ABSTRACT
With the deepening of research on intestinal microbiota, more and more evidence indicates that intestinal microbiota affects various physiological processes of the human body and is closely related to tumors, Researches show that intestinal microbiota can promote tumorigenesis, enhance the antitumor effect of chemotherapeutic drugs, mediate chemoresistance, regulate the effect of immunotherapy, and reduce intestinal damage during radiotherapy. The phenomena and mechanisms revealed in the above studies are incredible. Based on the above correlation, the genus found in the study are expected to become the new target of tumor prevention and treatment. The evidence obtained from current animal and clinical trials is refreshing, however, in view of the complexity of human intestinal microecology and the limitations of human understanding, its potential role in tumorigenesis and treatment still needs systematic and in-depth research.

ABSTRACT
The decline in skeletal muscle mass and function with age is referred as sarcopenia. It is characterized by the muscle fiber's quality, strength, muscle endurance and metabolic ability decreasing as well as the fat and connective tissue growing. Previous studies have shown that sarcopenia in itself features decreased number and cross-sectional area of muscle fibers and the net degradation of protein, which results from the joint effects of multiple factors such as the exacerbation of inflammation, oxidative stress injury, mitochondrial dysfunction, abnormal autophagy and dysregulation of muscle quality regulatory factors. In this review, we systematically displayed the molecular mechanism of sarcopenia, which will be helpful to deepen our understanding of sarcopenia and provide potential targets for the prevention and treatment of sarcopenia.
ABSTRACT
Under normal condition, there are a few lipid droplets in skeletal muscle. But in skeletal muscle acute injury, muscular dystrophy, muscle atrophy, obesity, diabetes and other pathological conditions, the fat deposition in skeletal muscle increases, which implicate that the fat deposition may play an important role in the pathogenesis of these diseases. However, the mechanisms of development and regulation of fat deposition in skeletal muscle are not clear. Clarifying the key signaling pathways and regulatory factors that affect fat deposition in skeletal muscle, and exploring new ways to improve the fat deposition in skeletal muscle will not only help to deepen our understanding of the pathogenesis of these diseases, but also provide new ideas for the treatment of these diseases. This paper reviews the research progresses and main mechanisms of fat deposition in skeletal muscle.

ABSTRACT
This study is to investigate the effect of rhein lysinate on inducing human breast cancer cell line SK-Br-3 apoptosis and the role of HER-2 signal pathway in the apoptosis. MTT assay was used to detect SK-Br-3 cell proliferation. Cell cycle and apoptosis were analyzed by flow cytometry. The protein expression and the protein phosphorylation of HER-2 signal pathway were detected by Western blotting. The level of HER-2 mRNA was detected by RT-PCR and the level of HER-2 expression was also detected by immunofluorescence cytochemical methods. The results showed that rhein lysinate remarkably inhibited breast cancer SK-Br-3 cell proliferation. The IC50 value for 48 h treatment was 85 micromol x L(-1). Apoptosis in SK-Br-3 cells was induced by rhein lysinate in a dose dependent manner. The protein expressions of HER-2, NF-KB, and the protein phosphorylation of HER-2 were downregulated, however the protein expression of p53 and p21 was upregulated after rhein lysinate treatment. The level of HER-2 mRNA decreased by using RT-PCR assay and the level of HER-2 expression was also decreased by using immunofluorescence cytochemical assay after rhein lysinate treatment. It can be concluded that rhein lysinate could inhibit SK-Br-3 cell proliferation and induce apoptosis. HER-2/NF-kappaB/p53/p21 signal pathway might be involved in this process. Rhein lysinate has a good prospect to be an adjuvant chemotherapeutic drug.

Objective: To investigate the differentiated expressions of EphB2 protein in colorectal carcinoma at different clinicopathologic staging and in different colorectal carcinoma cell lines with varying metastasis potential and explore the effect of EphB2 on colorectal cancer and its action mechanism. Methods: Immunohistochemistry was used to detect the expression of EphB2 and microvascular density (MVD) in colorectal carcinoma tissues (n = 43) and normal colon tissues (n = 10) was detected. Four colorectal carcinoma cell lines with varying metastasis potential, SW480, HT29, HCT116, and LoVo cells, were cultured in vitro. The expression of EphB2 protein was measured by Western blotting and the transcription of EphB2 mRNA was determined by RT-PCR. Results: EphB2 was expressed in the cytoplasm of the most colon carcinomas. EphB2 receptor expression was negatively related with tumor infiltration depth and distant metastasis; EphB2 receptor expression was positively related with the differentiation degree of colon cancer. Expression of EphB2 protein had positive correlation with MVD in tumor tissues; the expression level of EphB2 was decreased in high-metastatic HCT116 and LoVo cells compared with low-metastatic SW480 and HT29 cells. Conclusion: Decreased expression of EphB2 may promote the carcinogenesis and progression of colorectal carcinoma. One of the explanations for the effect of EphB2 is its regulaion on tumor angiogenesis.

Objective: Our aim was to investigate the alteration of p16 gene in human pancreatic carcinoma. Methods: A total of 66 human pancreatic tissue specimens, comprising 51 with pancreatic carcinomas and 15 normal pancreatic tissue specimens, were examined for homozygous deletion and mutation of p16 gene by using PCR-SSCP method. Results: No mutation and deletion was detected in 15 normal pancreatic tissue samples. Of 51 pancreatic carcinoma specimens, only one was found mutation for p16 gene in PCR-SSCP assay, and the deletion of the p16 gene in 23 samples were confirmed by using PCR, with a 45% p16 gene deletion rate. Conclusion: These data suggest that p16 gene alterations may play a role in the progression of human pancreatic carcinoma.
