ABSTRACT

Pancreatogenic diabetes is a type of diabetes mellitus secondary to exocrine pancreatic disease, and it was officially proposed by the American Diabetes Association in 2014, with chronic pancreatitis as the most common etiology, followed by pancreatic cancer. At present, the misdiagnosis rate of this disease is extremely high, and patients with pancreatogenic diabetes have a higher risk of death and readmission than patients with type 2 diabetes. Therefore, it is of great significance to fully understand, correctly identify, and diagnose pancreatogenic diabetes in the early state, so as to reduce the disability rate and mortality rate of this disease. This article reviews the advances in the possible pathogenesis, diagnosis, treatment, and management of pancreatic diabetes secondary to pancreatitis and pancreatic cancer.

ABSTRACT

Objective To observe and analyze the clinical effect of cefuroxime combined with methylprednisolone in the treatment of elderly patients with chronic obstructive pulmonary disease(COPD) complicated with pulmonary emphysema.Methods 92 COPD patients complicated with pulmonary emphysema were selected as study objects,and they were randomly divided into observation group and control group.46 cases in the control group were treated with antibiotic treatment, and 46 cases in the observation group were treated with cefuroxime combined with methylprednisolone.The clinical curative effect, pulmonary function, blood gas index and incidence of adverse reactions were compared between the two groups.Results The total effective rate of the observation group was 91.30%, which was significantly higher than 76.09% of the control group (x2=8.476,P0.05).Conclusion In elderly COPD patients with emphysema, using cefuroxime combined with methylprednisolone therapy can effectively alleviate the clinical symptoms, improve lung function and blood gas index, and has less adverse reactions, higher safety and high clinical application value.

ABSTRACT

OBJECTIVE:To study the effects of goserelin on vascular growth factor and immune function of rats with prostatic hyperplasia. METHODS:Rats were selected to establish prostatic hyperplasia model and randomly divided into model group,gose-relin low-dose,medium-dose and high-dose groups (0.4,0.8,1.2 mg/kg);normal rats were selected as normal control group, with 10 rats in each group. Normal control group and model group were given normal saline intragastrically,and goserelin groups were given relevant dose of drugs intragastrically,once a day,for consecutive 25 days. The prostate volume,wet weight,prostatic index of rats were detected as well as positive cell area of VEGF,TGF-β1,FGF,CD4 and CD8 in prostate tissue. RESULTS:Compared with normal control group,prostate volume,wet weight,prostatic index,positive cell area of VEGF,TGF-β1,FGF, CD4 and CD8 were all increased in model group (P0.05). CONCLUSIONS:Gosere-lin can relieve prostatic hyperplasia of rats,reduce the expression of VEGF in prostate tissue and regulate immune function.

ABSTRACT

ObjectiveTo investigate the efficacy of Baihe Xiaoyao powder in patients with chronic hepatitis B (CHB) and depression, and to explore its mechanism of action from the perspectives of endocrine and immune function. MethodsA total of 120 patients with CHB who visited the Outpatient Department of Zhejiang Hospital of Integrated Traditional Chinese and Western Medicine from September 2013 to February 2015 were enrolled, and according to the Hamilton Depression Scale (HAMD) scores, they were divided into CHB-depression group (80 patients) and CHB group (40 patients). The levels of cortisol (Cor) and interleukin-2 (IL-2) and the proportion of natural killer (NK) cells were compared between the two groups. The 80 patients in the CHB-depression group were further divided into control group and treatment group, with 40 patients in each group. The patients in the control group were treated with entecavir for 12 weeks, and those in the treatment group were treated with entecavir and Baihe Xiaoyao powder for 12 weeks. The effects of Baihe Xiaoyao powder on liver function [alanine aminotransferase (ALT) and aspartate aminotransferase (AST)], hepatitis B virus (HBV) DNA, HAMD score, Cor, and immune function (IL-2 and NK cells) were observed in patients with CHB and depression. The t-test was used for comparison of continuous data, the chi-square test was used for categorical data, and correlation analysis was also performed. ResultsCompared with CHB patients without depression, those with depression had a significantly higher level of Cor, a significantly lower level of IL-2, and a significantly lower proportion of NK cells (P<0.01), and Cor level was negatively correlated with IL-2 level (r=-0.786, P<0.01) and the proportion of NK cells (r=-0.681, P<0.01). After the treatment with Baihe Xiaoyao powder for 12 weeks, compared with the control group, the treatment group had significantly lower HAMD score and levels of Cor, ALT, and AST (P<0.01) and significantly higher level of IL-2, proportion of NK cells, and HBV DNA clearance rate (P<0.05). ConclusionThe CHB patients with depression have endocrine disorders, an increased serum level of Cor, a reduced level of IL-2, and a reduced proportion of NK cells. Baihe Xiaoyao powder can improve depressive emotion, reduce the level of Cor, improve the body's immune function and liver function, and increase HBV DNA clearance rate. A combination of Baihe Xiaoyao powder and antiviral therapy can improve the clinical outcome of CHB.

ABSTRACT

Objective: To explore the feasibility with the safety at peri-operative and early post-operative periods for left atrial appendage occlusion in patients with non-valvular atrial ifbrillation (AF). Methods: A total of 17 non-valvular AF patients with transesophageal echocardiography (TEE) and CAHDS2 evaluation who received left atrial appendage occlusion in our hospital were summarized. The effect and complications during peri-operative and 1 month post-operative periods were analyzed. Results: All 17 patients had successful operation without severe complications. There were 8 patients having small amount residual shunt by TEE examination, 2 having ecchymosis near the puncture point at peri-operative period and 3 having small amount of hydropericardium by peri-cardio ultrasound examination. At the average of 45 days follow-up study, only 3 patients had small amount residual shunt, no left atrium thrombus and hydropericardium were observed. Conclusion: Left atrial appendage occlusion has the high success rate, it is safe during peri-operative and early post-operative periods in non-valvular AF patients.

ABSTRACT

A label-free fluorescent assay for highly sensitive detection of DNA was developed based on structure switch of hairpin DNA probe to a G-quadruplex-based DNAzyme triggered by target DNA. The hairpin DNA probe includes sequences that correspond to the base sequence of the G-quadruplex and to the sequence complementary to the target DNA, respectively. The hairpin structure of the probe DNA is energetically favored over the G-quadruplex structure in the absence of target DNA, resulting in the protection of the G-quadruplex in an inactive hairpin configuration. The hybridization of target DNA to the loop domain opens the stem of the hairpin, resulting in the self-assembly of the uncaged G-quadruplex structure, which acts as a DNAzymatic label for the signal production and amplification in the presence of hemin. The peroxidase-like DNAzyme oxidizes non-fluoresecnt 10-acetyl-3,7-Dihydroxypenoxa-zin (ADHP) to the florescent product by H2 O2 , giving rise to fluorescence emission. This allowed the utilization of the H2 O2-ADHP fluorescent system for quantitative analysis of DNA. The experimental conditions were optimized as:pH 8. 0, 10 mmol/L K+, 0. 2 μmol/L Hemin, 50 μmol/L ADHP. The assay showed a linear relationship toward target DNA concentration in the range of 5. 0 pmol/L-1. 0 nmol/L, with a limit of detection of 3. 0 pmol/L (S/N=3). The assay exhibited good selectivity against single-base mismatched DNA.

ABSTRACT

Objective To observe the variation of helper T cell (Th)9/regular T cell (Treg cell) balance in the inflammatory process of hepatitis B virus (HBV) infection.Methods Two hundred patients with chronic HBV infection in Zhejiang Hospital of Integrated Traditional and Western Medicine during August 2011 to June 2012 were divided into four groups according to alanine aminotransferase (ALT) levels.:chronic HBV carriers,chronic hepatitis B (CHB) with mildly [upper limit of normal (ULN)<ALT<3 × ULN],moderately (3 × ULN<ALT<5 × ULN),or significantly (ALT>5 × ULN) elevated ALT,with 50 subjects in each group.In addition,30 healthy volunteers were enrolled as healthy controls (HC).ALT levels,HBV DNA,and Th9/Treg levels were measured by automatic biochemical analyzing equipment,real-time fluorescent quantitative polymerase chain reaction and flow cytometry,respectively.Th9/Treg levels of each group were observed.Means between groups were compared by t test,means of multiple groups were compared by one-way analysis of variance.Spearman rank correlation was used for correlation analysis.Results Age,sex distribution and HBV DNA levels of the four groups were comparable (P>0.05).ALT levels of HC were significantly different from those of mildly,moderately,or significantly elevated ALT (t=8.48,18.41,28.93; all P<0.05).Th9 cell levels,Treg cell levels and Th9/Treg ratio of HBV infected individuals were significantly different from those of HC (P<0.05).With the increasing degree of inflammation,proportion of Th9 cells increased and Treg cells decreased gradually.The differences among the four groups were of statistical significance (P< 0.05).Th9 cell level was positively correlated with ALT (r=0.804,P=0.01),while Treg cell level was negatively correlated with ALT (r=-0.828,P=0.01).Conclusions Th9/Treg is imbalanced in patients with HBV infection,and closely correlated with ALT levels.

ABSTRACT

Objective To evaluate the role of ERK1/2 signal transduction pathway in sevoflurane postconditioning-induced reduction of oxygen-glucose deprivation(OGD)injury in rat hippocampal slices.Methods Male adult SD rats weighing 80-100 g were anesthetized with ether and decapitated.The hippocampi were removed and sagittally sliced(400μm thick)and placed in artificial cerebrospinal fluid(aCSF)aerated with 95% O2-5%CO2.Fifty hippoeampal slices were randomly divided into 5 groups(t =10 each):OGD group; 4% sevoflurane postconditioning group(group Sevo); PD98059(specific inhibitor of ERK)group(group PD); dimethyl sulfoxide (DMSO)group; 4% sevoflurane postconditioning + PD98059 group(group SPD).OGD was induced by incubating the slices in glucose-free aCSF aerated with 95% N2-5% CO2 for 15 min in group OGD.The hippocampal slices were perfused with aCSF saturated with 4% sevoflurane for 30 min after OGD was induced in group Sevo.The hippocampal slices were perfused with aCSF containing PD98059 50 μmol/L for 10 min after OGD was induced in group PD.The hippocampal slices were perfused with aCSF containing DMSO 1 mol/L for 10 min after OGD was induced in group DMSO.The hippocampal slices were perfused with aCSF containing PD98059 50 μmol/L and aerated with 4% sevoflurane for 30 min after OGD was induced in group SPD.The hippocampal slices were then perfused with plain aCSF for 1 h again in all the groups.The electrophysiological technique was used 1o record the amplitude of orthodromic population spike(OPS)in the stralum pyramidale of the CAI region.TTC staining was used to determine the degree of tissue injury.Results Compared with group OGD,the recovery amplitude and rate of OPS were significantly increased,and the degree of tissue injury was significantly decreased in group Sevo(P <0.01),while no significant change was found in each parameter in the other three groups(P > 0.05).Compared with group Sevo,the recovery amplitude and tale of OPS were significandy decreased,and the degree of tissue injury was significantly increased in groups PD,DMSO and SPD(P < 0.01).Conclusion ERK1/2 signal transduction pathway is involved in sevoflurane postcondilioning-induced reduction of OGD injury in tat hippocampal slices.

ABSTRACT

Objective To evaluate the effects of shRNA-TGF-β1 plasmid on Smads signal transduction of rat renal allograft.Methods A Sprague-Dawley to Wistar rat orthotopic transplant kidney-sclerosis accelerated model was constructed and transfected with short hairpin RNA-TGF-β1 based on the hydromechanics.The recipients were divided into three groups:group T(plasmid group)injected with shRNA-TGF-β1;group H(vacant plasmid group)injected with vacant plasmid;group Y(simply transplantation group)injected with no plasmid.In group J(sham-operated group)only right kidney was removed with no transplantation as control group.Transplanted kidneys and blood samples were collected at the first,second and third month after transplantation.The blood urea nitrogen(BUN)and serum Cr were tested by enzyme-linked immunoadsordent assay.The gene transcriptional level of TGF-β1 and Smad3/7 was detected by RT-PCR,and the protein variations of TGF-β1 and phosphorylated Smad3/7 were examined by Western blotting.Results At each test time point,the BUN and serum Cr were significantly higher in the plasmid group than in the sham-operated group(P<0.05 or P<0.01),but obviously lower than in the vacant plasmid group and simply transplantation group(P<0.05 or P<0.01).The expression of TGF-β1 as well as phosphorylated Smad3 was significantly higher in the plasmid group than in the sham-operated group(P<0.05 or P<0.01),but obviously lower than in the vacant plasmid group and simply transplantation group(P<0.05 or P<0.01).However,the expression of phosphorylated Smad7 was significantly lower in the plasmid group than in the sham-operated group(P<0.05 or P<0.01),but obviously higher than in the vacant plasmid group and simply transplantation group(P<0.05 or P<0.01).Conclusion Short hairpin RNA-TGF-β1 plasmid could significantly improve the renal function of rat renal allografts probably by downregulating phosphorylated Smad3 and upregulating phosphorylated Smad7,leading to the inhibition of TGF-beta 1 promoting fibrosis role and delay of the allograft fibrosis.

ABSTRACT

Objective To investigate the changes in the expression of phosphor-p38 mitogen-activated protein kinase (p-p38MAPK) in the distal cerebrospinal fluid contacting neurons (CSF-CNs) in rats with neuropathic pain.Methods Forty-eight male adult SD rats aged 2-3 months weighing 230-270 g were randomly divided into 2 groups (n =24 each): sham operation group (group S) and chronic constrictive injury group (group CCI).Neuropathic pain was induced by CCI.Sciatic nerve was exposed and 4 loose ligatures were placed on the sciatic nerve at 1 mm intervals with 4-0 silk thread.Thermal withdrawal latency (TWL) was measured with radiation heat stimulus before (baseline) and 1,3,5,7,14 d after CCI in 4 animals at eachtime point in each group.The animals were then sacrificed.30% cholera toxin subunit B with horse radish peroxidase 3 μl was injected into the lateral ventricle at 48 h before the animals were sacrificed at each time point.The brain tissue in the region of the midbrain aqueduct was taken.The number of distal CSF-CN expressing p-p38MAPK was calculated.p-p38MAPK was detected by histochemical staining.Results The TWL was significantly shortened at 1-14 d after CCI in group CCI as compared with group S.The number of distal CSF-CNs expressing p-p38MAPK was significantly increased at 5-14 d after CCI in group CCI as compared with group S.Conclusion The distal CSF-CNs may be involved in the neuropathic pain induced by CCI through p38MAPK signaling pathway.p38MAPK may contribute only to the maintenance but not development of neuropathic pain.

ABSTRACT

Objective To explore the evaluation methods of deodorants for patients with axillary odor. Methods Four trained judges used a blind method to evaluate 60 subjects directly or indirectly before and 6 hours and 24 hours after using deodorants. New T-shirts were provided for the subjects each time after grading. Then the judges evaluated the odor from the T-shirt as indirect axillary evaluation method. Then statistic analysis was used to compare the odor changes of axillary odor. Results The odors of axillary area were all decreased by two evaluation methods after using 3 different deodorants. But the efficacy of 3 deodorants were not completely same. Conclusion Direct axillary odor evaluation is a good method to evaluate the efficacy of deodorants.

An auto-retractable self-disable safety syringe is introduced. The product mainly consists of barrel, plunger hander, needle holder, spring and needle cap. After injection, the clasp on top of plunger hander will be jammed with the outshoot of needle holder, meanwhile, the plunger hander tip expands the stop structure and consequently the compression spring pulls the needle and needle holder back into plunger hander. The technical parameters of present syringe such as break force, needle holder push out force and plunger hander lock force are very stable while working. With simple structure, reasonable design, easy manufacture and assemble, this product can been widely used in relevant area.

ABSTRACT

Objective To investigate the effect of Niaochangshu capsule on urodynamics in bladder filling period of patients with post-menopause urethral syndrome of renal deficiency and blood stasis type.Methods A total of 36 patients with post-menopause urethral syndrome of renal deficiency and blood stasis type were randomly assigned to two groups:the trial group(24 patients were treated with Niaochangshu capsule) and the control group(12 patients were treated with Nilestriol).All patients were treated for 8 weeks.The changes of the urodynamic indexes in bladder filling period(the capacity of the bladder,the compliance of the bladder and the sensitivity of the bladder) were compared between the two groups.Results After treatment,the urodynamic index,including the maximum cystic capacity,the volume at the first desire to void and the cystic compliance were evidently improved in both of the trial group and the control group(P0.05).Conclusion Niaochangshu capsule is effective in treating the patients with post-menopause urethral syndrome of renal deficiency and blood stasis type.It can increase the cystic capacity and compliance,step down the sensitivity of the bladder.Its effect is similar to Nilestriol.
