ABSTRACT

@#AIM: To evaluate the visual quality of patients with high myopia complicated with cataract after implantation of trifocal intraocular lens by OPD-Scan Ⅲ aberration analyzer.METHODS: Totally 32 patients(38 eyes)with high myopia complicated with cataract who underwent femtosecond laser assisted cataract phacoemulsification combined with trifocal intraocular lens implantation in Foshan Aier Eye Hospital from June 2018 to December 2020 were selected. Uncorrected distance visual acuity(UCDVA), uncorrected intermediate visual acuity(UCIVA)and uncorrected near visual acuity(UCNVA)were compared before, and 1wk, 3mo after surgery; OPD-Scan Ⅲ aberration analyzer measured objective visual quality before, and 1wk and 3mo after surgery. RESULTS: There were significant differences in UCDVA, UCIVA and UCNVA before, and 1wk, 3mo after operation(<i>P</i><0.05). The average orientation and centration distance was 0.12(0.08-0.15)mm and the average axial was(212.68±90.45)°in 3mo postoperative. There were significant differences in spherical aberration(<i>P</i><0.05), Strehl ratio(SR)and area ratio(AR)before, and 1wk, 3mo after operation(<i>P</i><0.001). In comparison, SR at 1wk and 3mo postoperative was higher than that before operation, whereas AR in 1wk and 3mo postoperative were both higher than that before operation(all <i>P</i><0.001). There was a negative correlation between SR and spherical aberration at 3mo postoperative(<i>rs</i>=-0.420, <i>P</i><0.01); There was a negative correlation between SR and trefoil at 3mo postoperative(<i>rs</i>= -0.418, <i>P</i><0.01); There was a negative correlation between AR and trefoil at 3mo postoperative(<i>rs</i>=-0.400, <i>P</i><0.05).CONCLUSION: Femtosecond laser assisted cataract surgery combined with trifocal intraocular lens implantation can provide a comfortable and natural full-range vision. The orientation and centricity of trifocal intraocular lens using OPD-Scan Ⅲ has shown that there was a good reliability and consistency. The vision quality using OPD-Scan Ⅲ is satisfactory.

ABSTRACT

Reactive oxygen species (ROS) is defined as the electron reduction product of oxygen with high reactivity which can maintain normal physiological functions and redox homeostasis. The tumor microenvironment is in a state of oxidative stress. ROS can affect multiple processes of tumor immune response by modulating the phenotype and functions of tumor cells and immune cells. With the rapid development of immunology, ROS-based tumor immunomodulation has been widely concerned and studied. In this review, the mechanism of ROS participating in tumor immune response is elaborated. Meanwhile, the research process and application of ROS in tumor immunomodulation in recent years are reviewed and analyzed.

ABSTRACT

Objective:To evaluate the clinical efficacy of Shaoyao-Bawei Decoction combined with moxibustion in the treatment of spleen stomach weakness syndrome of chronic atrophic gastritis (CAG). Methods:The 98 CAG patients admitted to the Zhaoqing Gaoyao District People’s Hospital from January 2019 to January 2021 who met the selection criteria were divided into 2 groups according to the random number table method, with 49 in each group. The control group received conventional western medicine treatment, and the observation group was treated with Shaoyao-Bawei Decoction combined with moxibustion on the basis of the control group. TCM symptom scores were performed before and after treatment, the levels of IL-6, IL-8 and TNF-α were detected by ELISA, and the levels of serum motilin (MLT), gastrin-17 (G17) and gastrin (gas) were detected by radioimmunoassay, the safety of medication was observed and the clinical efficacy was evaluated. Results:The total effective rate was 93.9% (46/49) in the observation group and 71.4% (35/49) in the control group. There was significant difference between the two groups ( χ2=8.611, P=0.043). After treatment, the scores of epigastric burning, fullness in stomach, dull appetite, dry mouth and bitter mouth, shortness of breath and unwillingness to speak, general lassitude, pale tongue, small and weak pulse in the observation group were significantly lower than those in the control group ( t=12.061, 7.331, 6.869, 5.975, 5.208, 10.567, 8.738, 8.631, respectively, all Ps<0.01), and the levels of serum IL-6, IL-8 and TNF-α were significantly lower than those in the control group ( t=8.573,13.423,12.099, respectively, all Ps <0.01). After treatment, the level of serum MLT (154.52 ± 26.25 ng/L vs. 180.26 ± 28.13 ng/L, t=4.488) in the observation group was lower than that of the control group ( P<0.01); the levels of G17 (14.28 ± 1.75 pmol/L vs. 10.28 ± 1.06 pmol/L, t=-7.966) and GAS (24.73 ± 3.42 ng/L vs. 19.02 ± 3.38 ng/L, t=-13.115) in the observation group were significantly higher than those in the control group ( t=-7.966 and -13.115, respectively, all Ps<0.01). During the treatment, the incidence of adverse reactions was 8.16% (4/49) in the observation group and 6.12% (3/49) in the control group, and there was no significant difference between the two groups ( χ2=0.152, P=0.695). Conclusion:Shaoyao-Bawei Decoction combined with moxibustion can alleviate the clinical symptoms of CAG patients with spleen stomach weakness syndrome, regulate the level of inflammatory cytokines, promote the recovery of gastrointestinal function and improve the clinical curative effect.

ABSTRACT

This study aimed to qualitatively analyze the chemical components in Xiaoer Chiqiao Qingre Granules(XRCQ) by UHPLC-LTQ-Orbitrap-MS/MS and identify its material basis. The absorbed components in plasma were combined for exploring the potential action mechanism by integrated network pharmacology. ACQUITY UPLC HSS T3(2.1 mm×100 mm, 1.8 μm) column and mobile phase system of 0.1% formic acid solution(A)-acetonitrile(B) were used for gradient elution, followed by high resolution liquid chromatography-mass spectrometry in both positive and negative ion scanning modes. According to the precise relative molecular mass and MS/MS fragment ions, a total of 124 chemical components were identified in XRCQ by the comparison with references and literature reports, among which 29 compounds were completely confirmed by comparison with reference substances. Then, the main absorbed components of XRCQ in plasma were also analyzed and clarified by UHPLC-LTQ-Orbitrap-MS/MS. BATMAN-TCM and SwissTargetPrediction were used for target prediction of absorbed components in plasma. Following the plotting of association network with Cytoscape 3.8.2, the core targets were subjected to GO and KEGG pathway enrichment analysis and a component-target-pathway network was constructed. A total of eight main targets of XRCQ against fever in children were obtained together with eight absorbed components in plasma, including glycyrhydinic acid, hesperidin, emodin, reticuline, daidzein, magnolignan C, magnolignan A, and magnolaldehyde D. It was inferred that XRCQ might improve alimentary system abnormality, inflammatory response, oxidative stress, and endocrine disorder through tumor necrosis factor, PI3 K-AKT, and other signaling pathways. The present study comprehensively expounded the chemical profiles of XRCQ and the main absorbed components in plasma and predicted the potential mechanism of XRCQ based on integrated network pharmacology, which has provided certain theoretical reference for the clinical application of XRCQ.

ABSTRACT

This study aimed to investigate the enhancing effect of muscone on the transdermal penetration of traditional Chinese medicine ingredients and explore its possible mechanism of action. The Franz diffusion cells were employed to investigate the effect of muscone on the transdermal permeation of a series of model drugs with a wide range of log P values. The solubilities at saturation and the stratum corneum(SC)/vehicle partition coefficients of model drugs were measured to evaluate the effect of muscone on drug thermodynamic activities and partition of drugs into SC. Attenuated total reflectance-Fourier transform infrared spectroscopy(ATR-FTIR) was employed to explore the effect of muscone on the molecular structure of SC. The results showed that muscone significantly promoted the transdermal penetration of hydrophilic and lipophilic drugs, and the enhancement ratio(ER) increased with the decrease in the log P. Muscone could interact with the SC lipids to increase the disorder and fluidity of lipid bilayer packing, which improved skin permeability and promoted transdermal absorption of drugs. This study provides a scientific basis for the application of muscone in traditional Chinese medicine topical preparations.

ABSTRACT

Objective To investigate the suitable services of telemedicine at present and the future from the perspective of medical service supplier,clarify the challenges in the development of telemedicine services at present,and provide evidence-based suggestions for improving the quality of telemedicine services. Methods A questionnaire was developed through literature review for the survey of telemedicine services from the perspective of service providers.From January to June in 2020,electronic questionnaires were collected from volunteers.The data were collated and analyzed by Excel 2010 and SPSS 21.0. Results A total of 614 questionnaires were distributed,and 582 effective questionnaires were collected,which showed an effective rate of 94.79%.The participants of this study were mainly young healthcare workers,including doctors,nurses,technicians,and medical students.Among them,68.73% expressed concern to telemedicine services-related work,and more than 50% only had a basic understanding of the related work.The top five developable telemedicine/healthcare services were health management,online consultation,disease re-examination,disease screening,and difficult disease consultation,which were basically consistent with the services suitable for future development. Conclusions Healthcare workers have a high degree of recognition to the development of telemedicine services,while their understanding of the related technologies remains to be improved.The available telemedicine services focus on online consultation,difficult disease consultation,and disease re-examination,while the complicated medical services need to be improved.In the future,efforts should be made to continuously improve the information construction level and narrow the regional gap of medical services,so as to promote the continuous improvement and the coordinated development of medical services between regions.

ABSTRACT

Objective:To explore the value of blood-brain barrier permeability (BBBP) parameters based on CT perfusion (CTP) in predicting delayed cerebral ischemia (DCI) in patients with aneurysmal subarachnoid hemorrhage (aSAH) within 24 hours of admission.Methods:Totally 69 patients underwent whole-brain CTP within 24 h after aneurysm rupture from July to November 2020 in Yijishan Hospital of Wannan Medical College. The volume transfer constant (K trans) reflecting BBBP, the time to drain (TTD) and transit time to the center of the impulse response function (TMax) reflecting cerebral perfusion were obtained. Patients were divided into DCI and non-DCI groups. Quantitative and qualitative CTP parameters, clinical data were compared between the two groups. On the basis of univariate analysis, the multivariate logistic regression analysis was used to determine the independent risk factors of DCI using the stepwise regression method. The predictive efficiency of clinical data and CTP parameters were evaluated by ROC analysis. Results:Twenty-one of 69 aSAH patients developed DCI. Whole brain average values of K trans (mK trans) in the DCI group [(0.67±0.16)ml/(100 ml·min)] were significantly higher than those in the non-DCI group [(0.41±0.15)ml/(100 ml·min), t=-6.454, P<0.001]. mK trans in the diffused hypoperfusion patients [(0.61±0.18)ml/(100 ml·min)] was significantly higher than that in the normal perfusion group [(0.36±0.15)ml/(100 ml·min), P<0.001] and localized hypoperfusion group [(0.43±0.16)ml/(100 ml·min), P<0.001]. Multivariate logistic regression analysis showed mK trans (OR=1.13, 95%CI 1.05-1.21, P=0.001), World Federation of Neurosurgery Scale (OR=5.35, 95%CI 1.12-25.65, P=0.036) and modified Fisher Score (OR=5.32, 95%CI 1.02-27.80, P=0.048) were significantly independent predictors of DCI. ROC curve analysis revealed that mK trans produced the highest AUC of 0.875 (95%CI 0.78-0.97), with a threshold of 0.545 ml/(100 ml·min) had sensitivity of 85.7%, specificity of 79.2% and Youden index of 64.9% for prediction of developing DCI. Conclusion:It is feasible to evaluate cerebral perfusion and BBBP status and predict the risk of developing DCI in aASH patients who admitted within 24 h after aneurysm rupture using whole-brain CTP.

ABSTRACT

Objective:To explore the effect of the "trinity" training mode with curriculum training as the core, lecture training as supplement and community training as reinforcement, and to improve the health education ability of higher vocational nursing students.Methods:A total of 209 nursing students were selected as the research objects by random cluster sampling, and they were randomly divided into experimental group and control group. The experimental group adopted the "trinity" system to improve the health education ability of higher vocational nursing students, while the control group carried out the routine teaching. The health education ability of nursing students was evaluated by using the health education ability scale of nursing staff, and the nursing students of the experimental group were investigated by questionnaire. Statistics analysis was made by SPSS 15.0 software, the t test was conducted for analysis of measurement data, and Z test was used for counting data. Results:The health education ability of the nursing students in the experimental group was significantly higher than that of the control group ( P < 0.001). Nursing students in the experimental group generally had high recognition of the training mode of "trinity" system. The pass rate was 98.13%. Among them, 68.87% of the nursing students thought it would be a big gain to attend the special lectures; 69.81% of the nursing students thought it would be a big gain to participate in the community activities. Conclusion:The "trinity" health education ability training model can significantly improve the health education ability of higher vocational nursing students.

Cancer is a major global public health problem. Statistics from the national cancer center of China also show that cancer has become a major disease threatening human health with increasing morbidity and mortality. The occurrence and development mechanism of cancer is complex, involving multiple stages, multiple genes and multiple signaling pathways. Conventional chemoradiotherapy and emerging targeted therapy methods are the main methods in treatment of tumor. However, the quality of life of patients as well as the sustained and effective therapeutic effect are seriously affected due to the toxic side effects and drug resistance. Therefore, it is the global focus to find safe and effective anti-cancer drugs. The research and development and application of anti-cancer herbal medicines such as paclitaxel, vinblastine, podophyllin, ginsenoside and ginseng polysaccharide have brought new hope for the treatment of cancer. Cucurbitacine from Chinese medicine cucurbitaceae plantsare is a kind of highly oxidized tetracyclic triterpene compound with extensive pharmacological effects and complex mechanism. In the family of cucurbitacines, cucurbitin B, D, E and I have been studied most frequently on anticancer effect, and in a large number of studies, they have been found to play an important role in tumor diseases of the digestive system, respiratory system, reproductive system, blood system, urinary system and so on. With significant effect in inhibiting tumor cells proliferation, blocking the cell cycle, inducing apoptosis and autophagy death, inhibiting cell migration and invasion, inhibiting tumor angiogenesis, regulating the levels of reactive oxygen species and regulating immune system, such cucurbitacins are expected to be developed as a new kind of anti-cancer drugs. The authors of this study aim to provide reference for the further research and development of new anti-cancer drugs about cucurbitines by summarizing the anti-tumor effect and mechanism of the cucurbitacins.

ABSTRACT

Qingxin Lianzi Yin (QXLZY), as an ancient classical formula for clearing the heart and nourishing the Yin, was composed of nine herbs (Scutellariae Radix, Ophiopogonis Radix, Lycii Cortex, Plantaginis Semen, Glycyrrhizae Radix et Rhizoma Praeparata Cum Melle, Nelumbinis Semen, Poria, Astragali Radix and Ginseng Radix et Rhizoma), coming from Prescriptions of the Bureau of Taiping People's Welfare Pharmacy. It could clear away the heart-fire, promote the interaction of the heart and kidney, replenish Qi and Yin, and stop strangury-turbidity. It was used to treat gonorrhea of urination, seminal emission, restlessness, wasting-thirst and so on. At present, the usage and dosage of QXLZY and its addition and subtraction are different in clinical practice. Most of the studies just focus on its clinical efficacy, and there is few review literature reflecting its historical evolution. Based on this, this paper systematically clarified the historical evolution, composition, preparation, interpretation, function, and modern clinical application of QXLZY. This work has been explained the historical evolution of QXLZY, and found that it was wildly used in modern clinical, especially suitable for the treatment of chronic urinary system diseases. At the same time, QXLZY also had significant therapeutic effects on neurasthenia, stomatitis, diabetic nephropathy and other aspects. Through the comprehensive analysis of ancient and modern literature, this work explores the true connotation of QXLZY from the perspective of traditional Chinese medicine theory, which can point out the direction of the clinical application and positioning of this famous classical formula after it comes into the market, and also can provide reference basis for its subsequent in-depth research and development.

ABSTRACT

Esophageal cancer has a high morbidity and mortality worldwide, and the burden of esophageal cancer is still heavy in China. The conventional treatments such as radiotherapy and chemotherapy fail to yield a satisfactory outcome, implying that the safe and effective treatments are not available for the majority of patients. At present, esophageal cancer is mainly treated by Chinese medicine combined with radiotherapy and chemotherapy. The retrieval of related papers in Chinese and English reveals that Kang'ai injection, Compound Kushen injection, Kanglaite injection, Xiaoaiping injection, Aidi injection, Bruceae Fructus Oil Emulsion, Cinobufotalin injection, and Shenmai injection have been commonly used in esophageal cancer treatment. These eight injections are either single medicinal or compound preparations, involving 11 Chinese medicinals, which synergize the effects of radiotherapy and chemotherapy and reduce their adverse reactions in the treatment of esophageal cancer. Due to the complexity of Chinese medicinal components, a series of adverse reactions such as phlebitis, phlebosclerosis, and hemolysis may be caused by irrational dosage, administration speed, and compatibility in the use of injection. There are many clinical studies on Chinese medicine injections for anti-esophageal cancer but few studies on their active components and molecular mechanism. This paper reviewed the active components, effectiveness, safety, and mechanism of Chinese medicine injections frequently employed for treating esophageal cancer, in order to provide reference for their clinical application as well as the development of new Chinese drugs.

ABSTRACT

Objective:To investigate the effect of Ranae Oviductus (RO) on ovarian follicular development, phosphatidylinositol 3-kinase (PI3K)/protein kinase B (Akt) signaling pathway, and pregnancy function in rats, and the estrogen-like mechanism of OR. Method:Seventy female Wistar rats were randomly divided into a normal group, a progynova+ luteohormone group (1 mg·kg<sup>-1</sup>+40 mg·kg<sup>-1</sup>), a clomiphene group (10 mg·kg<sup>-1</sup>), and high-dose(400 mg·kg<sup>-1</sup>) and low-dose(200 mg·kg<sup>-1</sup>) RO groups. Rats were administered correspondingly by gavage for eight weeks. After seven weeks of intragastric administration, the estrus cycle of all rats was measured. After eight weeks of intragastric administration, four rats from each group were selected to give birth. For other rats, blood was collected on the day of estrus, and the serum levels of estradiol (E<sub>2</sub>),progesterone (P), testosterone (T), follicle-stimulating hormone (FSH), and luteotropic hormone (LH) were detected. Uterus and ovaries were extracted and weighed to calculate organ index. One ovary was made into pathological sections, and the follicles at different developmental stages and corpus luteum were counted. Real-time fluorescence-based quantitative polymerase chain reaction(Real-time PCR)and Western blot were performed on the other ovary to detect mRNA and protein changes in the PI3K/Akt signaling pathway. Forty female Kunming mice were randomly divided into a normal group and RO groups (400 mg·kg<sup>-1</sup>) with 14 days, 28 days, and 56 days of intervention. Mice in the RO groups were raised with male mice in cages after intragastric administration of OR for 14, 28, and 56 days, respectively. After 18 days, the number of intrauterine fetuses on both sides and the number of stunted fetuses were counted. Result:After eight weeks of intragastric administration of OR, the rats showed decreased uterine index (<italic>P</italic><0.05), declining serum LH (<italic>P</italic><0.05), reduced luteum (<italic>P</italic><0.01), dwindled primary follicles (<italic>P</italic><0.05), and increased rate of follicle atresia (<italic>P</italic><0.01). Additionally, more luteal or interstitial glands degenerated into interstitial structures in the ovarian cortex in a short time. The mRNA expression levels of PI3K and Akt in the ovary were elevated (<italic>P</italic><0.01), while the mRNA expression levels of mTOR and PTEN were reduced (<italic>P</italic><0.01). The phosphorylation level of Akt protein showed a downward trend without significant difference. For the rats, the number of fetuses was decreased (<italic>P</italic><0.05). The pregnancy rate of mice was decreased to varying degrees after administration of RO for different durations, with the lowest in the 14 day RO group, as low as 30%. After 28 days of intragastric administration of RO, the difference in left and right uterine pregnancy increased (<italic>P</italic><0.05). Conclusion:Long-term administration of RO can lead to premature ovarian failure by over-stimulating the ovary, which is similar to clomiphene. Short-term administration can result in decreased pregnancy rate, excessive ovulation on one side, and inhibition of ovulation on the other side. The influence on follicles needs further exploration.

Objective:To explore effects of different extracts and monomers of <italic>Lepidium meyenii </italic>(Maca) on the proliferation of mouse splenic lymphocytes and induction of interleukin-2 (IL-2) and tumor necrosis factor-<italic>α</italic> (TNF-<italic>α</italic>) by observing their immunomodulatory effects. Method:An octadecylsilyl (ODS) column was used to enrich the methanol extract of <italic>L. meyenii</italic> in stages to obtain six fractions and three monomers. Different groups of extracts and monomers of <italic>L. meyenii </italic>at different doses were set up. Cell counting Kit-8 (CCK-8) was used to detect the effect on the proliferation of mitogen-free, concanavalin A (Con A)-induced, and lipopolysaccharides (LPS)-induced mouse splenic lymphocytes. Enzyme-linked immunosorbent assay (ELISA) was used to determine the levels of IL-2 and TNF-<italic>α</italic>. Result:<italic>L. meyenii </italic>extracts Fr<sub>3</sub> and Fr<sub>6</sub>, and monomers <italic>N</italic>-benzyl hexadecanamide and 1,2-dihydro-4-carboxaldehyde-3-benzyl-<italic>N</italic>-hydroxypyridine slightly promoted the proliferation of Con A-induced T lymphocytes and LPS-induced B lymphocytes (<italic>P</italic><0.01) as compared with the conditions in the model group. <italic>L. meyenii</italic> extracts and monomers significantly induced the secretion of IL-2 and TNF-<italic>α</italic> by splenic lymphocytes (<italic>P</italic><0.01). Conclusion:<italic>L. meyenii</italic> extracts and monomers can achieve immunological enhancement by promoting the secretion of IL-2 and TNF-<italic>α</italic>, and facilitate the proliferation of splenic lymphocytes. The active components are presumedly macamides and pyridine alkaloids, and the specific mechanism still needs to be further explored.

ABSTRACT

Spinal disease is an important cause of cervical discomfort, low back pain, radiating pain in the limbs, and neurogenic intermittent claudication, and its incidence is increasing annually. From the etiological viewpoint, these symptoms are directly caused by the compression of the spinal cord, nerve roots, and blood vessels and are most effectively treated with surgery. Spinal surgeries are primarily performed using two different techniques: spinal canal decompression and internal fixation. In the past, tactile sensation was the primary method used by surgeons to understand the state of the tissue within the operating area. However, this method has several disadvantages because of its subjectivity. Therefore, it has become the focus of spinal surgery research so as to strengthen the objectivity of tissue state recognition, improve the accuracy of safe area location, and avoid surgical injury to tissues. Aside from traditional imaging methods, surgical sensing techniques based on force, bioelectrical impedance, and other methods have been gradually developed and tested in the clinical setting. This article reviews the progress of different tissue state recognition methods in spinal surgery and summarizes their advantages and disadvantages.

The aim of this paper was to study the effect and mechanism of fucoxanthin on insulin resistance of obese mice induced by high-fat diet. Fifty C57 BL/6 J male mice were randomly divided into control group and high-fat diet group. The insulin resistance model was induced with high-fat diet for 12 weeks, and model mice were randomly divided into model group, fucoxanthin-0.2% group, fucoxanthin-0.4% group and metformin group. After dietary treatment for 6 weeks, the body weight and epididymal fat weight in each group were measured. Fasting blood glucose(FBG), fasting insulin(FINS), total cholesterol(TC), triglyceride(TG), low-density lipoprotein(LDL-C) and high-density lipoprotein(HDL-C) were measured, and insulin resistance index(HOMA-IR) was calcula-ted. The pathological morphology in liver was observed by hematoxylin eosin staining, and the expressions of some key proteins in insulin receptor substrate 1(IRS-1)/posphoinositide 3-kinase(PI3 K)/serine-threonine kinase(Akt) and peroxisome proliferators-activated receptor-γ(PPARγ)/sterol regulatory element binding protein-1(SREBP-1)/fatty acid synthetase(FAS) pathways in liver were detected by Western blot. According to the findings, compared with the model group, levels of body weight, epididymal fat weight, FBG, FINS, TC, TG, LDL-C and HOMA-IR, as well as protein expressions of PPARγ, SREBP-1 and FAS in liver were significantly reduced(P<0.05 or P<0.01), while level of HDL-C and protein expressions of p-IRS-1, IRS-1, PI3 K and p-Akt in liver were signi-ficantly increased after treatment with fucoxanthin(P<0.05 or P<0.01). And the pathological changes of liver tissue in fucoxanthin-treated mice were also improved obviously. The results showed that fucoxanthin could improve obesity, hyperglycemia and hyperlipidemia, and alleviate insulin resistance in obese mice, and its mechanism is possibly related to the regulation of IRS-1/PI3 K/Akt and PPARγ/SREBP-1/FAS pathways.

ABSTRACT

In recent years, adaptive laboratory evolution (ALE) has emerged as a powerful tool for basic research in microbiology (e.g., molecular mechanisms of microbial evolution) and efforts on evolutionary engineering of microbial strains (e.g., accelerated evolution of industrial strains by bringing beneficial mutations). The ongoing rapid development of next-generation sequencing platforms has provided novel insights into growth kinetics and metabolism of microbes, and thus led to great advances of this technique. In this review, we summarize recent advances in the applications of long-term and short-term ALE techniques mainly for microbial strain engineering, and different modes of ALE are also introduced. Furthermore, we discuss the current limitations of ALE and potential solutions. We believe that the information reviewed here will make a significant contribution to further advancement of ALE.

ABSTRACT

Objective@#This study aims to investigate the correlation of an ultrasonic scoring system with intraoperative blood loss (IBL) in placenta accreta spectrum (PAS) disorders.@*Methods@#A retrospective cohort study was conducted between January 2015 and November 2019. Clinical data for patients with PAS have been obtained from medical records. Generalized additive models were used to explore the nonlinear relationships between ultrasonic scores and IBL. Logistic regressions were used to determine the differences in the risk of IBL ≥ 1,500 mL among groups with different ultrasonic scores.@*Results@#A total of 332 patients participated in the analysis. Generalized additive models showed a significant positive correlation between score and blood loss. The amount of IBL was increased due to the rise in the ultrasonic score. All cases were divided into three groups according to the scores (low score group: ≤ 6 points, @*Conclusions@#The risk of blood loss equal to or greater than 1,500 mL increases further when ultrasonic score greater than or equal to 10 points, the preparation for transfusion and referral mechanism should be considered.

ABSTRACT

Pneumoconiosis, an interstitial lung disease that occurs from breathing in certain kinds of damaging dust particles, is a major occupational disease in China. Patients diagnosed with occupational pneumoconiosis can avail of free medical treatment, whereas patients without a diagnosis of occupational diseases cannot not claim free medical treatment in most provinces from the government before 2019. This study aimed to analyze the priority of medical facility selection and its influencing factors among patients with pneumoconiosis. A total of 1,037 patients with pneumoconiosis from nine provinces in China were investigated. The health service institutions most frequently selected by the patients were county-level hospitals (37.5%). The main reason for the choice was these hospitals' close distance to the patients' homes (47.3%). The factors for the choice of health care institutions were living in the eastern region (

BACKGROUND@#The impacts of previous cardio-cerebrovascular disease (pre-CCVD) on the outcomes of hematopoietic cell transplantation (HCT) are not well described. Patients with pre-CCVD may often be poor candidates for HCT. This study aimed to investigate the impact of pre-CCVD on transplant outcomes.@*METHODS@#A retrospective study was conducted between patients with and without pre-CCVD who consecutively received allogeneic or autologous HCT between November 2013 and January 2020 with a matching of age and disease status. The cardiovascular complications and HCT outcomes of the two groups were evaluated and compared. The primary endpoints were post-transplant cardio-cerebrovascular disease (post-CCVD) and non-relapse mortality (NRM). We used a multivariable Cox proportional hazard model and the Fine-Gray competing risk regressions for analyses to estimate the hazard ratios (HRs).@*RESULTS@#The outcomes of 23 HCT recipients with pre-CCVD were compared with those of 107 patients in the control group. No significant differences were noted in terms of engraftment, overall survival (OS) (67.00% vs. 67.90%, P = 0.983), or relapse (29.78% vs. 28.26%, P = 0.561) between the pre-CCVD group and the control group. The cumulative incidences of 2-year NRM were similar between patients with pre-CCVD and the controls (14.68% vs. 17.08%, P = 0.670). However, pre-CCVD was associated with an increased incidence of post-CCVD (HR: 12.50, 95% confidence interval [CI]: 3.88-40.30, P < 0.001), which was an independent risk factor for increased NRM (HR: 10.29, 95% CI: 3.84-27.62, P < 0.001) and inferior OS (HR: 10.29, 95% CI: 3.84-27.62, P < 0.001).@*CONCLUSIONS@#These findings suggest that the existence of pre-CCVD before transplantation might not result in increased mortality directly but superpose the toxicity of the transplantation procedure, leading to a risk of post-CCVD. Post-CCVD was a powerful predictor for high NRM and inferior OS. Further risk stratification of pre-CCVD is needed to reduce NRM in various transplantation settings.

OBJECTIVE@#To observe the effect of acupuncture at "Guanyuan" (CV 4) and "Xiajuxu" (ST 39) on intestinal flora and Toll-like receptors-4 (TLR4) in brain and intestinal tissue in rats with stress gastric ulcer (SGU), and to explore the possible mechanism of acupuncture for SGU.@*METHODS@#Thirty-one male SD rats were randomly divided into a blank group (@*RESULTS@#Compared with the blank group, the gastric mucosal damage index was significantly increased in the model group (@*CONCLUSION@#Acupuncture at "Guanyuan" (CV 4) and "Xiajuxu" (ST 39) could alleviate SGU in rats, and its mechanism may be related to increasing the diversity of intestinal flora, promoting the disorder of intestinal flora to normal, and reducing the overexpression of TLR4 in brain and intestinal tissues.
