ABSTRACT

This study focuses on the microbial quality control of the Chinese herbal decoction pieces. In view of the shortcomings of traditional culture methods such as slow detection speed and inability to detect unculturable microorganisms, a new method based on ATP bioluminescence technology combined with statistical analysis methods was established to rapidly predict and quantitatively detect the total aerobic microbial count (TAMC) and total yeast and mold count (TYMC) contaminated Bupleurum chinense DC. decoction pieces. Based on the optimized ATP bioluminesence detection system, accurate detection of pure bacterial solution of Escherichia coli, Bacillus subtilis and Staphylococcus aureus can be achieved, with detection limits of 47.86, 89.13 and 1 862.09 CFU·mL-1, respectively. The detection time was 6.5 h, and the detection cost was as low as 2 yuan/time. The upper and lower warning limits of TAMC were determined by the misjudgment rates of 10% and 20%, respectively. And the warning limit of TYMC was determined by the misjudgment rate of 20%. The proposed crossing method could quickly predict the amount of microbial contamination in Bupleurum chinense DC. decoction pieces. The constructed partial least squares regression (PLSR) model could accurately quantify the quantity of microbial contamination in Bupleurum chinense DC. decoction pieces. The optimal PLSR prediction model for TAMC had a correction coefficient (R2) of 0.826, a root mean square error of correction set (RMSEE) of 0.468 and a root mean square error of cross-validation set (RMSECV) of 0.465. The R2, RMSEE and RMSECV in the prediction model of TYMC were 0.778, 0.543 and 0.541, respectively. The aim of this study is to establish a kind of rapid detection method and prediction models for the microbial limit of traditional Chinese medicine and Chinese herbal decoction pieces, and to provide a more convenient and sensitive detection technology for the microbial quality process control of traditional Chinese medicine products.

ABSTRACT

ABSTRACT Purpose: To investigate the incidence, risk factors, and visual outcomes of epiretinal membrane development following rhegmatogenous retinal detachment repair. Methods: This was a retrospective study of 309 eyes that underwent initial surgery for primary uncomplicated rhegmatogenous retinal detachment. Examinations were conducted preoperatively and then postoperatively at 1, 3, 6, and 12 months. The study patients were categorized into two groups depending on the presence or absence of the epiretinal membrane. Results: The incidence of postoperative epiretinal membrane was 28.5%; 42.7% of these patients had severe epiretinal membrane development and therefore underwent the epiretinal membrane removal. Logistic regression analyses revealed that giant retinal tears (OR: 2.66; 95% CI: 1.045-6.792, p=0.040) and horseshoe tears (OR: 0.534; 95% CI: 0.295-0.967, p=0.039) were the significant predictors of postoperative epiretinal membrane. Triamcinolone acetonide staining was significantly associated with the prevention of epiretinal membrane (p=0.022). A total of 34 patients showed a better or an equal final best-corrected visual acuity; of which 4 eyes were evaluated at the final follow-up visit and exhibited a reduced best-corrected visual acuity. Conclusion: Our analysis demonstrated that horseshoe tears and giant retinal tears represent the risk factors for the postoperative epiretinal membrane. Triamcinolone acetonide staining had a significant preventive effect on the postoperative epiretinal membrane. Furthermore, a second round of pars plana vitrectomy, including membrane removal, led to a significant improvement in the final best-corrected visual acuity as per the last follow-up examination, albeit the recovery was limited.


RESUMO Objetivos: Investigar a incidência, fatores de risco e desfechos visuais do desenvolvimento da membrana epirretiniana após reparo do descolamento regmatogênico da retina. Métodos: Trata-se de um estudo retrospectivo de 309 olhos submetidos à cirurgia inicial para descolamento regmatogênico da retina primário sem complicações. Os exames foram realizados no pré-operatório aos 1, 3, 6 e 12 meses pós-operatórios. Os pacientes foram divididos em dois grupos, dependendo da presença ou ausência de membrana epirretiniana. Resultados: A incidência de membrana epirretiniana pós-operatória foi de 28,5%; 42,7% desses pacientes apresentaram desenvolvimento grave da membrana epirretiniana e, portanto, foram submetidos à remoção desta membrana. A regressão logística mostrou que as lágrimas retinianas gigantes (RC: 2,66; 95% IC: 1,045 - 6,792, p=0,040) e lágrimas em ferradura (RC: 0,534; 95% IC: 0,295-0,967, p=0,039), foram preditores significativos de membrana epirretiniana pós-operatória. A coloração com acetonida de triancinolona foi significativamente associada à prevenção da membrana epirretiniana (p=0,022). Trinta e quatro pacientes apresentaram acuidade visual melhorada, ou igual, ou acuidade visual final melhor corrigida; 4 olhos foram avaliados na consulta final de acompanhamento e apresentaram redução da acuidade visual melhor corrigida. Conclusão: Nossa análise demonstra que as lágrimas de ferradura e as lágrimas retinianas gigantes representam fatores de risco para a membrana epirretiniana pós-operatória. A coloração com acetonida de triancinolona teve um efeito preventivo significativo na membrana epirretiniana no pós-operatório. Além disso, uma segunda rodada de vitrectomia pars plana, incluindo remoção da membrana, levou a uma melhora significativa da acuidade visual final melhor corrigida na última consulta de acompanhamento, embora a recuperação tenha sido limitada.

OBJECTIVE@#To explore the effect of intermittent pneumatic compression(IPC) combined with 3M thermometer on the prevention of deep venous thrombosis(DVT) in patients with femoral intertrochanteric fracture.@*METHODS@#From March 2016 to August 2019, 127 patients with femoral intertrochanteric fractures who underwent proximal femoral nail antirotation(PFNA) were retrospectively analyzed. They were divided into two groups according to different methods of thrombus prevention and treatment. Among them, 63 patients in group A did not use IPC and 3M thermometer;64 cases in group B were treated with IPC combined with 3M thermometer. Color Doppler ultrasound was used to dynamically monitor the DVT and changes of lower limbs during perioperative period. The venous thrombosis of lower limbs was monitored at 0, 24, 72 h and > 72 h after operation(recheck every 3 days until discharge).@*RESULTS@#Occurrence of DVT of lower limbs after PFNA operation in two groups:there were 5 cases (7.8%) in group B and 20 cases (31.7%) in group A, there was significant difference between two groups (P=0.001). There was no significant difference in lower limb DVT between two groups at 0, 72 and > 72 h after operation(P>0.05), but the formation rate of group A was significantly higher than that of group B at 24 h after operation (P=0.049). There was no significant difference in DVT formation between group A and group B(P>0.05). However, the formation of DVT in group A was significantly higher than that in group B(P=0.012).@*CONCLUSION@#Intraoperative IPC combined with 3M thermostat can effectively prevent DVT of lower limbs in patients undergoing PFNA surgery.

ABSTRACT

Objective: To explore the patterns of cerebral structural abnormalities and cognitive function alterations in end-stage renal disease (ESRD) patients undergoing maintenance hemodialysis, and the underlying correlative factors. Methods: Thirty-seven ESRD patients undergoing maintenance hemodialysis without prior stroke in Shanghai Ninth People's Hospital, Shanghai Jiao Tong University School of Medicine were selected. All patients underwent brain magnetic resonance imaging (MRI) and assessment of cognitive function by using Mini-Mental State Examination (MMSE). Patients were divided into lacunar infarction group (n=33) and non-lacunar infarction group (n=4), or white matter hyperintensities (WMH) group (n=14) and non-WMH group (n=23). The difference of demographic characteristics, past history, blood parameter and dialysis adequacy between patients and their controls were analyzed by t test, Mann-Whitney U test, χ2 test and Fisher exact test. Spearman correlation analysis were performed to explore the relationship between clinical features, cerebral structural abnormalities and cognitive function. Results: Nineteen male and eighteen female patients participated in the study. The mean age was (59.4±12.3) years. The incidences of lacunar infarction and WMH were 89.2% and 37.8%, respectively. 24.3% of the participants were diagnosed as cognitive impairment. Patients with lacunar infarction were elder, who had lower level of hemoglobin, hematocrit, serum albumin and serum total protein, while parathyroid hormone (PTH), erythrocyte sedimentation rate, tumor necrosis factor-α and interleukin-6 were elevated. Patients with WMH were also significantly elder, lower in transferrin saturation and higher in PTH. The differences between the two groups were statistically significant (all P<0.05). Spearman correlation analysis showed MMSE score had a negative correlation with age (r=-0.471, P=0.003) and had positive correlations with education status (r=0.355, P=0.031) and hypertension (r=0.358, P=0.030). The study did not find the relationship among lacunar infarction, WMH and MMSE score; however, recall function was found negatively correlated with lacunar infarction (r=-0.357, P=0.030). Conclusion: ESRD patients undergoing maintenance hemodialysis have a high prevalence of cerebrovascular disease and cognitive impairment. Older age, anemia, inflammation status, chronic kidney disease-mineral and bone disorder may be the influencing factors of the cerebral structural abnormalities and cognition decline.

ABSTRACT

Renal calculus is a common disease of urinary system. Calculus formation is associated with dietary factors, genetics, comorbidities, microorganism environment, modulators in the urine and many other factors. It has become a kind of chronic metabolic condition which needs multidisciplinary treatment. Imaging, metabolic evaluation and calculus analysis provide a variety of tools for calculus diagnosis and evaluation. As the first-line imagine examination recommended by guidelines, CT has advantages in distinct sectional anatomy with high sensitivity and specificity, and dual-energy CT makes it possible to discriminate different calculus compositions noninvasively. The 24-hour urine test can help predict the risk of renal calculus. This article reviews recent research progresses in the risk factors, diagnosis and evaluation of renal calculus.

ABSTRACT

Since December 2019, Wuhan city of Hubei Province has been the center of a number of patients infected with coronavirus disease 2019 (COVID-19). People are susceptible to be infected. The infection has spread to newborn and small infants, and severe cases of children have now occurred. Cases of infection in children have drawn great attention from the society. The pathogenesis of COVID-19 is unclear, and there is no specific drug for treatment, which brings great difficulties to epidemic prevention and clinical treatment. In this paper, we try to understand COVID-19 in children from different perspectives of TCM and western medicine. We also try to analyze the etiology, epidemic characteristics, disease development characteristics and treatment measures of COVID-19 in children. At the same time, we try to explore the etiology, pathogenesis, syndrome differentiation and treatment of COVID-19 in children with TCM. It is hoped that the combination of TCM and western medicine can better understand the new coronavirus pneumonia in children and its pathological basis, analyze the current trend of the epidemic situation, so as to find out safe and effective prevention and control measures and provide reference for clinical prevention and treatment of COVID-19 in children.

ABSTRACT

N-methyladenosine (mA), a ubiquitous RNA modification, is installed by METTL3-METTL14 complex. The structure of the heterodimeric complex between the methyltransferase domains (MTDs) of METTL3 and METTL14 has been previously determined. However, the MTDs alone possess no enzymatic activity. Here we present the solution structure for the zinc finger domain (ZFD) of METTL3, the inclusion of which fulfills the methyltransferase activity of METTL3-METTL14. We show that the ZFD specifically binds to an RNA containing 5'-GGACU-3' consensus sequence, but does not to one without. The ZFD thus serves as the target recognition domain, a structural feature previously shown for DNA methyltransferases, and cooperates with the MTDs of METTL3-METTL14 for catalysis. However, the interaction between the ZFD and the specific RNA is extremely weak, with the binding affinity at several hundred micromolar under physiological conditions. The ZFD contains two CCCH-type zinc fingers connected by an anti-parallel β-sheet. Mutational analysis and NMR titrations have mapped the functional interface to a contiguous surface. As a division of labor, the RNA-binding interface comprises basic residues from zinc finger 1 and hydrophobic residues from β-sheet and zinc finger 2. Further we show that the linker between the ZFD and MTD of METTL3 is flexible but partially folded, which may permit the cooperation between the two domains during catalysis. Together, the structural characterization of METTL3 ZFD paves the way to elucidate the atomic details of the entire process of RNA mA modification.

ABSTRACT

Objective To explore an auxiliary method to improve DWI image quality of testis.Methods Thirty healthy male volunteers were selected,and EPI-DWI and FSE-T2WI scanning with the same parameters were successively performed according to three different schemes.Scheme A:Padding ultrasonic coupling agent-soft rubber bag at the scanning position.Scheme B:Padding physiological saline-soft rubber bag at the scanning position.Scheme C:No preexamination treatment at the scanning position.FSE-T2WI (the same scheme and the same slice) was used as the reference of anatomic form,and the images were conducted by two attending doctors.The image geometric distortion,signal uniformity and anatomical clarity were scored by 5 points method.SNR and CNR of were calculated by one of the two doctors.The scoring consistency between the two doctors was evaluated.The scoring of image geometric distortion,signal uniformity,anatomic clarity,SNR and CNR of the three schemes were statistically analyzed.Results The scoring consistency of image geometrical distortion,signal uniformity and anatomical clarity of the three schemes were relatively good or better between the two doctors (Kappa=0.805,0.737 and 0.713).There were significant differences of image geometrical distortion,signal uniformity and anatomical clarity scoring obtained from three schemes (comparisons between groups and overall comparison,all P< 0.05),superiority:Scheme A> Scheme B> Scheme C.Conclusion Ultrasonic coupling agent-soft rubber bag padding method can significantly improve DWI imaging quality of testis.

ABSTRACT

Objective · To investigate the regulatory effects of inflammatory signaling pathway on the expression of G protein-coupled receptor class C group 5 member A (GPRC5A).Methods· Nuclear factor κB (NF-κB)-driven luciferase mice were intraperitoneally injected with lipopolysaccharide (LPS) to evaluate the activation of NF-κB in lungs.GPRC5A expression in lungs was assessed by Western blotting in the C57BL/6J mice injected with LPS.In vitro tests,human lung tumor cell lines Calu-1 and H322,and human embryonic kidney cell line HEK293T were administered with tumor nccrosis factor α (TNF-α) or transfected with p65 expression plasmid;Western blotting,RT-PCR,luciferase reporter gene experiment and immunofluorescence assay were used to analyze the effect of inflammation on GPRC5A expression.Results · Intraperitoneal injection of LPS induced activation of NF-κB pathway in lung tissues,which suppressed the expression of GPRC5A in mice lungs.In Calu-1 cells,TNF-α treatment greatly suppressed the expression of GPRC5A protein and mRNA.In HEK293T cells,transfection of p65 subunit of NF-κB suppressed the expression of GPRC5A promoter-driven luciferase reporter.The H322 cells transfected with green fluorescent protein-p65 almost did not express GPRC5A.Conclusion · NF-κB pathway acts at the promoter of GPRC5A and suppresses its mRNA and protein expression.

Objective To study the incidence of stroke among elderly people in China's longevity area and its association with diseases such as hypertension,diabetes and heart disease.The differences in the following common hematological indicators in subjects with stroke and non-hypertension,diabetes,heart disease and stroke were studied:superoxide dismutase (SOD),malondialdehyde (MDA),hypersensitive c-reactive protein (hsCRP),albumin (propagated) glucose (GLU),cholesterol (CHO),triglyceride (TG),high-density lipoprotein cholesterol (HDLC),glycosylated serum protein (GSP) urea nitrogen (BUN),creatinine (CREA) and uric acid (UA).Methods Residents who participated in the project of biomedical research of aging population conducted in 2014 were selected from 8 longevity Areas in China.2 315 people aged 40 and over attended the study,including 22 aged 40 and over,238 aged 60 and over,490 aged 70 and over,629 aged 80 and over,518 aged 90 and over,418 aged 100 and over.Using the self-designed questionnaire to collect information about the characteristics of social demographics,the clinical doctors used the unified inspection tool to examine the subjects.The fasting blood samples were collected by vacuum tube at early morning.The contents of plasma SOD,MDA,hsCRP,ALB,GLU,CHO,TG,HDLC,GSP,BUN,CREA and UA were detected and compared among these elderly who were classified into different genders and different age groups and different healthy groups.Results The prevalence of high blood pressure,diabetes,heart disease and stroke increased with age,reaching a peak and then slowly decreasing.The age of peak was 90 ~ 99,60 ~ 69,70~ 79 and 80~ 89.The prevalence of hypertension was 71.62 % and 60.54 % respectively for stroke subjects and non-cerebral apoplexy subjects,and the difference was statistically significant.The prevalence of diabetes was 18.92% and 11.35% respectively,and the difference was statistically significant.The prevalence of heart disease was 20.98% and 5.26%,respectively,and the difference was statistically significant.The rates of non-hypertension,non diabetic and non-heart disease were 4.73% and 33.41% respectively,and the difference was statistically significant.In the groups of Stroke subjects and Non-high blood pressure,nomdiabetic,non-heart disease subjects the following indicators were Compared,values of SOD were 55.76±8.27 and 57.16±8.00 U/ml respectively,the difference between groups were not statistically significant (t=0.341,P=0.053),values of MDA were 5.81 ± 3.82 and 5.67± 3.16 μmol/L respectively,the difference between groups were not statistically significant (t=0.329,P =0.661),values of hsCRP were 4.15 ± 12.33 and 2.94 ± 6.25 mg/L,respectively,the difference between groups were not statistically significant (t=0.026,P=0.080),values of ALB were 41.60±4.51 and 42.08±3.94 g/L respectively,the difference between groups were not statistically significant (t=0.032,P=0.194),values of ALB were 41.60 ± 4.51 and 42.08± 3.94 g/L respectively,the difference between groups were not statistically significant (t =0.032,P=0.194),values of ALB were 41.60 ± 4.51 and 42.08 ± 3.94 g/L respectively,the difference between groups were not statistically significant (t =0.032,P=0.194),values of GLU were 5.89 ± 2.67 and 4.90 ± 0.90 mmol/L respectively,the difference between groups showed statistically significant (t=0.000,P=0.000)),values of CHO were 4.81 ± 1.00 and 4.71±1.02 mmol/L respectively,the difference between groups were not statistically significant (t =0.670,P=0.318),values of TG were1.33±0.69 and 1.14±0.57 mmol/L respectively,the difference between groups showed statistically significant (t=0.012,P=0.000),values of HDLC were 1.29±0.35 and 1.41±0.40 mmol/L respectively,the difference between groups showed statistically significant (t=0.004,P=0.001),values of GSP were 259.10±60.90 and 246.75±24.52 μmol/L respectively,the difference between groups showed statistically significant (t =0.000,P =0.000),values of BUN were 6.84±± 3.53 and 6.62 ± 2.20 mmol/L respectively,the difference between groups were not statistically significant (t=0.110,P=0.338),values of CREA were 84.92 ± 33.00 and 80.14 ± 24.64 μmol/L respectively,the difference between groups showed statistically significant (t=0.013,P=0.044),values of UA were 296.73±91.34 and 288.12±80.47 μmol/L respectively,the difference between groups were not statistically significant (t=0.123,P=0.247).Conclusion Diabetes,hypertension,and heart disease are risk factors for stroke.Abnormal blood glucose and lipid metabolism:the increase of GLU,TG and the decrease of HDLC are important common biochemical index of stroke.Patients with cerebral apoplexy have certain renal impairment.

ABSTRACT

Objective To obtain comprehensive expression profile of microRNAs(miRNAs)in endometrial cancer stem cells(ECSCs)during differentiation.Methods ECSCs were enriched by spheroid formation and spontaneously differentiated into cancer cells by attached culture.We characterized miRNA expression in ECSCs and its differentiated cells using miRNA microarrays and verified the results by RT-qPCR.Meanwhile,we applied bioinformatic method to predict the target genes of the differentially expressed miRNAs.Furthermore,gene expression profile was detected by genomic microarray.Finally,we summarized the relationship between the predicted target genes of miRNAs and gene expression profile.Results We observed that 10 miRNAs were significantly upregulated in ECSCs,including miR-522,miR-139-3p,miR-520c-5p,miR-518d-5p,miR-146b-5p,miR-34a,miR-526a,miR-193a-3p,miR-221,and miR-4674.Only 1 miRNA,miR-760,was downregulated in ECSCs.A total of 11 differentially expressed miRNAs were identified by RT-qPCR.The results showed these differentially expressed miRNAs(except miR-526a and miR-4674)were in accordance with those obtained by miRNA microarray analysis. By using bioinformatic approach,the target genes of these differentially expressed miRNAs could be found.The results of genomic microarray showed that 207 genes were more highly expressed and 238 genes were more lowly expressed in ECSCs compared with its differentiated cells.Analysis of the gene expression profile and the predicted target genes of miRNAs showed that some common genes except miR-139-3p could be found.Conclusion Our study provides an available information about miRNAs and target genes from different starting points,such as ECSCs differentiation.Further studies are needed to ascertain the role of these miRNAs in ECSCs'differentiation.

ABSTRACT

To assess the schistosomiasis transmission risk after flood damage in Wuhan City.Schistosomiasis epidemic villages in Wuhan City were selected by using probability proportional to size sampling (PPS). The field investigations of Oncomelania hupensis snails, the activity of people and cattle on marshland, water infectivity of schistosome, reservoir host infection and wild feces contamination were conducted. I–III levels of risk environments were assessed and treated with appropriate measures.A total of 90 schistosomiasis epidemic villages and 170 environments were monitored. Totally 9 811 snails were dissected but no Schistosoma japonicum infected snails were found. There were significant decreases in the density of snails and the survival rate of living snails after the flood (χ2 = 102.517, t = 4.724, both P < 0.01). Totally 289 pieces of wild feces were captured, and no eggs of S. japonicum were detected. A total of 11 surveillance and forecast sites were detected on water systems, and 221 sentinel mice were placed. After breeding, 219 sentinel mice survived (99.10%), and no schistosome infection was detected. In addition, 1 720 mouse traps were placed in 3 survey sites, and 66 Apodemus agrarius were captured, and no schistosome-infected ones were found. All of the environments were assessed as Grade III. Meanwhile, 5 key schistosomiasis control areas were determined, and no emergency happened after the implementation of comprehensive control interventions.The risk of schistosomiasis transmission remains low in Wuhan City after the flood, but the potential epidemic risk cannot be ignored.

BACKGROUND:Bio-Oss bone substitutes have been shown to prevent bone resorption by slowing bone resorption,reducing bone resorption,and restoring the height of the alveolar ridge.However,most clinical studies focus on the efficacy within 1-3 months.OBJECTIVE:To compare the effects of two kinds of bone substitutes on the height of alveolar ridge and the periodontal tissue health status after implantation into the extraction socket of the wisdom tooth.METHODS:Forty patients with impacted mandibular teeth were randomly divided into two groups:Bio-Oss Collagen material was implanted in experimental group (n=20),and Bio-Oss bone replacement materials in control group (n=20).Cone-beam CT was retrospectively reviewed at 1,3 and 12 months after implantation.Alveolar bone height,bone changes and the second molars mobility were examined.Meanwhile,healing status of tooth extraction and gingival mucosa as well as the material spills were observed.RESULTS AND CONCLUSION:(1) At 1 month after implantation,the gingival mucosa of both groups healed well without swelling.There was no spillover in the experimental group,while there were two cases of bone material spill in the control group.No tissues generated in the extraction socket and tooth loosening was observed in both groups.(2) At 3 months after implantation,the materials gradually degraded with no swelling and with generation of partial bone tissues in the experimental group,and the tooth mobility was improved.In the control group,there was also no swelling in the gums,and the materials degraded partially,but there was no presence of new bone and no improvement in the tooth mobility.(3) At 12 months after implantation,there was no swelling in the gums of the experimental group,obvious new bone tissues formed in the second molar with the presence of bone trabecula.The new bone tissues were integrated with the surrounding bone tissues.The crest of the alveolar ridge was located about 3 mm below the enamel cementum,the tooth mobility was restored to the level before extraction,and the chewing function recovered.In the control group,there was no swelling in the gums,and new bone tissues formed in the distal part of the molar,but the trabecular arrangement was not aligned.The crest of the alveolar was located about 3 mm below the enamel cementum,the tooth mobility returned to the pre-extraction level,and the chewing function was normal.(4) The height of the alveolar bone at 1 and 3 months after implantation was significantly higher in the experimental group than the control group (P < 0.01),while there was no significant difference between the two groups at 12 months after implantation.To conclude,implantation of Bio-Oss Collagen immediately after removal of the impacted wisdom tooth of the mandible is beneficial to alveolar bone height and bone quality recovery,and significantly improves the periodontal health of the second molar,but the long-term effect needs further observation.

ABSTRACT

<p><b>OBJECTIVE</b>To evaluate the clinical effects of French door segmented laminectomy decompression for severe cervical OPLL complicated with spinal cord injury.</p><p><b>METHODS</b>The clinical data of 38 patients with serious cervical OPLL complicated with spinal cord injury were retrospectively analyzed and these patients were treated with French door segmented laminectomy decompression and internal fixation from June 2012 to June 2014. There were 25 males and 13 females, aged from 42 to 78 years with an average of 58.2 years. Of them, 35 cases suffered from aggravating neurological symptoms with a definite precipitating factor. Spinal cord injury was related to minor injury of the neck, such as hyperextension of the neck in 3 cases. Preoperative Japanese Orthopaedic Score (JOA) was 8.1±1.7 and Neck Disability Index (NDI) was 19.8±4.4. Preoperative CT scans showed the range of OPLL was more than three segments. The spinal canal was occupied 50% to 85% with an average of 70.7%.</p><p><b>RESULTS</b>All the patients were followed up for 10 to 24 months with an average of 15.6 months. The operative time was 90 to 150 min with an average of 120 min and blood loss was 300 to 800 ml with an average of (480±80) ml. At final follow-up, NDI and JOA were 7.5±2.5 and 13.5±2.0, respectively, and they were obviously improved compared with preoperation. Preoperative cervical Cobb angle was (8.10±2.70)° and at final follow-up was (15.60±1.80)°, and there was significant difference between preoperative and postoperative (<0.05). Deep infection occurred in 1 case, epidural hematoma in 1 case, C₅ nerve root palsy in 3 cases, and axial symptom in 8 cases after operation. No serious complications, such as vertebral artery injury, cerebrospinal fluid leakage, deterioration of neurological dysfunction, or internal fixation failure was found.</p><p><b>CONCLUSIONS</b>French door segmented laminectomy decompression is safe and feasible for severe cervical OPLL complicated with spinal cord injury, and it is worth to be popularized in future.</p>

BACKGROUND:The roles of osteocyte and osteoblast apoptosis in steroid-induced avascular necrosis of the femoral head (ANFH) have arouse much attention,and its pathogenesis has been understood gradually.But there is a lack of knowledge about the mechanisms underlying osteocyte and osteoblast apoptosis.Meanwhile,hormones have been shown to enhance the Dickkopf-1 expression in Wnt signaling pathway.OBJECTIVE:To investigate the roles of Dickkopf-1 and cell apoptosis in steroid-induced ANFH,and to understand their correlations with steroid-induced ANFH.METHODS:Necrotic femoral head samples were removed from 14 patients with steroid-induced ANFH after total hip arthroplasty (experimental group),and normal femoral heads were from 8 patients with femoral neck fracture (control group).The cellular morphology was observed using transmission electron microscope;the number of empty lacunae was counted through hematoxylin-eosin staining;the cell apoptosis was detected by TUNEL assay;the expression level of Dickkopf-1 was detected by immunohistochemistry.Moreover,the correlation between the Dickkopf-1 positive rate and apoptosis index of osteocytes was analyzed by Spearman rank correlation analysis.RESULTS AND CONCLUSION:The number of empty lacunae,expression level of Dickkopf-1,and apoptosis index of osteocytes in the experimental group were significantly higher than those in the control group (P < 0.05).In the experimental group,the Dickkopf-1 positive rate was positively correlated to the apoptosis index of osteocytes (r=0.623).These results indicate that Dickkopf-1 and osteocyte apoptosis both play key roles in steroid-induced ANFH,and increased Dickkopf-1 level may promote the development of ANFH.

Objective @# To investigate the clinical effects of micro implant anchorage nail and Ni-Ti coilspring to lower the elongation molar. @*Methods@#60 patients with the first maxillary molar elongation were divided into 2 groups. 30 of them were treated with micro implant anchorage nail and Ni-Ticoil spring to lower the molar (observation group), and the other 30 patients were treated with segmental arch to lower the molar (control group). The average lowering time, depth and pain degree of the 2 groups were compared. @*Results@#The average lowering time was faster and the overage lowering depth was greater in the observation group than that in the control group; the slight pain rate in the observation group was significantly higher than that in the control group (P<0.05), while the ratio of moderate pain and severe pain was significantly lower than that in the control group (P<0.05).@*Conclusion@# To lower the elongation molar with micro implant anchorage nail and Ni-Ti coilspring might have shorter course and less pain.

ABSTRACT

<p><b>OBJECTIVE</b>To explore the association between preoperative platelet count and the outcomes of patients with colorectal cancer (CRC).</p><p><b>METHODS</b>This study was conducted among a cohort of 486 CRC patients, who underwent surgery in Sichuan Provincial Cancer Hospital between January, 2010 and July, 2013 and were prospectively followed up for their outcomes. The association between preoperative platelet counts and clinicopathologic factors of the patients were analyzed. Survival analysis of the patients was performed using log-rank test, and the factors affecting the patients' outcomes were analyzed by univariate and multivariate analyses using the Cox proportional hazard model.</p><p><b>RESULTS</b>In this cohort, preoperative platelet count was significantly associated with the tumor site, depth of tumor invasion (T), and distant metastasis (M) (all P<0.05). Log-rank tests showed that in patients with CRC and rectal cancer, the overall postoperative survival differed significantly between high and low preoperative platelet count groups (Χ(2)=8.813, P=0.003 and Χ(2)=5.110, P=0.024, respectively), but this difference was not observed in patients with colon cancer (P<0.05). Multivariate analysis indicated that CRC patients with a high preoperative platelet count had a higher risk of death compared to those with a low platelet level after adjustment for tumor site, tumor grade, TNM stage, vascular invasion, perineural invasion, and preoperative CEA level (RR=1.814, 95%CI: 1.056-3.115). In subgroup analysis, preoperative platelet count was identified as an independent prognostic factor in patients with rectal cancer (RR=2.718, 95% CI: 1.132-6.526), but not in patients with colon cancer (RR=1.396, 95%CI: 0.705-2.765).</p><p><b>CONCLUSION</b>As an independent prognostic factor in CRC patients, preoperative platelet count may serve as an important indicator for predicting the outcomes of rectal cancer, but its prognostic value for colon cancer needs further clarification.</p>

<p><b>OBJECTIVE</b>To investigate the relationship between mRNA expression of manganese superoxide dismutase (MnSOD) and manganese neurotoxicity.</p><p><b>METHODS</b>Thirty-one patients with occupational chronic manganese poisoning (case group), as well as 31 controls exposed to the same condition (control group), were included in the study. Whole blood RNA was extracted, and the mRNA expression of MnSOD was measured by RT-PCR; the two groups were compared in terms of the mRNA expression of MnSOD. PC12 cells were treated with 0, 100, 200, 400, 600, 800, and 1000 ümol/L MnCl₂ for l, 2, 3, and 4 d; the cell viability was determined by MTT assay, and the mRNA expression of MnSOD was measured by RT-PCR.</p><p><b>RESULTS</b>The case group had significantly lower mRNA expression of MnSOD than the control group (0.390 ± 0.080 vs 0.582 ± 0.219, P < 0.05). MnCl2 had a toxic effect on PC12 cells; the concentration of MnCl₂ was positively correlated with the toxic effect but negatively correlated with the mRNA expression of MnSOD.</p><p><b>CONCLUSION</b>MnSOD mRNA may be involved in the manganese-induced damage of nerve cells. It is hypothesized that high mRNA expression of MnSOD may play an inhibitory effect on manganese neurotoxicity.</p>

ABSTRACT

<p><b>UNLABELLED</b>objective: To investigate effects of buyang huanwu decoction (BYHWD) on the rats' myocardial hypertrophic model induced by abdominal aortic constriction, and to clarify the molecular regulatory mechanisms for sarcoplasmic reticulum calcium uptake.</p><p><b>METHODS</b>Hypertrophic myocardium rat model was induced by abdominal aorta constriction (AAC). Four weeks after modeling, rats were randomly divided into the sham-operation group (Group S), the AAC model group (Group M), the Enalapril group (Group E), and the BYHWD treatment group (Group BYHWD), respectively. The left ventricular systolic pressure (LVSP), left ventricular end-diastolic pressure (LVEDP), + dp/dtmax,-dp/dtmax, cardiac output (CO), heart mass index (HMI), and left ventricular mass index (LVMI) were observed in each group after 12-week medication. The serum contents of the atrial natriuretic peptide (ANP) and brain natriuretic peptide (BNP) were detected using ELISA. The SERCA2a activity, the ex- pressions of SERCA2a, phospholamban (PLN), and PLN phosphorylation were observed finally.</p><p><b>RESULTS</b>Compared with Group S, LVSP and LVEDP significantly increased,-dp/dtmax and CO obviously decreased, the myocardial tissue was obviously thickened, the serum contents of ANP and BNP increased, the activity and expression of SERCA2a decreased, the SERCA2a/PLN ratio and PLN phosphorylation degree decreased in Group M (all P <0.05). Compared with Group M, LVEDP obviously decreased, -dp/dtmax and CO obviously increased, the hypertrophy myocardial tissue was obviously lessened, the serum contents of ANP and BNP decreased, the activity of SERCA2a increased, the relative expression contents of SERCA2a, Ser16, and Thrl7 were elevated in Group BYHWD (all P <0.05). BYHWD had significant roles in elevating the SERCA2a/PLN ratio and PLN phosphorylation degree (P <0.05).</p><p><b>CONCLUSION</b>BYHWD could significantly improve hemodynamics of heart failure rats, elevate CO, lessen cardiac hypertrophy, and improve the capabilities for sarcoplasmic reticulum calcium uptake.</p>
