ABSTRACT

Objective: Diabetic retinopathy (DR) is the retinal consequence of chronic progressive diabetic microvascular leakage and occlusion. Non-proliferating diabetic retinopathy (NPDR) is the early stage of DR. It eventually occurs to some degree in all patients with diabetes mellitus. In recent years, many clinical trials have shown that Compound Danshen Dripping Pill (CDDP) may be associated with the improvement of NPDR symptoms. The aim of this study was to quantitatively summarize the association between CDDP and the therapeutic effects of NPDR. Methods: It was conducted that a systematic literature search of PubMed, Web of Science, CNKI, VIP and Wanfang Data updated in June 2020 with the following search terms: “diabetic retinopathy” or “retinopathy” or “DR” or “NPDR”, in combination with “Compound Danshen Dripping Pill” or “Salvia miltiorrhiza” or “Danshen”. Risk ratio (RR) and weighted mean difference (WMD) with their 95% confidence interval (CI) was calculated between treatment and control groups. The sensitivity analyses were undertaken by removing each individual study when high heterogeneity appeared. Subgroup analysis, Meta-regression, and publication bias analysis were also conducted. The strength of evidence was evaluated with the Grading of Recommendation, Assessment, Development, and Evaluation (GRADE) method. Results: Twenty-six RCTs involving 2047 subjects were included to conduct a Meta-analysis after screening the studies, extracting the data, and assessing the study quality. The Stata15.0 software was utilized for processing. Meta-analysis indicated that curative effects of treatment group with CDDP was significantly better than control [RR = 0.54, 95% CI (0.40, 0.73); moderate-quality evidence]. In addition, the results showed that CDDP was significantly associated with improving retinal hemorrhages [WMD = −0.62, 95% CI (−0.78, −0.46); low-quality evidence], the vision [WMD = 0.14, and 95% CI (0.09, 0.19), low-quality evidence], fundus fluorescence angiography [RR = 0.37 and 95% CI (0.23, 0.60); low-quality evidence], reduction of retinal microaneurysm [WMD = −3.74 and 95% CI (−4.38, −3.11); moderate-quality evidence], hemangioma volume [WMD = −3.15, 95%CI (−3.45, −2.85); moderate-quality evidence], macular thickness [WMD = −5.52, 95%CI = (−64.27, −48.78); low-quality evidence], mean defect [WMD = −1.65 and 95% CI (−1.95, −1.34); very low-quality evidence], fasting blooding glucose [WMD = −0.95, 95% CI (−1.19, −0.70); low-quality evidence), hemoglobin A1c [WMD = −0.62, 95% CI (−0.93, −0.30); low-quality evidence], high sensitive C reaction protein [WMD = −5.66, 95% CI (−8.01, −3.31); low-quality evidence]. Sensitivity, subgroup, and Meta-regression analyses were also assessed. Conclusion: The study demonstrated that CDDP has beneficial clinical effects for treating NPDR and improve the vision. Moreover, it indicated that oral CDDP in NPDR patients led to significant regulation of serum level of fasting blooding glucose, hemoglobin A1c and high sensitive C reaction protein, which was associated with the pathogenesis of NPDR. However, high-quality and large randomized clinical trials will be needed to prove the consequence in future.

ABSTRACT

Objective To systematically evaluate the effects of psychiatric nurse-led adherence therapy on clinical outcomes and medication adherence in schizophrenia patients.Methods Searched databases to collect randomized controlled trials about psychiatric nurse-led adherence therapy for patients with schizophrenia.Two reviewers independently screened literature,extracted data,and assessed the risk of bias of included studies.Then,meta-analysis was performed using Rev Man 5.2 software.Results Fifteen studies were recruited,including 683 cases of AT and 681 cases of TAU.The results showed that adherence therapy could improve attitude towards adherence,adherence behaviors,psychotic symptoms,function,insight and reduce hospital stay.But regarding stigma,quality of life and side effects,adherence therapy did not show benefits (P>0.05).Conclusion Psychiatric nurse-led adherence therapy is effective to improve schizophrenia patients' adherence,psychotic symptoms,function,insight and reduce hospital stay.

ABSTRACT

Objective To investigate the value of nasal continuous positive airway pressure (NCPAP) in the treatment of severe pneumonia. Methods The clinical data of 59 severe pneumonia infants who received NCPAP preferential ventilation strategy were retrospectively analyzed. Results After treatment, 50 infants were effective and 9 infants were ineffective. Before treatment, the age, RR, HR between effective group and ineffective group had no significant differences (P>0.05). But the levels of PaCO2, PaO2/FiO2 in ineffective group were significantly higher than those in effective group and the level of PaO2 in ineffective group was significantly lower than that in effective group (P<0.05). In effective group, the levels of PaO2 and PaO2/FiO2 were significantly increased after treatment for 1, 12 and 24 h, and the levels of PaCO2, RR and HR were significantly decreased (P<0.01). Conclusions NCPAP preferential ventilation strategy can reduce the use of invasive mechanical ventilation, improve oxygenation, ease dyspnea and make vital signs stable.

ABSTRACT

Objective To establish a quantitative model to rapidly determine content of 3-butylphthalide in Angelicae Sinensis Radix based on near-infrared diffuse reflectance spectroscopy (NIR). Methods Totally 145 batches of samples of Angelicae Sinensis Radix from 14 areas of Gansu and Yunnan Provinces were collected and dried in the shade. With the content of 3-butylphthalide determined by RP-HPLC-DAD as reference, the NIR spectra of 145 batches of Angelicae Sinensis Radix were collected by integrating sphere diffuse reflectance, and the PLS (partial least squares) quantitative analysis model was established between 7415-4056 cm-1. Results The Rc (correlation coefficient between actual content and predicted content), RMSEC (the root-mean-square error of calibration), RMSEP (the root-mean-square error of prediction), Rcv (cross-validation correlation coefficient), RMSECV (the root-mean-square error of cross-validation), and average predicted recovery reached 0.976 7, 0.614 3, 0.706 5, 0.897 8, 1.155 4 and 102.08%, respectively. Conclusion This prediction model was accurate, reliable, and can be used to quantitatively determine the content of 3-butylphthalide in Angelicae Sinensis Radix.

ABSTRACT

Objective To explore the predictive value of serum CEA and cytokeratin-19 fragments (CYFRA21-1)prior treatment for the epidermal growth factor receptor (EGFR) mutation and efficacy of tyrosine kinase inhibitors ( TKI ) in patients with non-small cell lung cancer.Methods The study was a clinical research.Totally 101 matched tissue and plasma samples were collected from Peking University People′s Hospital from 2012 to 2013.All clinical specimens were analyzed for EGFR mutations in exons of 18, 19, 20 and 21 by ADx-ARMS and direct sequencing, and the serum levels of CEA and CYFRA21-1 were analyzed by ECLI.The correlation between EGFR mutant status and efficacy of EGFR-TKI and clinicopathological parameters were analyzed by χ2 test, Log-rank text and Cox proportional hazards regression model.Results The mutation rate was 60.4%(61/101) by ADx-ARMS and 33.7%(34/101) by direct sequencing.Mutations were more frequently observed in the higher serum CEA level patients(≥5μg/L,78.8%).However, the rates of EGFR mutations of different CEA levels were similar.Among the patients receiving TKI therapy, the efficacy of EGFR-TKI was closely related to serum CYFRA21-1 level prior treatment and EGFR mutation (χ2 =8.903, P =0.003; χ2 =28.590, P <0.001 ).And serum CYFRA21-1 level prior treatment and EGFR mutation were independent factors for EGFR-TKI treatment affecting PFS (RR=0.298, P<0.001;RR=0.086, P<0.001).Conclusion The mutation rate of EGFR was significantly related with the expression level of CEA prior treatment, and serum CEA and CYFRA21-1 levels prior treatment could be potential predictors of EGFR-TKI efficacy.

ABSTRACT

Objective To investigate the correlation between mineral elements in soil and quality of Angelica sinensis. Methods The contents of five index chemical compositions, volatile oil, ethanol soluble extractive in fifty-six batches of Angelica sinensis from twelve habitats, and fifteen kinds of mineral elements in corresponding soil were determined. Pearson correlation analysis, partial correlation analysis and stepwise regression analysis were used. Results The results showed that there were significant(P<0.05) or very significant correlation (P<0.01) between many kinds of mineral elements in soil and quality constituents of Angelica sinensis. Mg, Na, Cr and Hg were the dominant factor of ferulic acid; K, Zn, Ni, Mn, Na, Pb, Cu and Sb were the dominant factor of 3-butylphthalide;K, Zn, Ni, Na, Cd, Cr and Hg were the dominant factors of Z-butylidenephthalide; Fe, Ca, Zn, Ni, Mn, Pb and Cu were the dominant factor of Z-ligustilide; Zn, Mn, As and Cu were the dominant factors of linolic acid; Fe, K, Ca, Zn, Mn, Na, Pb, Cd, Cr, Cu, Sb and Hg were the dominant factors of volatile oil; Fe, K, Mg, Ni, Mn, Pb, As, Cu and Sb were the dominant factors of ethanol soluble extractive. Conclusion Mineral elements in soil have an important and stable influence on the quality formation of Angelica sinensis. K, Zn, Ni, Mn, Pb, Cu, Hg, Fe, Sb, Cr and Na are the dominant factors of several quality constituents of Angelica sinensis at the same time, which should be selectively examined during planting base selection and cultivation.

ABSTRACT

Objective To explore the influence of the medication adherence and family function on mental disability status of the schizophrenia patients.Methods Morisky Medication Adherence Questionnaire(MAQ),Assessment Schedule Ⅱ (WHO-DAS Ⅱ),Family APGAR Index(APGAR) were used to analyze the influence of the medication adherence and family function on the schizophrenia mental disability patients.120 convalescence mental disability patients were included in Inner Mongolia Anding hospital.Results The mean score of medication adherence and family function of the schizophrenia mental disability patients was (2.40±1.10) points and (4.87±2.42) points,they both played a significant role on the effect of mental disability.The main effect of medication adherence and family function were significantly different.Through the simple effect analysis,the mental disability score was different in the various medication adherence level under the various family function situation,and the mental disability score was also different in the various family function situation under the medication adherence level.Conclusions Both medication adherence and family function have effect on mental disability status of the schizophrenia patients.The medical staff should take targeted measures to delay or improve patients' mental disability status.

ABSTRACT

Objective In view of the schizophrenia risk factors during the recovery,to explore a supportive intervention program suitable for the rehabilitative schizophrenic patients.Methods Totally 64 rehabilitative schizophrenic patients were divided into the experimental group and the control group with 32 cases in each group.All the patients received the same treatment with antipsychotic drugs.The patients in the control group received the routine health education.In addition,the patients in the experimental group received supportive intervention for 4 months.The patients were assessed by IBS,APGAR,GSES and SDSS before and after intervention.Results Main effect of group factor and time factor of GSES,SDSS,APGAR and IBS had statistical significances.There were significant interaction in the scores of SDSS,APGAR and IBS.Simple effect analysis demonstrated the scores of GSES,SDSS,APGAR and IBS in the experimental group were higher than those of the control group after the intervention.Except the score of GSES,there was no differences in other scales before and after the intervention of the control group.Conclusions Supportive intervention program for the rehabilitative schizophrenic patients have good practice guidance.It can improve the patients' self-efficacy,family function and social function,also can correct irrational beliefs and to help patients better adapt to society.

ABSTRACT

Objective To investigate the correlation between ACE gene polymorphisms and stroke of Han nationality people in Fangshan district of Beijing. Methods The Insertion/Deletion(ID) polymorphisms of ACE gene were detected in 63 patients with cerebral hemorrhage,and 713 patients with cerebral infarction and 235 health control by polymerase chain reaction(PCR). We observed the frequencies of genotype of deletion homozygote(DD),insertion homozygote(II) and insertion/deletion heterozygote (ID) and the alleles of D and I. Also we analyzed the association among I/D polymorphisms of ACE gene with serum glucose(GLU),triglyceride(TG),cholesterol(TC) levels. Results There was no significant difference in the frequencies of both genotypes of DD,ID,II and alleles of D and I in three groups. ABSTRACT

Objective To study the internalization mediated by heat shock protein 70 (HSP70) receptors expressed on the surface of dendritic cells (DCs). Methods DCs from C57BL/6 mouse bone marrow were divided into HSP70 receptor-blocking group, non-receptor-blocking group and control group. The HSP70-FITC positive cells were detected by flow cytometry (FCM). Results FCM indicated that in HSP70 receptor-blocking group, the HSP70 receptors could be almost totally blocked by 50?g HSP70. With the increase of HSP70-FITC dosage, the number of HSP70-FITC positive cells increased. In non-receptor-blocking group, the rates of the HSP70-FITC positive cells were up to 90% either when 50?g HSP70-FITC or 100?g HSP70-FITC was used with no remarkable difference between them. Conclusions The internalization mediated by HSP70 receptors plays a major role in the stage of ingestion of HSP70 with the characteristics of saturation and priority. Once the receptors are blocked, non-receptor pathway participates in internalization in a dose-dependent manner.

ABSTRACT

Objective To study the impact of viral infection on stress protein gene expression and investigate the possibility that heat shock proteins interact with viral components during viral assembly in the brain of suckling mice experimentally infected with Hantaan virus (HV). Methods New born mice less than 72 h of age were inoculated into brain with the 100 LD50 of Hantaan virus (strain 76 118). At the 8th day mice died from brain disease. Brains were prepared for tissue sections and extracts for the experiments. The Hantaan virus infection, HSP70 induction and their co localization in brain of infected suckling mice were demonstrated by immunohistochemical staining and laser scan co focal microscopic double labling. The possible intereaction between HV structural proteins and HSP70 was further analyzed by immunoprecipitation of infected tissue extracts with antibodies to either HSP70 or to viral nuclear capsule proteins HV NP by using SDS PAGE and Western blotting. Results HV infection induced an increased levels of HSP70 expression at the cytoplasma of neurons in the brain of suckling mice HV NP protein proved to be co localized with HSP70. Monoclonal antibody (mAb) to HSP70 could co immunoprecipitate the HV NP, and similarly mAb to HV NP could co immunoprecipitate HSP70 from virus infected tissue extracts, but not from mock infected tissue extracts. Antibody, which recognized one of these two proteins, did not cross react with the other. Thus, the association between HSP70 and HV NP appeared to be specific.Conclusions Hantavirus infection can directly induce HSP70 expression, leading to the formation of HV HSP 70 viral antigen peptide complex which plays some roles in the course of viral infection and replication.
