ABSTRACT
c-Myc protein encoded by c-Myc (cellular-myelocytomatosis viral oncogene) gene regulates the related gene expression through the Wnt/β-catenin signaling pathway, and has received extensive attention in recent years. The purpose of this study was to express Helicoverpa armigera c-Myc gene (Ha-c-Myc) by using prokaryotic expression system, prepare the polyclonal antibody, examine the spatio-temporal expression profile of Ha-c-Myc, and investigate the possible function of Ha-c-Myc in regulating H. armigera sterol carrier protein-2 (SCP-2) gene expression. The Ha-c-Myc gene was amplified by PCR and cloned into a prokaryotic expression plasmid pET-32a(+). The recombinant plasmid pET-32a-Ha-c-Myc was transformed into Escherichia coli BL21. IPTG was used to induce the expression of the recombinant protein. Protein was purified by Ni2+-NTA column and used to immunize New Zealand rabbits for preparing the polyclonal antibody. The Ha-c-Myc expression levels in different developmental stages (egg, larva, prepupa, pupa, and adult) of H. armigera and different tissues (midgut, fat body, head, and epidermis) of the prepupa were determined by real-time quantitative reverse transcription PCR (qRT-PCR). Ha-c-Myc siRNA was synthesized and transfected into H. armigera Ha cells. The relative mRNA levels of Ha-c-Myc and HaSCP-2 in Ha cells were detected by qRT-PCR. Results showed that the pET-32a-Ha-c-Myc recombinant plasmid was constructed. The soluble Ha-c-Myc protein of about 65 kDa was expressed in E. coli. The polyclonal antibody was prepared. Western blotting analysis suggested that the antibody had high specificity. Enzyme linked immunosorbent assay (ELISA) showed that the titer of the antibody was high. Ha-c-Myc gene expressed at all developmental stages, with high levels in the early and late instars of larva, and the prepupal stage. Tissue expression profiles revealed that Ha-c-Myc expressed in various tissues of prepupa, with high expression level in the midgut, but low levels in the epidermis and fat body. RNAi results showed that the knockdown of Ha-c-Myc expression significantly affected transcription of HaSCP-2, leading to a 50% reduction in HaSCP-2 mRNA expression level. In conclusion, the Ha-c-Myc was expressed through a prokaryotic expression system, and the polyclonal anti-Ha-c-Myc antibody was obtained. Ha-c-Myc may promote the expression of HaSCP-2 and play an important role in the lipid metabolism of H. armigera. These results may facilitate further study on the potential role and function mechanism of Ha-c-Myc in H. armigera and provide experimental data for exploring new targets of green pesticides.

Objective:To systematically evaluate the efficacy and safety of filiform fire needle combined with tacrolimus ointment in the treatment of vitiligo.Methods:The RCTs of combination of filiform fire needle and tacrolimus ointment on the treatment of vitiligo were searched in the Chinese Biomedical Literature Database (CBM), China Knowledge Network Full-text Database (CNKI), Wanfang Full-text Database, VIP and PubMed databases. The studies were selected with the inclusion and exclusion criteria, then data extracted, and the quality of the included studies was evaluated according to Cochrane System Evaluation Manual. Meta-analysis was performed using RevMen 5.3 software.Results:Four studies involving 218 patients were included. After analysis, it was found that the effective rate [ OR=4.42, 95% CI (1.39-14.03), P<0.05)] and the apparent efficiency [ OR=2.89, 95% CI (1.40-5.95), P=0.004] of filiform fire needle combined with tacrolimus ointment were significantly better than that of tacrolimus ointment alone. Adverse reactions werelocal slight redness, pain and itching. None of adverse reactionswas reported. Recurrence occurred in the follow-up of were reported in two studies, which found that the recurrence rate [ OR=0.17, 95% CI (0.04-0.77), P<0.05] in the combined treatment group was significantly lower than that in the control group. Conclusions:The clinical efficacy of filiform fire needle combined with tacrolimus ointment in the treatment of vitiligo is better than that of tacrolimus ointment alone, and the recurrence rate can be reduced without any adverse reactions. However, in view of the limited number, average quality and bias of the included studies, it is suggested to carry out more multi-center, large sample and high quality randomized controlled trials for further verification.

In order to overcome the shortcomings of high false positive rate and poor generalization in the detection of microcalcification clusters regions, this paper proposes a method combining discriminative deep belief networks (DDBNs) to automatically and quickly locate the regions of microcalcification clusters in mammograms. Firstly, the breast region was extracted and enhanced, and the enhanced breast region was segmented to overlapped sub-blocks. Then the sub-block was subjected to wavelet filtering. After that, DDBNs model for breast sub-block feature extraction and classification was constructed, and the pre-trained DDBNs was converted to deep neural networks (DNN) using a softmax classifier, and the network is fine-tuned by back propagation. Finally, the undetected mammogram was inputted to complete the location of suspicious lesions. By experimentally verifying 105 mammograms with microcalcifications from the Digital Database for Screening Mammography (DDSM), the method obtained a true positive rate of 99.45% and a false positive rate of 1.89%, and it only took about 16 s to detect a 2 888 × 4 680 image. The experimental results showed that the algorithm of this paper effectively reduced the false positive rate while ensuring a high positive rate. The detection of calcification clusters was highly consistent with expert marks, which provides a new research idea for the automatic detection of microcalcification clusters area in mammograms.

Objective To analyze the result and adverse reactions of radiation therapy in patients with pelvic recurrence following cervical cancer postoperative.Methods A retrospective analysis of 147 patients with pelvic recurrence after surgical treatment of cervical cancer in the Fourth Hospital of Hebei Medical University from August 2004 to December 2016 was performed.All patients received radiotherapy with or without chemotherapy.According to different clinical factors and pathological factors,Logistic regression analysis was used to analyze the factors influencing radiotherapy outcomes in patients with pelvic recurrence after cervical cancer surgery.The Kaplan-Meier method was used to analyze the survival rate,and the corresponding survival curve was drawn.The survival rate and prognosis related factors were compared by using the log-rank test.The COX proportional hazards regression model was used for multivariate analysis of statistically relevant factors in univariate analysis.After treatment,toxicities were analyzed using chi-square test.Results The median follow-up time was 33.2 months.95% of the patients completed radiation therapy with a dose of ≥ 67 Gy (median radiotherapy dose),and 91 patients (61.9%) had complete remission (CR).The 5-year local control (LC),progression-free survival (PFS),distant metastasis-free survival (DMFS),and overall survival (OS) were 63.6%,56.0%,73.9%,and 55.0%,respectively.Univariate logistic regression analysis showed that FIGO staging (stage 0-ⅠB and ⅡA-ⅡB),pelvic sidewall involvement,and recurrent tumor volume were associated with complete remission (P<0.05).Multivariate statistical analysis found that FIGO staging and pelvic sidewall invasion were independent factors influencing the efficacy and survival of patients with pelvic recurrence after cervical cancer surgery (P<0.05).Patients with pelvic wall invasion after cervical cancer surgery had a higher incidence of ≥ grade 2 proctitis than those without pelvic walls involved,which were 26.9% and 16.7%,respectively.Conclusions This study shows that after the surgical treatment of cervical cancer patients with pelvic recurrence can be tolerated by toxicities after radiation therapy.In addition,the incidence of toxicities in patients with pelvic wall invasion was significantly higher than those without pelvic wall invasion.Preoperative staging and the pelvic wall involvement are independent influencing factors influencing the effect of radiotherapy and long-term prognosis in patients with pelvic recurrence after cervical cancer surgery.

Existing arrhythmia classification methods usually use manual selection of electrocardiogram (ECG) signal features, so that the feature selection is subjective, and the feature extraction is complex, leaving the classification accuracy usually affected. Based on this situation, a new method of arrhythmia automatic classification based on discriminative deep belief networks (DDBNs) is proposed. The morphological features of heart beat signals are automatically extracted from the constructed generative restricted Boltzmann machine (GRBM), then the discriminative restricted Boltzmann machine (DRBM) with feature learning and classification ability is introduced, and arrhythmia classification is performed according to the extracted morphological features and RR interval features. In order to further improve the classification performance of DDBNs, DDBNs are converted to deep neural network (DNN) using the Softmax regression layer for supervised classification in this paper, and the network is fine-tuned by backpropagation. Finally, the Massachusetts Institute of Technology and Beth Israel Hospital Arrhythmia Database (MIT-BIH AR) is used for experimental verification. For training sets and test sets with consistent data sources, the overall classification accuracy of the method is up to 99.84% ± 0.04%. For training sets and test sets with inconsistent data sources, a small number of training sets are extended by the active learning (AL) method, and the overall classification accuracy of the method is up to 99.31% ± 0.23%. The experimental results show the effectiveness of the method in arrhythmia automatic feature extraction and classification. It provides a new solution for the automatic extraction of ECG signal features and classification for deep learning.

Objective To summary the clinical characteristics of acute necrotizing encephalopathy in childhood so as to estimate the prognosis and guide the therapy. Methods We collected and analyzed retro-spectively the data of 10 acute necrotizing encephalopathy cases hospitalized in the pediatric department of our hospital from January 2014 to April 2018. The clinical manifestations,laboratory examinations and imaging features were retrospectively analyzed. Results A total of 10 cases all had fever and convulsions (100%). There was no specific clinical manifestation in the early stage,always showing the symptoms of acute respira-tory infection or acute gastroenteritis. Mental malaise,lethargy,restlessness and frequent convulsions occured when the conditions aggravated. Nine patients had different degrees of consciousness disorder. It could be complicated by multiple organ dysfunction syndrome,hemophagocytic syndrome and so on. Eight patients un-derwent MRI. In acute phase, homogeneously prolonged T1 and T2 relaxation time of the brain lesions on MRI were found in most patients,and diffusion-weighted imaging ( DWI) and FLAIR showed high signal in-tensity. Six cases underwent head CT examination, hypodensities were seen on 4 cases. Cerebrospinal fluid (CSF) was detected in all patients,and the protein levels in CSF increased to varying degrees,while white blood cells did not increase. Of the 10 patients,3 died,1 was discharged normally,1 had hemophagocytic syn-drome,transferred to pediatric hematology department,and 5 patients were transferred to rehabilitation depart-ment. Conclusion Acute necrotizing encephalopathy in children always make progress rapidly, has high mortality,many survivors have severe neurological sequelae,which is worthy of being attention.

Objective To summarize the clinical characteristics of necrotizing pneumococcal pneu-monia in childhood,so as to estimate the prognosis and guide the therapy. Methods We retrospectively col-lected and analyzed the data of 15 necrotizing pneumococcal pneumonia cases hospitalized in the pediatric de-partment of Shengjing Hospital from January 2008 to April 2011. The clinical features and treatment outcomes were analyzed,which included sex, age, maximum body temperature, hospital stay, duration of fever, white blood cell count and C-reactive protein level. Fifteen strains of the clinical isolated streptococcus were ana-lyzed antimicrobial susceptibility test with the disk diffusion procedure,and eleven strains of them were ana-lyzed by with minimal inhibitory concentrations. Results All of 15 cases had fever and pleural effusion,1 case with extrapulmonary manifestations. In chest X-ray and CT scanning,the lesion location were on the left in 7 cases,on the right in 7 cases,bilateral in 1 case. All patients had increased white blood cell count and C-reactive protein. Fifteen strains of streptococcus pneumoniae were collected from clinic samples,which had high sensitivity to vancomycin,linezolid,rifampicin,levofloxacin,no resistant strains appeared. We observed that the resistant rates of streptococcus pneumoniae to penicillin was 7/15,and to erythromycin and clindamy-cin were 11/11 and 10/11. The average length of stay in hospital was 39 d and no death occurred during the hospitalization. Conclusion The treatment of necrotizing pneumococcal pneumonia in childhood usually takes a longer time,and patients always have obvious symptoms. The drug-resisting of streptococcus pneu-moniae are serious,and children with necrotizing pneumococcal pneumonia are easily complicated by pleural effusion. But through active treatment,the prognosis is good.

Objective To study the characteristic and manifestations in the middle aged and elderly peoplewith discoid meniscus injury and the outcome of the arthroscopic surgery. Methods Twenty-nine patients with discoid meniscus injury were treated by the arthroscopic surgery from January 2009 to May 2013. According to the injury of discoid meniscus in operation, 6 cases (6 knees , 20.69%) underwent meniscal plasty, 16 cases (16 knees, 55.17%) underwent subtotal meniscectomy, and 7 cases (7 knees, 24.14%) underwent meniscectomy. The Ikeuchi and Lysholm evaluation system was used to assess knee joint function and compared. Results All the postoperative patients were followed up for 12-18 months, and the average was 14 months. According to Ikeuchi knee rating, 5 cases were excellent, 17 cases were good, 4 cases were fair, 4 cases were poor and the rate of excellent and good was 75.86%. Lysholm scores 3, 6 and 12 months after operation were (65.76 ± 5.35), (70.27 ± 4.73) and (77.81 ± 6.24) scores, and before operation was (55.47 ± 3.63) scores. Three, 6 and 12 months after operation, Lysholm scores were significantly higher than those before operation (P<0.05). Conclusions The middle aged and elderly people discoid meniscus injury has its own characteristics compared with adolescent patients in symptoms, signs, and treatments. Arthroscopic surgery is an effective treatment for discoid meniscus with minimal.

Now 3-D printing products has penetrated into every aspect of our lives, at the same time, with the progressing of 3-D imaging technology and materials engineering, 3-D printing technology are gradually applying to medical practice, including the orthopaedics, treatment of congenital heart disease and valve replacement. There are few researches about the 3-D printing technology in the application of percutaneous coronary intervention (PCI), so the paper is to preliminary explore the application of 3-D printing in PCI from three aspects of technology teaching, doctor-patient communication and surgical planning.

ABSTRACT:Objective To evaluate the clinical therapeutic effect and safety of tumor necrosis factor alpha (TNF-α)blockers in treating moderately to severely active ulcerative colitis (UC)by meta-analysis.Methods Such databases as the Cochrane Central Register of Controlled Trials,PubMed,OVID,Embase,ISI,CBM,CNKI, VIP,and WanFang Data were searched from establishment to June 2013.All randomized clinical trials (RCTs)on tumor necrosis factor alpha blockers in treating UC were collected,and then selected on the basis of the inclusion and exclusion criteria.We assessed the methodological quality,extracted the data from the included articles and performed the meta-analysis with Revman 5.1.Results A total of 13 RCTs involving 3334 patients were analyzed.TNF-αblockers group was superior to the control group in the short-term clinical response (OR =2.5 1, 95% CI 1.73,3.64),short-term clinical remission (OR =2.74,95% CI 1.80,4.1 6),long-term clinical response (OR =2.98,95% CI 1.98,4.47),1ong-term clinical remission (OR =2.64,95% CI 1.89,3.67),and mucosal healing (OR =1.89,95% CI 1.39,2.59)compared with control group.TNF-αblockers could also reduce the rate of colectomy (OR =0.61,95% CI 0.41,0.89)and improve inflammatory bowel disease questionnaire scores (MD=14.74,95% CI 1 1.43,18.06 ).There was no significant difference between the two groups in all reported adverse effects (OR =1.14,95% CI 0.97,1.34)and serious adverse effects (OR=0.78,95% CI 0.56,1.09).Conclusion Compared with conventional therapy or placebo,TNF-αblocking agents can improve the therapeutics effect on UC in clinical response,clinical remission and mucosal healing,and also can reduce the rate of colectomy. In patients with moderately to severely active UC treated with TNF-α blocking agents,it is easier to achieve the improvement of life quality.TNF-αblocking agents treatment is safe for UC.This conclusion should be verified with more large-scale and high-quality RCTs.

Background and purpose:One of the reasons why cancer cells are resistant to chemotherapy is the existence of cancer stem cells. The purpose of this study was to investigate the chemoresistance of CD44+/CD24+ Siha cells to cisplatin and its mechanisms.Methods:Siha cells were cultivatedin vitro. The CD44+/CD24+ Siha cells were sorted out by fluorescence activated cell sorter (FACS) andin vitro proliferation was detected by MTT assay after treatment with the different concentrations of cisplatin. The cell apoptosis rate was detected by flow cytometry after 10 μg/mL cisplatin acted on CD44+/CD24+ Siha cells for 24, 48 and 72 h. The relative mRNA and protein expressions of Bcl-2, Oct-4 and ABCG2 were detected by quantitative real-time polymerase chain reaction (qRT-PCR) and Western blot, respectively. Results:The survival rates of CD44+/CD24+ Siha cells treated with different concentrations of cisplatin (0.1, 1, 5, 10, 15 and 20 μg/mL) were higher than those of their parental Siha cells [(88.42±1.51)%vs (92.87±1.5)%, (79.94±1.05)%vs (84.72±1.09)%, (69.78±0.81)%vs (75.13±2.86)%, (58.97±0.70)%vs (65.79±2.71)%, (49.60±0.88)%vs (52.10±0.52)%, (45.13±0.69)%vs (48.84±1.02)%,P<0.05]. Compared with their parental Siha cells, the apoptosis rates of CD44+/CD24+ Siha cells were lower after 10 μg/mL of cisplatin acting on them for 24, 48 and 72 h, respectively [(3.05±0.16)%vs (5.17±0.27)%, (17.94±2.02)%vs (32.60±4.28)% and (40.14±3.01)%vs (56.62±5.32)%,P<0.05]. The results from both qRT-PCR and Western blot indicated that Oct-4, ABCG2 and Bcl-2 were highly expressed on CD44+/CD24+ Siha cells. A significant difference was found in Oct-4, ABCG2 and Bcl-2 expression between CD44+/CD24+Siha cells and their parental cells (P=0.015<0.05).Conclusion:CD44+/CD24+ Siha cells could be resistant to apoptosis induced by cisplatin and expressed high levels of cancer stem cell markers such as Oct-4 and ABCG2. This study lays the basis for useful isolation and further targeted therapy of cervical cancer stem cells.

Objective To explore whether CD44 +/CD24 + expressing cervical cancer cells are resistant to X-ray irradiation and investigate the underlying mechanism.Methods Cervical cancer cell line (Siha) was cultured in vitro and the CD44 +/CD24 + expressing cells were sorted with a flow cytometer.The cells were irradiated with 8,16 and 30 Gy of 6 MV X-rays.Colony formation test was used to evaluate the radiosensitivity of CD44 +/CD24 + expressing cervical cancer cells.Cell morphology was observed by electronmicroscopy,cell apoptosis was analyzed with a flow cytometer and also verified with a DNA ladder assay.Gene expression was determined by RT-PCR.Results After radiation,the ratio of CD44 +/CD24 + cells significantly increased.Compared to Siha cells,the radiosensitivity of CD44 +/ CD24 + cells decreased (t =93.99-400.45,P <0.05),and the expressions of bcl-2,survivin and Oct4 mRNA increased in CD44 +/CD24 + cells (t =221.35,941.65,82.27,P <0.01).Both apoptotic body and specific DNA ladder pattern were observed in cells but not in the CD44 +/CD24 + Sihacells which had no obvious morphological changes of apoptosis.Conclusions The CD44 +/CD24 + expressing cervical cancer cells are resistant to X-rays due to expression of anti-apoptosis factors.

Atherosclerosis is a chronic inflammatory disease.Atherosclerotic plaque rupture and thrombosis may result in cardio-cerebrovascular events.Inflammatory mediator CD40L widely exists in cells associated with atherosclerosis.They participate in plaque inflammatory response,release proinflammatory cytokines,degrade extracellular matrix,improve procoagulant activity,and promote the progression of atherosclerosis and plaque vulnerability.Intervening CD40/CD40L signaling system may become an effective treatment strategy to slow the progress of atherosclerosis and stabilize atherosclerotic plaques.

The pathogens were tested in 682 cases of neonatal pneumonia,atypical pathogens were found in 167 cases (24.49%).Among the 167 cases,chlamydia trachomatis was detected in 124(74.25%),ureaplasma urealyticum in 55 (32.93%),chlamydia pneumoniae in 47 (28.14%) and mycoplasma pneumoniae in 35 (20.56%).The Chlamydia trachomatis-positive rate in mothers of 167 cases was 80.84%,ureaplasma urealyticum was 53.29%,chlamydia pneumoniae was 50.90%,Mycoplasma pneumoniae was 53.29%.The patients were treated with azithromycin and other antibiotics,151 cases were cured and 11 were effective with an overall efficacy rate of 97.01%.The liver and kidney function were tested before and after treatment; and there were no significant changes after treatment.The results show that azithromycin is safe and effective for treatment of neonatal infectious pneumonia from atypical pathogens.

A QRS complex detection algorithm based on empirical mode decomposition (EMD) and adaptive windowing technique is proposed in this paper. In this algorithm we mainly used Hilbert-Huang transform to propose EMD method suitable for QRS complex detection, with which the 30th signal in sddb database and the 208th signal in mit-db database could be processed, and then obtained R wave detection results. At the same time, Q and S points' detection technique was analyzed with adaptive windowing technique. The detection results, through proceeding R wave detection on part data of MIT/BIT arrhythmia database, showed that the proposed algorithm in this paper had a very good detection effect, and that its average correct detection rate of R wave reached 99.62%, its average sensitivity of QRS complex was 98.91%, and the corresponding average specificity was 99.35%.

Objective Coronary heart disease (CHD) is a multifactorial disease. This meta-analysis was performed to evaluate the relationship between angiotensinogen gene polymorphisms and CHD in the Chinese population. Methods We searched literature in pubmed (1990-2010.8) and CNKI (1990-2010.8) for all the relevant studies on 2 angiotensinogen polymorphisms (M235T and T174M) and risk of CHD. The meta-analysis software Stata 10.0 was used for ascertaining heterogeneity among individual studies and for combining all the studies. Furthermore,Egger's test and sensitivity analysis were performed to insure authenticity of the outcome.Results Ten associations studies on 2 angiotensinogen polymorphisms (M235T and T174M) were included in this meta-analysis. In a combined analysis, the summary per-allele odds ratio for CHD of the M235T polymorphism was 1.374 (95% confidence interval, 1.019 to 1.852) and T174M polymorphism was 4.089 (95% confidence interval, 1.697 to 9.851). Conclusions The M235T polymorphism had weak but statistically significant association with CHD while the T174M polymorphism was more strongly associated with a CHD risk in Chinese population, but further confirmation studies are needed.

Objective To analyze the relationship between clinical features of infantile cough after infection and microelements in peripheral blood, and to observe the therapeutic of treating this cough with Milkvetch Root and Radix Glycyrrhizae. Methods 75 infantile patients were randomly recruited into a therapeutic group and a reference group. 39 patients in the therapeutic group were treated with Huangqi Zhusheye and Tablete Glycyrrhiza, while 36 patients in the reference group were treated with Azithromycin. The therapeutic effects were evaluated after the treatment. Meanwhile,microelements in peripheral blood of the 75 patients were assayed and made a contrast to 40 healthy children. Results The contents of Fe, Zn, and Mg in peripheral blood of the 75 patients dropped on different levels, comparing to the healthy children.The therapeutic effects of the therapeutic group was much better than the reference group with significant difference (P<0.05).Liver and renal functional lesion was unseen in both groups after the treatment. Conclusion Infantile past-infection cough may related with the decrease of Mg, Fe, and Zn. Combined usage of Milkvetch Root and Radix Glycyrrhizae clinically has good therapeutic effects to this disease.

OBJECTIVE@#In order to investigate the effect of UPPP in patients with OSAHS, the range of the soft palate excision and impersonality standard for improving low oxygen after UPPP.@*METHOD@#A prospective study was designed to evaluate 34 adult patients with OSAHS and 27 adult ear patients who were not OSAHS. Blood routine and palatopharyngeal data in two groups were measured before operation and 15-20 days after operation. And follow up 6 months. The data before and after operation were analysed in statistical method (t test).@*RESULT@#Length of soft palate, distance of pharynx, distance of palate pharynx, red blood cell (RBC), hemoglobin (HGB), and hematocrit (HCT) were significantly different in normal and OSAHS group (independent t test, P<0.01). Their indexes were significantly different in OSAHS group before and after operation (paired-sample t test, P<0.01). Only HGB and the lowest SaO2 were negative correlation before operation in OSAHS group. The value is bigger for HGB and smaller for SaO2 (t test, P<0.01). Result of follow up 6 months (31 cases): The effective ratio of subjective improving was 74.2%, and the total effective ratio was 96.8%. One case had reflux of nasopharynx.@*CONCLUSION@#Severe OSAHS patients should be underwent synthesized treatment. It is daytime sleepiness that was improved the most obviously. Soft palate excision scope can't exceed 1.5 cm. HGB can be an impersonality index to assess hypoxemia and improvement of SaO2 after operation for OSAHS.
