ABSTRACT
Rheumatoid arthritis (RA), as an autoimmune disease, is mainly characterized by persistent synovitis. It often involves multiple joints symmetrically and can lead to joint deformity, joint function loss, and even disability in severe cases. The pathogenesis of RA is complex, and the prevention and treatment are complicated. Therefore, it is difficult to cure the disease completely. Previous studies have validated important targets and mechanisms for the prevention and treatment of RA, including the nuclear factor-κB (NF-κB) signaling pathway that controls the inflammatory process, nuclear factor E2-related factor 2 (Nrf2)/heme oxygenase-1 (HO-1) signaling pathway that regulates oxidative stress, inhibits inflammation, and maintains cell homeostasis, Wnt/β-catenin signaling pathway that plays a key role in cell growth, differentiation, apoptosis, and inflammatory response, anti-inflammatory, anti-oxidation, and silent information regulator 1 (SIRT1) signaling pathway that regulates synovial cells, anti-inflammatory adenylate-activated protein kinase (AMPK) signaling pathway that regulates energy metabolism, and hypoxia-inducible factor-1α (HIF-1α)/vascular endothelial growth factor (VEGF) signaling pathway related to angiogenesis in RA. At the same time, many studies have confirmed that traditional Chinese medicine prevents and treats RA by regulating the above signaling pathways and exerting their related effects, indicating the advantages of traditional Chinese medicine such as multiple regulatory pathways, long-term effects, and less adverse reactions. In this paper, by consulting many research reports, the role of the above-mentioned signaling pathways in RA was clarified, and the latest research results of traditional Chinese medicine intervention in the above-mentioned signaling pathways in the prevention and treatment of RA in recent years were summarized in detail. This paper aims to promote the in-depth study of the pathogenesis of RA and its treatment with traditional Chinese medicine, provide a scientific basis for the rational application of traditional Chinese medicine, and offer useful enlightenment for the development of new drugs and clinical practice for the treatment of RA in the future.
ABSTRACT
Puerarin (Pue), known as a phytoestrogen, has salient bioactivities and is promising against cardiovascular diseases. This article summarizes the underlying molecular mechanisms of Pue in treating cardiovascular diseases, especially regulating the intracellular signal transduction, influencing ion channels, modulating the expression of microRNA, and impacting on the autophagy, which are mainly involved in the inflammatory signaling pathways, fatty acid/lipid metabolism, oxidative stress, apoptosis, and the like. The protective effect of Pue against cardiovascular diseases mainly involves attenuating the myocardial injury and decreasing the myocardial fibrosis, improving the myocardial ischemia/reperfusion injury, as well as inhibiting the myocardial hypertrophy and atherosclerosis. The molecular mechanisms of Pue's cardiovascular protective effects for the first time and comment on the state-of-the-art research methods and principles of Pue's regulation of small molecules were reviewed, so as to provide the rationale for its basic research and clinical applications.

Aiming at the current situation of high cost, huge volume, complex operation and difficulty in real application of pulse analyzer, this study designs and implements a portable pulse detection system based on IoT. The design utilizes Raspberry Pi 3B+, STM32 series MCU and cloud server to collect, store, display and recognize pulse signals at CUN, GUAN and CHI. The system is small in size and low in cost, which can be connected with cloud server through network to make full use of resources. The experimental results show that the recognition accuracy of the main feature points of the pulse signal by the portable pulse analyzer is higher than 97%, which has a broad prospect of development and application.

Objective@#To investigate the serum expression and influencing factors of hepcidinin patients with classical paroxysmal nocturnal hemoglobinuria (PNH) .@*Methods@#Retrospective analysis of 36 classical PNH patients from 2016.3 to 2017.3. Serum hepcidin concentration was measured by ELISA method. The relationship between serum hepcidin concentration and erythropoiesis and iron homeostasis parameters was evaluated.@*Results@#The median serum hepcidin level of 36 classical PNH patients was 32.03 (23.11, 118.48) μg/L, it was significantly lower than of 181.42 (106.80, 250.53) μg/L in 292 normal control subjects (z=-5.107, P<0.001) . The median serum hepcidin of 56.41 (44.60, 95.06) μg/L in PNH patients with normal ferritin was significantly lower than that in normal controls. The median serum hepcidin concentration 23.75 (21.77, 30.35) μg/L in iron deficiency PNH patients was lower than that in the normal ferritin PNH patients. However, the median serum hepcidin level of classical PNH with elevated ferritin patients 336.19 (304.19, 375.08) μg/L was significantly higher not only than that of normal ferritin and iron deficiency PNH ones, but also than that of normal control subjects. Regression analysis showed that serum ferritin, transferrin saturation and serum albumin level were independent influencing factors of serum hepcidin level in patients with classical PNH.@*Conclusion@#The decreased serum hepcidin level in patients with classical PNH was mainly influenced by iron metabolism factors.

Objective@#To compare the effects of different hemolytic diseases on the level of glycosylated hemoglobin (HbA1c) to further explore the relationship between HbA1c and laboratory indexes to disclose implications of HbA1c in hemolytic diseases.@*Methods@#The distribution of 192 decreased HbA1c cases in 4 categories of hemolytic diseases was analyzed. Laboratory indexes related to hemolysis were tested and analyzed in each kind of disease, and relationship between laboratory indexes and HbA1c was statistically explored.@*Results@#Diagnoses of decreased HbA1c cases mainly included erythrocyte membranopathies (88 cases), immunohemolytic anemia (72 cases), hemoglobinopathy (4 cases) and erythrocyte enzymopathy (5 cases). The distribution of HbA2 and normal HbF subjects in immunohemolytic anemia and hemoglobinopathy was significantly different from those of HbA2 and / or abnormal HbF subjects (41.7% vs 22.0%, χ2=5.574, P=0.018; 0.7% vs 7.3%, P=0.031). Compared with non-hemolytic disease patients, those who suffered from 4 categories of hemolytic diseases showed lower HbA1c level and higher reticulocyte percentage (Ret), indirect bilirubin (IBIL) and free hemoglobin (F-Hb). Different levels of Ret, reticulocyte hemoglobin content (Ret-He), mean corpuscular volume (MCV), IBIL and F-Hb among the 4 kinds of diseases were observed, but the causes of the differences were not the same. HbA1c was negatively correlated with other laboratory indexes in erythrocyte membranopathies and immunohemolytic anemia.@*Conclusions@#Hemolytic disease resulted in false lower HbA1c, but impact of difference on HbA1c between different diseases was not significant. HbA1c was closely connected to laboratory indexes related to hemolysis, which might have potential implications for hemolytic diseases such as erythrocyte membranopathies and immunohemolytic anemia.

Objective To evaluate the significance of hemoglobin H in the diagnosis of alpha thalassemia and Myelodysplastic syndrome. Methods A retrospective analysis of the complete clinical data of hemoglobin H in our hospital from January 2007 to October 2017 was performed. The hemoglobin H content in 34 cases of alpha thalassemia and the corresponding blood routine, reticulocyte, Bilirubin, and plasma free hemoglobin levels were analyzed by Pearson correlation. Results 39 cases of hemoglobin H positive cases, of which 34 cases diagnosed as alpha thalassemia,5 cases diagnosed as myelodysplastic syndrome; for alpha thalassemia, hemoglobin H content and reticulocyte, plasma free hemoglobin and Indirect bilirubin test results are related(r =0. 453 ,0. 398,0. 412,P<0. 05). Conclusion Hemoglobin H is not only found in alpha thalassemia but detected in other types of hematologic disorders.

Clinical data of 19 patients with congenital pyruvate kinase deficiency were analyzed. Insufficient pyruvate kinase confirmed the diagnosis. Laboratory parameters of hemolysis were summarized. In cases of neonatal hyperbilirubinemia and unexplained hemolytic anemia, pyruvate kinase activity and next generation sequencing test may help the early diagnosis.

Objective:To detect the levels of PA,FN,RBP,CRP,Iron,Zinc,VitD in peripheral blood of children with HFMD and to explore the relationship between them and the disease .Methods:The levels of PA,FN,RBP and CRP were detected by immune turbidimetric assay;and the levels of Iron and Zinc were detected by flame atomic absorption spectrometry ;while the levels of VitD were detected by electro chemilumin escence .Results:The levels of PA,FN,RBP,Iron,Zinc and VitD in the severe group were lower than those in other groups ,while the levels of CRP were higher than those in other groups ( P0.05 ) .The levels of PA,FN,RBP,Fe,Zn and VitD in the convalescence were higher than those in the acute phase of the severe cases (P0.05).There were negative correlation between the levels of PA ,FN,RBP and the state of HFMD(P<0.05),while there were positive correlation between the levels of CRP and the state of HFMD (P<0.05). And there were low negative correlation between the levels of Iron , Zinc, VitD and the state of HFMD ( P <0.05 ), respectively.Conclusion:There were nutritional deficiency in severe cases of HFMD ,and the nutritional status should be assessed dy-namically,then proper amount of nutrition should be supplement .

Objective: To study the antibacterial activity in vitro of Sanhuang Shaoshangling on several common bacteria after changing the preparation process. Methods:Using the new technology of ethanol reflux extraction and water extraction, Sanhuang Sha-oshangling was prepared. A micro-broth dilution method was used to determine the minimum inhibitory concentration ( MIC) of San-huang Shaoshangling for escherichia coli, pseudomonas aeruginosa, staphylococcus aureus and type B paratyphoid salmonella after chan-ging the preparation process. The antibacterial effect of Sanhuang Shaoshangling prepared by the new and the old technology was com-pared, and the inhibition zone was detected by a paper strip method. Results:The antibacterial effect of Sanhuang Shaoshangling pre-pared by the new technology on escherichia coli, pseudomonas aeruginosa, staphylococcus aureus and type B paratyphoid salmonella was better than that of the products prepared by the old technology with the minimum inhibitory concentration ( MIC) of 16 μg·ml-1 , 16μg·ml-1 , 8 μg·ml-1 and 16 μg·ml-1 , respectively, while that of the product prepared by the old technology was 32 μg·ml-1 , 64 μg·ml-1 , 16 μg·ml-1 and 64 μg·ml-1 , respectively. Conclusion: The antibacterial effect of Sanhuang Shaoshangling after changing the preparation process is more significant, which shows certain clinical significance.

<p><b>OBJECTIVE</b>To investigate the sensitivity and specificity of eosin-5'-maleimide (EMA)assay for the diagnosis of hereditary spherocytosis (HS), and to verify the stability of reagent and samples.</p><p><b>METHODS</b>EMA flow cytometry test, NaCl-osmotic fragility test and acidified glycerol lysis test were performed using peripheral blood samples from 80 patients with HS and 44 patients with other blood diseases, the sensitivity and specificity of the three methods were compared, and the feasibility of EMA binding test was estimated. The stability of EMA reagent and HS samples stored at different temperatures were tested.</p><p><b>RESULTS</b>Among the 124 tested samples, the sensitivity and specificity of EMA binding test was 0.925 and 0.954, that of NaCl-osmotic fragility test was 0.950 and 0.455, and that of acidified glycerol lysis test was 1.000 and 0.318, respectively. Although the sensitivity of NaCl-osmotic fragility test and acidified glycerol lysis test was a little higher than that of EMA binding test, the specificity of the former two methods was poor, they couldn't clearly distinguish whether spherocytosis is hereditary spherocytosis. The experiment results showed that EMA was sensitive to the temperature and should not be stored in a small aliquots at -80 ℃ over a period of 6 months. The stability of the HS sample was better, 6 days storage at 4 ℃ and 3 days storage at room temperature had no influence on the results.</p><p><b>CONCLUSION</b>EMA binding test by flow cytometry showed good sensitivity and specificity for HS diagnosis. EMA reagent should be stored at-80 ℃ and the HS samples should be tested within 6 days storage at 4 ℃ and 3 days at room temperature.</p>

ObjectiveTo investigate the characteristics of trace elements and amino acid metabolism in the blood of patients with hepatocellular carcinoma (HCC) and to guide clinical treatment. MethodsThis study included 125 untreated patients with primary HCC in Tangshan Maternal and Child Health Hospital and Tangshan Guzhi District Hospital from March to November, 2013, as well as 50 healthy controls who underwent physical examination in Tangshan Maternal and Child Health Hospital. Blood trace elements were measured by atomic absorption spectrophotometry, and the levels of amino acids were determined by an amino acid analyzer. Comparison of continuous data was made by t test. ResultsCompared with the controls, the HCC patients had a significantly higher Cu level (t=6.30, P<0.05) and significantly lower Zn, Fe, and Mg levels (t=10.15, 3.98, and 2.25, all P<0.05). The HCC patients showed significant increases in tyrosine, phenylalanine, and methionine levels (t=10.10, 13.46, and 2.34, all P<0.05) and significant decreases in valine, isoleucine, leucine, and arginine levels (t=3.65, 2.98, 5.67, and 8.35, all P<0.05). ConclusionHCC patients show changes in various amino acids and trace elements in blood. Measurement of trace elements and amino acids is of certain significance for evaluating HCC patients′ condition and guiding clinical treatment.

Objective:To evaluate the alternation of homocysteine (Hcy),folic acid(FA),CA15-3 and vitaminB12(VitB12) in patients with breast cancer and explore the correlation of them.Methods:The levels of FA ,CA15-3 and VitB12 in serum of 55 breast cancer and 35 benign tumor of breast and 50 controls were measured by electro-chemiluminescence immunoassay ( ECLI ) , the Hcy levels were measured by colorimetric method.Results:Compared with the normal controls and benign tumor of breast cancer ,there were some significantly changes in breast cancer patients including Hcy ,CA15-3 increased and FA ,VitB12 reduced ( P<0.05 ) ,there were no significant differences in Hcy,FA,CA15-3,VitB12 levels between benign breast tumors and normal controls (P>0.05).Companied with TNM stage increased ,the levels of FA breast cancer groups decreased ,the levels of Hcy,CA15-3 increased.The correlation analysis showed that Hcy was negatively correlated with FA and VitB 12,(r=-0.341,r=-0.540),CA15-3 was negatively correlated with FA, CA15-3 was postively correlated with Hcy (r=0.378,r=-0.361).Conclusion: Serum Hcy,CA15-3,FA and VitB12 detection in patients with breast cancer are helpful in the severity judgment and regimen prognosis .

ObjectiveTo explore the relationship of marriage quality with individual characteristics and copy style among the epilepsy patients.MethodsThe Chinese Marriage Quality Questionnaire,and Eysenck Personality Questionnaire Short Form Scale-Chinese edition (EPQ-RSC),and the Questionnaire of Simple Way to Coping were assessed used to assess 97 cases of the aduh married epilepsy patients and 93 cases of the control group 1 (epilepsy patients spouses) and 91 cases of the control group 2 (healthy crowds ).ResultsThe total scores of the marriage quality of epilepsy patients ( 295.78 ± 40.37 ) was lower than ones in the control group 2 ( ( 354.85 ± 28.11 ),P =0.000) and higher than that of the control group 1 ( (278.55 ± 42.23 ),P =0.014),the difference was all statistically significant(P< 0.05).The total scores of marriage quality of the epilepsy patients was negative correlated with the sex,and the age,and the marriage age and the severity,and the duration,and psychoticism( the Pearson correlation was respectively:-0.204,-0.436,-0.243,-0.691,-0.568,-0.635 ),and was positive correlated with which the formal schooling,and the attack types (symptoms epilepsy),and the introversion or extroversion,and the hidness,and the positive( the Pearson correlation was respectively 0.317,0.750,0.267,0.386,0.188) the difference was all statistically significant (P < 0.05).Those seven variables that the attack type,and the hidness,and the severity,and the sex,and the education,and the psychoticism,and the positive could be predict to marriage quality of epilepsy patients,Adjusted R square was 0.903,After adjustment was 0.021 (P < 0.05 ).ConclusionThe lever of marriage quality of the epilepsy patients is lower,and correlate with severity of epilepsy disease,and personality,copy style,and can be do comprehensive intervention from those.

Objective To investigate the effect of propofol on the gap junction function of BRL-3A cells and cisplatin-induced toxicity to rat hepatic BRL-3A cells. Methods Rat hepatic BRL-3A cell line was provided by professor Tao laboratory of department of pharmacology of our university and cultured in DMEM liquid culture medium at 37℃. The cells were randomly divided into 6 groups: group Ⅰ control (group C) ; group Ⅱ was exposed to intralipid (the solvent) 10 μg/ml (group Ⅰ) ; group Ⅲ was exposed to oleamide (gap junction function inhibitor) 25 μmol/L (group O) and group Ⅳ ,Ⅴ , Ⅵ were exposed to propofol l.5, 2.8 and 4.1 μg/ml respectively (groups P1, 2,3) . Using parachute dye-coupling assay, the dye spread rate and inhibition rate were calculated to measure the gap junction function of BRL-3 A cells. BRL-3 A cells were seeded in two different densities:high density group (seeding density = 1×105 /ml) and low density group (seeding density = 500/ml). Both groups were further divided into 5 subgroups: subgroup Ⅰ control; subgroup Ⅱ was exposed to cisplatin 2.5 μmol/L and subgroup Ⅲ ,Ⅳ ,Ⅴ were exposed to cisplatin 2.5 μmol/L + intralipid 10 μg/ml or oleamide 25 μmol/L or propofol 2.8 μg/ml respectively. The effect of propofol on the cytotoxicity of cisplatin was evaluated by standard colony-forming assay. Results The dye spread rate was significantly lower and inhibition rate higher in groups O,P1 , P2 and P3 than in group C. Propofol decreased the dye spread rate and increased inhibition rate in a concentration-dependent manner. Propofol and oleamide significantly increased colony formation rate in high density group,but had no significant effect on colony formation rate in low density group. Conclusion Propofol can inhibit the gap junction function of BRL-3A cells in a concentration-dependent manner and reduce cisplatin-induced cytotoxicity by inhibiting gap junction function.

Objective To investigate the effect of propofol on the function of gap junction (GJ) in HeLa cells transfected with Cx32/Cx26 plasmid. Methods Cervical cancer HeLa cells transfected with Cx32/Cx26 was given as present by professor Andrew L. Harris from New Jersey Dental Medical School, department of pharmacology and physiology. The transfected cells were selected by G-418. The effective GJ channels were identified by "Parachute Assay". The cells were randomly divided into 6 groups: Ⅰ control group (group C); Ⅱ fat emulsion group was exposed to fat emulsion 10 μg/ml (group E); Ⅲ 18-α-GA group was exposed 18-α-GA (gap junction blocker) 1.0 μg/ml (18-α-GA); Ⅳ, Ⅴ, Ⅵ propofol groups were exposed to propofol 1.3, 2.2 and 3.2μg/ml respectively (group P1, P2, P3). The transfected HeLa cells were incubated at 37 ℃ for 4 h. Gap junction function was assessed using fluorescent indicators Calcine-AM which emits green fluorescence and CM-Dil which emits red fluorescence. The small molecular Calcine-AM can pass through gap junction and enters HeLa cells while the large molecular CM-Dil cannot pass through gap junction and stays in the loading cells. Fluorescent indicator transmissibility and inhibition rate were calculated. Results The fluorescent indicator transmissibility was significantly lower and inhibition rate higher in group 18-α-GA, P1, P2 and P3 than in control group. There was nosignificant difference in the fluorescent indicator transmissibility and inhibition rate between group C and E. The inhibition of GJ function by propofol was dose-dependent. Conclusion Propofol can inhibit the function of GJ in HeLa cells transfected by Cx32/Cx26 in a dose-dependent manner.

Objective:Nipple discharge is a common complaint in women.There were no accurate techniques in detecting ductal lesions in nipple discharge patients before the application of fiberoptic ductoscopy.The aim of this study was to evaluate fiberoptic ductoscopy in the diagnosis and treatment of nipple discharge.Methods: From June 2007 to November 2008,a total of 95 patients with nipple discharge were submitted to fiberoptic ductoscopy,and 50 of them underwent surgery and pathological examination.Results: Of the 95 cases,2 diagnosed as breast cancer,58 intraductal papilloma,23 mammary duct ectasia,and 10 inflammatory disease of the mammary duct.Compared with the results of pathological examination,the accuracy of the fiberoptic ductoscopy was 81% for intraductal papilloma and 100% for breast carcinoma.Conclusions:Fiberoptic ductoscopy,as a convenient,safe and accurate method for the diagnosis and treatment of nipple discharge,can be used to confirm etiological factors and locate the site of the complaint.

Mental illness is prone to occur during adolescence,thus it is one major threat to the health of college students.However,it remains to be a serious medical problem and heated research subject to effectively treat mental-disordered students in students' management affairs.This article carries out a systematic case analysis of mental-disordered students in Shandong University at Weihai from 1996-2008,and puts forwards concrete approaches and countermeasures of prevention and treatment for those specific students.It concludes that an early diagnosis of the illness,retaining of school rolls during medical treatment,good management of medical records,prevention of potential accidental injuries,and promising employment prospect prove to be conducive for the treatment and prevention of college students' mental illness.

Objective To explore the distinguishing method and threshold of optimum pulse pressures on Cun pulse and Chi pulse to solve the problem how to exert pressure upon Three Regions Pulse Detecting Instrument.Methods Totally 55 subjects meeting the inclusion criteria were collected.The data of optimum pulse pressures on Cun Guan Chi of the same hand of the same tested person were collected respectively and the comparative analysis on the data was carried out.Results In normal subjects,the attenuation degree of optimum pulse pressures on Guan pulse compared with Cun pulse was 0.189?0.053,and the attenuation degree of optimum pulse pressures on Chi pulse in comparison with Guan pulse was 0.226?0.044.Conclusion Through calculating the attenuation degree of optimum pulse pressures reasonably,the identified threshold of Cun and Chi pulses can be judged from the threshold of Fu-superficial,Zhong-medium,and Chen-deep of Guan pulse.
