ABSTRACT

【Objective】 To investigate the imaging features of weak response after anti-vascular endothelial growth factor (VEGF) treatment for refractory diabetic macular edema (DME) with vesicles. 【Methods】 We made a retrospective analysis of the clinical data of 120 patients with DME (120 eyes) who received anti-VEGF therapy in Ophthalmology Department of The Second Affiliated Hospital of Xi’an Jiaotong University from March 2018 to September 2019 and the same anti-VEGF medicine with the therapeutic regimen of 3+PRN. Of the 50 patients with cystoid macular edema (CME), 40 of them had a significant response and 10 had a weak response. The characteristics of optical coherence tomography (OCT) and fundus fluorescein angiography (FFA) for the patients with weak response after anti-VEGF therapy were comparatively analyzed. 【Results】 Eyes with vesicular DME that responded weakly to anti-VEGF treatment had similar imaging characteristics. The OCT images showed impurity-independent vesicles located in the inner layer of the retina. Compared with the obvious response group, the reflection signal of the vesicle area was significantly enhanced. In early images of FFA, micro-angiomas related to vesicle formation were seen in both groups. However, in the late FFA image, the leakage intensity of vesicles in the weak response group was significantly lower than that in the obvious response group. 【Conclusion】 Vesicular DME with poor anti-VEGF treatment has the characteristics of high reflection in OCT and weak fluorescence leakage in FFA. This study is helpful for preoperative evaluation of anti-VEGF treatment.

ABSTRACT

Objective To study the clinical characteristics and treatment of primary nasal B-cell lymphoma (PNBCL). Methods A retrospective analysis was performed based on the clinical records of 18 PNBCL cases who were treated from January 2009 to June 2015. The clinical manifestations, imaging features, diagnosis approaches and treatment of them were analyzed. Results The main symptoms were nasal obstruction and rhinorrhea. Of all patients, 15 cases were in Ann Arbor stageⅠE-ⅡE, and 3 cases were in Ann Arbor stageⅢE-Ⅳ. The median age was 51 years (12-76 years). The ratio of men to women was 11:7. Only 1 patient had B symptoms. Elevated LDH levels were observed in 4 patients. 13 patients were diffuse large B-cell lymphoma(DLBCL), 3 patients were mantle cell lymphoma, and 2 patients were Burkitt lymphoma. CT examination showed the abnormal nasal soft tissue shadow, with unilateral location and light to moderate enhancement. 14 patients received combination chemotherapy only, and 3 patients received chemotherapy and radiotherapy. Total effective rate was 82.3 % (14/17). At the time of last follow-up, 5 patients died, and the 3-year OS rate was 54.5%(6/11). Conclusions Most PNBCL patients are in Ann Arbor stageⅠE-ⅡE and B symptoms are rare, and the most common pathological types is DLBCL. The treatment for PNBCL is chemotherapy, radiotherapy can assist, but the prognosis is poor, and innovative chemotherapy regimens are necessary.

ABSTRACT

Primary hepatic lymphoma (PHL) is an extremely rare disease without any unified diagnostic criterion.The symptoms are usually nonspecific.Liver biopsy remains the most valuable tool for diagnosis of PHL.The predominant histology of PHL is diffuse large B-cell lymphoma.The therapeutic modalities are variable,including surgery,chemotherapy,radiotherapy,or combination of the various processes.This article described a 33-year-old man with diffuse large B-cell PHL who was treated at the Affiliated Cancer Hospital of Shanxi Medical Univeitity Blood Disease Diagnosis and Treatment Center in February 2014.The patient benefited from eight-cycle chemotherapy.At present,the patient is disease-free and undergoes regular follow-up.

ABSTRACT

Objective To detect peripheral blood level of CD4+ CD25high CD127low regulatory T (Treg) cells in patients with B cell non-Hodgkin lymphoma (B-NHL) and to explore its clinical significance.Methods The levels of peripheral blood CD4+ CD25high CD127low Treg cells of 100 newly diagnosed patients with B-NHL and 50 healthy adults were detected and analyzed by flow cytometry.Results The levels of peripheral blood CD4+ CD25high CD12low Treg cells were 5.00 % and 7.20 % in healthy adults and newly diagnosed B-NHL patients,respectively.The difference was statistically significant (P < 0.001).The level of peripheral blood CD4+ CD25high CD127low Treg cells was significantly higher in the male patients than that in the female (P < 0.01).The level of peripheral blood CD4+ CD25high CD127low Treg cells was significantly higher in elevated LDH patients than that in normal LDH patients (P < 0.01).The level of peripheral blood CD4+ CD25high CD127low Treg cells in stage Ⅲ-Ⅳ patients was significantly higher than that in stage Ⅰ-Ⅱ patients (P < 0.01).The level of peripheral blood CD4+ CD25high CD127low Treg cells in B symptom patients was significantly higher than that in no B symptom patients (P < 0.01).There was no significant relationship between the level of peripheral blood Treg cells and age,IPI score,bulky disease or pathologic subtype (all P > 0.05).Conclusions B-NHL patients were in an immunosuppresive statue.Male,elevated LDH level,B symptom and Ⅲ-Ⅳ stage were correlated with higher level of peripheral blood CD4+ CD25high CD127low Treg cells.Level of peripheral blood Treg cells may be a useful prognostic factor for B-NHL.

ABSTRACT

Objective To assess the value of 18F-FDG PET-CT in monitoring early chemotherapy response in patients with diffuse large B-cell lymphoma (DLBCL).Methods 56 patients with DLBCL received 18F-FDG PET-CT before and after 2 cycles of chemotherapy (CHOP or R-CHOP protocol).The patients were divided into 3 groups,including complete response group (30 cases),partial response group (18 cases) and no response group (8 cases).The x2 test was performed on comparison of complete remission rate and the t test on SUVmax.Results Complete remission rate of complete response group,partial response group and no response group were 93.33 % (28/30),66.67 % (12/18),and 12.50 % (1/8),respectively (x2 =21.62,P < 0.005).Complete remission rate of complete response group was higher than that of partial response group (x 2 =4.00,P < 0.05).Complete remission rates of complete response group and partial response group were higher than that of no response group (x2 =18.58,P < 0.005; x2 =4.51,P < 0.05).A significant difference was observed in SUVmax of partial response group before and after chemotherapy (17.18±4.68 vs 5.20±3.35,t =14.32,P < 0.001) and no obvious difference was showed in SUVmax of no response group between before and after chemotherapy (16.63±6.49 vs 13.81±5.95,t =2.073 2,P > 0.05).Conclusion 18F-FDG PET-CT is very useful and helpful for predicting early chemotherapy response in DLBCL.

ABSTRACT

Objective To explore the relationship between CD+4CDhigh25CDlow127 Treg level of peripheral blood and NHL.Methods The level of peripheral blood CD+4CDhigh25CDlow127 Treg of 123 newly diagnosed NHL and 41 healthy adults were test by flow cytometry.Results The average peripheral blood CD+4CDhigh25CDlow127 Treg level were 5.0 % (2.7 %) and 7.2 %(3.8 %) in healthy adults and newly diagnosed NHL patients,respectively.The difference between the two groups was statistically significant(Z=-2.816 P=0.001 ).The peripheral blood Treg level was significantly higher in the male NHL patients than that in the female (7.4% vs 6.1%,Z=- 2.729,P=0.006).The peripheral blood Treg level was significantly higher in elevated LDH patients than that in normal LDH patients(7.8% vs 7.0%,Z=-2.623 P=0.008).The peripheral blood Treg level was significantly higher in stage Ⅲ-Ⅳ patients than that in stage Ⅰ -Ⅱ patients (7.3% vs 5.9%,Z=-2.057 P=0.04). There was no significant relationship between peripheral blood Treg level and age, IPI score,extranodal involvement,performance status,bulky disease,pathologic subtype or B symptom (P >0.05).Conclusions NHL patients are in an immunosuppresive statue.Patients of male,with inreased LDH level or in stage Ⅲ-Ⅳ have higher peripheral blood CD+4CDhigh25CDlow127 Treg level.

ABSTRACT

Objective To study the effect and safety of GDP regimens (gemcitabine,cisplatin,dexamethasone) as salvage chemotherapy in relapsed refractory non-Hodgkin’s lymphoma (NHL).Methods Clinical response and adverse effects of 34 NHL patients treated by GDP regimens were analyzed retrospectively.Results The overall response rate (ORR) was 52.9 % (18/34),there was no statistic difference between ORR in relapsed NHL [72.7 % (8/11)] and that in refractory ones [43.5 % (10/23)](P =0.11).The ORR for B-NHL was 64.0 % (16/25),while for T-NHL was 22.2 % (2/9),there was no statistic difference between them (P =0.052).The ORR for low risks groups was 66.7 % (6/9),while it for intermediate-risk and high-risk ones were 55.6 % (10/18) and 28.6 % (2/7),respectively,the effect of GDP regimens for low-risk group was better for high-risk ones,but there was no statistic difference (P =0.349).The incedience of Ⅲ-Ⅳ granulopenia reduced was 23.5 % (8/34) and there were no Ⅲ-Ⅳ gastrointestinal adverse reactions or liver and kidney toxicity.Conclusion GDP regimens were efficient and less toxic as second-line salvage chemotherapy.

ABSTRACT

Objective To study clinical characteristics and prognostic factors in systemic anaplastic large cell lymphoma (S-ALCL). Methods Clinical date of 30 systemic ALCL patients from Department Hemotology of Shanxi Cancer Hospital from December 1998 to September 2009 were reviewed. Results The median age of the 30 patients was 36 years old. The male/female ratio was 1.5:1.18 (60.0 %) 18/30 of the patients had B symptoms, 22 (73.3 %) had Ann Arbor stage Ⅲ-Ⅳ, and 18 (60.0 %) had extranodal disease at diagnosis. 14 (46.7 %) had elevated lactate dehydrogenase (LDH) level. The positive rate of ALK was 18 (60.0 %). Compared with ALK negative patients, ALK-positive patients were younger (u = 3.92, P=0.001).ALK expression and LDH level were important prognostic factors by Long-rank test. Conclusion S-ALCL usually occur in younger patients, with good response to chemotherapy and good prognosis. ALK negative and elevated LDH level are correlated to unfavorable prognosis. Combination chemotherapy is the major choice to these patients. High-dose chemotherapy maybe benefit to the patients who have adverse prognostic factors.

ABSTRACT

Objective To investigate the impact of genetic and environmental factors on mental health status in adolescents twins. Methods A total of 52 pairs of twins aged 11 years were recruited with support from educational committees and school. After the guardians of these twins had signed an informed consent form,the Chinese version growth and the state of health evaluation (Development and Well-Bing Assessment,DAWBA) carries on twins' mental health growth condition investigation to the twins of Age greater than 11-year-old,and focus on a-nalysis of DAWBA youth-assessment version of more than 11-year-old twins. Buccal mucosa samples were collected from all twins for DNA extraction and zygosity identification test. Results The mood, the behavior symptom and the symptom produced the influence of the DAWBA and in the computer diagnosis' s result showed that the intra-pair correlation coefficience of the emotional disorder(rMZ=0, 200, P = 0. 28; r DZ = 0. 198, P = 0. 447) (MZ means was monozygotic,DZ means was dizygotie, r means was correlation), the special phobia symptom(r MZ = 0.440, P = 0.013;rDZ =0.419, P=0.094),the social phobia (rMZ =0.2%, P = 0. 106; r DZ = -0.119, P = 0.648),the depressive symptom (rMZ =0.556, P=0.001; rDZ = -0.254, P=0.325), and the oppositional/con-duct disorder(rMZ =0.503, P = 0.014; rDZ = -0.270, P = 0.295),eating disorder(r MZ =0.764, P=0.046; rDZ = -0.091, P=0.728) in Monozygotic twins was more remarkable than that in dizygotie twin. Conclusion The genetic factors play the vital role in affects in the child twins' mental health growth.

ABSTRACT

Objective Confirmatory Factor Analysis(CFI)to the adolescent family Satisfaction Scale (AFSS).Methods Investigated 1306 subjects aged from 12 to 23 year old adolescent at school,used of CFI and cross-validation to confirm AFSS structure.Results Obtained revised structure of 3 factors and 23 items,AFSS including 10 items interpersonal atmosphere,7 items parental and children emotional interaction,6 items management style.Fit index X2/df less than 5,RMSEA less than 0.08,GFI,NFI,TFI,IFI more than 0.9,cross-validation and reliability were better.Conclusion The adolescent family satisfaction is first-order structure including three factors,AFSS is a reliable and validity scale for assessing the family satisfaction of adolescents.

Objective To investigate the mental health of the frequent hit-and-run driver,and to explore its relationship with their life events and personality.Methods Stratified cluster sampling method was used to draw 870 motor vehicle drivers,and chose the 620 met the study conditions.They were investigated by the demographic questionnaire,life event scale,symptom check list-90(SCL-90)and eysenck personality questionnaire (EPQ).Results (1)The SCL-90 total score and the factor scores of the accident group were significantly higher than the non-accident group(respectively,65.17±39.68 vs 18.65±13.85,9.24±5.44 vs 3.48±2.84,6.96±4.81 vs 1.83±2.11,10.19±7.19 vs 2.53±2.74,7.00±4.96 vs 1.48±1.77,4.68±3.66 vs 1.24±1.48,3.51±3.58 vs 0.62±1.29,3.84±3.36 vs 1.00±1.33,5.34±4.17 vs 1.27±1.72,8.93±6.54 vs 2.96±2.97,6.08±4.81 vs 2.11±2.18),the differences were significant(P<0.01).(2)The related analysis showed there existed the statistically positive relation between the total scores of SCL-90 and the factors of the life event scale and EPQ expect for the sense of concealing factor(r=0.146~0.393,P<0.05).(3)Analysis of regression showed the factors of life event and personality had different influences on total scores and factor scores of SCL-90,and negative life events were the most important predictor of SCL-90 total score of frequent hit-and-run driver.Conclusion There are many mental problems among the frequent hit-and-run drivers.Life events,mood and psychoticism factors of personality are important influencing factor to their mental health.

ABSTRACT

Objective To test different expression protein markers of the serum from B-cell non-Hodgkin lymphoma(B-cell NHL) between diffuse large B-cell lymphoma(DLBCL) and follicular lymphoma(FL)patients using sudace enhanced laser desorption/ionization-time of flight-mass spectrometry(SELDI-TOF-MS)protein chip technology. Further, to test different expression of the protein markers of B-eell NHL patients after chemical therapy in order to discuss clinical significance. Methods Different expression of protein markers were analysed in serum between 54 B-cell NHL patients and 27 healthy volunteers by using SELDI-TOF-MS WCX-2 pertein chip. Meanwhile different expression of protein markers relative to pathology classification between 23 DLBCL patients and 12 FL patients were screened; and protein markers which affected prognosis of 23 DLBCL patients were screened. Results There were five specific marker proteins in 54 B-cell NHL patients and 27 healthy volunteers. Their relative molecular weights were 7974, 15 938, 3398,8564, and 8692. The protein markers of 7974 and 15 938 were at high level in patients and the protein markers of 3398, 8564 and 8692 were at low level in patients. There were two protein markers which affected the prognosis, with better outcome when the expression of 4795 and 4998 were increased. Conclusion SELDI-TOF-MS protein chip technology is a quick, easy and convenient method, with high-throughput analyzer which can screen several relatively specific protein markers from the serum of patients to diagnose B-cell NHL The relatively specific protein markers can be used to make pathology classification and to judge the prognosis of B-cell NHL, and have better clinical value.

ABSTRACT

Objective To research correlation of serum protein spectrometry and lymphoma markers for diffuse large B cell lymphoma (DLBCL) patients. Whether there is relative between the protein and prognosis will be further researched. Methods Serum protein spectrometry of 62 DLBCL patients was detected by the SELDI-TOF-MS technique and Weak cation exchange 2 (WCX2) chip. Lactate dehydrogenase (LDH) was detected by biochemistry method. Beta-2-microglobulin (β2-MG) and cancer antigen125(CA125)were detected by enzyme-linked immunosorbent assay (ELISA). The level of LDH, β2-MG and CA125 for DLBCL patients between 11×103~12×103 protein expressed positively and negatively was analyzed. Meanwhile,correlation analysis and survival analysis were done. Results LDH, β2-MG and CA125 in 11×103~12×103protein expressed positive group were (523.30±435.96)U/L, (3.23±1.24)mg/L, (81.07±61.39)U/L respectively,and they were higher than that in negative group (P<0.05). 11×103~12×103 protein was positive correlated to LDH, β2-MG and CA125 (P<0.01). The survival time in 11×103~12×103 protein expressed in positive group,in which median survival time was 11 months, was shorter than that in negative group(P <0.01). The survival time in LDH normal group was longer than that in increased group(P <0.01). The survival time of β2-MG and CA125 had no significant difference between increased group and normal group. Conclusion LDH and 11×103~12×103 protein are expected to be prognosis indicators for DLBCL patients.
