ABSTRACT
Laboratory medicine is an important subject of auxiliary clinical diagnosis, and the postgraduate education of clinical laboratory diagnostics is the key to cultivating high-end laboratory talents. This study analyses the differences in the postgraduate education model of clinical laboratory diagnostics between China and Australia from the four aspects of academic degree system, curriculum setting, teaching methods, and autonomous learning ability training and discusses the postgraduate education reform of laboratory medicine in China. With reference to the advanced experience of countries with a well-developed education system, the education reform in China can improve the comprehensive quality of postgraduate student in laboratory medicine and enhance the medical level of China.
ABSTRACT
Objective To develop an oxybutynin nanotransdermal formulation with high skin permeability based on cubsomes carrier.Methods The oxybutynin loaded cubsomes(OXY-CS)was prepared by ultrasonic method.Using particle size,polydispersity index and encapsulation efficiency as evaluation indexes,stirring temperature,stirring time,stirring speed,ultrasound time,ultrasound power,glycerol monooleate/poloxamer 407 ratio(GMO/F127),carrier(GMO+F127)/drug ratio and drug concentration were optimized.The effects of drug concentration,permeation promoter and gel matrix type on skin permeability were investigated using cumulative permeation and skin retention as evaluation indicators.Results The prepared OXY-CS had small particle size and high encapsulation efficiency at stirring temperature of 50℃,stirring time of 30 min,stirring speed of 1 000 r/min,ultrasound time of 10 min,ultrasound power of 100 W,GMO/F127 ratio of 2∶1,carrier/drug ratio of 3∶1 and drug concentration of 1%,respectively.OXY-CS had high cumulative penetration and skin retention at concentration ≥ 3%.The addition of a permeation promoter to OXY-CS did not increase the cumulative penetration of the drug.The skin permeability of oxybutynin cubsomes gel made with carbomer was significantly better than those of other gel matrices.Conclusion In this study,oxybutynin is made into a topical cubsomes gel,which is expected to reduce the incidence of adverse effects associated with oral administration.

Objective:To construct a eukaryotic expression vector for bacteriophage D29 LysinB/Holin fusion protein and study its bactericidal efficacy against Mycobacterium tuberculosis ( Mtb) in a cell infection model. Methods:A recombinant plasmid pET32a-LysinB was constructed and induced to express LysinB. The polyclonal antibody against LysinB was prepared after the purification of LysinB. A recombinant plasmid pcDNA3.1(+ )-LysinB/Holin was constructed and transfected into mononuclear macrophages RAW264.7. After the expression of the prepared polyclonal antibody was identified, a cell infection model was established and the bactericidal efficacy of LysinB/Holin fusion protein was measured by acid-fast staining and colony counting.Results:The polyclonal antibody against LysinB was successfully prepared. The recombinant plasmid pcDNA3.1(+ )-LysinB/Holin could effectively express LysinB/Holin fusion protein in eukaryotic cells without inducing significant cytotoxicity. LysinB/Holin fusion protein was effective in killing Mtb in cells. Conclusions:The recombinant plasmid pcDNA3.1(+ )-LysinB/Holin has a better killing effect on intracellular Mtb without inducing obvious cytotoxicity against eukaryotic cells, showing a potential in the treatment of tuberculosis.

Apoptosis is particularly important for the body to inhibit or eliminate Mycobacterium tuberculosis ( Mtb) infection. A series of mechanisms to regulate macrophage apoptosis are triggered after macrophages are infected by Mtb. In Mtb-infected macrophages, there are differentially expressed microRNAs (miRNAs), which can directly bind to the binding sites in the 3′-untranslated region (3′ UTR) of apoptotic genes to regulate the expression of apoptotic genes and the apoptosis of macrophages through the mitochondrial or death receptor pathway. This paper reviewed the miRNAs associated with the apoptosis of Mtb-infected macrophages and the major mechanisms.

The realization of this purpose depends on the construction of an innovative practical teaching curriculum system. This article combines the four modules of basic practice, professional practice, vocational skill practice, and innovative practice to improve the problems existing in the traditional practice teaching system, and proposes a "3+1" three-dimensional practice teaching model in order to adapt to the rapid development of medical laboratory technology.

As an important course in medical laboratory science, clinical immunology is composed of advanced immunological knowledge and advanced testing technology, which are closely related to many disciplines, such as basic medicine, clinical medicine and so on. Medical educators introduced case-based teaching into clinical immunology, selected the typical, enlightening and interesting clinical cases, designed some reasonable questions for students majoring in medical laboratory science and improved students' participation via pre-class discussion and answering their questions during class, so as to help them study actively. Case-based teaching has been proved through practice as one of the important ways to improve the learning efficiency of medical students and promote their overall development, and it is a successful method for teaching reform.

OBJECTIVE@#To analyze the characteristics of cervical microecology in late reproductive-age women with cervical lesions and explore new methods for preventing cervical lesions.@*METHODS@#Cervical smears were obtained from a total of 147 women of late reproductive age, including 24 with high-risk HPV infection (HR-HPV), 27 with low-grade squamous intra-epithelial lesions (LSIL), 36 with high-grade squamous intra-epithelial lesions (HSIL), 35 with cervical cancer (CC) and 25 healthy women. llumina MiSeq sequencing of V3-V4 region of the 16S rRNA gene amplicons was used to characterize the vaginal microbiota of the women. OTUs analysis of the valid data was performed, and the α-diversity (Chao1, Simpson's Index and Shannon Index) and β-diversity (T-test, weighted UniFrac β diversity, and MetaStat analysis) were evaluated.@*RESULTS@#Dilution curve and species accumulation boxplot validated the quality of the samples. Pyroptosis is an inflammatory programmed cell death dependent on caspase, and partic-ipates in the development of infectious diseases. Mycobacterium tuberculosis can reside in macrophages, and therefore Mycobacterium tuberculosis infection is closely related to the pyroptosis of macrophages. This review reviewed and discussed the molecular mechanism of interactions between Mycobacterium tuberculosis and macrophage inflammasomes. It is helpful to further understand the occurrence and development of tuberculo-sis and provide new ideas for the prevention and control of Mycobacterium tuberculosis infection with the in-depth study of the mechanism of macrophage pyroptosis.

Interaction in classroom teaching is a bilateral activity that is difficult to control. Teachers and students are the two main aspects to realize the synchronization of teachers and students' thinking. For teachers, teachers should first make full use of multimedia resources, bring a colorful courseware into class-room teaching; secondly, teachers should stimulate students' interest in a reasonable way, so as to inject vitality into the classroom teaching; finally, teachers should mobilize the students' thinking and carefully regulate the changes in thinking between teachers and students through a proper teaching methods. The adjustment of Laboratory Medicine to the first class subject is both challenges and opportunities. In order to meet the requirements of the modern laboratory medicine on the improved knowl-edge and skill, Weifang Medical University sets a goal of cultivatinghigh-qualified and applicative profes-sional personnel of Laboratory Medicine Science. To realize that, we build a new curriculum system with features of Medical Laboratory Science, including the theoretical curriculum system and the practical teach-ing system. The theoretical curriculum system breaks the traditional framework of the old curriculum which designed according to the discipline category, builds a new curriculum system of platform and module, scientifically integrates the basic course group in clinical laboratory medicine, and adds the course of clinical examination skills with its own professional characteristics. In the practical teaching system, we focus on the cultivation of practice ability and innovation ability, encourage the students to go to the hospital and company earlier, and promote the integration of production, teaching and research in various approaches.
ABSTRACT
Objective To reduce the screening positive rate (SPR) and improve clinical efficiency of maternal serum screening for Down's syndrome.Methods Nine thousand and thirty-three cases of second trimester maternal serum screening for Down's syndrome were included from Apr.2013 to Apr.2014 in the present study.The screening results,all basic data and equation curves were analyzed retrospectively.Based on the data from the authors' laboratory,the important adjustment parameters were simulated.Combined with postnatal follow-up results,the quality and clinical performance of second trimester serum screening for Down's syndrome were evaluated.Results The SPR of second trimester serum screening for Down's syndrome was 6.69%(604/9033),the detection rate (DR) was 75%(3/4),and FPR was 6.65%(601/9033).The median multiple of median (MOM) of alpha-fetoprotein (AFP) was low and SPR was high,and MOM of free human chorionic gonadotropin β subunit (free hCGβ) were high and SPR was high,while MOM of unconjugated estriol (uE3) were a little bit low,and SPR was slightly high.Considering these three factors,it is believed that the screening positive rate is high.By the simulation adjustments of MOM value equations (AFP and free hCGβ) and weight correction equation,the SPR reduced to 4.11%(371/9033) after recalculating the risk,FPR declined to 4.07%(368/9033),and no more Down's syndrome fetus were missed compared with postnatal follow-up results.Conclusion Based on a localized setting depending on the local laboratory data,we suggest that the MOM value distributions(AFP,free hCGβ and uE3) and maternal weight should be regularly adjusted since it is a useful way to reduce the false-positive rate and improve clinical efficiency of maternal serum screening for Down's syndrome.

Objective To amplify the higA gene from the Mycobacterium tuberculosis,and to analyze the structure and function of their encoded proteins by using bioinformatics.Methods Total DNA was extracted from Mycobacterium tuberculosis.PCR of higA was performed and the products were sequenced.The biological features of the higA protein including,its physical and chemical properties,signal peptide,spatial structure and epitopes were analyzed by using software online.Results The PCR products of higA were 450 bp in length,which were consistent with the expected size.The higA protein consisted of 149 amino acids and had the following characteristics:a theoretical isoelectric point of 7.93,a fat-soluble factor of 94.30,and instability coefficient of 36.57.The higA protein had no signal peptide,containing 10 phosphorylation sites and multiple potential epitopes.Conclusion Mycobacterium tuberculosis higA gene can be amplified by PCR and the characteristics of higA protein is identified.

The culture of modern medical talents is the requirements of the times on cultivating talents of Medical Laboratory of Medical College. In order to establish a high level teaching team and im-prove the quality of teaching and personnel training, through the reform of the traditional teacher training way, we created the teacher training mode of multi echelon teachers including department, university, domestic and international training ways. New curriculum teaching system framework was established which was based on theoretical teaching, practical teaching and scientific research. Besides, innovative and pro-gressive practice teaching platform was set up which included basic experiment, clinical skills, professional training, and practice in company. Course of Clinical Examination Skills was opened. The teaching team achieved some results in the above teaching reform.

Construction of course group,which overcomes the narrow limits of knowledge structure for single course,carries training objectives of comprehensive knowledge and innovation ability,and plays an important role in realizing the goal of higher medical education.Basic course group in clinical laboratory medicine has been built based on the close relation,promotion,and infiltration of five courses contents.It has been explored and reformed from the restructuring and optimizing of course group contents,teaching methods,practice step,and so on.Practice shows that the course group is helpful to improving students' ability of integrated use of knowledge and presents obvious effect for improvement of the cultivation of students' comprehensive quality.
