ABSTRACT

OBJECTIVE@#To explore the genetic basis for a Chinese patient with amyotrophic lateral sclerosis (ALS).@*METHODS@#Peripheral blood samples were collected from the patient and his parents for the extraction of genomic DNA. Genetic variant was identified by whole exome sequencing. Candidate variant was verified by Sanger sequencing of his parents and healthy controls.@*RESULTS@#The patient was found to harbor a heterozygous c.420C>G (p.Asn140Lys) variant of the SOD1 gene. The same variant was not detected in his parents and 100 healthy controls. The variant has not been included in HGMD, dbSNP and other databases.@*CONCLUSION@#The c.420C>G variant of the SOD1 gene may underlie the ALS in this patient. Above finding has enriched the spectrum of SOD1 gene variants.

ABSTRACT

OBJECTIVE@#To analyze the pathogenic variant of preaxial polydactyly in a Chinese Han pedigree and identify the cause of polydactyly.@*METHODS@#The peripheral blood DNA of the proband and her parents was extracted. The polydactyly-related genes were detected by trio whole exome sequencing, and the suspected pathogenic gene was screened out. Sanger sequencing was applied to other members of the pedigree.@*RESULTS@#The results of gene sequencing showed that the LMBR1 gene had a heterozygous variant of c.423+4909(IVS5)C>T in 6 patients of the pedigree. The same variant was not detected in family members with normal phenotype. Based on the ACMG guidelines, c.423+4909(IVS5)C>T of the LMBR1 gene was predicted to be pathogenic (PM1+PM2+PP1-S(PS)+PP4+PP5).@*CONCLUSION@#The heterozygous C>T variant at position 4909 of intron 5 of the LMBR1 gene probably underlies the disease in this pedigree.

ABSTRACT

Objective:To investigate the dynamic expression of histone methyltransferase (enhance of zeste homolog 2, EZH2) in peripheral blood B lymphocytes (CD19 +B) and memory B lymphocytes (CD19 +CD27 +B) of septic patients and its value in predicting prognosis in sepsis. Methods:From June 2018 to January 2020, 48 septic patients in the Intensive Care Unit of Shanghai East Hospital were enrolled, and 40 healthy adult volunteers were recruited as healthy controls. Septic patients were divided into the non-survivors (18 cases) and the survivors (30 cases) according to whether the patients survived at 28 days. Blood samples were collected at day 1, 3 and 7, blood routine, IL-6 and blood gas analysis were collected, and SOFA and APACHE Ⅱ scores were counted. Flow cytometry was used to detect the positive rate and the mean fluorescence intensity of EZH2 on CD19 +B lymphocytes, and the positive rate of EZH2 on CD19 +CD27 +B lymphocytes at different time points. In the healthy controls, fasting was taken only once in the morning. ROC curve was drawn and the area under the curve (AUC) was calculated to evaluate the value of expression of EZH2 on CD19 +B lymphocytes and CD19 +CD27 +B lymphocytes in predicting the prognosis of septic patients. Results:(1) Compared with the healthy controls, the positive rate and average fluorescence intensity of EZH2 on CD19 +B lymphocytes and the positive rate of EZH2 expression on CD19 +CD27 +B lymphocytes were significantly increased at day 1, 2 and 3 in septic patients ( P<0.05). Over time, the expression of EZH2 in CD19 +B lymphocytes and CD19 +CD27 +B lymphocytes increased gradually ( P<0.05). (2) Compared with the survivors, the positive rate of EZH2 on CD19 +B lymphocytes of the non-survivors was increased at day 1, but the positive rate of EZH2 on CD19 +CD27 +B lymphocytes of the non-survivors was decreased at day 3 and 7 ( P<0.05). (3) The positive rate of EZH2 on CD19 +B lymphocytes, APACHE Ⅱ score, SOFA score and IL-6 level in septic patients at day 1 were independently associated with 28-day mortality. (4) The AUC of APACHEⅡ score was 0.907 (95% CI: 0.825-0.990), and the sensitivity and the specificity were 88.89% and 76.67%. The AUC of SOFA score was 0.831 (95% CI: 0.706-0.955), and the sensitivity and the specificity was 66.67% and 86.67%; The AUC of EZH2 positive rate on CD19 +B lymphocytes were 0.799 (95% CI: 0.657-0.941), and the sensitivity and specificity were 88.89% and 80.77%, respectively, the sensitivity was better than SOFA score, and the specificity was higher than APACHEⅡ score. Conclusions:The high expression of EZH2 on B lymphocytes in septic patients is associated with poor prognosis. Dynamic monitoring of EZH2 expression on B lymphocytes has certain predictive value for sepsis.

ABSTRACT

Objective:By using the network pharmacology method to predict the active constituents and action targets of Suzi-Jiangqi Decoction in the treatment of COPD, and to explore its potential molecular mechanism with multi-component, multi-target and multi-pathway characteristics. Methods:The active constituents and targets of Suzi-Jiangqi Decoction were collected, screened and predicted according to the Traditional Chinese Medicine Systems Pharmacology Database and Analysis Platform (TCMSP) and UniProt software. Search for the gene targets related to COPD in GeneCards, online human Mendelian genetic database (OMIM) and TTD database. The intersection targets of component targets and disease targets were obtained by Veen map online software. The network model with the sequence of active constituents-target-disease was constructed and analyzed by Cytoscape software, and the protein-protein interaction network (PPI) was constructed by STRING database. The gene ontology function annotation (GO) and Tokyo genome encyclopedia (KEGG) pathway enrichment analysis of common targets with metascape online tool. Results:A total of 163 active constituents of Suzi-Jiangqi Decoction were screened, 283 targets were predicted, and 159 targets involved in the treatment of COPD. Quercetin, kaempferol, naringin and luteolin were the key active ingredients. IL6, TNF, MAPK3, JUN, CASP3, CXCL8, CXCL10, MMP9 and MAPK1 were important gene targets. GO analysis showed that the biological processes involved in the enrichment of key gene targets included the response to bacteria, the cytokine mediated signaling pathway, the cell's response to inorganic substances, the response to oxidative stress, the response to LPS, and so on. The enrichment analysis of KEGG pathway showed that the signaling pathway of Suzi-Jiangqi Decoction in the treatment of COPD included TNF signaling pathway, IL-17 signaling pathway, cell cycle, Influenza A, HTLV-I infection, AGE-RAGE signaling pathway, Tuberculosis, Epstein-Barr virus infection and so on. Conclusion:Suzi-Jiangqi Decoction can treat COPD through multi-target and multi-pathway mechanisms of anti-inflammatory, anti infection and immune regulation, which lays a foundation for further study of its molecular mechanism.

Objective:The construction of public medical group is an important strategic measure to deepen the health reform in China. Taking Beijing Traditional Chinese Medicine Hospital as an example, the article introduces the measures, achievements, existing problems and possible development direction of the discipline co-construction in the entrusted district hospital.Methods:The article analyzes the measures of discipline co-construction and its implementation effect in detail.Results:It has effectively promoted the reasonable allocation of resources between " well-developed leading disciplines" and " disciplines to be nurtured" in the medical group, these disciplines entering a new rapid development phase through optimized organization and management, system construction, new research platform construction.Conclusions:" Disciplines to be nurtured" has been improved in medical service, scientific research capabilities, talent training, academic inheritance and quality of scientific research achievements, based on which, the goal of promoting the development of medical group through discipline co-construction was achieved.

ABSTRACT

Objective:To evaluate the perioperative safety of pancreaticoduodenectomy(PD)in elderly patients.Methods:Clinical data of 152 patients undergoing PD in Beijing Hospital between 2016 and 2019 were retrospectively analyzed.According to the age, patients were divided into the elderly group(age≥65 years)and the non-elderly group(age<65 years). The perioperative and postoperative parameters were compared between the two groups.Results:There was no significant difference in the operative time, intraoperative blood loss and intraoperative urine output between the elderly group and the non-elderly group(352.7±69.5)min vs.(359.0±94.4)min, (708.7±672.7) vs.(662.6±896.7)ml and(875.8±497.3)ml vs.(1010.6±568.2)ml, all P>0.05). The perioperative mortality, postoperative complications(Clavien-Dindo classification ≥ grade Ⅲ)and reoperation rate had no significant difference between elderly group and the non-elderly group(6.1% vs.1.2%, 21.2% vs.19.8%, 10.6% vs.8.1%, respectively, χ2=1.487, 0.048 and 0.272, all P>0.05). The incidences of operation-related complications, cardiovascular or cerebrovascular adverse events in elderly group were comparable to non-elderly group( P>0.05). The postoperative hospital stay had no significant difference between the elderly group and the non-elderly group[(28±19)d vs.(27±18)d, P>0.05]. Conclusions:PD is safe in elderly patients.Age is not a contraindication to pancreaticoduodenectomy.The comprehensive systemic evaluation, intraoperative delicate operation and enhanced perioperative management are necessary in the elderly patients undergoing PD.

ABSTRACT

Objective:To explore the effect of applying management by objective (MBO) and key performance indicator (KPI) method in the quality management of physical examination.Methods:The MBO and KPI methods were used to determine the focus of annual physical examination quality control, and to set 9 key physical examination quality control indicators and assessment standards. The results of physical examination quality control in the first quarter of 2018 were selected as the control group, and those in the fourth quarter of 2018 as the observation group. The two groups were compared using the chi-square test or Fisher′s exact test, after which the application effects of MBO and KPI method were discussed.Results:The scores of physical examination quality control in the observation group were higher than those in the control group; however, the difference was not statistically significant. The key indicators of physical examination quality control in the observation group all showed improvement to different degrees when compared with the control group. Among these indicators, the qualified rate of staff hand hygiene compliance, the qualified rate of cardiopulmonary resuscitation operation, and the coincidence rate of major abnormal results of physical examination were increased, but the difference was not statistically significant (95.0% vs 87.5%, P=0.432; 97.5% vs 92.5%, P=0.615; 55.6% vs 51.0%, P=0.232, respectively). Results error rate of physical examination department, completion rate of physical examination quality control documents, client satisfaction, follow-up success rate of major abnormal results of physical examination, and qualified rate of physical examination report were all significantly improved, with statistical significance (1.2% vs 2.2%, P<0.001; 100.0% vs 85.0%, P=0.026; 98.7% vs 96.8%, P<0.001; 68.6% vs 62.6%, P=0.014; 96.2% vs 90.6%, P<0.001, respectively). Conclusion:The application of MBO and KPI methods in the physical examination quality management of physical examination institutions can effectively improve the quality of physical examination.

Kidney transplantation can improve the quality of life of end-stage renal disease patients and is the most effective treatment.But renal transplantation is also a high risk factor for bladder cancer.Bladder cancer is a common malignant tumor in department of urology.Bacillus Calmette-Guerin intravesical instillation is a first-line therapy for high-risk non muscle invasive bladder cancer, which can directly kill bladder cancer cells.We reviewed the mechanism and clinical application of BCG in the treatment of bladder cancer after renal transplantation, in order to provide references for better utilization of Bacillus Calmette-Guerin in adjuvant treatment of bladder cancer after renal transplantation.

ABSTRACT

OBJECTIVE:To prepare chelerythrine nanoparticles(CHE-NPs),optimize their formulation ,and evaluate its drug release behavior in vitro and its inhibitory effect on melanoma. METHODS :Using methoxy polyethylene glycol-poly (lactic-co- glycolic acid )(mPEG-PLGA)as carrier ,CHE-NPs were prepared by the nano-precipitation method. HPLC method and dialysis bag method were used to determine entrapment efficiency and drug loading. The formulation of CHE-NPs was optimized by Box-Behnken response surface design using overall desirability (OD)of them as dependent variables ,CHE dosage ,mPEG-PLGA concentration and poloxamer 188(F68)concentration as independent variables. The particle size and Zeta potential of CHE-NPs prepared by the optimal formulation were detected ;the characteristics of drug release in vitro were investigated ;the effects of CHE and CHE-NPs on survival rate of mice B 16 melanoma cells were compared ,and median inhibition concentrations (IC50)of them were calculated. RESULTS :The optimal formulation included CHE of 2 mg,mPEG-PLGA of 13 mg/mL,F68 of 1.8%. Average entrapment efficiency rate of CHE-NPs prepared by the optimal formulation was (80.18±1.11)%,average drug loading was (11.36±0.28)%,average OD value was 0.96±0.04 [the relative deviation from predicted value (0.90)of OD was 6.67%]; particle size was (113.1±1.40)nm,and Zeta potential was (-21.6±0.29)mV;polydispersity index was 0.07±0.01(n=3); accumulative release rates of CHE control and CHE-NPs were 90.87% and 68.68% within 8 h,and drug release behavior in vitro of the latter was in accordance with Weibull kinetic model. Inhibitory effect of CHE-NPs on B 16 melanoma cells was significantly stronger than that of CHE ;the 24 h IC 50 of CHE-NPs and CHEwere 69.35 and 107.36 μg/mL,respectively. CONCLUSIONS :The prepared CHE-NPs show good sustained-effect and high capacity of drug loading ,and strengthen the inhibitory effect of CHE on melanoma.

ABSTRACT

Objective:To investigate the mechanism of joint cavity injection of Dioscoreae Rhizoma polysaccharides (DRP) in protecting against cartilage degeneration and inhibiting the expression of inflammatory factors in the rabbit model of knee osteoarthritis to provide relevant references for the development and further research on DRP. Method:Fifty-five New Zealand white rabbits were selected for the induction of knee osteoarthritis model by the modified Hulth's modeling method. The model rabbits were randomly divided into a model group, a sodium hyaluronate group (1.00 mg·kg<sup>-1</sup>), and low- (0.7 mg·kg<sup>-1</sup>), medium- (1.43 mg·kg<sup>-1</sup>), and high-dose (2.15 mg·kg<sup>-1</sup>) DRP group according to a random number table. One week after modeling, the rabbits in the groups with drug intervention were treated correspondingly for five weeks, once per week, and no intervention was performed in the model group. Five weeks later, the joint specimens were observed by visual observation. The articular cartilage tissues were observed under the light microscope for pathological sections and scores by hematoxylin-eosin (HE) staining and toluidine blue (TB) staining. The expression levels of interleukin-6 (IL-6), interleukin-1<italic>β</italic> (IL-1<italic>β</italic>), and tumor necrosis factor-<italic>α </italic>(TNF-<italic>α</italic>) in the synovial fluid were measured by enzyme-linked immunosorbent assay (ELISA), and the expression of matrix metalloproteinase-13 (MMP-13), transforming growth factor-<italic>β</italic><sub>1</sub> (TGF-<italic>β</italic><sub>1</sub>), and type Ⅱ collagen (Col-Ⅱ) in the articular cartilage were measured by immunohistochemistry. Result:After five weeks of DRP intervention, compared with the model group, the DRP groups exhibited lowered levels of IL-6, IL-1<italic>β</italic>, and TNF-<italic>α</italic> in the synovial fluid (<italic>P</italic><0.05), reduced expression of MMP-13 in the articular cartilage (<italic>P</italic><0.05), and increased levels of TGF-<italic>β</italic><sub>1 </sub>and Col-Ⅱ (<italic>P</italic><0.05). Compared with the low-dose and high-dose DRP groups, the medium-dose DRP group showed reduced levels of IL-6, IL-1<italic>β</italic>, and TNF-<italic>α</italic> in the knee joint (<italic>P</italic><0.05), increased levels of TGF-<italic>β</italic><sub>1</sub> and Col-Ⅱ in cartilage tissues (<italic>P</italic><0.05), and dwindled level of MMP-13 (<italic>P</italic><0.05). Compared with the sodium hyaluronate group, the medium-dose DRP group showed no significant differences in IL-6, IL-1<italic>β</italic>, and TNF-<italic>α</italic> in rabbit knee joints, and TGF-<italic>β</italic><sub>1</sub>, Col-Ⅱ, and MMP-13 in cartilage tissues. Conclusion:Joint cavity injection of DRP can significantly reduce the expression of IL-6, IL-1<italic>β</italic>, and TNF-<italic>α</italic> in rabbit synovial fluid, effectively inhibit the expression of MMP-13 in the articular cartilage to suppress the degradation of articular cartilage collagen and promote the synthesis of TGF-<italic>β</italic><sub>1</sub> and Col-Ⅱ. Therefore, DRP can protect and repair articular cartilage to delay the degeneration of articular cartilage.

ABSTRACT

Objective:To investigate the effect of body weight-supported treadmill training on neuropathic pain and expression of glutamate decarboxylase-65/67 (GAD-65/67) in spinal dorsal horn of rats with spinal cord injury (SCI). Methods:A total of 24 Sprague-Dawley rats were randomly divided into sham group, SCI-sedentary (SCI-Sed) group and SCI-Exercise (SCI-Ex) group, with eight rats in each group. Allen's method was used to make T10 incomplete SCI model. Seven days after SCI, SCI-Ex group was given body weight-supported treadmill training. Before SCI, and seven days, 14 days, 21 days, 28 days and 35 days after SCI, the von Frey filaments and thermal stimulation pain tester were used to evaluate the mechanical and thermal pain thresholds. Then, Western blotting and immunohistochemical analysis were performed on the spinal cord of all rats to detect the expression of GAD-65 and GAD-67. Results:The mechanical and thermal pain thresholds were higher in SCI-Ex group than in SCI-Sed group 21 days, 28 days and 35 days after SCI (P < 0.01). Compared with the sham group, the expression of GAD-65 and GAD-67 decreased in SCI and SCI-Ex groups (P < 0.05), and increased in SCI-Ex group compared with that of SCI-Sed group (P < 0.05). Conclusion:Body weight-supported treadmill training could increase the synthesis of GAD-65/67 in the distal spinal cord dorsal horn of incomplete SCI rats, and improve the pain thresholds of hind limbs in rats with SCI.

ABSTRACT

Objective:To observe the affected elbow flexor activities and balance of stroke patients in standing and walking as fixing the affected upper extremity with different methods. Methods:From September to December, 2018, 14 stroke patients from Affiliated Suzhou Science & Technology Town Hospital of Nanjing Medical University were included. They were measured elbow flexor contraction ratio (integrated electromyogram of flexor/integrated electromyogram of antagonist) with wireless surface electromyography as standing and walking in normal speed, and sway index with Biodex as standing, in four conditions: the affected arm without fixing, and fixed with elbow orthosis, and with unaffected hand backwards and forwards. Results:The contraction ratio was the most as the arm fixed with elbow orthosis in both standing and walking (F > 2.687, P < 0.05). The sway index was the most as the arm fixed with unaffected hand forwards (F = 21.091, P < 0.05). Conclusion:The elbow orthosis restraint may lead to increased contraction of the affected elbow flexor, which is unsatisfactory to inhibit flexor activity of the upper extremities; while restraint of unaffected hand forwards may lead to instable balance.

OBJECTIVE@#To explore the genetic basis for a case of Lamb-Shaffer syndrome.@*METHODS@#Genomic DNA was extracted from peripheral blood samples and subjected to whole exome sequencing(WES). Suspected variant was verified by Sanger sequencing.@*RESULTS@#The patients was found to harbor a heterozygous c.1495delA(p.Thr499Glnfs*5) frameshift variant of the SOX5 gene by WES. Sanger sequencing confirmed that the same variant was a de novo variant. Based on the American College of Medical Genetics and Genomics guidelines, c.1495delA(p.Thr499Glnfs*5) variant of the SOX5 gene was predicted to be pathogenic (PVS1+PS2+PM2).@*CONCLUSION@#The c.1495delA(p.Thr499Glnfs*5) variant of the SOX5 gene probably underlies the Lamb-Shaffer syndrome in this patient.

ABSTRACT

Objective To investigate the clinical manifestations,diagnosis,treatment,and laboratory examination characteristics of 8 pemphigus patients with high titers of anti-desmoglein antibodies in remission. Methods A retrospective study was conducted for the pemphigus patients diagnosed and treated in the department of dermatology from January 2013 to September 2020.The patients should have the serum anti-desmoglein antibodies ≥150 U/ml in remission or the antibody levels dropped less than 20%(calculated based on the maximum detection limit of 150 U/ml)of their initial ones detected before treatment,and the clinical and laboratory data of patients eligible for the inclusion criteria were collected. Results Among the 134 pemphigus patients with available follow-up data during this period,a total of 8 patients met the criteria,with the follow-up period of 21-85 months and the remission duration of 18-70 months.They all received less than or equal to 10 mg/d prednisone and had high titers of anti-desmoglein antibodies.At their first visit,the number of patients with positive anti-desmoglein 1/desmoglein 3 antibodies was 7.Two patients still had high titers of anti-desmoglein 1 antibodies 19 months and 21 months after they achieved remission,and 5 patients had high titers of anti-desmoglein 3 antibodies in 18-70 months.There was one patient showing high titers of both antibodies,especially for anti-desmoglein 1 antibodies.This patient relapsed after 19 months' remission while other patients were still in clinical remission. Conclusions Some pemphigus patients showed persistent high titers of anti-desmoglein antibodies in remission.Anti-desmoglein 3 antibodies were more common to keep positive,while high titer of anti-desmoglein 1 antibodies was less observed.The high titer of anti-desmoglein 1 antibodies had a correlation with recurrence.For the pemphigus patients with long-term clinical remission but high antibody titer,the dosages of corticosteroids should be adjusted carefully according to their actual clinical manifestations and the positive antibody type.For the patients with high titer of anti-desmoglein 1 antibodies,the dosage reduction of corticosteroids should be appropriately slower.

ABSTRACT

@#Objective    To evaluate the effect of smoking and drinking status on the prognosis of patients with esophageal squamous cell carcinoma (ESCC). Methods    The clinical data of 483 patients with ESCC who underwent surgical treatment in Shannxi Provincial People's Hospital from 2007 to 2016 were retrospectively analyzed. Among them, 352 patients were male and 131 were female, with a median age of 64 (37-80) years. There were 311 smokers and 172 drinkers. The relationship between preoperative drinking or smoking status and the clinicopathological characteristics of patients with ESCC was analyzed. Log-rank method and Cox risk regression were used to conduct univariate and multivariate survival analysis, respectively. Results    The preoperative smoking status was related to the patient's tumor location (P=0.030). Drinking status was associated with tumor location (P=0.001), degree of differentiation (P=0.030), pathological T stage (P=0.024) and pathological N stage (P=0.029). Univariate survival analysis showed that smoking status did not affect the disease-free survival (DFS) (P=0.188) and overall survival (OS) (P=0.127) of patients with ESCC. However, patients who drank alcohol had worse PFS than non-drinking patients (29.37 months vs. 42.87 months, P=0.009). It was further proved that alcohol consumption was an independent risk factor affecting patients' recurrence and metastasis by using multivariate analysis (RR=1.28, P=0.040). Alcohol consumption also reduced the OS of patients by 21.47 months (P=0.014), however, multivariate analysis did not yield significant results. Conclusion    Preoperative drinking status is related to the stage and differentiation of patients with ESCC. It is an independent risk factor affecting the recurrence and metastasis of ESCC.

ABSTRACT

In recent years, the phenomenon of glass transition has been gradually applied to the field of pharmaceutics. And it exhibits important influences on multiple operating units of pharmaceutical preparations, and the properties and storage of pharmaceutical intermediates and products. At present, it has been widely used in the process of preparations such as drying, granulation, coating, tableting, holt-melt extrusion, cryogenic comminution, and so on. Meanwhile, it showed guiding significance for the process of preparation intermediates and their products, such as solid dispersion, microcapsule, liposome, particle, tablet, and other preparation intermediates and their products. Therefore, this article conducts a detailed analysis and systematic summary of the application guidance of the phenomenon of glass transition in the preparation process, and its influence on the preparation intermediates and products, so as to provide theoretical guidance for preparation production and product storage.

ABSTRACT

Traditional Chinese medicine (TCM) preparations have made tremendous progresses in modernization, whereas there exist relatively few researches pertaining to preparation structures. As demonstrated by the theory and practice of structure pharmaceutics, the structure properties of dosage forms have significant influences on the quality and efficacy of drugs, which might offer reference for the research and development of TCM dosage forms. With the application of synchrotron radiation X-ray micro-computed tomography (SR-μCT) and other novel technologies in recent years, researches in structure pharmaceutics have made huge advancement, which provide reference and methodology basis for the study of TCM preparations. The article generalized and summarized the recent progresses and methods in the structure researches of pharmaceutics and TCM preparations, and further explored the significance of the researches of structure of TCM preparations. It is expected to provide the basis for the dosage form design, production process improvement, and quality evaluation of TCM and promote the modernization of TCM preparations.

OBJECTIVE@#To detect the mutation site in a pedigree affected with autosomal dominant polycystic kidney disease (ADPKD) and verify its impact on the protein function.@*METHODS@#Peripheral blood samples were collected from the proband and his pedigree members for the extraction of genomic DNA. Mutational analysis was performed on the proband through whole-exome sequencing. Suspected variant was verified by Sanger sequencing. A series of molecular methods including PCR amplification, restriction enzyme digestion, ligation and transformation were also used to construct wild-type and mutant eukaryotic expression vectors of the PKD2 gene, which were transfected into HEK293T and HeLa cells for the observation of protein expression and cell localization.@*RESULTS@#The proband was found to harbor a c.2051dupA (p. Tyr684Ter) frame shift mutation of the PKD2 gene, which caused repeat of the 2051st nucleotide of its cDNA sequence and a truncated protein. Immunofluorescence experiment showed that the localization of the mutant protein within the cell was altered compared with the wild-type, which may be due to deletion of the C-terminus of the PKD2 gene.@*CONCLUSION@#The c.2051dupA (p. Tyr684Ter) mutation of the PKD2 gene probably underlay the pathogenesis of ADPKD in this pedigree.

ABSTRACT

OBJECTIVE:To systematically evaluate the effectiveness and safety of low-m olecular-weight heparin combined with progesterone in the treatment of repeated implant failure (RIF)in embryo transfer patients ,and to provide reference for clinical use of it. METHODS :Retrieved from Cochrane library ,PubMed,Embase,CNKI and Wanfang database ,randomized controlled trail (RCT)or observational study about low-molecular-weight heparin combined with progesterone (trial group )versus progesterone(control group )for RIF patients were collected during the inception to Jul. 2020. After data extraction of studies met included criteria ,the Cochrane 5.1.0 bias risk assessment tool and the Newcastle-Ottawa Scale evaluation scale were used to evaluate the quality of RCTs and observational studies. Rev Man 5.3 software was used for Meta-analysis. RESULTS :A total of 15 studies were included ,including 10 RCTs and 5 observational studies ,with a total of 2 353 patients. Results of Meta-analysis showed that the implantation rate [RR =1.41,95%CI(1.24,1.60),P<0.01],pregnancy rate [RR =1.38,95%CI(1.23,1.56), P<0.01],live birth rate [RR =1.41,95%CI(1.19,1.67),P<0.01] of trial group were significantly higher than those of control group,with statistical significance ;while the abortion rate [RR =0.54,95% CI(0.33,0.86),P=0.01] of trial group was significantly lower than that of control group ,with statistical significance. The results of subgroup analysis showed that the implantation rate [RR =2.38,95%CI(1.31,4.31),P=0.004] and pregnancy rate [RR =2.58,95%CI(1.62,4.11),P<0.001] of patients over 35 years old in trial group were higher than control group ,with statistical significance. The main ADR of 2 groups were a small amount of bleeding and ecchymosis at the injection site. No severe ADR was observed. CONCLUSIONS: Low-molecular-weight heparin combined with progesterone can improve the embry o implantation rate , pregnancy rate and live birth rate of RIF patients ,reduce the abortion rate ,with good safety.
