ABSTRACT

The Coronavirus disease 2019 (COVID-19) has spread globally. Although mixed liver impairment has been reported in COVID-19 patients, the association of liver injury caused by specific subtype especially chronic hepatitis B (CHB) with COVID-19 has not been elucidated. In this multi-center, retrospective, and observational cohort study, 109 CHB and 327 non-CHB patients with COVID-19 were propensity score matched at an approximate ratio of 3:1 on the basis of age, sex, and comorbidities. Demographic characteristics, laboratory examinations, disease severity, and clinical outcomes were compared. Furthermore, univariable and multivariable logistic and Cox regression models were used to explore the risk factors for disease severity and mortality, respectively. A higher proportion of CHB patients (30 of 109 (27.52%)) developed into severe status than non-CHB patients (17 of 327 (5.20%)). In addition to previously reported liver impairment markers, such as alanine aminotransferase, aspartate aminotransferase, alkaline phosphatase, and total bilirubin, we identified several novel risk factors including elevated lactate dehydrogenase (⩾ 245 U/L, hazard ratio (HR) = 8.639, 95% confidence interval (CI) = 2.528-29.523; P < 0.001) and coagulation-related biomarker D-dimer (⩾ 0.5 µg/mL, HR = 4.321, 95% CI = 1.443-12.939; P = 0.009) and decreased albumin (< 35 g/L, HR = 0.131, 95% CI = 0.048-0.361; P < 0.001) and albumin/globulin ratio (< 1.5, HR = 0.123, 95% CI = 0.017-0.918; P = 0.041). In conclusion, COVID-19 patients with CHB were more likely to develop into severe illness and die. The risk factors that we identified may be helpful for early clinical surveillance of critical progression.

ABSTRACT

OBJECTIVE@#To explore the clinical characteristics and genetic basis of a child with 5α-reductase type 2 deficiency.@*METHODS@#Clinical data of the child was retrospectively analyzed. Targeted capture-next generation sequencing and Sanger sequencing were carried out to detect potential variants.@*RESULTS@#The patient's main features included micropenis and hypospadia. He was found to harbor compound heterozygous c.680G>A (p.R227Q) and c.3G>T (p.M1I) variants of the SRD5A2 gene. Among these, c.680G>A (p.R227Q) was inherited from his father and was a known pathogenic mutation, while c.3G>T (p.M1I) was inherited from his mother and was unreported previously.@*CONCLUSION@#The compound heterozygous variants of the SRD5A2 gene probably underlay the disease in this child, who was eventually diagnosed with 5α-reductase 2 deficiency.

ABSTRACT

Objective:To compare the results of three detection methods, single antigen-bead assay(SAB), Luminex screening assay(LMX), and ELISA assay for detecting HLA antibody, and compares the two screening methods, LMX and ELISA with SAB detection as a reference method to provide a reference for organ transplantation laboratories to choose a reasonable HLA antibody test strategy.Methods:A lot of 124 consecutive samples were tested using SAB, ELISA, and LMX methods at the same time, and analyze the differences of these results. SAB testing was used as a reference method to analyze the sensitivity and specificity of the two screening assays. Chi-square analysis was used to compare the two methods, and P<0.05 was considered statistically significant. Results:Both ELISA and LMX methods showed low sensitivity of 34.4% and 31.3% for HLA class I, and 29.7% and 51.3% for class Ⅱ. Otherwise, the specificity of the ELISA and LMX method was much higher. For class, I both was 98.9%, and for class Ⅱ were 100% and 91.9% respectively. Out of 124 samples, the number of SAB(+ )ELISA(-)LMX(-) results was 17, and SAB(-)ELISA(+ )LMX(+ ) results was zero indicating that there were considerably screening assays probably with missed detection. In the cases of SAB(+ )ELISA(-)LMX(-), the distribution of MFI value of SAB assay ranges from 750 to 7000.Conclusions:Because the sensitivity of the two screening methods is relatively low, there is a greater risk of missed antibody detection in the scheme of testing for specific antibodies after the screening test is positive. This should be paid attention to, especially for patients with a history of sensitization. For negative screening test results, SAB or other assays should be considered to check the result. It could provide more accurate results when SAB which is recognized as higher sensitivity and specificity is directly used as an initial test. At the same time, the MFI value of the SAB test can serve as an indicator to determine whether to add other assays to check the ASB result.

ABSTRACT

Objective:To investigate the status of viral reservoirs in prostate tissue of patients with human immunodeficiency virus/acquired immunodeficiency syndrome (HIV/AIDS), and to investigate the effect of highly active anti-retroviral therapy (HAART) on HIV-1 DNA in prostate tissue of HIV/AIDS patients.Methods:Twelve patients with HIV infection and hyperplasia of prostate who required surgical treatment and admitted to Guangzhou Eighth People′s Hospital from July 2017 to October 2019 were included. Blood and prostate specimens of these patients were collected, and HIV-1 RNA in plasma, CD4 + T lymphocyte count in peripheral blood and HIV-1 DNA level in prostate tissue were tested respectively. The independent sample t test or Mann-Whitney U test was used for statistical analysis. Results:Among the 12 patients, the CD4 + T lymphocytes was (519.8±121.5)/μL and HIV-1 DNA in the prostate tissue was 2 602 (365, 10 700) copies/10 6cells in six patients who had not started HAART. The CD4 + T lymphocytes was (182.8±69.7)/μL and the HIV-1 DNA in the prostate tissue was 144 (36, 563) copies/10 6cells in the six patients who underwent HAART for over six months. There were statistically significant differences in CD4 + T lymphocytes and HIV-1 DNA in the prostate tissue between the two groups ( t=-5.889 and Z=-2.082, respectively, both P<0.05). Conclusion:Prostate tissue can be used as an HIV-1 virus repository with or without HAART, and the size of the prostate tissue virus repository can be reduced by HAART after immune reconstitution.

ABSTRACT

Objective To evaluate the clinical value of holographic image navigation in urological laparoscopic and robotic surgery.Methods The data of patients were reviewed retrospectively for whom accepted holographic image navigation laparoscopic and robotic surgery from Jan.2019 to Dec.2019 in Beijing United Family Hospital and other 18 medical centers,including 78 cases of renal tumor,2 cases of bladder cancer,2 cases of adrenal gland tumor,1 cases of renal cyst,1 case of prostate cancer,1 case of sweat gland carcinoma with lymph node metastasis,1 case of pelvic metastasis after radical cystectomy.All the patients underwent operations.In the laparoscopic surgery group,there were 27 cases of partial nephrectomy,1 case of radical prostatectomy,2 cases of radical cystectomy and 2 cases of adrenalectomy.In the da Vinci robotic surgery group of 54 cases,there were 51 cases of partial nephrectomy,1 case of retroperitoneal lymph node dissection,1 case of retroperitoneal bilateral renal cyst deroofing and 1 case of resection of pelvic metastasis.There were 41 partial nephrectomy patients with available clinical data for statistic,with a median age of 53.5 years (range 24-76),including 26 males and 15 females.The median R.E.N.A.L score was 7.8 (range 4-11).Before the operation,the engineers established the holographic image based on the contrast CT images and reports.The surgeon applied the holographic image for preoperative planning.During the operation,the navigation was achieved by real time fusing holographic images with the laparoscopic surgery images in the screen.Results All the procedures had been complete uneventfully.The holographic images helped surgeon in understanding the visual three-dimension structure and relation of vessels supplying tumor or resection tissue,lymph nodes and nerves.By manipulating the holographic images extracorporeally,the fused image guide surgeons about location vessel,lymph node and other important structure and then facilitate the delicate dissection.For the 41 cases with available clinical data including 23 cases of robotic-assisted partial nephrectomy and 18 cases of laparoscopic nephrectomy,the median operation time was 140 (range 50-225) min,the median warm ischemia time was 23 (range 14-60) min,the median blood loss was 80(range 5-1 200) ml.In the robotic surgery group,the median operation time was 140 (range 50-215)min,the median warm i schemia time was 21 (range 17-40)min,the median blood loss was 150(range 30-1 200)ml.In the laparoscopic surgery group,the median operation time was 160(range 80-225)min,the median warm ischemia time was 25 (range 14-60)min,the median blood loss was 50 (range 5-1 200) ml.All the patients had no adjacent organ injury during operation.There were 2 cases with Clavien Ⅱ complications.One required transfusion and the other one suffered hematoma post-operation.However,the tumors were located in the renal hilus for these 2 cases and the R.E.N.A.L scores were both 11.Conclusions Holographic image navigation can help location and recognize important anatomic structures during the surgical procedures..This technique will reduce the tissue injury,decrease the complications and improve the success rate of surgery.

ABSTRACT

Objective To summarize the preliminary experience of extraperitoneal laparoscopic radical prostatectomy (C.R.P.C.four-step) for localized prostate cancer and the outcomes based on early follow-up.Methods A total of 102 prostate cancer patients were screened by prostate specific antigen (PSA) and diagnosed by prostate magnetic resonance imaging and prostatic puncture biopsy with cT1c-cT3b,with average age of (67 ±5) years old,average preoperative total PSA value of (45.32 ± 18.33) ng/ml,and average prostate volume was (42 ± 12)cm3.All these patients underwent extraperitoneal laparoscopic radical prostatectomy by the four-step technique,abbreviating as C.R.P.C.[C:control DVC (dorsal deep venous complex).R:recognize three anatomical layers (prostate and bladder junction,seminal vesicle,and Denonvilliers' fascia surface).P:preserve urethral sphincter and bladder neck.C:continuous anastomosis between urethra and bladder neck (4 key needles at 3,5,7 and 9 o'clock)].The operative time,estimated blood loss,length of hospital stay and postoperative complications were recorded,and the postoperative PSA was followed up.Results All the 102 cases were successfully treated by iaparoscopic radical prostatectomy.The operative time was from 55 to 156 min (mean 92 min),and the estimated blood loss was from 55 to 185 ml (mean 105 ml).There was no case converted of open surgery,only one case received blood transfusion for postoperative hemorrhage (0.98%),and positive surgical margin was found in 15 case (14.70%) by pathological examination.Postoperative urinary extravasation within one week occurred in 2 (1.96%) cases,and resolved after tensioning the catheter and prolonging the indwelling time.During the follow-up period of 12 to 45 months,2 cases were incontinent (grade I-II),and the other cases(98.04%) had no incontinence or dysuria.However,11 cases (10.78%) developed to biochemical recurrence within 6 months after the operation.Conclusions The C.R.P.C.four-step technique of lparoscopic radical prostatectomy is easily to be grasped and performed by the greenhand urologists,and was efficient and safe.

ABSTRACT

Objective:To explore the feasibility and value of individualized scheme of dose of iodine contrast medium in enhanced abdominal CT imaging based on liver volume (LV) measurement.Methods:The examination data and technical parameters of 64 patients, who underwent multi-phase dynamic enhanced abdominal CT imaging from February to September 2019 at First People's Hospital of Yunnan Province were retrospectively analyzed. The density of liver on CT pre-contrast images and post-contrast images, LV and average iodine content in the liver (AII) of all subjects were measured. The difference of liver density (ΔHU), total iodine dose of contrast agent injected (IM) and total intrahepatic iodine (TII) were calculated. Pearson correlation analysis was performed to evaluate the association between AII and ΔHU, as well as IM and TII. The difference between the injection volume of contrast agent predicted by the formula and the actual injection volume of contrast agent was tested by paired t test. Results:The ΔHU and AII showed highly positive correlation ( r=0.926, P<0.05). The regression equation was Y AII=0.034X ΔHU+0.296. The TII showed significantly positive correlation with the IM ( r=0.759, P<0.05), and the regression equation was Y IM=3.649X TII+16 486.754. With the liver enhancement ΔHU=50 HU as a reference, the individually dose of contrast medium injection could be deducted based on LV, with Y IM=7.225X LV+ 16 486.754. This formula was introduced into all the subjects to calculate the predicted value of contrast agent application, and the calculated injection volume of contrast agent was (64.27± 4.92) ml. Paired t test was conducted to compare the predicted injection volume of contrast agent with the actual injection volume of contrast agent, and the difference was statistically significant ( t= 6.009, P<0.05). Conclusion:The feasibility of calculating individualized scheme of contrast agent based on LV in enhanced abdominal imaging is verified. This method can be used to predict the enhancement level of liver and to reduce the dosage of iodine contrast agent.

ABSTRACT

Purpose@#Chronic stress and related hormones are key in cancer progression. Peroxisome proliferator-activated receptor γ (PPARγ) and its agonists was reported that inducing anti-tumor effect. However, the function of PPARγ in pro-tumorigenic effects induced by chronic stress in breast cancer remains unknown. Herein, we have characterized a novel role of PPARγ and vascular endothelial growth factor (VEGF)/fibroblast growth factor 2 (FGF2) signals in breast cancer promoted by chronic stress. @*Materials and Methods@#We performed experiments in vivo and in vitro and used bioinformatics data to evaluate the therapeutic potential of PPARγ in breast cancer promoted by stress. @*Results@#Chronic stress significantly inhibited the PPARγ expression and promoted breast cancer in vivo. VEGF/FGF2-mediated angiogenesis increased in the chronic stress group compared to the control group. PPARγ agonist pioglitazone (PioG) injection offset the pro-tumorigenic effect of chronic stress. Moreover, specific β2-adrenergic receptor (β2R) antagonist ICI11-8551 inhibited the effect of chronic stress. In vitro, norepinephrine (NE) treatment had a similar tendency to chronic stress. The effect of NE was mediated by the β2R/adenylate cyclase signaling pathway and suppressed by PioG. PPARγ suppressed VEGF/FGF2 through reactive oxygen species inhibition. Bioinformatics data confirmed that therewas a lowPPARγ expression in breast invasive carcinoma. Lower PPARγ was associated with a significantly worse survival. @*Conclusion@#β2R activation induced by chronic stress and related hormones promotes growth and VEGF/FGF2-mediated angiogenesis of breast cancer by down-regulating PPARγ. Our findings hint that β receptor and PPARγ as two target molecules and the novel role for their agonists or antagonists as clinical medicine in breast cancer therapy

ABSTRACT

Although the tumor suppressor P53 is known to regulate a broad network of signaling pathways, it is still unclear how certain drugs influence these P53 signaling networks. Here, we used a comprehensive single-cell multiomics view of the effects of ginsenosides on cancer cells. Transcriptome and proteome profiling revealed that the antitumor activity of ginsenosides is closely associated with P53 protein. A miRNA-proteome interaction network revealed that P53 controlled the transcription of at least 38 proteins, and proteome-metabolome profiling analysis revealed that P53 regulated proteins involved in nucleotide metabolism, amino acid metabolism and "Warburg effect". The results of integrative multiomics analysis revealed P53 protein as a potential key target that influences the anti-tumor activity of ginsenosides. Furthermore, by applying affinity mass spectrometry (MS) screening and surface plasmon resonance fragment library screening, we confirmed that 20()-protopanaxatriol directly targeted adjacent regions of the P53 DNA-binding pocket and promoted the stability of P53-DNA interactions, which further induced a series of omics changes.

ABSTRACT

Members of the proto-oncogene superfamily are indispensable molecular switches that play critical roles in cell proliferation, differentiation, and cell survival. Recent studies have attempted to prevent the interaction of RAS/GTP with RAS guanine nucleotide exchange factors (GEFs), impair RAS-effector interactions, and suppress RAS localization to prevent oncogenic signalling. The present study aimed to investigate the effect of the natural triterpenoic acid inhibitor glycyrrhetinic acid, which is isolated from the roots of plant species, on RAS stability. We found that glycyrrhetinic acid may bind to the P-loop of RAS and alter its stability. Based on our biochemical tests and structural analysis results, glycyrrhetinic acid induced a conformational change in RAS. Meanwhile, glycyrrhetinic acid abolishes the function of RAS by interfering with the effector protein RAF kinase activation and RAS/MAPK signalling.

ABSTRACT

In order to study the influence of tympanic membrane lesion and ossicular erosion caused by otitis media on the hearing compensation performance of round-window stimulation, a human ear finite element model including cochlear asymmetric structure was established by computed tomography (CT) technique and reverse engineering technique. The reliability of the model was verified by comparing with the published experimental data. Based on this model, the tympanic membrane lesion and ossicular erosion caused by otitis media were simulated by changing the corresponding tissue structure. Besides, these simulated diseases' effects on the round-window stimulation were studied by comparing the corresponding basilar-membrane's displacement at the frequency-dependent characteristic position. The results show that the thickening and the hardening of the tympanic membrane mainly deteriorated the hearing compensation performance of round-window stimulation in the low frequency; tympanic membrane perforation and the minor erosion of ossicle with ossicular chain connected slightly effected the hearing compensation performance of round-window stimulation. Whereas, different from the influence of the aforementioned lesions, the ossicular erosion involving the ossicular chain detachment increased its influence on performance of round-window stimulation at the low frequency. Therefore, the effect of otitis media on the hearing compensation performance of round-window stimulation should be considered comprehensively when designing its actuator, especially the low-frequency deterioration caused by the thickening and the hardening of the tympanic membrane; the actuator's low-frequency output should be enhanced accordingly to ensure its postoperative hearing compensation performance.

Objective@#To explore the prognostic and diagnositic value of serum and urine humancartilage-glycoprotein-39(YKL-40) in patients with pancreatic cancer.@*Methods@#A total of 58 pancreatic cancer patients and 50 healthy controls were enrolled as pancreatic cancer group and control group in the Maternity and Child health Care Center of Jingmen and the Second People′s Hospital of Jingmen from July 2014 to July 2017. The levels of serum and urine YKL-40 were detected by ELISA assay. The diagnostic efficiency of serum YKL-40 was determined by receiver operating characteristic. According to the serum, urine YKL-40 concentrations, pancreatic cancer patients were divided into two groups: the YKL-40 high group and the YKL-40 low group. The correlation between YKL-40 levels and clinical characteristics was analyzed by χ2 test. Median survival was calculated by Kaplan-Meier method. The risk factors of patients with poor prognosis were analyzed by univariate and multivariate Cox regression analysis model.@*Results@#The levels of serum YKL-40 were significantly increased in the pancreatic cancer group compared to those of the control group [(117.16 ± 46.78) ng/ml vs. (68.88 ± 12.37) ng/ml] (P < 0.05). The area under ROC curve(AUC) of serum YKL-40 for the diagnosis of pancreatic cancer were 0.839 (95% CI 0.755-0.902), with a sensitivity and specificity of 67.2% and 98.0%, respectively. Age, differentiation, vascular invasion, tumor TNM, positive surgical margin stage had significant relationship with serum YKL-40 level, and vascular invasion, tumor TNM, positive surgical margin stage had significant relationship with urine YKL-40 level (all P<0.05). Compared with lower YKL-40 group, the median survival of higher YKL-40 group decreased remarkablely, and the difference was statistically (all P<0.05). A high serum YKL-40 level (HR: 3.696, 95% CI 1.680-8.130, P=0.001), high urine YKL-40 level (HR: 2.846, 95% CI 1.311-6.178, P=0.008), vascular invasion (HR: 0.308, 95% CI 0.146-0.651, P=0.002), high tumor TNM (HR: 1.997, 95% CI 1.005-3.967, P=0.048) was identified as an independent risk factor of poor prognosis for patients with pancreatic cancer (all P<0.05).@*Conclusions@#Serum YKL-40 can be used as a diagnostic marker for pancreatic cancer, and both serum and urine YKL-40 can be served as prognostic marker for pancreatic cancer. However, urine YKL-40 has a better clinical value for this index with advantages of non-invasive operation.

Objective To evaluate the accuracy and clinical significance of the vesical imagingreporting and data system (Ⅵ-RADS) in predicting muscle-invasive bladder cancer (MIBC).Methods The data of 59 bladder cancer patients who underwent multiparametric magnetic resonance imaging and surgery between 2014 March and 2019 May were retrospectively analyzed,which includes 51 males and 8 females,aged 36-82 years old,with a median age of 62 years old.According to the scoring methods specified by Ⅵ-RADS,radiologists read and scored all mpMRIs including T2-weighted imaging (T2WI),diffusion-weighted imaging(DWI),and dynamic contrast enhancement MRI(DCE-MRI) of all the included patients.And then the Ⅵ-RADS were compared with pathological diagnosis.Proportions of MIBC in each score category were calculated,and ROC curve was plotted and the area under the curve (AUC) was estimated to assess the sensitivity and specificity of Ⅵ-RADS in diagnosing MIBC.Results The number of patients in Ⅵ-RADS score category 1 to 5 were 12,28,2,15 and 2,respectively.And there were 0,2 (7.4%),1 (50.0%),13 (81.3 %),2 (100.0%) MIBC patients in each score category,respectively.When Ⅵ-RADS ≥3 was used to define MIBC,it came to the largest Youden's Index(0.7913),with an AUC of 0.924.And the sensitivity and specificity were 88.9% and 90.2%,respectively.Conclusions Ⅵ-RADS has high accuracy in predicting MIBC,and it is worthy of application and verification in further clinical practice.The urologists should be highly alert to the existence of MIBC when Ⅵ-RADS ≥3.

Objective@#To evaluate the role of long non-coding RNA maternally expressed gene 3 (MEG3) in hyperglycose-induced neurocyte damage and the relationship with mitochondrion-dependent apoptosis in rats.@*Methods@#Normally cultured PC12 cells were divided into 5 groups (n=18 each) using a random number table method: normal concentration of glucose control group (C group), normal concentration of glucose plus MEG3 group (C+ MEG3 group), high-concentration glucose group (HG group), high-concentration glucose plus MEG3 group(HG+ MEG3 group), and high-concentration glucose plus negative lentiviral vector (LV-NC) group (HG+ NC group). PC12 cells were cultured in DMEM medium with 25 mmol/L glucose in group C. PC12 cells were cultured in DMEM medium with 25 mmol/L glucose after being transfected with MEG3 lentiviral vector (LV-MEG3) in C+ MEG3 group.PC12 cells were cultured in DMEM medium with 250 mmol/L glucose in HG group.PC12 cells were incubated in DMEM medium containing 250 mmol/L glucose after being transfected with LV-MEG3 in HG+ MEG3 group.PC12 cells were incubated in DMEM medium containing 250 mmol/L glucose after being transfected with LV-NC in HG+ NC group.After the cells were cultured or incubated for 1 day, the cell viability was measured by CCK8 assay, the apoptosis rate and level of reactive oxygen species (ROS) were determined by flow cytometry, and the amount of lactic dehydrogenase (LDH) released was measured by DCFH-DA, the expression of Cyt c, caspase-3, caspase-9, Bcl-2, Bax and Apaf-1 was determined by Western blot, and the opening of mitochondrial permeability transition pore (mPTP) was determined by fluorescent method.Blc-2/Bax ratio was calculated.@*Results@#Compared with group C, the cell viability was significantly decreased, the amount of LDH released, ROS level and apoptosis rate were increased, the opening of mPTP was increased, and the expression of caspase-3, caspase-9, Cyt c, Bax, Bcl-2 and Apaf-1 was up-regulated in HG, HG+ MEG3 and HG+ NC groups, and Bcl-2/Bax ratio was increased in HG+ MEG3 group and decreased in HG and HG+ NC groups (P<0.05). Compared with HG group and HG+ NC group, the cell activity was significantly increased, the amount of LDH released, ROS level and apoptosis rate were decreased, the opening of mPTP was decreased, the expression of caspase-3, caspase-9, Cyt c, Bax, and Apaf-1 was down-regulated, the expression of Bcl-2 was up-regulated, and Bcl-2/Bax ratio was increased in HG+ MEG3 group (P<0.01).@*Conclusion@#MEG3 may be involved in the endogenous protective mechanism during hyperglycose-induced neurocyte damage by inhibiting mitochondrion-dependent apoptosis in rats.

ABSTRACT

OBJECTIVE: To observe the effect of n-hexane subchronic exposure on the serum level of neuron specific enolase(NSE), neurofilament light chain protein(NF-L) and nerve growth factor(NGF) in rat, and to explore the feasibility of using NSE, NF-L and NGF as biomarkers of n-hexane neurotoxicity. METHODS: Specific pathogen free male SD rats were randomly divided into control group and low-, medium-and high-dose exposure groups, with 6 rats in each group. Rats in the low-, medium-and high-dose exposure groups were given n-hexane solution at doses of 168, 675 and 2 700 mg/kg body mass, respectively, while rats in the control group were gavaged with 0.9% sodium chloride solution, once a day, 5 days a week for 6 weeks. At week 0, 2, 4, and 6 of exposure, the body mass of the rats was weighed, the gait scores were performed, and the serum levels of NSE, NF-L, and NGF were detected.RESULTS: Body mass, gait score and serum levels of NSE and NF-L in rats were statistically significant in terms of the n-hexane exposure dose and exposure time(P<0.01). At the 6 th week of n-hexane exposure, the body mass of the three dose exposure groups was lower than that of the control group(P<0.05), and the gait score was higher than that of the control group(P<0.05). Moreover, the abnormal gait of the rats showed a dose-effect relationship with the increasing n-hexane poisoning dose. At week 2, 4 and 6, the serum levels of NSE and NF-L in these three dose exposure groups were higher than that in the control group(P<0.05). In addition, the serum level of NF-L in rats in the medium-and high-dose exposure groups increased with the n-hexane exposure time increasing and showed a time-effect relationship(P<0.05). The level of serum NGF in rats was statistically significant in the main effects of n-hexane dose and duration of exposure(P<0.05). The serum NGF level in the high-dose exposure group was lower than that in the control group, the low-dose and medium-dose exposure groups(P<0.05). NGF level in serum of rats at week 6 was lower than that at week 0, 2 and 4(P<0.05). CONCLUSION: Both NSE and NF-L in serum can be used as biomarkers for the early effect of n-hexane on peripheral nerve injury. The feasibility of using serum NGF as a biomarker for the early effect of n-hexane on peripheral nerve injury warrants further investigation.

ABSTRACT

explore the prognostic and diagnositic value of serum and urine humancartilage-glycoprotein-39(YKL-40) in patients with pancreatic cancer. Methods A total of 58 pancreatic cancer patients and 50 healthy controls were enrolled as pancreatic cancer group and control group in the Maternity and Child health Care Center of Jingmen and the Second People′s Hospital of Jingmen from July 2014 to July 2017. The levels of serum and urine YKL-40 were detected by ELISA assay. The diagnostic efficiency of serum YKL-40 was determined by receiver operating characteristic. According to the serum, urine YKL-40 concentrations, pancreatic cancer patients were divided into two groups: the YKL-40 high group and the YKL-40 low group. The correlation between YKL-40 levels and clinical characteristics was analyzed by χ2 test. Median survival was calculated by Kaplan-Meier method. The risk factors of patients with poor prognosis were analyzed by univariate and multivariate Cox regression analysis model. Results The levels of serum YKL-40 were significantly increased in the pancreatic cancer group compared to those of the control group [(117.16 ± 46.78) ng/ml vs. (68.88 ± 12.37) ng/ml] (P<0.05). The area under ROC curve(AUC) of serum YKL-40 for the diagnosis of pancreatic cancer were 0.839 (95% CI 0.755-0.902), with a sensitivity and specificity of 67.2% and 98.0%, respectively. Age, differentiation, vascular invasion, tumor TNM, positive surgical margin stage had significant relationship with serum YKL-40 level, and vascular invasion, tumor TNM, positive surgical margin stage had significant relationship with urine YKL-40 level (all P<0.05). Compared with lower YKL-40 group, the median survival of higher YKL-40 group decreased remarkablely, and the difference was statistically (all P<0.05). A high serum YKL-40 level ( HR : 3.696, 95% CI 1.680-8.130, P=0.001), high urine YKL-40 level ( HR : 2.846, 95% CI 1.311-6.178, P=0.008), vascular invasion ( HR : 0.308, 95% CI 0.146-0.651, P=0.002), high tumor TNM ( HR : 1.997, 95% CI 1.005-3.967, P=0.048) was identified as an independent risk factor of poor prognosis for patients with pancreatic cancer (all P<0.05). Conclusions Serum YKL-40 can be used as a diagnostic marker for pancreatic cancer, and both serum and urine YKL-40 can be served as prognostic marker for pancreatic cancer. However, urine YKL-40 has a better clinical value for this index with advantages of non-invasive operation.

ABSTRACT

Objective To explore the application of osteopontin (OPN) and Stathmin- 1 expression in the evaluation of prostate cancer recurrence and survival prognosis. Methods One hundred patients with prostate cancer were selected from June 2014 to June 2016. The expressions of OPN and Stathmin-1 in prostate cancer tissue were detected by immunohistochemical staining SP method, and its correlation with clinicopathological features was analyzed. Results The OPN positive expression in prostate cancer tissue was in 76 cases (76.00%, 76/100), the Stathmin-1 positive expression in prostate cancer tissue was in 79 cases (79.00%, 79/100), and both the OPN and Stathmin-1 positive expression in prostate cancer tissue was in 60 cases (60.00% , 60/100). Spearman correlation analysis result showed that the expression of OPN in prostate cancer tissue had positive correlation with Stathmin-1 (r = 0.491, P<0.01). The positive expressions of OPN and Stathmin-1 in prostate cancer tissue were relative with Gleason score, T staging and bone metastasis (P < 0.05), but were not relative with recurrence (P>0.05). The survival time in OPN positive expression, Stathmin-1 positive expression and both OPN/Stathmin-1 positive expression was (18.45 ± 2.03), (17.95 ± 2.01) and (15.24 ± 1.72) months;the survival time in OPN negative expression, Stathmin-1 negative expression and OPN/ Stathmin-1 single positive or both negative was (24.67 ± 2.62), (23.79 ± 2.58) and (26.68 ± 2.72) months, and there was statistical difference (P<0.05). Conclusions The OPN and Stathmin-1 expressions in prostate cancer tissue are high, and show relations with Gleason score, bone metastases and T staging, and no correlation with recurrence. But OPN and Stathmin-1 can provide reference for survival in patients with prostate cancer prognosis assessment.

ABSTRACT

Objective To evaluate the curative effect of compound nutrient assisted phototherapy on neonatal jaundice.Methods Neonatologists at seven hospitals participated in the study.A total of three hundred and twenty full-term newborns with high indirect bilirubin admitted to hospital from September 2017 to January 2018 were selected.One hundred and sixty-six cases in the observation group,and one hundred and fifty-four cases in the control group,all enrolled neonates were given single-sided,conventional intensity phototherapy.Observation group took compound nutrient at the same time.The average gestational age,age,birth weight of two groups before treatment were not significantly different.Serum total biilirubin,indirect bidirubin,liver function (ALT,AST) and phototherapy time were monitored before treatment and 3 days after treatment.Results The serum total bilirubin in the observation group was significantly lower than that of the control group after 3 days of treatment[(196.7 ± 57.2) μmol/L vs (216.5 ± 54.6) μmol/L],(t=3.17,P<0.01).The indirect bilirubin in the observation group was significantly lower than that of the control group after 3 days of treatment [(176.3 ± 54.3) μmol/L vs (197.2 ± 52.9) μmol/L],(t=3.50,P<0.01).The time of phototherapy of the children in the observation group was significantly shorter than that of the control group[(19.8 ± 14.4)d vs (22.9 ± 13.3) d],(t =2.00,P < 0.01).Rash,fever,bronze disease,spilled milk,vomiting,abdominal distention,diarrhea,constipation,liver damage etc.were no significant difference the observation group and the control group(P > 0.05).Conclusion Compound nutrients had good efficacy and safety in adjuvant phototherapy for neonatal high indirect bidirubin.

Objective:To investigate the effect of co-expression of sleeping beauty(SB)transposon and IL-10 mediated by adenovirus on the balance of Th17/Treg cells in the splenocytes of the non-obese diabetes(NOD)mice, and to illuminate the possible mechanism of IL-10 in the treatment of NOD mice.Methods:The splenocytes were extracted from spleen of the C57BL6 mice and cultured.The splenocytes were divided into control group,empty vector group and therapy group.The cells in control group didn't receive any treatment,the cells in empty vector group were co-infected with the adenovirus vector without IL-10 gene containing transposon sequence and the adenovirus vector without transposase,and the cells in therapy cells were co-infected with the adenovirus vector containing IL-10 gene and transposon sequence and the adenovirus vector with transposase.After 48 h of infection, the expression leves of IL-10 mRNA in the splenocytes of the mice in various groups were assessed by RT-PCR. The cells of above three groups were subcutaneously injected into popliteal space in right hind leg of the NOD mice in control group,empty vector group and therapy group;there were six mice in each group;once a week and six times.The mice were killed 4 weeks after injection,the expression levels of IL-10 in serum of the NOD mice in various groups were assessed by ELISA,and the proportions of CD4+IL-10+,CD4+IFN-γ+,Th17 and Treg cells in the splenocytes were detected by flow cytometry.Results:Compared with control group and empty vector group,the expression level of IL-10 mRNA in splenocytes of the C57BL6 mice in therapy group was increased (F=72.71,P<0.05);the expression level of IL-10 in serum of the NOD mice was increased(F=8.89,P<0.05);the proportions of CD4+IL-10+ cells and Treg cells were significantly increased(F=72.09,P<0.05;F=12.98,P<0.05);the proportion of Th17 cells was decreased(F=6.39,P<0.05).The proportion of CD4+IFN-γ+ cells had no significant differences between various groups(F=2.72,P>0.05).Conclusion:The co-expression of SB transposon and IL-10 can significantly increase the IL-10 level in serum of the NOD mice,increase the proportion of CD4+IL-10+ cells,decrease the proportion of Th17 cells and increase the proportion of Treg cells in the splenocytes,which can regulate the balance of Th1/Th2 and Th17/Treg cells and play a therapeutic effect in the NOD mice.
