ABSTRACT
At present, there are still some problems in the standardized residency training. Some standardized training bases often regard the resident trainees as ordinary practitioners, and devote their time and energy to the daily medical procedural work without giving enough training and teaching, ignoring the basic skills training of resident trainees. Therefore, this study constructed an active knowledge push system based on business scenarios. The system mainly includes three parts: sensitive operation identification layer, knowledge index layer and resource push layer in order to cultivate the norms of diagnosis and treatment of standardized training students and reasonably solve the problem of inconvenient resource acquisition in clinical work. Through the preliminary application, it was found that the system has effectively improved the mini-clinical evaluation exercise (Mini-CEX) score of the trainees and achieved good results.

Abstract@#Flood disasters are the common public health emergencies, mainly leading to environmental damage, water pollution, food pollution, vector breeding, infectious disease epidemic and other risk factors of sanitary and anti epidemic work. The guideline has been formulated with reference to the technical documents such as Guideline for Environmental Sanitation Disposal and Preventive Disinfection in Flooded Areas and Technical Proposal for Sanitary and Anti epidemic Measures after Flood Disasters, as well as the latest research progress at home and abroad. In order to guide the sanitary and anti epidemic measures in flooded areas, protect the health and safety of students and teachers and ensure the normal educational and teaching order, the guideline introduces the key measures that should be taken by schools, teachers and students in flood striken areas.

Purpose@#This retrospective study aimed to re-evaluate the effect of concurrent chemotherapy in patients with locally advanced nasopharyngeal carcinoma (NPC) in the era of intensity-modulated radiotherapy (IMRT). @*Materials and Methods@#A total of 498 patients who received neoadjuvant chemotherapy (NCT) combined with concurrent chemoradiotherapy (CCRT) or IMRT were retrospectively reviewed. The distribution of baseline characteristics was balanced using propensity score matching. Additionally, the results of NCT+IMRT and NCT+CCRT were compared using Kaplan-Meier survival analysis, and differences in survival rates were analyzed using the log rank test. @*Results@#There were no significant differences in overall survival (OS), progression-free survival (PFS), distant metastasis-free survival (DMFS), and local progression-free survival (LRFS) between the two groups. Patients were further categorized into risk subgroups based on pretreatment Epstein-Barr virus (EBV) DNA cutoff values using receiver operating characteristic curve analysis. There were no statistically significant differences in OS, PFS, DMFS, and LRFS between patients who received NCT+CCRT and NCT+IMRT in the high-risk group. In the low-risk group, although there were no differences between NCT+CCRT and NCT+IMRT in OS, PFS, and LRFS, patients who received NCT+CCRT had better DMFS than those who received NCT+IMRT. @*Conclusion@#Pretreatment EBV DNA level can be used to individualize concurrent chemotherapy for patients with locally advanced NPC. Patients with low pretreatment EBV DNA levels may benefit from concurrent chemotherapy, whereas those with high levels may not. Other treatment modalities need to be explored for high-risk patients to improve their prognosis.

Objective:To predict the epitopes of the spike protein of 2019 novel coronavirus (2019-nCoV) and to analyze the mutation characteristics of this protein.Methods:The gene sequences of 2019-nCoV was downloaded from GenBank and Global Initiative on Sharing All Influenza Data database (as of March 4, 2020). ClustalW, MEGA 7.0, Immune Epitope Database and other software were used for predicting potential B lymphocyte and T lymphocyte epitopes of spike protein, and then mutations were compared in epitope and non-epitope regions. Statistical analysis was performed with chi-square test.Results:There were 130 potential epitopes, including 25 linear B lymphocyte epitopes, 18 non-linear B lymphocyte epitopes, 71 human leukocyte antigen (HLA) class-Ⅰ restricted T lymphocyte epitopes, and 16 HLA class-Ⅱ restricted T lymphocyte epitopes in spike protein. S 404-426displayed the higher antigenicity-score, and its population coverage was 73.4% in China. Compared with the spike protein before January 31, the mutation frequencies of spike protein were increased both in non-epitope (15.4% vs 21.7%) and epitope regions (6.2% vs 8.7%) after January 31, but the differences were not statistically significant (both P>0.05). Conclusions:The study identifies several epitopes of 2019-nCoV spike protein, which provides a preliminary basis for the study of 2019-nCoV vaccine. Mutations in spike protein have an increasing trend which needs more attention and research in the future.

Lung cancer is the most common cause of cancer-related death worldwide. There are two classes of lung cancer: non-small cell lung cancer (NSCLC) and small cell lung cancer (SCLC). NSCLC represents approximately 85% of all lung cancer cases. Immune checkpoint inhibitors (ICIPs) are a class of inhibitors of programmed death-1 and programmed death-ligand 1. Preclinical studies have shown that ICIPs have shown good clinical efficacy and durable response in diverse cancers. Among them, atezolizumab (MPDL3280), an anti-PD-L1 monoclonal antibody, is being investigated as a potential therapy against solid tumors and hematologic malignancies in humans. Pseudoprogression is reported as one of the unique phenomena with immune therapeutic agents. Here we report case of a person with advanced NSCLC who developed pseudoprogression after receiving immunotherapy. We hope this case could help clinicians to make appropriate decision when assessing therapeutic effects of immunotherapy. .

To improve the blast resistance of elite rice restorer line Fuhui 673, 3 blast resistance genes Pi-1, Pi-9 and Pi-kh were introduced into Fuhui 673 from a good-quality restorer line Jinhui 1059 through 3 successive backcrosses followed by one selfing using the technique of marker-assisted selection. Ten near-isogenic lines (NILs) of Fuhui 673 carrying the 3 introduced resistance genes were created. Genotype analysis using 68 SSR markers evenly distributed in the genome indicated that 92.96%-98.59% of the NILs' genetic background had been recovered to Fuhui 673. Both indoor and field resistance tests indicated that the NILs and their hybrids with sterile line Yixiang A were all resistant to rice blast, with resistance levels significantly higher than those of controls Fuhui 673 and hybrid Yiyou 673 (Yixiang A Fuhui 673). In addition, among the 10 hybrids between the NILs and Yixiang A, 2 showed significantly higher yield than and 4 displayed similar yield to that of control Yiyou 673, suggesting that most of the NILs retained the elite characteristics of Fuhui 673. Two new hybrid rice cultivars Liangyou 7283 and Jintaiyou 683 from NIL Line 9 showed high yield, good resistance to blast and moderate growth period in regional trial, suggesting that the NIL Line 9 has a good prospect for application.

Objective To study the clinical application of three-dimensional (3D) visualization technology in liver resection of complicated liver cancer.Methods A retrospective analysis of 28 patients with complicated liver cancer treated from June 2017 to June 2018 in the Department of Hepatobiliary Surgery,the Affiliated Tumor Hospital of Guangxi Medical University.There were 26 males and 2 females,aged (46± 10) years old.A treatment plan on how to perform liver resection for these patients was developed under the guidance of 3D visualization technology.The actual surgical procedures,operation time,intraoperative blood loss,and postoperative complications were documented.The virtual resected liver volume was compared with the actual resected liver volume.The virtual surgical resection margin was also compared with the actual surgical resection margin.Results All the 28 patients with complicated liver cancer completed the 3D visualization analysis with the location,shape and quantity of tumor being clearly shown.Of the 27patients who underwent liver resection,13 underwent anatomical hepatectomy,and 14 underwent nonanatomical hepatectomy.The operation time ranged from 145 to 350 min (median 240 min).The intraoperative blood loss ranged from 100 to 1 500 ml (median 300 ml).The incisional wound healed slowly in 4 patients,pleural effusion developed in 8 patients,and ascites in 2 patients.There were no significant differences in the virtual resected liver volume compared with the actual resected liver volume (P > 0.05).There was an excellent positive correlation between the patient's virtual resected liver volume and the actual resected liver volume (r =0.986,P < 0.05).There was no significant difference between the virtual surgical resection margin and the actual surgical resection margin (P > 0.05).There was an excellent positive correlation between virtual surgical resection margin and the actual surgical resection margin (r =0.983,P < 0.05).Conclusion Three-dimensional visualization technology accurately assessed the liver status,optimized surgical procedures,and played an important role in liver resection of complex liver cancer.

ABSTRACT
Objective To evaluate the clinical efficacy of Taohua decoction in treating antibiotic-associated diarrhea (AAD). Methods Sixty-two patients diagnosed with AAD admitted to the department of intenive care unit (ICU) of Jiaxing Hospital of Traditional Chinese Medicine (TCM), Jiaxing First Hospital, Jiaxing Second Hospital and Haining Hospital of TCM from April 2015 to November 2016 were enrolled, and they were divided into an experimental group (32 cases) and a control group (30 cases) according to the random number table method. The two groups were given routine treatment of western medicine, the patients in experimental group were additionally given Taohua decoction (ingreients: red halloysite 30 g, zingiberis 15 g, japonica rice 30 g), while in the control group, the patients only received the routine treatment of western medicine. The therapeutic course in the two groups was 14 days. The TCM syndrome score, the duration of diarrhea, the time of abdominal pain relief and the time using antibiotics, besides acute physiology and chronic health evaluation Ⅱ (APACHEⅡ) score, albumin level, endoscopic and pathological changes were observed before and after treatment in the two groups. Results After treatment, the scores of TCM syndrome and APACHE Ⅱin the two groups were all lower than those before treatment, the level of albumin was higher than that before treatment, and the changes of above indexes in the experimental group were more significant than those in the control group [the scores of TCM syndrome: 9.32±8.86 vs. 13.50±6.52, APACHE Ⅱ score: 11.08±4.37 vs. 14.06±5.42, albumin (g/L): 33.89±5.02 vs. 30.85±6.44, all P < 0.05]; in experimental group, the time of diarrhea duration (days:12.28±2.28 vs. 15.36±8.68), time of abdominal pain relief (days: 10.09±6.41 vs. 14.27±7.52), time of using antibiotics (days: 11.77±4.72 vs. 15.08±6.98) were significantly shorter than those of the control group (all P < 0.05). The examinations of endoscopy and light microscopy showed that after treatment the numbers of patients with mild intestinal mucosal changes in the experimental group were increased compared with those in the control group (endoscope:18 cases vs. 8 cases, light microscopy: 19 cases vs. 9 cases), but the numbers of patients with moderate (endoscope:14 cases vs. 20 cases, light microscopy: 13 cases vs. 19 cases) and severe (endoscope: 0 vs. 2 cases, light microscopy:0 vs.2 cases) changes were less than those in the control group. Conclusions Taohua decoction can improve the clinical efficacy of mild AAD patients.

Objective To study the value of serum prealbumin-bilirubin score (PALBI) in predicting posthepatectomy liver failure (PHLF) for patients with HBV related hepatocellular carcinoma (HCC).Methods A retrospective study was conducted on 919 HBV-related HCC patients who underwent hepatectomy from September 2013 to December 2016 at the Affiliated Tumor Hospital of Guangxi Medical University.These patients were divided into a training cohort (n =689) and a validation cohort (n =230) using the 3 ∶ 1 matching principle.The training cohort was divided into the control group (n=546) and the PHLF group (n=143) according to whether PHLF occurred.The multivariate logistic regression model was used to analyze the factors related to PHLF in the training cohort,and then the PALBI score was established.The ability of the PALBI score to predict PHLF was evaluated by the area under the receiver operating characteristic curve (AUC) and compared with the Child-Pugh,model for end-stage liver disease (MELD),and albumin-bilirubin (ALBI) scores.Results Univariate and multivariate logistic regression analyses showed the factors including HBV-DNA≥ 103 IU/ml,total bilirubin,prealbumin,platelet count,AST,prothrombin time,intraoperative blood loss ≥400 ml and major liver resection were closely related to PHLF.The ability of the PALBI score (AUC =0.733) to predict PHLF preoperatively was superior to the ChildPugh score (AUC =0.562),the MELD score (AUC =0.652) and the ALBI score (AUC =0.683) in the entire training cohort.Similar results were obtained in the entire validation cohort (AUC:0.752 vs.0.599 vs.0.641 vs.0.678).To eliminate the effect of a small residual liver volume on PHLF,the ability of each of these scores in the training and validation cohorts to predict PHLF was calculated respectively in these 2 cohorts of patients who underwent only minor liver resection,and similar results were obtained.Conclusion The PALBI score was significantly superior to the Child-Pugh,MELD and ALBI scores in predicting PHLF in patients with HBV-related HCC who underwent liver resection.The PALBI score is a simple,non-invasive and reliable novel model in predicting PHLF.

Objective To compare the performance of four commercial anti-dsDNA antibody assays,i.e,BioPlex 2200 (BioPlex),Farr radioimmunoassay (Farr),MESACUP DNA-Ⅱ TEST ds [MBL-enzyme linked immunosorbent assay (ELISA)] and Anti-dsDNA-NcX ELISA (IgG) (EURO-ELISA),Antoantibodies Profile Assay Kit (HOB-Chemiluminescent Immunoassay) in disease activity assessment of systemic lupus erythematosus (SLE).Methods SLE patients (n=119) as well as healthy controls (n=200) and disease controls (n=100) were recruited and their serum anti-dsDNA antibodies were detected by BioPlex,Farr,MBL-ELISA,EURO-ELISA,and a standard Crithidia luciliae indirect immunofluorescence test (CLIFT).The consistency between above four methods to CLIFT was analyzed.The correlation of anti-dsDNA antibody level of these four methods to SLE disease activity was assessed.All data analyses were performed with Statistical product and service solutions (SPSS) 16.0 (SPSS.Inc) and GraphPad Prism 4.0.3 (GraphPad).Unless otherwise specified,all data in this study were expressed as mean±standard deviation.Cut-off values of the anti-dsDNA quantification methods were set by the manufacturers.Chi square and kappa coefficients were adopted to assess the agreement determination and correlation analysis between anti-dsDNA level and SLE disease activity (SLEDAI).Receiver-operator characteristic (ROC) curve analysis was used to compare the specificity and sensitivity of the anti-dsDNA assays.Student's t test was adopted for the comparison of anti-dsDNA levels by different methods between SLE and SLE+LN groups.A p value small than 0.05 was considered statistically significant.Results Using cut-off values set by the manufacturers,BioPlex demonstrated the highest overall agreement with CLIFT,while MBL-ELISA and EURO-ELISA showed the highest positive agreement with CLIFT.Disease activity correlation analysis showed that SLEDAI score correlated poorly with anti-dsDNA level in Farr assay,but strongly with the other three assays.Bioplex had a better performance in terms of SLE activity index corelation (r=0.297 6,P=0.001 2).Moreover,anti-dsDNA level differed in SLE patients with renal lupus nephritis in BioPlex assay (P=0.026 8),but not in the other assays.In ROC curve analysis,BioPlex showed the largest area under the curve (AUC) over other assays.Conclusion Bio Plex assay has better sensitivity and specificity than Farr,MBL-ELISA and EURO-ELISA and correlates well with SLE disease activity.

Objective To develop a new type of locating rod with easy installation and high reliability for radiotherapy.Methods The locating rod was composed of a joining beam,two clamping buckles and buckle holders.The joining beam had the length being 530+03 mm.One of the buckles was put at one end of the beam,and the other slided along a stool.An eccentric wheel drove the sliding buckle to determine the space between two buckles,and a returning spring was involved in to enhance the compliance of the sliding buckle.Results The rod gained easy installation and operation,and was compatible with the bed board with the width of 5300 mm.There was no sideslip occurred after locking the rod,and the sideslip shift was not more than 1 mm in case of 20 kgf (1 kgf=9.8 N) lateral traction.Conclnsion The rod has high performances and meets clinical requirements.
ABSTRACT
Objective To observe the clinical efficacy of bloodletting plus acupoint sticking in treating diabetic peripheral neuropathy (DPN). Method Eighty DPN patients were randomized into a treatment group of 40 cases and a control group of 40 cases. In addition to the basic treatment (general blood glucose control) in both groups, the treatment group was given bloodletting plus acupoint sticking with Chinese medication, while the control group was given oral administration of Mecobalamin tablets. Result The treatment group had a higher total effective rate (92.5%) than the control group (67.5%), and the improvement of nerve conduction velocity in the treatment group was more significant than that in the control group (P<0.05). Conclusion Bloodletting plus acupoint sticking is effective in treating DPN.

Direct-acting antiviral agents (DAAs) achieve a high sustained virologic response rate in the treatment of chronic hepatitis C virus infection. However, drug resistance-associated mutations play an important role in treatment failure and have attracted more and more attention. This article elaborates on the clinical significance of drug resistance-associated mutations from the aspects of their definition, association with genotype, known drug resistance-associated mutations and their prevalence rates, the impact of drug resistance-associated mutations on treatment naive and treatment-experienced patients, and the role of clinical detection, in order to provide a reference for clinical regimens with DAAs and help to achieve higher sustained virologic response rates.

Objective:To observe the clinical efficacy of Shanghai-style tuina method based on syndrome differentiation in treating infantile diarrhea. Methods:Sixty patients were randomly divided into an observation group and a control group, 30 cases in each group. The patients in the observation group were treated by Shanghai-style tuina method based on syndrome differentiation, while the patients in the control group were treated by conventional Shanghai-style tuina method, once per day, continuous 5 d for a treatment course. The efficacy was evaluated according to the score of the quantization table for symptoms differentiation in infantile diarrhea after one course. Results:During the treatment, two cases in the control group dropped out. After treatment, the scores were significantly decreased in both groups and the intra-group differences were statistically significant (both P<0.01), indicating the two treatment methods were effective with infantile diarrhea. In the efficacies of traditional Chinese medicine (TCM) syndrome, the cured and markedly effective rate in the observation group was significantly higher than that in the control group (P<0.05), suggesting that Shanghai-style tuina method based on syndrome differentiation was more effective than conventional Shanghai-style tuina method. Conclusion:Conventional Shanghai-style tuina method is characterized by fixed acupoints, remembering easily and operating conveniently. It has a significant effect in treating children with diarrhea, and is worthy of clinical promotion and popularization. However, Shanghai-style tuina method based on syndromes differentiation requires the solid theoretical foundation and rich clinical experience of TCM. And because Shanghai-style tuina method based on syndromes differentiation is applied under the guidance of syndrome differentiation and TCM theory, it can improve the clinical efficacy.

Objective To observe the effects of different modes treadmill training on cognitive func-tion and transforming growth factor β1 ( TGF-β1 ) expression in cerebral cortex of rats. Methods Two months old rats were divided into the control group,piecewise training group and intermittent training group ( n=10 in each group) . The training was performed five times a week for 6 weeks. Learning and memory a-bility of all rats was detected by water maze at 6 weeks after the training. TGF-β1 expression and localization in cerebral cortex was tested by QRT-PCR and immunofluorescence, respectively. Results The platform time in piecewise group ((30±28) s) and intermittent group ((25±23)s) was both significantly shorter than that in control group ((58±50)s). In the space exploration,the time around Ⅳ quadrant platform in piecewise group((23.6±3.9)s) and intermittent group ((24.3±8.9)s) was significantly higher than that in the control group((17.7±2.0)s). The expression of TGF-β1 mRNA in cerebral cortex in intermittent group (0.0067±0.0043)was obviously higher than that in piecewise group (0.0035±0.0006) and control group (0.0041±0.001). TGF-β1 was located in cell membrane and cytoplasm,and the relative optical density of intermittent group (0.0045±0.0017) was significantly higher than that of control group (0.0019±0.0004) and staging group (0.00175±0.00045). Conclusion (1)Learning and memory function both were im-proved after treadmill six weeks with piecewise and intermittent training models. ( 2) The level of TGF-β1 gene and protein was significantly increased after interval training in cortex of rats.

Objective To study the effects of miR?21 on the proliferation,apoptosis and MMP2 expression of HeLa cells. Methods The miR?21 or the miRNA scramble control was transfected into Hela cells. The cell proliferation was detected by the Celltiter?GloTM assay 72 h after the transfection,and the apoptosis was evaluated by the Caspase3/7 Glo? Kit. The MMP2 RNA expression was quantified by quantitative real time PCR. Results The proliferation of HeLa cells transfected with miR?21 was significantly increased compared to that of the cells transfected with miRNA scramble control. The caspase 3/7 activity in HeLa cells transfected with miR?21 was downregulated compared to that of the cells transfect?ed with miRNA negative control. The MMP2 RNA expression in HeLa cells transfected with miR?21 was increased significantly compared to the cells transfected with miRNA negative control. Conclusion miR?21 can significantly promote the proliferation of HeLa cells. In addition,it ex?hibits the anti?apoptosis effect in HeLa cells. The transfection of miR?21 significantly increased the MMP2 RNA expression,which suggests that miR?21 may promote the tumor invasion and might be a therapeutic intervention target.

Objective To investigate the changes of A20 expression stimulated by free fatty acids (FFA) and its pathway.Methods HepG2 cells and U937 cells were stimulated by 0.5 mmol/L mixed FFA.The expression of A20,phosphor-p65 and phosphor-IκBα of neclear factor (NF)-κB pathway and phosphor-c-Jun N-terminal kinase (JNK),JNK,phosphor-extracellular signal-regulated kinase (ERK),ERK,phosphor-p38 and p38 of mitogen-activated protein kinase (MAPK) pathway were detected by Western blotting.The level of interleukin (IL)-12p,IL-1β,tumor necrosis factor (TNF)-α,IL-6,IL-10 and IL-8 cytokines in the supernatant of cell culture were detected by flow cytometry.T-test was performed for statistical analysis.Results The level of A20 changed along with the stimulated time of FFA.NF-κB and MAPK pathways were activated after FFA stimulation.The secretion of IL-6 and IL-8 increased after HepG2 cells stimulated by FFA and both reached peak at 24 hour.Compared with control group,the difference in IL-8 was statistically significant ((423.8 ± 8.9) pg/mL vs (12.4 ± 4.5) pg/mL,t=41.28,P<0.01).The difference in IL-6 was also statistically significant ((4 082±423.6) pg/mL vs (52.9±29.5) pg/mL,t=9.49,P<0.01).After U937 cells were stimulated by FFA,the secretion of IL-8 increased compared with control group.And in a certain period of time the secretion was time dependence.The maximum secretion of 24 hours was (200.6±5.7) pg/mL vs (5.0±3.9) pg/mL,and the difference was statistically significant (t=28.16,P<0.01).IL-10,IL-12p,IL-1β and TNF-α were detected.Both NF-κB pathway and MAPK pathway were detected.Conclusions The in vitro FFA mediated steatotic cell model could induce the expression change of A20 and secretion of inflammatory cytokines.NF-κB and MAPK pathways involved in the response to FFA in HepG2 cells and U937 cells.

Racemic (±)-F18 (10-chloromethyl-11-demethyl-12-oxo-calanolide A), an analog of nature product (+)-calanolide A, is a new anti-HIV-1 nonnucleoside reverse transcript inhibitor (NNRTI). A successful enantioseparation of (±)-F18 offering (R)-F18 and (S)-F18 was achieved by a chiral stationary phase prepared HPLC. Their absolute configurations were determined by measurement of their electronic circular dichroisms combined with modem quantum-chemical calculations. Further investigation revealed that (R)-F18 and (S)-F18 shared a similar anti-HIV activities, however, (R)-F18 was more potent than (S)-F18 against wild-type virus, K101E mutation and P225H mutation pseudoviruses.
