ABSTRACT

Drowning is a leading cause of accidental injury in children and has a great impact on family and society. The prevention and treatment of drowning is of great importance for reducing mortality rate. This consensus reviews the literature on the epidemiology, rescue, resuscitation, and acute clinical management and prevention of drowning. The panel determines the score of available evidence according to the criteria of Oxford Centre for Evidence-Based Medicine and then makes recommendations on evidence based on such criteria, so as to provide a basis for further reducing the mortality and disability rates caused by drowning.

ABSTRACT

OBJECTIVE@#To investigate the imaging characteristics of F-FDG positron emission computed tomography (F-FDG PET/CT) in multiple myeloma (MM) patients and to analyze its application value in MM and bone metastases.@*METHODS@#A retrospective analysis was made on MM patients (n=72) and bone metastases patients (n=50) admitted to Hainan Western Central Hospital from January 2017 to March 2019. All patients underwent F-FDG PET/CT examination. The distribution of lesions, bone destruction, maximum standardized uptake (SUV) and metabolic homogeneity were analyzed in both groups.@*RESULTS@#More than 80% of MM and bone metastases involved thoracic bone, spine and pelvis, followed by limbs. MM was more common in the lesions of thoracic bone and skull than those in bone metastases, the difference was statistically significant (P<0.05). The majority of MM patients presented osteolytic bone destruction (97.2%), mostly showing "insect-like phagocytic pattern", so the bone showed dilated changes, and osteogenic changes were rarely seen (2.8%). Osteolytic bone destruction accounted for 74.0% in patients with bone metastatic tumor, presenting "focal" appearance more often, and osteogenic changes accounted for 26.0%. Osteolytic bone destruction in patients with MM was significantly higher than that in patients with bone metastases(χ=14.757,P<0.05). The SUV of MM (4.25±2.16)was significantly lower than that of bone metastases (7.84±3.25) (t=6.830, P<0.05). Diffuse mild uptake of F-FDG was more common in patients with MM, and heterogeneous high uptake of F-FDG was more common in patients with bone metastasis, the difference was statistically significant (P<0.05).@*CONCLUSION@#F-FDG PET/CT examination is helpful to acquire the imaging features of bone structure and metabolic changes, and shows an important clinical value in the differential diagnosis of MM and bone metastases.

ABSTRACT

OBJECTIVE@#To study the clinical effect and safety of double filtration plasmapheresis (DFPP) combined with double pulse therapy with methylprednisolone (MP) and cyclophosphamide (CTX) in the treatment of children with severe Henoch-Schönlein purpura nephritis (HSPN).@*METHODS@#A total of 60 children with severe HSPN who were admitted to the hospital from January 2014 to March 2018 were enrolled and were randomly divided into an observation group and a control group (n=30 each). In addition to routine treatment, the children in the control group were given MP+CTX pulse therapy. Those in the observation group were given DFPP treatment in addition to the treatment in the control group, with three courses of treatment in total. After three courses of treatment, the two groups were compared in terms of 24-hour urinary protein, urinary microproteins, renal function parameters, adverse reactions, and clinical outcome.@*RESULTS@#After three courses of treatment, the observation group had significantly greater reductions in 24-hour urinary protein, urinary albumin, urinary immunoglobulin G, urinary β2-microglobulin, serum creatinine, and blood urea nitrogen than the control group (P0.05).@*CONCLUSIONS@#Compared with MP+CTX pulse therapy alone in the treatment of severe HSPN in children, DFPP combined with MP+CTX pulse therapy can further alleviate renal injury and improve clinical outcome and does not increase the incidence rate of adverse reactions.

ABSTRACT

Mushroom cultivation has gained increased attention in recent years. Currently, only four types of spawn, including sawdust spawn, grain spawn, liquid spawn, and stick spawn, are commonly available for mushroom cultivation. This limited spawn diversity has led to difficulty in selecting suitable inoculum materials in some cultivation. In this study, three small blocks of lignocellulosic agro-wastes and one block of a synthetic matrix were prepared as support for growing Pleurotus ostreatus in liquid medium. Mycelium-adsorbed blocks were then evaluated for their potential as block spawn for fructification. Our results indicated that the edible fungus was adsorbed and abundantly grew internally and externally on loofah sponge and synthetic polyurethane foam (PUF) supports and also has the ability to attach and grow on the surface of sugarcane bagasse and corncob supports. The mycelia of P. ostreatus adhered on corncob exhibited the highest metabolic activity, while those on the PUF showed the least activity. Mycelial extension rates of block spawns made of agro-waste materials were comparable to that of sawdust spawn, but the block spawn of PUF showed a significantly lower rate. No significant differences in cropping time and yield were observed among cultivations between experimental block spawns and sawdust spawns. Moreover, the corncob block spawn maintained its fruiting potential during an examined period of 6-month storage. The developed block spawn could be practically applied in mushroom cultivation.

ABSTRACT

Copy number aberrations (CNAs) in chromosome arm 8q have been associated with unfavorable clinical outcomes of several cancers and progressive tumor characteristics of hepatocellular carcinoma (HCC).This study was to identify correlation of CNAs in 8q with clinical outcomes of HCC patients,and further screen for differentially expressed genes in outcome-related CNAs.Array comparative genomic hybridization and expression arrays were performed to detect CNAs and expression levels,respectively.The correlations between CNAs in 8q and outcomes were analyzed in 66 patients,with a median follow-up time of 45.0 months (range,2.6-108.6 months).One hundred and nine cases were further evaluated to identify differentially expressed genes in the potential outcome-related CNAs.Copy number gain in 8q was observed in 22 (33.3 %) of the 66 HCC cases.The most recurrent gains (with frequencies >20%) were 8q13.3-21.3,8q21.3-23.3,8q23.3-24.13,8q24.13-24.3,and 8q24.3.Survival analysis showed that 8q24.13-24.3 gain was significantly associated with reduced overall survival (P=0.010).Multivariate Cox analysis identified 8q24.13-24.3 gain as an independent prognostic factor for poor overall survival (HR=2.47;95%CI=1.16-5.26;P=0.019).A panel of 17 genes within the 8q24.13-24.3 region,including ATAD2,SQLE,PVT1,ASAP1,and NDRG1 were significantly upregulated in HCCs with 8q24.13-24.3 gain compared to those without.These results suggest that copy number gain at 8q24.13-24.3 is an unfavorable prognostic marker for HCC patients,and the potential oncogenes ATAD2,SQLE,PVT1,ASAP1,and NDRG1 within the regional gain,may contribute coordinately to the 8q24.13-24.3 gain-related poor prognosis.

ABSTRACT

Exposure to free silica induces silicosis and myofibroblasts are regarded as primary effector cells. Fibrocytes can differentiate into myofibroblast. Therefore, the present study was designed to investigate whether fibrocytes participate in silicosis. The rat model of silicosis was established. Hematoxylin-eosin stainings and Masson stainings were used to evaluate the histopathology and collagen deposition. Flow cytometry and immunofluorescence were performed to detect the number of fibrocytes and their contribution to myofibroblasts. Results showed that fibrocytes participate in silicosis. Trend analysis of different sources of myofibroblasts during silicosis indicated that fibrocytes and lung type II epithelial cell-derived myofibroblasts play an important role in the early stage of silicosis, while resident lung fibroblast-derived myofibroblasts play a predominant role during the fibrosis formative period.

ABSTRACT

In recent years, extracellular vesicles are found as an important medium for intercellular signal communication in prokaryotic and higher eukaryotic cells for regulating a variety of biological processes.Extracellular vesi-cles include exosomes,microvesicles and apoptotic bodies,and can be released into extracellular media by almost all types of cells in vivo and in vitro.Extracellular vesicles are released under physiological and pathological conditions, including liver diseases,and have a wide range of effects on the target cells.This review summarizes the progress in understanding the role of extracellular vesicles in chronic liver diseases.Specifically, how extracellular vesicles regulate non-alcoholic steatohepatitis,alcoholic liver disease, viral hepatitis, liver fibrosis and hepatocellular carcinoma is discussed in detail highlighting extracellular vesicles as a promising therapeutic target for chronic liver diseases.

Methods Hospital medical data were classified, and kinds of data layering technologies were applied to constructing the data center,and overall considerations and hierarchical arrangement were executed for types of medical data.The high efficiencies and high feasibility of business systems were realized to ensure data security. Results Classified and hierarchical storage technology enhanced business response and reliability with the same hardware infrastructure.Conclusion Types of technologies have to be combined according to the characteristics of the business system and its data so as to implement high-efficiency and-reliability storage of hospital medical data.

ABSTRACT

Objective To observe the influence of ratio of endoscope-sheath diameter on renal pelvic pressure during PCNL.Methods The model of 24 isolated adult porcine kidneys were used to imitate percutaneous nephrolithotomy from September 2016 to June 2017.Each tract was established (F12,F14 and F16).Three kinds of endoscopes (F8 all-seeing needle percutaneous nephroscope,F6/7.5 and F8/ 9.8 rigid ureteroscope),input a 200μm laser,were adopted.There were 8 combinations,and 3 kidneys were used for each one.Renal pelvic pressure was measured 3 times repeatedly in every combination with steady irrigation (50,100,150,200,250,300,350,400,450,500 cmH2O).Results The linear equations,ratios of endoscope-sheath diameter and highest renal pelvic pressures for each combination were:F8-F12,Pr =0.026 P0-1.533,0.67,12.2 ± 0.54;F6/7.5-F 12,Pr =0.112 P0-5.001,0.92,51.2 ± 0.93;F8-F 14,Pr =0.010P0 + 1.067,0.58,6.2 ± 0.48;F6/7.5-F14,Pr =0.020P0 + 1.000,0.79,10.8 ± 0.46;F8/9.8-F14,Pr =0.144P0 +20.933,0.87,84.7 ± 1.17;F8-F16,Pr =0.005P0 + 1.067,0.50,2.8 ±0.34;F6/7.5-F16,Pr =0.009P0 + 0.533,0.68,5.1 ± 0.32;F8/9.8-F16,Pr =0.020P0 + 2.200,0.75,12.6 ± 0.56.The combinations of F6/7.5-F12 and F8/9.8-F14 might lead to a high renal pelvic pressure without proper irrigation (<401.7 cmH2O for F6/7.5-F12;< 132.4 cmH2O for F8/9.8-F14).Conclusion In order to keep a safe pressure,a proper ratio of endoscope-sheath diameter (< 0.80) and appropriate irrigation must be considered.

ABSTRACT

Objective To discuss primary experience the clinical use of "All-seeing needle set" combined with Holmium laser to treat the single renal stone lesser than 2 cm in diameter through a super minimal tract (F12).Methods From January 2015 to December 2016,43 patients were enrolled into this retrospective study.There were 23 males and 20 females who were diagnosed as single renal stone less than 2 cm in diameter(age range from 23-65 years).There were 8 upper pole renal stones,13 lower pole renal stones and 22 renal pelvis stones.The mean stone size was (1.63 ± 0.32) cm in diameter (range from 1.2 cm to 2.0 cm)."All-seeing needle system" was applied during percutaneous puncture.After building a F12 minimal tract by fascia dilator,all patients received lithotripsy with Holmium laser.Indwelling catheters for 3 days and an F6 double-J tube was left for 2 weeks without a tube in the percutaneous tract after operation.Finally,KUB and/or urinary CT were used to check the results on day 1.Result All operations were completed successfully.The operation time was 23-65min [averaged (31.0 ±9.2) min].41 cases' renal stones were store free,and the 2 stone residues was clear with later extracorporeal shock wave lithotripsy.No severe complications occurred.Conclusion As an alternative to standard procedures for treatment of renal stones less than 2 cm in diameter,adopting "All-seeing needle set" with Holmium laser through a super minimal tract (F12) is safe,minimally invasive,fast and effective with a low complication rate.

ABSTRACT

Hepatocellular carcinoma ( HCC) is the most com-mon primary liver cancer resulted from various etiological fac-tors, among which HBV infection is the most important reason in China. The development and progression of HCC are regulated by a variety of cytokines and intracellular signaling pathways. In recent years, both clinical and basic studies have shown that fer-roptosis plays an important role in the occurrence of HCC. Fer-roptosis can affect the development of liver diseases by regulating the level of intracellular iron and lipid peroxidation. Many schol-ars believe that ferroptosis can be used as a target for the diagno-sis, prevention and treatment of HCC. In this review, the role of ferroptosis in HCC and its research progress has been summa-rized, including ferroptosis summary, the relationship between ferroptosis and HCC, and the molecular mechanisms of ferropto-sis in HCC.

ABSTRACT

<p><b>OBJECTIVE</b>To evaluate the effects of polyethylene oxide (PEO) on blood perfusion in hind limb skeletal muscles in a rat model of chronic hind limb ischemia.</p><p><b>METHDOS</b>Twelve rat models of chronic hind limb ischemia established by unilateral femoral artery ligation were randomized into PEO and control groups (n=6) and treated with intravenous infusion of PEO and saline through the internal jugular vein every other day for 2 weeks. Contrast-enhanced ultrasonography was performed after the treatments to evaluate the blood flow in the skeletal muscles at different time points and blood flow reserve in the ischemic hind limbs on day 28.</p><p><b>RESULTS</b>Starting from 7 days after femoral artery ligation, blood flow in the ischemic hind limb skeletal muscles was significantly higher in PEO group than in the control group (P<0.05). On day 28, blood flow reserve in the ischemic hind limb was significantly higher (P=0.012), and blood volume was significantly increased in PEO group as compared that in the control group (P=0.024).</p><p><b>CONCLUSIONS</b>PEO can increase blood flow, blood flow reserve and vascular volume in the hind limb skeletal muscles in rats with chronic hind limb ischemia, suggesting that PEO can promote angiogenesis and arterial formation by increasing blood flow shear stress to improve blood supply of ischemic hind limbs.</p>

ABSTRACT

Objective:To explore the analysis of the static absorption compound anesthesia with intravenous anesthesia alone stress reaction and cellular immune function in patients with gastric cancer surgery.Methods:From March 2014 to March 2014 were lines of gastric cancer patients,78 cases of elective surgical procedureS,using the random number table method was divided into control group and experimental group,each group of 39 cases,control group given simple intravenous anesthesia,patients given static absorption compound anesthesia,compared two groups of patients with preoperative 30 min (T0),cut the skin (T1),bi (T2),tube drawing immediate (T3),24 h after surgery (T4) different time points,such as serum C-reactive protein (CRP) and interleukin 2 (IL-2),the level of interleukin 8 (IL-8),CD4,CD8,the number of NK cells,and CD4/CD8 ratio.Re· sults:T2 IL-2,IL-8 point test group dropped to the lowest level,CRP rose to the highest,to T4 point back to the level before anesthesia,IL-2 and the control group,while the level of IL-8 T3 point minimum,CRP rose to the highest,and T4 point is still not fully recovered to the level before anesthesia,CRP T1 and T4 each point test group,IL-2 and IL-8 cytokine levels were significantly better than the control group,with significant difference (P<0.05).T1 point,two groups of patients with CD4,NK cells and CD4/CD8 ratios were significantly decreased,CD8 levels increased significantly,CD4 T3 point test group began to rebound,NK cells and CD4/CD8 ratio,level of CD8 began to decline,and T4 point before anesthesia recovery level,and T4 point control indicators are still relatively preoperative levels have obvious deviation,T1 and T4 each point group CD4,NK cells and CD4/CD8 ratio than the control group obviously on the higher side,had clear statistical significance(P<0.05),and each point CD8 levels have no obvious difference between groups(P>0.05).Conelusions:Different anesthetic methods on gastric cancer surgery the patient's level of stress reaction and cell immune function all can produce certain effect,compared with the conventional simple intravenous anesthesia,the static absorption compound anesthesia can obviously relieve open perioperative stress response in patients with gastric cancer,reduce the inhibition of cellular immune level,is a kind of can effectively guarantee the anesthesia effect and prognosis of patients with ideal anesthesia method.

Sudden sensorineural hearing loss (SSHL),which is a common and frequently encountered disease,is considered to be a medical emergency in otolaryngology.The prevalence of SSHL is increasing in China.The pathogenesis of SSHL is not clear yet.Microcirculatory disorder of inner ear is considered as one of the most important causes of SSHL.In recent years,several reports have found the levels of serum lipids were changed in patients affected by SSHL.The relationship between SSHL and serum lipids was reviewed to provide new ideas for the diagnosis and treatment of SSHL.

ABSTRACT

To investigate the genotype and the biological characteristics of Tick-borne encephalitis virus (TBEV) in Charles Hilary endemic foci in the China-Kazakhstan border area in Xinjiang,ticks were collected by flagging during May to June in 2012 and 2014,and were stored in liquid nitrogen.TBEV strains were isolated from tick samples by inoculating BALB/c mice and BHK-21 cells.The FE gene fragments of TBEV-Far and the S gene fragments of TBEV-Sib were detected by RT-PCR from infected mice brain tissue and BHK cells,and then subjected to sequence alignment.Totally 16 TBEV strains were isolated from Ixodes persulcatus and Dermuceuter silvarum,among 13 strains were Far eastern subtype,three strains were Siberian subtype.It was first time that the TBEV-Sib was isolated in China.The Charles Hilary TBE natural foci were in the China-Kazakhstan border area,and both TBEV-Far and TBEV-Sib co-circulated.

Background and purpose: Ovarian lymphoma (OL) is usually misdiagnosed as ovarian cancer with bulk lymph node invasion (OC-BLN), and vice versa. Therefore, to distinguish these two types of disease, we compared their clinical characteristics in this study. Methods: This study retrospectively reviewed 14 OL and 14 OC-BLN patients from Fudan University Shanghai Cancer Center and Shanghai Eighth People's Hospital. The clinical char-acteristics, image and laboratory examination data were compared. Results: There was no significant difference in age, symptom, fever, weight loss and volume of ascitic fluid between the two groups. Comparing with OC-BLN, OL patients have larger tumor in ovaries [(13.04±5.94) cm vs (7.78±6.38) cm, P=0.033], and higher percentage of solid ovarian tumor (85.71% vs 28.5%, P=0.006). Lactate dehydrogenase(LDH)/CA125 was higher in OL (7.66±8.03) than OC-BLN (0.31±0.27, P=0.009). Using LDH/CA125 to diagnose OL, area under the curve (AUC) was 0.952. When the threshold value was set at 1, the sensitivity and specificity was 91.7% and 100%, respectively. Conclusion: OL and OC-BLN are easily to be misdiagnosed. OL has larger and more solid tumor than OC-BLN. LDH/CA125 can help to distinguish these two diseases and guide clinical decision making.

ABSTRACT

<p><b>OBJECTIVE</b>To compare and analyze the differentially expressed plasma proteome between patients with stable angina pectoris (SAP) and healthy donors to identify the biomarkers for early diagnosis of SAP.</p><p><b>METHODS</b>Plasma samples from 60 patients with SAP and 60 healthy controls were collected. Twenty samples (100 mL each) randomly selected from each group were pooled and after removing high-abundance proteins from the pooled plasma, two-dimensional gel electrophoresis (2DE) was performed to isolate the total proteins. The protein spots with more than 2 fold changes were selected after 2D analysis using software, and the differentially expressed proteins were identified by MALDI TOF/TOF mass spectrometer. ELISA was performed to detect hemoglobin subunit delta (HBD) levels in 40 randomly selected samples from each group for verification of the results of 2DE.</p><p><b>RESULTS</b>A total of 7 differentially expressed proteins were found in plasma samples from patients with SAP, including 3 up regulated proteins (serum albumin, hemoglobin subunit alpha and hemoglobin subunit delta,) and 4 down?regulated ones (apolipoprotein L1, apolipoprotein C3, apolipoprotein E and complement C4B). ELISA results showed that HBD level was increased in SAP plasma, which was consistent with the results of 2DE.</p><p><b>CONCLUSION</b>Patients with SAP have different plasma protein profiles from those of healthy controls, and HBD may serve as a potential specific biomarker for early diagnosis of SAP.</p>

<p><b>OBJECTIVE</b>To investigate the effects of adipose-derived stem cells (ADSC) and non-methylated CpG-oligodeoxynucleotides (CpG-ODN) on the expression of peripheral blood CD4CD25regulatory T (Treg) cells in young mice with food allergy, as well as their immune intervention effects.</p><p><b>METHODS</b>A total of 40 female BALB/c mice were randomly divided into control group, allergic group, ADSC treatment group, and CpG-ODN treatment group, with 10 mice in each group. A mouse model of food allergy was established by intraperitoneal injection and intragastric administration of ovalbumin (OVA) for sensitization and challenge. The mice in the control group were treated with normal saline at the same dose; the mice in the ADSC treatment group were given intraperitoneal injection of ADSC (1×10cells for each mouse) before and after OVA challenge, and those in the CpG-ODN treatment group were given intraperitoneal injection of non-methylated CpG-ODN solution (40 μg for each mouse) at 1 hour before challenge by gavage. The allergic symptom scores were determined for each group after model establishment. ELISA was used to measure the serum level of OVA-IgE. Flow cytometry was used to measure the percentage of peripheral blood CD4CD25Treg cells. Hematoxylin and eosin staining was used for the pathological analysis of the jejunum.</p><p><b>RESULTS</b>The allergic group had significantly higher allergic symptom scores and serum level of OVA-IgE than the control group (P<0.05). There were no significant differences in the allergic symptom score and the serum level of OVA-IgE between the ADSC treatment group and the CpG-ODN treatment group (P>0.05), but these two groups had significantly lower allergic symptom scores and serum level of OVA-IgE than the allergic group and significantly higher allergic symptom scores and serum level of OVA-IgE than the control group (P<0.01). The allergic group had a significantly lower percentage of peripheral blood CD4CD25Treg cells than the control group (P<0.05). The ADSC treatment group and the CpG-ODN treatment group had a significantly higher percentage of peripheral blood CD4CD25Treg cells than the allergic group (P<0.05); there were no significant differences between these two groups or between them and the control group (P>0.05). Pathological results showed structural damage and edema in the jejunal villi, a large number of eosinophils, and lymphocyte infiltration in the allergic group, while the ADSC treatment group and the CpG-ODN treatment group had less structural damage and edema in the jejunal villi, a lower number of eosinophils, and less lymphocyte infiltration.</p><p><b>CONCLUSIONS</b>ADSC and non-methylated CpG-ODN have a certain effect in the treatment of food allergy and can increase the percentage of peripheral blood CD4CD25Treg cells and reduce the level of OVA-IgE. They may be associated with the induction of immune tolerance and these two treatment have comparable effects. Detailed mechanisms of action still need further investigation.</p>

ABSTRACT

<p><b>OBJECTIVE</b>The aim of this study was to investigate the effects of SiO2 on fibrocytes and whether fibrocytes participate in silicosis in vivo.</p><p><b>METHODS</b>A macrophagocyte (AM)/fibrocyte coculture system was established, and AMs were treated with 100 μg/mL SiO2. Flow cytometry was used to detect the number of fibrocytes. Real-time PCR was performed to measure the expression of collagen I, collagen III, and α-SMA mRNA. The levels of collagen I, collagen III, and TGF-β1 protein were determined by ELISA. Immunohistochemical staining was performed to measure α-SMA protein expression. A rat silicosis model was induced by intratracheal instillation of SiO2. Lung histopathological evaluation was conducted using HE and Masson's trichrome staining after 1 and 9 weeks. The number of fibrocytes in peripheral blood or lung tissue of rat was detected by flow cytometry. Double-color immunofluorescence was applied to identify fibrocytes in the lung tissue.</p><p><b>RESULTS</b>Peripheral blood monocytes were found to differentiate into fibrocytes in vitro in a time-dependent manner, and exposure to crystalline silica might potentiate fibrocyte differentiation. In addition, fibrocytes were able to migrate from peripheral blood to the lung tissue, and the number of fibrocytes was increased after SiO2 exposure.</p><p><b>CONCLUSION</b>Silica exposure potentiates fibrocyte differentiation, and fibrocytes may participate in silicosis in vivo.</p>
