ABSTRACT

Objective:To investigate the incidence of hypoalbuminemia in elderly patients undergoing endovascular treatment(EVT)for large vessel occlusion and to analyze the correlation between hypoalbuminemia and prognosis of EVT.Methods:A total of 640 elderly patients receiving EVT due to acute large vessel occlusion, aged(71.5±7.4)years with 377 males(58.9%), were enrolled into the prospective multicenter cohort study from July 2018 to May 2019.The patients were grouped as the low albumin group(serum albumin levels within 24 h after EVT <35 g/L, n=191, 29.8%)and as the normal albumin group(serum albumin levels≥35 g/L, n=449, 70.2%). The primary outcome was 90-day nerve functional independence defined by the modified Rankin Scale score 0-2.Outcomes were assessed as odds ratio( OR)by using multivariable logistic regression analysis. Results:The proportion of prognosis of good nerve functional independence at 90 d was lower in the low albumin group than in the normal albumin group[22.5%(43 cases) vs.41.9%(188 cases), OR=1.88, 95% CI: 1.20~2.95, P=0.01]. The incidence of early neurological deterioration was higher after treatment( OR=0.57, 95% CI: 0.37~0.88, P=0.01), and the proportions of multiple systemic complications were higher( P<0.01)in the low albumin group than in the normal albumin group. Conclusions:Hypoalbuminemia after EVT is correlated with the poor prognosis at 90 d in elderly patients.Elderly patients with hypoalbuminemia after EVT have higher incidences of multiple systemic complications.

ABSTRACT

Hypoxic-ischemic encephalopathy [HIE] is a common and potentially devastating condition in the neonate, associated with high mortality and morbidity. Effective treatment options are limited and therefore alternative therapies such as acupuncture are increasingly used. Previous studies have shown that electro acupuncture promoted proliferation of neural progenitor cell and increased expression of neurotrophic factor in HIE. However, effects of electro acupuncture on downstream signaling pathways have been rarely researched. So, in the present study, we aimed to evaluate the neuroprotective effects of electro acupuncture on HIE and to further investigate the role of GDNF family receptor member RET and its key downstream PI3-K/Akt pathway in the process. A rat HIE model was constructed by the left common carotid artery [LCCA] ligation method in combination with hypoxic treatment. Considering that Baihui [GV20], Dazhui [GV14], Quchi [LI11] and Yongquan [KI1] are commonly used in clinics for stroke treatment and are easy to locate, we chose the above four acupoints as the combination for electro acupuncture treatment which was performed once a day for different time periods. Hematoxylin-eosin [HE] staining and transmission electron microscopy results showed that electro acupuncture could ameliorate neurologic damage and alleviate the degenerative changes of ultra structure of cortical neurons in rats subjected to HIE. And the longer acupuncture treatment lasted, the better its therapeutic effect would be. This was accompanied by gradually increased expression of GDNF family receptor RET at the mRNA level and its downstream signaling Akt at the protein level in the ischemic cortex. These findings suggest that electro acupuncture shows neuroprotective effects in HIE, which at least in part is attributed to activation of PI3-K/Akt signaling pathway

ABSTRACT

Objective To study the distribution of pathogenic bacterium and its sensitivity to antibiotic in severe cerebral vascular disease patients with nosocomial pneumonia. Methods Retrospective studies were done in 42 cases of severe cerebral vascular disease patients in neurological intensive care unit (NICU). Secretion from lower respiratory tract was cultivated and done medicine sensitive test at 3 d、4 d、5 d、9 d、16 d after hospitalized, then the etiologic data of nosocomial pneumonia were analyzed. Results Among 42 cases,15 pathogenic microorganisms and 163 strains were cultivated, the fisrt 4 strains were Staphylococcus aureus,Pseudomonas aeruginosa,Klebsiella and Enterobacter cloacae. The results of medicine sensitive test indicated that Gram-negative bacteria was sensitive to Imipenem, and Gram-positive bacteria was sensitive to Vancomycin. Conclusions The main pathogen of nosocomial pneumonia in severe cerebral vascular disease patients may be Staphylococcus aureus,Pseudomonas aeruginosa. Staphylococcus aureus was sensitive to Vancomycin,so Vancomycin can be act as the first choice drug to deal Gram-positive bacteria. Pseudomonas aeruginosa is relatively sensitive to Amikacin and Ciprofloxacin, while Imipenem is the second, but drug fast is to cephalosporins. So the first two antibiotics can be as the experience drugs to deal Pseudomonas aeruginosa.
