ABSTRACT

ObjectiveTo investigate the possible mechanism of action and combination of medicinals of Zhuyu Pill (茱萸丸) in the treatment of atherosclerosis. MethodsThirteen normal C57BL/6J mice were used as blank group, and 40 ApoE-/- mice of the same strain were randomly divided into model group, Huanglian (Coptis chinensis Franch.) group, Wuzhuyu (Tetradium ruticarpum) group, and Zhuyu Pill group, with 10 mice in each group. Except the blank group, the other groups were fed with high-fat diet for modeling, and the total modeling cycle was 12 weeks. After modelling, Huanglian group was given Huanglian solution 143.34 mg/(kg·d), Wuzhuyu group with Wuzhuyu solution 301.78 mg/(kg·d), Zhuyu Pill group with 261.07 mg/(kg·d) of Zhuyu Pill solution, blank group and model group were given 0.3ml of normal saline. Mice in all groups were gavaged for 12 weeks. The levels of total cholesterol (TC), triglyceride (TG), low density lipoprotein cholesterol (LDL-C) and fasting blood glucose were measured by biochemical method; fasting insulin in serum was detected by enzyme-linked immunosorbent assay (ELISA) and insulin resistance index was calculated; HE staining was used to observe the pathological and morphological changes of aortic tissue, liver and epididymal fat in mice, and oil red O staining was used to observe the lipid deposition of aortic tissue in mice, and the percentage of positive area of aortic HE staining, the percentage of positive area of aortic oil red O staining, the non-alcoholic fatty liver activity score (NAS) score, and the cross-sectional area of epididymal fat were calculated; serum levels of interleukin 1β (IL-1β) and interleukin 18 (IL-18) were detected by ELISA; protein expression of nucleotide-binding oligomerisation structural domain-like receptor family pyridoxal structural domain protein 3 (NLRP3) in aortic tissues was detected by Western blot; and immunofluorescence was used to detect the levels of apoptosis-associated granulocyte-like protein (ASC), which contains the CARD structural domain (ASC) and cysteine-containing aspartic protease 1 (Caspase-1) protein expression; and real-time fluorescence PCR was used to detect NLRP3, ASC, Caspase-1, IL-1β, and IL-18 mRNA expression in mouse aortic tissues. ResultsCompared with blank group, serum TC, TG and LDL-C were increased, insulin was decreased, fasting blood glucose and insulin resistance index were increased in model group (P<0.01). Aortic plaque area increased, liver lipid deposition increased, epididymal fat cells volume increased, liver NAS score increased, and epididymal fat cross-sectional area increased. Serum IL-1β and IL-18 increased, and the expression levels of NLRP3, ASC, Caspase-1 protein and mRNA in aortic tissue increased (P<0.01). Compared with model group, serum TC, TG and LDL-C in Huanglian group, Wuzhuyu group and Zhuyu Pill group decreased, fasting insulin increased, fasting blood glucose and insulin resistance index decreased (P<0.01); aortic plaque area decreased significantly, liver lipid deposition decreased, liver NAS score decreased, epididymal fat cell volume decreased, epididymal fat cross section area decreased, serum IL-1β and IL-18 were decreased, the expression levels of NLRP3, ASC, Caspase-1 protein and mRNA in aortic tissue were decreased (P<0.01), and the improvement of all indexes in the Zhuyu Pill group was better than that in Huanglian and Wuzhuyu groups (P<0.01). ConclusionZhuyu Pill has a good therapeutic effect on atherosclerosis in mice, and the combination of Huanglian and Wuzhuyu has a synergistic effect, the mechanism of which may be related to regulation of the NLRP3/ASC/Caspase-1 aortic signaling pathway, and thus reducing the vascular inflammatory response.

ABSTRACT

The membranous tube of sanjiao (三焦) is not only the path of the transport of fluid and qi, but the way of the invasion of pathogenic factors, therefore, it cooperates with the skin mucous membrane physically and influence on each other pathologically. It is believed that the core pathogenesis of chronic urticaria is pathogens intruding sanjiao, membrane collaterals acute spasm, and fluid and qi disturbance, of which defense qi insufficiency and pathogens intruding sanjiao initiates the disease, while struggle between healthy and pathogenic qi and membrane collaterals acute spasm is the intermediate stage, and disturbed fluid, qi and blood movement is the terminal stage. Following the core treatment principle of dredging sanjiao, the internal treatment is to open striae and interstices and dispel pathogens out using self-made Guben Shufeng Decoction(固本疏风汤)modifications, and the external treatment is to dredge and regulate membrane collaterals, move qi and fluid, and treat sanjiao simultaneously, commonly using cutting therapy on Danzhong (RN 17) to move qi and fluid, seal umbilical therapy on Shenque (RN 8) to supplement and nourish ying-wei (营卫), and natural moxibustion on Xuehai (SP 10) to move blood and unblock collaterals.

ABSTRACT

This article summarized the clinical experience of Professor AI Rudi in staged treatment according to pattern differentiation of systemic lupus erythematosus (SLE). It is believed that SLE is characterized by kidney yin deficiency and heat toxin obstruction as the core disease mechanism. The staged treatment principle is to nourish yin and clear heat, invigorate blood and resolve toxins. In the early stage of heat toxins accumulation, the focus is on treating the branch and strengthen the kidneys as early as possible, recommending self-designed Liangxue Xiaofeng Powder (凉血消风散) with modifications. In the middle stage, there is yin deficiency with internal heat, which requires nourishing kidney yin and eliminating pathogenic factors. The prescription suggested is self-designed Shouwu Dihuang Decoction (首乌地黄汤). In the late stage, there is yin impairment affecting yang, which requires a gradual consolidation of the foundation and harmonization of yin and yang, usually with Zhenwu Decoction (真武汤) and Zishen Tongguan Powder (滋肾通关散) with modifications. At the same time, the treatment emphasizes the integration of Chinese and Western medicine, and use of self-designed Ershen Dihuang Pill (二参地黄丸) after initial remission to consolidate the effect and prevent relapse. External treatment should also be given importance to, and Hongyou Ointment (红油膏) can be used to treat SLE skin lesions, reduce swelling, and relieve pain.

ObjectiveTo reveal the correlation of Rehmannia glutinosa-soil feedback process with the formation of its continuous cropping obstacles through the identification of the root exudates of R. glutinosa and analysis of the specific rhizomicrobes recruited by the root exudate. MethodThe root exudates of R. glutinosa seedlings germinated under sterilized condition and those enriched in the rhizosphere of R. glutinosa cultivated in the field were collected and analyzed using the ultra-performance liquid chromatography quadrupole time-of-flight mass spectrometry(UPLC-Q-TOF-MS). The highly abundant compounds identified in the root exudates were added into blank soil, and the soil microbial community was profiled using Illumina Miseq sequencing. The bacterial and fungal functions were predicted by PICRUSt and FUNGuild, respectively. ResultThe identification results showed that seven phenylethanoid glycosides were found in R. glutinosa root exudates, and acteoside possessed the highest abundance. In the soil enriched with acteoside, the bacterial genera such as Agromyces, Pseudomonas, Lysobacter, Sphingobium, Pseudoxanthomonas and Sphingomonas were enriched. For the fungi, the genera Neocosmospora, Plectosphaerella and Dactylonectria, and the species such as Neocosmospora rubicola, Plectosphaerella cucumerina, Dactylonectria alcacerensis and Fusarium solani showed higher abundance. The functional analysis indicated the above-mentioned bacterial genera may realize rapid proliferation by utilizing, biodegrading and transforming phenylethanoid glycosides, and some potential fungal pathogens were colonized. ConclusionThe R. glutinsoa-soil feedbacks were likely generated by the phenylethanoid glycosides in the root exudates together with the specific rhizomicrobes. The investigations of R. glutinsoa-soil feedbacks under continuous cropping system are critical to the further understanding of the underlying mechanisms related to its continuous cropping obstacles.

ABSTRACT

Jupi Zhurutang originated from Synopsis of the Golden Chamber (《金匮要略》), which consists of Pericarpium Citri Reticulatae, Caulis Bambusae in Taenia, Ginseng Radix et Rhizoma, Zingiberis Rhizoma Recens, Jujubae Fructus, and Glycyrrhizae Radix et Rhizoma and is used to treat retching. It has been put on the list of Catalogue of Ancient Classical Prescription (First Batch) released by National Administration of Traditional Chinese Medicine. With the bibliometric method, we searched the medical classics containing Jupi Zhurutang and systematically examined the information on the origin of the prescription, the indications, compatibility rule, medicinals in the prescription, dosage and usage, processing method, and decocting method. It was found that there are many versions of Jupi Zhurutang, and there are common grounds of main symptoms, pathogenesis, composition and dosage between the same prescription with different names and different prescriptions with the same name. The prescription which is closest to the original version in Synopsis of the Golden Chamber is mainly used for the treatment of stomach deficiency and qi counterflow without obvious cold or heat. According to the weights and measures, ratio of Pericarpium Citri Reticulatae, Caulis Bambusae in Taenia, Radix Ginseng, Rhizoma Zingiberis Recens, and Radix Ginseng in Synopsis of the Golden Chamber is approximately 6∶2∶8∶5∶1. The Jupi Zhurutang derived from other ancient classics such as Yanshi Jisheng Fang(《严氏济生方》) is a different prescription for hiccups caused by the stomach heat, and the ratio of Pericarpium Citri Reticulatae to Caulis Bambusae in Taenia in this prescription is about 1∶1. It is also found that cold herbs such as Red Poria, Eriobotryae Folium and Ophiopogon Japonicus are added to the formula in later generations. Therefore, the Jupi Zhurutang used in modern times is mostly modified and different from that in Synopsis of the Golden Chamber. This study summarizes the historical evolution of Jupi Zhurutang and identifies the key information, with a view to providing a reference for the rational modification of this prescription in clinical settings and further research.

It is believed that the key pathogenesis is the heat toxin accumulation, yin depletion and collaterals obstruction. The clinical treatment should focus on “heat exuberance” and “yin depletion”. ABSTRACT

This paper reports a rare clinical case of 46 X, i(X)(q10)TS in a female patient with horseshoe kidney and right duplication kidney with renal pelvic and ureteral junction stenosis and double kidney cysts, secondary to right renal calculi and hydronephrosis. After open surgical treatment, the curative effect was definite, which effectively relieved the stenosis at the junction of the renal pelvis and ureter and relieved the clinical symptoms of the patient. The patient was followed up for 3 months and recovered well. When TS is combined with horseshoe kidney with UPJO, open pyeloplasty with partial isthmus and renal fixation are still classic surgical procedures.

ABSTRACT

Objective:To investigate the diagnostic performance of repeat biopsy 68Ga-PSMA PET/CT to distinguish between benign and malignant prostate disease. Methods:The clinical data and medical imaging of thirty-nine patients underwent repeat prostate biopsy were analyzed respectively in this study. The median age of patients was 65 years (range 46-81 years), the median PSA level was 11.0ng/ml (range 5.4-49.8 ng/ml), f/tPSA was 0.15(0.01-16.50)ng/ml, prostate volume was 43.80(7.79-108.63)ml, and PSA density was 0.24(0.09-2.31)ng/ml 2. All patients underwent pre-biopsy 68Ga-PSMA PET/CT and the standard transrectal ultrasound-guided systematic prostate biopsy. Based on the biopsy results, 68Ga-PSMA PET/CT images of all patients were visually and semi-quantitatively analyzed. By visual analysis, 68Ga-PSMA uptake in prostate was defined as focal, multimodal and inhomogeneous, and then the detection rate of prostate cancer in each subgroup was analyzed. The value of the ROC curve in the diagnosis of prostate cancer was analyzed based on the SUV max of prostate cancer(SUV max), tumor-to-normal-prostate background(SUV T/BGp)as semi-quantitative parameters of 68Ga-PSMA PET/CT. Results:Prostate cancer was detected in 18 patients (46.2%) and 12 patients (30.8%) had clinically significant disease. There were 11, 5 and 2 patients with prostate cancer respectively in men with a focal (12 patients), multifocal(7 patients) and inhomogeneous (20 patients) 68Ga-PSMA uptake. The ROC analysis revealed a SUV max 5.3 and SUV T/BGp1.8 as an optimal cut-off level to distinguish between non-prostate cancer and prostate cancer in 68Ga-PSMA PET/CT, the sensitivity and specificity were 100.0% and 85.7% for SUV max (AUC=0.979), 83.3% and 90.5% for SUV T/BGp (AUC=0.915). Conclusions:Pre-biopsy 68Ga-PSMA PET/CT could help to distinguish between benign and malignant prostate disease before repeat prostate biopsy and detect the foci of prostate cancer.

ABSTRACT

Ultrasonography plays an important role in liver surgery of colorectal cancer liver metastases. Recently, besides grey and doppler ultrasonography, contrast-enhanced ultrasonography, elastography and infusion-image technique has developed and attracted increasing interest. Intraoperative ultrasonography is applied for tumor detection, localization and characterization. It is also used for liver resection and planning surgical strategy, which may improve surgical effect and safety. This article discussed the application of intraoperative ultrasonography in liver resection of colorectal cancer liver metastases.

Objective To investigate the value of shear wave elastography ( SWE) to evaluate local advanced rectal cancer after neoadjuvant radiochemotherapy . Methods In a retrospective study ,endorectal ultrasound( ERUS) and endorectal SWE were performed in 73 patients with local advanced rectal cancer before and after neoadjuvant radiochemotherapy . T he mean and maximum values of Young′s modulus for SWE to evaluate the lesions before and after neoadjuvant radiochemotherapy were recorded . According to the postoperative pathological T stage ,the lesions were divided into reduction of T stage group and non‐reduction of T stage group . T he efficacy of ERUS in diagnosing reduction of T stage was calculated ,and the differences of the mean and maximum values of Young′s modulus between reduction of T stage group and non‐reduction of T stage group was calculated ,and the differences between the two groups were compared . ROC curves were constructed by the difference of mean and maximum Young′s modulus of lesions before and after neoadjuvant radiochemotherapy ,respectively ,to evaluate the diagnostic value of the difference in predicting reduction of T stage . Results A total of 57 cases had reduction of T stage after neoadjuvant radiochemotherapy ( 57/73 ,78 .1% ) . The mean and maximum values of Young′s modulus before and after neoadjuvant radiochemotherapy were compared ,and the differences were statistically significant ( all P < 0 .01) . After neoadjuvant radiochemotherapy ,the values of Young′s modulus of the lesions increased with the increase of pT stage . Compared with the mean values of Young′s modulus of the lesions in pT3 stage , the differences of the mean values of Young′s modulus of the lesions in pT 0 ,pT1 and pT2 stages were statistically significant ( all P < 0 .01 ) . Compared with the maximum values of Young′s modulus of the lesions in pT3 stage ,the differences of the maximum values of Young′s modulus of the lesions in pT0 and pT1 stage were statistically significant( all P <0 .01) . T he differences of the mean value and the maximum value of Young′s modulus in the reduction of T stage group and the non‐reduction of T stage group was statistically significant ( all P < 0 .01 ) . T he ROC curve was established and determined by calculation . Taking the average difference of 34 .7 kPa as the best diagnostic threshold ,the average hardness of the lesion after neoadjuvant radiochemotherapy decreased more than 34 .7 kPa to diagnose the reduction of T stage ,the sensitivity ,specificity and accuracy were 87 .7% ,93 .8% and 89 .0% ,respectively . Compared with ERUS , the difference was statistically significant ( P = 0 .032 ) . Conclusions Shear wave elastography is an effective technology to help ERUS in evaluating the lesions of rectal cancer after neoadjuvant radiochemotherapy and has a promising future .

The differences of the mean value and the maximum value of Young′s modulus in the reduction of T stage group and the non-reduction of T stage group was statistically significant(all P<0.01). The ROC curve was established and determined by calculation. Taking the average difference of 34.7 kPa as the best diagnostic threshold, the average hardness of the lesion after neoadjuvant radiochemotherapy decreased more than 34.7 kPa to diagnose the reduction of T stage, the sensitivity, specificity and accuracy were 87.7%, 93.8% and 89.0%, respectively. Compared with ERUS, the difference was statistically significant(P=0.032).@*Conclusions@#Shear wave elastography is an effective technology to help ERUS in evaluating the lesions of rectal cancer after neoadjuvant radiochemotherapy and has a promising future.

ABSTRACT

Objective To investigate the efficacies and differences of spinal cord stimulation (SCS) and pulsed radiofrequency (PRF) in the treatment of herpes zoster neuralgia in the elderly. MethodsSixty-three patients with herpes zoster neuralgia over 50 years old, admitted to our hospital from January 2018 to January 2019, were divided into SCS group (n=31) and PRF group (n=32) by prospective, randomized and controlled study. The stimulating electrodes were placed in epidural spinal ganglion segment for two weeks after percutaneous puncture in SCS group. In the PRF group, the radiofrequency needles were percutaneously punctured to the dorsal root ganglion under the guidance of X-ray. After the location of contrast medium was confirmed, each target was treated by pulsed radiofrequency for 360 seconds, followed by anti-inflammatory compound liquid injection. Numeric Rating Scale (NRS) scores, effective rate (percentage of patients having pain relief>50%) and signficant differences were noted between the two group at each time point (P>0.05).The effective rates of pain treatment of patients from the two group reached to 56.67%-81.25%,and there was no significant difference between the two groups at each time point (P>0.05).The complete pain relief rate reached to 37%-71% in the two groups,and there was no significant difference between the two groups at each time point (P>0.05).Number of patients used analgesics and calcium channel antagonists before and after operation in the two groups showed significant differences (χ2=20.917,P=0.000; χ2=18.273,P=0.000),and number of patients used analgesics and calcium channel antagonists 4,12 and 24 weeks after operation in the two groups was significantly smaller than that before operation (P<0.05); but,there were no significant differences between the two groups at each time point (P>0.05).Conclusion SCS and PFR can effectively treat herpes zoster neuralgia and reduce the incidence of sequelae,without no significant difference in the clinical efficacy.

Anoectochilus, the fresh or dry whole grass of Anoectochilus roxburghii (Wall.) Lindl., has the effects of cooling blood, dispelling wind, dispelling dampness and resolving toxin, resulting in a wide range of clinical applications and large market demand. However, in the planting process, there are problems such as irregular planting management, excessive addition of pesticides, irregular processing links, even the phenomenon of selling adulteration, which seriously affects the quality of the medicine. In order to guarantee the quality and productivity of Chinese herbal medicines, this paper developed the standard operating procedures for the non-pollution Anoectochilus, including the non-pollution cultivation environment of Anoectochilus, planting methods, field management, pest control and quality control, etc., which provides guidance to promote the healthy development of the Anoectochilus industry.

Objective To investigate the value of percutaneous radiofrequency ablation (RFA) in the treatment of large hepatic carcinoma adjacent to diaphragm.Methods Totally 176 patients with larger hepatic carcinoma adjacent to diaphragm (adjacent diaphragm group) and 157 patients with larger hepatic carcinoma not adjacent to the diaphragm (control group) underwent ultrasound-guided percutaneous RFA treatment,and the data of these patients were retrospectively analyzed.The patients,age,gender,the largest diameter of lesions,ablation information,early necrosis rate,recurrence rate,new tumor rate and survival rate were analyzed and compared between the two groups.Results Cases of injection physiological saline below the diaphragm during RFA were significantly higher in adjacent diaphragm group than that in control group (P=0.016).The recurrence rate of adjacent diaphragm group was significantly higher than that of control group (P=0.028).There was no significant difference of patients,age,gender,the largest diameter of lesion,RFA instrument,RFA needles,early necrosis rate,new tumor rate,nor complications rate between the two groups (all P>0.05).There was no significant difference of the 1-,2-,3-,4-and 5-year survival rate between the two groups (P=0.203).Conclusion Large hepatic carcinoma adjacent to diaphragm is more likely to recur after ultrasound-guided percutaneous RFA.The method of injection physiological saline below diaphragm and other individualized treatment plan and strategy should be used during RFA treatment.

ABSTRACT

Objective To investigate the efficacy and prognostic factors of percutaneous ultrasound-guided radiofrequency ablation (RFA) for liver metastases from gastric cancer.Methods Clinical and imaging data of 55 patients with liver metastasis from gastric cancer who underwent percutenous ultrasound-guided RFA were retrospectively analyzed,and the overall survival rates and prognostic factors were assessed.Results The overall survival rates of 1-,2-,3-and 5-year was 70.45%,42.90%,20.32% and 10.16%,respectively.The ablation rate was 94.12% (96/102) 1 month after RFA,and the local recurrent rate was 15.69% (16/102),the new metastasis rate was 52.73% (29/55).Age (P=0.015),tumor number (P=0.011),extrahepatic metastasis before RFA (P=0.026) and chemotherapy after RFA (P=0.031) were significantly prognostic factors.Age (P=0.033),tumor number (P=0.004) as well as chemotherapy after RFA (P=0.001) were independent prognostic factors.The severe complication rate was 1.82% (1/55),while no treatment-related death occurred.Conclusion Percutaneous ultrasound-guided RFA is a safe and effective therapeutic option for liver metastases from gastric cancer.Age,tumor number,chemotherapy after RFA are independent prognostic factors.

ABSTRACT

Objective To investigate the incidence and risk factors of liver abscess after radiofrequency ablation (RFA) for liver cancer.Methods A retrospective study was performed on 1 643 patients from January 2000 to June 2016.All the patients were diagnosed with hepatocellular carcinoma (HCC,n =942),cholangiocellular carcinoma (CCC,n =31) or metastatic liver carcinoma (MLC,n=670).Univariate and multiple Logistic regression analysis were used to evaluate the risk factors of liver abscess.Results The incidence of liver abscess after RFA was 0.79%% (13/1 643).Univariate analysis indicated that the liver abscess was significantly correlated with the history of diabetic mellitus (DM),Child-Pugh level,history of surgery and tumor location (all P<0.05).Multivariate analysis showed that the history of DM,history of surgery and tumor location were independent risk factors of liver abscess after RFA for liver cancer.Conclusion History of DM,surgery and tumor location are important factors that result in liver abscess after RFA.

ABSTRACT

Objective Toinvestigatetheexpressionofsuppressorofcytokinesignaling1(SOCS1)inoverloaded ventricle papillary muscle, so as to understand its expression characteristics in structural remodeling after the overloading and the biomechanical properties of the muscle under cubic jellyfish toxin-1(CfTX-1) pretreatment that can affect cell signal transduction. Methods Abdominal aortic-venous fistula (AVF) were operated in Kunming mice (n=5), and the cardiac left ventricles were harvested after two weeks of fistulation. The mice in normal group were sham operated as a control (n=5). In vitro culture, the left ventricular papillary muscle of normal mice was used (n=20). In the stretching group, the isolated papillary muscles were double-ratio stretched and fixed on silicone plate. In the relaxation group, the muscles were not stretched. A separated subgroup that transfected with SOCS1 plasmids were set in each group of stretching and relaxation. The papillary muscle samples of each group were cultured in culture medium for 3 days at 37 ℃, and then homogenized for extracting total protein. The total protein was separated by 10% sodium dodecyl sulfate-polyacrylamide gel electrophoresis. The 23 ku band with SOCS1 was used as the target band, and the integrated optical density (IOD) value was measured by computer image analysis method. The expression of SOCS1 protein was detected by Western Blot and the imprinted IOD value was also measured. The papillary muscle in the stretching group was stretched by micro-positioned stretching method, and the initial load was 1 g. After stabilization, the papillary muscle was stretched by 15 mm for continuously 5 times, and the passive tension characteristic curves during the first and fifth stretching were observed and recorded. The peak passive tension (PTmax) and its deceleration velocity (DV) of the papillary muscle were calculated based on the curves. Results Comparing with the AVF group, the normal group had higher IOD values of 23 ku band and SOCS1 blot in total protein of the papillary muscle, and the differences were statistically significant (all P<0.01). The IOD value of 23 ku band in the SOCS1 transfected stretching group was significantly higher than those of the two relaxation groups, and the differences were statistically significant (all P<0.01). However, the difference of this value was not statistically significant between the two relaxation groups. The average IOD value of SOCS1 blot in the SOCS1 transfected stretching group was higher than those of the normal stretching group and the SOCS1 transfected relaxation group, and the differences were statistically significant (all P<0.01). Comparing with the normal group, the AVF group had higher PTmax and ultimate PTmax of the papillary muscles, and had a lower DV values, and the differences were statistically significant (all P<0.01). Conclusions The expression of SOCS1 is sensitive to tension load, and has a positive effect as an overload-sensitive signal in improving myocardial adaptability, protecting myocardial structure and maintaining systolic and diastolic function. CfTX-1 also has a positive effect on improving the compliance of ventricular papillary muscles.

Objective To evaluate the value of endorectal elastography with strain ratio to estimate local advanced rectal cancer after neoadjuvant radiochemotherapy.Methods In a retrospective study, endorectal ultrasound,endorectal elastography and enhanced rectal MRI were performed in 67 patients with local advanced rectal cancer after neoadjuvant radiochemotherapy.The imaging results were compared with postoperative pathological T stage and NCCN TRG.Results There was no significant difference in the diagnosis accuracy between T stage of ERUS(55.2%)and MRI(56.7%).Endorectal elastography results showed lesions confined to the rectal wall(T0-2 stage)were softer than lesions invaded the peripheral fat (T3)and the difference was statistically significant(P <0.05).When the cut-off point was set at SR<2.78,the sensitivity,specificity and accuracy of diagnosis of T0-2 were 64.7%,87.5% and 70.1% respectively.The lesion tended to have a greater SR value when residual tumor components increased(a higher NCCN TRG).Conclusions Endorectal elastography is an useful and effective imaging method to evaluate local advanced rectal cancer after neoadj uvant radiochemotherapy.It can help ERUS and rectal MRI to evaluate the lesions.

ABSTRACT

The analysis of stable nitrogen isotopic composition (δ15 N) of individual amino acid was recognized as an effective method for estimating the trophic level of organisms and detecting the nitrogen flow in food webs. In this study, we evaluated a two-stage procedure of esterification followed by acylation, in which biological samples underwent hydrolysis in acid and the released individual amino acids were derivative into the corresponding N-pivaloyl-iso-propyl ( NPP ) esters for gas chromatography-combustion-isotope ratio mass spectrometric ( GC-C-IRMS) analysis. A total of 13 kinds of individual amino acid derivatives were baseline separated on a nonpolar gas chromatography column (DB-5ms). The amount of sample for each test was not less than 20 ng N on column. High correlations were observed between the δ15 N values respectively obtained by GC-C-IRMS and element analysis-isotope ration mass spectrometry (EA-IRMS). Furthermore, the mean precision of this method was better than 1‰. Cation-exchange chromatograph was used to purify the samples, and the difference of the detection δ15 N values before and after purification by the resin was within 1‰. This method was applied to estimate the trophic level of various natural freshwater organisms from Aha Lake. The present study provided a new idea for the application of stable nitrogen isotope (δ15 N ) in the trophic level estimation of organisms and metabolism analysis of amino acid.
