ABSTRACT

OBJECTIVE@#To explore the relationship between plasma sST2/Reg3α levels and acute graft-versus-host disease (aGVHD) in children after allogeneic hematopoietic stem cell transplantation (allo-HSCT).@*METHODS@#The clinical data of 29 pediatric patients received allo-HSCT treatment in Department of Hematology and Oncology of Wuhan Children's Hospital from January 2019 to January 2020 were collected. Peripheral blood samples were collected at 14 and 28 day after allo-HSCT. The plasma concentrations of sST2 and Reg3α were detected by Luminex assay.@*RESULTS@#Among 29 patients there were 15 males and 14 females with a median age of 53 (29-117) months. After allo-HSCT, 18 patients developed grade 0-I aGVHD; while 11 patients developed grade II-IV aGVHD. These included skin aGVHD in 6 cases, gastrointestinal aGVHD (GI-aGVHD) in 3 cases and gastrointestinal/skin aGVHD in 5 cases. Plasma sST2 level in II-IV aGVHD group showed significantly higher than that in 0-I aGVHD group at 28 days after allo-HSCT [101.81 (73.94-150.77) ng/ml vs 48.97 (28.82-56.69) ng/ml, P=0.021]. Also, the plasma sST2 level was significantly higher in GI-aGVHD group than that in no-aGVHD group at 28 days after allo-HSCT [118.74 (87.00-243.36) ng/ml vs 48.97 (23.55-61.40) ng/ml, P=0.004]. Plasma sST2 level ≥65.34 ng/ml at 28 days after allo-HSCT showed a sensitivity of 85.7% and a specificity of 87.5% in predicting II-IV aGVHD. And the patients with a plasma sST2 level ≥65.34 ng/ml showed a significantly higher incidence of II-IV aGVHD than those with plasma sST2 level of < 65.34 ng/ml after allo-HSCT (P=0.021). There was no significant difference in plasma Reg3α level between the patients with II-IV aGVHD and the non-aGVHD ones.@*CONCLUSION@#The increasing plasma sST2 level after allo-HSCT in children indicates the development of II-IV aGVHD, so sST2 is promising as a biomarker for predicting II-IV aGVHD.

ABSTRACT

OBJECTIVE@#To summarize the long-term efficacy of cyclosporine (CsA) in the treatment of non-severe aplastic anemia (NSAA) in children, and explore the early significant indicators.@*METHODS@#Data of 36 NSAA children in Department of Hematological Oncology, Wuhan Children's Hospital, Tongji Medical College of Huazhong University of Science and Technology from January 2013 to December 2017 were analyzed retrospectively. All the children received oral CsA immunosuppressive therapy, and CsA trough concentration was checked to maintain at the rage of 200-250 μg/L after 2 weeks. The evaluation time points were at 3, 6, 12, 18 and 24 months, and assessment items were peripheral white blood cell differential count and reticulocyte's percentage and count.@*RESULTS@#The 36 NSAA cases were composed of 16 males and 20 females, whose median age was 5.46 (2.92-7.99) years old, and median follow-up time was 28.00 (10.00-38.25) months. After taking oral CsA for 24 months, the number of cumulative effective cases was 21. There were 4 cases of complete remission (CR), 17 cases of partial remission (PR), and 15 cases of non-remission (NR). The total effective rate was 58.33%, and median effect-acting time of CsA was 3.0 (0.5-10.0) months. Compared with ineffective group, neutrophil (NEU) and red blood cell (RBC) of effective group (CR+PR) began to increase significantly at the 3rd month, and hemoglobin (Hb), platelet (PLT) and white blood cell (WBC) increase significantly at the 6th month after oral CsA administration (P<0.05). Except for 2 cases who received component transfusion within 3-12 months after taking oral CsA for 3 months in effective group, the others did not need.@*CONCLUSION@#The overall effective rate of oral CsA in children with NSAA was 58.33%. Stopping blood transfusion after the 3 months of treatment may be considered as a turning point for disease outcomes, and levels of NEU, RBC at the 3rd month and Hb, PLT, WBC at the 6th month as indicators for predicting disease prognosis.

ABSTRACT

This project aimed to explore the protective effect of ginsenoside Rg_1 on hypoxia/reoxygenation(H/R)-induced H9 c2 cardiomyocyte injury and its underlying signaling pathway. The H/R model of H9 c2 cardiomyocytes was established and then the cells were divided into different treatment groups. CCK-8(cell counting kit-8) was used to detect the activity of cardiomyocytes; Brdu assay was used to detect the proliferation of H9 c2 cells; the caspase-3 activity was tested, and then the protein expression was assessed by Western blot. Flow cytometry was used to evaluate the apoptosis level of cardiomyocytes. Ginsenoside Rg_1 inhibited H/R-induced cardiomyocyte apoptosis and caspase-3 activity, promoted nuclear transcription of nuclear factor erythroid-2 related factor 2(Nrf2), and enhanced the expression of the downstream heme oxygenase-1(HO-1). Ginsenoside Rg_1 could increase Nrf2 nuclear transcription and HO-1 expression with the increase of concentration(10, 20, 40, 60 μmol·L~(-1)). However, the protective effect of ginsenoside Rg_1 on cardiomyocytes was significantly weakened after the transfection of Nrf2-siRNA. Ginsenoside Rg_1 could protect cardiomyocytes by activating the Nrf2/HO-1 pathway.

ABSTRACT

Objective To evaluate clinical utility of the personalized drug delivery decision support system, Smart Dose, applied to the monitoring of therapeutic drugs in Chinese population. Methods Use Smart Dose system to predict the trough concentration of vancomycin in patients, analyze the difference between the predicted value and the measured value of the trough concentration, and to evaluate the prediction performance of the system for vancomycin blood concentration. Results Smart Dose adjusts the difference between the predicted value of concentration and the measured value, the average percentage error, and the average absolute percentage error is less than the difference between the predicted value of initial concentration and the measured value. The difference between the initial concentration prediction value and the measured value of the neurosurgery group was smaller than that of the non-neurosurgery group, and the prediction efficiency was better than that of the non-neurosurgery group. The predicted initial concentration of the high trough concentration group and the low-age group (<59 years old) are closer to the measured value. The predictive performance of different BMI for the initial concentration is similar. Conclusion Smart Dose system is more suitable for predicting the adjusted concentration of vancomycin; When used for initial concentration prediction, the prediction values of neurosurgery group, high trough concentration, and low age group are more accurate. Different BMI has similar performance in predicting initial concentration.

ABSTRACT

Objective To determine the in vitro recovery rate and influencing factors of cefradine microdialysis. Methods Two different methods (loss method, gain method) of microdialysis concentration and LC-MS/MS were used to determine the in vitro recovery rate of cefradine. The effect of flow rate and concentration of the perfusate on the recovery rate were investigated. To explore the feasibility of microdialysis technology for pharmacokinetic studies in cefradine. Results The LC-MS/MS analysis method was linear in the required range and the method was sensitive and reliable. There was no significant difference in the recovery rate measured by gain or loss method. Under the same conditions, the in vitro recovery of the probe decreased with increasing flow rate, independent of the drug concentration around the probe. Conclusion Microdialysis technique could be used to study the pharmacokinetics of cefradine, and loss method could be used to determine the in vivo recovery rate and pharmacokinetics of cefradine on microdialysis.

ABSTRACT

Objective::To explore the effect of strong light stress on the growth, physiological and biochemical and key enzyme gene expression of the Atractylodes lancea, in order to provide the scientific basis for the standardized cultivation of the A. lancea. Method::The two-year-old A. lancea seedlings were taken as experimental materials. Poplar forest (light transmittance between 18.26%-36.04%) was taken as control group(ck). Different density shading networks were used to simulate different degrees of high light stress (51.10%, 80.73%, 100%) in late July. The growth state of A. lancea was observed. On the 0th, 5th, 10th, 15th, 20th days, the physiological and biochemical indexes of malondialdehyde (MDA) content, cell membrane permeability, proline (Pro) content, antioxidant enzyme activity and chlorophyll content in the leaves of A. lancea were measured. The relative expression levels of 3-hydroxy-3-methylglutarate monoacyl coenzyme A reductase (3-hydroxy-3-methylglutaryl coenzyme A, HMGR) and farnesyl pyrophosphate synthase gene (farnesyl pyrophosphate synthase, FPPS) in leaves of A. lancea under intense light stress were determined by real-time fluorescence quantitative PCR(Real-time PCR). Result::After strong light stress, the color of the leaves of A. lancea changed from dark green to light green and yellowish green, and the burn of leaves became more and more serious. The contents of MDA, conductivity and Pro showed an upward trend with the increase of transmittance. Peroxidase (POD), superoxide dismutase (SOD) and catalase (CAT) tended to increase first and then decrease. The chlorophyll content decreased with the increase of light transmittance. The relative expression of HMGR in leaves of A. lancea decreased with the increase of light transmittance, while FPPS increased first and then decreased. Conclusion::The results showed that A. lanceaa could alleviate the inhibition of strong light stress by increasing the activity of antioxidant enzymes and regulating the content of osmotic pressure under certain strong light stress. Excessively strong intensity light stress leads to disequilibrium of metabolic mechanism of A. lancea, and seriously inhibits the plant growth.

ABSTRACT

BACKGROUND: Hanshi Bi granule has been used to treat ankylosing spondylitis in the clinic, but the pharmacological mechanism B is still unclear. OBJECTIVE: To screen the compound and drug targets of Hanshi Bi granules based on network pharmacology, and construct a compound-target network to systematically investigate the pharmacological mechanism for treating ankylosing spondylitis. METHODS: The compounds and drug targets of Hanshi Bi granules were collected from TCMSP database. Ankylosing spondylitis targets were from DRUGBANK, GeneCards, Home-OMIM-NCBI, and PALM-IST databases. The obtained drug targets and disease targets were corrected by Uniprot database and common genes were obtained using Draw Venn Diagrams analysis tool. PPI analysis was performed on the common gene in combination with the STRING database and Cytoscape 3.6.1. GO enrichment analysis and KEGG pathway enrichment analysis were performed on the common gene by using Cytoscape 3.6.1 plugin ClueGO. RESULTS AND CONCLUSION: (1) Totally 69 active ingredients and 142 drug targets were obtained from TCMSP, and 595 ankylosing spondylitis targets were obtained using DRUGBANK, GeneCards, Home-OMIM-NCBI, and PALM-IST databases. (2) Using the Draw Venn Diagrams tool to analyze all targets, 39 common genes were obtained. Through PPI analysis, tumor necrosis factor, interieukin-6 and prostaglandin-in-peroxidase 2 were highly connected in the PPI network. (3) Through GO enrichment analysis, it mainly involved biological functions such as regulation of reactive oxygen metabolism, fatty acid metabolism, acute inflammatory reaction, neurotransmitter biosynthetic process, and metabolism of arachidonic acid. (4) Through KEGG pathway enrichment analysis, it mainly involved in AGE-RAGE signaling pathway, fluid shear stress and atherosclerosis, irtterieukin-17 signaling pathway, nuclear factor-KB signaling pathway, tumor necrosis factor signaling pathway, and Toll-like receptor signaling pathway. (5) This study preliminarily predicts the pharmacological mechanism underlying the treatment of ankylosing spondylitis with Hanshi Bi granules, and provides new ideas for secondary development of old drugs and experimental research.

ABSTRACT

Objective:To investigate the effect of dexmedetomidine on sedation and hemodynamics in patients with alcohol withdrawal syndrome (AWS).Methods:From January 2019 to January 2020, 140 patients with AWS admitted to the Seventh People's Hospital of Hangzhou were divided into control group and observation group according to the random digital table method, with 70 cases in each group.Diazepam was injected intravenously in the control group, and dexmedetomidine was injected intravenously in the observation group. The changes of Ramsay sedation score and hemodynamics before treatment, 3h and 24h after treatment, alcohol withdrawal status score(CIWA-Ar) and adverse reactions were compared between the two groups. Results:The Ramsay sedation scores of the observation group at 3h after treatment[(3.43±0.45)points] and 24h after treatment[(4.37±0.41)points] were higher than those of the control group [(2.56±0.36)points and (3.58±0.46)points]( t=12.631, 10.726, all P<0.05). The heart rates in the observation group at 3h and 24h after treatment [(83.24±6.7) times/min, (87.98±5.37) times/min] were higher than those in the control group [(88.29±4.53)times/min and (95.47±3.29)times/min]( t=5.192, 9.951, all P<0.05). The mean arterial pressure (MAP) of the observation group at 3h and 24h after treatment [(72.38±3.10)mmHg, (75.46±2.67)mmHg] were higher than those of the control group [(75.21±2.19)mmHg and (78.63±1.76)mmHg]( t=6.238, 8.294, all P<0.05). The CIWA-Ar score in the observation group at 24h after treatment (19.37±3.10) was lower than that in the control group (26.48±2.76)( t=14.332, P<0.05). The incidence of adverse reactions in the observation group(2.86%) was lower than that in the control group(15.71%)(χ 2=6.869, P<0.05). Conclusion:Dexmedetomidine has obvious sedative effect on the patients with AWS, it can improve the hemodynamics of the patients, and has less adverse reactions.

ABSTRACT

Objective:To explore the effect of intensive nursing mode on the nursing of colostomy complications of patients with rectal cancer after MILES and its influence on psychological status and quality of life.Methods:62 patients with colostomy after Miles operation were selected as the observation group, and 62 patients with Miles operation after Miles operation between February 2015 and January 2017 were selected as the control group. Comparisons of stoma complications within 3 months after operation were compared between the two groups. Changes of psychological status (fear, depression, anxiety, sensitivity and obsessive-compulsive symptoms), quality of life (physical, cognitive, emotional, role and social function) were compared before and after intervention. Results:The total incidence of complications in the observation group was 11.29% (7/62), which was lower than that in the control group (37.10% (23 / 62). The difference was statistically significant ( χ2 value was 11.261). After 3 months of operation, the scores of phobia, depression, anxiety, sensitivity and obsessive-compulsive symptoms decreased, and the scores of body, cognition, emotion, role and social function increased ( P < 0.05). After 3 months of operation, the scores of phobia, depression, anxiety, sensitivity and the scores of obsessive-compulsive symptoms were (2.51 ± 0.49), (2.33 ± 0.57), (2.33 ± 0.57), (2.07 ± 0.54), (2.66 ± 0.53) and the scores of body, cognition, emotion, role and social function was(16.05 ± 5.45), (13.54 ± 4.13), (14.39 ± 4.26), (15.51 ± 4.73), (16.90 ± 5.03) respectively, which were significantly higher than those of the control group (1.93) ± 0.67), (1.47 ± 0.45), (1.83 ± 0.55), (1.44 ± 0.42), (1.82 ± 0.63), and (9.23 ± 3.25), (8.21 ± 2.44), (9.08 ± 3.13), (6.89 ± 2.25), (7.35 ± 2.17). The difference between the two groups was statistically significant ( t values were 4.263-13.730, P < 0.01). Conclusion:Intensive nursing intervention can significantly reduce the complications of colostomy patients after Miles operation, improve the bad psychological and emotional state, improve the quality of life, and the clinical nursing effect is satisfactory.
