ABSTRACT

Chronic hepatitis B is a public health issue worldwide. Nucleotide analogues and interferon therapy can effectively inhibit HBV replication, but they still have the shortcomings such as inability to achieve the clearance of HBV cccDNA and low HBsAg clearance rate. The academic viewpoint of "kidney-tonifying therapy for chronic hepatitis B" provides new ideas and methods for the treatment of hepatitis B. During long-term clinical practice, Department of Hepatology in Shuguang Hospital has identified that "deficiency of spleen and kidney with damp heat remaining" is the key pathogenesis of the continuous progression of chronic hepatitis B and has established the treatment regimen for chronic hepatitis B with the basic treatment method of tonifying the kidney, strengthening the spleen, and promoting diuresis. The clinical research of National Science and Technology Major Project from The 11th Five Year Plan to The 13th Five Year Plan has validated the clinical efficacy of this regimen and clarified that regulating the immune function of the body is the main mechanism of the kidney-tonifying, spleen-strengthening, and diuresis-promoting therapy in the treatment of chronic hepatitis B.

ABSTRACT

OBJECTIVE To provide reference for the clinical application and further research and development of Shenling baizhu powder. METHODS Using the methods of textual research and bibliometrics, the key information such as the composition, dose, processing and usage of Shenling baizhu powder in ancient and modern literature were systematically summarized and sorted out, and the number of publications, keywords, and other contents of modern literature were visually analyzed. RESULTS & CONCLUSIONS Shenling baizhu powder was first collected in Taiping Huimin Hejiju Fang (abbreviated “Jufang”), the composition of core drugs and most widely used dose ratio in previous generations were the same as those in Jufang, basically focusing on powder making and being taken after mixed with liquid; the dosage was concentrated on 2-3 qian; the function and application of it had been expanded to a certain extent compared with Jufang; its clinical application had gradually extended from internal medicine to surgery, gynecology, pediatrics, ophthalmology, dermatology, etc., covering all aspects of the disease caused by spleen deficiency in general; the composition mechanism revolved around “strengthening the spleen, replenishing qi and removing dampness”; the recommended prescription drugs were all selected from the corresponding varieties in the pharmacopoeia. Modern research mainly focuses on clinical practice, with traditional Chinese medicine or traditional Chinese and Western medicine combined with this formula as the main modified treatment for various intestinal diseases, diarrhea diseases, and lung diseases. The efficacy and application are the frontier hot spots today, and the mechanism of action has begun to be explored. The usage in the literature and the inclusion in the pharmacopoeia are inconsistent, and further research is needed.

ABSTRACT

Objective To observe the clinical efficacy of Recipe for Clearing Heat and Removing Dampness combined with Butyl Flufenamate Ointment for the treatment of facial seborrheic dermatitis. Methods A total of 92 patients with facial seborrheic dermatitis were randomly divided into treatment group and control group,46 cases in each group. The treatment group was treated with oral use of Recipe for Clearing Heat and Removing Dampness combined with external use of Butyl Flufenamate Ointment, and the control group was only treated with external use of Butyl Flufenamate Ointment. The treatment for the two groups covered 4 weeks. Before treatment and on treatment day 7, 14 and 28, scores of clinical symptoms, related skin barrier function indexes,and the serum level of cathepsin S(Cat S)in the two groups were observed. Results(1)On treatment day 14,28,the total effective rate in the treatment group was 84.8%, 89.1%,and that in the control group was 65.2%,71.7% respectively,the difference being significant(P < 0.05).(2)On treatment day 7,14,28, the scores of clinical symptoms in the two groups were obviously decreased(P < 0.05 compared with those before treatment),and the decrease in the treatment group was superior to that in the control group (P < 0.05).(3) On treatment day 7,14,28,facial sebum secretin,skin water content,and transepidermal water loss(TEWL) of the treatment group were improved to various degrees compared with those of the control group,the differences being significnat(P < 0.05 or P < 0.01).(4)On treatment day 28,serum Cat S of the two groups was markedly decreased(P < 0.01 compared with that before treatment),and the decrease in the treatment group was superior to that in the control group (P < 0.01). Conclusion Recipe for Clearing Heat and Removing Dampness combined with Butyl Flufenamate Ointment exerts certain effect for the treatment of facial seborrheic dermatitis.

ABSTRACT

[Objective] To observe the clinical effects of filiform-fire needle combined with abdominal acupuncture in treating ankylosing spondylitis(cold-dampness syndrome). [Methods] Sixty patients who met the criteria of AS were included in the study. They were randomly divided into treatment group and control group for 30 patients in each group. The patients of control group were treated with Diclofenac sodium sustained -release tablet and Salazosul-fpyridine intestinal solution, while treatment group were treated with filiform-fire needle combined with abdominal acupuncture on the basis of western medicine mentioned above.All patients were treated and observed for three months, and they were evaluated by score of TCM symptoms, thoracic activity, spinal activity, finger-floor distance, pillow-wall distance and blood parameters, such as C-reactive Protein(CRP), Erythrocyte Sedimentation Rate (ESR)and Immune Globulin A(IgA). [Results] The total effective rate in treatment group was higher than that in control group(P<0.05).After treatment, the two groups all got lower scores of TCM symptoms than before, and the scores of the treatment group were significantly lower than the control group(P<0.05).The thoracic activity, spinal activity, finger-floor distance and pillow-wall distance of all patients improved in varying degrees, and the blood parameters of CRP, ESR and IgA decreased compared with that before treatment, and significantly the treatment group was superior to control group(P<0.05).[Conclusion] The filiform-fire needle combined with abdominal acupuncture can effectively relieve pain, improve functional activity of the spine and peripheral joints, reduce inflammation, slow disease progression and raise the quality of life of the patients.

ABSTRACT

Objective To observe the clinical efficacy of heat-clearing, dampness-removing, and Qi-blood-tonifying Chinese herbal medicine compound for the treatment of recurrent genital herpes(RGH), and to investigate the expression levels of Th17 cells and Treg cells in peripheral blood of patients with RGH as well as their role in the pathogenesis of RGH. Methods Sixty RGH patients were randomly divided into experimental group and control group, 30 cases in each group. The experimental group was treated with Chinese medicine decoction, and the control group was treated with Aciclovir tablets orally. The treatment course covered 6 months. The percentages of Th17 cells and Treg cells in the peripheral blood of the RGH patients were detected with flow cytometry. Results (1) After treatment for 3, 6 months, the total effective rate of the experimental group was 53.33%, 80.00%, and that of the control group was 23.33%, 46.67% respectively. There were significant difference between the two groups(P 0.05). The inter-group comparison showed the decrease of Th17 cell percentage and Th17/Treg cell ratio as well as the increase of Treg cell percentage in the experimental group was superior to that in the control group, the differences being significant (P<0.05). Conclusion The curative effect of Chinese herbal compound for RGH is better than that of the control group. Chinese herbal medicine compound can decrease the level of Th17 cells, increase the level of Treg cells, thus to decrease the ratio of Th17/Treg cells, indicating that heat-clearing, dampness-removing, and Qi-blood-tonifying Chinese herbal medicine compound can correct the imbalance of Th17/Treg cells, and then plays a role in anti-inflammation and immune regulation.

ABSTRACT

Objective To evaluate the therapeutic effect of clearing heat and removing dampness detoxiifcation prescription in the elderly SLE of side effects. Method 68 cases of elderly patients with SLE receiving treatment in our hospital in 2011 April-2013 year in June were randomly divided into observation group and control group, 34 cases in each group. Patients in control group only use hormone therapy, while patients in observation group were added traditional chinese medicine prescription for clearing heat and wet detoxiifcation treatment on basis of this. Finally, the clinical therapeutic effect of two groups and side effects of the drug were analyzed. Results The total effective rate of clinical observation in treatment group was 91.2%, which higher than that of the control group ,and the difference was signiifcant (P<0.05). Compared with control group, the SLEDAI score in observation group was signiifcantly lower, and the difference was signiifcant (P<0.05);drug side effects in observation group was 14.7%, while that in control group was 52.9%, the difference between them was signiifcant (P<0.05). Conclusion Using clearing heat and removing dampness detoxification prescription in elderly patients with SLE treatment, not only can the clinical therapeutic effectiveness of improving, but also can effectively reduce the appearance of drug side effects, improve the life quality of patients, it is worth to promotion in clinical use.

ABSTRACT

ObjectiveTo observe the clinical effect of the treatment of clearing liver fire,removing dampness,strengthening spleen and tonifying kidney in treating middle-aged and old woman with chronic urinary tract infection.Methods82 cases were randomly divided into two groups.42 cases in treatment group were treated with herbal prescription of clearing liver fire,removing dampness,strengthening spleen and tonifying kidney,and 40 cases in control group were treated with sensitive antibiotics.The clinical effect,safety and changes of immune index were compared between two groups.ResultsIn the treatment group,the recent cure rate was 76.8%,the total effective rate was 90.5% and the complete cure rate was 61.9%; while in the control group,the recent cute rate was 50%,the total effective rate 65% and the complete cure rate 27.5%.It showed significant difference between two groups (Z=2.740,P<0.05).There was no significant difference between two groups in hepatic and renal function,serum and urine β2-MG level before and after treatment and it showed a better clinical safety in treatment group.The level of IgA,IgG and IgM increased after the treatment in treatment group.Comparing with the control group,the results showed significant differences(t=9.1,10.5 and 11.2 respectively,P<0.01).In treatment group the level of CD4+was increased,CD8+was decreased,and the ratio of CD4+/CD8+ was increased obviously after the treatment.The differences before and after the treatment and between groups were significant(t=13.3,8.6,P<0.01).In control group the level of CD4+、CD8+、CD4+/CD8 +did not change significantly.The differences before and after the treatment had no statistical significance (P>0.05).ConclusionThe treatment with herbs of clearing liver fire,removing dampness,strengthening slpeen and tonifying kidney had better effect,less side effect and good research prospect.

【Objective】To observe the jaundice-relieving effect of therapy of clearing heat and draining dampness combined with cooling blood and normalizing gallbladder function for severe icterohepatitis with damp-heat in liver and gallbladder.【Methods】Seventy-one severe icterohepatitis patients with damp-heat in liver and gallbladder were randomized into groups A(n=38) and B(n=33).The two groups received routine basic treatment and symptomatic treatment,group A was treated with heat-clearing,dampness-draining,blood-cooling and bile-draining herbs,and group B with intravenous drip of small-dose 654-2 additionally.Serum levels of total bilirubin(TBil),direct bilirubin(DBil),glutamic pyruvic transaminase(ALT),prothrombin activation(PTA) and jaundice duration were observed for the evaluation of jaundice-relieving effect.Meanwhile,the changes of gallbladder ultrasonograms were observed.【Results】After treatment,serum levels of TBil and ALT were decreased in both groups(P0.05)but the markedly effective rate in group A was higher than that in group B(P

[Objective] To investigate the synergistic action of fuming-washing therapy with Qingbixifang ( a herbal lotion mainly composed of Herba Siegesbeckiae, Cortex Phellodendri, Radix Paeoniae Rubra, Radix Rehmanniae, Radix Stephaniae Tetrandrae, Rhizoma Atractylodis, Herba Ecliptae, Nidus Vespae, Olibanum, Myrrha, etc.) on rheumatoid arthritis (RA) with heat retention. [Methods] Sixty patients of RA with heat retention were randomized into groups A and B. Group A ( n = 30) was treated by fuming and washing the affected part with Qingbixifang, and taking Tongbiling Tablets and nimesulide; group B (n = 30) was treated by taking Tongbiling Tablets and nimesulide only. The treatment course of the two groups lasted one month. The time of morning stiffness, grip strength, the number of joints with tenderness, tenderness index, resting pain in joints, the number of swelling joints, swelling index, evaluation by patients themselves and by the doctor were compared before and after treatment. Laboratory parameters such as blood rheumatoid factor (RF), C-reactive protein (CRP), erythrocyte sedimentation rate (ESR), white blood cell (WBC) count and platelet (PLT) count were also observed before and after treatment. [Objective] To observe the therapeutic effect of oral use of heat-clearing, damp-removing and toxicity-clearing herbal medicine combined with ultrasonic wave and herbal medicine mask treatment in treating acne simplex (AS). [Methods] Two hundred and ten cases of AS were randomized into groups A and B. Group A ( n=113) was treated with modified Yinchenhao Decoction and Qingfei Decoction for oral use and added with ultrasonic wave and mask prepared with herbal medicine including Borneolum Syntheticum, Flos Lonicerae, Radix Scutellariae, Radix Salviae Miltiorrhizae, Fructus Forsythiae; group B ( n=97) was treated with metronidazole 0.2 g, tid, and vitamin Bg 20mg, tid, as well as erythromycin ointment for external application. After a 4-week treatment, the therapeutic effect was compared. 【Objective】 To investigate the synergistic action of therapy of strengthening Qi,warming yang,activating blood to promote diuresis on routine western medicine for congestive heart failure(CHF) and to observe its effect on neuroendocrine factors.【Methods】Seventy CHF patients with cardiac function of grade Ⅱ~Ⅳ which were classified into Qi deficiency and blood stasis complicated with fluid retention,were randomized into groups A(n=42) and B(n=28).Group B received routine treatment with digoxin,diuretics,angiotensin converting enzyme inhibitors and ? receptor antagonists.Group A additionally received oral use of decoction of herbal medicines of Radix Astragali,Herba Epimedii,Herba Leonuri,Rhizoma Alismatis,etc.,which has the actions of strengthening Qi,warming yang,activating blood to promote diuresis.Four weeks constituted one treatment course.The therapeutic effect as well as the changes of cardiac function and neuroendocrine factors before and after treatment was observed.【Results】In group A,24 were markedly effective,16 effective,2 ineffective and the total effective rate was 95.24%;in group B,19 were markedly effective,4 effective,5 ineffective and the total effective rate was 82.14%.The therapeutic effect in group A was superior to group B(P

ABSTRACT

【Objective】To observe the influence of medicated diet with spleen-invigorating and phlegm-removing herbs on the therapeutic effect of acupuncture for simple obesity.【Methods】Ninety-six female simple obesity patients with phlegm-damp retention in the interior due to spleen deficiency were equally randomized into two groups by simple random method.Groups A(n=48)and B(n=48)were given dietary control,acupuncture on acupoints of Zusanli(ST36),Pishu(BL20) and Feishu(BL13),and catgutembedment(CE)in auricular points of spleen,stomach and tri-energizer.Group A additionally took a medicated diet for invigorating Qi and strengthening spleen,removing phlegm and draining damp.After one-month treatment,body weight,index of body weight and height were measured,and serum levels of total cholesterol(TC),triglyceride(TG),low-density lipoprotein(LDL)and high-density lipoprotein(HDL)were determined.【Results】Body weight,index of body weight and height,and blood lipid levels were decreased in groups A and B after treatment(P

ABSTRACT

[Objective] To observe the effect of therapy of strengthening spleen to eliminate blood-stasis and fluid dampness for ascites due to liver cirrhosis. [Methods] Sixty - five cases were randomly allocated to Group A and Group B by interval randomization. Group A was treated with Jianpi Xiaogu Decoction (mainly composed of Radix Codonopsis, Polyporus, Rhizoma Atractylodis Macrocephalae, Pericarpium Arecae), one dose per day, which is combined with routine western medicine when necessary and Group B was treated with routine western medicine alone. The treatment course lasted one month and then a half-a-year follow- up was carried out. [Results] After treatment, 3 was cured, 12 markedly effective, 18 effective and 2 ineffective in Group A and 1,5, 14 and 10 respectively in Group B.
