ABSTRACT

Objective: To investigate the application of conventional ultrasound and 16-slice CT examination in clinical diagnosis of hepatocellular carcinoma (HCC) and hepatic focal nodular hyperplasia. Methods: To choose 94 cases patients were receiving treatment in our hospital who were diagnosed as hepatic focal lesion through percutaneous biopsy of hepatocyte or histopathological examination of postoperation, and then take conventional ultrasound and 16-slice CT, respectively, to detect them. To compare and analyze the two methods in series of indicator, such as accuracy, the number of lesion size and the imageology character. Results: Conventional ultrasound diagnosis of benign tumor lesions was consistent with spiral CT results, and the difference was not statistical significance (x2=0.5556, P>0.05). The accuracy of spiral CT for hepatic carcinoma was higher than that of conventional ultrasound, and the difference was significant (x2=8.1987, P<0.05). The consistent rate of the spiral CT group with real result for micro-miniature carcinoma (the diameter less than 1 cm) was higher than that of conventional ultrasound group, and their difference was significant (x2=14.4028, P<0.05). The disappear and persist time of hepatic carcinoma patients were longer than that of non-hepatic carcinoma patients, and the difference between them was significant (t=9.7531, P<0.05). Conclusion: It is higher accuracy that 16-spiral CT can diagnose malignancy lesions in earlier stage.

ABSTRACT

Objective:evaluate the diagnostic use of 16-slice CT cholangiography with multiplanar reformation(MPR)for the assessment of patients with biliary obstruction.Methods:16-slice CT cholangiography with the MPR technique was performed in 60 patients who were thought to have biliary obstruction.No cholangiographic contrast agent was administered.MRCP in 24 patients,Endoscopic retrograde cholangiopancreatography(ERCP)in 16 patients,and 28 patients underwent biopsy or surgery.The findings on 16-slice CT cholangiography were compared with those of MRCP,ERCP,biopsy or surgery.the surgical-pathological result was regarded as gold standard.Results:The findings were as follows:choledocholithiasi(sn=34),malignant stricture(n=14),benign stricture(n=2),and cholelithiasi(sn=1).A small common bile duct stone in one patient could not be detected by 16-slice CT cholangiography.Two patients with initial diagnoses of small common bile duct stone by 16-slice CT cholangiography were disclosed to have malignant bile ductstricture by reference examination.The accuracy of 16-slice CT cholangiography for the diagnoses of bile duct stones and bile duct stricture were 91.1% and 88.9% respectively.Conclusion:16-slice CT cholangiography with the MPR technique is a fast and non-invasive technique with relatively high accuracy for the diagnoses of the causes of biliary obstruction.

ABSTRACT

Objective To study 3D imaging and pelvic angiography for multiple pelvic fracture. Methods 30 cases suspected with multiple pelvic fracture with vascular injury were performed with 3D imaging and pelvic angiography, whose dada were analyzed retrospectively. Results 30 cases had multiple pelvic fracture to different extents, of which 25 ones had no abnormality at lower part of abdominal aorta, left & right common iliac artery, left & right iliac artery, external iliac artery and upper part of left & right femoral artery, and 1 case, complicated with local hematoma, had a raptured left external iliac artery at the middle piece of the ilium, and another two cases, also complicated with local hematoma, had a raptured left iliac artery due to the comminuted fracture of the sacrum and a raptured right anterior femoral artery due to the fracture of light femur respectively. Conclusion 16-slice CT has to be involved in when the patient with multiple pelvic fracture is suspected with vasculaer injury.
