ABSTRACT

Objective To investigate the pathogenistic, clinical and pathologic characteristics and treatment in elder patients with thyroid carcinoma(TC). Methods The clinical and pathologic manifestation, the treatment and prognosis of forty elder patients with thyroid carcinoma were analysed. Results The incidence of TC in elder patients was 14.2% in all of the thyroid carcinomas in the same period. The clinical course was longer and the neoplasm was bigger. A majority of the pathology was follicular adenocarcinoma(35%), followed by adenocarcinoma, undifferentiated carcinoma and pipallary adenocarcinoma. Surgerical management was the primary therapy. 17 cases underwent redical thyroidectomy with neck dissection, 13 total thyroidectomy or subtotal thyroidectomy, 4 palliative resection. Isotopic or radiotherapy as an adjuvant therapy was done if necessary. Five year survival rat was 52.0%, mortality was 45.0% in follow up period. Conclusions The main causes of the poorer prognosis of elder patients with thyroid carcinoma are follows: ① the malignant degree of the tumor is higher; ② the reaction for the tumor in elder patients is duller. So it is important to pay attention to elder patients with thyroid nodule, and an operation should be done as quick as possible.

ABSTRACT

Objective To determine the serum gastrin level, the expression of gastrin in colorectal carcinoma cells and observe the ultrastructure of gastrin secretory granule in colorectal carcinoma cells,in order to explore the relationship between gastrin and clinical behavior of colorectal carcinoma. Methods The serum gastrin and gastrin expression in colorectal carcinoma tissues of the 53 cases were examined by using radio-immunity analysis(RIA), immunohistochemistry and immunoelectron microscopic technique. Results Compared with control group, the preoperative level of serum gastrin in colorectal group was significantly increased, especially in well-differentiation adenocarcinoma . In the tissue of colorectal carcinoma, the gastrin expression rate was 56.6%. The expression rate of well-differentiated adenocaroinoma was higher than that in moderate and poor differentiation adenocarcinoma. Immunoelectron microscopy showed that the granules of protein A-gold (PAG) could be seen in different electro- density secretion granules in carcinoma cells, in intercellular space and on the surface of membrane of microvillus. Conclusions The level of serum gastrin and the expression of gastrin in cancer tissues in colorectal carcinoma patients are increased. The colorectal carcinoma cells may synthesize and secrete gastrin themselves, which may be correlated with clinical behavior of colorectal carcinoma.

ABSTRACT

Objective To study the relationship between the type of histopathology and prognosis of contained mucous adenocarcinoma(CMAC) of stomach and its clinical significance. Methods Eighty-seven cases of CMAC of stomach were observed by means of pathomorphology, histochemistry, immunohistochemistry and follow up etc. Results The malignant behavior of the cancer was significant difference according to the nature and quantity of the CAMC and the primary pathological type of the cancer. The clinical type in simple mucous carcinoma was mainly Borrmann type I;in tubular papilla mucous adenocarcinoma was mainly Borrmman type II; in signet-ring cell mucous carcinoma was mainly Borrmman type IV,and in mixed cell mucous carcinoma was mainly Borrmman type III. There was significant difference in the lymphatic metastasis and survival rate between the 4 groups. Conclusions Correct differentiation of the pathological type of contained mucous adenocarcinoma of stomach is important for guading the treatment and predicting the prognosis.Cathepsin D expression can help for understanding the biological behavior of CMAC of the stomach.

ABSTRACT

Objective To evaluate the role of CD44v6 and matrix metalloproteinase-2(MMP-2) in the invasion and metastasis of gallbladder carcinoma (GC). Methods Immunohistochemical techniques were adopted to detect the expression of CD44v6 and MMP-2 in 42 cases of GC and 10 cases of cholecystolithiasis. Results (1)The expression of CD44v6 in the GC group (64.3%, 27/42) was significantly higher than that in the cholecystolithiasis group (0%,0/10). The expression of CD44v6 was correlated with GC histologic grade (P

ABSTRACT

Objective To determine the correlation between the expression of interleukin 8 (IL 8) and vascularity in human gastric carcinoma speciemens, and to provide a possible new clinical way for anti angiogenesis of tumor . Methods IL 8 mRNA expression was examined with reverse transcription polymerase chain reaction (RT PCR).IL 8 protein secretion level and its specific expression were determined by enzyme linked immunosorbent assay (ELISA).The number of blood vessels in the gastric carcinoma was determined by antibody against factor Ⅷ associated antigen and immunohistochemical analysis. Results Most of the tumors (26/29,89.7%) and metastatic lymph nodes (17/20,85%) expressed IL 8 at higher levels than that in corresponding normal mucosa of resection end of stomach and negative metastatic lymph node. The expression of IL 8 protein and IL 8 mRNA were at the same level. The level of IL 8 mRNA in the gastric carcinoma tissue correlated strongly and positively with vascularization (P= 0.001).Conclusions There is a positive correlation between IL 8 mRNA expression and the vascularity of gastric carcinoma.IL 8 produced by tumor cells may up regulate the microvascularization in the gastric cancer, and enhance the growth, spreading and lymph node metastasis in human gastric carcinoma. So decreasing the IL 8 expression might inhibit the microvascularization, growth and metastasis of gastric cancer.
