ABSTRACT

SUMMARY: Myocardial bridges are inborn anomalies frequently found in authopsies. Although tipically clinically silent, they are occasionally associated with severe clinical manifestations, e.g. myocardial ischemia or even sudden death. The pathophysiology and risk factors for these manifestations have not yet been completely elucidated. The connective tissue underneath the bridge has been considered as one of the factors the symptoms depend on. Thus, the aim of this research was to determine the histological characteristics of the connective tissue lying underneath the myocardial bridge and to contribute to a better understanding of the protective effects this passive compartment might have in prevention of severe clinical manifestations of myocardial bridging. The study was carried out on twenty hearts with myocardial bridges. Length of the bridge was determined using a precise electronic caliper. Sections of the myocardial bridges with the underlying connective tissue were obtained and prepared for qualitative and quantitative analysis. The connective tissue underneath the bridges was composed of adipose tissue and loose connective tissue in different ratios. The tissue underneath thin bridges was predominantly composed of adipose tissue, while loose connective tissue was the dominant component under thick bridges. The myocardial bridges had an average thickness of 0,98 ± 0.44 mm and an average length of 15,25±5,65 mm. We found a strong positive correlation between the myocardial bridge thickness and length (r = 0,860, p = 0,0001). The thickness of the passive connective tissue compartment under the myocardial bridges was 0,58±0,22 mm, and there was no correlation between this parameter and the myocardial bridge thickness (r = -0,011; p = 0,963). In the clinical evaluation of patients with these anomalies it is necessary to take into account independently the myocardial bridge thickness and length on one side and the thickness of the connective tissue lying underneath it on the other.


Los puentes miocárdicos son anomalías congénitas que se encuentran con frecuencia en las autopsias. Aunque típicamente éstos son clínicamente silenciosos, ocasionalmente se asocian con manifestaciones clínicas graves, como isquemia miocárdica o incluso muerte súbita. La fisiopatología y los factores de riesgo de estas manifestaciones aún no se han dilucidado por completo. El tejido conectivo debajo del puente se ha considerado como uno de los factores de los que dependen los síntomas. Por lo tanto, el objetivo de esta investigación fue determinar las características histológicas del tejido conectivo que se encuentra debajo del puente miocárdico y contribuir a una mejor comprensión de los efectos protectores que este compartimento pasivo podría tener en la prevención de manifestaciones clínicas graves de puente miocárdico. El estudio se llevó a cabo en veinte corazones con puentes miocárdicos. La longitud del puente se determinó utilizando un calibrador electrónico preciso. Se obtuvieron secciones de los puentes miocárdicos con el tejido conjuntivo subyacente y se prepararon para análisis cualitativo y cuantitativo. El tejido conectivo debajo de los puentes estaba compuesto de tejido adiposo y tejido conectivo laxo en diferentes proporciones. El tejido debajo de los puentes delgados estaba predominantemente compuesto de tejido adiposo, mientras que el tejido conectivo laxo era el componente dominante debajo de los puentes gruesos. Los puentes de miocardio tenían un espesor promedio de 0,98 ± 0,44 mm y una longitud promedio de 15,25 ± 5,65 mm. Encontramos una fuerte correlación positiva entre el grosor y la longitud del puente miocárdico (r = 0,860, p = 0,0001). El grosor del compartimiento de tejido conectivo pasivo debajo de los puentes miocárdicos era de 0,58±0,22 mm, y no hubo correlación entre este parámetro y el grosor del puente miocárdico (r = -0,011; p = 0,963). En la evaluación clínica de pacientes con estas anomalías es necesario tener en consideración de forma independiente el grosor y la longitud del puente de miocardio por un lado y el grosor del tejido conectivo que se encuentra debajo del mismo por el otro.

ABSTRACT Objective To determine whether passive smoking causes morphological and structural changes in the arcuate arteries of rats exposed for 7 to 28 days. Methods Wistar rats aged eight weeks and weighing 260g on average were allocated to a Control or a Smoker Group. Groups were further divided into 4 groups containing 5 animals each. Morphological-functional analysis of the right kidneys was carried out after 7 and 28 days of exposure to the smoke of 40 cigarettes per day. Cigarettes were burned at set times using automated cigarette-burning equipment ("Smoking Machine" - SM-MC-01). At the end of each exposure period, the kidneys were dissected and submitted to histological processing for morphological and quantitative analysis. Results Exposure to cigarette smoke for 7 days led to a decrease in inner vascular diameter. Decreased thickness of the vascular tunica media was observed after exposure for 28 days. Increased thickness of the tunica adventitia, increased total vascular wall thickness, increased total vascular diameter and qualitative increase in collagen deposition were observed. Vascular volume increased after 28 days of exposure. Conclusion Passive smoking has a negative impact on renal vasculature.

ABSTRACT

Interstitial fluid (ISF) flow through vascular adventitia has been discovered recently. However, its kinetic pattern was unclear. We used histological and topographical identification to observe ISF flow along venous vessels in rabbits. By magnetic resonance imaging (MRI) in live subjects, the inherent pathways of ISF flow from the ankle dermis through the legs, abdomen, and thorax were enhanced by paramagnetic contrast. By fluorescence stereomicroscopy and layer-by-layer dissection after the rabbits were sacrificed, the perivascular and adventitial connective tissues (PACTs) along the saphenous veins and inferior vena cava were found to be stained by sodium fluorescein from the ankle dermis, which coincided with the findings by MRI. The direction of ISF transport in a venous PACT pathway was the same as that of venous blood flow. By confocal microscopy and histological analysis, the stained PACT pathways were verified to be the fibrous connective tissues, consisting of longitudinally assembled fibers. Real-time observations by fluorescence stereomicroscopy revealed at least two types of spaces for ISF flow: one along adventitial fibers and another one between the vascular adventitia and its covering fascia. Using nanoparticles and surfactants, a PACT pathway was found to be accessible by a nanoparticle of <100 nm and contained two parts: a transport channel and an absorptive part. The calculated velocity of continuous ISF flow along fibers of the PACT pathway was 3.6‒15.6 mm/s. These data revealed that a PACT pathway was a "slit-shaped" porous biomaterial, comprising a longitudinal transport channel and an absorptive part for imbibition. The use of surfactants suggested that interfacial tension might play an essential role in layers of continuous ISF flow along vascular vessels. A hypothetical "gel pump" is proposed based on interfacial tension and interactions to regulate ISF flow. These experimental findings may inspire future studies to explore the physiological and pathophysiological functions of vascular ISF or interfacial fluid flow among interstitial connective tissues throughout the body.

BACKGROUND: Peripheral nerve injury has been very common in clinical work. Although microsurgical technique can restore the continuity of the injured nerve well, the nerve repair effect is still unsatisfactory due to the high degree of differentiation and low regeneration ability of the peripheral nerve tissue, which seriously affects the quality of life of patients. At present, there is no unified conclusion on the microenvironment of peripheral nerve injury, and there are many repair methods in common use. OBJECTIVE: To review the microenvironment of peripheral nerve injury and the repair methods of peripheral nerve injury. METHODS: A computer-based online search of PubMed and CNKI databases was performed for the articles published from January 1964 to September 2019. The key words were “peripheral nerve injury; microenvironment; microsurgical technique; small gap bridging” in English and Chinese, respectively. Finally, 57 eligible articles were included to review. RESULTS AND CONCLUSION: (1) After a series of animal experiments and clinical studies, changes in microenvironment such as the establishment of nerve regeneration channels, neurotrophic factors, immune response, inflammatory response, and hormone regulation have been confirmed to be important factors affecting peripheral nerve repair. (2) It is feasible to repair the peripheral nerve injury by using biological conduit small gap bridging instead of the traditional external and fascicular membrane.

ABSTRACT

Abstract Perivascular adipose tissue (PVAT) is a source of factors affecting vasomotor tone with the potential to play a role in the performance of saphenous vein (SV) bypass grafts. As these factors have been described as having constrictor or relaxant effects, they may be considered either beneficial or detrimental. The close proximity of PVAT to the adventitia provides an environment whereby adipose tissue-derived factors may affect the vasa vasorum, a microvascular network providing the vessel wall with oxygen and nutrients. Since medial ischaemia promotes aspects of graft occlusion the involvement of the PVAT/vasa vasorum axis in vein graft patency should be considered.

Background: Coronary artery disease also called asischemic heart disease which develops when majorblood vessels that supply blood to heart becomedamaged or diseased. It is also one of the leading causesof death. Morphometry and elastic fibre distribution incoronary arteries may help in using appropriate sizecatheters for diagnostic procedures. Aim andObjectives: To study the morphometry and elastic fibredistribution in left and right coronary artery. Materialand Methods: Segment of left and right coronaryarteries each having length of 0.5 cm were collectedfrom 30 adult human heart specimens fixed in 10%formalin. Outer and inner diameters were measured.Tissues were processed and stained by verhoeff's VanGeison stain. Stained slides were scanned by “IScapture software” under 10 × magnifications. Results:Outer diameter of left coronary artery is 4.225 ±0.6963mm and right coronary artery is 3.827 ±0.6869mm. Inner diameter of left coronary artery is3.338 ± 0.5853 and that of right coronary is 2.869 ±0.6104mm.Conclusion: Diameter, wall thickness,thickness of tunica intima, tunica media is more in leftcoronary artery than the right. Also left coronary arteryshows more number of elastic fibers than right coronaryartery.

PURPOSE: Adventitial cystic disease (ACD) is a rare condition that causes intermittent claudication and non-atherosclerotic disease without cardiovascular risk factors. The etiology and optimal treatment of ACD remain controversial. The purpose of this study was to analyze surgical treatment results for ACD and to elucidate optimal treatment options.MATERIALS AND METHODS: We retrospectively reviewed 30 patients with ACD who underwent surgery from 2006 to 2018. Twenty-two patients had arterial ACD, six had venous ACD, and two had combined venous and arterial ACD. We reviewed demographic and clinical characteristics, treatment details, and procedure outcomes.RESULTS: Recurrence occurred in 6 cases either after cyst excision alone (4/17) or patch angioplasty (2/2). There was no recurrence after vessel excision with interposition grafting (0/7). Therefore, vessel excision was a statistically significant factor in recurrence prevention (P=0.026). Among the six recurrences, joint connections of the cystic lesions were found in four of the six (66.7%).CONCLUSION: As a curative surgery for ACD, vessel excision with interposition grafting is a better strategy to prevent recurrence than simple cyst excision alone.

ABSTRACT

Two patients were admitted to our department because of recent aggravation of claudication in the leg, which was exacerbated by walking. They were diagnosed as having a Baker cyst or acute thrombosis in the popliteal fossa at another hospital. There was no evidence of ischemia, and the ankle brachial index was normal. Computed tomography and magnetic resonance imaging were performed, revealing a cystic mass of the popliteal artery (PA). Intraoperatively, the cystic lesion was found within the adventitia of the PA; based on the biopsy findings, both patients were diagnosed as having adventitial cystic disease of the PA.

ABSTRACT

More than 700 adventitial cystic diseases (ACDs) have been reported in the literature, with most cases affecting the popliteal artery in young men. Here, we describe our treatment and etiologic consideration of a patient who presented with an ACD of the external iliac artery, known to be an extremely rare location. On preoperative imaging, the ACD had a connection to the nearby hip joint and was treated with resection of the affected segment, including ligation of the joint connection and interposition with a prosthetic graft. The pathogenesis of ACDs is not fully understood; however, we believe that joint connections are important in their development and treatment.

Objective To study the feasibility of microsurgical technique to denervate sympathetic of femoral artery in rabbit, providing a reliable animal experimental model for further study of the mechanism of neuralization in bone tissue engineering.Methods From July, 2014 to July, 2015, 21 New Zealand white rabbits were divided into 4 groups randomly: the control group (n =3), the 4 weeks group (n =6), the 8 weeks group (n =6) and the 12 weeks group (n =6).Bilateral femoral arteries of the 21 rabbits were exposed.Adventitia of femoral arteries in 3 test groups were removed for about 2cm by microsurgical technique, whereas adventitia of the control group remained intact without any treatment.The arteries samples were collected at 4 weeks, 8 weeks and 12 weeks after treatment.The structure of vascular were indicated by hematoxylin-eosin (HE) staining, and the distribution and volume of the sympathetic fibers were evidenced by glyoxylic acid staining and the expression of tyrosine hydroxylase (TH), the marked protein of sympathetic.Results The adventitia of 3 test groups were invisible or lost most of it while the control group remained intact shown by HE staining.For glyoxylic acid staining, the fluorescence intensity value of the control group, 4 weeks group, 8 weeks and 12 weeks were 0.08124 ± 0.00260, 0.02920 ± 0.00206, 0.02661 ± 0.00233, 0.03094 ± 0.00211, respectively (n =6).The distribution and fluorescence intensity of sympathetic nerve were both significantly reduced in test groups compared to the control group (P < 0.05).And there was no statistical difference among the 3 test groups (P > 0.05).Semi-quantitative analysis of the expression of TH was 0.8626 ± 0.03519, 0.3631 ± 0.03019, 0.3964 ± 0.02239, 0.3487 ± 0.02356 respectively, which showed the same tendency as glyoxylic acid staining test.Conclusion Microsurgical technique is promising as an ideal method for the local denervation of sympathetic nerve from artery system as it can significantly reduce sympathetic fibers on adventitia without regeneration during the experimental period.

ABSTRACT

Aim To observe the effect of“Ying-nutrient and Wei-defense unblocking collaterals prescription”on the carotid sympathetic nerve in early atherosclero-sis. Methods The rabbits were randomly divided into 6 groups:Control group, high-fat group, Complex mod-el group, GSTL-H group, GSTL-L group and ATO group. The control group was fed with common food-stuffs, High-fat group with high-fat diet to build early atherosclerosis model, and all the other groups with high-fat diet combined with carotid artery cannula to build early atherosclerosis carotid artery injury rabbit model. All groups were given corresponding medicines intragastrically once a day for 4 weeks. The GSTL-H and GSTL-L group was given Guishaotongluo ultrafine powder 0. 6 g·kg-1·d-1, 0. 3 g·kg-1·d-1, and the ATO group was given suspension of atorvastain so-lution 2. 5 mg · kg-1 · d-1 . Biochemical method was used to detect blood lipid change. HE staining was ap-plied to observe the pathological morphology of intima-media, The content of NE in the carotid arterial was detected by ELISA. The immunohistochemical staining was used to detect the protein expression of NGF, TH around the carotid arterial. The expression of NGF, TH in the carotid artery adventitia was detected by Western blot. Results Compared with normal group, the levels of TC,TG and LDL-C in high-fat group and complex model group were significantly increased( P<0. 01). The degree of the intimal hyperplasia,the con-tent of NE and sympathetic density ( NGF, TH protein expression and distribution ) of the cartoid artery in complex model group were heavier compared with those in high-fat group; the lipid levels, degree of the inti-mal hyperplasia and sympathetic density ( NGF, TH protein expression and distribution ) in the GSTL-H, GSTL-L group were milder in varying degrees compa-ring with those in the complex model group(P<0. 05, P<0. 01 ) . Conclusion “Ying-nutrient and Wei-de-fense unblocking collaterals prescription” can reduce rabbit carotid atherosclerosis, and the mechanism may be related to regulating the sympathetic nerve of arterial wall.

ABSTRACT

In this study, we describe our diagnosis and treatment of two patients who presented with femoral vessel compression caused by a cystic lesion in the groin. One case was diagnosed as adventitial cystic disease (ACD) of the common femoral artery resulting in leg claudication and the other was diagnosed as a ganglion cyst (GC) causing femoral vein compression and unilateral leg swelling. The operative findings differed between these two cases with respect to the dissection of the cyst and femoral vessel, but the postoperative histological examination results were similar. The pathogenesis of ACD and GC is not fully understood, and further investigation is needed to delineate the exact pathology of these uncommon conditions.

ABSTRACT

This study was designed to investigate the activity of Shenlian tablet in stabilization of the atherosclerosis (As) plaque in apoE-/- mice and explore the mechanisms. Rat peritoneal mast cells were randomly allocated and treated with Shenlian tablet (100, 50, 25, 12.5 mg ·L-1) or cromoglicate sodium (200 μg·L-1) for 2 h before exposure to substance P. Histamine, tryptase, IL-1β and NF-κB were measured in the cell culture supernatant by ELISA assay. The plaque formation was induced by common carotid artery cannula method combined with high-fat diet in apoE-/- mice, and the plaque instability was induced by substance P through local mast cell degranulation. Mice were divided into eight groups that included the model 1 (M1, sham-operated group), M2 (carotid artery cannula combined with high-fat diet), M3 (M2 combined with substance P 0.5 μg/mouse), Shenlian extract (95, 190 and 380 mg·kg-1·d-1), atorvastatin (2.6 mg·kg-1·d-1) and normal control group. Total cholesterol (TC), high-density lipoprotein (HDL-C), high-sensitivity C-reactive protein (hs CRP), matrix metalloproteinases 9 (MMP-9) and histamine were measured by ELISA. Thickness, plaque area, mast cell degranulation were observed by hematoxylin and eosin staining, toluidine blue staining. CD117 antigen expression were observed by confocal microscopy. Intracellular phosphorylation was detected using the Bio-Plex 6-plex phosphoprotein assay kit. The results show that the mast cell membrane was stabilized by Shenlian tablet. Histamine, tryptase, interleukin l-β and NF-κB exhibited a significantly reduction in the Shenlian tablet-treated group (PP-/- mice model group. The proliferation, degranulation and inflammation of mast cell were significantly inhibited by Shenlian tablet. On the other hand, the same treatment decreased hs-CRP, MMP-9 and histamine in serum. IκB, p38 MAPK phosphorylation, intraplaque hemorrhage and collagen degradation were reduced in the presence of Shenlian tablet, which increased the stability of the As plaque. The results show that the vulnerable plaque model induced by mast cell activation in adventitia was established. Shenlian tablet exhibited a protective effect in this model. Shenlian tablet may increase the plaque stability via inhibition of mast cell-mediated inflammatory response.

Actualmente se acepta que la adventicia tiene: un importante rol fisiológico al determinar el nivel de nutrición, oxigenación, reparación arterial, regulación de la vasomotricidad, control de la poscarga ventricular, control de la función arterial, etcétera, a la vez que tiene una importante participación en procesos patológicos (por ejemplo, aterosclerosis, hipertensión arterial, génesis de aneurismas de aorta abdominal). Sin embargo, dado lo reciente de la mayoría de los estudios que han redefinido el rol de la adventicia, aún persiste mucho desconocimiento en la comunidad biomédica acerca de la fisiología de la capa adventicia arterial. El presente trabajo tiene como objetivo revisar el rol que actualmente se reconoce para la capa adventicia de la pared arterial.

AIM:To investigate the effect of a treatment proposal , which consisted of tongxinluo , atorvastatin and aspirin, on adventitial inflammation of early atherosclerosis in rabbit carotid artery .METHODS: The atherosclerotic model was established in the rabbits with silicone collar , which was positioned around the carotid arterial adventitia +high-cholesterol diet .New Zealand rabbits ( n =72 ) were randomly divided into 6 groups ( n=12 ): control group , model group, tongxinluo group, atorvastatin group, aspirin group, and three-drug combination group.The rabbits in control group were fed with common foodstuffs , and the rabbits in all the other groups were fixed the right carotid arteries with the silicone tube, and were fed with fatty foodstuffs .The rabbits in tongxinluo group , atorvastatin group and aspirin group were given the suspension of tongxinluo supermicropowder (0.3 g· kg-1· d-1), atorvastatin (2.5 mg· kg-1· d-1) and aspirin (12 mg· kg-1· d-1) respectively,and the rabbits in three-drug combination group were given the suspension of tongxinluo su-permicropowder (0.3 g· kg-1· d-1), atorvastatin (2.5 mg· kg-1· d-1) and aspirin (12 mg· kg -1· d-1) together. The rabbits in each group were fed with the corresponding medicines for 4 weeks .The tissue slices of carotid artery were observed under light microscope with HE staining .The change of blood lipid was detected by biochemical assay .The pro-tein levels of MCP-1, IL-1βand IL-10 in the carotid arterial adventitia were detected by ELISA .The immunohistochemical staining was used to detect the protein expression of IL-8 around the carotid arterial adventitia .RESULTS:Compared with control group, the contents of TC, TG and LDL-C were significantly increased , and the content of IL-10 was significantly decreased in model group .The levels of TC , TG and LDL-C were significantly decreased in tongxinluo group and atorvasta-tin group compared with model group , no significant difference between tongxinluo group and atorvastatin group was ob -served.In the three-drug combination group, the levels of TC, TG and LDL-C were lower than those in atorvastatin group and tongxinluo group .Compared with control group , the contents of MCP-1 and IL-1βwere significantly increased , and the content of IL-10 was significantly decreased in model group .Compared with model group , the contents of MCP-1 and IL-1βwere decreased in tongxinluo group , atorvastatin group and aspirin group , no significant difference between the 3 groups was observed.The content of IL-10 was decreased in three-drug combination group , and the contents of TC , TG and LDL-C were lower than those in tongxinluo group , atorvastatin group and aspirin group .The content of IL-8 was decreased in tongxinluo group , atorvastatin group , aspirin group and three-drug combination group .CONCLUSION: The strategy of three-drug combination enhances the effect of regulating the lipid metabolism and inhibiting the adventitia inflammation .It plays an important role to intervene in the process of atherosclerosis .

ABSTRACT

A claudicação intermitente está frequentemente associada à doença aterosclerótica, mas diagnósticos diferenciais devem ser pesquisados em pacientes sem fatores de risco tradicionais. A doença cística adventicial, de etiologia incerta, acomete em maior proporção a artéria poplítea e, eventualmente, apresenta-se como claudicação intermitente. Apresentamos um caso da doença e seu manejo cirúrgico, e discutimos a etiopatogenia, os aspectos diagnósticos e terapêuticos da enfermidade.


Intermittent claudication is frequently associated with atherosclerotic disease, but differential diagnosis must be sought in patients with no traditional risk factors. Cystic adventitial disease, of unknown etiology, most frequently affects the popliteal artery, and occasionally presents as intermittent claudication. We report a case of this disease and the surgical treatment, and discuss some aspects related to etiopathogenesis, diagnosis and treatment of this condition.

ABSTRACT

Adventitial cystic disease (ACD) of venous system is an extremely rare condition. Very few reports of ACD in venous system have been described. In this report we discuss two cases of common femoral vein ACD that presented with a swollen leg by the obstruction of the vein. Ultrasound imaging showed the typical hypoechoic fluid filled cyst with a posterior acoustic window. Computed tomography scan and ascending venogram showed a stenosis to flow in the common femoral vein caused by an extrinsic mass. Trans-adventitial evacuation of cyst with removal of vein wall was performed for both cases. During operation we found the gelatinous material in the cysts arising in the wall of the common femoral vein and compressing the lumen. The patients were released after short hospitalization and have remained symptom free with no recurrence.

ABSTRACT

Por ser capaz de mostrar aspectos morfológicos e patológicos de ateroscleroses, o Ultrassom Intravascular (IVUS) se tornou uma das modalidades de imagens médicas mais confiáveis e empregadas em intervenções cardíacas. As características de sua imagem aumentam as chances de um bom diagnóstico, resultando em terapias mais precisas. O estudo de segmentação da fronteira média-adventícia, dentre muitas aplicações, é importante para o aprendizado das propriedades mecânicas e determinação de algumas medidas específicas (raio, diâmetro, etc.) em vasos e placas. Neste trabalho, uma associação de técnicas de processamento de imagens está sendo proposta para atingir alta acurácia na segmentação da borda média-adventícia. Para tanto, foi feita uma combinação das seguintes técnicas: Redução do Speckle por Difusão Anisotrópica (SRAD), Wavelet, Otsu e Morfologia Matemática. Primeiramente, é usado SRAD para atenuar os ruídos speckle. Posteriormente, é executada Transformada Wavelet para extração das características dos vasos e placas. Uma versão binarizada dessas características é criada na qual o limiar ótimo é definido por Otsu. Finalmente, é usada Morfologia Matemática para obtenção do formato da adventícia. O método proposto é avaliado ao segmentar 100 imagens de alta complexidade, obtendo uma média de Verdadeiro Positivo (TP(%)) = 92,83 ± 4,91, Falso Positivo (FP(%)) = 3,43 ± 3,47, Falso Negativo (FN(%)) = 7,17 ± 4,91, Máximo Falso Positivo (MaxFP(mm)) = 0,27 ± 0,22, Máximo Falso Negativo (MaxFN(mm)) = 0,31 ± 0,2. A eficácia do nosso método é demonstrada, comparando este resultado com outro trabalho recente na literatura.


By being able to show morphological and pathological aspects of atherosclerosis, the Intravascular Ultrasound (IVUS) be¬came one of the most reliable and employed medical imaging modality in cardiac interventions. Its image characteristics in¬crease the chances of a good diagnostic, resulting in a precise therapy. The study of media-adventitia borders segmentation in IVUS, among many applications, is important for learning about the mechanical properties and determining some specific measurements (radius, diameter, etc.) in vases and plaques. An approach is proposed to achieve high accuracy in media-adventitia borders segmentation, by making a combination of different image processing operations: Speckle Reducing Anisotropic Diffusion (SRAD), Wavelet, Otsu and Mathematical Morphology. Firstly, SRAD is applied to attenuate the speckle noise. Next, the vessel and plaque features are extracted by performing Wavelet Transform. Optimal thresholding is car¬ried out by Otsu method to create a binarized version of these features. Then, Mathematical Morphology operations are used to obtain an adventitia shape. The proposed approach is evaluated by segmenting 100 challenging images, obtaining an average of True Positive (TP(%)) = 92.83 ± 4.91, False Positive (FP(%)) = 3.43 ± 3.47, False Negative (FN(%)) = 7.17 ± 4.91, Max False Positive (MaxFP(mm)) = 0.27 ± 0.22, Max False Negative (MaxFN(mm)) = 0.31 ± 0.2. The effectiveness of our approach is demonstrated by comparing this result with another recent work in the literature.

ABSTRACT

Objective To observe the effects of the chronic adventitia injury on the vasoconstriction of the rat carotid artery. Methods A non-occlusive silicone collar was positioned around the right carotid artery of rats. Blood flow and vascular reactivity to 5-HT were examined, and both carotids were harvested for morphometry at day 3,7 and 14 after operation. Results In the early stage of advenfitia injury induced by positioning a silicone collar around file rat carotid artery, there appeared the characteristic histological changes of chronic vasospasm in collared artery, such as the reduction of the luminal area for (12.15±2.29)% at day 3 after operation (P =0.003 ) and (45.17±3.84)% at day 7 (P 〈 0.001 )] ,corrugation of the internal elastic lamina,medial thickening up to [ (23.04±5.96)% at day 3 (P =0.009), (61.65±10.32)% for day 7 (P 〈 0.001 )] ,the reduction of the blood flow and the increase of vascular reactivity to 5-HT when compared to con- tralateral arteries. Two weeks after collar placement, the vascular wall remodeling was observed in injured artery, such as the medial thickening for [(31.52±4.56) %,P =0.012] and a diffuse intimal hyperplasia,the reduction of the lunfinal area [(37.17±4.57)% (P 〈 0.001)] and the carotid artery blood flow. The average neointima area was (0.19±0.05) rom2 in collared arteries. The vascular reactivity to 5-HT came back to the normal level. Conclusions Collar-induced advenfitia injury caused the enhancement of vascular contractility and the neointima formation. The change of vascular contractility appeared before the formation of neointima.
