ABSTRACT

Bone regeneration and repair after avascular necrosis of the femoral head are a complex process involving interactions of numerous kinds of cells. In the past decade when more and more cytokines have been identified and characterized, basic and clinical researches have found the efficacy of various cytokines in healing avascular necrosis of the femoral head. This article summarizes findings about some cytokines associated with treatment of avascular necrosis of the femoral head in order to deepen our understanding. The article also discusses the functions, current problems and future applications of cytokines in this field.

ABSTRACT

Objective To investigate the pathogenesis of avascular necrosis of femoral head (ANFH) and to compare the effectiveness of ultrashort wave diathermy and Chinese medicine in the treatment of avascular necrosis of femoral head (ANFH) through animal experiment. Methods A total of 40 New Zealand white rabbits were divided into four groups: a control group, a model group, an ultrashort wave group, a Chinese me-dicine group. All the groups were injected with horse blood serum and methyl-prednisolone, which can induce ANFH. The TXB_2, 6-keto-PGF_ 1? , TG, TCh and hemorheology index were observed in the study. Results When compared with control group, the difference of model group was significant (P0.05) between ultrashort wave group and Chinese group. Conclusion Both ultrashort wave diathermy and Chinese medicine were effective for the treatment of ANTH in the early stage.

ABSTRACT

Objective To observe the effectiveness of early intervention with ultrashort wave diathermy in the treatment of experimental avascular necrosis of femoral head (ANFH). Methods Thirty New Zealand white rabbits were divided into three groups:a normal group(group A),a model group(group B) and an ultrashort wave diathermy group(group C). The animals in groups B and C were subject to injection of horse blood serum and methyl prednisolone to induce ANFH, while those in group A were administered with injection of normal saline. The TXA 2, PGI 2, TG, TCh and hemorrheological indice were tested with all the rats. Results The animals in the model group were found with ANFH manifesting avasular necrosis of femoral head bone cells, decrease of bone marrow cells, high blood viscosity and imbalance of TXA 2 PGI 2. The ultrashort wave diathermy group demonstrated significant improvement with regard to the blood supply, the TG level and blood viscosity,as well as the PLT activity and the TXA 2 PGI 2 ratio. Conclusion The ultrashort wave diathermy could be an effective treatment method for preventing ANTH when used in the early stage of development of the condition.

ABSTRACT

Objective To evaluate a new type of bone graft material made by combining bFGF,BMP with AACB in the clinic repair of ANFH(avascular necrosis of the femoral head)in animal experiments.Methods The combined material of bFGF/BMP/AACB was made and then this composite was transplanted to the models of canine femoral head defects and necrosis established by liquid nitrogen freezing,The specimens were harvested respectively at the end of three,six and 12 weeks after operation.A series of examinations were carried out to evaluate the repair results,including gross observation,radiography,histology,image pattern analy sis,blood vessel immuhistochemical staining,EDAX(energy dispersion analysis X-ray),and mechanical measurement.Results All the animals had no toxic or inflammatory reactions.Little new bone formed in group A(control group)by the end of 12 weeks.Group B(AACB group)was better than group A,and group C(BMP/AACB group)better than group B in osteogenesis.All the results showed that group D(bFGF/BMP/AACB group)was much superior to groups A,B and C in angiogenesis and osteogenesis at all intervals,and even better than group E(bone autograft group)by the end of three weeks and six weeks.By the end of 12 weeks,bone repairing and remodeling effects in group D were as good as in group E.Conclusions Since the bFGF/BMP/AACB compsite has great potential for enhacing revascularization and osteogenesis in repairing the defect and necrosis model of canine femoral head,it can be used as a substitute bone graft material for clinical patients with ANFH.

ABSTRACT

Objective To address the medical problems, in treatment of avascular necrosis of the femoral head, on loosening and subsidence of the prosthesis of femoral head. A set of Mushroom Shaped Surface Prosthesis of Femoral Head was designed. Methods Based on biomechanical of the femoral head and the principle of transferring the force through femoral head while reducing stress-sheltered, nickel-titanium alloy and other new medical alloy materials were used to develop a prosthesis, which not only can be a weight-bearing device, but also promotes bone-growth. Results The prosthesis is novel, compact and little-volume, and the manipulation of surgery is convenient and secure. Conclusion The design of this prosthesis can provide new ways or ideas for designing the femoral head replacement prosthesis to solve problems on loosening and subsidence.
