ABSTRACT

Objective@#Throughing the investigation and analysis of seven cases of acute lung injury caused by a welding operation, the hazards and production pathways of harmful gases such as phosgene and nitrogen oxides are understood, which will serve as a reference for preventing similar events in the future.@*Methods@#Throughing the accident scene environment investigation and clinical case analysis, combined with the laboratory test results, carried on the comprehensive analysis.@*Results@#The accident caused a total of 7 people to suffer from acute lung injury to varying degrees. Three of them were sent to the ICU ward for treatment due to acute respiratory failure. After active treatment, 7 people were discharged from the hospital with obvious relief.@*Conclusion@#Welding is also a kind of profession that causes the high incidence of acute lung injury accidents. It is necessary to strengthen the knowledge training and protection awareness education of welding workers to avoid the occurrence of acute harmful gas poisoning accidents.

ABSTRACT

BACKGROUND: In order to obtain a deeper understanding of an incident, it needs to be investigated to “peel back the layers” and examine both immediate and underlying failures that contributed to the event itself. One of the key elements of an effective accident investigation is recording the scene for future reference. In recent years, however, there have been major advances in survey technology, which have provided the ability to capture scenes in three dimension to an unprecedented level of detail, using laser scanners. METHODS: A case study involving a fatal incident was surveyed using three-dimensional laser scanning, and subsequently recreated through virtual and physical models. The created models were then utilized in both accident investigation and legal process, to explore the technologies used in this setting. RESULTS: Benefits include explanation of the event and environment, incident reconstruction, preservation of evidence, reducing the need for site visits, and testing of theories. Drawbacks include limited technology within courtrooms, confusion caused by models, cost, and personal interpretation and acceptance in the data. CONCLUSION: Laser scanning surveys can be of considerable use in jury trials, for example, in case the location supports the use of a high-definition survey, or an object has to be altered after the accident and it has a specific influence on the case and needs to be recorded. However, consideration has to be made in its application and to ensure a fair trial, with emphasis being placed on the facts of the case and personal interpretation controlled.

ABSTRACT

Introdução: O acidente ocorrido em Santa Maria-RS encontra-se dentre os mais propalados pela mídia inclusive com repercussão internacional. A investigação criminal, levada a cabo pela delegacia de polícia e também a feita pelos técnicos do conselho regional de engenharia, resultou em um conhecimento dos fatos imediatos, ou seja, um olhar da relação causa e efeito, nitidamente estabelecida sem buscar as razões de origem das causas. Em outros termos, aqui buscamos contextualiza o evento acidentário dentro dos fatores culturais, políticos, regulatórios e econômicos. Ao não colocar luzes sob tais aspectos correlacionados, o retorno da experiência, o aprendizado e a própria gestão de riscos, ficaram circunscritos à esfera proximal do acidente. Questões sobre o uso e comercialização de materiais pirotécnicos, o emprego de materiais de revestimentos em boates e clubes noturnos, a promoção de eventos com grande afluxo de pessoas para locais fechados, não são aprofundados nos referidos relatórios. Também não se discutiu, de forma ampla, questões sobre a atuação de órgãos públicos na concessão de funcionamento de boates, sobre o papel dos especialistas em prevenção de risco, sobre o papel dos agentes públicos em grandes desastres e acidentes ampliados, especialmente os relacionados ao acidente químico que neste caso matou, em minutos, duzentos e quarenta e dois jovens, todos levados a óbitos pela ação do gás cianídrico.


Introduction: The accident in Santa Maria - RS is one of the most publicized by the media having international repercussions. The criminal investigation conducted by the police and also by the technicians of the Regional Engineering Board, resulted in an understanding of immediate facts, i.e., the causes and effects which clearly established without seeking for the roots of the problem. In other words, here we seek to contextualize the accident within the cultural, political, regulatory and economic aspects. If we did not analyze the correlated aspects, the feedback, learning and risk management itself would be restricted to the accident proximal sphere. Questions about the use and sale of fireworks, the use of lining materials in clubs and nightclubs, the organization of events with large amounts of people indoors, are not detailed in the reports. It was also not broadly discussed questions about the role of governmental agencies in providing operating licenses for nightclubs, the role of risk prevention experts and the role of public officials in major disasters and major accidents, especially those related to chemical substances, that, in this case, killed two hundred forty two young people in a few minutes, all of whom died due to the action of the cyanide gas. Objectives: In face of these and other shortcomings, this paper proposes a method of coping with these issues

ABSTRACT

Partindo de apresentação da proposta de análise de acidentes baseada no modelo sistêmico e não linear de acidentes, dito de ressonância funcional, desenvolvido por Erik Hollnagel, este texto discute críticas às concepções deterministas de acidente e aos modelos lineares de representação desses eventos. Aspectos do modelo de ressonância funcional são usados como eixo definidor de proposta de agenda de discussões a ser desenvolvida pelos interessados no tema da análise de acidentes, destacando-se: a) a defesa do abandono da idéia de causasde acidentes e sua substituição pela de explicação desses eventos; b) apresentação dos conceitos de variabilidade de desempenhos e adaptações locais e a proposta de sua utilização como bases para a identificação de perfis ou aspectos típicos de acidentes. Por fim, discute-se a necessidade de incorporação dos conceitos apresentados no debate sobre novos caminhos para as práticas de análises e de prevenção de acidentes em nosso país.


Starting from presentation of accident analysis proposals based on systemic and non-linear model of accidents called functional resonance, developed by Erik Hollnagel, this paper discusses criticism of deterministic accident conceptions and linear models representing these events. Aspects of Functional Resonance Accident Model are used as an axis to define an agenda for discussions to be developed by those interested in the accident analysis subject, specifically: a) the need to cease using the idea of accident causes in favor of explaining these events; b) presenting the concepts of performance variability and local adaptations and their utilization as bases to identify patterns of accidents. Finally, it discussesthe need to include these concepts in the debate on new practices for accident analysis and prevention in Brazil.
