ABSTRACT

@#Objective     To investigate short-term outcomes of robot-assisted Heller-Dor myotomy (RAHM-Dor) for achalasia of cardia and our learning curve experience. Methods     The clinical data and recent follow-up results of 42 patients who received RAHM-Dor from November 2015 to January 2020 in the Department of Thoracic Surgery of the First Affiliated Hospital of Nanchang University were retrospectively analyzed, including 20 males and 22 females with a mean age of 40.8±18.4 years. Results    Dysphagia was the most common symptom, followed by heartburn and regurgitation. The mean operation time was 122.8±23.9 min. The mean blood loss was 47.5±32.7 mL. Two patients suffered mucosal injury, and successfully repaired by suturing during surgery. There was no esophageal fistula, conversion to an open operation or perioperative death in this series. The median length of hospital stay was 8 (6, 9) d. In all patients, the Stooler and Eckardt scores of postoperative 1, 6 and 12 months decreased compared to those of pre-operation (P<0.001). Conclusion     RAHM-Dor is a safe and feasible avenue for the treatment of achalasia of cardia, and can achieve a satisfying short-term results. The learning curve shows a transition to the standard stage from the learning stage after 16-18 operations.

ABSTRACT

Objective To investigate the clinical effects of peroral endoscopic myotomy (POEM) in the treatment of achalasia of cardia. Methods Clinical data of 27 cases who were diagnosed with achalasia of cardia recived POEM from May 2014 to March 2016 were collected retrospectively. The surgical results, before and after POEM, parameters measured by esophageal manometry and complications after POEM and during follow-up were analyzed. Results POEM were successful 100.0% in the 27 patients. There was 1 case of subcutaneous emphysema, 1 case of aeropleura symptoms were significantly improved in all patients who had successful POEM; parameters measured by esophageal manometry were also improved obviously (P < 0.05). Conclusion POEM has appreciable short-term effects in the treatment for achalasia of cardia, and it can be relieve dysphagia and other adverse symptoms in postoperation, but the long-term efficacy and complications need further follow-up observations.

ABSTRACT

Objective:To analyze the clinical efficacy of POEM treating patient with achalasia of cardia and observe the risk factor of effecting curative efficacy.Methods: 66 patients with achalasia of cardia who were treated by using POEM were enrolled in this study, and series of indicators of esophagus dynamics including LESRP, LESP, LESRR and LESL were detected in pre and post operation. Clinical symptom score system(Eckardt score) of achalasia of cardia was adopted to evaluate the effect of surgical treatment for patients at the pre and post operation 7d, and the diameter of widest part of esophagus at pre and post operation 7d were detected. The incidence rate of pneumoderm, substernal ache, pulmonary infection, pleural effusion, tunnel bleeding, postoperative reflux and esophageal perforation and other complications were analyzed and the relative risk factors of effecting curative effect were observed.Results: The LESRP and LESP of patients with achalasia of cardia of post-operative 7d were significantly lower than that of pre-operative 7d(t=12.74,t=17.56,P<0.05), while the LESRR and LESL of post operative 7d were significantly higher than that of pre-operative 7d(t=9.10,t=14.86,P<0.05) , respectively. And the Eckardt score and widest diameter of post-operative 7d were significantly lower than that of pre-operation(t=19.49, t=7.83,P<0.05), respectively. Besides, the incidence rate of complication of patients in post-operation was 48.48%. As the results of Logistic regression, age, Chicago type and Ling type were the risk factors that affect curative effect of POEM for achalasia of chardia. And as the analysis, the prognosis was better when the patient's age was less than 60 years, Chicago II type and Ling type.Conclusion: POEM can significantly improve the indicators of esophagus dynamics, and the complication will be few and the curative effect will be exact in middle and short term of post-operation by using this method. Therefore, it is a safe and effective surgical mode.

ABSTRACT

Objective To explore the clinical features,efficacy and prognosis of different treatments for children with achalasia of cardia(AC).Methods In this retrospective study,the clinical features,laboratory examination and treatment of 36 children with AC who had been admitted to Department of Gastroenterology,Beijing Children's Hospital,Capital Medical University from August 2006 to September 2015 were reviewed,and the efficacy and prognosis of different treatments were compared.The symptoms of the children were graded using the AC clinical symptom score(Eckardt score),and the Eckardt score ≤ 3 scores was defined as the effective treatment.SPSS 19.0 statistical software was used to analyze the data,and P<0.05 for the difference was statistically significant.Results Thirty-six children with AC included 24 boys and 12 girls.Ages ranged from 1.4 to 15.5 years old,with a mean age of(10.0±3.4)years old.Course of disease ranged from 1 month to 9 years,with a mean course of 0.5(0.2,3.0)years.In the 36 children,33 cases(91.7%)had vomiting,23 cases(63.9%)had dysphagia,16 cases(44.4%)had weight loss,and 9 cases(25.0%)had chest pain.The effective rates of treatment in surgical treatment group and drug treatment group were 100.0%(13/13 cases)and 71.4%(5/7 cases),respectively in 3 months,and there was no significant difference between the 2 groups(P=0.111).The effective rates of treatment were 100.0%(13/13 cases)and 50.0%(3/6 cases),respectively in 6 months,and the difference was statistically significant between the 2 groups(P=0.021).Within 12 months,there was no recurrence in surgical treatment group and the effective rate was 100.0%.Children in drug treatment group had 1 case who stopped taking medicine,while the other children received surgical treatment in other hospitals due to poor drug treatment.Conclusions Drug and surgical treatment of AC both have good short-term effect,however,the medium and long-term efficacy of surgical treatment is higher than that of drug treatment in children.Symptomatic relief is more stable,and symptom is not easy to relapse for the children with surgical treatment.

ABSTRACT

Achalasia of cardia caused by neuromuscular dysfunction at esophagus-stomach junction is a functional disease of esophageal dynamic dysfunction. It is characterized by absence of peristalsis of esophageal body and failure of the lower esophageal sphincter to relax. The methods of therapy include botulinum toxin injection, stent placement, laparoscopic Heller myotomy and balloon dilatation. However,these methods have some shortages,such as easy to recur, causing larger trauma,etc. . With the development of technology of endoscopy,a new method peroral endoscopic myotomy ( POEM)is widely used in clinical practice,and it can used in some special patients such asⅢ type achalasia,pediatric and elderly patients,as well as sigmoid-type achalasia. This article reviewed the current status and progress of POEM in treatment of achalasia of cardia.

ABSTRACT

Objective To summarize the experience of nursing achalasia of cardia patients after peroral endoscopicmyotomy ( POEM ) . Method Eighty-five achalasia of cardia patients were cared with careful preoperative preparation , active operational cooperation and postoperative psychological care , pain care , dietary instruction , complication treatment and discharge education . Results All patients went through POEM successfully even with severe complications. In the follow-up for 1~24 months, their dysphagia were alleviated and their weights increased by 1~30 kg. Conclusion Careful preoperative assessment and digestive tract preparation before operation and postoperative close observation, dietary education and nursing of complications are key for the successful POEM.

ABSTRACT

Objective To evaluate the efficacy and safety of endoscopic balloon dilatation for patients with achalasia. Methods Patients with achalasia, were treated by single or double esophageal balloon dilatation under endoscopy,f ollowed up survey by gastroduodenal endoscopy and esophageal barium radiography as well as their symptoms were investigated periodically. Results The dysphagia symptoms remitted in all of the 43 patients,and mucosal lacerations,abrasions a nd oozing of blood, were found in this process but without complication of perf oration,haematemesis and melena.The following up ranged from 3 months to over 4. 8 yrs,and average was 2.6 yrs.The rate of remission of dysphagia symptoms 90.7 % (39/43) at six months,85.3% (29/34) at 1 yr,82.6% (19/23) at 2 yrs and 82.4 % (14/17) at 3 yrs,and the rate of recurrence of symptoms was 9.3% (4/43)、 1 4.7% (5/34)、 17.4% (4/23)、 17.6% (3/17),respectively.In addition,there were 37.2% (16 cases)of patients with acid- reflux symptoms,30.2% (13 cases)heart burn and 14.0% (6 cases)reflux esophagitis during the followed- up. Conclusion Endoscopic esophageal balloon dilatation for achalasia is an effective and saf e method not only in the short- term but also in the long- term.
