ABSTRACT

Resumen El granuloma actínico es una dermatosis granulomatosa poco frecuente, que se observa en adultos de mediana edad con antecedentes de fotoexposición. Su patogénesis no es clara; siendo la teoría más aceptada la radiación solar como factor disparador. Se caracteriza por placas anulares con atrofia central y bordes eritematosos elevados que son similares a los observados en el granuloma anular. Se presenta el caso clínico de una paciente de 71 años, con placas anulares en antebrazos compatibles con granuloma actínico. Se comunica el caso por lo infrecuente de esta entidad y se realiza una revisión bibliográfica.


Abstract Actinic granuloma is a rare granulomatous dermatosis, seen in middle-aged adults with a history of photoexposure. Its pathogenesis is not clear; the most accepted theory being solar radiation as a triggering factor. It is characterized by annular plaques with central atrophy and raised erythematous borders that are similar to those seen in granuloma annulare. The clinical case of a 71-year-old patient with annular plaques on the forearms compatible with actinic granuloma is presented. The case is reported due to the infrequent nature of this entity and a bibliographical review is carried out.

Comunicamos un caso del denominado granuloma anular elastolítico, con células gigantes, en una mujer de 73 años, quien lamentablemente no concurrió al control evolutivo. La ubicación nosológica y la relación con el granuloma actínico y el granuloma anular son discutidas.


A 73-year-old white female, with a several months of history of ring shaped skin lesions, localized in her proximal limbs are communicated. The annular lesions have erythematous raised borders, and an atrophic hypo pigmented central patches. The hematoxylin and eosin staining showed a granulomatous infiltrate in the upper and middle dermis, with elastophagocytosis and elastolisis. The histological diagnosis was annular elastolytic giant cell granuloma. The relationship with the actinic y annular granulomas is discussed.

ABSTRACT

Annular elastolytic giant cell granuloma is a condition characterized histologically by damaged elastic fibers associated with preponderance of giant cells along with absence of necrobiosis, lipid, mucin, and pallisading granuloma. It usually occurs on sun‑damaged skin and hence the previous name actinic granuloma. A similar process occurs on the conjunctiva. Over the past three decades only four cases of conjunctival actinic granuloma have been documented. All the previous patients were females with lesions in nasal or temporal bulbar conjunctiva varying 2-3 mm in size. We report a male patient aged 70 years presenting with a 14 mm × 7 mm fleshy mass on right lower bulbar conjunctiva. Clinical differential diagnoses were lymphoma, squamous cell carcinoma in situ and amyloidosis. Surgical excision followed by histopathology confirmed it to be a case of actinic granuloma. This is the first case of isolated conjunctival actinic granuloma of such a large size reported from India.

El granuloma actínico es una rara afección de la piel que se desarrolla en general en áreas expuestas al sol. Su patogénesis no es clara.La teoría más aceptada es la implicancia de la radiación solar como factor disparador. Típicamente la enfermedad se presenta en individuos mayores de 30 años con historia de exposición solar. Se manifiesta con lesiones anulares de bordes elevados y atrofia central,asintomáticas, con poca tendencia a la involución. Se han probado múltiples terapéuticas con éxito variado. Esta entidad poco frecuente fue descripta por primera vez en 1975 por O`Brien. Presentamos dos pacientes con granuloma actínico. El primero correspondió a una mujer de 83 años de edad con una lesión en pómulo izquierdo. El segundo a un varón de 70 años con una gran lesión que se extendía desde el epigastrio hasta las clavículas.


Actinic granuloma is a rare skin disorder usually developing uponsun-exposed areas. While its pathogenesis is unclear, the acceptedhypothesis maintains that solar radiation is the triggering factor.Typically the disease develops in individuals over 30 years of age witha history of sun exposure, showing annular lesions with raised bordersand central atrophy, asymptomatic, with little tendency to regression.Multiple therapies have been tested with variable response. Thisuncommon entity was described for the first time in 1975 by O’Brien.We present two patients with actinic granuloma. The first onecorresponded to an 83 years-old woman with a lesion on the leftcheek. The second one was a 70 years-old male with a large lesion thatextended from the epigastrium to the collar bones.

ABSTRACT

Actinic granuloma is a rare chronic granulomatous dermatosis that is characterized by the elastophagocytosis by multinucleated giant cells. This malady presents in middle-aged individuals and it manifests as asymptomatic papules or annular plaques in the sun-exposed area. There are four main histopathologic patterns of actinic granuloma, including the giant cell, necrobiotic, histiocytic and sarcoidal variants. However, there are no previously reported cases of actinic granuloma associated with the histopathologic findings of suppurative granuloma. We herein report on an unusual case of actinic granuloma that showed the histopathologic findings of suppurative granuloma in a 58-year-old female.

Se comunica un caso de granuloma actínico en una paciente de sexo femenino, caucásica, la cual presentaba concomitantemente arteritis de la temporal y polimialgia reumática. Actualmente se considera que debido a la fisiopatogenia de las tres entidades mencionadas, podría tratarse la primera, de un marcador de las dos restantes.


We present a female, white, with an actinic granuloma, concomitantly with temporal arteritis and polymyalgia rheumatica. Today it is considered that, due to the phisiopathogenia of the three entities mentioned before, the first one could be used as a reference for the other two.

ABSTRACT

Actinic granuloma develops in the chronically sun-damaged skin of the neck, face, upper chest or arms. Lesions present as skin colored to erythematous papules and plaques that coalesce to form centrifugally enlarging annular patterns. Histologically, elastolytic granuloma is formed by a dense infiltrate of giant cells and histiocytes with active phagocytosis of elastoclastic fibers on the background of solar elastosis in the upper dermis. We report a clinically rare presentation of actinic granuloma, limited to both hands of a 75-year-old female with colon cancer.

ABSTRACT

A 63-year-old Korean man presented with generalized, asymptomatic, discrete papules and coalescent annular plaques for one year. Interestingly, the patient complained of conjunctival congestion, concurrently with aggravation of the skin lesion. Histopathologic findings of the skin lesion and conjunctiva revealed giant cells engulfing fragmented elastic fibers, and a diagnosis of actinic granuloma with ocular involvement was made. Ten months later, the patient revisited with a two-week history of bilateral temporal headache, neck pain, intermittent jaw claudication, and blurred vision. Biopsy of the temporal artery confirmed the clinical diagnosis of temporal arteritis. Both actinic granuloma and temporal arteritis show similar pathologic process, and this case supports the hypothesis that actinically degenerated elastic tissue is likely to be the antigenic basis of actinic granuloma of the skin and temporal arteritis.

ABSTRACT

Wolf's isotopic response describes the occurrence of a new, unrelated disease that appears at the location of a previously healed disease, most commonly herpes zoster. Several cutaneous reactions including granuloma annulare have been known to occur within resolved herpes zoster lesions. We report a case of actinic granuloma which developed on a site previously affected by herpes zoster, and is thought to be an isotopic response.

ABSTRACT

We report a case of actinic granuloma in a 70-year-old woman with diabetes mellitus. Several annular lesions developed in both shins and persisted for five years, and similar lesions developed later mainly in sun-exposed areas. Histopathological study from the annular plaques of the shoulder showed actinic granuloma composed of numerous giant cells with disappearance of elastic fibers. Histopatholoc finding from the shin showed actinic granuloma of several foci of necrobiosis. From this case, we observed two different patterns of actinic granuloma in the same patient.

ABSTRACT

We report a case of actinic granuloma in a 39 year-old woman who was presented with multiple anetoderma on non-sun exposed skin such as the neck, upper arms, elbows and calfs. The anetodermic presentation of actinic granuloma has never been reported and the common pathogenesis involving reduction of elastic fibers link the two disorders. Anetoderma secondary due to actinic granuloma should to be included in the list of secondary anetoderma and may suggest one of the possible pathogenesis of anetoderma.

BACKGROUND: Actinic granuloma is an annular inflammatory reaction that develops in skin that has had long-term sun-exposure. OBJECTIVES: The purposes of this study were aimed to characterize the clinical and histopathologic features of actinic granuloma. METHODS: Examinations were performed on eight patients with actinic granuloma regarding the age, sex, duration, clinical morphology, distribution, associated diseases, and treatment and course. Histo-pathologic studies of ten specimens obtained from eight patients were performed with special stains, such as Verhoeff-van Gieson, Gomori-Methenamin silver and alcian blue.

ABSTRACT

Actinic granuloma (O'Brien) is an annular inflammatory reaction that develops in skin after long-term sun-exposure. Anetoderma is characterized by discrete skin lesions that appear to be loose and wrinkled. The primary type arises in clinically normal skin, and the secondary type appears in lesions of other disorders. We describe a case of anetoderma that developed on the neck, both arms and calves, and showed histopathological findings of actinic granuloma.

ABSTRACT

Actinic granuloma is a specific disease entity characterized by clinical lesions indistinguishable from granuloma annulare, but occuring on solar damageci skin. Ilistologically, elastic tissues are destroyed by the granulomatous process in actinic granuloma, but not, in granuloma anriulare. We report two cases of actinic granuloma with each diagnosed as a giant cell and sarcoirlal variant of the disease. Interestingly, one case developed on the non sun-exposed skin and anothr case related to the history of an insect bite.
