ABSTRACT

[Objective] To clarify the utilization of characteristics and reactions of acupuncture points, and the frequency of using acupuncture points in acupuncture and moxibustion treatment in Japan.[Methods] We conducted a questionnaire survey involving members of the Japan Society for Oriental Medicine and those of the Japan Society of Acupuncture Course in Universities. The contents of the questions included age, sex, profession, length of clinical experience of acupuncture/moxibustion, treatment method, utilization of characteristics and reactions of acupuncture points, and frequency of using acupuncture points in both acupuncture and moxibustion treatment.[Results] The number of valid questionnaires was 59 (59 for acupuncture treatment and 50 for moxibustion treatment). The respondents were as follows : average age of 52 ± 12 years, 45 males and 14 females, 13 medical doctors and 46 acupuncturists, clinical experience of 21.1 ± 11.5 years. Most practitioners utilized ill ness-specific features of acupuncture points, de qi, and reactions they feel with their needling and pressing hands. Ten acupuncture points were used in 50% of patients in acupuncture treatment, and ST 36 and SP 6 were the most frequently used. On the other hand, no single acupuncture point was used in more than 50% of patients in moxibustion treatment. Thirteen acupuncture points were used in 30% of patients in moxibustion treatment, and BL 23, SP 6, and ST 36 were the most frequently used.[Conclusion] Our survey clarified the usage of acupuncture points in acupuncture and moxibustion treatment in Japan.

ABSTRACT

The anatomical structure of acupuncture points is the carrier for needling stimulation induced neurobiological effects. Although researchers have never stopped investigating its structure and experienced different stages from macroscopic to microcosmic morphological material basis, our understanding still largely lags behind the requirements of the times. In the present paper, we review development of researches on acupuncture points in recent decades from 1) macroscopic anatomical structure including skin, bones, muscles, nerves, blood vessels, etc.; 2) macro-microcosmic structure as the thickness of skin and muscles, distribution of blood vessels and nerve fibers, and innervation; 3) microstructure including cell categories (mast cells, fibroblasts, macrophages, etc.), peptidergic nerve fibers, various receptors, chemical substances, and their relationship with the brain and immune-endocrine system (cutaneous-neuro-immune-endocrine-mental biological communication), etc. These research outcomes will not only help us demonstrate the acupoint's structure better at the level of the body surface and regional anatomy, but also, at the microstructural level, reveal different kinds of cells and their chemical compositions in its structure as well as their relationship with the regulatory system of the whole body. From the perspective of the cutaneous and systemic neuro-immuno-endocrine networks, it undoubtedly prompts us to profoundly explore the underlying biological mechanisms of acupuncture. To observe the regional mutual connection and interaction of different kinds of cells and their chemical mediators and molecules in the acupuncture point will improve our understanding about the nature of acupuncture points from the static to the dynamic point of view. This might be a new approach for us to further reveal the functional roles of acupuncture points.

ABSTRACT

OBJECTIVETo explore the correlation between single acupoints used and the recurrence rate of cystitis among cystitis-prone women receiving acupuncture as a prophylactic treatment.

METHODSIn all, 58 cystitis-prone women were included in the analysis. Customised acupuncture treatments were given twice a week, over 4 weeks. The main effect parameter was the number of cystitis episodes during the 6-month observation time. Residual urine was measured at baseline, 2, 4 and 6 months using portable ultrasound equipment. Sympathetic and vagotone nerve activities were measured by using skin conductance and respiratory sinus arrhythmia, respectively.

RESULTSThe main acupoints used for patients with Kidney (Shen) qi/yang deficiency were Shenshu (BL23), Taixi (KI3), Zhongji (CV3), Sanyinjiao (SP6) and Pangguangshu (BL28), compared with Taichong (LR3), CV3, BL28, Yinlingquan (SP9) and SP6 for Liver (Gan) qi stagnation, and SP6, CV3, BL28, Zusanli (ST36) and SP9 for Spleen (Pi) qi/yang deficiency patients. The combination BL23 and KI3 were used in 16 women, 13 of which were Kidney pattern related patients. When used, the number of symptomatic episodes were reduced to a third compared with what occurred in the 42 women where this combination was not used (3/16 vs. 28/42, P<0.05). BL23 application correlated to a significant reduction in residual urine measured a few days after treatment. Patients with the pattern of Spleen qi/yang deficiency had an initial increase in residual urine after treatments.

CONCLUSIONTreating Kidney pattern related patients with the combination of BL23 and KI3 resulted in far better outcome than other points/combination of points for other Chinese medicine diagnoses. The acupoint SP6 may be less indicated than previously assumed when treating cystitis-prone women prophylactically.

ABSTRACT

ObjectiveTo establish a surface location method and acupuncture manipulation standard by dissecting the local structure of rat point Renying(ST9)and make a validation through the hypotensive effect of acupuncture.MethodA Wistar rat was sacrificed and fixed by cryogenic freezing. According the anatomical characteristics of human point Renying, cervical point Renying region was dissected layer by layer, and the common carotid artery, the internal carotid artery and the external carotid artery were bluntly separated. Location and measurement were made using vernier calipers and digital photographs were taken. Body surface location and acupuncture point depth were statistically analyzed to establish acupuncture manipulation standards. Point Renying and a non-acupoint were separately acupunctured to treat rat spontaneous hypertension. The changing tendency of blood pressure was statistically analyzed after four weeks.ResultAccording to the 95% reference value range, the surface location of rat point Renying was determined to be (8±0.3)mm below a line connecting bilateral mandibular angles and (5.5±0.4)mm lateral to the anterior midline, one on each side. Acupuncture manipulation standards were perpendicular insertion (5.5±0.4)mm and cautious lifting and thrusting to avoid injuring the artery. Acupuncture at point Renying had a marked hypotensive effect as compared with a non-acupoint (P<0.05).ConclusionThe surface location of rat point Renying is reliable and can be applied to animal experimental study.

ABSTRACT

We created a new medical interview form which is called chronological analysis for East Asian Traditional Medicine (EATM).
I would like to focus on our medical interview in EATM. Our patient comes to see a doctor with his or her life and medical histories.
When we interview and write them on CHRONOLOGICAL ANALYSIS CHART, we can give them into several groups. Each group is characterized as a chronic health problem (CHP) and named by the traditional major pattern diagnosis and the symptom-to-medicine pattern diagnosis. The former base on TCM (Traditional Chinese Medicine) and the latter Japanese Kampo Medicine.
On this chart the patient's life events are written in the left side and his or her active symptoms are written in right side. We write them to make in the same longitudinal time line. Comparison with light life events and right illnesses makes us understand the structure consisting of several groups. We already saw more than three hundred thousand patients in 30 years. A 52 year-old women came to see us. We interviewed by our chronological analysis. Her history consist of several groups:The 1 st group is Detoxication pattern, the 2 nd is heat in intestine blood, the 3rd is congestion in blood, and the 4 th is Liver "CHI"congestive dysfunction and finally we diagnosed as Toukakujyoukitou decoction for climacteric dysfunction.
The most significance is we could guess that she had a stressful time in the blue colored part.
After observing each patient's chronic health problems, we relate them chronologically to their life events and develop a graphic structure for each patient comprehensively. To see a patient from this kind of this view, we call emphasize them in both modern and traditional medicine. We show a new interview way using chronological chart to understand patients more deeply.
Amid the growing medical needs of an aging society this chronological analysis is significant to understand our patients comprehensively.
Through these medical approach we understand patients and give them traditional healing of mind and body.

ABSTRACT

[Introduction] In 2006 the WHO and WPRO agreed on standard acupuncture point locations. To promotepagate these standards, in 2009 the Second Japan Acupuncture Standardization Committee published a Japanese edition of 'WHO STANDARD ACUPUNCTURE POINT LOCATIONS FOR THE WESTERN PACIFIC REGION.' Based on this Japanese edition, a new textbook was published by the Japan Association of Massage & Acupuncture Teachers and the Japan College Association of Oriental Medicine. Since one year has passed since the start of education based on standard acupuncture point locations at Japanese universities, colleges, vocational schools and training centers for anma (Japanese traditional massage), massage, and shiatsu (acupressure); acupuncture; and moxibustion therapies, we administered a questionnaire survey as an evaluation of international standardization and the problems of introducing standard acupuncture point locations .
[Subjects and methods] Subjects were mainly teachers and included a small number of researchers, clinicians, and other groups concerned with acupuncture and moxibustion. We used a questionnaire that we originally created at the Second Meeting of the Japan Standardization of Acupuncture Point Locations Committee.
[Results] Among the 180 institutions surveyed, we obtained answers from 149 people from 93 institutions in total. Agreement on the question of standard acupuncture points, "functional existence" (44.3%) was most common, and "anatomical existence" came next at 26.6%. For the question on acupuncture treatment, 82.4% replied with "use ofboth acupuncture points and reaction points." For the answers to agreeing with international standardization, "no opinion" was 41.7% and 51.7% for "appreciate." However, both of those groups appreciated globalization of acupuncture and moxibustion by a common language. There were many opinions on proportional bone measurement. Specifically, opinions indicated a change "from the cubital crease to the wrist crease" (from 10B-cun to 12B-cun) and a need for proportional bone measurement of the upper arm. Whereas, for individual acupuncture points, opinions expressed the difficulty of locating application points and not understanding reasons for change and notations including body surface segments.
[Discussion] We were able to classify the opinions collected into the following groups: (1) problems that can be corrected immediately, including typographic errors, (2) problems that need to be reviewed at the next international gathering, and (3)problems that need to be understood by making full use of related documents.
[Conclusion] We were able to determine primarily for a wide range of teachers, problems understanding individual acupuncture point locations, including consideration of acupuncture points, evaluation of standardization of acupuncture point locations, and other guidelines
