ABSTRACT

Introduction: Sudden intraoperative desaturation may be dueto numerous reasons such as- ETT kinking, dislodgement,obstruction by tight gauze, obstruction off ETT lumen byforeign body, blood, bronchospasm, tension pneumothoraxand hemothorax, pulmonary embolism and finally equipmentmalfunction.Case Report: We hereby present a case of 28 year old femalethat was diagnosed with hydatid cystic.Conclusion: Regular cuff pressure measurement and vigilantpatient monitoring goes a long way to prevent catastrophe.

ABSTRACT

Objective To investigate the feasibility and safety of endoscopic treatment of ureteral calculi acute obstruction with urinary extravasation. Methods 56 patients with ureteral calculi acute obstruction and urinary extravasation were randomly divided into two groups:the treatment group and the control group,28 cases in each group. Patients in the treatment group were given URSL or percutaneous nephrostomy drainage, and the secondary fistula was given URSL stone clearance treatment. Patients in control group were given traditional ureterolithotomy treatment. The stone clearance rate, the average recovery time after surgery, postoperative wound infection rate and the abnormal rate of postoperative albumin were observed in two groups. Results In the treatment group,28 patients had no residual stones with mean postoperative recovery time of (5.2 1.3) days,postoperative fever was found in 3 cases,obvious abnormal postoperative albumin in 3 cases. In the control group,residual stones were found in 3 cases,the average recovery time after surgery was (7.9 2.6) days,postoperative fever was found in 10 cases, and obvious abnormal postoperative albumin in 11 cases. There were statistically significant differences in stone clearance rate, the average recovery time after surgery, postoperative wound infection rate and the abnormal rate of postoperative albumin between two groups (P<0.05) . Conclusion Endoscopic treatment of ureteral calculi acute obstruction and urinary extravasation has advantages including better efficacy, less trauma, less complications and quicker recovery.

ABSTRACT

Objective To investigate the method of surgical treatment of acute intestinal obstruction caused by colorectal cancer,and affirm the feasibility and validity of the new way ofearly precolonic postoperative enteral nutritionafter operation of acute intestinal obstruction caused by colorectal cancer.Methods Retrospective analized the clinical data of 78 cases of acute intestinal obstruction caused by colorectal caner from September 2007 to September 2012 and the incidence of complications (incision infection,intra-abdominal abscess,anastomotic leakage,and pneumonia) was observed.Results Sixty-two patients received radical resection and primary anastomosis.All of them were cured and no death occurred.Two patients had complication of postoperative infection and none suffered from anastomotic or drainage leakage.Conclusion Radical resection and primary anastomosis using Intra-operative colonic lavage are safe and effective methods for patients with colorectal cancer associated with acute intestinal obstruction,and early precolonic postoperative enteral nutritioncould better eliminate perioperative malnourished patients more,reduce operation complication,prevent anastomotic leakage,and increase the success rate of the operation.

ABSTRACT

In case of acute obstruction secondary to colorectal carcinoma, emergent surgical intervention is needed and many types of operation including staged operation, subtotal colectomy and intraoperative colonic lavage have been tried. But those operations are not elective and have high morbidity and mortality. By the use of the Flexible covered stent, we resolved obstruction in 4 cases of colorectal carcinoma and prepared the bowel mechanically by polyethylene glycol. After improvement of the general state of the patients and assessment of the extent of the disease, we performed elective and single stage operation in all cases and had no complications associated with the stent. The treatment with the Flexible covered stent is a safe and effective procedure for malignant colorectal obstruction that makes it possible to perform a single stage operation.
