ABSTRACT

Objective:To investigate the clinical efficacy and adverse reactions of pemetrexed disodium in the maintenance treatment of advanced lung adenocarcinoma after chemotherapy with pemetrexed disodium and platinum.Methods:The clinical data of 35 patients with stage Ⅳ lung adenocarcinoma who received chemotherapy with pemetrexed disodium and platinum and were well treated in Beijing Huairou Hospital from January 2013 to August 2020 were retrospectively analyzed. Maintenance therapy with pemetrexed disodium was initiated after the completion of combination chemotherapy until disease progression. The clinical characteristics, therapeutic effects, adverse reactions, progression-free survival, and overall survival of the 35 patients were evaluated.Results:Among the 35 patients, no patients had complete remission, 11 patients had partial remission, 22 patients had stable disease, and 2 patients had progressive disease. The objective remission rate was 31.4%, disease control rate was 94.3%, median progression-free survival was 9.53 months, median overall survival was 18.21 months, 1-year survival rate was 68.6%, 2-year survival rate was 31.4%, and 3-year survival rate was 11.4%. Gender, age, smoking, and the baseline characteristics of patients undergoing first-line pemetrexed disodium or second-line pemetrexed disodium treatment had no effects on progression-free survival (all P > 0.05). Positive gene mutation and receiving four or more chemotherapy cycles had a protective effect on progression-free survival (both P < 0.05). Chemotherapy-related adverse reactions mainly included myelosuppression, nausea, elevated transaminase, and nephrotoxicity, all of which were mild and were relieved after symptomatic treatment. Conclusion:Pemetrexed disodium is effective and safe in the maintenance treatment of advanced lung adenocarcinoma. The results of this study are scientific.

ABSTRACT

Objective:To investigate the changes in serum miRNA-21 and miRNA-4534 levels in patients with lung adenocarcinoma and their relationships with pathological characteristics and prognosis.Methods:A total of 148 patients with lung adenocarcinoma who received treatment in the First People's Hospital of Huzhou from January 2018 to January 2019 were included in the observation group. An additional 100 healthy controls who concurrently received physical examinations were included in the control group. The relative expression of serum miRNA-21 and miRNA-4534 was measured using real-time fluorescence quantitative PCR. The relative expression of serum miRNA-21 and miRNA-4534 was compared between the two groups. The relative expression of serum miRNA-21 and miRNA-4534 was compared between patients with different pathological characteristics. Total survival time was compared between patients with high and low miRAN-21 expression and between patients with high and low miRNA-4534 expression.Results:The relative expression of miRNA-21 and miRNA-4534 in the observation group was 3.27 ± 0.87 and 6.74 ± 1.56, respectively, which was significantly higher than 1.00 ± 0.01 and 1.02 ± 0.02 in the control group ( t = 20.07, 36.64, both P < 0.05). There was no significant difference in the relative expression of miRNA-21 and miRNA-4534 between different genders, ages, body mass index values, tumor regions, and tumor diameters (all P > 0.05). The relative expression of serum miRNA-21 in patients with stage Ⅲ-Ⅳ lung adenocarcinoma was 4.45 ± 1.05, which was significantly higher than 1.92 ± 0.53 in patients with stage Ⅰ-Ⅱ lung adenocarcinoma. The relative expression of serum miRNA-21 in patients with lymph node metastasis was higher than that in patients without lymph node metastasis (4.97 ± 1.28 vs. 1.34 ± 0.60, t = 17.53, 23.48, both P < 0.05). The relative expression of miRNA-4534 in patients with stage Ⅲ-Ⅳ lung adenocarcinoma was higher than that in patients with stage Ⅰ-Ⅱ lung adenocarcinoma (8.97 ± 1.85 vs. 4.48 ± 1.09, t = 17.17, P < 0.05). The relative expression of miRNA-4534 in patients with lymph node metastasis was higher than that in patients without lymph node metastasis (9.65 ± 1.35 vs. 3.78 ± 0.91, t = 30.59, P < 0.05). The total survival time in patients with high miRNA-21 expression was shorter than that in patients with low miRNA-21 expression [(18.37 ± 4.35) months vs. (29.93 ± 3.24) months, t = 18.45, P < 0.05]. The total survival time in patients with high miRNA-4534 expression was shorter than that in patients with low miRNA-4534 expression [(17.56 ± 4.21) months vs. (30.43 ± 3.63) months, t = 19.97, P < 0.05)]. Conclusion:Patients with lung adenocarcinoma have high expression of miRNA-21 and miRNA-4534 in the serum, which is closely related to the tumor-node-metastasis stage, lymph node metastasis, and prognosis.

ABSTRACT

Objective:To analyze the clinical and CT imaging characteristics of sub-centimeter solitary pulmonary nodules (SSPNs) with ground glass opacity.Methods:The clinical and imaging data of 258 patients with SSPN who underwent surgical treatment in Beijing Tongren Hospital from May 2015 to May 2020 were retrospectively reviewed. There were 113 males and 145 females with a median age of 45 (range 30-84) years. The CT images of the lesions showed ground glass opacity, including 60 cases of pure ground glass nodule (pGGO) and 198 cases of mixed ground glass nodule (mGGO). The largest diameter of the lesion was ≤ 10 mm, and the lesions were all resected by video-assisted thoracic surgery, and diagnosed by postoperative pathological examination. The relationship between the lesion size and the detection rate of malignancy; the diagnostic value of CT imaging features for differentiating benign and malignant SSPN, and postoperative immunohistochemical indexes and target gene mutations for differentiating primary from metastatic lung cancer were analyzed.Results:Of the 258 patients, 59 had lesions<7 mm in diameter and 199 between 7 and 10 mm in size. There were no intraoperative deaths. The postoperative pathological examination confirmed primary bronchial lung cancer in 190 cases, metastatic lung cancer in 12 cases, and benign lung lesions in 56 cases. The detection rates of malignant lesions in mGGO and pGGO were 81.31% (161/198) and 68.33% (41/60), respectively (χ 2=4.66, P = 0.032). The detection rates of malignant lesions and primary cancer in mGGOS with diameter 7-10 mm was higher than those with diameter<7 mm[(87.01%(134/154) vs. 61.37%(27/44) and 83.77%(129/154) vs. 56.82%(24/44), χ 2=14.82, χ 2=12.93, all P<0.001 ]. The border irregularity on CT imaging was presented in 47.03% (95/202) of malignant lesions, which presented in 48.42% (92/190) of primary lung cancers; while only presented in 17.86% (10/56) of benign lung lesions (χ 2=15.46, χ 2=16.64, all P<0.001). The frequency of vascular signs was 56.44% (114/202) in lung malignant lesions, 54.74% (110/190) in primary bronchial lung cancer; while only 5.35% (3/56) in lung benign lesions (χ 2=46.16, χ 2=48.07, all P<0.001). Among mGGO patients with lesion diameter<7 mm, those with irregular lesion borders and vascular signs on CT imaging 91.30% (21/23) were pulmonary malignant lesions, while those without these two signs only 28.57% (6/21). Immunohistochemistry results showed that the primary bronchial lung cancer group had a higher positive rate of new aspartic proteinase A (Napsin A, 79.47%) and thyroid transcription factor-1 (TTF-1, 75.79%) than the metastatic lung cancer group. There were three patients with BRAF-v600e mutation in our series, and all of them were finally confirmed as metastatic thyroid cancer. Conclusion:In sub-centimeter solitary pulmonary nodules with ground glass opacity, mGGO lesions are more likely to be malignant lesions than pGGO lesions, and the lesions with larger size are more likely to be malignant. Imaging features such as irregular lesion borders and vascular signs are important in differentiating between benign and malignant lesions. For metastatic lesions, immunohistochemical and genetic testing are helpful in determining the primary tumor site.

Objective@#To analyze the expression of c-MET and its prognostic correlation in patients with lung adenocarcinoma.@*Methods@#The clinical data and pathological specimen of patients with lung adenocarcinoma in Dalian 5th People′s Hospital from January 2006 to December 2011 were retrospectively analyzed. The expression difference of c-MET between lung adenocarcinoma tissue and normal adjacent tissues was compared. The correlation of c-MET with the pathology and clinical factors was also analyzed.@*Results@#A total of 82 patients were retrospective analyzed, including 82 pathological specimens of lung adenocarcinoma and 45 specimens of normal adjacent tissues. Among 53 patients with stage Ⅰ-Ⅱ lung adenocarcinoma, 31 cases had low expression of c-MET and 22 cases had high expression of c-MET. Among 29 patients with stage Ⅲ lung adenocarcinoma, 10 cases had low expression of c-MET and 19 cases had high expression of c-MET. There was a correlation between TNM stage and c-MET positive expression in lung adenocarcinoma (P=0.037). The positive expression rate of c-MET was not significantly correlated with age, sex and differentiation degree (P > 0.05). Among 82 cases of lung adenocarcinoma, 39 cases had low expression of c-MET and 43 cases had high expression of c-MET; 45 cases of para-carcinoma tissuehad low expression of c-MET. The positive expression rate of c-MET in lung adenocarcinoma tissues was significantly higher than that in para-carcinoma tissue (P < 0.01). Kaplan-Meier survival curve analysis showed that the median disease-free survival was 41.5 months in c-MET high-expression group and 55.3 months in low-expression group. The expression level of c-MET was closely related to disease-free survival in lung adenocarcinoma patients (P < 0.05).@*Conclusions@#The expression of c-MET is significantly increased in patients with lung adenocarcinoma. The expression level has relevance with TNM staging and prognosis.

ABSTRACT

Objective To investigate the expression of hypoxia inducible factor 1α (HIF-1α) and glucose transporter 1 (GLUT1) in lung adenocarcinoma and its correlation with tumor metastasis. Methods SP immunohistochemistry was used to detect GLUT1 and HIF-1α protein expression in 125 lung adenocarcinoma, including 41 cases without metastasis, 38 cases with lymphatic metastasis and 46 cases with brain metastasis. The correlation of GLUT1 and HIF-1α in lung adenocarcinoma metastasis was analyzed by using x 2 test and Pearson correlation analysis. Results Most lung adenocarcinoma were histologically heterogeneous, which contained more than one adenocarcinoma type. 73.2 % (30/41) cases were acinar predominant adenocarcinoma in lung adenocarcinoma without metastasis; 53.6 % (15/38) cases were acinar predominant adenocarcinoma and 26.3 % (10/38) cases were solid predominant adenocarcinoma in lung adenocarcinoma with lymphatic metastasis; 47.8 % (22/46) cases were papillary predominant adenocarcinoma and 34.8 % (16/46) cases were solid predominant adenocarcinoma in lung adenocarcinoma with brain metastases. The expression level of GLUT1 and HIF-1α in lung adenocarcinoma with lymphatic metastasis group was higher than that of the group without tumor metastasis (P< 0.05); the expression of GLUT1 and HIF-1α were positively correlated (r=0.407, P=0.000). Conclusions Papillary adenocarcinoma is the most histological type in lung adenocarcinoma with brain metastasis, suggesting that papillary adenocarcinoma is more prone to brain metastasis. The expression of GLUT1 and HIF-1α play an important role in lymph node metastasis and brain metastasis of lung adenocarcinoma.

ABSTRACT

Objective To explore the expression of tankyrase (TNKS) and its relationship with WNT/β-catenin signal?ing pathway in lung acinar adenocarcinoma. Methods Seventy-two samples of single subtype alveolar like lung adenocarci?noma (lung adenocarcinoma group) and 67 specimens of normal lung tissue adjacent to carcinoma (adjacent to carcinoma group) were collected. Immunohistochemical method was used to detect expressions of TNKS, beta-catenin (β-catenin) and c-myc protein. The correlation of each protein expression in lung adenocarcinoma tissues was analyzed. The differential ex?pression of TNKS was detected by Western blot assay in two groups. Results Tankyrase protein was mainly expressed in cy?toplasm. The expression ofβ-catenin protein was mainly in cytoplasm and nuclear of lung adenocarcinoma. The expression ofβ-catenin was mainly in cytoplasm, and a small amount was in nuclear of the adjacent group. The c-myc protein was ex?pressed mainly in the nucleus. The positive expression rates of TNKS,β-catenin and c-myc protein were significantly high?er in lung adenocarcinoma group than those of adjacent to carcinoma group (P<0.05). The expression ofβ-catenin in cyto?plasm and nucleus was positively correlated with the expression of TNKS and c-myc (P<0.05). Western blot analysis showed that the relative expression level of TNKS was significantly higher in lung adenocarcinoma group than that of adja?cent to carcinoma group (0.497 ± 0.021 vs. 0.237 ± 0.015, t=13.00, P<0.01). Conclusion Abnormally high expression of TNKS in lung adenocarcinoma may promote the occurrence of lung cancer by regulating the WNT signaling pathways. Inhib?iting TNKS expression may become a new target to treat lung adenocarcinoma.

ABSTRACT

Objective To study the expressions and significances of leptin,estrogen and estrogen receptor (ER) in pulmonary squamous carcinoma and adenocarcinoma.Methods The expressions of leptin and estrogen receptor were detected in 58 cases of lung adenocarcinoma and 63 cases of pulmonary squamous cell carcinoma and 50 cases of normal lung tissue samples by immunohistochemical menthod,the levels of estrogen were also detected in patients with venous blood at the same time.Comparison of differential expression of leptin,estrogen and estrogen receptor in lung adenocarcinoma and lung squamous cell carcinoma,normal tissues,and explore their relationships with lung adenocarcinoma.Results Leptin,estrogen and estrogen receptor positive rates in lung adenocarcinoma group were 65.5%,36.2% and 58.6% respectively,and 33.3%,15.9%,30.2% in lung squamous cell carcinoma group.There were a statistical difference between the two groups (x2 =4.324,P<0.050;x2 =5.372,P <0.050;x2 =5.718,P <0.050).In the normal control group the positive rates were 24.0%,4.0% and 0 respectively,and there was a statistical difference compared with lung adenocarcinoma group (x2 =7.126,P <0.010;x2 =9.683,P<0.005;x2 =22.308,P <0.005).In lung adenocarcinoma group,leptin,estrogen and estrogen receptor positive rate have no relationships with tumor stage (x2 =0.001,P=0.950;x2 =0.061,P =0.900;x2 =0.178,P=0.750) and primary tumor size (x2=0.023,P=0.900;x2 =0.001,P=0.950;x2 =0.001,P=0.950).Conclusion Leptin,estrogen and ER were expressed highly in adenocarcinoma of lung tumor.The expressions of leptin,estrogen and ER may associated with the carcinogenesis,development and clinical type of adenocarcinoma of lung.

ABSTRACT

Primary tumors of the lung are uncommon in pediatric patients, particularly bronchioloalveolar carcinoma (BAC). An 11-year-old female suffering from back pain for 1 month was referred to Seoul St. Mary's Hospital for treatment of a pathologic fracture of the lumbar spine. Comprehensive evaluation disclosed numerous pulmonary metastases of rhabdomyosarcoma (stage IV). During chemotherapy, most of the lung lesions regressed, with the exception of two nodules. Wedge resections, intended for diagnosis and cure, yielded a histologic diagnosis of BAC.

ABSTRACT

O carcinoma bronquíolo-alveolar e os adenocarcinomas mistos com componente bronquíolo-alveolar têm apresentações variadas e múltiplos padrões por imagem. O objetivo deste ensaio foi descrever e ilustrar os achados mais característicos desses tumores em TC. São descritas três apresentações: nódulo pulmonar solitário, consolidação e apresentação difusa, sendo que as duas últimas são importantes no diagnóstico diferencial com acometimento infeccioso. O conhecimento das diversas apresentações e a utilização de propedêutica diagnóstica adequada são definitivos para o diagnóstico precoce e o aumento na sobrevida.


Bronchioloalveolar carcinoma has various presentations and a wide spectrum of imaging patterns, as does adenocarcinoma with a bronchioloalveolar component. The objective of this essay was to describe and illustrate the CT findings that are most characteristic of these tumors. Three presentations are described: solitary pulmonary nodule, consolidation, and diffuse pattern. The last two should be included in the differential diagnosis, together with infectious diseases. Knowledge of the various presentations and the use of proper diagnostic procedures are crucial to early diagnosis and to improving survival.

ABSTRACT

Objective To analyse the difference between stage Ⅰ pure bronchioloalveolar carcinoma (BAC) and stage Ⅰ adenocarcinoma of the lung among operative cases.Methods We use the Lobectomy Cases Registration and Statistics System database (2006-2011) to compare the epidemiology,clinical presentation,image characteristics,surgical outcomes,recurrence and overall survival between BAC and adenocarcinoma groups.All the patients received lobectomy procedure in the department of thoracic surgery of Peking University People' s Hospital.Results Excluding those cases with both BAC and adenocarcinoma aspects,337 patients were enrolled.Thirty-nine patients were stage Ⅰ pure BAC and 298 patients were stage Ⅰadenocarcinoma.BAC has its proper clinical spectrum,occurring more frequently in women (69.2% vs.52.0%,P =0.042)and in younger patients (57.4 vs.61.8,P =0.014).BAC also seems to be less dependent on tobacco exposure (12.8% vs.29.9%,P =0.026).The percentage of ground-glass opacity (GGO) in CT scan of BAC patients was much more than that registered in adenocarcinoma patients (35.9% vs.9.7%,P <0.001).And the tumor size of BAC group was smaller than that of the adenocarcinoma group (1.4 cm vs.2.3 cm,P <0.001).The operation method,time,blood loss and complications were similar between the two groups.Kaplan-Meier survival curves showed that both 3-year disease-free survival (DFS) and overall survival (OS) were significantly higher in patients affected by BAC (100% vs.76.1%,P =0.030 and 100% vs.86.1%,P =0.041).Conclusion BAC presents specificity in its epidemical,clinical,radiological and evolutionary aspects.Stage Ⅰ pure BAC patients have better prognosis following video-assisted thoracoscopic lobectomy and system lymph node dissection than the similar stage adenocarcinoma patients.

ABSTRACT

ObjectiveTo explore the expression and clinical significance ofaquaporin (AQP) 1 and AQP 4 in human pulmonary adenocarcinoma H1299 cell line.MethodsH1299 cell line in human pulmonary adenocarcinoma(pulmonary adenocarcinoma group) were obtained,the expressions of AQP1 and AQP4 in mRNA level and their locations were determined in H1299 cell line respectively by semiquantitative reverse transcription polymerase chain reaction (RT-PCR) and Western blot analysis.The migration of tumor cells were observed by Matrigel invasion assay.Then normal tissues adjacent of pulmonary adenocarcinoma (above cancer line 3 cm,no tumor cell with pathological proven) were as control group.ResultsThe results of RT-PCR showed that AQP1,AQP4 mRNA was 1.030 ± 0.070 and 1.140 ± 0.190 in conlrol group,which were lower than those in pulmonary adenocarcinoma group (2.021 ± 0.250 and 2.180 ±0.180)(P<0.05 ).The results of Western blot showed AQP1,AQP4 located on the membrane of H1299 cell.Both AQPI and AQP4 mRNA expressed very high in pulmonary adenocarcinoma group,while expressed very low in control group (P<0.05).Matrigel invasion assay showed that the invasion was positively related to AQP1,AQP4(r =0.351,P < 0.05 ).ConclusionAQP1,AQP4 significantly over express in H1299 cell line,both of them phy important roles in the growth of tumor tissue and cell migration.

ABSTRACT

OBJETIVO: Analisar os aspectos clínico-patológicos do carcinoma bronquioloalveolar (CBA) e a sobrevida em uma amostra de pacientes com estadiamento clínico I. MÉTODOS: Foram estudados retrospectivamente 26 pacientes com diagnóstico de CBA e estágio clínico I, , segundo a classificação tumor-node-metastasis (TNM, tumor-linfonodo-metástase),(15)operados no Instituto de Doenças do Tórax da Universidade Federal do Rio de Janeiro, na cidade do Rio de Janeiro, RJ, entre 1987 e 2007, quanto a variáveis clínico-patológicas e radiológicas, mortalidade e sobrevida. Os dados foram colhidos dos prontuários médicos dos pacientes e analisados estatisticamente. RESULTADOS: Houve predomínio de mulheres (n = 16). A idade média ao diagnóstico foi de 68,5 anos. Houve predomínio de tabagistas (69,2 por cento). As formas de apresentação assintomática (84,6 por cento) e nodular (88,5 por cento) foram as mais comuns. Houve predileção pelos lobos superiores (57,7 por cento). O estágio patológico IB foi o mais comum, seguido pelos estágios IA e IIB (46,2 por cento, 38,4 por cento e 15,4 por cento, respectivamente). Não houve óbitos hospitalares. Quatro pacientes faleceram durante o seguimento pós-operatório, com tempo livre de doença médio de 21,3 meses. A taxa de sobrevida global em cinco anos foi 83 por cento. A probabilidade de sobrevida para os pacientes diagnosticados depois de 1999 tendeu a ser maior do que para aqueles diagnosticados até 1999 (taxa de sobrevida em três anos: 92 por cento vs. 68 por cento; p = 0,07). CONCLUSÕES: Os aspectos clínico-patológicos da amostra estudada foram semelhantes àqueles de estudos anteriores em pacientes com CBA.


OBJECTIVE: To analyze the clinicopathological aspects of bronchioloalveolar carcinoma (BAC) and the survival in a sample of patients at clinical stage I. METHODS: A retrospective study involving 26 patients diagnosed with clinical stage I BAC and undergoing surgery at the Thoracic Diseases Institute of the Federal University of Rio de Janeiro, in the city of Rio de Janeiro, Brazil, between 1987 and 2007. We analyzed clinicopathological and radiological aspects, as well as mortality and survival. The data, which were collected from the medical charts of the patients, were statistically analyzed. RESULTS: Females predominated (n = 16). The mean age at diagnosis was 68.5 years. Most patients were active smokers (69.2 percent). The most common forms of presentation of BAC were the asymptomatic form (84.6 percent) and the nodular form (88.5 percent). Involvement of the upper lobes predominated (57.7 percent). Stage IB was the most common pathological stage, followed by stages IA and IIB (46.2 percent, 38.4 percent and 15.4 percent, respectively). There was no in-hospital mortality. Four patients died during the postoperative follow-up, with a mean disease-free survival time of 21.3 months. The overall five-year survival rate was 83 percent. The probability of survival for the patients diagnosed after 1999 showed a trend toward an increase when compared with that for those diagnosed up through 1999 (three-year survival rate: 92 percent vs. 68 percent; p = 0.07). CONCLUSIONS: The clinicopathological aspects of this study sample were similar to those of patients with BAC evaluated in previous studies.

The patient presented with hemoptysis and had a history of pulmonary tuberculosis. Chest radiographs and computed tomography revealed a well-circumscribed, thin-walled cavitary lesion in the right upper pulmonary lobe. Gross examination of a lobectomy specimen showed a bronchiectatic cavity and a fungus ball within it. There were also several ill-defined small gray-white nodules around the cyst, nodules that were mucinous BAC. On microscopy, they were composed of columnar tumor cells along the intact alveolar walls in a single layer.

ABSTRACT

Objective To compare the clinic characteristics, recurrences and prognosis in patients with stage Ⅰ bronchioloalveolar carcinoma (BAC) and adenocarcinoma of the lung. Methods The data of 56 patients with stage Ⅰ BAC and 169 patients with stage Ⅰ adenocarcinoma were analyzed retrospectively. Results The overall 1-, 3-, 5- year survival rates were 94.7%, 83.5% and 61.2%, respectively. Compared with adenocarcinoma of the lung, BAC showed a better survival rate(x2 =6.36, P =0.012). After surgery patients with BAC were prone to develop intrathoracic recurrence, and adenocarcinoma was equal between intrathoracic recurrence and extrathoracic metastasis. The rate of intrathoracic recurrence and extrathoracic metastasis between BAC and adenocarcinoma was significantly different (14/16 vs. 27/59, x2 =8.85, P=0.004). In both group, preoperative asymptomatic patients had better survival rate(x2 = 7.28, P = 0.007; x2 = 6.07, P = 0. 014). Univariate analysis indicated that sex, age(< 60 years and ≥60 years), location of tumor and smoking history did not significantly influence survival in patients with stage Ⅰ BAC or adenocarcinoma (P > 0.05). Conclusion The prognosis of stage Ⅰ BAC is superior to that of stage Ⅰ adenocarcinoma. BAC is prone to develop intrathoracic recurrence, and adenocarcinoma is equal between intrathoracic recurrence and extrathoracic metastasis. Early diagnosis of lung cancer could improve long-term survival.

ABSTRACT

Purpose To explore the value of muti-planar reconstruction (MPR) with 64-slices spiral CT in diagnosing solitary bronchioloalveolar carcinoma (BAC).Materials and Methods The CT manifestations of pathologically-proved BAC were analyzed. MPR images were obtained with 3D reconstruction software, and the findings on the MPR images were compared with primary axial images.Results For all 30 cases of BAC, MPR images were significantly superior to conventional axial images in the aspect of demonstrating ground glass opacity, deep lobulation, halo sign, spiculation, pleural indentation, vascular convergence sign and air-bronchogram. Meanwhile, the false vacuole sign was reduced on MPR images. Conclusions MPR with spiral CT, easily performed, can show the morphology of BAC more precisely than routine axial images, which could benefit the diagnosis of solitary BAC.

Tumores neurogênicos malignos do mediastino em adultos são raros e extremamente agressivos. Este artigo relata o caso de um paciente de 61 anos com a ocorrência simultânea de schwannoma maligno de mediastino e carcinoma bronquíolo-alveolar. Apesar do carcinoma bronquíolo-alveolar estar presente em 4-7 por cento dos tumores torácicos sincrônicos ressecados, essa associação nunca foi apresentada na literatura. É, no entanto, um achado frequente em pacientes com infiltrados aparentemente inflamatórios e com opacidades em vidro fosco, como apresentado neste caso.


Malignant neurogenic mediastinal tumors in adults are uncommon and extremely aggressive. We report the case of a 61-year-old male patient with the simultaneous occurrence of malignant mediastinal schwannoma and bronchioloalveolar carcinoma. Although bronchioloalveolar carcinoma is present in 4-7 percent of the resected synchronous thoracic tumors, this association has never been reported in the literature. However, it is a common finding in patients presenting apparently inflammatory infiltrates and ground-glass opacities, as in the case presented here.
