ABSTRACT

Purpose: To report the intermediate-term safety and efficacy of Aurolab aqueous drainage implant (AADI) in patients with glaucoma. Methods: Retrospective review of patients who underwent AADI between January 2013 and December 2016. Patients aged >16 years and with a minimum follow-up of 6 months were included. Success was defined as complete when the intraocular pressure was ?6 and ?21 mmHg without antiglaucoma medication and as qualified if those requiring additional antiglaucoma medications were included. Results: The study included 55 patients (55 eyes) with a mean age � standard deviation (SD) of 47.3 � 18.1 years with a mean follow-up of 16.7 � 11.4 months. Mean intraocular pressure reduced from 30.8 � 11.1 mmHg to 13.1 � 4.7, 14.1 � 4.8, 15.7 � 2.5 (P < 0.001) mmHg at 6 months, 1 year, and 2 years, respectively. The mean number of antiglaucoma medications reduced from 3.4 � 1 to 0.8 � 1.2, 0.7 � 1.1, 0.8 � 1 (P < 0.001) at 6 months, 1 year, and 2 years, respectively. The cumulative probability of complete and qualified success was 62% and 100% at 6 months, 54% and 92% at 1 year, and 43% and 88% at 2 years, respectively. Four patients failed during the follow-up period. Postoperative complication occurred in 28 eyes (51%), of which 17 eyes (31%) required intervention. Conclusion: AADI is a safe and effective treatment for the control of intraocular pressure in patients with glaucoma.

ABSTRACT

We investigated whether there was any difference in the extent of cupping reversal after reduction of intraocular pressure(IOP) by trabeculectomy between advanced glaucoma(stage IV of Jonas classification; 6 eyes) and less advanced glaucoma(stage III or less; 5 eyes). Computer-aided morphometry was performed in 11 eyes of 10 adult patients with chronic open-angle glaucoma in whom reversal of glaucomatous cupping was apparent photographically. Mean preoperative IOP, postoperative IOP, and percent reduction of IOP respectively were 42.5+/-8.3mmHg, 12.5+/-3.8mmHg, and 69.1+/-13.2%(range, 40.6 to 81.0%) over a mean period of 22.7+/-14.6 months(range, 5 to 53 months). Mean cup area and cup to disc ratio decreased significantly(P<0.01, Wilcoxon signed rank test). The reversal of rim area/disc area ratio showed a marginally significant correlation with the percent reduction of IOP(Spearman`s correlation coefficient=0.57, P=0.07). The reversal of vertical cup to disc ratio was significaltly lower in eyes with advanced glaucoma than in eyes with less advenced glaucoma(P=0.03, Mann-Whitney U test). Although reversal of other disc parameters tended to be lower in eyes with advanced glaucoma, they were not statistically significant. In eyes with advanced glaucoma, reversal of rim area/disc area ratio was more pronounced in the nasal disc region than in the superior or inferior disc region. Our results suggest that the extent of cupping reversal after reduction of IOP may decrease in the advanced stage of glaucoma than in mild to moderate stages of glaucoma.

ABSTRACT

Using Heidelberg Retina Tomograph (HRT, Heidelberg Engineering, Heidelberg, Germany) we investigated whether reversal of glaucomatous optic disc cupping occurs in adult glaucoma patients following intraocular pressure (IOP) reduction by trabeculectomy, and whether there is any regional difference in topographic change of optic disc. Thirteen eyes of 13 glaucoma patients in whom the percent IOP reduction was 25% or more at postoperative follow-up of 2 months, were enrolled. Cup-to-disc area ratio was decreased significantly, while rim area and rim volume were increased significantly(p<0.05, Wilcoxon signed rank test) at postoperative follow-up of 2 months. Among the optic disc sectors, rim area in superior and inferior quadrant, especially in superotemporal and inferotemporal octant, increased significantly. Decrease in maximum cup depth correlated significantly with the percent IOP reduction (r=0.698, p=0.016). Reversal of glaucomatous optic disc cupping after IOP reduction following trabeculectomy in adult patients was confirmed by HRT, Furthermore, the regions of selective reversal in superior and inferior quadrant coincide with the well-known portion of early functional and structural glaucomatous optic nerve head damage.
