ABSTRACT

The fifth edition of the World Health Organization (WHO) Classification of Tumors of the Central Nervous System (WHO CNS5) features several changes in the classification, diagnostic criteria, nomenclature, and grading of diffuse gliomas. Adult-type diffuse gliomas are genetically defined and include astrocytoma, isocitrate dehydrogenase (IDH)-mutant, oligodendroglioma, IDH-mutant and 1p/19q codeleted, and glioblastoma, IDH-wildtype. This review briefly discusses two tumor types: astrocytoma, IDH-mutant, and oligodendroglioma, IDH-mutant and 1p/19q codeleted, with emphasis on relevant changes in their classification and defining molecular genetic alterations. A simplified approach to the diagnosis of these tumors is provided.

Background: Despite the evidence that lifestyle modifications (LSMs) play important role in glycaemic control, adherence to healthy lifestyles has been variable in Nigeria and this has great impact on the success or failure of pharmacological care. Aim: The study was designed to determine adherence to LSMs among ambulatory type 2 diabetic Nigerians in a resource-poor setting of a primary care clinic in Eastern Nigeria. Study Design: This was a primary care clinic-based cross-sectional study done on consecutively sampled 120 adult type 2 diabetic patients who were on management for diabetes mellitus for at least 3 months at the primary care clinic of a tertiary hospital in Nigeria. Place and Duration of Study: The study was carried out at the primary care clinic of a tertiary hospital in South-Eastern Nigeria from April 2011 to December 2011. Methodology: Instrument of data collection was pretested, structured and intervieweradministered questionnaire. Each item of lifestyle was scored on a five points Likert scale ordinal responses of always, most times, sometimes, rarely and none. Adherence to lifestyle modifications was assessed in the previous 12 months for alcohol and tobacco use and 7 days for physical activity, dietary fruits, vegetables, fats and oil consumptions. Each of the domains of lifestyle was given a score of one point for healthy lifestyle and zero point for unhealthy lifestyle. Type 2 diabetic patients who scored 6 points in all the evaluated domains were adherent. Awareness of LSMs and Specific adherence to lifestyle factors were also determined. Results: The awareness and overall adherence rates to LSMs were 88.3% and 22.5% respectively Specifically, adherence was highest with the non-tobacco use (100.0%) followed by adequate dietary vegetables consumptions (96.7%) and non-use of alcohol (90.8%). Other adherence rates were consumptions of adequate dietary fruits (51.7%), dietary fat and oils (23.3%) and physical activity (22.5%). Adherence was significantly associated with young age <40 years (p=0.039). Conclusion: The awareness of LSMs was high but was not translated to comparative adherence. The adherence to non-tobacco use was rated highest and physical activity the lowest. Young age <40 years was significantly associated with adherence. Targeting the domains of inadequate adherence for improvement is a primary care imperative especially in resource-poor settings where there are limited options for healthy living

ABSTRACT

Objective To evaluate the outcome of endovascular treatment with coarctation of aorta (CoA).Methods Between November 2007 and March 2012,11 patients,including 8 male and 3 female from 17 to 43 years old,with native CoA who were diagnosed by computed tomography angiography accepted the endovascular treatment.The arterial pressure difference between the femoral artery and the narrow segment was measured during the operation,and the stent was released by expanding the balloon.Results Endovascular treatment were successful in all cases without any major adverse events.Significant reduction of systohc blood pressure was observed from (64.09 ± 7.81) mmHg (1 mmHg =0.133 kPa) to (11.18 ± 3.37) mmHg.The mean CoA diameter pre-stent (4.000 ± 0.775) mm,post-stent (17.090 ± 2.427) mm.1 patient associated with patent ductus arteriosis had no persistent left-to-right shunt after covered stent implantation.The follow up range was 33 months.1 patient in simple balloon was retreatment by surgical because of restenosis.And no retraction was observed of 10 patients after stent implantation.Conclusion The early and midterm result of endovascular treatment with CoA is satisfactory,and longer follow-up is needed for its longterm outcomes.

La prevalencia de hipolactasia tipo adulto está influenciada por la etnicidad y la geografía. Los genotipos CC y GG, de los SNPs C/T-13910 y G/A-22018, respectivamente determinan hipolactasia en ciertos grupos étnicos y países del mundo. El objetivo de este estudio fue analizar estos SNPs en muestras de los tres grupos étnicos que habitan el Caribe Colombiano. Trescientos sesenta y un sujetos, agrupados como afrodescendientes, indígenas y mestizos, fueron genotipificados usando PCR/RFLP. El análisis genético se hizo mediante Arlequin 3.11 y las frecuencias genotípicas fueron comparadas con Statgraphics Centurion XVI. Solamente el SNP C/T-13910 mostró equilibrio de Hardy- Weinberg y no hubo desequilibrio de ligamiento entre los SNPs estudiados. La frecuencia del genotipo CC-13910 fue 90% en afrodescendientes, 95% en indígenas y 80% en mestizos. En indígenas la frecuencia de GG-22018 fue 23% pero dicho genotipo no se halló en afrodescendientes y mestizos. El genotipo AA-22018 no se halló en indígenas. Ningún grupo presentó el genotipo TT-13910. Las frecuencias genotípicas fueron estadísticamente diferentes entre los grupos estudiados y las de los genotipos CC-13910 y GG-22018 no concordaron con las frecuencias fenotípicas reportadas en otros estudios. Los resultados sugieren que la posibilidad diagnóstica de hipolactasia mediante genotipificación de estos polimorfismos es escasa en el Caribe Colombiano.


The prevalence of adult-type hypolactasia is influenced by ethnicity and geography. The CC and GG genotypes of the SNPs C/T-13910 and G/A-22018, respectively indicate hypolactasia in certain ethnic groups worldwide. The aim of this study was to analyse these SNPs in samples of the three ethnic groups that inhabit the Colombian Caribbean. Three hundred and sixty-one subjects were genotyped using PCR/RFLP. These subjects were grouped as being of African descent, Indigenous and Mestizo. The genetic analysis was performed through Arlequin 3.11 and genotype frequencies were compared with Statgraphics Centurion XVI. Only SNP C/T-13910 showed Hardy-Weinberg equilibrium and there was no linkage disequilibrium between the SNPs. The frequency of CC-13910 was 90% in Afro-descendant, 95% in indigenous people and 80% in mestizos.The frequency of GG-22018 was 23% in indigenous people, but this genotype was not present in afro-descendants and Mestizos. The indigenous people did not have AA-22018, and none of the groups had TT-13910. The genotype frequencies were statistically different among the groups studied and the frequencies of CC-13910 and GG-22018 were not in concordance with the phenotype frequencies reported in other papers. The results suggest that diagnostic possibility of hypolactasia by genotyping of those polymorphisms in the Colombian Caribbean population is scarce.


A prevalência da hipolactasia primária tipo adulto é influenciada pela etnicidade e a geografia. Os genótipos CC e GG, dos SNPs C/T-13910 e G/A-22018, respectivamente, são determinantes da hipolactasia em alguns grupos étnicos e países do mundo. O objetivo do presente estudo foi analisar estes SNPs em amostras dos três grupos étnicos do Caribe Colombiano. Trezentas e sessenta e uma pessoas reunidas como afrodescendentes, indígenas e mestiços foram genotipificadas utilizando PCR/RFLP. A análise genética foi realizada usando o software Arlequin 3.11 e as frequências genotípicas foram comparadas com o Statgraphics Centurion XVI. Somente o SNP C/T-13910 mostrou equilíbrio de Hardy-Weinberg e não houve desequilíbrio de ligamento entre os SNPs estudados. A frequência do genótipo CC-13910 foi de 90% para afrodescendentes, 95% para indígenas e 80% em mestiços. Nos indígenas a frequência de GG-22018 foi de 23% mas tal genótipo não esteve presente na população de afrodescendentes e mestiços. O genótipo AA-22018 não foi encontrado em indígenas. Nenhum grupo apresentou o genótipo TT-13910. As frequências genotípicas foram estatisticamente diferentes entre os grupos avaliados e as dos genótipos CC-13910 e GG-22018, não concordaram com as frequências fenotípicas relatados em outros estudos. Os resultados sugerem que a possibilidade diagnóstica de hipolactasia através de genotipificação destes polimorfismos é escassa em populações do Caribe Colombiano.

ABSTRACT

Computed tomography of a Japanese man in his mid-forties with a complaint of right-side chest pain showed a dome-shaped smooth-surfaced mediastinal mass, which was extirpated. The cut surface was highly hemorrhagic and necrotic and not related to the original pulmonary tissues. Although routine sectioning detected bronchial cartilage, immunohistochemical analyses clearly showed the presence of alveolar type II cells; only the alveolar type II cells located at the periphery of this mass showed positive staining for cytokeratins, thyroid transcription factor 1, surfactant protein A, epithelial membrane antigen and Krebs von den Lungen-6. Thus, these analyses are useful for the detection of pulmonary components, even in severely hemorrhagic and necrotic tissues with marked sequestration. The clinical diagnosis was a rare, adult type of extralobar pulmonary sequestration accompanied by chest pain.

ABSTRACT

From June 1990 to October 1998, 100 patients with adult-type atopic dermatitis (59 males and 41 females, 25±8 years) were admitted to our hospital to receive balneotherapy using Kusatsu hot-spring water. The atopic dermatitis in all but 9 cases occurred while the patients were still under 20 and had been refractory to various treatments including steroid ointment therapy over a long period of time. The patients took a 10-minute 40-42°C hot-spring bath followed by immediate application of white petrolatum 1-2 times daily for 75±46 days. The main components of the hot-spring water are aluminium, sulphates and chlorides, and its pH is 2.0. The skin symptoms of 79 of 100 cases (79%) were improved through the balneotherapy and furthermore pruritus was improved in 55 of the 79 cases (70%). The improvement of skin manifestations was supported by a significant decrease in serum LDH levels. In contrast, pruritus was not improved in the remaining 21 cases who showed no changes in skin symptoms and serum LDH levels. Moreover, changes in the number of <i>Staphylococcus aureus</i> on the skin surface were examined before and after balneotherapy. In the 69 cases examined whose skin symptoms were improved, many <i>Staphylococci aureus</i> were detected in 52 of the cases but not in the other 17 cases before starting balneotherapy. They disappeared in 24 cases and decreased in 18 cases of the 52 cases, but were not changed in the remaining 10 cases through the balneotherapy. On the other hand, the number of <i>Staphylococcus aureus</i> on the skin surface was not changed in 11 of the 14 cases examined whose skin symptoms were not improved. Our previous study reported that bactericidal activity against <i>Staphylococcus aureus</i> is expressed by the co-existence of manganese and iodide ions contained in the hot-spring water under an acidic (pH 2.0-3.0) condition. Thus, the mechanisms of the improvement of skin manifestations through the balneotherapy may be explained by considering bactericidal activity of Kusatsu hot-spring water against <i>Staphylococcus aureus</i> inducing acute flares of skin manifestations. Therefore, balneotherapy at Kusatsu can be useful for the treatment of refractory cases of adult-type atopic dermatitis as a suitable method of skin care.

ABSTRACT

Rhabdomyoma have been found in many areas of the body, though most commonly in the heart. The extracardiac appearance of rhabdomyoma is unusual and the head and neck area is involved most commonly. Affected sites have included the larynx, pharynx, oral cavity, neck, nasopharynx and orbit. Particularly, the larynx is an uncommon site. This neoplasms in the larynx have been reported only 23 cases in the literature so far and not reported in Korea. The diagnosis was based on routine histological and immunohistological staining, and electron microscopy. The extracardiac rhabdomyoma was divided into three types according to histopathological findings: adult, fetal cellular and fetal myxoid type. The differential diagnosis and requisite diagnostic procedure are discussed. The treatment of choice is surgical excision. Recently, we have experienced a rhabdomyoma of the larynx, so we report with a review of literatures.

ABSTRACT

Vitelliform macular dystrophy, adult type, is a type of pattern dystrophies of the pigment epithelium characterized by autosomal daminant inheritance, mid-life onset and small, round or oval, yellow deposits located at the level of the pigment epithelium. The authors report a case of this dystrophy which showed small yellow round lesion measuring 1/4-1/3 D.D. within the macula, normal electroretinogram, subnormal electrooculogram light-peak/dark-traugh ratio. typical irregular ring-like transmitted fluorescence surrounding the central non-fluorescent Iesion and leakage from perifoveal capillaries on fluorescent angiogram.

ABSTRACT

The authors studied 16 cases of craniopharyngioma who admitted to the Department of Neurosurgery of Dong San Medical center, Keimyung university, during the past 5 years from 1981 to 1985. Of these 16 cases, 9 belonged to a juvenile group with ages under 20 based on full pubertal development, and 7 cases belonged to an adult group. It was observed the difference between the age groups of the clinical, neuroradiogical and histopathopathological findings. Raised ICP symptoms were showed in juvenile group and decreased vision and visual field defect in adult group, respectivelly, as the chief complaint. All cases of the juvenile group were classified as juvenile type and three cases out of seven in the adult group were adult types in their histopathological pattern. Four juvenile types of adult group showed poor outcome. We concluded that subtotal resection with radiotherapy gave a better prognosis in both group and that hormone therapy before and operation decreased mortality and morbidity and improved the quality of survival significantly.
