ABSTRACT

Objective To explore the characteristics and significance of functional connectivity (FC) of affective network (AN) in patients with postpartum depression (PPD) under resting state. Methods A total of 23 patients with PPD (PPD group) and 28 healthy postpartum women (control group) were examined using resting-state fMRI. As two critical nodes of AN, amygdala (AMYG) and subgenual anterior cingulate cortex (sgACC) were selected as the regions of interest (ROI) to analyze the differences of functional connectivity strength (FCS) of two regions from other brain regions between two groups, followed by Pearson correlation analysis on the abnormal FCS and the Edinburgh postnatal depression scale (EPDS) score in PPD group. Results Compared to the control group, the patients in PPD group showed the extensively reduced FCS (P<0.05, Alphasim correction) between AMYG and frontal cortex, temporal cortex, hippocampus, cerebellum and orbitofrontal cortex, while there were enhanced FCS (P<0.05, Alphasim correction) between sgACC and parietal cortex, occipital cortex, thalamus, superior temporal gyrus and cingulate cortex. Moreover, in PPD group, the reduced FCS between left AMYG and left medial orbitofrontal cortex was negatively correlated with EPDS scores (r=-0.62, P=0.02). Conclusion Patients with PPD have dysfunctional connectivity of AN in multiple brain regions. The weaker FCS between left amygdala and left medial orbitofrontal cortex is, the more severe depression. The dysfunctional connectivity of AN may provide an effective mechanism-based biomarker underlying PPD.

ABSTRACT

Objective To explore abnormalities in functional connectivity of the affective network (AN) in relapse of major depressive disorder (MDD) after antidepressant treatment combined with resting state functional connectivity analysis.Methods Eleven recurrent MDD subjects after treatment,seventeen non recurrent MDD subjects after treatment and seventy-two healthy controls underwent fMRI scan.The amygdala,the pallidum,the insular cortex and the anterior cingulate cortex of the AN were selected as the template.Group independent component analysis (ICA) was performed to decompose the fMRI images into spatially independent components and the independent component which fit this template best was selected as AN.Two-sample t-tests were performed to investigate the changes in functional connectivity of the AN.Finally,the right amygdala and the medial prefrontal cortex were defined as seed regions.Results Compared with healthy control subjects and non-recurrent MDD group,recurrent MDD group showed significantly increased functional connectivity in the right amygdala in AN(P<0.001).Meanwhile,the functional connectivity between the right amygdala and the medial prefrontal cortex was significantly decreased in recurrent MDD group(P <0.05).Conclusion Abnormal resting-state functional connectivity of the right amygdala after antidepressant treatment in MDD was found,suggesting that altered amygdala functional connectivity may serve as a predicator of relapse of the MDD.

ABSTRACT

A presente investigação tem como objetivo principal, compreender o processo de superação de famílias envolvidas em episódios de violência intrafamiliar, contra seus filhos. Explora opiniões de autores acerca da importância da rede de apoio social e afetivo, na superação da violência, praticada por pais contra seus filhos. Apresenta resultados referentes a famílias residentes em Petrolina- PE, atendidas pelo Conselho Tutelar. Os dados foram coletados através de análise documental e estudo de caso. Os relatos das entrevistas foram tratados por análise de conteúdo de Bardin. Expressar como a violência ocorreu em cada família representou uma dificuldade para os participantes, existindo uma tendência dos autores em não admitir o fato. Encontrar uma motivação, que justificasse a prática de algum tipo de violência contra os filhos esteve presente nas falas dos entrevistados. Nas narrativas dos denunciantes, observou-se uma tendência em acusar o autor das agressões. A boa qualidade de atendimento do Conselho Tutelar foi referida pela maioria dos entrevistados, contudo, quanto aos serviços de apoio às famílias, percebeu-se uma desarticulação entre eles, refletindo na dificuldade de respostas às demandas geradas. A importância da participação da rede afetiva é mencionada como o principal mecanismo de proteção de que dispõem. Apontam como desafios na superação dos episódios vivenciados, as dificuldades de relacionamento entre autor e vitima e a falta de acompanhamento do Conselho Tutelar. Pretende-se contribuir para a efetivação dos direitos da criança e do adolescente, fornecendo elementos para a estruturação de redes de combate à violência intrafamiliar, e capacitação dos seus profissionais.
