ABSTRACT

Introducción. La kinesiología respiratoria cuenta con una amplia variedad de estrategias terapéuticas para el tratamiento de pacientes con disfunción respiratoria, entre las cuales se pueden mencionar las técnicas instrumentales. En la actualidad, existe una amplia variedad de ellas, la gran mayoría frecuentemente utilizadas en la práctica clínica. No obstante, la literatura que respalda su uso es heterogénea al igual que sus protocolos de aplicación. El objetivo de esta revisión es describir las técnicas kinesiológicas instrumentales más utilizadas en la práctica clínica teniendo como base una propuesta de clasificación. Se incluyen los siguientes dispositivos: Threshold PEP, Mascarilla PiPEP, TheraPEP, Flutter, Acapella, RC-Cornet, chaleco oscilatorio/compresivo torácico de alta frecuencia, ventilación percusiva intrapulmonar e incentivador volumétrico y flujométrico. Estas se describen de acuerdo a sus características principales, principios fisiológicos, protocolos de aplicación y evidencia disponible en la literatura.


Background. Respiratory physiotherapy has various therapeutic strategies for treating patients with respiratory dysfunction, including mechanical devices. Currently, a wide variety of these devices exist, and many are frequently used in clinical practice. However, the literature supporting their use is heterogeneous, as well as their application protocols. This paper aims to provide an overview of the most used devices in respiratory physiotherapy at clinical practice based on a proposed classification. The following devices were included: Threshold PEP, PiPEP mask, TheraPEP, Flutter, Acapella, RC-Cornet, High frequency chest wall compression/oscillation, Intrapulmonary Percussive Ventilation, flow and volume spirometer. They were described according to their main characteristics, physiological mechanisms, application protocols and evidence from literature.

Airway management is a crucial loop in the care and treatment of critically ill children.There are many kinds of airway clearance techniques, which can assist in the treatment of a variety of severe diseases in children, reduce airway obstruction caused by mucus deposition, and reduce the damage of airway and lung parenchyma caused by infection and inflammation.This review described the pathophysiological mechanism, drug application and non-drug application of airway clearance technology in different kinds of diseases.

ABSTRACT

Cystic fibrosis (CF) is a multisystem disease mainly caused by pathogenic mutation of the cystic fibrosis transmembrane conduction regulator gene.In recent years, with the deepened understanding of the disease and the popularization of gene detection technology, an increasing number of children are diagnosed with CF in China.Lung involvement is reported to affect the prognosis of the disease.Lung involvement is closely related to the airway, and good airway management can prolong the life of children.In this paper, the selection of airway clearance techniques and inhaled drugs were expounded, so as to improve the long-term airway management of children with CF in China.

Children with prolonged mechanical ventilation have complex conditions, high treatment cost of long-term stay in PICU, high nosocomial complications and mortality.Effective airway management and treatment of complications are essential to reduce ventilator-related pneumonia, prolong life, and reduce economic burden.This review aimed to summarize the updated studies to provide some evidences for children′s prolonged mechanical ventilation airway management and complications management.

@#Objective To observe the effect of abdominal electrical stimulation combined with high-frequency chest wall oscillation on airway clearance ability in critical ill patients with tracheostomy. Methods From January to June, 2021, a total of 84 critical ill patients with tracheostomy in the department of Critical Care Medicine, Zhongda Hospital, Southeast University, were randomly divided into control group (n = 28),experimental group A (n = 28) and experimental group B (n = 28). All the groups received routine therapy and early activities; while high-frequency chest wall oscillation was added to experimental group A, and abdominal electrical stimulation combined with high-frequency chest wall oscillation were added to experimental group B, for two weeks. Their involuntary cough peak flow (ICPF), Clinical Pulmonary Infection Score (CPIS), diaphragmatic excursion (DE), diaphragmatic thickness fraction (DTF) and thickness of abdominal muscle (Tab) were measured before and after treatment. Results The improvement of CPIS, ICPF and Tab were better in the experimental group B than in the other two groups (P < 0.05). The improvement of DE and DTF were slightly better in experimental group B, however, there was no significant difference among groups (FDE = 0.514, FDTF = 1.582, P > 0.05). The thickness d-values of rectus abdominis, musculi obliquus internus abdominis and musculus transversus abdominis were positively correlated with the d-value of ICPF in the exprimental group B (r > 0.415, P < 0.05). ICPF was highly negatively correlated with CPIS before treatment for all the patients (r = -0.702, P < 0.001). No adverse events occurred during the intervention period. Conclusion Abdominal electrical stimulation combined with high-frequency chest wall oscillation could improve airway clearance ability in critical ill patients with tracheostomy.

Background: cardiac surgery is associated with the occurrence of pulmonary complications, which may be defined as any pulmonary condition that occurring during the post-operative period which may produce dysfunction that is clinically significant and may adversely affects the clinical course. Objective: The objective of the study was to evaluate the effect of flutter device on airway mucus clearance, physiological parameters, and arterial blood gas analysis in patients with coronary artery bypass graft (CABG) in during Phase 1 cardiac rehabilitation. Materials and Methods: A total of 40 patients postoperative CABG surgery were randomly assigned to receive either experimental group along with flutter device and control group received conventional physiotherapy, outcome measure include arterial blood gas analysis, physiological parameters, and sputum amount. Result: This control trial of 5 days gave result that flutter device and conventional exercise have an effect on PCO2 and sputum index both experimental and control group while there was effect on PO2 in the investigational group and pulse rate in control group. Flutter device when analyzed from baseline to 5th post-operative day sputum score and PCO2 analysis. Other measures showed no significant differences. Conclusion: Flutter device should be incorporated as a routine practices along with other chest physiotherapy techniques during Phase 1 of cardiac rehabilitation, which can have positive results in the quality of life and improving overall functional status in subjects following CABG, thereby minimizing the hospital stay.

Resumen: La atrofia muscular espinal (AME) es la enfermedad genética mortal más frecuente en lactantes, con severidad variable. Se clasifica en cuatro subtipos: tipo 0 de inicio prenatal y recién nacido ya afecta do, con ausencia de esfuerzo respiratorio y ningún desarrollo motor, tipo 1 de inicio en menores de 3 meses que no logran sentarse, tipo 2 que logran sentarse, pero no caminar y tipo 3 que consiguen caminar. La causa más seria de morbimortalidad es la neumonía y la insuficiencia respiratoria. La información a los cuidadores debe contemplarse desde el diagnóstico, para la toma de decisiones anticipadas. Los objetivos del manejo incluyen el estímulo de la tos, evitar la deformación de la caja torácica, la hipoventilación, y tratar oportunamente las infecciones respiratorias, el trastorno de de glución, el reflujo gastroesofágico y la malnutrición. El objetivo de esta actualización es discutir los nuevos desafíos en cuidados respiratorios con un enfoque preventivo, considerando la reciente dis ponibilidad de tratamientos específicos -oligonucleótidos antisentido nusinersen- y otros que están en desarrollo, incluída la terapia génica.


Abstract: Spinal muscular atrophy (SMA) is the first inherited cause of mortality in infants, with four subtypes: SMA0 prenatal onset, SMA1 babies less than 3 months non sitters, SMA2 sitters and SMA3 walkers. Pneumonia and respiratory insufficiency are the most severe complications. Informed parental de cisions are relevant. Respiratory management includes cough assistance, prevention of lung under development due to chest deformity, prompt treatment of respiratory infections, hypoventilation, swallow problems, gastro esophageal reflux and malnutrition. In view of the FDA and EMA approval of the nonsense oligonucleotides nusinersen, the first specific treatment for SMA and the future with gene therapy and others under development, we need to optimize preventive respiratory manage ment with the new standard of care.

ABSTRACT

La kinesiterapia respiratoria, es una especialidad terapéutica que tiene el rol principal de prevenir y tratar las complicaciones pulmonares de forma sencilla, sin incorporar recursos sofisticados en su ejecución. Su objetivo es optimizar la función respiratoria para lograr un adecuado intercambio de gases y mejorar la relación ventilación perfusión. Para esto se efectuan maniobras de permeabilización bronquiales, que cobran especial valor en la infancia, dado que este grupo etáreo, presenta mayor producción de secreciones y una VA de conducción muy estrecha, ambos elementos predisponen a obstrucciones que deben ser manejadas cuando se amerite. La kinesiterapia, también incluye otras técnicas como ejercicios respiratorios, movilización, posicionamientos, maniobras de reexpansión pulmonar asociados con dispositivos especialmente adaptados para este objetivo. En este escrito, se dan las bases de esta terapia física, con énfasis en la edad pediátrica, junto a la descripción global y específica de las principales técnicas, que el pediatra debiese conocer.


Chest physioterapy is a therapeutic specialty that aims to prevent and treat lung complications without adding sophisticated resources for his execution as its main role. Its goal is to optimize the pulmonary function to achieve appropriate gaseous exchange and to improve ventilation/perfusion ratio. To this end airway clearance techniques are performed, these techniques are of special value during childhood, since this age group present higher quantities of secretions and very narrow conducting airways, both elements predispose to obstructions that must be handle if the case merits. Chest physioterapy also include others techniques as deep breathing, early movilizations, lung recruitment maneuvers with specially adapted devices. This paper set the stage of this pshysical therapy with emphasis in pediatric age, adding a global and specific description of the main techniques that the pediatritian should know.

ABSTRACT

Objective To explore the clinical effect of application of mechanical insufflation-exsufflation (MI-E) for airway mucus clearance in patients with artificial airway.Methods 72 patients were divided into the observation group (36 cases) and the control group (36 cases)according to admission order by random number table.All subjects were given with individual treatment and nursing,while the control group was given with the application of rock sputum ejection machine twice per day,the observation group was given with application of both cough assist and rock sputum ejection machine for sputum excretion.The clinical effect of airway mucus clearance after 5 days' treatment was compared between two groups.Results Compared with the control group,oxygenation index after 5 days' treatment in the observation group increased [(48.16±9.57) kPa vs.(37.99±14.67) kPa,t=-3.48,P<0.01],clinical pulmonary infection score [(3.39±0.77) scores vs.(5.33±1.47) scores] and procalcitonin level [0.10 (0.07,0.27) μg/L vs.0.43 (0.12,0.79) μg/L] were lower (t=-7.02,Z=-3.28,P<0.01),the mean sputum amount was bigger [(3.64±0.43) ml vs.(2.31 ±0.44) ml] (t=-4.84,P<0.01),and the mean interval of suction was longer [(97.81 ±7.28) min vs.(82.00±8.88) min](t=-3.08,P<0.05).Conclusions MI-E can clear the airway secretion effectively so as to improve oxygenation and the inflammatory markers.And MI-E is a simple,noninvasive and safe technique.

JUSTIFICATIVA E OBJETIVOS: A fisioterapia respiratória (FR) em pacientes submetidos a suporte ventilatório invasivo, atua diretamente no sistema ventilatório podendo alterar a mecânica pulmonar através da complacência pulmonar dinâmica (Cdyn) e da resistência do sistema respiratório (Rsr). Porém, as alterações descritas após a realização de FR permanecem controversas. O objetivo deste estudo foi avaliar as alterações da mecânica pulmonar em pacientes em ventilação mecânica invasiva (VMI). MÉTODO: Foi realizado estudo prospectivo e aleatório, controlado do tipo cruzado, incluindo pacientes com mais de 48 horas em VMI. Os protocolos de fisioterapia respiratória (PF) e de aspiração traqueal isolada (PA) foram aleatorizados para a ordem de aplicação, com intervalo de 24 horas entre eles. Dados da mecânica pulmonar e das variáveis cardiorrespiratórias foram coletados antes da aplicação do protocolo, imediatamente após; 30 minutos e 120 minutos após a aplicação dos protocolos. RESULTADOS: Doze pacientes completaram o estudo. A pneumonia foi a causa mais comum de insuficiência respiratória (IRpA). Não houve diferença estatisticamente significativa entre os grupos em relação à Cdyn, volume de ar corrente (VAC) e volume-minuto (VM). A Rsr diminuiu de forma significativa imediatamente após (de 10,4 ± 3 cmH2O/L/seg para 8,9 ± 2 cmH2O/L/seg; p < 0,02), 30 minutos após (de 10,4 ± 3 cmH2O/L/seg para 9 ± 2 cmH2O/L/seg; p < 0,01) e 120 min após (de 10,4 ± 3 cmH2O/L/seg para 9 ± 2 cmH2O/L/seg; p < 0,03) a aplicação do protocolo de fisioterapia respiratória. Quando comparado com o protocolo de aspiração traqueal isolada foi significativamente menor nos momentos 30 (9 ± 2 cmH2O/L/seg versus 10,2 ± 2 cmH2O/L/seg; p < 0,04) e 120 minutos (9 ± 2 cmH2O/L/segundo versus 10,4 ± 3 cmH2O/L/seg; p < 0,04). BACKGROUND AND OBJECTIVES: The chest physiotherapy (CP) in patients submitted to invasive support ventilation acts directly in the breathing system, and it could alter the lung mechanics through the dynamic lung compliance (DynC) and resistance of the breathing system (Rbs). However the alterations after the accomplishment of CP are still controversy. The objective of this study was to evaluate the alterations of the lung mechanics in patients in invasive mechanical ventilation (IMV). METHODS: It was a prospective, randomized, and controlled and crossover study, with patient with more than 48 hours in IMV. The protocol of chest physiotherapy and isolated tracheal aspiration they were randomized for the application order with a window of 24 hours among them. Data of lung mechanics and its varied cardiorespiratory were collected moments before the protocol, immediately after the application of the protocol, 30 minutes and 120 minutes after the application of the protocols. RESULTS: Twelve patients completed the study. Pneumonia was the mean cause respiratory failure (RF). There was not statistical difference among the groups in relation to Cdyn, volume tidal (Vt) and volume minute (Ve). Rbs decreased in a significant way immediately after (of 10.4 ± 3 cmH2O/L/seg for 8.9 ± 2 cmH2O/L/seg; p < 0.02), 30 minutes after (of 10.4 ± 3 cmH2O/L/seg for 9 ± 2 cmH2O/L/seg; p < 0.01) and 120 minutes after (of 10.4 ± 3 cmH2O/L/seg for 9 ± 2 cmH2O/L/seg; p < 0.03) application the protocol of chest physiotherapy. When compared with the protocol of isolated tracheal aspiration it was significantly smaller in the 30 (9 ± 2 cmH2O/L/seg versus10.2 ± 2 cmH2O/L/seg; p < 0.04) and 120 minutes (9 ± 2 cmH2O/L/seg versus 10.4 ± 3 cmH2O/L/seg; p < 0.04). CONCLUSIONS: The protocol of chest physiotherapy was effective in the decrease of Rsr when compared with the aspiration protocol. ABSTRACT

Trata-se de estudo descritivo, cujo objetivo foi analisar a aplicabilidade de intervenções da NIC no atendimento a crianças com o diagnóstico de enfermagem "desobstrução ineficaz de vias aéreas relacionada à presença de via aérea artificial" em um centro de terapia intensiva pediátrico (CTIP). Foi desenvolvido em três etapas. Na primeira, foram levantadas, junto às enfermeiras do CTIP, as atividades que precrevem cotidianamente para crianças com esse diagnóstico de enfermagem. Foi realizado mapeamento comparativo dessas atividades, com 17 intervenções apresentadas na NIC para o diagnóstico de enfermagem "desobstrução ineficaz de vias aéreas". Na segunda etapa, esse mapeamento foi submetido à revisão por enfermeiras peritas. Na terceira etapa, 08 dessas intervenções foram selecionadas para análise, pelas enfermeiras, quanto à aplicabilidade, no CTIP, das atividades que não foram por elas mencionadas como prescritas cotidianamente na primeira etapa. Como resultados, obteve-se um total de 49 atividades listadas pelas enfermeiras como prescritas cotidianamente no CTIP para crianças com o diagnóstico de enfermagem "desobstrução ineficaz de vias aéreas relacionada à presença de via aérea artificial". Por meio do mapeamento comparativo verificou-se que, 74 das 403 atividades constantes das intervenções prioritárias e sugeridas da NIC possuíam correspondência com 39 atividades prescritas pelas enfermeiras. Na terceira etapa, as 08 intervenções selecionadas para análise pelas enfermeiras somaram 165 (100%) atividades. Destas, 45 (27,3%) haviam sido mapeadas como correspondentes às indicadas pelas enfermeiras na primeira etapa. Das 120 (100%) atividades restantes que foram analisadas quanto à sua realização ou não, 83 (69,2%) foram consideradas pela maioria das enfermeiras como realizadas, 36 (30,0%) como não realizadas e 1 (0,8%) foi considerada pela metade das enfermeiras como realizadas e pela outra metade como não ) realizada. Somando-se as atividades correspondentes na primeira etapa e as atividades consideradas realizadas na terceira eta, se obteve um total de 128 (77,6%) atividades consideradas pela maioria das enfermeiras como realizadas. Quanto ao percentual atribuído à realização das atividades por intervenção, obtivemos os seguintes resultados: "monitorização respiratória" (84,6%), "aspiração de vias aéreas" (84,0%); "controle de vias aéreas" (80,0%); "inserção e estabilização de vias aéreas artificiais" (80,0%); "controle de vias aéreas artificiais" (80,0%); "fisioterapia respiratória" (76,9%); "assistência ventilatória" (75,0%) e "incremento da tosse" (36,4%). Concluímos que, houve um número menor de atividades consideradas não realizadas no CTIP em relação à intervenção incremento da tosse, porém, a maioria das atividades constantes na NIC e submetidas à análise pelas enfermeiras possui aplicabilidade no CTIP


This descriptive study aimed at analyzing the applicability of NIC interventions in the care to children with the nursing diagnosis "ineffective airway clearance related to artificial airway" at a Pediatric Intensive Care Unit (PICU). The study was developed in three phases. In the first one, the author found out, with nurses from PICU, the activities that are daily prescribed to children with this diagnosis. Based on this information, the activities were mapped and compared with the 17 interventions presented by NIC related to the nursing diagnosis "ineffective airway clearance". In the second phase, this mapping was submitted to a peers review. In the third phase, 8 interventions were selected for analysis by the nurses regarding their applicability at the PICU, among the activities that were not mentioned as daily precribed in the first phase. Results showed a total of 49 activities mentioned by nurses as daily prescribed at the PICU for children with the nursing diagnosis "ineffective airway clearance related to artificial airway". Through comparative mapping, the author verified that 74 among the 403 activities related in the priority interventions and suggested by NIC had correspondence with 39 activities prescribed by nurses. In the third phase, the 08 interventions that were selected for analysis by nurses resulted in 165 (100.0%) activities. Among them, 45 (27.3%) were mapped with the correspondents indicated by nurses in the first phase. Among the 120 (100%) other activities that were analyzed regarding its performance or not, 83 (69.2%) were considered by the majority of nurses as performed, 36 (30.0%) as not performed and 1 (0.8%) was considered by half of the nurse as performed and by the other half as not performed. Summing up the activities of the first and third phases, the author obtained a total of 128 (77.6%) activities considered by the majority of nurses as performed. With respect to the percentage regarding the performance of these activities, it is important to mention the following results: Respiratory Monitoring (84.6%); Airway Suctioning (84.0%); Airway Management (80.0%); Airway Insertion and Stabilization (80.0%); Artificial Airway Management (80.0%); Chest Physiotherapy (76.9%); Ventilation Assistance (75.0%); Cough Enhancement (36.4%). The author concluded that there was few activities considered as not performed at PICU related to the intervention Cough Enhancement, however, the majority of NIC activities that were submitted to nurses´analysis are applicable at PICU
