ABSTRACT

MethodIn the experiment, 46% vol Red Star Erguotou (10 mL·kg·d-1) was used to establish the AONFH rat model, and the intervention effect of JPHGP at different doses (2.5, 5.0, 10.0 g·kg-1) was observed. Jiangusheng pill (JGS, 1.53 g·kg-1) was selected as the positive control. After 8 weeks of administration, the bone histomorphometry of the femoral head was analyzed by Micro-CT imaging, and the area of medullary microvessels in the femoral head was detected by ink perfusion. The pathological change was observed by hematoxylin and eosin (HE) staining. The protein expressions of Platelet endothelial cell adhesion molecule-1 (CD31), VEGF, VEGFR2, PI3K, phosphor-Akt (p-Akt) and phosphatase and Tensin homologue deleted on chromosome 10 (PTEN) in the femoral head were determined by immunohistochemistry and Western blot. ResultCompared with normal group, the model group presented the fracture and thinning of trabeculae in the femoral head, increased empty bone lacunae, and elevated number and diameter of adipocytes (P<0.01). Micro-CT imaging revealed a decrease in bone mineral density (BMD), bone volume fraction (BV/TV), trabecular thickness (Tb.Th) and trabecular number (Tb.N) (P<0.05, P<0.01) while an increase in bone surface-to-volume ratio (BS/BV) and trabecular separation (Tb.Sp) (P<0.01). The results of ink perfusion showed that the area of medullary microvessels in the femoral head was reduced (P<0.01). Compared with model group, JPHGP lowered the empty bone lacunae rate as well as the number and diameter of adipocytes in the femoral head of AONFH rats. Micro-CT imaging indicated that JPHGP low-dose group had elevated BV/TV, Tb.Th and Tb.N (P<0.05, P<0.01) while decreased BS/BV (P<0.01), and there was an upward trend in BMD while a downward trend in Tb.Sp, but without statistical difference. In addition, JPHGP medium- and high-dose groups had a rise in BMD, BV/TV, Tb.Th and Tb.N (P<0.05, P<0.01), a decrease in BS/BV and Tb.Sp (P<0.05, P<0.01) and enlarged area of medullary microvessels in the femoral head (P<0.05, P<0.01). The expressions of CD31, VEGF, VEGFR2, PI3K, p-Akt in the model group were lower than those in the normal group (P<0.01), and after medium and high doses of JPHGP treatment, the expressions of CD31, PI3K and p-Akt in the femoral head of rats were up-regulated (P<0.01) while the protein expression of PTEN was down-regulated (P<0.01). Moreover, JPHGP up-regulated the expressions of VEGF and VEGFR2 (P<0.05, P<0.01). ConclusionJPHGP can repair the vascular injury in AONFH, and its mechanism may be related to the activation of VEGF/VEGFR2/PI3K/Akt signaling pathway. This study provides certain scientific basis and reference for the clinical application of JPHGP. ObjecctiveTo observe the repair effect of Jianpi Huogu prescription (JPHGP) on vascular injury in experimental alcohol-induced osteonecrosis of femoral head (AONFH), and to explore its mechanism based on vascular endothelial growth factor (VEGF)/VEGFR2/phosphoinositide 3-kinase (PI3K)/protein kinase B (Akt) signaling pathway.

ABSTRACT

ObjectiveTo investigate the protective effect of Geju Hugan tablets on the liver of mice with alcohol-induced liver injury, and explore the underlying mechanism based on nuclear factor-κB p65 (NF-κB p65) and B-cell lymphoma-2 (Bcl-2)/Bcl-2-associated X protein (Bax) signaling pathways. MethodAccording to the body weight, 60 SPF-grade male ICR mice were randomized into normal, model, Compound Yiganling tablets (0.16 g·kg-1), and low-, medium-, and high-dose (0.2, 0.4, 0.8 g·kg-1, respectively) Geju Hugan tablets groups. The drugs were administrated at the corresponding doses by gavage, and the normal and model groups with equal volume of pure water once a day for 28 consecutive days. On day 29, the mice in other groups except the normal group were administrated with liquor (53% Vol) by gavage twice a day at the doses of 20, 10 mL·kg-1 and with the interval of 6 h. Samples were harvested on day 30. The histopathological changes in the liver were observed by hematoxylin-eosin (HE) staining, and the ultrastructural changes in hepatocytes were observed by transmission electron microscopy. The enzyme-linked immunosorbent assay was employed to measure the levels of malonaldehyde (MDA), reduced glutathione (GSH), and triglycerides (TG) in the liver tissue and the levels of alanine aminotransferase (ALT) and aspartate aminotransferase (AST) in the serum. Western blotting was employed to determine the protein levels of NF-κB p65, phosphorylated p-inhibitor kappa B alpha (p-IκBα), Bcl-2, and Bax in the liver tissue. ResultCompared with the normal group, the model group showed increases in the ALT, AST, MDA, and TG levels, a decrease in the GSH level, and increases in the liver injury scores evaluated based on the HE, oil red O, and transmission electron microscopy (P<0.01). Moreover, the model group showed up-regulated expression of NF-κB, p-IκBα, and Bax (P<0.05, P<0.01) and down-regulated expression of Bcl-2 (P<0.05) in the liver tissue. Compared with the model group, Geju Hugan tablets of all the doses lowered the ALT, AST, MDA, and TG levels and elevated the GSH level (P<0.01). The liver injury scores assessed based on HE staining and transmission electron microscopy in the medium- and high-dose Geju Hugan tablets groups were lower than those in the model group (P<0.01). Compared with the model group, medium- and high-dose Geju Hugan tablets down-regulated the protein levels of NF-κB, p-IκBα, and Bax (P<0.01) and all doses of Geju Hugan tablets up-regulated the protein level of Bcl-2 (P<0.01). ConclusionGeju Hugan tablets protect mice from alcohol-induced liver injury by down-regulating NF-κB signaling pathway to alleviate inflammation in the liver tissue and down-regulating the expression of Bax and up-regulating the expression of Bcl-2 to inhibit hepatocyte apoptosis.

ABSTRACT

RESUMEN La enfermedad hepática alcohólica tiene un amplio espectro de enfermedades, incluida la hepatitis alcohólica, que en sus formas graves puede conducir al síndrome hepatorrenal. La anemia es común en pacientes alcohólicos, pero una anemia hemolítica asociada con hiperlipidemia e ictericia se reconoce como síndrome de Zieve. Un varón de 42 años con consumo excesivo de alcohol fue admitido por ictericia y dolor abdominal. Durante su evolución presentó azoemia progresiva y anemia hemolítica. Se realizó el diagnóstico de síndrome hepatorrenal asociado a hepatitis alcohólica, así como un síndrome de Zieve. Fue tratado con corticoterapia y la combinación de albúmina y noradrenalina, además del retiro de alcohol, con resultados favorables.


ABSTRACT Alcoholic liver disease has a broad spectrum of diseases, including alcoholic hepatitis, which in its severe forms can lead to hepatorenal syndrome. Anemia is common in alcoholic patients, but a hemolytic anemia in association with hyperlipidemia and jaundice is recognized as Zieve's syndrome. A 42 year old man with heavy alcohol consumption was admitted for jaundice and abdominal pain. During his evolution, he presented progressive azotemia and hemolytic anemia. The diagnosis of hepatorenal syndrome associated with alcoholic hepatitis was made, as well as a Zieve's syndrome. He was treated with corticosteroid therapy and the combination of albumin and norepinephrine, in addition to alcohol withdrawal, with favorable results.

ABSTRACT

ABSTRACT BACKGROUND: It has been estimated that 17% of individuals aged 50 years or older suffer from addiction to legal or illegal drugs. Use of alcohol and psychoactive substances has been correlated with several diseases, e.g. psychiatric conditions and cardiovascular and sexual dysfunctions. Objective: To discuss the Brazilian profile of mental and behavioral disorders caused by use of alcohol and psychoactive substances among older adults and elderly people, over the period from 2008 to 2019. DESIGN AND SETTING: Cross-sectional study conducted among Brazilians aged 50 years or older. METHODS: Hospitalization due to mental and behavioral disorders caused by use of alcohol and psychoactive substances was assessed through data obtained from the National Health System Department of Informatics (Departamento de Informática do Sistema Único de Saúde, DATASUS). RESULTS: Decreasing and steady trends of hospitalization due to mental and behavioral disorders caused by use of alcohol among both men and women at all ages were observed. Similar trends were reported for all age ranges among men and women aged 60 years and older. In contrast, a slight increase was seen among women aged 50 to 59 years. CONCLUSION: These data are crucial for qualifying mental healthcare for older adults and elderly people and for planning mental health services.

ABSTRACT

Objective:To investigate the clinical efficacy of risperidone combined with diazepam in the treatment of alcohol-induced mental and behavioral disorders.Methods:Sixty patients with alcohol-induced mental and behavioral disorders admitted to Huainan Psychiatric Hospital from January 2018 to January 2022 were included in this study. They were randomly assigned to undergo either diazepam alone (control group, n = 30) or risperidone combined with diazepam (study group, n = 30). Clinical efficacy, Positive and Negative Syndrome Scale (PANSS) score, incidence of adverse reactions, quality of life, treatment satisfaction were compared between the two groups. Results:Total response rate differed significantly between the two groups ( Z = 6.25, P < 0.05). In the control group, PANSS scores measured before and after treatment were as follows: positive symptoms: (26.07 ± 3.70) points vs. (16.38 ± 2.04) points; negative symptoms: (24.04 ± 2.98) points vs. (13.86 ± 1.84) points; psychopathological symptoms: (44.06 ± 5.28) points vs. (31.83 ± 4.04) points; general score: (91.84 ± 9.76) points vs. (40.84 ± 6.26) points. In the study group, PANSS scores measured before and after treatment were as follows: positive symptoms: (25.84 ± 3.82) points vs. (10.30 ± 1.17) points; negative symptoms: (24.48 ± 3.26) points vs. (7.48 ± 0.82) points; psychopathological symptoms: (43.28 ± 5.21) points vs. (21.06 ± 3.72) points; general score: (92.06 ± 9.85) points vs. (68.27 ± 7.02) points. There were no significant differences in above indices measured before treatment between the two groups (all P > 0.05). After treatment, scores of positive, negative, psychopathological symptoms and general scores in the study group were significantly lower than those in the control group ( t = 14.16, 17.34, 10.74, 15.97, all P < 0.001). There was no significant difference in the incidence of adverse reactions between the two groups ( P > 0.05). Before and after treatment, the quality of life scores (physiological, psychological, social and environmental scores) of the control group were (12.64 ± 2.50) points vs. (13.87 ± 2.16) points; (12.47 ± 2.12) points vs. (13.28 ± 2.18) points; (9.54 ± 2.16) points vs. (14.20 ± 1.27) points; (10.97 ± 1.84) points vs. (12.78 ± 2.42) points. Before and after treatment, the quality of life scores (physiological, psychological, social and environmental scores) of the study group were (12.68 ± 2.53) points vs. (15.37 ± 2.07) points; (12.49 ± 2.14) points vs. (14.90 ± 2.20) points; (9.37 ± 2.14) points vs. (15.03 ± 1.27) points; (10.94 ± 1.81) points vs. (13.86 ± 2.18) points. After treatment, physiological, psychological, social and environmental scores in the study group were significantly higher than those in the control group ( t = 2.74, 2.86, 2.50, 2.81, all P < 0.001). There was significant difference in treatment satisfaction between the two groups ( χ2 =5.19, P = 0.022). Conclusion:Risperidone combined with diazepam can help improve the clinical symptoms of patients with alcohol-caused mental and behavioral disorders, further improves the treatment effect and quality of life, and is safe.

ABSTRACT

ObjectiveTo explore the pharmacodynamic effect of the water extract of Citri Grandis exocarpium (WEC) on mice with alcohol-induced acute liver injury and provide data support for the development of this medicinal for anti-alcoholism and liver protection. MethodThe main components of WEC were determined by high performance liquid chromatography (HPLC). Sixty Balb/c mice were randomized into 6 groups: control group (equal volume of 0.5% carboxymethyl cellulose sodium solution), model group (equal volume of 0.5% carboxymethyl cellulose sodium solution), low-, medium-, and high-dose WEC groups (0.5, 1.0, 2.0 g·kg-1), and Haiwang Jinzun tablet positive control group (2.0 g·kg-1). The administration lasted 14 days. One day before the end of the administration, mice were fasted for 12 h with free access to water. The mice, except the control group, were given 56° Chinese liquor (13 mL·kg-1). After 2 h, blood was taken from eyeballs and the liver was dissected and weighed. Automatic biochemical analyzer was employed to detect the expression of alanine aminotransferase (ALT), aspartate aminotransferase (AST), and alcohol dehydrogenase (ADH). The pathological changes of liver tissues were observed based on hematoxylin-eosin (HE) staining, and apoptosis of hepatocytes based on TUNEL/DAB staining. The expression of proteins related to apoptosis was detected by Western blot. ResultAccording to the HPLC fingerprint, the main components of WEC were rhoifolin and naringin. Compared with the control group, the model group showed increase in liver/body weight ratio (P<0.01) and the expression of ALT and AST (P<0.05, P<0.01), decrease in the expression of ADH (P<0.05), blurred structure of hepatic lobules, pathological changes of liver tissue, loose cytoplasm with edema, severe steatosis, rise of the TUNEL-positive rate (P<0.01), reduction in expression of Bcl-2 (P<0.01), and increase in Bax and Caspase-3 (P<0.01). Compared with the model group, medium-dose WEC lowered liver/body weight ratio (P<0.05). All doses of WEC depressed the activity of ALT and AST (P<0.05, P<0.01), up-regulated the expression of ADH (P<0.05), significantly improved the pathological features of alcohol-induced cytoplasmic porosity, edema, and steatosis, down-regulated the TUNEL-positive rate (P<0.05, P<0.01), enhanced the expression of Bcl-2 (P<0.05), and decreased Bax and Caspase-3 (P<0.01). ConclusionWEC regulates the expression of ALT, AST, and ADH and improves hepatic steatosis and hepatocyte apoptosis to fight against acute liver injury.

Resumen Objetivo: Analizar la diferencia en la pérdida auditiva según el consumo de alcohol, en mujeres de entre 20 y 40 años de edad, en la comuna de Temuco, Chile. Metodología: Estudio de corte, donde se evaluaron 30 mujeres consumidoras y no consumidoras de alcohol, divididas en 3 grupos (G): G1 abstemias, G2 consumidoras de 40-59 y G3 consumidoras demás de 60g de alcohol diarios (15, 6 y 9 respectivamente), con un muestreo intencional, contactadas en centros comerciales. Se realizaron pruebas auditivas: audiometría, discriminación de la palabra y potenciales evocados auditivos de tronco cerebral. Resultados: El oído derecho presentó diferencias significativas en las frecuencias medias y discriminación de la palabra en G1 vs. G2 (p = 0,045 y 0,010, respectivamente). El oído izquierdo mostró diferencias significativas en las frecuencias agudas en G1 vs. G3, en las latencias interpeakde las ondas iii-v (tiempo en milisegundo transcurrido entre la onda iii y v posterior al estímulo) en G1 vs. G2 (p = 0,003) y G2 vs. G3 (p = 0,005) a 80 dB; en las latencias interpeaki-iiiy iii-v en G1 vs. G2 (p = 0,039) a 60 dB y en la discriminación de la palabra en G1 vs. G2 (p = 0,009) y G1 vs. G3 (p = 0,013). Conclusiones: En el grupo estudiado existe pérdida auditiva relacionada con un consumo diario a partir de los 40g de alcohol, siendo el oído más afectado el izquierdo, en las frecuencias medias y agudas, en la cóclea y el tronco cerebral.


Abstract Objective: To analyze the relationship between alcohol consumption and loss hearing in women between 20 and 40 years of age in Temuco city. Methodology: Cutting study. We evaluated 30 consuming and non alcoholic women divided into 3 groups (G): G1 abstemies, G2 consumers of 40-59 and G3 consumers of> 60 grams alcohol daily (15, 6 and 9 respectively), intentional sampling contacted in shopping centers. Auditory tests were performed: Audiometry, Discrimination of the Word and Auditory Evoked Potentials of the Brain Stem. Results: In the right ear, there were significant differences in medium frequencies and word discrimination between groups 1 versus 2 (p = 0.045 and 0.010, respectively). In left ear, there were significant differences in the high frequencies between groups 1 versus 3, interpeak latencies of iii-v waves (time elapsed between iii and v waves in millisecond after the stimulus) between groups 1 versus 2 (p = 0.003) and groups 2 versus 3 (p = 0.005) to 80 dB; in interpeak latencies i-iii and iii-vbetween groups 1 versus 2 (p = 0.039) at 60 dB and in word discrimination between groups 1 versus 2 (p = 0.009) and 1 versus 3 (p = 0.013). Conclusion: In the group studied there is hearing loss related to a daily consumption from the 40 grams of alcohol being the most affected ear the left in the medium and high frequencies and affecting to the cochlea and also brainstem.


Resumo Objetivo: O objetivo foi analisar a relação entre consumo de álcool e perda auditiva em mulheres entre 20 e 40 anos. Metodologia: Foi realizado um estudo transversal não experimental em que 30 consumidoras femininas e não o álcool foram divididas em 3 grupos: abstêmios G1, consumidores G2 de 40-59 gramas e consumidores G3> 60 gramas de álcool por dia. Testes auditivos foram realizados: audiometria, discriminação de palavras e potencial evocado auditivo de tronco encefálico. O teste não paramétrico de Mann-Whitney U foi utilizado para a análise estatística. Resultados: Na orelha direita, há diferenças significativas nas frequências médias e discriminação de palavras entre os grupos 1 e 2 (p = 0,045 e 0,010, respectivamente). Na orelha esquerda, há diferenças significativas nas freqüências agudas entre os grupos 1 versus 3, nas latências interpicos das ondas iii-v entre os grupos 1 versus 2 (p = 0,003) e os grupos 2 versus 3 (p = 0,005) a 80dB; nas latências interpicos i-iii e iii-v entre os grupos 1 versus 2 (p = 0,039) a 60 dB e na discriminação de palavras entre os grupos 1 versus 2 (p = 0,009) e 1 versus 3 (p = 0,013). Conclusões: Há uma perda auditiva relacionada ao consumo diário de 40 gramas de álcool, sendo que a orelha mais afetou a esquerda nas freqüências média e aguda, afetando a cóclea e o tronco cerebral.

ABSTRACT

OBJETIVO: identificar a prevalência do uso em binge em indígenas Karipuna e verificar a associação desse uso com variáveis sociodemográficas, clínicas e comportamentais da amostra. MÉTODO: trata-se de um estudo transversal realizado com 230 indígenas de 12 aldeias Karipuna em Oiapoque. Obteve-se o rastreio do uso em binge por meio da Questão-Chave. Coletaram-se os dados entre maio e dezembro de 2017. Realizaram-se, a priori, a análise de frequência das variáveis envolvidas no estudo e, na sequência, o teste qui-quadrado e o modelo de regressão logística. RESULTADOS: revela-se que a prevalência do uso em binge foi de 24,8% de uma a três vezes; de 20,4% de quatro a seis vezes; de 12,2% de sete a dez vezes e de 9,6% em mais de dez vezes. Associaram-se os seguintes fatores: estudante (OR=2,99); migração da aldeia de origem (OR=2,22); uso de preservativo (OR=2,62) e relações sexuais após o consumo de álcool (OR=1,61). CONCLUSÃO: alerta-se que o uso ocasional de risco de álcool demanda consideração, bem como o conhecimento das particularidades da população ora investigada, a fim de estabelecer controle, planejamento de recursos terapêuticos para que se alcancem resultados efetivos nas ações planejadas e principalmente nas que são adotadas na prática a fim de prevenir um uso abusivo de álcool.


OBJECTIVE: to identify the prevalence of use in binge in indigenous Karipuna and to verify the association of this use with sociodemographic, clinical and behavioral variables of the sample. METHOD: this is a cross-sectional study carried out with 230 indigenous people from 12 Karipuna villages in Oiapoque. Binge use screening was obtained through the Key Question. Data was collected between May and December 2017. A priori, the frequency analysis of the variables involved in the study was carried out, following the chi-square test and logistic regression model. RESULTS: it is revealed that the prevalence of use in binge was 24.8% from one to three times; 20.4% four to six times; 12.2% seven to ten times and 9.6% more than ten times. The following factors were associated: student (OR = 2.99); migration from the village of origin (OR = 2.22); condom use (OR = 2.62) and sexual intercourse after alcohol consumption (OR = 1.61). CONCLUSION: it is warned that the occasional use of alcohol risk demands consideration, as well as knowledge of the particularities of the population now investigated, in order to establish control, planning of therapeutic resources so that effective results are achieved in the planned actions and especially in those that are adopted in practice in order to prevent alcohol abuse.


OBJETIVO: identificar la prevalencia del uso de binge en indígenas Karipuna y verificar la asociación de este uso con las variables sociodemográficas, clínicas y comportamentales de la muestra. MÉTODO: se trata de un estudio transversal realizado con 230 indígenas de 12 aldeas Karipuna en Oiapoque. Se obtuvo el rastreo del uso de binge por medio de la pregunta clave. Se recogieron los datos entre los meses de mayo y diciembre de 2017. Se realizaron, a priori, el análisis de la frecuencia de las variables del estudio y a continuación se realizó el test chi-cuadrado y el modelo de regresión logístico. RESULTADOS: se revela que la prevalencia del uso de binge fue de 24,8% de una a tres veces; de 20,4% de cuatro a seis veces; de 12,2% de siete a diez veces y de 9,6% en más de diez veces. Se asociaron los siguientes factores: estudiante (OR=2,99); migración de la aldea de origen (OR=2,22); uso de preservativo (OR=2,62) y relaciones sexuales después de consumo de alcohol (OR=1,61). CONCLUSIÓN: se advierte que el uso ocasional de riesgo de alcohol demanda atención, así como tomar conocimiento de las particularidades de la población que está siendo investigada, con la finalidad de establecer control y planificar recursos terapéuticos, para que sean alcanzados resultados efectivos, en las acciones planificadas y, principalmente, en aquellas que son adoptadas en la práctica, con la finalidad de prevenir el uso abusivo del alcohol.

ABSTRACT

BACKGROUND: The pathological changes of alcohol-induced osteonecrosis of the femoral head (ONFH) are the result of a combination of various factors, and its specific pathogenesis is inconclusive. Related studies have shown abnormal bone metabolism in patients with avascular necrosis of the femoral head. OBJECTIVE: To explore the bone turnover markers in patients with alcohol-induced ONFH and to explore the relationship between different stages of ONFH and bone metabolism. METHODS: This study retrospectively selected 193 male patients with alcohol-induced ONFH (necrosis group), including 35 cases of ARCO stage II, 131 cases of ARCO stage III and 27 cases of ARCO stage IV. Among them, there were 65 cases of a little drinking of alcohol 52 cases of moderate drinking, and 76 cases of heavy drinking. Another 182 healthy males undergoing physical examination with no history of drinking were taken as control group. The bone turnover markers [procollagen type 1 N-terminal propeptide (P1NP), C-terminal cross-linked telopeptides of type 1 collagen (β-CTX), molecular fragment of N-terminal osteocalcin (N-MID) and 25 hydroxyvitamin D (25OHD)] and biochemical indexes were tested and compared between two groups, followed by logistic regression analysis and correlation analysis. The study protocol was performed in line with the relevant ethic requirements of the First Affiliated Hospital of Guangzhou University of Chinese Medicine. All participants in the trial had been fully informed of the trial process. RESULTS AND CONCLUSION: In the necrosis group, P1NP, β-CTX, N-MID, 25OHD, serum calcium, uric acid, alkaline phosphatase, serum phosphorus levels were significantly higher than that of the control group (P < 0.05), and apolipoprotein A1 and high-density lipoprotein levels in the necrosis group were significantly lower than those in the control group (P < 0.05). Compared with patients with low level of alcohol, β-CTX and N-MID levels were significantly reduced in the patients with heavy drinking (P < 0.05). The P1NP level of patients with ARCO stage IV was significantly higher than that of patients with ARCO stage II and III (P < 0.05), and the β-CTX level of patients with ARCO stage IV was significantly higher than that of patients with ARCO stage III (P < 0.05). The correlation analysis results showed that alcohol intake levels were negatively correlated with β-CTX, alkaline phosphatase level was positively correlated with P1NP and β-CTX, and 25OHD was negatively correlated with low-density lipoprotein. Logistic regression analysis results showed that: P1NP (odds ratio=0.984, P=0.004), β-CTX (odds ratio=0.325, P=0.043), and high-density lipoprotein (odds ratio=2.622, P=0.014). To conclude, male patients with alcohol-induced ONFH have active bone formation and bone resorption, and obvious abnormalities in lipid metabolism. The progression of alcohol-induced ONFH can be predicted by these bone turnover markers.

ABSTRACT

Objective To investigate the effects of heat shock protein Gp96 on alcoholic liver fibrosis in mice. Methods A total of 220 male healthy C57BL/6 J mice were randomly divided into four groups; normal control group (n = 10), saline+alcohol induced liver fibrosis group (n = 70), the injection of CRISPR expression Gp96-sgRNA3 by tail vein+ alcohol induced liver fibrosis group (n = 70), the intraperitoneal injection of nuclear factor kappa B(NF-κB) inhibitors PDTC+alcohol induced liver fibrosis group (n = 70). The blood was got from eyeballs and the mice were killed after 8 weeks of ethanol induction. We detected the activity of serum aspartate aminotransferase (AST) in mice of different groups. The pathological changes were detected by HE staining, sirius red staining and periodic acid-Schiff (PAS) staining in the liver of mice. The expression of Gp96 and transforming growth factor βl ( TGF-βl ) were detected by Western blotting. Results Compared with the normal control group, the AST enzyme activity and liver fibrosis increased significantly, glycogen decreased significantly in other three groups (P<0.01). Compared with the saline+alcohol group, the AST enzyme activity and liver fibrosis increased more significantly, glycogen decreased more significantly, Gp96 expression decreased significantly and TGF-βl expression increased significantly in Gp96-sgRNA3+ alcohol group and NF-κB inhibitors PDTC+ alcohol group (P<0.01 or P<0.05). Conclusion The injection of CRISPR expression plasmid Gp96-sgRNA3 by tail vein significantly inhibited the Gp96 expression, promoted the degree of alcoholic liver fibrosis in mice, and NF-κB signaling pathway played a certain role in regulating the expression of Gp96.

RESUMO Objetivo avaliar as atitudes dos profissionais de Centros de Atenção Psicossocial frente ao álcool, alcoolismo e alcoolista. Métodos estudo transversal, avaliativo, com 288 profissionais de 12 serviços de saúde. Coletaram-se dados sociodemográficos, Escala de Satisfação dos Pacientes com os Serviços de Saúde Mental e Escala de Atitude para álcool, alcoolismo e alcoolistas. Resultados os profissionais que demonstraram postura mais crítica em relação ao seu cotidiano de trabalho e os que atuavam nos serviços por mais tempo apresentaram atitudes positivas em relação ao álcool, alcoolismo e alcoolistas. Profissionais da equipe administrativa e técnicos de saúde apresentaram atitudes mais negativas. Conclusão as atitudes dos profissionais ao álcool, alcoolismo e alcoolista, no geral, são positivas e associaram-se ao maior tempo de atuação na área e à expressão de incômodos com o trabalho.


ABSTRACT Objective to assess the attitudes of professionals from Psychosocial Care Centers towards alcohol, alcoholism, and alcoholics. Methods a cross-sectional evaluation study with 288 professionals from 12 healthcare services. Sociodemographic data, Patient Satisfaction Scale with Mental Health Services and Attitude Scale for alcohol, alcoholism, and alcoholics were collected. Results the professionals who showed a more critical attitude towards their work routine and those who worked in the healthcare services for longer had positive attitudes towards alcohol, alcoholism, and alcoholics. Professionals from the administrative team and health technicians had more negative attitudes. Conclusion the attitudes of professionals towards alcohol, alcoholism, and alcoholics, in general, are positive and were associated with longer working time in the field and the manifestation of disapproving situations with work.

RESUMO Objetivo verificar o consumo de bebidas alcóolicas e a prática do binge drinking entre os cabeleireiros. Métodos estudo transversal realizado com 51 profissionais de salões de beleza. Utilizou-se de um questionário com características sociodemográficas e sobre as práticas do consumo de bebidas alcoólicas. Para a identificação do uso em binge drinking, pautou-se a questão-chave. Realizou-se a análise estatística descritiva e inferencial. Resultados 84,3% eram consumidores de álcool, 51,0% tinham de um a dez anos de consumo e 72,5% consumiam a cerveja. Em relação ao uso em binge, 37,3% da amostra faziam uso ocasional de risco, pelo menos, uma vez ao mês. Os maiores índices de binge drinking estavam relacionados ao gênero masculino, aos solteiros e jovens e a religião evangélica foi associada a um menor ou nenhum consumo de bebidas alcoólicas. Conclusão os dados apontaram o consumo alcoólico e o uso em binge frequente relacionados à provável dependência alcoólica.


ABSTRACT Objective to verify the consumption of alcoholic beverages and the practice of binge drinking among hairdressers. Methods cross-sectional study conducted with 51 beauty salon professionals. We used a questionnaire with socio-demographic characteristics and about the practices of consumption of alcoholic beverages. For the identification of the use in binge drinking, the key question was guided. Descriptive and inferential statistical analysis was performed. Results 84.3% were alcohol consumers, 51.0% had between one and ten years of consumption and 72.5% consumed beer. In relation to the use in binging, 37.3% of the sample made occasional use of risk, at least once a month. The highest rates of binge drinking were related to male gender, single and young and the evangelical religion was associated with a lower or no consumption of alcoholic beverages. Conclusion the data pointed to alcohol consumption and frequent binge use related to likely alcohol dependence.

BACKGROUND/AIMS: An excessive inflammatory response is typical in acute pancreatitis and a significant cause of early mortality in severe acute pancreatitis. This is believed to be caused by inflammatory molecules or upregulated cytokine levels in the serum of patients. The aim of this study was to identify the serum-mediated apoptosis-inducing effects in acute pancreatitis patients.METHODS: A skin tissue-derived cell line, BJ, was treated for 24 hours with the sera of 22 healthy volunteers (control) and 71 acute pancreatitis patients (22 with gallstone pancreatitis, 16 with alcoholic pancreatitis, and 11 with pancreatitis with other causes) collected at the time of hospital admission (active) and discharge (resolved). Apoptosis was analyzed by flow cytometry.RESULTS: The average percentage of living cells, early apoptotic cells, and late apoptotic cells ranged from 78.8% to 85.0%, 5.5% to 7.3%, and 7.7% to 13.1%, respectively. The number of live cells increased significantly using the serum from the resolved state of gallstone-induced pancreatitis. In addition, the number of early apoptotic cells increased significantly using the serum from the resolved state of pancreatitis with other causes. The number of late apoptotic cells decreased significantly with the serum from the resolved state compared to the active state of gallstone- and alcohol-induced pancreatitis.CONCLUSIONS: Serum samples from patients with pancreatitis induced a change in the apoptosis profiles of skin-derived cells. These results indicate changes in the serum components in patients with acute pancreatitis.

ABSTRACT

OBJECTIVE: Alcohol-induced blackout (blackout) is a typical early symptom of cognitive impairment caused by drinking. However, the first onset age of blackout or the duration after onset of blackout has not been directly compared in previous studies. The purpose of this study was to investigate the differences in cognitive function to the first start age of blackouts and their duration. METHODS: Thirty-one male subjects were included in this study. Their age at the first blackout and the duration after the onset of blackout were investigated. Neuropsychological tests were conducted to determine their attention, memory, and executive function. Subjects were divided into three groups according to their age of the first onset blackout (group O1, 40 years). Subjects were also divided into three groups by duration after the onset of blackout (P1, 30 years). We then examined differences in neurocognitive function among these groups. RESULTS: O1 tended to have a lower memory score than O2 (F = 3.28, p = 0.053). Significant differences were observed in attention and executive function between groups P1 and P3 (Digit Span_backward: F = 6.07, p < 0.05; visual span_forward: F = 4.19, p < 0.05; executive intelligence quotient: F = 3.55, p < 0.05). CONCLUSION: Greater memory impairment was detected in subjects having an earlier age of the first blackout. The longer the duration after the onset of blackout, the more impaired their attention and executive function skills.

ABSTRACT

Evidências mostram que atividade física (AF) pode se associar a níveis mais elevados de consumo de bebida alcóolica. Assim, esse estudo teve como objetivo verificar se existe ou não associação do nível de AF de lazer com o consumo de bebida alcóolica entre adultos de ambos os sexos. Dados secun-dários foram coletados do Ministério da Saúde (Vigitel) referente ao inquérito transversal realizado no ano de 2014, na cidade de Curitiba, PR, Brasil. A AF foi a variável de exposição e o consumo de bebida alcoólica a variável de desfecho. Participaram do estudo 1.504 adultos, sendo 557 homens com idade média de 51,7 ±18,6 anos e 947 mulheres com idade média de 55,7 ± 16,9 anos. O nível de AF foi categorizado em inativo e ativo e o consumo de bebida alcóolica em regular e abusivo. Os resultados foram controlados por sexo, idade e escolaridade. Os adultos ativos tiveram risco pelo menos 1,5 vezes maior (Razão de Prevalência (RP)= 1,5; IC95%: 1,2­1,9) para o consumo regular de bebida alcoólica e pelo menos 1,3 vezes maior (RP= 1,3; IC95%: 0,8­2,1) para o consumo abu-sivo, quando comparado aos inativos. Os homens ativos tiveram risco pelo menos 1,7 vezes maior (RP= 1,7; IC95%: 1,2­2,5) para o consumo regular e 2,3 vezes maior (RP= 2,3; IC95%: 0,9­5,7) para o consumo abusivo, enquanto mulheres ativas tiveram risco pelo menos 1,4 vezes maior (RP= 1,4; IC95%: 1,1­1,8) para o consumo regular e 0,9 vezes maior (RP= 0,9; IC95%: 0,6­1,7) para o consumo abusivo. Os resultados apoiam a existência de uma associação positiva incongruente entre o consumo de bebida alcoólica e a prática de AF.


However, recent studies indicated that higher levels of physical activity may be associated with higher levels of alcohol consumption. The objective of this study is to verify the association between physical activity level and abusive alcohol consumption of adults of both sexes. Secondary data were collected from the Ministry of Health/Vigitel/2014. The PA was the exposure variable and alcohol consumption as the outcome variable. Data were collected from the individual behavior analysis of 1,504 Brazilian adults, 557 men with a mean age of 51.7 ± 18.6 years and 947 women with a mean age of 55.7 ± 16.9 years from Curitiba city. Were categorized according the level of PA (inactive and active) and alcohol consumption (regular use and abu-sive use). The results were controlled by gender, age and schooling. Actives had at least 1.5 times higher risk (Prevalence Ratio (PR)= 1.5; 95%CI: 1.2­1.9) for regular consumption of alcoholic beverages and at least 1.3 times higher risk (PR= 1.3; 95%CI: 0.8­2.1) for abusive consumption, when compared to inactives. Active men had at least 1.7 times higher risk (PR= 1.7; 95%CI: 1.2­2.5) for regular consumption and 2.3 times higher risk (PR= 2.3; 95%CI: 0 9­5.7) for abusive consumption, while active women had at least 1.4 times higher risk (PR= 1.4; 95%CI: 1.1­1.8) for regular consumption and 0.9 times higher risk (PR= 0.9; 95%CI: 0.6­1.7) for abusive consumption. For the examined sample, the results provide support for the existence of an incongruent positive association between regular alcohol consumption and PA practice.

ABSTRACT

Hangover is characterized by a number of unpleasant physical and mental symptoms that occur after heavy alcohol drinking. In addition, consistently excessive alcohol intake is considered as a major reason causes liver disease. The present study investigated the in vivo effects of DA-5513 (Morning care® Kang Hwang) on biological parameters relevant to hangover relief and alcoholic fatty liver. Blood alcohol and acetaldehyde concentrations were determined in rats administered a single dose of alcohol and treated with DA-5513 or commercially available hangover relief beverages (Yeomyung® and Ukon®). The effects of DA-5513 on alcoholic fatty liver were also determined in rats fed alcohol-containing Lieber-DeCarli diets for 4 weeks. Serum liver function markers (aspartate and alanine aminotransferase activities) and serum/liver lipid levels were assessed. Blood alcohol and acetaldehyde concentrations were lower in the groups treated with DA-5513 or Yeomyung®, as compared with control rats. However, Ukon® did not produce any significant effects on these parameters. Treatment with DA-5513 significantly reduced serum aspartate and alanine aminotransferase activities and markedly reduced serum cholesterol and triglyceride levels, as compared with control rats. Histological observations using Oil Red O staining found that DA-5513 delayed the development of alcoholic fatty liver by reversing hepatic fat accumulation. These findings suggest that DA-5513 could have a beneficial effect on alcohol-induced hangovers and has the potential to ameliorate alcoholic fatty liver.

ABSTRACT

Introdução: a temática de álcool e outras drogas ainda é pouco explorada entre as populações indigenas brasileiras; considerando-se a necessidade de explorar esse problema e a escassez de estudos sobre este fenômeno na região norte do Pais, realizou-se esta pesquisa. Objetivo: identificar os padrões do consumo de substâncias psicoativas na etnia Karipuna do municipio de Oiapoque, estado do Amapá, verificando a associação desses padrões com variáveis sociodemográficas, clínicas e comportamentais da amostra. Método: estudo transversal, realizado em 12 aldeias da etnia Karipuna, localizadas no município de Oiapoque. A amostra constitui-se de 230 individuos predominantemente do sexo masculino (51,3%), faixa etária de 16 a 30 anos (46,5%), católicos (71,1%), com ensino fundamental incompleto (28,3%). Para a coleta de dados foram utilizados o Alcohol Use Disorder Identification Test (AUDIT), a Questão chave (QC) e o Alcohol, Smoking and Substance Involvment Screening Test (ASSIST). Para a análise dos dados realizou se o teste de qui quadrado para verificar a associações entre as variáveis de interesse e o padrão de consumo das substâncias; posteriormente foram selecionadas as variáveis para compor o modelo de regressão logística com prefixação do p-valor de 0,20, obtendo-se daí os valores do Odds Ratio (OR), entre as variáveis selecionadas na regressão logística. Resultados: de acordo com o AUDIT 59,5% dos entrevistados faziam uso de baixo risco, 32,1% uso de risco, 6% uso nocivo e 2,4% apresentavam provável dependência. A prevalência de alteração de pressão arterial entre os entrevistados foi de (19%), diabetes (3,1%), presença de pensamento suicida (13,5%), tentativas de suicidio (9,5%), mais de um terço dos entrevistados (35,6%), referiu manter Relações Sexuais Após Consumo de Álcool (RSACA). Os maiores preditores do uso de risco e nocivo de acordo com o AUDIT foram ser do sexo masculino (OR=2,47), apresentar pensamento suicida (OR=3,02) e manter RSACA (2,21). Os resultados do ASSIST mostraram que 73,4% dos entrevistados faziam uso ocasional de álcool, 26% faziam uso abusivo e 0,6% apresentava possível dependência. Os preditores do uso problemático, segundo o ASSIST, foram: ser do sexo masculino (OR=2,33), apresentar alteração de pressão (OR=4,03) e manter RSACA (OR=2,34). Em relação à Questão-chave, 42,2% dos entrevistados faziam uso de risco mais de 4 vezes ao ano, como principais preditores desse uso: ser estudante (OR=2,99), ter migrado da aldeia de origem (OR=2,22), fazer uso de preservativo (OR=2,62); manter RSACA (OR=1,61). O uso problemático do tabaco foi observado em 16,6% da amostra, os principais preditores desta condição foram: pertencer ao sexo masculino (OR=4,24); ter migrado da aldeia de origem (OR=3,27). Conclusão: as drogas de maior prevalência entre os indigenas da etnia Karipuna são licitas, e seu padrão de uso problemático é maior que o observado entre a população geral. A prática RSACA foi importante preditor do uso problemático e álcool, independentemente do instrumento de rastreio; medidas preventivas devem, pois, ser maximizadas entre esta população. Os dados deste estudo têm potencial para subsidiar a realização de pesquisas futuras nesta área, não só em relação aos povos de etnia Karipuna, mas a outras etnias indigenas da região norte brasileira e do Brasil.


Introduction: Alcohol and other drugs themes are still little explored in Brazilian indigenous population. Considering the need to explore this problem and the scarcity of studies investigating this phenomenon in the northern region of the country, this research was carried out. Objective: To identify patterns of psychoactive substance used in the Karipuna ethnic group of the city of Oiapoque in the state of Amapá verifying the association of these patterns with socio-demographic, clinical and behavioral variables of the sample. Method: This is a cross-sectional study carried out in twelve villages of ethnic Karipuna, located in the municipality of Oiapoque. The sample was 230 individuals predominantly male (51.3%), from 16 to 30 years old (46.5%), Catholic (71.1%), with incomplete elementary school (28.3%). For the data collection, the Alcohol Use Disorder Identification Test (AUDIT), the Key Question (KQ) and the Alcohol, Smoking, and Substance Involvement Screening Test (ASSIST) were used. The chi-square test was used to analyze the associations between the variables of interest and the pattern of consumption of the substances. Then, the variables were selected to compose the logistic regression model with pre fixation of the p-value of 0.20, obtaining the Odds Ratio (OR) values from this model, among the variables selected in the logistic regression. Results: According to the AUDIT, there were 59.5% of the interviewees who used psychoactive substance at low risk, 32.1% at risk, 6% at a harmful use and 2.4% had a possible dependency The prevalence of altered blood pressure in the interviewees was (19%), diabetes (3.1%), suicidal thoughts (13.5%), suicide attempts (9.5%), more than a third of respondents (35.6%), reported maintaining sexual relationships after alcohol consumption (SRAAC). The highest predictors of risk and harmful use of a psychoactive substance according to the AUDIT were male (OR 2.47), suicidal thoughts (OR=3.02) and SRAAC (2.21). The ASSIST results showed that 73.4% of the interviewees had occasional use of alcohol, 26% had an abusive use of alcohol) and 0.6% had a possible dependency. The predictors of problematic use using ASSIST were: male (OR=2.33), having pressure changes (OR=4.03) and maintain SRAAC (OR 2.34). Regarding the Key Question, there were 42.2% of respondents having a risk of psychoactive substance use in more than 4 times a year, with students as the main predictors of this use (OR 2.99), having migrated from the village of origin (OR=2, 22), using a condom (OR=2.62) and maintaining SRAAC (OR=1.61). The problematic use of tobacco was observed in 16.6% of the sample. The main predictors of this condition were male (OR=4.24) and migrated from the village of origin The drugs with the highest prevalence in the Karipuna ethnic are legal and the pattern of their problematic use is greater than the general population. The SRAAC practice proved to be an important predictor of this problematic use and alcohol regardless of the screening instrument indicating that preventive measures should be maximized in this population. The data from this study has the potential to support future research in this area, involving not only ethnic Karipuna people but also other indigenous ethnic groups from the northern Brazilian region and the rest of Brazil.

ABSTRACT OBJECTIVE To describe the prevalence of alcohol-related disorders and associated factors in an exclusively rural population. METHODS This is a cross-sectional, population-based study of a rural research consortium, conducted in a medium-sized city in Southern Brazil, with adults living in a rural area, using the AUDIT (Alcohol Use Disorders Identification Test). The analysis included the prevalence of alcohol-related disorders and associated factors, such as the sociodemographic, family, and health factors; it was carried out by Poisson regression, in a hierarchical analysis model, with a 95% confidence interval. RESULTS The final sample amounted to 1,519 subjects. The prevalence of alcohol-related disorders (AUDIT ≥ 8) was 8.4% (95%CI 7.0-9.8). Risk factors for alcohol-related disorders were being male (PR = 8.2, 95%CI 4.82-14.16), age group between 18 and 29 years (PR = 3.29, 95%CI 1.80-6.0), and smoking (PR = 1.88, 95%CI 1.03-3.43). The practice of religion (PR = 0.38, 95%CI 0.25-0.58) and education level between nine and 11 years (PR = 0.33, 95%CI 0.16-0.69) were protective factors with statistical significance. Marital status and social status were not associated with the outcome studied. CONCLUSIONS The prevalence of alcohol-related disorders in the rural population is high, but, on average, it is lower than that found in urban populations. Risk and protective factors were similar to those found in previous studies. Men, younger persons, and smokers are at higher risk for alcohol-related disorders. On the other hand, practicing a religion and having a higher education level were protective factors.


RESUMO OBJETIVO Descrever a prevalência de transtornos relacionados ao uso de álcool e fatores associados em uma população exclusivamente rural. MÉTODOS Estudo transversal de base populacional, integrante de um consórcio de pesquisa rural, realizado em cidade de médio porte do Sul do Brasil, com adultos residentes na zona rural, utilizando o AUDIT (Alcohol Use Disorders Identification Test). A análise incluiu a prevalência dos transtornos relacionados ao uso de álcool e fatores associados, como sociodemográficos, familiares e de saúde; foi realizada por regressão de Poisson, em modelo hierarquizado de análise, com intervalo de confiança de 95%. RESULTADOS A amostra final foi de 1.519 indivíduos. A prevalência de transtornos relacionados ao uso de álcool (AUDIT ≥ 8) foi de 8,4% (IC95% 7,0-9,8). Os fatores de risco para transtornos relacionados ao uso de álcool foram sexo masculino (RP = 8,26; IC95% 4,82-14,16), faixa etária entre 18 e 29 anos (RP = 3,29; IC95% 1,80-6,0) e tabagismo (RP = 1,88; IC95% 1,03-3,43). Como fatores de proteção com significância estatística encontramos a prática de religião (RP = 0,38; IC95% 0,25-0,58) e escolaridade entre nove e 11 anos de estudo, (RP = 0,33; IC95% 0,16-0,69). Estado civil e classe social não estiveram associados com o desfecho estudado. CONCLUSÕES A prevalência de transtornos relacionados ao uso de álcool na população rural é alta, porém, em média, mais baixa do que as encontradas em populações urbanas. Os fatores de risco e proteção foram similares aos encontrados em estudos prévios. Os homens, as pessoas mais jovens e os tabagistas têm maior risco para apresentar transtornos relacionados ao uso de álcool. Por outro lado, praticar uma religião e ter maior escolaridade foram fatores de proteção.
